ABSTRACT A total of 395 Haemophilus influenzae strains from 226 Japanese institutions participating in the Nationwide Surveillance Study Group for Bacterial Meningitis were received from 1999 to 2002. All strains were analyzed by PCR to identify the resistance genes, and their susceptibilities to β-lactam agents were determined. Of these strains, 29.1% were β-lactamase nonproducing and ampicillin (AMP) susceptible (BLNAS) and lacked all resistance genes; 15.4% were β-lactamase producing and AMP resistant and had the bla TEM-1 gene; 30.6% were β-lactamase nonproducing and AMP resistant (low-BLNAR) and had a Lys-526 or His-517 amino acid substitution in ftsI encoding PBP 3; 13.9% were β-lactamase nonproducing and AMP resistant (BLNAR) and had an additional substitution of Thr-385 in ftsI; 9.1% were amoxicillin-clavulanic acid resistant (BLPACR I) and had the bla TEM-1 gene and a Lys-526 or His-517 amino acid substitution in ftsI; and 1.8% showed resistance similar to that of the BLPACR I group (BLPACR II) but had bla TEM-1 gene and ftsI substitutions, as was the case for the BLNAR strains. All but three strains were serotype b. The prevalence of BLNAR strains has increased rapidly: 0% in 1999, 5.8% in 2000, 14.1% in 2001, and 21.3% in 2002. The MICs at which 90% of BLNAR isolates were inhibited were as follows: AMP, 16 μg/ml; cefotaxime, 1 μg/ml; ceftriaxone, 0.25 μg/ml; and meropenem, 0.5 μg/ml. All of these values were higher than those for the BLNAS counterpart strains. The relatively wide distributions of the β-lactam MICs for BLNAR strains presumably reflect variations in ftsI gene mutations. Pulsed-field gel electrophoresis suggested the rapid spread of specific H. influenzae type b strains throughout Japan. Expedited vaccination, rapid identification, and judicious antibiotic use could slow their spread.

All-solid-state batteries (SSBs) using lithium metal anode are expected to realize SSBs with high energy density. This is because lithium metal anode has the lowest redox potential and the theoretical capacity of 3861 mA h g-1 is ca. ten times larger than that of commonly-used graphite anode (372 mA h g-1). In fact, lithium metal anode has been successfully used in thin-film-type SSBs and stable charge-discharge reactions are reported [1]. In SSBs, both positive and negative electrode materials are combined with solid electrolytes, and they are assembled from the initial state in general. On the other hand, conventional SSBs apply lithium transition metal oxides (LiCoO2, etc.) as the positive electrode material, and lithium metal is electrochemically plated during the charging process. Appling electrochemically-plated lithium as a anode material can simplify the battery manufacturing process and will be an effective way to form pure lithium metal anode. Such “Li-free” SSBs has been successfully prepared in thin-film-type SSBs [2]. Also, this anode-less idea has been applied to develop large-sized SSBs in recent years [3]. In this presentation, the author will talk about the factors for lithium plating morphology on inorganic solid electrolytes, such as current collector species, current densities, temperature, etc. [4]. The model solid electrolytes are LiPON-coated LATP or garnet-structured Ta-doped LLZ. Abstract Since the 1960s the survival of patients with MM had remained rather stable, until the introduction of high-dose melphalan with autologous stem cell transplant (ASCT) in the early 90s which improved the survival of younger patients. In 1999, thalidomide was introduced in the treatment of relapsed/refractory MM, followed by two other novel drugs: bortezomib and lenalidomide. Clinical trials have indicated that novel agent-based treatment are associated with improvement of response and survival in both relapsed/refractory and newly diagnosed patients. A recent report from Mayo Clinic (Kumar et al Blood2008, 111, 1516–20) has indicated that the survival of patients with MM treated at this major referral center, has improved significantly over the last decade. The purpose of our analysis was to confirm this observation in a large number of unselected patients who were treated in several hospitals throughout Greece and to assess the current applicability of ISS, a staging system that has been developed from a large database of patients who were treated with conventional or high-dose therapy before the introduction of novel drugs. Therefore we compared the outcome of two patient cohorts who started treatment before or after the introduction of the first novel drug, thalidomide: since January 1985, 1376 patients were entered in to the database of GMSG: 859 patients started treatment before 31/12/1999 (Group A) and 517 patients after 1/1/2000 (Group B), when thalidomide became available in Greece. Patients in group A were younger (p<0.001), had less often hypercalcemia (p=0.039), less often anemia (p=0.095), lower levels of urine Bence Jones protein (p=0.027) and less bone marrow plasma cell infiltration (p=0.030). One hundred and sixty seven patients (32%) patients in Group B were treated upfront with novel-drug based regimens compared to only 2 (0.2%) in Group A (p<0.0001). At least partial response to first line treatment was significantly higher in Group B compared to Group A (67% vs 56%, p<0.001). Despite more favorable characteristics of patients in Group A, the median overall survival for patients in group A was 36 months and for patients in Group B 48 months (p<0.001). For patients ≤70 years of age, median survival has improved almost two-fold in group B (74 months vs 39 months in group A, p<0.001). For patients >70 years of age, median survival for groups A and B was 26 and 33 months respectively (p=0.27). Early death rates were similar among patients in Group A and B: 8% and 9% (p=0.65) of patients survived less than 3 months in each group respectively. We then validated the applicability of the International Staging System (ISS) in patients of Group B which included patients who could receive either upfront or at a later stage of their disease novel-drug based regimens: 34.5% of patients were rated as ISS stage I, 22.2% as ISS stage II and 43.3% as stage III. The 5-year survival rate was 66% for ISS stage I patients, 45% for ISS stage II patients and 18% for ISS III patients (p<0.001). When only the 169 patients who were actually treated upfront with novel agent-based were analyzed, the 4-year survival rate was 85%, 61% and 26% for ISS stage I, II and III patients respectively (p=0.001). In conclusion, we confirmed that the introduction of novel drugs in the treatment of a large number of unselected patients with MM has significantly improved the outcome of this disease. Our data indicate that the survival benefit was more pronounced in patients ≤70 years of age. The widely available and reproducible ISS is applicable in patients treated with novel agent-based regimens and can be used for the staging of such patients. Despite the improvement of the survival of myeloma patients over the last decade, the outcome of older patients and of those with ISS stage III remains unsatisfactory.

