Résumé :
The partitioning of above ground nitrogen (N) in chickpea (Cicer arietinum), lentil (Lens culinaris), lupin (Lupinus angustifolius) and pea (Pisum sativum) at crop maturity was investigated. The plants were grown at different plant populations and sowing depths in 1998/99 and 1999/2000 in Canterbury, New Zealand. In all four legumes the N concentration was highest in seed (29–36 mg/g N) followed by senescent leaves. The lowest N concentration was in stems. Lupin had the highest seed N yield at 16·82 and 19·29 g/m2 followed by chickpea at 10·26 and 13·10 g/m2, in 1998/99 and 1999/2000, respectively. Lentil had the lowest N concentrations and yield. The distribution of N to all legume plant parts increased as population increased up to twice the optimum and 400 plants/m2 in 1998/99 and 1999/2000, respectively. Over all plant populations, the nitrogen harvest index (NHI) was stable in each of the four species across the two seasons. However, the NHI changed with changes in plant population and sowing depth. The N accumulation efficiency (EN) was highest in lentil at 0·024 and 0·027 g N/g DM and lowest in chickpea at 0·018 and 0·021 g N/g DM in 1998/99 and 1999/2000, respectively. The NHI and the crop harvest index (CHI) were correlated and both were strongly associated with seed yield. Thus, NHI might be useful as a selection criterion to improve seed yield in grain legumes.

Résumé :
Simulation-based learning activities in the emergency department (ED) improve communication and teamwork and familiarise personnel with existing protocols. The authors’ objective was to develop standardised in-situ simulations and to assess their effects on team performance during simulated patient care. The study was a prospective, single-centre pre-in-situ and post-in-situ simulation-based intervention in the ED of an academic hospital between March 2017 and February 2018. Teams of three to five participants (n=46) were in two simulation interventions 2 weeks apart; each simulation was followed by debriefing with good judgement. The adapted Simulation Team Assessment Tool (STAT) Score was the primary measure for team performance. Skills are measured on a scale of 2–0 based on the complete and timely performance of tasks for a total (adapted) score of 171. Overall STAT scores improved significantly between simulations I (60.5 (28.3)) and II (81.1 (24.6)), p=029; notably in airway and teamwork domains, p=022 and p=023, respectively. A sub-analysis showed that participants performed significantly better when treating adult versus paediatric simulated patients (87.9 (20.1)), p=003, particularly in teamwork, p=01. The study yielded statistically significant improvement in clinical management, teamwork and resource management skills among ED personnel.

Résumé :
8568 Background: Ipilimumab (Ipi) is a human monoclonal antibody directed against cytotoxic T-lymphocyte antigene-4 (CTLA-4) recently approved for the treatment of metastatic melanoma (MM) and currently under investigation in the adjuvant setting. Methods: Retrospective analysis of patients treated with ipi between June 2006 and September 2011 in our department in Marseille. As some patients are still blinded in trials, the exact number of patient under ipi is unknown. We present a minimal percentage (>%) assuming that the 120 patients received ipi. Results: A total of 120 patients were treated: 76 stages IV MM, from which 16 in the BMS clinical trials (CA184-022, -024, and-025 and MDX 010-20) and 44 stages III MM (in the BMS CA184-029 trial). Stage IV MM were administered 0.3, 3 or 10mg/kg IV dosage, while stages III MM were randomly assigned to receive 10 mg/ kg or placebo (1:1 ratio). Hypophysitis was diagnosed in 12 patients (>10%): 2/76 patients with stage IV MM (>2. 6 %) and 10/44 patients with stage III MM (>22.7). Diagnosis was performed at the 1st, 3rd and 4th administration in respectively 2 (1.6%), 6 (50%) and 4 patients (33.3%). Clinical symptoms included headaches (n=11; 91.6%), asthenia (n=7; 58.3%) and decreased libido (n=2; 1.6%). Adrenal, thyroidal and gonadal axis were affected in respectively 6 (50%), 9 (75%) and 7 patients (58.3%). MRI changes were observed in 7 patients (58.3%): pituitary swelling in 5 patients (41.6%) and heterogeneous enhancement in 4 patients (33.3%) including 2 patients with normal biology. Corticosteroids supplementation was required in 11 patients and thyroidian supplementation in 4 patients. Clinical symptoms regressed within one week in 8 patients (66.6%). Conclusions: Ipi-induced hypophysitis is detectable only if clinicians are aware of these unspecific signs. Only MRI can make diagnosis in some patients without clinical and/or biological signs. Our data suggest that it develops especially for 10mg/kg dosage, after the third administration, and that the rate could be higher in patients with a normal immune system (adjuvant treatment), than in metastatic ones. Hormonal supplementation usually controls the disease.

