Texto completoThis dissertation provides an analysis and evaluation of factors influencing states to enact inpatient health care transparency laws between 1971 and 2006 inclusive, using event history analysis. The primary research question investigates "What factors influence a state legislature to enact a health care transparency law?" To narrow the scope of study, I focus on factors influencing states to enact health care transparency laws to collect and publicly report inpatient data.
The Unified Model of State Policy Innovation, developed by F.S. Berry and W.D. Berry (1990, 1999), provides the framework for the study hypotheses and the analysis of inpatient health care transparency law enactments by states. The Unified Model of State Policy Innovation posits a unified explanation for state policy adoptions. The model unifies the internal determinants and regional diffusion approaches of analysis for state policy adoption.
This study tests eight hypotheses using event history analysis (EHA). EHA is an analytical technique that allows for the testing of a state government innovation theory that incorporates internal determinants and regional influences on state policy adoption. Although there are numerous methods to conduct event history analysis, this study uses the Cox proportional hazards model (also known as Cox regression). Cox regression is a popular method for studying time-to-event data for policy adoption and diffusion studies. This study's quantitative analysis provides support for legislative ideology and unified party control of state government acting as factors influencing inpatient health care transparency law enactments by states. Additionally, the health care crisis and neighbors variables were statistically significant, but in an opposite direction than predicted.
The findings of this research suggest that state adopters of an inpatient health care transparency law are more likely to enact an inpatient health care transparency law when the state government is increasing in liberalism and when unified political party control of the governor and the governorship of both houses of the state legislature is increasing.
To generate new insights into the enactment of inpatient health care transparency laws, I conduct a case study of a national health care data professional association using several techniques, including telephone interviews. The qualitative analysis provides support for professional associations and policy champions as diffusion agents for inpatient health care transparency law enactments by states.
This dissertation supports variables traditionally used in policy adoption research including legislative ideology and unified political party control in state government. However, it will be interesting to see whether internal determinants such as professional associations gain traction over the traditional regional diffusion influences such as states sharing borders as factors influencing state policy adoption. Meanwhile, as evidenced in this study, there continues to be support for a model incorporating both internal and regional influences to explain policy adoption by states. The theory of policy innovation and diffusion to predict the factors influencing the spread of policies and the use of Berry & Berry's (1990, 1999) Unified Model of State Policy Innovation prosper as their applicability to numerous public policy areas, including health care, are continually demonstrated. Similarly, event history analysis and specifically the Cox regression method continue to gain support as their value as analytical methods and appropriateness for use in public policy studies is repeatedly demonstrated.
The outlook for the future of the health care transparency movement looks promising. The health care transparency movement promotes improved access to information, patient empowerment, improved patient safety and quality of care, improved provider accountability, and lower health care costs. This movement is not a fad, but rather a permanent change being implemented in all health care settings across the United States. Improved health through reliable, accessible data and data-supported decisions is increasingly becoming the norm and less an idealistic scenario to be realized in the distant future.

Texto completoThe amending of the Volstead Act and repeal of national prohibition did not answer the "liquor question" but passed the issue to the states. This thesis examines New York's reaction to the change in national alcohol policy and the states decision to legalize and regulate the beverage with the establishment of the New York State Liquor Authority. It traces the activities of Jean Sheppard who led the state division of the Women's Organization for National Prohibition Reform (WONPR) and became one of the key architects of New York's modern alcohol control system. As an expert in alcohol control policies Sheppard developed a plan she believed would be respected by the public while also mitigating the problems associated with alcohol. Sheppard proposed an elaborate system of control which made temperance the objective. Through her position as Chairman of the New York State WONPR Sheppard gained the attention of Governor Herbert H. Lehman who nominated her to the New York State (Conway) Commission on Alcoholic Beverage Control Legislation. As a member of the Commission and then the New York State Alcohol Beverage Control Board, Sheppard was given the opportunity to propose her theories on control. The final legislation creating the New York State Liquor Authority embodied Sheppard's plan in regards to administrative structure but fell well short of her dream of a system that used the full power of the state to put temperance ahead of revenue.