Objective Measurement of IGF-I is important in the management of patients with growth hormone disorders. Here we aim to establish normative data for two new IGF-I assay kits based on a large random sample of the French general adult population. Subjects and methods We measured IGF-I in 911 healthy adults (18–90 years) with two immunoassays (ROCHE Elecsys® and IMMULITE-2000 calibrated against the new IS 02/2547). We compared the data with those of the six immunoassays (iSYS, LIAISON XL, IMMULITE-2000 calibrated against the first IS 87/518, IGF-I RIACT, Mediagnost ELISA, and Mediagnost RIA) that we reported previously. The pairwise concordance among the eight assays was assessed with Bland–Altman plots for both the IGF-1 raw data and the standard deviation scores (SDS), as well as with the percentage of observed agreement and the weighted Kappa coefficient for categorizing IGF-I SDS (ClinicalTrials.gov Identifier: NCT01831648). Results The normative data included the range of values (2.5–97.5 percentiles) given by the two new IGF-I assays according to age group and sex. A formula for the SDS calculation is provided. As for the previous six assays, the lower limits of the reference intervals of the two new assays were similar, but the upper limits varied markedly. The pairwise concordances were only moderate (kappa 0.57). Conclusions Data obtained for these two new IGF-I immunoassays confirm that despite being obtained in the same large healthy population, the reference intervals of the eight commercial IGF-1 assay kits showed noteworthy differences. The agreement among the various methods was moderate to good.

The surfacing of the coronavirus COVID-19 pandemic in the latter part of 2019 drastically forced school systems to restructure and go full-blast with remote learning. Despite the uncertainties, the educational sector must still meet academic ends and so must be resilient in facilitating flexible learning. This shift towards flexible "remote" learning has been predictable and has become, even, the most pragmatic alternative at this time towards providing effective learning delivery systems. To support flexible learning without compromising authenticity and shared identity in the context of natural science virtual teaching and learning, the researchers reviewed and consequently, proposed a recalibration of the instructional systems design (ISD) as used by Natural Science teachers and professors of the Mendiola Consortium from October 2020 through March 2021. The proposed e-ISD, arising from flexibility, authenticity, and result-orientedness as eligibility criteria, enforces the importance of content and context feedback on the instructional process. Applying Argyris' perspective (1976) on feedback loops and theories of action, it could be said that seeking the perspectives of the subject matter experts themselves, the Natural Science teachers, helped the researchers create a new meaning for Science Instruction— a meaning that is flexible and adaptable alongside the changing world.References Almario, A., & Austria, R. The study was aimed to investigate clinical and functional particularities of cardiovascular system in patients with COPD and chronic heart failure (CHF). We examined 1737 patients, of them, 498 with COPD (the 1st group), 721 with COPD and concomitant class I to III stable angina (the 2nd group), and 519 with class I to III stable angina (the 3rd group). All the patients also had CHF. The study included clinical assessment using modified R. Cody's scale, ECG, Holter monitoring, lung function testing, Doppler echocardiography, 6-minute walk test (6MWT). Obstructive disorders were found in 424 of the 2nd group patients (58.8 %) and 268 of the 3rd group patients (51.7 %). Number of patients with class II to IV chronic cor pulmonale was similar in all groups. The class III was determined more often in the 2nd group (36.5 %; р < 0.05), the class I prevailed in the 1st group (17.8 %; р < 0.05). The stage I CHF predominated in the 1st group (31.9 %; р < 0.05), the stages IIА and IIБ were seen mainly in the 2nd group (51.6 % and 26.4 % respectively; р < 0.05). The majority of the 3rd group patients (57.3 %) had the stage IIA CHF. According to most clinical signs, CHF was more severe in the 2nd group. 6MWT was 308.08 ± 6.03 m in the 1st group, 156.4 ± 4.38 m in the 2nd group, and 212.8 ± 5.65 m in the 3rd group (р < 0.05). Low left ventricular (LV) ejection fraction was seen significantly more often in COPD patients with concomitant angina. This group as well as COPD patients had significantly higher mean pulmonary artery pressure (mPAP) (24.7 ± 0.41 mm Hg and 29.9 ± 0.27 mm Hg respectively) compared to 21.7 ± 0.07 mm Hg in the 3rd group patients. Right ventricular (RV) diastolic dysfunction was revealed in 154 patients of the 1st group (83.7 %), 194 patients of the 2nd group (91.9 %) and 72 patients of the 3rd group (43.1 %). In conclusion, all the patients had RV and LV remodeling signs. COPD patients had more prominent RV hypertrophy and dilation, angina patients had enlargement of left heart and moderate decrease in LV contractility. The patients with COPD and angina had the greatest increase in size and wall thickness of both LV and RV and low pump function of LV.