Résumé :
Purpose To determine whether addition of gemcitabine to concurrent cisplatin chemoradiotherapy and as adjuvant chemotherapy with cisplatin improves progression-free survival (PFS) at 3 years compared with current standard of care in locally advanced cervical cancer. Patients and Methods Eligible chemotherapy- and radiotherapy-naive patients with stage IIB to IVA disease and Karnofsky performance score ≥ 70 were randomly assigned to arm A (cisplatin 40 mg/m2 and gemcitabine 125 mg/m2 weekly for 6 weeks with concurrent external-beam radiotherapy [XRT] 50.4 Gy in 28 fractions, followed by brachytherapy [BCT] 30 to 35 Gy in 96 hours, and then two adjuvant 21-day cycles of cisplatin, 50 mg/m2 on day 1, plus gemcitabine, 1,000 mg/m2 on days 1 and 8) or to arm B (cisplatin and concurrent XRT followed by BCT only; dosing same as for arm A). Results Between May 2002 and March 2004, 515 patients were enrolled (arm A, n = 259; arm B, n = 256). PFS at 3 years was significantly improved in arm A versus arm B (74.4% v 65.0%, respectively; P = .029), as were overall PFS (log-rank P = .0227; hazard ratio [HR], 0.68; 95% CI, 0.49 to 0.95), overall survival (log-rank P = .0224; HR, 0.68; 95% CI, 0.49 to 0.95), and time to progressive disease (log-rank P = .0012; HR, 0.54; 95% CI, 0.37 to 0.79). Grade 3 and 4 toxicities were more frequent in arm A than in arm B (86.5% v 46.3%, respectively; P < .001), including two deaths possibly related to treatment toxicity in arm A. Conclusion Gemcitabine plus cisplatin chemoradiotherapy followed by BCT and adjuvant gemcitabine/cisplatin chemotherapy improved survival outcomes with increased but clinically manageable toxicity when compared with standard treatment.

Résumé :
ABSTRACT Since 2002, an epidemic of Clostridium difficile infections has occurred in southern Quebec, Canada. At Hôpital Maisonneuve-Rosemont, Montreal, Quebec, Canada, the incidence of C. difficile infections increased from 11/1,000 admissions (1999 to 2002) to 27/1,000 admissions (2003 to 2005). We compared the exposures and outcomes for patients infected with strains with different ribopatterns isolated before (n = 55) and during (n = 175) the epidemic, as well as the in vitro activities of antibiotics against those isolates. During the preepidemic period, 46 isolates (84%) were of ribotype 001, 1 was of ribotype 027, and 8 were of other ribopattern types. During the epidemic period, ribotype 027 strains accounted for 140 (80%) isolates; 26 (15%) were of ribotype 001, and 7 were of other ribopattern types. Ribotype 027 strains were highly resistant to fluoroquinolones (FQs) but were susceptible to clindamycin. A pattern of prior specific antibiotic exposure that selected for antibiotic-resistant ribotype C. difficile infections was observed for FQs (ribotype 027) and clindamycin (ribotype 001). The rate of mortality was higher among older patients, those with a high Charlson comorbidity index, and those with longer previous hospitalizations. By multivariate analysis, patients infected with ribotype 027 were twice as likely to die within 30 days of diagnosis than patients infected with other ribotypes (adjusted odds ratio, 2.06; 95% confidence interval, 1.00 to 4.22). The observations from this study support the notion that continued selective antibiotic pressure resulted in the superimposition of the hypertoxigenic ribotype 027 clone on top of the prior dominant ribotype 001 clone in a setting of preexisting high endemicity, thus leading to the high rates of morbidity and mortality seen in the Quebec outbreak. Stringent antibiotic stewardship measures, combined with aggressive infection control, are required to curtail the epidemic of C. difficile infections.

