Introduction. This study examines weight gain and assesses complications associated with refeeding hospitalised adolescents with restrictive eating disorders (EDs) prescribed initial calories above current recommendations.Methods. Patients admitted to an adolescent ED structured “rapid refeeding” program for >48 hours and receiving ≥2400 kcal/day were included in a 3-year retrospective chart review.Results. The mean (SD) age of the 162 adolescents was 16.7 years (0.9), admission % median BMI was 80.1% (10.2), and discharge % median BMI was 93.1% (7.0). The mean (SD) starting caloric intake was 2611.7 kcal/day (261.5) equating to 58.4 kcal/kg (10.2). Most patients (92.6%) were treated with nasogastric tube feeding. The mean (SD) length of stay was 3.6 weeks (1.9), and average weekly weight gain was 2.1 kg (0.8). No patients developed cardiac signs of RFS or delirium; complications included 4% peripheral oedema, 1% hypophosphatemia (<0.75 mmol/L), 7% hypomagnesaemia (<0.70 mmol/L), and 2% hypokalaemia (<3.2 mmol/L). Caloric prescription on admission was associated with developing oedema (95% CI 1.001 to 1.047;p=0.039). No statistical significance was found between electrolytes and calories provided during refeeding.Conclusion. A rapid refeeding protocol with the inclusion of phosphate supplementation can safely achieve rapid weight restoration without increased complications associated with refeeding syndrome.

Background. Postoperative new-onset atrial fibrillation (POAF) is a common complication following cardiothoracic surgery, but little is known regarding its occurrence and outcome following noncardiothoracic surgery. This study was intended to examine the incidence of POAF in noncardiothoracic surgeries performed under general anesthesia and its effects on the length of hospitalization stay, short-term and long-term morbidity, and mortality. Methodology. We conducted a retrospective observational descriptive study. The study population consists of patients hospitalized in surgical wards from January 2014 to December 2017. Surgery was defined as noncardiac or thoracic procedure conducted under general anesthesia. Results. A total of 24,125 general anesthesia operations were performed at 7 surgical wards. About two-fifth of the operations (40%) were operated electively, and the rest underwent emergency surgery. The mean age was 63.78 ± 11.50, and more than half (56.9%) of the participants were female. The prevalence of POAF was 2.69 per 1000 adult patients (95% CI: 2.11–3.43) and vary significantly among wards. The highest prevalence was observed after hip fixation and laparotomy surgeries (54.9 and 26.7 per 1000 patients, respectively). The median length of hospitalization was significantly higher in POAF patients (21.0 vs. 4.8 days, p < 0.001 ). Patients who developed POAF had significantly higher mortality rates, both inhospital (200 vs. 7.56 deaths per 1000, p = 0.001 ) and 1 year (261.5 vs. 33.3 per 1000, p = 0.001 , respectively). There was no significant association between outcome and treatment modalities such as rate or rhythm control and anticoagulant use. Conclusion. New-onset AF following noncardiac surgery is rare, yet poses significant clinical implications, both immediate and long-term. POAF is associated with a longer length of hospitalization and a significantly higher mortality rate, both in short- and long-term.

In this research, it was aimed to determine fattening performance and carcass characteristics of Simmental (SIM), Aberdeen Angus (ANG), Hereford (HER), Limousin (LIM) and Charolais (CHA) breeds. For this purpose, 606 male cattle between 10 and 12 months old imported from Uruguay and France in 2015 were used. All animals were fed ad-libitum for the same ration throughout the fattening period for approximately 7-9 months. Since the animals were slaughtered between April-June 2016, their fattening period were different. As a result of variance analysis, the effects of breed, initial weight (IW) and fattening period (FP) were found to be significant. For SIM (n=100), ANG (n=147), HER (n=149), LIM (n=104) and CHA (n=106); IW were 261.6, 267.3, 276.7, 264.1, 276.7 kg; FP were 206.7, 238.1, 261.4, 227.0, 283.6 days; final weight were 523.4, 543.3, 563.1, 545.5, 589.7 kg; daily weight gain were 1362.9, 1275.9, 1214.2, 1266.9, 1101.1 gr; hot carcass weight were 303.4, 317.7, 332.1, 319.3, 351.2 kg, respectively. CHA performed better than the others for the carcass weight, while SIM for daily weight gain is higher.

Abstract Defining the incidence and the prognostic relevance of cytogenetic evolution (CE) in MDS, excluding any effect of the primary defect and other well-known prognostic variables, are the main goals of the present study which included 153 patients (pts) examined between January 1990 and December 2005. They were 73 females and 80 males with a median age of 60.5 years (inter-quartile, IQ, range:49.8-68.3). FAB, IPSS score, IPSS cytogenetic categories were applied to all pts, WHO to 142. On clinical diagnosis 94 pts presented an abnormal karyotype. Median follow-up was 45.2 months (IQ range:22.8-75.6). At the time of analyses 107pts (69.9%) are alive and 46 (30.0%) have died. A clinical progression (CP) occurred in 65 (42.4%) pts and a CE in 47 (30.7%). Median progression-free survival was 65.2 months (IQ range:16.6-not reached). When a Cox model analysed CE as a time-dependent variable predicting disease outcome, a true CE occurred in a total of 40 pts who presented a survival significantly shorter than that of pts without such an evolution (Hazard ratio, HR=7.1, p&lt;0.000 [95% confid. intervals, CI=3.9-12.8]), death rates 47.5 (95% CI=30.3-74.4) versus 5.4 (95%CI=3.7-7.9). A bivariable analysis was applied in order to test whether the relevance of CE remained significant also when it was adjusted for other variables with a well-known impact on disease outcome. This investigation revealed that CE predicted survival independently of the presence of the primary defect (HR=6.6, p&lt;0.000 [95%CI=3.6-12.0]), IPSS cytogenetic categories (HR=6.6, p&lt;0.000 [95%CI=3.6-12.2]), FAB subtypes (HR=4.2, p&lt;0.000 [95%CI=2.3-7.8]), WHO subtypes (HR=4.4, p&lt;0.000 [95%CI=2.3-8.5]) and IPSS categories (HR=3.8, p&lt;0.000 [95%CI=2.0-7.2]). No single secondary defect had a significant impact on survival. When considering CP, the 40 pts with CE presented a risk of disease progression higher than that of pts without CE (HR=35.8, p&lt;0.000 [95%CI=20.6-62.1]), progression rates: 183.9 (95% CI=129.3-261.5) versus 7.3 (95%CI=5.2-10.3). The relevance of CE remained significant when it was adjusted for the presence of primary cytogenetic aberrations (HR=33.9, p&lt;0.000, [95% CI=19.1-60.0]). CE impact was much more significant in pts without than in those with primary defects: HR=61.8 (p&lt;0.000, [95%CI=18.0-212.5]) versus 21.2 (p&lt;0.000, [95%CI=11.1-40.3]). Bivariable analyses revealed that the impact of CE remained significant in all IPSS cytogenetic categories (HR=32.8, p&lt;0.0000 [95%CI=18.6-57.8]), FAB subtypes (HR=21.6, p&lt;0.000 [95%CI=12.2-38.1]), WHO subtypes (HR=27.7, p&lt;0.0000 [95%CI=14.6-52.8]) and IPSS categories (HR=27.3,p&lt;0.0000 [95%CI=14.6-50.9]). Monosomy 7, del(7q) and del(17p) were the secondary defects significantly effecting the risk of disease progression (p&lt;0.0000). In conclusion, in our series CE presented: i) an incidence of 30%; ii) a significant impact on survival independently of the primary defect, FAB/WHO classifications, IPSS score and IPSS cytogenetic categories; iii) a significant impact on the risk of CP independently of the primary defect, FAB/WHO classification, IPSS score and IPSS cytogenetic categories. In addition, monosomy 7, del(7q) and del(17p) were the secondary defects significantly effecting the risk of CP.

Background In transplant-eligible newly diagnosed multiple myeloma (NDMM), triplet combinations including proteasome-inhibitors and immunomodulators are the backbone of induction therapy before autologous stem cell transplant (ASCT). Post-ASCT maintenance with lenalidomide is the standard of care. This approach yields deep responses and long overall survival. In Latin America (LATAM) there is scarce data about the outcome of bortezomib-based triplets and ASCT. The aim of this study was to evaluate overall survival (OS) in Latin American transplant-eligible NDMM patients induced with bortezomib-based triplets and ASCT. Methods Retrospective international multicenter cohort study. Patients older than 18 years with MM, who received bortezomib-based triplets followed by ASCT as first line, between 2010 and 2022 were analyzed. Data were collected from clinical records in a standardized report form. We analyzed clinical characteristics at diagnosis and frontline therapy outcomes. Descriptive statistics were performed. Comparisons of characteristics between groups were made using the Chi-square test, Student´s T-test and ANOVA, as appropriate. Survival analysis was performed using Kaplan-Meier curves, comparisons between groups by Log Rank method, and calculations of the risk relationships by Cox regression. Statistical analysis was performed by using IBM SPSS version 25.0. Results A total of 279 patients with NDMM were included, 124 from Argentina, 20 from Chile, 30 from Mexico, 20 from Paraguay, 43 from Peru, 33 from Uruguay, and 9 from Venezuela. Median age was 57 years (range 29-75) with a male predominance (54.8%). Most patients (58%) were treated in private centers. 56.1% were IgG subtype, 24.8% IgA and 17.3% light chain MM. According to the ISS, 69.2% were classified as ISS II or III. Bone disease was the most frequent myeloma-defining events (75.9%), followed by anemia (61.4%), renal failure (24.2%), and hypercalcemia (20.1%). Fluorescence in situ hybridation (FISH) analysis was performed in 53.4% of patients (only 41% with plasma cell sorting), del17p was the most frequent anomaly found (17.4%), followed by t(4;14) (6%), and t(14;16) (2%). The most frequently used induction regimen was cyclophosphamide-bortezomib-dexamethasone (CyBorD) (38.7%), followed by bortezomib-thalidomide-dexamethasone (VTD) (33.7%), and bortezomib-lenalidomide-dexamethasone (VRD) (27.6%). Very good partial remission (VGPR) or better was achieved in 88.3% for VRD, 81.9% for VTD, and 76.8% for CyBorD (p=0.138). Median time from diagnosis to ASCT was 261.5 days. Only 7 patients received tandem ASCT as first-line consolidation. Maintenance treatment was administered to 88.2% of patients and was based on lenalidomide, thalidomide, bortezomib, and lenalidomide+bortezomib in 67.9%, 16.3%, 8.9% and 3.3%, respectively. Treatments and responses are shown in detail in Table 1. With a median follow-up of 45 months (range 7-140), median progression-free survival (PFS) was 33 months (95% CI, 26.7 - 39.3). Median PFS according to treatment was 21 months (95% CI, 14.3 - 27.7) for VRD, 35 months (95% CI, 22.2 - 47.8) for VTD, and 36 months (95% CI, 28 - 44) for CyBorD, p=0.004. Median OS of the whole cohort was not reached (NR), and 86% at 45 months; 75 months for VRD (95% CI, 44 - 106), and NR for CyBorD and VTD (p=0.284) (Figure 1). In the multivariate analysis hypercalcemia (p=0.01) and extramedullary disease (p=0.03) were the only independent risk factors. Discussion FISH was performed only in half of our patients, and the majority without plasma cell sorting. The main used regimen was CyBorD. Although better PFS was obtained with CyBorD, no significant differences in responses or OS were found between VRD, VTD or CyBorD. The reason why VRD PFS and OS were lower merits further study. We report a high rate of maintenance treatment.