Celem pracy było określenie kierunków zmian w produkcji, konsumpcji oraz handlu zagranicznym mięsem drobiowym na świecie, u największych i średnich producentów i konsumentów, a także eksporterów i importerów. Określenie długoterminowych tendencji jest ważne dla praktyki gospodarczej. Badania obejmowały lata 2001-2019. Do pomiaru tendencji wykorzystano najlepiej dopasowaną do danych empirycznych funkcję trendu. Wykorzystano także indeksy, statystyczne oraz średnie tempo zmian, wskaźniki struktury. Produkcja mięsa drobiowego ogółem rosła przeciętnie w roku w tempie 3,4%. Udział największych producentów, w wytwarzaniu produkcji zmniejszył się z 62,7% w 2001 r. do 53, 9% w 2019 r., a zwiększył średnich i pozostałych. U największych i średnich producentów mięsa drobiowego, za wyjątkiem Meksyku i Japonii wzrost produkcji był większy niż konsumpcji, co spowodowało zwiększenie możliwości eksportowych lub ograniczenie importu. Tempo wzrostu eksportu było szybsze niż produkcji, co powodowało zwiększenie udziału eksportu w zagospodarowaniu produkcji. Import mięsa drobiowego jest bardziej rozproszony niż jego eksport. Udział 3-ch największych eksporterów w handlu światowym w 2001 r. wynosił 58,7%, a w 2019 r. 51,9%, a wskaźniki te dla 3-ch największych importerów wynosiły odpowiednio: 33,8% i 23,6%. W Polsce szybko rosła produkcja (7,4% rocznie), a znacznie wolniej konsumpcja, co skutkowało rozwojem eksportu. W latach 2001-2014 na rynki zagraniczne było kierowane 81% przyrostu produkcji, a w latach 2015-2019 - 99,3%. Rosnącemu eksportowi sprzyjają ceny mięsa. W latach 2014-2019 ceny drobiu na rynkach światowych były wyższe od cen w Polsce o 14,3%, a ceny w UE-27 wyższe od cen w Polsce o 51,7%. Do 2030 r. możliwy jest dalszy wzrost produkcji mięsa drobiowego w Polsce powodowany głównie popytem zagranicznym.

Выполнено сравнение отклика локальных популяций березы повислой (Betula pendula) на факторы среды по двум морфометрическим индексам: флуктуирующей асимметрии (ФА) и ксероморфизма (ИК) листьев. Показано, что изменения значений исследованных индексов практически не зависят от концентрации тяжелых металлов (ТМ) в тканях листа и существенно обусловлены действием экзогенных факторов. Особенно сильна зависимость ФА и ИК от уровня суммарного загрязнения почв ТМ, в частности свинцом и цинком. Биоиндикационная карта г. Саранска и его окрестностей свидетельствует, что 75% исследованной территории принадлежит к третьей категории качества среды – с ощутимым антропогенным прессом на растения. Неблагоприятное воздействие факторов среды заметно снижено в крупных зеленых зонах (~18% территории). Зоны сильного воздействия (четвертая категория качества среды) занимают менее 6% исследованной территории. In rodents and humans there is a sexually dimorphic pattern of GH secretion that influences the serum concentration of IGF-I. Pattern differences can be identified in children, but it is not known how early this difference is established. We studied the plasma concentrations of IGF-I, IGF-II, IGF-binding protein-3 (BP-3), and GH in cord blood taken from the offspring of 1650 singleton Caucasian pregnancies born at term and related these values to birth weight, length, and head circumference. Pregnancies complicated by preterm delivery, antepartum hemorrhage, pregnancy-induced hypertension, preeclampsia, or gestational diabetes and where cigarette smoking continued were excluded, resulting in a cohort of 987. Cord plasma concentrations of IGF-I, IGF-II, and IGFBP-3 were influenced by factors influencing birth size: gestational age at delivery, mode of delivery, maternal height, and parity of the mother. Plasma GH concentrations were inversely related to the plasma concentrations of IGF-I and IGFBP-3; 10.2% of the variability in cord plasma IGF-I concentration and 2.7% for IGFBP-3 was explained by sex of the offspring and parity. None of the factors, apart from maternal height, influenced cord serum IGF-II concentrations (adjusted r2 = 1%). Sex of the baby, mode of delivery, and parity influenced cord serum GH concentrations (adjusted r2 = 2.6%). Birth weight, length, and head circumference measurements were greater in males than females (P &lt; 0.001). Mean cord plasma concentrations of IGF-I (males, 66.4 ± 1.2 μg/liter; females, 74.5 ± 1.3 μg/liter; P &lt; 0.001) and IGFBP-3 (males, 910 ± 13 μg/liter; females 978 ± 13 μg/liter; P &lt; 0.001) were significantly lower in males than females. Cord plasma GH concentrations were higher in males than females (males, 30.0 ± 1.2 mU/liter; females, 26.9 ± 1.1 mU/liter; P = 0.05), but no difference was noted between the sexes for IGF-II (males, 508 ± 6 μg/liter; females, 519 ± 6 μg/liter; P = NS). After adjustment for gestational age, parity, and maternal height, cord plasma concentrations of IGF-I and IGFBP-3 along with sex explained 38.0% of the variability in birth weight, 25.0% in birth length, and 22.7% in head circumference. These data demonstrate that in a group of singleton Caucasian babies born at term, cord plasma IGF-I, IGFBP-3, and GH concentrations relate to birth size, with evidence for sexual dimorphism in the GH-IGF axis.

Up to 1920 processors of a cluster of distributed shared memory machines at the NASA Ames Research Center are being used to simulate ocean circulation globally at horizontal resolutions of 1/4, 1/8, and 1/16-degree with the Massachusetts Institute of Technology General Circulation Model, a finite volume code that can scale to large numbers of processors. The study aims to understand physical processes responsible for skill improvements as resolution is increased and to gain insight into what resolution is sufficient for particular purposes. This paper focuses on the computational aspects of reaching the technical objective of efficiently performing these global eddy-resolving ocean simulations. At 1/16-degree resolution the model grid contains 1.2 billion cells. At this resolution it is possible to simulate approximately one month of ocean dynamics in about 17 hours of wallclock time with a model timestep of two minutes on a cluster of four 512-way NUMA Altix systems. The Altix systems' large main memory and I/O subsystems allow computation and disk storage of rich sets of diagnostics during each integration, supporting the scientific objective to develop a better understanding of global ocean circulation model solution convergence as model resolution is increased.