Résumé :
AbstractObjectiveTo examine trends in stunting and overweight in Peruvian children, using 2006 WHO Multicentre Growth Reference Study criteria.DesignTrend analyses using nationally representative cross-sectional surveys from Demographic and Health Surveys (1991–2011). We performed logistic regression analyses of stunting and overweight trends in sociodemographic groups (sex, age, urban–rural residence, region, maternal education and household wealth), adjusted for sampling design effects (strata, clusters and sampling weights).SettingPeru.SubjectsChildren aged 0–59 months surveyed in 1991–92 (n 7999), 1996 (n 14 877), 2000 (n 11 754), 2007–08 (n 8232) and 2011 (n 8186).ResultsChild stunting declined (F(1, 5149) = 174·8, P ≤ 0·00) and child overweight was stable in the period 1991–2011 (F(1, 5147) = 0·4, P ≤ 0·54). Over the study period, levels of stunting were highest in rural compared with urban areas, the Andean and Amazon regions compared with the Coast, among children of low-educated mothers and among children living in households in the poorest wealth quintile. The trend in overweight rose among males in coastal areas (F(1, 2250) = 4·779, P ≤ 0·029) and among males in the richest wealth quintile (F(1, 1730) = 5·458, P ≤ 0·020).ConclusionsThe 2011 levels of stunting and overweight were eight times and three and a half times higher, respectively, than the expected levels from the 2006 WHO growth standards. The trend over the study period in stunting declined in most sociodemographic subgroups. The trend in overweight was stable in most sociodemographic subgroups.

Résumé :
Background:Tumour necrosis factor (TNF) antagonists have proved effective as treatment against rheumatoid arthritis (RA), but the unresolved issue of whether the use of anti-TNF therapy increases the already elevated risk of lymphoma in RA remains a concern. Methods:Using the Swedish Biologics Register (ARTIS), the Swedish Cancer Register, pre-existing RA cohorts and cross-linkage with other national health and census registers, a national RA cohort (n = 67 743) was assembled and patients who started anti-TNF therapy between 1998 and July 2006 (n = 6604) were identified. A general population comparator (n = 471 024) was also assembled and the incidence of lymphomas from 1999 to 31 December 2006 was assessed and compared in these individuals. Results:Among the 6604 anti-TNF-treated RA patients, 26 malignant lymphomas were observed during 26 981 person-years of follow-up, which corresponded to a relative risk (RR) of 1.35 (95% CI 0.82 to 2.11) versus anti-TNF-naive RA patients (336 lymphomas during 365 026 person-years) and 2.72 (95% CI 1.82 to 4.08) versus the general population comparator (1568 lymphomas during 3 355 849 person-years). RA patients starting anti-TNF therapy in 1998–2001 accounted for the entire increase in lymphoma risk versus the two comparators. By contrast, RR did not vary significantly by time since start of first treatment or with the accumulated duration of treatment, nor with the type of anti-TNF agent. Conclusion:Overall and as used in routine care against RA, TNF antagonists are not associated with any major further increase in the already elevated lymphoma occurrence in RA. Changes in the selection of patients for treatment may influence the observed risk.

Background: Cannabis demand (i.e., relative value), assessed cross-sectionally via a hypothetical marijuana purchase task (MPT), has been associated with cannabis use, problems, and dependence symptoms, among others. However, neither the prospective stability of the MPT, nor the cyclical relationship between demand and use over time has been investigated. Moreover, cannabis demand among cannabis using veterans has yet to be examined. Method: Two waves of data from a veteran sample (N=133) reporting current (past 6-month) cannabis use were analyzed to assess stability and change in cannabis demand over six months. Autoregressive cross-lagged panel models assessed the longitudinal associations between demand indices (i.e., intensity, Omax, Pmax, breakpoint) and cannabis use. Results: Models revealed unique directions of effects for each demand index. Baseline cannabis use predicted greater intensity (ß = .32, p<.001), Omax (ß = .37, p<.001), breakpoint (ß = .28, p<.001), and Pmax (ß = .21, p = .017) at 6-months. Conversely, baseline intensity (ß = .14, p=.028), breakpoint (ß = .12, p=.038), and Pmax (ß = .12, p = .043), but not Omax, predicted greater use at 6-months. Discussion: Cannabis demand indices demonstrated prospective stability over six months and varied along with natural changes in cannabis use. Importantly, intensity, Pmax, and breakpoint displayed bidirectional predictive associations with cannabis use, and across indices, the prospective pathway from use to demand was consistently stronger. Findings highlight the value of assessing cannabis demand longitudinally, particularly among clinical samples, to determine how demand fluctuates in response to experimental manipulation, intervention, and treatment.