This research was conducted to evaluate the anticancer activity of new compounds of benzyl-1,10- phenanthroline derivatives and their selectivity. In vitro anticancer activity of 11 benzyl-1,10-phenanthroline derivatives were conducted on three human cancer cell lines, cervical cancer (HeLa), myeloma (NS-1), and breast cancer (MCF-7) using MTT-based cytotoxicity assay. The cytotoxicity of each compound was assessed to normal Vero cell line by the same method. The in vitro anticancer activity and cytotoxicity was expressed by the concentration inhibiting 50% of the cell growth (IC50), and the selectivity index (SI) was determined by calculating ratio of the IC50 on Vero cell line and the human cancer cell lines. The results showed that among the 11 compounds tested, the (1)-N-(4-butoxybenzyl)-1,10-phenanthrolinium bromide exhibited the best in vitro anticancer activity with an IC50 27.60 ± 2.76 µM on HeLa, 6.42 ± 5.53 µM on NS-1 and 9.44 ± 2.17 µM on MCF-7 cell lines. Its SI were 377.65 ± 39.97 on HeLa, 6158.72 ± 5306.34 on NS-1 and 1140.11 ± 261.85 on MCF-7 cell lines. This study demonstrated that (1)-N-(4-butoxybenzyl)-1,10-phenanthrolinium bromide possessed a potential in vitro anticancer activity on cancer cell lines with high selectivity

Background Aerobic exercise can improve symptoms, reduce airway inflammation, and even ameliorate airway remodeling in asthmatic animals and patients. However, previous studies have focused mainly on the effect of aerobic exercise on steroid-sensitive asthma (SSA). The goals of this study were to determine the effect of low-intensity aerobic exercise training on airway hyperresponsiveness, inflammation, and remodeling in a rat model of steroid-resistant asthma (SRA) and to identify the potential mechanisms underlying these effects. Methods Endotoxin-free ovalbumin with or without lipopolysaccharide were applied to establish rat models of SRA and SSA, respectively. Airway hyperresponsiveness, inflammation, remodeling, expression of interleukin (IL)-25, IL-33, thymic stromal lymphopoietin (TSLP), high mobility group box-1 (HMGB1), and IL-17 in bronchoalveolar lavage fluid (BALF), and the role of dexamethasone (DXM) were compared between these two asthmatic rat models. The effect of low-intensity aerobic exercise training and anti-HMGB1 treatment on airway hyperresponsiveness, inflammation, and remodeling in SRA rats also was evaluated. Results SRA rats developed neutrophil-dominated airway inflammation ((29.5±4.1)% of the total cell numbers in BALF), whereas SSA rats developed eosinophil-dominated airway inflammation ((24.0±6.1)% of the total cell numbers in BALF). Compared with SSA rats, SRA rats had more severe airway hyperresponsiveness, lower levels of IL-25 ((33.6±10.3) vs. (104.8±24.9) pg/ml), IL-33 ((87.5±25.0) vs. (226.6±40.7) pg/ml), and TSLP ((1 933.2±899.5) vs. (7 224.0±992.1) pg/ml), and higher levels of HMGB1 ((21.2±4.5) vs. (5.4±1.6) ng/ml) and IL-17 ((780.5±261.7) vs. (291.4±76.4) pg/ml) in BALF (all P <0.05). However, there was no significant difference in goblet cell hyperplasia, subepithelial collagen thickness, and airway smooth muscle remodeling between the two groups. Compared with control SSA rats, airway hyperresponsiveness, inflammation, and remodeling in SRA rats were less sensitive to DXM treatment. Anti-HMGB1 treatment attenuated airway hyperresponsiveness, inflammation, and remodeling in SRA rats to a certain extent and was accompanied by lower levels of IL-17 ((369.2±126.7) vs. (780.5±261.7) pg/ml in control SRA rats) in BALF (P <0.05). Low-intensity aerobic exercise training decreased the expression of both HMGB1 ((14.1±2.9) vs. (21.2±4.5) ng/ml in control SRA rats) and IL-17 ((545.3±148.6) vs. (780.5±261.7) pg/ml in control SRA rats) in BALF (all P <0.05) and was accompanied by improved airway hyperresponsiveness, inflammation, and remodeling in SRA rats (all P <0.05). Conclusions Low-intensity aerobic exercise training attenuated airway hyperresponsiveness, inflammation, and remodeling in a rat model of SRA. Decreased HMGB1 and IL-17 levels in BALF by aerobic exercise training at least partly contributed to the improvements of SRA.

This study analyzed the performance evaluation of alkali-activated composites (AAC) with an alkali-sulfate activator and determined the expected effects of applying AACs to actual sites. Results revealed that when the binder weight was increased by 100 kg/m3 at 7 days of age, the homogel strength of ordinary Portland cement (OPC) and AAC increased by 0.9 and 5.0 MPa, respectively. According to the analysis of the matrix microstructures at 7 days of age, calcium silicate hydrates (C–S–H, Ca1.5SiO3.5·H2O) and ettringite (Ca6Al2(SO4)3(OH)12·26H2O) were formed in AAC, which are similar hydration products as found in OPC. Furthermore, the acid resistance analysis showed that the mass change of AAC in HCl and H2SO4 solutions ranged from 36.1% to 88.0%, lower than that of OPC, indicating AAC’s superior acid resistance. Moreover, the OPC and AAC binder weight ranges satisfying the target geltime (20–50 s) were estimated as 180.1–471.1 kg/m3 and 261.2–469.9 kg/m3, respectively, and the global warming potential (GWP) according to binder weight range was 102.3–257.3 kg CO2 eq/m3 and 72.9–126.0 kg CO2 eq/m3. Therefore, by applying AAC to actual sites, GWP is expected to be 29.5 (28.8%)–131.3 (51.0%) kg CO2 eq/m3 less than that of OPC.

Background A low concentration of lidocaine (10 microM) has been shown to reduce anoxic damage in vitro. The current study examined the effect of low-dose lidocaine on infarct size in rats when administered before transient focal cerebral isehemia. Methods Male Wistar rats (weight, 280-340 g) were anesthetized with isoflurane, intubated, and mechanically ventilated. After surgical preparation, animals were assigned to lidocaine 2-day (n = 10), vehicle 2-day (n 12), lidocaine 7-day (n = 13), and vehicle 7-day (n = 14) groups. A 1.5-mg/kg bolus dose of ildocaine was injected intravenously 30 mm before isehemia in the lidocaine 2-day and 7-day groups. Thereafter, an infusion was initiated at a rate of 2 mg x kg(-1) x h(-1) until 60 min of reperfusion after isehemia. Rats were subjected to 90 min of focal cerebral isehemia using the intraluminal suture method. Infarct size was determined by image analysis of 2,3,5-triphenyltetrazolium chloride-stained sections at 48 h or hematoxylin and eosin-stained sections 7 days after reperfusion. Neurologic outcome and body weight loss were also evaluated. Results The infarct size was significantly smaller in the lidocaine 2-day group (185.0+/-43.7 mm3) than in the vehicle 2-day group (261.3+/-45.8 mm3, P &lt; 0.01). The reduction in the size of the infarct in the lidocaine 7-day group (130.4+/-62.9 mm3) was also significant compared with the vehicle 7-day group (216.6+/-73.6 mm3, P &lt; 0.01). After 7 days of reperfusion, the rats in the lidocaine group demonstrated better neurologic outcomes and less weight loss. Conclusions The current study demonstrated that a clinical anriarrhythmic dose of lidocaine, when given before and during transient focal cerebral isehemia, significantly reduced infaret size, improved neurologic outcome, and inhibited postisehemic weight loss.

Korean red ginseng (KRG) is known to exert beneficial effects on cardiovascular health. Meanwhile, reduced estrogen at menopause has been shown to have various adverse impacts on cardiovascular risk factors, including blood lipids. The aim of this pilot study was to investigate the effect of KRG on cholesterol metabolites, which are surrogate markers of cholesterol absorption and biosynthesis, in postmenopausal women with hypercholesterolemia. The present study is an exploratory study which used data from a 4-week, double-blinded, placebo-controlled clinical pilot study in 68 postmenopausal women with hypercholesterolemia. Patients received KRG (2 g) or placebo (2 g) once daily. The primary endpoints were changes in the levels of nine sterols. Serum sterols were analyzed using liquid chromatography-mass spectrometry (LC-MS)/MS analysis. Among the sterols, reduction in cholesterol level were significantly larger in the KRG group than in the placebo group (the changes: −148.3 ± 261.1 nmol/mL in the ginseng group vs. −23.0 ± 220.5 nmol/mL in the placebo group, p = 0.039). Additionally, changes in 7-hydroxycholesterol (7-OHC) were significantly larger in the KRG group than in the placebo group (the changes: −0.05 ± 0.09 nmol/mL in the ginseng group vs. −0.002 ± 0.1 nmol/mL in the placebo group, p = 0.047). Oxysterols, cholesterol derivates, have been known to play a role in chronic inflammation diseases such as cardiovascular diseases. KRG improves sterol metabolism by decreasing cholesterol and 7-OHC levels in postmenopausal women with hypercholesterolemia.

A reduced mitochondrial DNA (mtDNA) copy number, the ratio of mitochondrial DNA to genomic DNA (mtDNA:gDNA), has been linked with dysfunctional mitochondria. Exercise can acutely induce mtDNA damage manifested as a reduced copy number. However, the influence of a paired (exercise and temperature) intervention on regional mtDNA (MINor Arc and MAJor Arc) are unknown. Thus, the purpose of this study was to determine the acute effects of exercise in cold (7 °C), room temperature (20 °C), and hot (33 °C) ambient temperatures, on regional mitochondrial copy number (MINcn and MAJcn). Thirty-four participants (24.4 ± 5.1 yrs, 87.1 ± 22.1 kg, 22.3 ± 8.5 %BF, and 3.20 ± 0.59 L·min−1 VO2peak) cycled for 1 h (261.1 ± 22.1 W) in either 7 °C, 20 °C, or 33 °C ambient conditions. Muscle biopsy samples were collected from the vastus lateralis to determine mtDNA regional copy numbers via RT-qPCR. mtDNA is sensitive to the stressors of exercise post-exercise (MIN fold change, −1.50 ± 0.11; MAJ fold change, −1.70 ± 0.12) and 4-h post-exercise (MIN fold change, −0.82 ± 0.13; MAJ fold change, −1.54 ± 0.11). The MAJ Arc seems to be more sensitive to heat, showing a temperature-trend (p = 0.056) for a reduced regional copy number ratio after exercise in the heat (fold change −2.81 ± 0.11; p = 0.019). These results expand upon our current knowledge of the influence of temperature and exercise on the acute remodeling of regional mtDNA.