Abstract Background: Oncofertility counseling in BRCA carriers has unique challenges due to a possible negative impact of germline pathogenic/likely pathogenic variants (PVs) on reproductive potential and the indication to undergo risk-reducing bilateral salpingo-oophorectomy. While previous studies demonstrated the safety of pregnancy in women with prior breast cancer (BC) history, limited evidence exists in BRCA carriers specifically. We report the results of the largest study to date investigating likelihood of pregnancy after BC, reproductive and disease outcomes specifically in BRCA carriers. Methods: This was an international, multicenter, hospital-based, retrospective cohort study including young BRCA carriers with history of BC (NCT03673306). Eligible patients were women harboring germline PVs in BRCA1 and/or BRCA2 and diagnosed with stage I-III invasive BC at the age of ≤ 40 years between January 2000 and December 2020. BRCA healthy carriers or patients with BRCA variants of unknown significance, non-invasive BC, lack of follow-up, prior ovarian cancer or other malignancies without history of BC were excluded. Primary endpoints were pregnancy rate and disease-free survival (DFS). Overall survival (OS), BC specific survival (BCSS), pregnancy, fetal and obstetric outcomes were secondary endpoints. To mitigate the impact of guaranteed time bias, two survival analyses were performed: 1) Extended Cox model with occurrence of pregnancy as a time-varying covariate; 2) Case-control analysis by matching 1:3 patients with and without a pregnancy for type of BRCA PV, hormone receptor status, nodal status and year at diagnosis; moreover, each control had to have a disease-free interval longer than the time occurring between BC diagnosis and conception of the matched case. Results: From 78 centers worldwide, 4,732 patients were included, of whom 659 had at least one pregnancy after BC (pregnancy cohort) and 4,073 did not (non-pregnancy cohort). Patients in the pregnancy cohort were significantly more likely to carry BRCA1 PVs, to be younger at BC diagnosis, to have node-negative and hormone receptor-negative (HR-) BC (all p&lt; 0.01). Pregnancy rate at 10 years was 22% (95% CI 21%-24%) overall, 18% (95% CI 16%-21%) and 26% (95% CI 24%-26%) in patients with HR+ and HR- BC, respectively (p&lt; 0.01). Median time from BC diagnosis to conception was 3.5 years (IQR 2.2-5.3 years), significantly longer in patients with HR+ than HR- BC (4.3 vs 3.2 years, p&lt; 0.01). Among 659 patients with a pregnancy after BC, 45 (6.8%) and 63 (9.6%) experienced an induced abortion or a miscarriage, respectively. Of the 517 (78.5%) patients with completed pregnancy, 54 (10.4%) had twins and 406 (78.5%) delivered at term (delivery ≥37 weeks). Among the 571 babies born, 5 (0.9%) had congenital anomalies. Median follow-up was 7.8 years (IQR, 4.5-12.6 years). No significant difference in DFS was observed between the pregnancy and non-pregnancy cohorts (unadjusted HR 0.97, 95% CI 0.82-1.15, p=0.74; adjusted HR [aHR] 0.99, 95% CI 0.81-1.20, p=0.90). Patients in the pregnancy cohort showed significantly better BCSS (HR 0.53, 95% CI 0.38-0.74, p&lt; 0.01; aHR 0.59, 95% CI 0.41-0.86, p&lt; 0.01) and OS (HR 0.52, 95% CI 0.38-0.72, p&lt; 0.01; aHR 0.58, 95% CI 0.40-0.85, p&lt; 0.01). Similar results were obtained in the case-control analysis. Subgroup analyses of survival endpoints according to patient, tumor and treatment characteristics will be presented at the conference. Conclusions: In this global study, 1 out of 5 young BRCA carriers conceived within 10 years after a BC diagnosis, higher than rates previously reported in unselected young BC populations. Pregnancy following BC in BRCA carriers did not appear to adversely impact maternal prognosis or fetal outcomes. These findings should be incorporated into oncofertility counseling of young BRCA carriers with BC. AbstractIt is known that, for generic $q$, the $\mathcal{H}$-invariant prime ideals in $O_{q}(\mathcal{M}_{m,p}(\mathbb{C}))$ are generated by quantum minors (see S. Launois, Les idéaux premiers invariants de $O_{q}(\mathcal{M}_{m,p}(\mathbb{C}))$, J. Alg., in press). In this paper, $m$ and $p$ being given, we construct an algorithm which computes a generating set of quantum minors for each $\mathcal{H}$-invariant prime ideal in $O_{q}(\mathcal{M}_{m,p}(\mathbb{C}))$. We also describe, in the general case, an explicit generating set of quantum minors for some particular $\mathcal{H}$-invariant prime ideals in $O_{q}(\mathcal{M}_{m,p}(\mathbb{C}))$. In particular, if $(Y_{i,\alpha})_{(i,\alpha)\in[[1,m]]\times[[1,p]]}$ denotes the matrix of the canonical generators of $O_{q}(\mathcal{M}_{m,p}(\mathbb{C}))$, we prove that, if $u\geq3$, the ideal in $O_{q}(\mathcal{M}_{m,p}(\mathbb{C}))$ generated by $Y_{1,p}$ and the $u\times u$ quantum minors is prime. AbstractInterleukin-21 (IL-21) is a type I cytokine that modulates functions of T, B, natural killer (NK), and myeloid cells. The IL-21 receptor (IL-21R) is closely related to the IL-2 receptor β chain and is capable of transducing signals through its dimerization with the common cytokine receptor γ chain (γc), the protein whose expression is defective in humans with X-linked severe combined immunodeficiency. To clarify the molecular basis of IL-21 actions, we investigated the role of tyrosine residues in the IL-21R cytoplasmic domain. Simultaneous mutation of all 6 tyrosines greatly diminished IL-21–mediated proliferation, whereas retention of tyrosine 510 (Y510) allowed full proliferation. Y510 efficiently mediated IL-21–induced phosphorylation of Stat1 and Stat3, but not of Stat5, and CD8+ T cells from Stat1/Stat3 double knock-out mice exhibited decreased proliferation in response to IL-21 + IL-15. In addition, IL-21 weakly induced phosphorylation of Shc and Akt, and consistent with this, specific inhibitors of the MAPK and PI3K pathways inhibited IL-21–mediated proliferation. Collectively, these data indicate the involvement of the Jak-STAT, MAPK, and PI3K pathways in IL-21 signaling.