Résumé :
Crucial to the development and characterization of thermal interface materials (TIMs) is an understanding of the squeeze flow process that is commonly used to form thin bond layers in micro-electronic assemblies. A single model TIM, a dense, fairly monomodal suspension of submicron alumina particles suspended in a silicone-based resin, is first characterized as a Bingham fluid using a parallel disk rotational viscometer. Next, the model TIM is squeezed from ∼1 mm initial thickness to ∼.01 mm limiting thickness under nominally constant applied load (68 to 345 kPa) between 20 mm diameter aluminum plates in an axial compression test apparatus (the type commonly used for materials testing). The test plates are flat (∼10 μm flatness deviation over the plate) and smooth (Ra ∼ 20 nm), and are fixed in the test column with epoxy for optimum parallelism. Bond layer thickness is estimated using the LVDT built into the compression tester. The thickness measurement resolution is limited by LVDT noise of 10–20 microns. Squeezing forces are well above the ∼.02 N noise level of the 100 N load cell. Of the test system compliance, inertia, and friction, only the compliance is significant to our testing, and is corrected for. Squeeze flow tests of Newtonian standards are used to qualify the test process. In the case of the model paste, Bingham fluid model parameters from rotational viscometry are used in a lubrication model of squeeze flow that shows good agreement with the measured gap vs. time behavior during squeezing. Improved agreement is obtained by including plate flatness deviation and time-dependent force in the lubrication model. Parallel disk viscometry and squeeze flow testing of the base resin of the model TIM shows Newtonian behavior.