Nitrogen doped as-cast Co-Cr-Mo alloy with markedly enhanced hardness and wear resistance was successfully produced by pack nitriding process. Nitriding proses used the in-pack process using urea fertilizer as a nitrogen source. It was carried out with variations in temperature of 400°C, 500°C, 600°C and followed by annealing. The wear resistance of the specimen is tested using pin-on-disk. The total sample will be soaked in the lactic acid solution for 3 days. The results obtained from the After nitriding process, the hardness of of speciment N400 obtained a hardness value of 261.54 HV speciment N500 of 309.68 HV, and speciment N600 of 429.14 HV, the results of the hardness value increased in each speciment. The wear resistance value obtained for each speciment N400, N500, N600 is 2,81 x 10-6 mm3/Nm, 7,50 x 10-7 mm3/Nm, 1,87 x 10-7 mm3/Nm. The nitriding process forms a layer on the surface of the as-cast Co-Cr-Mo, it’s called Layer of Cr-N which is the result of heating/decomposing urea fertilizer into nitrogen gas which can coat the surface so that Cr-N bonds are formed in the layer. The thickness of the Cr-N layer on the N400 specimen is 0.28 mm, N500 is 0.35 mm, N600 is 0.38 mm. In addition, the surface results that have been tested for wear resistance form quite fine scratches but the formation of abrasions, such as abrasion, abrasion groove, delamination, and cohesive failure

New data were obtained on the implementation of the Alaria alata life cycle in the conditions of the Central Black-Soil Region. Based on the materials of our research, paratenic hosts of A. alata were identified: Natrix natrix, Vipera berus, Neovison vison, Myodes glareolus. Alaria mesocercariae were observed in all the studied Natrix natrix (27 specimens) and Vipera berus (7 specimens) with the extensiveness of infestation (EI) of 100%. Intensity of infestation (II) averaged 12.0 specimens in N. natrix (2–18 specimens) and 261.3 specimens (12–627 specimens) in V. berus. Alaria mesocercariae were found in the Neovison vison with localization mainly in the esophagus, subcutaneous tissue, striated muscles, and serous integuments. The maximum number of mesocercariae was found in the esophagus – 50 specimens. The M. glareolus was registered as a paratenic host of A. alata with localization of mesocercariae in the M. glareolus in the esophagus. Taking into account the earlier helminthofaunistic materials on the study of the life cycle of A. alata conducted in the Voronezh Region and adjacent areas, other species of mammals were also registered as paratenic hosts. Mesocercariae have been recorded in Sus scrofa, mouse-like rodents: M. glareolus and Microtus arvalis, mice: Apodemus uralensis, Apodemus flavicollis and A. agrarius, Dryomys nitedula and Castor fiber.

Objective. To evaluate, in three Spanish tertiary referral centres, the short-term safety and efficacy of golimumab (GLM) for treatment of immune-mediated uveitis resistant to previous immunosuppressive therapy.Methods. Nonrandomized retrospective interventional case series. Thirteen patients with different types of uveitis that were resistant to treatment with at least 2 previous immunosuppressors were included in this study. All included patients were treated with GLM (50 mg every four weeks) during at least 6 months. Clinical evaluation and treatment-related side effects were assessed at least four times in all included patients.Results. Eight men and 5 women (22 affected eyes) with a median age of 30 years (range 20–38) and active immune-mediated uveitides were studied. GLM was used in combination with conventional immunosuppressors in 7 patients (53.8%). GLM therapy achieved complete control of inflammation in 12/13 patients (92.3%) after six months of treatment. There was a statistically significant improvement in mean BCVA (0.60 versus 0.68,P=0.009) and mean 1 mm central retinal thickness (317 versus 261.2 μ,P=0.05) at the six-month endpoint when compared to basal values. No major systemic adverse effects associated with GLM therapy were observed.Conclusions. GLM is a new and promising therapeutic option for patients with severe and refractory uveitis.

This study mapped the bioenergy production from sugarcane vinasse according to the mesoregions of the State of São Paulo (SP), Brazil, assessing the magnitude of biogas-derived electricity and biomethane production and estimating the greenhouse gas (GHG) emissions. SP holds 45% of the Brazilian ethanol-producing plants, in which 1.4 million m3 of carbon-rich vinasse are generated daily. The electricity generated from vinasse has the potential to fully supply the residential consumption (ca. 6.5 million inhabitants) in the main sugarcane-producing mesoregions of the state (Ribeirão Preto, São José do Rio Preto, Bauru, Araçatuba and Presidente Prudente). In another approach, biomethane could displace almost 3.5 billion liters of diesel, which represents a 26% abatement in the annual state diesel consumption. Energetically exploiting biogas is mandatory to prevent GHG-related drawbacks, as the eventual emission of methane produced under controlled conditions (261.2 × 106 kg-CO2eq d−1) is ca. 7-fold higher than the total emissions estimated for the entire ethanol production chain. Meanwhile, replacing diesel with biomethane can avoid the emission of 45.4 × 106 kg-CO2eq d−1. Implementing an efficient model of energy recovery from vinasse in SP has great potential to serve as a basis for expanding the utilization of this wastewater in Brazil.

Background and Purpose: Although physical activity (PA) is important for all ages including older adults, participation may be influenced by weather variation and access to programming. Our primary aim was determine if PA participation is influenced by season and place of residence. A secondary aim was to compare objective and subjective measure of PA participation. Methods: Participants included older individuals (age>65) living in a residential retirement community (RR) with access to an on-site fitness facility (n=7) and additional volunteers (n=9) who lived at home and traveled to exercise at a nonresidential community (NR) activities center Accelerometers were used to measure daily PA during the summer and again during the winter. Results: PA for the NR group was higher in the summer (268.4�73.7 min vs. RR=186.8�68.0, p=0.039), but not in the winter (NR=261.8�92.6 min, RR=182.0�72.5, p=0.082). No within-group change in PA was noted from summer to winter for either group (p>0.05). The correlation between subjective and objective measures of PA was low (r=.262). Conclusion: Access to appropriate facilities and programming serves to help older individuals maintain PA levels despite seasonal weather variations. The low correlation between objective and subjective measures suggest a need to re-evaluate methods of tracking PA participation by older adults.

SUMMARYDesert birds often experience a scarcity of drinking water and food and must survive episodes of high ambient temperature (Ta). The physiological mechanisms that promote survival during extended periods of high Ta have received little attention. We investigated the physiological responses of wild-caught and captive-reared Houbara bustards, Chlamydotis macqueenii, to Ta values ranging from below 0°C to 55°C, well above those in most previous studies of birds. Captive-reared Houbara bustards (mass 1245±242 g, N=7, mean ± s.d.) in summer have a resting metabolic rate (RMR) of 261.4 kJ day–1, 26 % below allometric predictions, and a total evaporative water loss (TEWL) at 25°C of 25.8 g day–1, 31 % below predictions. When Ta exceeded body temperature (Tb), the dry heat transfer coefficient decreased, a finding supporting the prediction that birds should minimize dry heat gain from the environment at high Ta values. Houbara bustards withstand high Ta values without becoming hyperthermic; at 45°C, Tb was on average 0.9°C higher than at 25°C. RMR and TEWL of captive-bred Houbara bustards were 23 % and 46 % higher in winter than in summer, respectively. Captive-reared Houbara bustards had a 17 % lower RMR and a 28 % lower TEWL than wild-born birds with similar genetic backgrounds. Differences in body composition between wild-caught and captive-reared birds were correlated with differences in physiological performance.

We aimed to evaluate the effects of the COVID-19 lock down on lifestyle in China during the initial stage of the pandemic. A questionnaire was distributed to Chinese adults living in 31 provinces of China via the internet using a snowball sampling strategy. Information on 7-day physical activity recall, screen time, and emotional state were collected between January 24 and February 2, 2020. ANOVA, χ² test, and Spearman’s correlation coefficients were used for statistical analysis. 12,107 participants aged 18–80 years were included. During the initial phase of the COVID-19 outbreak, nearly 60% of Chinese adults had inadequate physical activity (95% CI 56.6%–58.3%), which was more than twice the global prevalence (27.5%, 25.0%–32.2%). Their mean screen time was more than 4 hours per day while staying at home (261.3 ± 189.8 min per day), and the longest screen time was found in young adults (305.6 ± 217.5 min per day). We found a positive and significant correlation between provincial proportions of confirmed COVID-19 cases and negative affect scores (r = 0.501, p = 0.004). Individuals with vigorous physical activity appeared to have a better emotional state and less screen time than those with light physical activity. During this nationwide lockdown, more than half of Chinese adults temporarily adopted a sedentary lifestyle with insufficient physical activity, more screen time, and poor emotional state, which may carry considerable health risks. Promotion of home-based self-exercise can potentially help improve health and wellness.

The substitution of slag-based geopolymer materials for cement-stabilized macadam in road bases is in line with the demand for green and sustainable development in the transportation industry. Thus, slag/fly-ash-based geopolymer materials were prepared to stabilize macadam of road bases in this study. The mechanical properties, freeze-thaw resistance, and dry shrinkage durability of slag/fly-ash-based stabilized macadam materials were studied to analyze the influence of geopolymer dosage on these properties of stabilized macadam. Microscopic tests such as XRD, FITR, and SEM were carried out to explore the formation mechanism of strength and the characteristic of interface transition zone (ITZ). Results show that the 28 d compressive strength, compressive elastic modulus, and tensile strength of slag/fly-ash-based stabilized macadam increase linearly with the increase of geopolymer content. When the dosage of slag-based geopolymer is 4%, the 7 d unconfined compressive strength of slag/fly-ash-based stabilized macadam reaches 8.76 MPa, and the strength still reaches 14.84 MPa after five freeze-thaw cycles (28 d), which satisfy the application requirements of expressway and first-class highway base (JTG/T F20-2015). The dry shrinkage property of slag/fly-ash-based stabilized macadam is better than that of cement-fly-ash-stabilized macadam. When the amount of geopolymer is 3%, the dry shrinkage strain of slag/fly-ash-based stabilized macadam is 231.2 με, which is smaller than that of cement-fly-ash-stabilized macadam (261.3 με). No obvious porosity around the ITZ was detected, indicating good binding between the geopolymer and the aggregate.

A geophysical study comprising vertical electrical soundings was carried out in four (4) locations (Udi 1, Udi 2, Udi 3 and Udi 4) in Udi L.G.A. to assess groundwater potentials. The method involves the use of resistivity equipment (ABEM Terrameter), the field data were obtained utilizing Schlumberger configuration. The WINRESIST software was utilized in the data processing. According to the study’s findings, the geological formations present in these locations contain five to seven layers. Layers 1 has resistivity values ranging from 158.1 – 418.1 m at depths of 0.6 – 1.6 m, layer 2 has resistivity values ranging from 1103.9 – 41387.3  m at depths of 0.8 – 8.9 m, layer 3 has resistivity values ranging from 313.0 – 4782.9 m at depths of 6.9 – 28.2 m, layer 4 has resistivity values ranging from 1847.9 – 6956.8 m at depths of 11.8 – 154.6 m, layer 5 has resistivity values ranging from 115.0 – 2838.4 m at depths of 101.2 – 219.2 m, layer 6 has resistivity values ranging from 51.1 – 261.7 m at a depth 190.6 m, and layer 7 have a resistivity value of 47.1 m at an unknown depth. Layer 4 from all locations shows a prolific aquifer where enormous groundwater can be explored due to its high resistivity values, depth and thickness. The aquifer is estimated to be at the depths of 90.0 – 120.0 m. The results of this geophysical examination revealed that the studied region has good groundwater potential, which if properly utilized, will go a long way toward alleviating the effects of water insufficiency in Udi L.G.A.

Abnormal vascular smooth muscle cell (VSMC) proliferation has an important role in the pathogenesis of both atherosclerosis restenosis and hypertension. Vascular endothelial growth factor (VEGF) has been shown to stimulate VSMC proliferation. In addition, angiogenesis is one of the hallmarks of cancerous growth. VEGF is the key modulator for the initial stages of angiogenesis that acts through the endothelial-specific receptor tyrosine kinases (VEGFRs). VEGFR-2 blockage is a good approach for suppression of angiogenesis. In order to discover novel VEGFR-2 TK inhibitors, we have designed and synthesized three new series of pyridine-containing compounds. The new compounds were all screened against a panel of three cell lines (HepG-2, HCT-116, and MCF-7). Promising results encouraged us to additionally evaluate the most active members for their in vitro VEGFR-2 inhibitory effect. Compound 7a, which is the most potent candidate, revealed a significant increase in caspase-3 level by 7.80-fold when compared to the control. In addition, Bax and Bcl-2 concentration levels showed an increase in the proapoptotic protein Bax (261.4 Pg/ml) and a decrease of the antiapoptotic protein Bcl-2 (1.25 Pg/ml) compared to the untreated cells. Furthermore, compound 7a arrested the cell cycle in the G2/M phase with induction of apoptosis. The immunomodulatory effect of compound 7a, the most active member, showed a reduction in TNF-α by 87%. Also, compound 7a caused a potent inhibitory effect on smooth muscle proliferation. Docking studies were also performed to get better insights into the possible binding mode of the target compounds with VEGFR-2 active sites.