Abstract Radiotherapy (RT) can be curative in patients with localized follicular lymphoma (FL), with historical series showing a 10-year disease-free survival of 40 to 50%. As 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography with computerized tomography (PET-CT) upstages 10 to 60% of patients compared to CT, we sought to evaluate outcomes in patients staged by PET-CT, to determine if more accurate staging leads to better patient selection and results. We conducted a multicenter retrospective study under the direction of the International Lymphoma Radiation Oncology Group (ILROG). Inclusion criteria were: RT alone for untreated stage I to II FL (grade 1-3A) with dose equivalent ≥24 Gy, staged by PET-CT, age ≥18 years, and follow-up ≥3 months. End points were freedom from progression (FFP), local control, and overall survival (OS). A total of 512 patients treated between 2000 and 2017 at 16 centers were eligible for analysis; median age was 58 years (range, 20-90); 410 patients (80.1%) had stage I disease; median RT dose was 30 Gy (24-52); and median follow-up was 52 months (3.2-174.6). Five-year FFP and OS were 68.9% and 96%. For stage I, FFP was 74.1% vs 49.1% for stage II (P &lt; .0001). Eight patients relapsed in-field (1.6%). Four had marginal recurrences (0.8%) resulting in local control rate of 97.6%. On multivariable analysis, stage II (hazard ratio [HR], 2.11; 95% confidence interval [CI], 1.44-3.10) and BCL2 expression (HR, 1.62; 95% CI, 1.07-2.47) were significantly associated with less favorable FFP. Outcome after RT in PET-CT staged patients appears to be better than in earlier series, particularly in stage I disease, suggesting that the curative potential of RT for truly localized FL has been underestimated.

Abstract Abstract 319 Aim: The Italian Association of Pediatric Haematology and Oncology (AIEOP) patients, diagnosed in the period September 2000-July 2006, were treated in the context of the AIEOP-BFM ALL 2000 Study. Some differences in high risk (HR) treatment and hematopoietic stem cell transplantation (HSCT) indications justify separate reporting of results obtained by AIEOP and BFM respectively. We report here the AIEOP experience. Patients and methods: Overall, 1999 AIEOP Ph negative Acute Lymphoblastic Leukemia (ALL) patients were eligible to the AIEOP-BFM ALL 2000 Study. High Risk (HR) criteria were: t(4;11) translocation, Prednisone Poor Response (PPR), no complete remission (CR) at day 33, high minimal residual disease (MRD) levels (≥10-3) at day 78 (HR-MRD). Treatment consisted of protocol I (patients were randomised to receive either dexametasone or prednisone in induction), 3 HR polychemotherapy blocks, a randomized comparison between delayed intensification based on protocol II repeated twice or protocol III repeated thrice, cranial radiotherapy (CRT), maintenance therapy for a total of 2 years of treatment. Results: 311 patients were classified as being at HR (15.6% of the total ALL population) and had an overall event-free survival (EFS) and Survival of 58.7%(standard error 2.9) and 70.1%(2.7), respectively. For the 204 patients randomized to different steroids in protocol I, we observed a 5-year EFS of 62.7%(5.0) and 62.3%(4.8) and a 5-year Survival of 72.7%(4.7) and 72.8%(4.3) for dexamethasone and prednisone arm, respectively. The 5-year EFS was 44.4%(4.5) in 132 patients at HR for MRD, 36.4%(14.5) for the 11 patients at HR for t(4;11), 41.2%(11.9) for the 17 patients at HR for no CR at day 33, 74.6%(3.7) for the 151 patients at HR only for PPR. Patients at HR with <10-3 (44/258) or negative (82/258) MRD levels at day 78 had a 5-year EFS of 63.4%(7.3) and 79.4%(4.9), respectively. Among 80 patients who underwent allogeneic HSCT at a median time of 6 months from diagnosis, 68 had HR-MRD or t(4;11) or no CR at day 33. After adjusting for waiting time to transplantation, their 5-year EFS was 51.7%(6.6) compared with a 5-year EFS of 44.6%(5.8) in patients with the same features and treated given chemotherapy only (p=0.72). Conclusions: These data show that AIEOP-BFM ALL 2000 HR therapy is very effective for patients defined at HR for PPR. These patients could thus be considered for some treatment reduction, i.e. sparing most toxic therapeutic elements such as CRT. MRD identifies patients at HR (8.7% of the analysed population, 42% of HR patients), who have a poor outcome despite receiving intensive BFM treatment, including HSCT, and who may thus be eligible for innovative therapies. The aim of this study was to evaluate the effect of the thiazide-like diuretic indapamid retard on cardiovascular remodeling processes in patients with arterial hypertension. The study included 20 previously untreated patients (mean age 51,7 ± 2,7 years) with Stages I - П arterial hypertension and a disease history of 2 to 6 years. The duration of indapamid retard therapy was 2 to 6 months. In patients with arterial hypertension, indapamid retard therapy leads to a decrease in left ventricular myocardial weight and to a larger number of more patients with arterial hypertension with the normal geometry of the left ventricle. There was a significant decrease in the rale of pulse wave primarily in the arteries of elastic type. By having a high antihypertensive efficacy, excellent tolerance, metabolic neutrality, indapamid retard produced a significant effect on remodeling processes of the heart, large and middle arteries in patients with arterial hypertension.