Résumé :
With the development of the next generation of Earth observation and science Space missions, there is an increasing trend towards highly performing payloads. This trend is leading to increased detector resolution and sensitivity, as well as longer integration time which directly drive pointing requirements to higher stability and lower line-of-sight (LOS) jitter [1]. Such instruments typically come with stringent pointing requirements and constraints on attitude and rate stability over an extended frequency range well beyond the attitude control system bandwidth, by entailing micro-vibration mitigation down to the arcsecond (arcsec) level or less [2]. Micro-vibrations are defined in [3] as low-level vibrations causing a distortion of the LOS during on-orbit operations of mobile or vibratory parts. However, in order to guarantee high pointing performance, it is necessary to entirely characterise the transmission path between the micro-vibration source and the payload. The earlier the model is available, the easier it is to meet the stringent pointing requirements, by designing appropriate control strategies. The main difficulties encountered in Space system characterisation are both the impossibility to correctly identify the system on ground due to the presence of gravity and the consideration of all possible system uncertainties. Several uncertainties are in fact determined by: manufacture imperfections of structures and mechanisms, evolution of the system during the mission (i.e. material exposition to Space environment, mass and inertia variation due to ejected ergol), misknowledge of sensors/actuator dynamics. Uncertainty quantification is the preliminary step to be accomplished before designing robust control laws which provide a certificate on the closed-loop system stability and performance. The increased need in pointing performance together with the use of lighter and flexible structures directly come with the need of a robust pointing performance budget from the very beginning of the mission design. An extensive understanding of the system physics and its uncertainties is then necessary in order to push control design to the limits of performance and constrains the choice of the set of sensors and actuators. An analytical methodology to model all flexible elements and mechanisms of a scientific satellite and its optical payload in a multi-body framework is presented. In particular the Two-Input Two-Output Ports approach is used to propose novel models for a reaction wheel assembly including its imbalances and two kinds of actuators to control the line-of-sight: an FSM and a set of PMAs. This approach allows the authors to assemble a complex industrial spacecraft where detailed finite element models can be easily included as well. All these feature are available in the Satellite Dynamics Toolbox Library (SDTlib) [4]. Since in this framework an uncertain Linear Parametric-Varying system can be directly derived by including all possible configurations and uncertainties of the plant, two novel robust active control strategies are proposed to mitigate the propagation of the microvibrations to the LOS error. A first one consists in synthesising an observer of the LOS error by blending the low-frequency measurements of the LOS directly provided by a CCD camera and the accelerations measured in correspondence of the most flexible optical elements (primary and secondary mirrors of a space telescope) together with the accelerations measured on a passive isolator placed at the base of the payload. An FSM then uses this information to mitigate the pointing error. In order to obtain even tighter micro-vibration attenuation, a second stage of active control was proposed as well. This strategy consists in measuring the accelerations of the payload isolator and actuating six PMAs attached to the same isolator. Thanks to this double-stage active control strategy, the propagation of the micro-vibrations induced by the RWs and SADMs is finely reduced on a very large frequency band. In particular, a reduction of the pointing error to 10 arcsec is guaranteed at low frequency approximatively 1 rad/s) with a progressive reduction of the jitter until 40 marcsec for higher frequencies where micro-vibration sources act. This application finally allows the authors to demonstrate the interest of the proposed modelling approach, that is able to finely capture the dynamics of a complex industrial benchmark by including all possible uncertainties in a unique LFT model. This modular framework, which permits to easily build and design a multi-body flexible structure, is in fact conceived in order to perfectly fit with the modern robust control theory. In this way the authors demonstrate how to push the control design to the limits of achievable performance, which is fundamental in the preliminary design phases of systems with very challenging pointing requirements. The present work synthesises the results obtained in an ESA Open Invitations to Tender initiative “Line of Sight Stabilization Techniques (LOSST)” performed together with Thales Alenia Space, Cannes, France (Contract NO. 1520095474 / 02) [5]. Résumé :
As Thermal/Infrared (TIR) sensors have become more advanced, they are being considered for Guidance, Navigation and Control (GNC) systems, in addition to the existing roster of sensors. A crucial step in the development of GNC systems is the testing of sensors, including analysis methods that utilise the sensor data. To expedite this process VIS image generation software like the Planet and Asteroid Natural Generation Utility (PANGU) [1] and Airbus's SurRender [2] are used to generate synthetic images representative of the kind of data that would be gathered. For spacecraft this was previously limited to the VIS spectral range and LiDAR, but as a result of the development in TIR sensors, methods of generating representative synthetic TIR images of spacecraft are now required to aid in development and testing. PANGU v6 added TIR simulations of planetary surfaces and spacecraft in 2021 [3]. This paper will summarize and review problems involved in generating representative TIR images of spacecraft using traditional rendering techniques and proposes a method of real-time image generation to simulate time-based effects. Traditional thermal analysis is performed using Lumped Parameter (LP) and Finite Element Analysis (FEA) techniques that utilize standard heat transfer equations tailored for spacecraft in custom-made software tools like ESATAN-TMS. Modern engineering CAD software like SolidWorks can simulate thermal systems using modern FEA techniques, though few take into account the orbital nature of spacecraft and the resulting changes in thermal behaviour. For this reason, ESATAN's orbital module ensures it stands as one of the few tools available for directly simulating spacecraft thermal behaviour. To generate TIR images, we can adapt techniques from traditional thermal analysis software to more flexible rendering tools like PANGU, which generates representative images using efficient GPU-based rendering techniques over computationally expensive techniques like FEA. A stateless model was developed for PANGU v6.0, using traditional thermal analysis equations to compute TIR radiance in each simulated image [3]. However, there remain some discrepancies in the thermal profile, and images, generated using this technique [3], possibly because it functions without any consideration of changes that occur over more than a single frame. To more accurately simulate the thermal profile of real-world spacecraft, time and its effects on the thermal behaviour of the simulated spacecraft should be considered. While radiative and conductive properties define the ability of a material to transfer thermal energy, the thermal capacitance (a.k.a. inertia) of an object describes the rate at which it can do so. To assist in the approximation of the capacitance of a mesh CAD model, which describes the boundary surfaces of an object rather than defining the solid object itself, we can adapt traditional thermal analysis techniques. One possible method is to create a thermal mathematical model (TMM) [4] limited to the surface thermal properties, that could be used to simulate the approximate flow (conduction) of heat across the surface using node capacitance. The creation of a TMM of the surface of a model requires mesh stitching, i.e. joining together the meshes that make up a CAD model so that a point, which may be described multiple times in order to describe multiple polygons (e.g. the corner of a cube), may be represented as a single node. While not as accurate as describing the conduction of heat through a solid object, the approximation of surface conduction has the potential to increase the accuracy of generated images where conduction plays a significant effect, i.e. due to a rapid change of incoming radiation e.g. entry and exit of eclipse. This paper reports on the development of a new GPU-based renderer that aims to include the thermal behaviour of spacecraft over time in TIR image generation, in order to improve the representative quality of the images. Two techniques are being developed in the renderer: the first is a capacitance model that involves the creation of a TMM and the assignment of thermal properties to each node, to keep track of the temperature of each. The TMM created will be constructed using a mesh "stitching" technique, that creates a geometrically unique set of nodes that more accurately depicts physical properties. The second utilises the TMM to simulate conductive effects over edge nodes, calculating the conduction from one polygon to another. At each frame (i.e. time steps) the temperature and then radiance of each node is calculated using its thermal properties (i.e. emissivity, absorptivity and conductivity) and the previous temperature. The radiance is calculated in the GPU fragment shaders; ensuring that the temperatures passed to the GPU from the vertex shader are properly interpolated, allowing gradients across surfaces with differing node properties. The full paper will examine the quality of the prototype renderering system described above, and examine how valid and representative the generated images are using a traditional thermal simulation tool, ESATAN-TMS. Using this, we can create equivalent CAD models and use them to generate an overall thermal profile of a simple spacecraft in a known orbit. We can then compare this thermal profile to the surface temperature profile generated using the prototype developed, in order to evaluate how representative the images generated are compared to the stateless model currently in use within PANGU.