Phenacoccus solenopsis appeared as a major pest in recent years and caused huge losses to cotton. Control of cotton mealybug is difficult because of the waxy covering on the body which prevents penetration of insecticides in nymph and adult stages. Present study was carried out under laboratory conditions to evaluate the synergistic effects of minerals oil (Diver®) on the toxicity of diafenthiuron, nitenpyram and thiacloprid against different instars of cotton mealybug. Diafenthiuron, nitenpyram and thiacloprid alone demonstrated mortality in the range of 10.3-46.0%, 13.3-50% and 10.30-46.70% in 1st instar, 8.00-42%, 10.30-44% and 6.60-42.3% in 2nd instar while 7-38%, 7.30-38% and 3.3-32% in adult female of P. solenopsis, respectively. Admixing of 500 ml of Diver® with diafenthiuron administered 65.2-332.7% and 114.6-602.8%; 78.0-428.8% and 128.6-750.0%; and 94.6-457.1% and 147.3-852.9% increase in mortality of 1st instar, 2nd instar and adult female stages of P. solenopsis, respectively. Similarly, fraternization of 1000 ml of Diver® with diafenthiuron administered 42.0-202.6% and 100.0-490.2%; 60.6-261.4% and 127.3-600.2% and 79.8-336.6% and 154.4-854.9% increase in mortality of 1st instar, 2nd instar and adult female stages of P. solenopsis respectively. However, thiacloprid, when admixed with 500 ml and 1000 ml of Diver®, explained 16.7-114.3% and 114.3-633.3%; 24.9-103.6% and 132.3-900.9%; and 0.0-303.9% and 191.7-1990.9% increase in mortality of 1st instar, 2nd instar and adult female stages of P. solenopsis, respectively. In conclusion, Diver® can be exploited as synergist for resistance management of insecticides against P. solenopsis.

Methods: This retrospective study included 108 participants (214 eyes) who underwent ICL implantation. The efficacy of the manufacturer's nomogram, Parkhurst nomogram, NK formula, KS formula, Zhu formula, and ZZ formula was assessed by comparing the indicated ICL sizes to the postoperative vault measurements. Additionally, Bland-Altman plots and the Friedman test were used to assess the agreement and absolute error between the actual vault and predicted vault. Results: The proportions of the ideal ICL category recommended by the manufacturer's nomogram, Parkhurst nomogram, NK formula, KS formula, Zhu formula, and ZZ formula were 50.5%, 45.3%, 46.7%, 42.5%, 50.0%, and 28.5%, respectively. The mean differences between the actual vault and the predicted vault using the NK, KS, Zhu, and ZZ formulas were 144.1 ± 261.1, −19.3 ± 179.6, 70.8 ± 284.2, and 182.6 ± 361.5 μm, respectively. The predicted ICL vault tended to overestimate the actual ICL vault, particularly when choosing a larger ICL size. The KS formula shows the smallest deviation in prediction error and is least affected by variation in ICL size. Conclusions: White-to-white distance from the Pentacam (Oculus Optikgeräte GmbH) coupled with the manufacturer's nomogram performed well for ICL size selection. Four vault prediction formulas tended to overestimate the actual ICL vault, particularly when selecting a larger ICL size. The KS formula appeared to have the least bias of the formulas. Object Transpedicular instrumentation at C-7 has been well accepted, but salvage techniques are limited. Lamina screws have been shown to be a biomechanically sound salvage technique in the proximal thoracic spine, but have not been evaluated in the lower cervical spine. The following study evaluates the anatomical feasibility of lamina screws at C-7 as well as their bone-screw interface strength as a salvage technique. Methods Nine fresh-frozen C-7 cadaveric specimens were scanned for bone mineral density using dual energy x-ray absorptiometry. Prior to testing, all specimens were imaged using CT to obtain 1-mm axial sections. Caliper measurements of both pedicle width and laminar thickness were obtained. On the right side, pedicle screws were first inserted and then pulled out. Salvage intralaminar screws were inserted into the left lamina from the right spinous process/lamina junction and then pulled out. All screws were placed by experienced cervical spine surgeons under direct fluoroscopic visualization. Pedicle and lamina screws were 4.35- and 3.5-mm in diameter, respectively. Screws sizes were chosen based on direct and radiographic measurements of the respective anatomical regions. Insertional torque (IT) was measured in pounds per inch. Tensile loading to failure was performed in-line with the screw axis at a rate of 0.25 mm/sec using a MiniBionix II system with data recorded in Newtons. Results Using lamina screws as a salvage technique generated mean pullout forces (778.9 ± 161.4 N) similar to that of the index pedicle screws (805.3 ± 261.7 N; p = 0.796). However, mean lamina screw peak IT (5.2 ± 2.0 lbs/in) was significantly lower than mean index pedicle screw peak IT (9.1 ± 3.6 lbs/in; p = 0.012). Bone mineral density was strongly correlated with pedicle screw pullout strength (r = 0.95) but less with lamina screw pullout strength (r = 0.04). The mean lamina width measured using calipers (5.7 ± 1.0 mm) was significantly different from the CTmeasured mean lamina width (5.1 ± 0.8 mm; p = 0.003). Similarly, the mean pedicle width recorded with calipers (6.6 ± 1.1 mm) was significantly different from the CT-measured mean pedicle width (6.2 ± 1.3 mm; p = 0.014). The mean laminar width measured on CT at the thinnest point ranged from 3.8 to 6.8 mm, allowing a 3.5-mm screw to be placed without difficulty. Conclusions These results suggest that using lamina screws as a salvage technique at C-7 provides similar fixation strength as the index pedicle screw. The C-7 lamina appears to have an ideal anatomical width for the insertion of 3.5-mm screws commonly used for cervical fusions. Therefore, if the transpedicular screw fails, using intralaminar screws appear to be a biomechanically sound salvage technique.

Abstract Introduction Women who suffered preeclampsia (PE) have two to four times higher risk for development of cardiovascular disease (CVD) compared with women with a history of normotensive pregnancy. Microvascular and endothelial dysfunction, mediated by different vasoactive factors have been suggested as attainable pathophysiological pathways. The study aimed to: (1) determine changes in circulating levels of key vasoactive peptides in sera of women with history of PE and in women who had a normal pregnancy 1 year after delivery and (2) investigate whether an association exists between these molecules and cardiovascular risk factors. Materials and methods The current research examined 32 women one year after preeclampsia (mean age 25.3 ± 6.3 years) and control group of 20 women one year after normal pregnancy (mean age 25.6 ± 5.6 years). The enzyme-linked immunosorbent assay (ELISA) was used to determine serum levels of angiotensin II (AngII), angiotensin-(1-7) [Ang-(1-7)] and pro-endothelin-1 (Pro-ET-1). Results We found that at 1 year after delivery 38.46% of women who suffered PE have developed hypertension and 5.77% have developed diabetes mellitus. Women who had normal pregnancy developed neither hypertension, nor diabetes mellitus 1 year after delivery. Serum AngII levels in women one year after PE were statistically significantly lower than in women one year after normotensive pregnancy 0.9 (0.55÷1.7) vs. 2.3 pg/ml (2.0 ÷ 2.9) (KW = 20.849; p = 0.0001). Ang-(1-7) concentrations in women one year after PE were lower than in women one year after normal pregnancy, but not significantly 1.7 (0.3÷4.5) vs. 3.2 ng/ml (0.2÷8.0) (p>0.05). Levels of serum Pro-ET-1 in women one year after PE were statistically significantly higher than in women one year after normal pregnancy 322.65 (261.75÷391.85) vs. 248.7 pmol/L (231.05÷282.15) (KW = 6.639; p = 0.009). AngII showed correlation with AH grade (r = -0.33; 0.02), Ang-(1-7) (r = 0.27; 0.05), DBP (r = -0.28; p = 0.04), mean arterial pressure (r = -0.43; p = 0.002), pulse pressure (r = -0.28; p = 0.04), BMI (r = -0.30; p = 0.03), TC (r = -0.31; p = 0.03) and LDL (r = -0.30; p = 0.03). Pro-ET-1 correlated with age (r = 0.30; p = 0.03), mean arterial pressure (r = -0.25; p = 0.05) and pulse pressure (r = 0.41; p = 0.003). Conclusion Our data showed an association between key vasoactive peptides and major CVD risk factors in women one year after PE. We suggest that imbalance between AngII, Ang-(1-7) and Pro-ET-1 could have a potential imply on the vascular wall after PE, reflecting persistent microvascular and endothelial injury/dysfunction postpartum. Further studies are warranted to clarify these vasoactive peptides’ role in the ongoing vascular endothelial function after delivery and the pathogenic mechanisms determining development of CVD in previously PE women.

ABSTRACT. Extracellular nucleotides regulate renal transport. A luminal P2Y2 receptor in mouse cortical collecting duct (CCD) principal cells has been demonstrated elsewhere. Herein the effects of adenosine triphosphate (ATP) and uridine triphosphate (UTP) on electrogenic Na+ absorption in perfused CCD of mice kept on a low-NaCl diet were investigated. Simultaneously, transepithelial voltage (Vte), transepithelial resistance (Rte), and fura-2 [Ca2+]i fluorescence were measured. Baseline parameters were Vte, −16.5 ± 1.2 mV; Rte, 80.8 ± 7.1 Ω cm2; and equivalent short-circuit current (Isc), −261.0 ± 25.1 μA/cm2 (n = 45). Amiloride (10 μM) almost completely inhibited Isc to −3.9 ± 3.8 μA/cm2 (n = 10). Luminal ATP (100 μM) reduced Vte from −16.5 ± 2.1 to −12.5 ± 1.93 and increased Rte from 113.1 ± 16.2 to 123.8 ± 16.7 Ω cm2, which resulted in a 31.7% inhibition of amiloride-sensitive Isc (n = 12). Similarly, luminal UTP reversibly reduced Vte from −22.0 ± 2.1 to −13.6 ± 2.1 mV and increased Rte from 48.4 ± 5.3 to 59.2 ± 7.1 Ω cm2, which resulted in 49.1% inhibition of Na+ absorption (n = 6). In parallel, luminal ATP and UTP elevated [Ca2+]i in CCD, increasing the fura-2 ratio by 2.7 ± 0.7 and 4.0 ± 1.2, respectively. Basolateral ATP and UTP (100 μM) also inhibited amiloride-sensitive Isc by 21.8 (n = 14) and 20.1% (n = 8), respectively. Inhibition of luminal nucleotide-induced [Ca2+]i increase by Ca2+ store depletion with cyclopiazonic acid (3 μM) did not affect nucleotide-mediated inhibition of Na+ transport (n = 7). No evidence indicated the activation of a luminal Ca2+-activated Cl− conductance, a phenomenon previously shown in M-1 CCD cells (J Physiol 524: 77–99, 2000). In essence, these data indicate that luminal ATP and UTP, most likely via P2Y2 receptors, mediate inhibition of amiloride-sensitive Isc in perfused mouse CCD. This inhibition appears to occurs independently of an increase of cytosolic Ca2+.