Background: Acne Vulgaris is the most common skin disorder of the pilosebaceous unit with excess sebum production, follicular epidermal hyperproliferation, inflammation and Propionibacterium acnes activity, affecting about 80% of teenagers and has considerable psychological and social consequences and physical disability. Use of established topical and oral drugs assumes paramount importance in the treatment of acne vulgaris. Therefore, periodic auditing of prescription is necessary to increase therapeutic benefit and decrease adverse effects. Aim and objectives of the study was to evaluate the pattern of prescription and its rationale in the drug therapy of acne vulgaris. To monitor the adverse effects, if any.Methods: A prospective, hospital based, observational study. Data was collected for a period of 1 year from January 2015 to December 2015 from the outpatient records in the OPD of Dermatology at Justice K.S. Hegde Charitable Hospital, Deralakatte, Mangalore, in a specifically designed proforma.Results: The prescription data of 346 patients were analyzed of which 45.1% were males with an average age of 21.94±0.3 years. Among the four grades of Acne Vulgaris, Grade II (53.17%) was more prevalent followed by Grade I (26.58%), Grade III (13.87%) and Grade IV (6.35%). The number of drugs prescribed for topical use was 514 of which the most commonly prescribed drugs were Benzoyl Peroxide (19.46%), a combination of Tretinoin and Clindamycin (17.12%), Tretinoin alone (12.45%), Clindamycin alone (10.51%) etc. The number of drugs prescribed for systemic use was 98 consisting of Doxycycline (55.1%), Azithromycin (34.7%), Isotretinoin (6.12%) and Erythromycin (4.08%).Conclusions: There was rationality in most of the prescriptions giving no scope for polypharmacy.

L’autorizzazione della commercializzazione, in Italia, del Norlevo - prodotto a base di levonorgestrel - come contraccettivo d’emergenza (Decreto AIC/UAC n. 510/2000 del 26.09.2000) ha sollevato un intenso dibattito sul comportamento del medico che non vuole prescrivere per ragioni deontologiche ed etiche, sostanze che possano ostacolare il proseguimento dello sviluppo del concepito. Ci si chiede, inoltre, se questa decisione possa rientrare nella fattispecie prevista dall’art. 9 della Legge 194/78 in materia di obiezione di coscienza. Per dare una risposta a questo interrogativo, è stato necessario studiare, innanzitutto, il meccanismo di azione del levonorgestrel: solo qualora si fosse riscontrata la possibilità di un effetto abortivo, sarebbe stato possibile appellarsi alla suddetta legge. Poiché si è giunti alla conclusione che, accanto all’unico effetto contraccettivo (inibizione/ritardo dell’ovulazione) dimostrato, sono presenti anche e soprattutto effetti in fase post-fertilizzazione, è stata presa in esame la riflessione giuridica in materia. Alla luce dell’ordinamento italiano e delle decisioni della Corte Costituzionale, il rifiuto di prescrivere/somministrare il Norlevo rientra nelle previsioni dell’art. 9 della Legge 194/78, ma anche qualora questo non venisse riconosciuto, si rende sempre configurabile - a fronte del bene “vita umana” - anche una sorta di “clausola di coscienza” in base alla quale il medico ha diritto ad agire secondo i propri convincimenti interiori.

Objetivo. Determinar teóricamente y empíricamente las teorías que corroboran la relación entre la gestión de investigación, desarrollo y el producto bruto interno del Perú, adecuándola a la estructura peruana liderada por el INIA en función del modelo de Joseph Stiglitz en el marco del sistema nacional de innovación. Métodos. Se realizó un estudio cuantitativo, explicativo y transversal, la muestra representada se obtuvo de las estadísticas de la base de datos de la gestión de investigación y desarrollo, analizadas en las variables del conocimiento científico productivo y producción con valor agregado de un total de 512 publicaciones en SCOPUS de las variables que datan del periodo 2000–2020, la valoración fue según Joseph Stiglitz. Se utilizaron técnicas de análisis documental, extrayendo datos del Banco Mundial, BCRP, SCOPUS y RICYT, los datos fueron analizados con programas E-Views, Stata, Vosviewer y Ucinet; el nivel de significación establecido fue valor de p (Prob) menor a 0.05. Resultados. Los hallazgos evidenciaron que la gestión de investigación y desarrollo (I+D) explica el producto bruto interno del Perú en un 96%. Mientras tanto, la producción con valor agregado explica en un 89% y el conocimiento científico productivo en 67%, lo que indica que son significativos estadísticamente. Conclusión. La gestión del capital humano, investigación, desarrollo (I+D) y procesos de innovación son determinantes en los procesos productivos técnicamente eficientes desde todas las perspectivas. Sin embargo, en el caso peruano tal proceso tiene poca consistencia técnica para penetrar en mercados competitivos por el pobre impacto de la gestión del conocimiento científico productivo.