Abstract Excess consumption of sweet potato storage roots (SWP) poses an acidosis risk to cattle that may be mitigated by limit-feeding strategies. Yet limit-feeding is often administered to achieve mean desired daily DMI in groups of cattle with little regard for DMI variation among individuals. The study objective was to assess variation in daily DMI of SWP by beef heifers in limit-fed, competitive feeding groups. Twenty-four 9- to 10-mo-old Bos taurus crossbred heifers were stratified by initial BW (261.9 ± 8.8 kg) to 4 paddocks (6 heifers/ paddock) and offered SWP (24.9% DM on an as-fed basis; 7.7% CP, 5.9% ADF and 80% TDN on a DM basis) supplementation daily at a rate of 3.4 kg DM/ paddock with ad libitum access to mixed-grass pasture. After a 7-d acclimation period in which all heifers were trained to use a GrowSafe system, daily SWP DMI was monitored for 39 days. Standard deviation estimates were calculated from DMI data and used to classify DMI into the following levels: low (2.0 kg, >0.5 SD). Half of the heifers (mean SWP DMI = 0.0 ± 0.02 kg) never consumed SWP at the high DMI level, but 42% of the heifers (mean SWP DMI = 1.1 ± 0.02 kg) consumed SWP at the high DMI level for at least 18 days over the study duration. No heifers consumed at the moderate DMI level more than 16 days during the trial. This suggests that DMI of individual cattle offered SWP in limit-fed groups often either exceeds by approximately 2-fold per animal mean values of SWP offered per group or is at or near zero. Additional research is needed to elucidate optimal group feeding strategies to achieve individual DMI targets among cattle offered SWP.

ABSTRACT Background: Tai Chi (TC), a mind-body exercise, may be a potential exercise maintenance strategy for patients with chronic obstructive pulmonary disease (COPD) after pulmonary rehabilitation. We sought to characterize the acute cardiorespiratory response during TC versus walking in patients with COPD after a 24-week intervention. Methods: Cardiorespiratory data were available on 26 adults with COPD (46% female, 54% GOLD stage III–IV) randomized to TC (n = 19) or group walking (n = 7). At 24 weeks, we examined between-groups standard cardiorespiratory measures (heart rate (HR), oxygen consumption (VO2), expired carbon dioxide (VCO2), respiratory rate (RR), and ventilation (VE)) during a multiphase in-class characterization protocol. Continuous HR data during resting and exercise phases were analyzed for time- and frequency-domain HR variability (HRV) indices. Results: At 24 weeks, during exercise phases, those in TC exhibited a mean HR of 80 ± 15 b · min−1, RR of 16.5 ± 4 breaths·min−1, and VO2 of 434.8 ± 146.5 mL·min−1; and in walking 95.7 ± 9.2 b·min −1, 26.2 ± 8 breaths·min−1, and 901.3 ± 261.2 mL·min−1, respectively (P &lt; 0.05). Overall, TC was less strenuous with lower HR, VO2, VCO2, RR, and VE (P &lt; 0.05). At rest, TC demonstrated more favorable respiratory efficiency (VE/VCO2; 35.53 ± 5.65 versus 41.07 ± 5.21, P &lt; 0.05). During the postexercise recovery phase, time-domain HRV indices decreased after walking (e.g., pNN20: 35.7 ± 24.1 baseline, 10.3 ± 9.5 postwalk; pNN50: 20.9 ± 18.5 baseline, 3.9 ± 3.7 postwalk), while they remained relatively unchanged after TC. Frequency-domain HRV measures suggested greater total power (TP) across all phases of TC versus walking, particularly during meditation (P &lt; 0.05 for LnTP). Conclusion: Preliminary data support that TC may be associated with improved pulmonary efficiency and reduced rapid shallow breathing compared with walking and be a viable exercise maintenance option after pulmonary rehabilitation.

ABSTRACT Objective: To determine the relationship of the surgeon handedness and operative site laterality on operative duration and hearing improvement in otologic surgery, and to further explore whether this relationship may be related to surgeon experience. Methods:Design: Retrospective CohortSetting: Tertiary Private Teaching HospitalParticipants: Seventy-three (73) patients aged 18 to 65 years old who underwent primary ear surgery under general anesthesia between January 2016 and December 2019 were retrospectively divided into two groups: 39 contralateral and 34 ipsilateral. The operative durations and hearing improvements were compared using independent t-tests, with consideration of surgeon experience in years further stratifying patients. Results: There was no significant difference in operative duration, t(71) = 1.14, p = .26, between the contralateral (M = 281.95 minutes, SD = 71.82) and ipsilateral (M = 261.15, SD = 79.26) groups. This same pattern was more pronounced among surgeons with 10+ years of experience although there was also no significant difference in operative time, t(33) = 1.31, p = .19 for both ipsilateral and contralateral surgeries There was no statistically significant difference, t(36) = -0.72, p = .47, in overall mean hearing gain among patients in the contralateral (M = +2.22 dB, SD = 10.54) and ipsilateral (M = +5.12 dB, SD = 14.26) groups. Although the difference was also not statistically significant, t(16) = -1.94, p = .07 for contralateral (M = 0.00, SD = 5.43) and ipsilateral (M = +7.95 dB, SD = 11.52) procedures performed by surgeons with experience of 10 years or more, a mean hearing gain of +7 dB in the ipsilateral group compared to 0 dB in the contralateral group was notable. Conclusion: This study did not prove that regardless of surgeon experience, right-handed surgeons operating on the right ear and left-handed surgeons operating on the left ear have better ear surgery outcomes of operative duration and hearing improvement compared to right- handed surgeons operating on the left ear and left-handed surgeons operating on the right ear. Future studies on larger samples with more complete data may yet demonstrate this effect.

Background. Hepatectomy is always a challenge to surgeons and requires an appropriate approach for specific tumors to achieve effective complication management. Selective hepatic pedicle clamping is more considerable strategy when comparing with total hepatic pedicle clamping in the balance between reducing blood loss and transfusion with causing the hepatic parenchyma damages (two main complications affecting liver resection result). Objectives. In this study, we aim to describe the application of selective hepatic inflow vascular occlusion (SHIVO) and anatomical anterior approach in liver resection and evaluate the results, focusing on intraoperative and postoperative complications. Methods. We enrolled 72 patients who underwent liver resection with SHIVO at Viet Duc University Hospital in 4-year period (2011-2014) and then followed up all of them until June 2020 (in 52.6 ± 33 months; range, 2-105 months) or up to the time of death. All the patients were diagnosed with primary or secondary liver cancer, and their future remnant liver volume measured on 64-slice CT scan (dm3) to body weight kg > 0.8 % (for right hepatectomy). Perioperative parameters were collected and analyzed. Results. The average operation time was 196.2 ± 62.2 minutes, and blood loss was 261.4 ± 202.9 ml; total blood transfusion proportion during and after surgery was 16.7%. Complications accounted for 44.5% of patients in which pleural effusion was the most common one (41.7%). There were no liver failure and biliary fistula after surgery. No deaths were recorded during 30 days postoperatively. Average hospital stay was 11.4 ± 3.7 days. Blood transfusions during the operation and major liver resection were the factors significantly affecting the percentage of complications after liver surgery in our study. In the last follow-up evaluation, 44 patients were dead and 28 patients were alive, in which 7 with recurrence and 21 without recurrence. The overall survival rate was 38.9%. Conclusion. SHIVO in anatomical liver resection is a safe and feasible approach to help resect precisely targeted tumors and manage several complications in liver resection.

Background: Cardiac rehabilitation with aerobic exercises is the first strategy for nonpharmacological treatment in the postoperative period of individuals undergoing coronary artery bypass grafting (CABG) to improve functional capacity and vascular health. However, other exercise modalities remain uncertain regarding the same benefits. Objectives: Evaluation of the effect of different modalities of exercise, such as early cardiac rehabilitation on subjects submitted to CABG in the six-minute walk test (6-MWT) and on the percentage of flow-mediated dilatation (FMD) of the brachial artery. Methods: A randomized clinical trial in which 15 patients (62.7 ± 6.7 years) who underwent CABG were randomly assigned to the following groups: isometric (IG, Handgrip Jamar®), ventilatory muscle training (VG, PowerBreathe®) and control (CG, conventional respiratory and motor physiotherapy). All patients were attended to physically twice a day (20 min/session) for a consecutive week after the CABG (hospital admission). Functional capacity was assessed by 6-MWT and endothelial function was assessed through the technique of FMD, before and after (~7 days) admission to CABG. The doppler ultrasound videos were analyzed by Cardiovascular Suite® software (Quipu, Pisa, Italy) to measure %FMD. Statistics: Generalized estimation equation, followed by Bonferroni post hoc (p < 0.05). Results: Systolic, diastolic and mean arterial pressure (SBP/DBP/MAP, respectively) were 133, 76 and 95 mmHg. The groups presented walking meters (m) distance before and after intervention of: IGbasal 357.80 ± 47.15 m vs. IGpost 306.20 ± 61.63 m, p = 0.401 (+51 m); VGbasal 261.50 ± 19.91 m vs. VGpost 300.75 ± 26.29 m, p = 0.052 (+39 m); CG basal 487.83 ± 83.23 m vs. CGpost 318.00 ± 31.08, p = 0.006 (−169 m). %FMD before and after intervention was IGbasal 10.4 ± 4.8% vs. IGpost 2.8 ± 2.5%, p = 0.152; VGbasal 9.8 ± 5.1% vs. VGpost 11.0 ± 6.1%, p = 0.825; CGbasal 9.2 ± 15.8% vs. CGpost 2.7 ± 2.6%, p = 0.710 and resting mean basal blood flow was IGbasal 162.0 ± 55.0 mL/min vs. IGpost 129.9 ± 63.7 mL/min, p = 0.662; VGbasal 83.74 ± 12.4 mL/min vs. VGpost 58.7 ± 17.1 mL/min, p = 0.041; CGbasal 375.6 ± 183.7 mL/min vs. CGpost 192.8 ± 115.0 mL/min, p = 0.459. Conclusions: Ventilatory muscle training for early cardiac rehabilitation improved acute functional capacity and modulated mean flow of individuals undergoing CABG.

Aim. To highlight the results of the research conducted in 2020 – 2021 aiming at studying the ffect of multi-depth and multifraction sowing of the hybrid corn seeds of the DMS Sticker mid-early maturity group on the yield when grown in the conditions of the Northern Steppe of Ukraine and determine the economic efficiency of the approaches.Materials and methods. Winter wheat was a predecessor crop in the study. Tillage methods and related conditions are described in the article. Herbicides were used to protect against weeds. Sowing was carried out in the third decade of April at a soil temperature of +8 – 10 ºС. The experiment considered 2 factors, namely the depth of sowing seeds and the size of the seed fraction. The results obtained were processed using measurement, mathematical and statistical methods of research, as well as calculation and comparison approaches.Results and discussion. On average, depending on the factors studied, the height of the plants varied significantly, as a rule, the tallest plants were obtained from large seeds, which had the highest mass of 1000 seeds. Thus, in particular, on average over two years of research, the height of the plants was 250.4 cm when the weight of 1000 seeds was 255 g, and the seeds were wrapped by 4 – 5 cm; when the weight of 1000 seeds was 300 g, the height was 251.1 cm, while with the weight of 350 g it was 258.3 cm; with the wrapping depth of 7 – 8 cm the height was 252.1 cm, 255.8 and 268.5 cm, and with the wrapping depth of 10 – 11 cm it was 257.6 cm, 261.8 and 266.1 cm.Conclusions. The use of the large seed fraction provided an increase in the yield of the DMS Sticker corn hybrid by 1.09 – 1.79 t ha-1 compared to the use of the small seed fraction and was 8.70 t ha-1. When using the large seed fraction and the wrapping depth of 10 – 11 cm, the cost of production was 56,550 UAH ha-1. The cost price of 1 ton of production was the lowest and amounted to UAH 2,247.1, the conditional net profit was the highest – 37,000 UAH ha-1, and the level of profitability was 189.3 %.

Abstract Introduction Central diseases of hypersomnolence (CDH) are lifelong disorders characterized by excessive sleep and daytime sleepiness. Treatment options include modafinil, armodafinil, psychostimulants, pitolisant, clarithromycin, and gamma-hydroxy butyrate. Although the mechanism of action behind efficacy is poorly understood, modafinil is often the first-line therapy. Modafinil’s efficacy relies upon the integrity of brain dopamine (DA) signaling: it increases extracellular dopamine by inhibiting the dopamine transporter (DAT), and genetic deletion of the DAT renders modafinil Synaptic dopamine signaling is further highly dependent upon brain iron availability, leading us to consider whether systemic iron deficiency might impact the efficacy of common wake-promoting agents in CDH. Report of case(s) A 30-year-old woman diagnosed with Idiopathic Hypersomnia (IH) at the age of 22 self referred to our sleep center expressing concern over the loss of her modafinil’s wake-promoting benefits. Her PSG/MSLT had shown a sleep efficiency of 96%, a mean sleep latency of 4.6 minutes, and a single SOREMp on her 2nd nap of 5. She was prescribed modafinil (200mg QAM increased to BID over the past year), which provided symptomatic relief for nearly a decade. Her laboratory work revealed 5% transferrin saturation, low ferritin level ( 7 ng/ml), and normal CSF iron and orexin levels of 33 ng/ml and 261.8 pg/ml, respectively; therefore, she was started on oral iron supplements. At 2 weeks follow-up, serum transferrin saturation and ferritin levels increased to 23% and 17 ng/ml, respectively. Follow-up CSF orexin level was 266.6 pg/ml , and repeat assessments of CSF iron parameters are pending. After iron repletion, modafinil was restarted and titrated over one week to 200 mg in the morning and 100 mg in the afternoon. Subjective sleepiness improved, as evidenced by the patient regaining confidence in driving safely in excess of 70 minutes continuously. Iron deficiency negatively impacts sleep, vigilance, and motor programs in human infants and impairs synaptic dopamine signaling in animal studies. This could explain why the patient responded to Modafinil after iron was repleted. Conclusion Pre-anemic and anemic iron deficiency should be considered a potential readily treatable cause of modafinil’s lack of efficacy by way of effects upon wake-promoting brain dopamine circuits. Abstract Introduction: We and others have reported markedly increased ROS formation in primary myelofibrosis (MF) including post-ET or PV-MF when compared with essential thrombocytosis (ET), polycythemia vera (PV) and normal controls. A growing body of evidence illustrates the interdependency between the Keap1-Nrf2 pathway and p62-mediated selective autophagy, are playing an important role in major cellular defense mechanisms against oxidative and electrophilic stress ; dysregulation of p62-Keap1-Nrf2 axis has been implicated in tumor development. Therefore, we studied p62 mediated autophagy and Nrf2 pathways in patients with MF ,ET and PV. M aterials and Methods :33 patients including 15 patients with MF, 7 patients with PV, 11 patients with ET and 10 normal controls were studied. Cellular ROS measurement: Cellular ROS was determined by a dichlorofluorescein (DCF) assay. The oxidation of H2DCFDA was measured by flow cytometry. Real-time PCR assay: Total RNA was extracted from normal control or patient mononuclear cells using RNeasy Kits from Qiagen (Germantown, MD). Real-time PCR was performed using SsoAdvanced™ Universal SYBR® Green Supermix (Bio-Rad, Hercules, CA) on Bio-Rad iQ5 Multicolor Real-Time PCR Detection System. Western blotting: Total cell lysates were obtained from normal control or patient mononuclear cells. Primary antibodies against Beclin-1, LC3 and p62 were purchased from Cell Signaling Technology (Danvers, MA). Results: Fig.1a shows significantly elevated ROS in MF (2127±261.6) when compared with PV (1090 ± 163.8) or ET (1031 ± 140.9) or controls (549.1 ± 52.50). Fig.1b. shows that Nrf2 protein levels (ratio to controls) was significantly reduced in MF (41.84 ± 29.33), PV (14.42 ± 2.96), ET (14.05 ± 3.02), than the controls ( 101.3 ± 7.64). Fig.1c shows that the RT-PCR values of p62 was significantly increased in MF (1.34 ± 0.06) and grouped PV (1.21 ± 0.07), ET (1.14 ± 0.08) and MF as MPN (1.27 ± 0.04) than controls (1.00 ± 0.05),( P<0.05); ET or PV were not significantly elevated than controls. Conclusions: Patients with MF had increased levels of ROS when compared with PV and ET while Nrf2 levels were significantly reduced in all three( ET,PV, MF) which implies defective detoxification process in MPNs. P62, a marker for defective autophagy was significantly increased in MF than controls . This implies that there is a defective autophagy process in MF, contributing further to the formation of P62 and ROS. There is also a defective detoxification mechanism which further leads to more ROS formation and further DNA damage. The genus Hypericum comprises a large number of species. The flower, leaf, stem, and root of the Hypericum species are widely used in traditional medicine in different cultures. Many Hypericum species have been well investigated phytochemically and pharmacologically. However, only a few reports are available on the H. cordifolium native to Nepal. The present study aims to evaluate the phytochemical composition of different extracts, qualitative analysis of methanol extract of the flower and leaf using thin-layer chromatography (TLC), and the antioxidant properties of components by the TLC-DPPH. assay. The phenolic and flavonoid contents were estimated in different extracts of the leaf and stem, and their antioxidant and antibacterial activities were evaluated. In the phytochemical screening, phenolics and flavonoids were present in ethyl acetate, methanol, and 50% aq methanol extracts of both the leaf and stem. In TLC analysis, the methanol extract of flowers showed the presence of 11 compounds and the leaf extract showed the presence of 8 compounds. Both extracts contained chlorogenic acid and mangiferin. Hyperoside and quercetin were present only in the flower extract. In the TLC-DPPH. assay, almost all of the flower extracts and 5 compounds of the leaf extract showed radical scavenging potential. Estimation of phenolics and flavonoids showed that all the leaf extracts showed higher amounts of phenolics and flavonoids than stem extracts. Among leaf extracts, greater amounts of phenolics were detected in 50% aqueous methanol extract (261.25 ± 1.66 GAE/g extract) and greater amounts of flavonoids were detected in methanol extract (232.60 ± 10.52 CE/g extract). Among stem extracts, greater amounts of flavonoids were detected in the methanol extract (155.12 ± 4.30 CE/g extract). In the DPPH radical scavenging assay, the methanol extract of the leaf showed IC50 60.85 ± 2.67 µg/ml and 50% aq. methanol extract of the leaf showed IC50 63.09 ± 2.98 µg/ml. The methanol extract of the stem showed IC50 89.39 ± 3.23 µg/ml, whereas ethyl acetate and 50% aq. methanol extract showed IC50 > 100 µg/ml. In the antibacterial assay, the methanol extract of the leaf showed the inhibition zone of 12-13 mm and the stem extract showed the inhibition zone of 7–11 mm against S. aureus, E. coli, and S. sonnei, whereas both extracts were inactive against S. typhi. The findings of this study support the traditional use of this plant in Nepal for the treatment of diseases associated with bacterial infections. The present study revealed that the underutilized anatomical parts of H. cordifolium could be the source of various bioactive phytochemicals like other Hypericum species.

Background:The assessment of activity in spondyloarthritis (SpA) and psoriatic arthritis (PsA) involves several domains, including enthesitis. Clinical enthesitis evaluation has shown low sensitivity, specificity and reliability. Ultrasound (US) examination of enthesitis can be an accurate and objective way to evaluate this domain, supporting its inclusion in the assessment of the global state of the diseaseObjectives:The main objective of this study is to analyze de prevalence of Doppler enthesitis in active SpA and PsA patients and to evaluate its association with the disease activity at patient level prior to start a biological therapyMethods:A prospective multicenter cross-sectional study in patients with SpA and PsA with active disease (defined as patients who were going to start or switch biological therapy according to physician criteria and in agreement with clinical guidelines) was undertaken. Basal assessment included clinical features, physical examination and laboratory tests. Patients underwent bilateral US examination of peripheral entheses according to the MAdrid Sonographic Enthesitis Index (MASEI). MASEI and Outcome Measures in Rheumatology (OMERACT) enthesitis Power Doppler (PD) definitions were checked. Each enthesis was scanned in two planes: longitudinal and transverse, and 5 second videos were recorded for reliability. An inter-reader analysis by three readers was performed at each included center. For statistical analysis Mann-WhitneyU and Kruskal-Wallis tests were used. Intraclass correlation coefficient (ICC) and kappa test were used for reliabilityResults:64 consecutive patients were included, of whom 19(29.7%) were ankylosing spondylitis (AS), 7(10.9%), non-radiographic axial spondyloarthritis (nr-axSpA) and 38(59.4%) PsA patients. Mean age was 52.4±12.5 years and 36(56.3%) were males. Mean DAS28 (3.6±1.3) for peripheral involvement, mean BASDAI (5.6±2.2) for axial involvement, and CRP values (10±10.9) reflect moderate-high disease activity at baseline. Demographic, clinical and MASEI baseline characteristics are shown in Table 1. Mean global MASEI score was 29.4 (±11.4) and 55 patients (86%) scored ≥18 (proposed cut-off point to diagnose SpA). At the patient level, abnormal US findings consistent with at least one enthesis showing PD signal were observed in 52(81.3%) of patients using MASEI PD and 48(75%) using OMERACT PD definition without significant variation among the different SpA subtypes (p=0.8 and p=0.6, respectively). In February 2019, Russia's first Population-based Cancer Registry (PCR) was created at the federal district level with a database of more than 1 million 350 thousand observations. It is important to note that the levels of standardized morbidity and mortality indicators in the North-Western Federal District (NWFD) and on average in Russia do not have significant differences: morbidity – 261.3 and 249.5 0/0000 (both sexes), men - 293.2 and 286.8 0/0000, women - 252.7 and 234.5 0/0000, respectively; mortality is both sexes (117.6 and 106.8 0/0000, men - 164.7 and 152.4 0/0000, women - 91.3 and 79.5 0/0000, respectively, which allows us to consider the data obtained from the database of the population cancer registry of the Northwestern Federal District to be close to the national average. After we created the first population cancer registry in Russia in St. Petersburg in 1993, operating according to international standards, the Ministry of Health of the Russian Federation issued Order No. 420 dated December 23, 1996 "On the creation of the State Cancer Registry", which included our recommendations on the nature of work, staffing and technical support. Cancer registries have been established in all administrative territories of the country, many doctors have been trained in St. Petersburg. In this paper, we will focus our attention on the main important criteria of one-year lethality, its true magnitude and the patterns of its changes. Methods. The research material was the database of the population cancer registry of the North-Western Federal District. Statistical methods recommended by the International Association of Cancer Registries were used to assess the patterns of death of patients. Purpose. To reveal the true state of the oncological service of Russia by the leading parameter - the level of one-year mortality. The main criteria for evaluating the effectiveness of anti-cancer measures is the level of the proportion of patients who died during the first year after registration in a territorial oncological institution. Results. According to official data, the one-year mortality rate of patients with malignant neoplasms in Russia and the Northwestern Federal District of the Russian Federation is currently (for both sexes) 20.6 and 20.9% (F. No. 7 of state reporting). The planned indicator is to reduce this level to 16-17% in the coming years. The real values of this indicator, calculated on the basis of the population cancer registry database, are at least 30-40%, as in other countries. All this is what we have to figure out. Such a low value of the indicator is explained by the fact that when preparing state reports by superimposing tables from the district level to the reverse, those who died in the last months of the calendar year are not taken into account. They have not yet been received by oncologists from the State Statistics Committee, and the report for the past year must be submitted from the district level on January 20. The main misconception of all management structures is that they do not know on what basis the state reporting is formed and that the database of the population cancer registry has nothing to do with it. This is what our attention has been directed to in recent years - to compile a state report based on the database of the population cancer registry. Conclusions. Taking into account the presence of existing territorial cancer registries in all administrative territories of Russia, which have accumulated extensive statistical material, switch to calculating one-year mortality rates only on the basis of the population cancer registry database after the data for the reporting year is fully completed. Present these data not only together for both sexes, but separately for men and women. To bring the database of the population cancer registry in full compliance with International standards of its management, to calculate not only the mortality rate in the first year of observation, but also the one-year and five-year observed and relative survival. The assessment of the activity of the oncological service should be carried out according to the dynamic series of survival only on the basis of the database of the population cancer registry.