Abstract Background Studies of hospital discharge data and large observational cohorts show that the incidence of venous thromboembolism (VTE) varies by race. We sought to determine the incidence of VTE in each of the following ethnic groups Caucasians, Africans, Asians, and Hispanics. Methods A systematic literature search strategy was used to identify potential studies in MEDLINE, EMBASE, and CENTRAL using an OVID interface. The methodological quality of eligible studies was assessed according to Newcastle-Ottawa Quality Assessment Scale. The primary outcome measure was to identify the incidence/prevalence of VTE of each ethnic group available for the study in the context of a population based study. Results Out of 3418 potential studies, 19 met our inclusion criteria (Table). Of these, 9 studies were population based studies: 5 reported VTE incidence per 100,000 person-years (PY), 2 measured the standardized incidence ratio, and 2 European studies assessed VTE rate according to country of origin but not ethnicity (data not shown). In addition, 7 studies measured the hospital incidence rate of VTE, and 3 assessed VTE prevalence. Data could not be pooled due to marked heterogeneity including varied periods of study. VTE incidence per 100,000 PY was found to be between 162 and 439 in Caucasians, 143 and 746 in Africans, 3.2 and 16.6 in Asians. Hospital incidence of VTE per 100,000 PY was found to be between 21 and 131 in Caucasians, 22 and 155 in Africans, 2 and 26 in Asians, 33.1 and 71 in American Indians/Alaskan Indians, and 9 in Hispanics. Conclusions Our findings suggest a wide variation in reported incidence rates for VTE among different ethnic groups, even within the same group. In general, studies in Asian populations suggest a lower incidence of VTE. A marked heterogeneity of study designs, population settings prevent drawing firm conclusions. A significant risk of bias cannot be excluded for several studies. Further studies assessing concurrently the risk of VTE among different ethnicities in the same geographical area are needed. The present study examines the effect of animal-source-food (ASF) intake on arm muscle area growth as part of a larger study examining causal links between ASF intake, growth rate, physical activity, cognitive function and micronutrient status in Kenyan schoolchildren. This randomised, controlled feeding intervention study was designed with three isoenergetic feeding interventions of meat, milk, and plain traditional vegetable stew (githeri), and a control group receiving no snack. A total of twelve elementary schools were randomly assigned to interventions, with three schools per group, and two cohorts of 518 and 392 schoolchildren were enrolled 1 year apart. Children in each cohort were given feedings at school and studied for three school terms per year over 2 years, a total of 9 months per year: cohort I from 1998 to 2000 and cohort II from 1999 to 2001. Food intake was assessed by 24 h recall every 1–2 months and biochemical analysis for micronutrient status conducted annually (in cohort I only). Anthropometric measurements included height, weight, triceps skinfold (TSF) and mid-upper-arm circumference (MUAC). Mid-upper-arm muscle area (MAMA) and mid-upper-arm fat area (MAFA) were calculated. The two cohorts were combined for analyses. The meat group showed the steepest rates of gain in MUAC and MAMA over time, and the milk group showed the next largest significant MUAC and MAMA gain compared with the plain githeri and control groups (P< 0·05). The meat group showed the least increase in TSF and MAFA of all groups. These findings have implications for increasing micronutrient intake and lean body mass in primary schoolchildren consuming vegetarian diets.