Introduction: Subarachnoid hemorrhage (SAH) is one of the most devastating forms of hemorrhagic stroke and it is a serious medical condition caused by bleeding of a ruptured aneurysm. The etiopathology of the disease includes early brain injury, vasospasm, and delayed cerebral ischemia. Blood byproducts cause the walls of arteries to constrict and spasm. At physiological concentrations, we and others have reported that carbon monoxide (CO) has functional roles in neuroprotection through anti-inflammation, anti-oxidative stress, and improving blood flow. Hence, this study focuses on the safe noninvasive therapeutic strategy to treat SAH by using CO as therapeutic medical gas. Hypothesis: We propose that CO may prevent SAH by mitigating vasospasm, limiting brain injury and improving functional outcomes. Methods: SAH was induced in adult mice using the endovascular perforation method. Mice were treated either with 250ppm CO or medical Air (at the same flow rate) after 2h of SAH for 1h daily for 7d. Such CO administration protocol keeps carboxyhemoglobin within normal range. Neurobehavioral outcomes such as focal neurological deficit score, open field activity, and motor functions were monitored at day 1 and day 7. Data acquisition and statistical analysis were performed by an investigator blinded to the intervention. Results: At 7d after SAH, CO decreased neurological deficit score (47.4±10.5%, p<0.05), and increased activity (30.0±9.1%, p<0.05) and stereotypic counts (261.5±62.1%, p<0.01) as compared to the air control group. No significant changes in motor function were observed in CO treated mice as compared to SAH at either day 1 or day 7). Of interest, there was a significant increase in CO treated group’s lumen area/wall thickness ratio in the middle cerebral artery (173.5±19.3%, p<0.01) and trended to increase in the anterior cerebral artery (25.5±4.3%, p=0.58) at 7d. Conclusion: This is a first report to demonstrate that CO prevents delayed SAH-induced neurobehavioral deficits, which suggest that post-treatment with CO should be considered a therapeutic strategy against SAH.

Aim: To determine the impact of initiation of dulaglutide therapy on the course of COVID-19 and the dynamics of inflammatory markers in hospitalized patients with type 2 diabetes mellitus (T2DM). Methods: The inclusion criteria were history of T2DM, BMI &gt; 27 kg/m2, PCR-confirmed diagnosis of COVID-19. The intervention group of 53 patients started dulaglutide therapy (1.5 mg once weekly) during the first 24 hours of admission, the control group consisted of 50 patients, who proceeded with glucose-lowering therapy. In both groups we compared COVID-19 outcomes (mortality/survival rate, ICU admission, need for assisted ventilation) and the duration of hospitalization. On days 3 and 7 of hospitalization we assessed laboratory parameters (FPG, CRP, ALT, AST, LDH, lymphocyte levels, D-dimer). Results: The initiation of dulaglutide therapy reduced the risk of death and transfer to assisted ventilation by 3.5 times in the intervention group compared to the control one (5.7% vs 20.0%, p=0.038). No differences were found in the risk of ICU admission (17.0% vs 28.0%, p=0.18) and the duration of hospitalization (10 bed-days vs 11-bed days, p=0.26). The groups were comparable in respect to carbohydrate metabolic compensation. Compared to those in the control group, patients on dulaglutide therapy demonstrated a lower CRP level (15.8 vs 24.6 mg/L, p=0.035), higher lymphocyte levels (1.2 vs 0.9 × 10*9/L, p=0.049) on day 3 of hospitalization, and a significantly lower LDH concentration level on day 7 (261.6 vs 326.1 U/L, p=0.016). There were no differences in ALT, AST, and D-dimer levels. Conclusion: The initiation of dulaglutide therapy in hospitalized patients with T2DM and COVID-19 decreased the risk of lethal outcomes and transfer to assisted ventilation, as well as proved effective in regards to inflammatory markers (CRP, lymphocytes levels). Background Minimally invasive procedures are demanding in terms of cardioprotection. In many leading centres Bretschneider HTK solution is used for mitral valve surgery. The study was designed to provide comparison of the del Nido and Bretschneider HTK protocol. Methods Patients who underwent minimally invasive mitral valve repair for primary mitral regurgitation and received single delivery of either del Nido (Group 1) or Bretschneider HTK cardioplegia (Group 2) were matched on basis of age, gender and length of the cross-clamp time. The groups were compared in terms of major adverse cardiac and cerebrovascular events (death, myocardial infarction, stroke), high sensitivity troponin T (hs-TnT) and creatine kinase- MB isoenzyme (CK-MB) release at 12 h and 24 h following the surgery, incidence of low cardiac output syndrome (LCOS), postoperative arrhythmia, transfusions and postoperative renal function. Results Case control matching selected 38 pairs of patients. None of patients died, nor suffered from myocardial infarction or stroke. Troponin values did not differ at 12 h (median: 281.0 pg/mL vs 313.0 pg/mL; p = .38) and 24 h (median: 261.0 pg/mL vs 299.0 pg/mL; p = .54), as well as CK-MB at 12 h (median: 25.0 ng/mL vs 29.0 ng/mL; p = .31) and 24 h (median: 11.0 ng/mL versus 9.6 ng/mL; p = .46). Difference in occurrence of LCOS was insignificant (2 vs 7; 5.2% vs 18.4%; p =.15). No difference was shown in incidence of postoperative arrhythmia, transfusions and renal function. Conclusions Del Nido cardioplegia can be used safely as an alternative for Bretschneider HTK for minimally invasive mitral valve surgery.

Introduction: The Black Carbon (BC) and Particulate Matters (PMs) was measured in the Diwali festival during 3-7 November, 2021. Diwali is being one of the main festivals of India accompanied by lighting and firecrackers bursting. Materials and methods: BC concentration was measured by aerosol black carbon low-cost sensor/detector and PM1, PM2.5 and PM10 mass concentration as well as number concentration was measured using Grimm Aerosol Spectrometer at Agra, India. Results: Average mass of PM1 (<1 μm), PM2.5 ( <2.5 μm) and PM10 ( <10 μm) were 218.32±56.82 μg/m3, 261.56±74.38 μg/m3 and 380.40±67.78 μg/ m3 respectively in Diwali day (2021). The observed concentration of PM was much higher than National Ambient Air Quality Standards (NAAQS) and World Health Organization (WHO) standard limits. Diurnal peaks for all the pollutants (PM1, PM2.5 and PM10) were perceived in the evening time which was due to festival celebration events. Particle Size number Distribution (PSND) showed one major peak in the particle <1 μm size range. BC mass concentration was found to be 16.28±5.62 μg/m3 in Diwali days. BC/PMs ratio decreases as the PM size increases due to the heavy load of PM2.5. Conclusion: BC and PM concentration was significantly amplified on Diwali due to firecrackers bursting and candles and biomass burning which was responsible for the poor air quality. Particle Mass Size Distribution (PSMD) showed multiple peaks while PSND showed one major peak in fine mode. Higher PM1/PM2.5 indicated the abundance of fine particles as compared to coarse. BC concentration was increased by 20 % on Diwali day due to firecracker bursting, other burning materials and more vehicles movement.

Abstract Background It is crucial to differentiate between benign and malignant pleural effusions while making a diagnosis. The purpose of this research was to investigate the diagnostic significance of GSDMD and VEGF in discriminating between different kinds of pleural effusion and their correlation with both progression-free and overall survivals in the malignant type. Methods Ninety-one pleural fluid samples, which were classified as transudates or exudates (tuberculous, para-infectious, or malignant) by pleural fluid classifications, were tested for GSDMD using sandwich ELIZA kits, and 41 of the exudative samples were randomly selected for VEGF testing. Both markers' diagnostic accuracy was assessed. Results The lowest level of GSDMD was associated with the transudate group (mean and SD of 2.35 ± 0.44 ng/mL) and the highest in the malignant effusion group (mean and SD of 4.38 ± 1.67 ng/mL). The specificity and sensitivity of GSDMD in the diagnosis of exudative PE were 97% and 98%, respectively (p = 0.001) with the cutoff point = 2.89). Regarding VEGF, its level was 222.3 ± 53.4 pg/ml for all studied samples where MPE (n = 21) was 261.2 ± 48.2 pg/ ml (mean ± SD), TBPE (n = 7) was 185.4 ± 6.96 pg/ml (mean ± SD), and PIPE (n = 13) was 179.3 ± 13.9 pg/ml (mean ± SD). The diagnostic accuracy of VEGF for the detection of MPE was 90% with a sensitivity of 100% and specificity of 80% and the cutoff point was 191.5 pg/ml. There were highly significant inverse correlations between progression-free survival and both GSDMD (r =− 0.531, p = 0.009) and VEGF (r = − 0.582, p = 0.006) in MPE. Conclusion Pleural effusion GSDMD can be an effective marker for differentiating the different kinds of PE, and VEGF levels can be a useful adjuvant marker in screening out MPE as a possible diagnosis, leading to the proper selection of patients who may benefit from more invasive procedures.

Recent evidence suggests that the reninangiotensin system (RAS) plays a vital role in adipocyte biology and the pathophysiology of metabolic syndrome. Obesity is a major risk factor and mesenchymal stem cells (MSCs) are suggested to be an important source of new fat cells. Previously we reported that MSCs have all the components of the RAS and that angiotensin II (Ang II) can influence adipocyte differentiation. However, the effect of Ang II type 2 receptor (AT2R) deletion on adipogenesis in mouse model is unknown. Accordingly, we employed AT2R-null mice and examined the role of AT2R in the differentiation of MSCs to adipocytes. Methods and Results: Murine MSCs were isolated from AT2R-null mice and wildtype littermates (WT). Confluent MSCs were differentiated into adipocytes by the established protocol. Adipogenesis was quantitated by assessing lipid droplet accumulation using 2 different methods. Using the fluorescent dye which binds to lipid droplets (AdipoRed), AT2R-null group showed significantly increased total fluorescence (261.6 ± 49.6 % vs. WT) on day 7. Oil red O staining, followed by extraction of the absorbed dye and measurement of the absorbance on day 14, also exhibited that AT2R-null group showed significantly increased lipid droplet accumulation (202.7 ± 14.1 % vs. WT). We next examined the expression of adipocyte-specific genes by real-time RT-PCR. AT2R-null group exhibited a significantly increased expression of PPARγ, fatty acid synthase, and adiponectin (vs. WT). Since Wnt 10b/beta-catenin signaling is reported to play an important role to inhibit adipogenesis, we further examined it by real-time RT-PCR and immunoblotting. The AT2R-null group exhibited significantly decreased Wnt 10b expression accompanied by a decrease of cellular beta-catenin (vs. WT). Conclusion: Our results demonstrate that the AT2 receptor has an inhibitory effect on adipogenic differentiation in murine mesenchymal stem cells. Moreover, this inhibitory effect is associated with Wnt 10b/beta-catenin signaling which plays an important role to inhibit adipogenesis. These results provide important insights into the pathophysiology of obesity and obesity related consequences such as metabolic syndrome and may identify potential therapeutic targets.

Despite β-cell failure in youth with dysglycemia (DYSG) (IGT + T2D), fasting insulin concentrations are elevated. Here, we examined 1) metabolic clearance rate of insulin (MCRI) in youth with normal glucose tolerance (NGT) and DYSG, 2) racial/ethnic differences in insulin dynamics and 3) physical and metabolic correlates of MCRI. In 95 Black (B) + 111 White (W) youth, age 14.4 ± 2.0 [SD] yrs., BMI 34.7 ± 6.4 kg/m2, MCRI and peripheral insulin sensitivity (PIS) were calculated from the hyperinsulinemic-euglycemic clamp, 1st-phase insulin (1stPhI) from the hyperglycemic clamp, disposition index (DI=PIS x 1stPhI), % body fat by DXA, visceral adipose tissue (VAT) by MRI. In DYSG vs NGT, MCRI was lower (261.6 ± 11.9 vs 334.1 ± 8.5 ml/m2/min, p&lt;0.001) and fasting insulin was higher (46.9 ± 4.4 vs 32.9 ± 1.2 μU/mL, p&lt;0.001). In B vs W youth (Image), MCRI was lower driven by DYSG. Lower PIS, lower MCRI and higher 1stPhI were characteristics of B youth with DYSG vs W with equally impaired DI. In B and W youth, positive correlates of MCRI were PIS and DI and negative correlates were fasting insulin, % body fat and VAT. In conclusion, higher fasting insulin concentrations in DYSG despite a failing β-cell could be explained by decreased MCRI. Racial/ethnic contrast in insulin dynamics differs by glycemic status and is more pronounced in DYSG manifested by lower PIS and lower MCRI in B vs W youth with no difference in β-cell function relative to insulin sensitivity. Objectives: To compare the choroidal thickness in eyes of diabetic patients with eyes of age matched controls using optical coherence tomography in a tertiary care hospital. Methods: This Cross sectional study was conducted at the Department of Ophthalmology, Jinnah Postgraduate Medical Centre Karachi, for six months from13thJanuary 2020 to13thJuly 2020. The study group comprised of 44 patients with 88 eyes. Patients who fulfill the inclusion criteria that is age ranging from 35 to 80years, either gender, known case of diabetes mellitus and having any type of diabetic retinopathy (HbA1c >7), non-diabetic healthy individuals (HbA1c <7) and those giving informed consent were included in the study. However, patients having active ocular infections, history of myocardial infarction, stroke, uveitis, any ocular surgery, lasers, intravitreal injections, poor fundus view and not giving consent were excluded. A pre-designed proforma was filled. A baseline ocular examination was performed and choroidal thickness was assessed from retinal pigment epithelium to choroid sclera junction in diabetic and healthy participants of the study group using high resolution Swept source OCT (DRI-OCT-2 Triton; Topcon). Results: The average age of the patients was 39.41±15.95 years. According to our study mean central subfoveal choroidal thickness in diabetic eyes was 268.5 ± 66.22 (95% CI 240 – 297) and in non-diabetic healthy participants it was 339.3 ± 71.49 (95% CI 308 – 369) with a p-value of 0.001. However, average choroidal thickness was 261.8 ± 61.93 (95% CI 235 – 288) and 336.0 ± 74.35 (95% CI 304 – 367) in diabetic and non-diabetic healthy population with a p-value of 0.001. Choroidal thickness comparison between gender in diabetic and non-diabetic population also showed similar trend. Conclusion: In this study, mean central choroidal thickness as well as average choroidal thickness was significantly reduced in eyes having diabetic retinopathy as compared to participants with non-diabetic healthy eyes. Women with polycystic ovary syndrome (PCOS) are characterized by hyperandrogenemia, hirsutism, infertility, and enlarged cystic ovaries. Obesity and hypertension are also frequently found in women with PCOS, although the mechanisms responsible for the elevated blood pressure (BP) are unclear. Thus we tested the hypothesis that the melanocortin-4 receptor (MC4R), known to contribute to obesity hypertension in males, contributes to the elevated BP in hyperandrogenemic female rats, a model of PCOS. Female Sprague Dawley rats (4 wks) were implanted with dihydrotestosterone (DHT; 7.5mg/90 days sc) or placebo pellets (PL) (n=5/grp) and aged to 12 wks. Body weight and food intake (whether rats were pair fed or had ad libitum access) were measured daily. Two wks following implantation of radiotelemetry transmitters and intracerebroventricular cannulae, baseline mean arterial pressure (MAP) was measured for 5 days; then rats received MC3/4R antagonist, SHU-9119 (SHU; 1 nmol/h ICV) or vehicle for 7 days and MAP was recorded. DHT-treated rats had higher body weight and MAP than PL rats (BW: PL: 266.0±8.7; DHT: 348.5±10.4 g, p<0.01; MAP: PL rats: 102±5; DHT: 114±5 mmHg, p<0.05). SHU significantly increased food intake and body weights in both placebo (PL) and DHT-treated rats fed ad libitum (PL: 379.2±28.5; DHT: 451/3±7.3 g, p<0.01 DHT vs PL; 0.01 SHU vs control), but had no effect on MAP compared to controls (PL: 104±5; HAF rats: 114±5 mmHg; p<0.05, HAF vs PL; p=NS, SHU vs controls). However, in other rats, when pair fed with little increase in body weight (PL: 253.7±2.0, SHU: 261.0±0.6, p<0.05; DHT: 306.7±2.6, SHU: 316.7±1.5 g, p<0.05), SHU decreased MAP in DHT treated rats but not placebo controls (PL rats: 102±1, SHU: 103±2 mmHg; p=NS; DHT rats: 110±1 vs. SHU: 97±1 mmHg; p<0.001). Thus MC4R antagonist reduces MAP in DHT-treated rats only when food intake and body weight are controlled. These data suggest that activation of MC4R contributes to elevated BP in our model of PCOS and may also contribute to the elevated BP in women with PCOS. Abstract Background Antimicrobial resistance is a public health problem of global concern and there is a high turnover of antibiotics developing resistance when compared with new drugs being developed. Therefore, there is a need to develop multifunctional systems that protect and potentiate the current drug on the market. Herein, we present novel dual pH/enzyme-responsive capsules for ciprofloxacin free base (CFB) delivery against MRSA and P. aeruginosa. Methods Nano capsules preparation was carried out using the interfacial deposition method and was characterized for their physicochemical properties, as well as binding activities to HAase, and in vitro antibacterial properties. The preliminary biocompatibility was also evaluated via MTT assay. Results The cytotoxicity results revealed the viability of the MCF-7 celines to be above 75% after exposure to the prepared capsules, therefore indicating its biosafety. Capsules had hydrodynamic diameter (DH), polydispersity index (PDI) and a surface-charge (ζ) of 261.4 ± 0.70 nm, 0.26 ± 0.01, and −15.2 ± 2.36 mV at pH 7.4 and 258.9 ± 2.51 nm, 0.28±0.01, and −5.27 ± 0.12 mV at pH 6 respectively, with an entrapment efficiency (EE%), of 85.63 ± 2.83%. Results from the release study indicated a faster CFB release from the prepared capsules at pH 6 compared with pH 7.4 confirming pH-responsive drug release. Also, upon the addition of HAase enzyme, the release rate of CFB at pH 7.4 was faster compared with the capsules without the enzyme at the same pH. MST results showed the prepared capsules to have a higher binding activity to bacterial enzyme HAase compared with the natural substrate with Kd of 0.1325 ± 0.0099 μM. Capsules were seen to show a better antibacterial efficacy compared with bare CFB with 16-fold and 128-fold recorded at pH 7.4; 128-fold and 8-fold recorded at pH 6, against MRSA and P. aeruginosa, respectively. Intracellular component leakage studies revealed the capsules to destroy the cell wall of the bacteria, thereby causing a greater reduction in the quantity of intracellular constituents such as protein and DNA, compared with bare CFB. Also, capsules showed better inhibition of efflux pump by a show of higher fluorescence intensity compared with bare CFB. Conclusions Thus, the prepared capsules are a potential nano delivery system for CFB antibiotics, with the potential of improving the antibacterial efficacy of CFB against infections and disease condition induced by MRSA and P. aeruginosa.

This study examined the effect of transfer of a 2.4 Mbp region of Brown Norway (BN) rat Chr 1 into the Fawn Hooded Hypertensive (FHH) genetic background on autoregulation (AR) of cerebral blood flow (CBF) and the myogenic response of isolated middle cerebral arteries (MCA). Autoregulation of CBF measured by laser Doppler flowmetry was poor in FHH rats (AR index (AI), 0.8±0.1) and in FHH.1BN congenic strains which excluded the critical region (AR-: AI, 0.9± 0.1). In contrast, autoregulation of CBF was completely restored by transfer of the region of BN Chr 1 between 258.8 to 261.2 Mbp in AR+ FHH.1BN congenic strains (AI, 0.3 ± 0.1). The diameter of MCA of FHH rats and AR- congenic strains increased by ∼10% (140 ± 1 to 157± 2 μm), when transmural pressure was increased from 40 to 140 mmHg. In contrast, the diameter of the MCA in AR+ congenic strain fell from 127 ± 2 to 65 ± 1 μm. Whole-cell patch-clamp of cerebral VSM cells revealed a 4.3-fold increase in BK channel current densities at depolarized potentials in FHH versus AR+ rats (105 ± 2 versus 24 ± 4 pA/pF at +80mV). Using single channel analysis we found that the increase in BK channel current was largely due to a marked increase in the NPo of BK channel in FHH as compared to AR+ rats (0.9 ± 0.1 and 0.2 ± 0.1 at +80mV). To explore the significance of the impaired myogenic response, we compared changes in CBF and infarct size following transient occlusion and reperfusion of the MCA in FHH rats and the AR- and AR+ congenic strains. Occlusion of MCA reduced CBF similarly in all the strains. However, the hyperemic response following reperfusion in FHH and AR- strains was significantly greater and more prolonged than that seen in AR+ rats (AR-: 173 ± 1%, 45 min versus AR+: 124 ± 5%, 15 min). Moreover, infarct size and edema formation was significantly greater in the AR- congenic strain (39 ± 3 % and 12 ± 2 %) in comparison to that seen in the AR+ strain (28 ± 2 %; 7 ± 1%). These results indicate that there is a gene that plays a critical role in the regulation of the myogenic response of the cerebral vasculature by altering BK channel activity in the critical 2.4 Mb region of Chr 1 containing just 15 genes and that transfer of this region from BN to FHH rats restores autoregulation of CBF, vascular reactivity and reduces infarct size following ischemia/reperfusion injury.