Abstract The Pliocene sites of the Djurab region (Chad, Africa) have yielded 19 avian remains. They belong to seven different taxa: a cormorant, Phalacrocorax cf. carbo, a darter, Anhinga cf. melanogaster, a heron (Ardeidae), a Ciconiidae of the tribe Leptoptilini, the size of the extant African species of marabou stork or Saddlebill, an extinct marabou stork (or adjutant) of large size – Leptoptilos sp. B, an Anserinae (swan or goose), and a duck (Anatinae). These birds indicate freshwater environments, a slow river or a lake. Also represented in the landscape were open as well as forested areas, like gallery forest. The avifaunas are thus well congruent with other paleoenvironmental results obtained at these sites, particularly regarding the humid character of the region in the Pliocene, and generally of the climate for that period in a part of the Sahel which is now desertic. Introduction The researches led in the Djurab desert (northern Chad, Africa) by the MPFT (Mission Paléoanthropologique francotchadienne) yielded major contributions in paleoanthropology as well as in other fields of paleontology for the African Mio-Pliocene [Brunet et al., 1995, 1996, 1997, 2002]. Some bird remains were found between 1996 and 2000 in Koro Toro (KT) and Kossom Bougoudi (KB) areas. These are described here, and some interpretations are given regarding paleoenvironment and paleobiogeography. Geological and biochronological context KT and KB are both situated about 700 km N-E of N’Djamena and Lake Chad, and are dated biochronologically at respectively 3–3.5 Ma and about 5 Ma [Brunet et al., 1995, 1997 ; Brunet and MPFT, 2000]. They belong to the « paleo-Lake Chad » system, and the fossils were mainly found in fluviatile sandstones [Duringer et al., 2000]. Systematic paleontology Pelecaniformes – Phalacrocoracidae –Phalacrocorax cf. carbo Distal left ulna, KB24-97-019 (pl. I, fig. 1). Morphologically this fragment resembles the genus Phalacrocorax, and its size indicates the species P. carbo, the only living cormorant of this size in Africa and most of Eurasia. None of the extinct cormorants or shags described from Africa reaches the size of P. carbo. Anhingidae – Anhinga cf. melanogaster Distal left carpometacarpus, KT12-00-002 (pl. I, fig. 2), proximal left carpometacarpus, KT12-00-003 (pl. I, fig. 3), distal right ulna, KT13-96-389, distal left ulna, KT13-96-534 (pl. I, fig. 4), and four bones found together and almost certainly belonging to a single individual, all labelled KB3-98-099: a fragment of a right coracoid, a fragment of a left scapula, a right distal humerus (pl. I, fig. 5), and a left distal carpometacarpus. Morphologically all of them match the genus Anhinga, and the distal ulnae are especially close to the living Old World species A. melanogaster. They also correspond in size to this species. All extinct species of anhingas are morphologically different, and/or larger in size than A. melanogaster and the Chadian fossils. Family incertae sedis Ardeidae Ardeidae indet. A fragment of coracoid, KB24-97-021, indicates the presence of a heron, but no diagnostic feature is preserved that could allow a more precise identification. Ciconiiformes – Ciconiidae –Leptoptilini indet. – sp. A A fragmentary distal part of a right tibiotarsus, KB3-97-315 (pl. I, fig. 6) belongs to either Leptoptilos or Ephippiorhynchus, and in size is similar to the same element of the African L. crumeniferus or E. senegalensis. Leptoptilos sp. B This species is represented by an incomplete left tibiotarsus, KB3-97-161 (pl. I, fig. 7). The robust proportions indicate the genus Leptoptilos, as opposed to Ephippiorhynchus, and the specimen is larger than any of the living species at genus. Other material from the Pliocene of Ethiopia allows a more precise identification as an extinct species that is also represented in Asia (Louchart et al. in prep.). cf. Leptoptilos sp. B Two parts of a left carpometacarpus, the proximal, KT13-96-504 (pl. I, fig. 8), and the distal end, KT13-98-004 (pl. I, fig. 8), probably belong to the same species as the KB3-97-161 tibiotarsus, but the incompleteness of these remains does not allow exclusion of the genus Ephippiorhynchus. Leptoptilos sp. indet. An incomplete juvenile right tibiotarsus, KT 13.00.012 (pl. I, fig. 9), belongs to the genus Leptoptilos, as indicated by its relatively robust proportions. However, the juvenile condition and the fragmentary preservation prevent more precise identification. Anseriformes – Anatidae – Anserinae A proximal left humerus, KT13-96-518 (pl. I, fig. 10), belongs to a fairly large species of the Anserinae. Anatinae indet. A fragmentary distal left ulna, KB24-97-020 (pl. I, fig. 11), belongs to a duck, about the size of the Gadwall, Anas strepera or the Pintail, A. acuta. However, the bone is too incomplete to allow even a generic identification. Paleoenvironment and paleobiogeography Except Leptoptilos, all the birds of KT and KB (tabl. I) are strictly aquatic. The best represented, the darter Anhinga cf. melanogaster, preferentially inhabits freshwater slow streams or lakes and eats fish. The cormorant Phalacrocorax cf. carbo indicates open fresh or marine water and feeds mainly on fish in shallow water. The heron indicates a humid environment. The Leptoptilini indet. – sp. A – of KB may indicate dry savanna or swamps. Leptoptilos sp. B may also indicate open country. The juvenile Leptoptilos sp. indet. of KT indicates breeding nearby, probably in trees as for the genus today. Duck or goose-like taxa also most likely required aquatic environments. Today the spatially closest populations of Phalacrocorax carbo and Anhinga melanogaster are found around Lake Chad [Del Hoyo et al., 1992]. These differences of distributions from today are due to the moistness of the area in the Pliocene. Studies of other groups (especially mammals, reptiles and fish) and the sedimentology indicate for KB and KT mixed environments of gallery-forest, savanna with open herbaceous areas, and areas of temporary streams occasionally becoming lakes [Brunet et al., 1995, 1997 ; Brunet & MPFT, 2000 ; Duringer et al., 2000]. The aquatic element of this environment is representative of the paleo-Lake Chad system, which was considerably more extensive than the Lake Chad of today [Duringer et al., 2000]. The avifauna is therefore in accordance with previous studies, indicating the moistness of the Pliocene climate in this part of Sahel, which is desert today. Conclusion The birds of the Djurab Pliocene are essentially aquatic. They belong to modern genera, and two or three are indistinguishable from extant species, whereas one is extinct: a Leptoptilos marabou stork. Their ecological requirements indicate a humid environment, lake or slow stream. For Koro Toro, the presence of trees is suggested, beside dry or humid open areas. All of this is in accordance with other results. The birds are especially indicative of the humid regional climate which allowed the considerable extension of paleo-Lake Chad at that period.

Abstract. We report here the results of three experiments, which are slight variations of the 15N method (JGOFS protocol) for determination of new production. The first two test the effect of (i) duration of incubation time and (ii) concentration of tracer added on the uptake rates of various N-species (nitrate, ammonium and urea) by marine phytoplankton; while the third compares in situ and deck incubations from dawn to dusk. Results indicate that nitrate uptake can be underestimated by experiments where incubation times shorter than 4h or when more than 10% of the ambient concentration of nitrate is added prior to incubation. The f-ratio increases from 0.28 to 0.42 when the incubation time increases from two to four hours. This may be due to the observed increase in the uptake rate of nitrate and decrease in the urea uptake rate. Unlike ammonium [y{=}2.07x{-}0.002\\, (r2=0.55)] and urea uptakes [y{=}1.88x{+}0.004 (r2=0.88)], the nitrate uptake decreases as the concentration of the substrate (x) increases, showing a negative correlation [y{=}-0.76x+0.05 (r2=0.86)], possibly due to production of glutamine, which might suppress nitrate uptake. This leads to decline in the f-ratio from 0.47 to 0.10, when concentration of tracer varies from 0.01 to 0.04μ M. The column integrated total productions are 519 mg C m-2 d-1 and 251 mg C m-2 d-1 for in situ and deck incubations, respectively. The 14C based production at the same location is ~200 mg C m-2 d-1, which is in closer agreement to the 15N based total production measured by deck incubation.