Texto completoBeginning with the Celtic Tiger years, Ireland’s remarkable economic growth and the inequalities existing in its wake, this PhD tries to re-assess the issue of contemporary inequalities in modern societies emphasising the Irish case from 1922 to 2002. To what extent did inequalities exist in Ireland prior to the advent of the Celtic Tiger? What were Irish attitudes to inequalities and how have they evolved? Do Irish people care about equality? Based on an economic, social, historical and political analysis resting on recent comparative studies of the development of welfare state systems and the varying extents of their redistributive agendas as well as on reflections on inequalities and fairness in our societies, this thesis aims to show that current inequalities in Ireland can best be understood in the light of an inability to create a more just society from 1922 onwards. The author argues that three particular sets of factors, demographic and economic factors, political factors, and Irish mentalités have, in close interaction with one another, provided a strong framework which has prevented the advent of a more just society between 1922 and 2002

Resumo: Introdução: Pesquisas gerontológicas têm ressaltado a importância da reconstrução de memórias de pessoas mais velhas para favorecimento de reflexões acerca do sentido de vida e do estabelecimento de novas metas e formas de enfrentamento. Objetivos: Reconstruir a memória social de grupos comunitários de bairros populares que se envolveram ativamente nos processos de criação e manutenção de grupos comunitários como Mulheres da Periferia, Grupo Reviver, Cantinho da Nossa Senhora de Guadalupe e Giravida. Procuramos identificar a) os significados atribuídos pelos moradores à participação nos movimentos sociais; b) fatores que influenciaram no envolvimento inicial dos participantes e sua permanência nos grupos; c) a relação entre a sua participação social atual e passada e as características de sua rede de suporte social e d) a relação entre geratividade e a participação em grupos comunitários dos segmentos meia-idade, velhice e velhice avançada. Procedimentos: A pesquisa utilizou-se da metodologia da História Oral. Os depoimentos foram coletados através de entrevistas e um Inventário de Geratividade (Neri, 1998), em um grupo de 14 mulheres todas praticantes do catolicismo, e membros e/ou organizadoras grupos comunitários. Análise de dados: As histórias e outras informações foram analisadas dentro do paradigma fenomenológico, com vistas a favorecer a compreensão do envelhecimento através das informações sobre as trajetórias de vida dos atores sociais que fizeram - e ainda fazem - a história da região estudada. Resultados: Atitudes pessoais como religiosidade e envolvimento com novos projetos apareceram como favoráveis para a formações de valores de solidariedade e geratividade, ambos associados à dedicação ao outro. Esses fatores tem forte relação com melhoria da qualidade de vida e implementação de uma visão mais positiva do envelhecimento. A reconstrução da memória, a partir dos depoimentos orais, favoreceu o desenvolvimento da consciência das depoentes sobre sua função social pelo fato de serem detentoras de informações históricas que, uma vez divulgadas na comunidade, pode exercer importante papel agregador e educacional; além disso, descobriram-se como marcos da trajetória do engajamento feminino na região. Conclusões: O envolvimento em grupos comunitários foi favorável para o bem envelhecer das depoentes devido à ampliação na rede de suporte social através de redes solidárias, visão positiva sobre o envelhecimento e desejo de perpetuação dos grupos. A História Oral constituiu uma importante ferramenta, que permitiu reviver informações úteis a compressão das relações sociais; por outro lado, favoreceu aos idosos, que participaram como informantes, uma reconstrução de suas histórias, melhoria na auto ¿estima e valorização social
Abstract: Introduction: Researches in Gerontology have highlighted the importance of rebuilding the memories of older people to aid the awareness on the sense of life and the establishment of new goals and ways of coping. Objectives: To rebuild the social memory of community groups of popular suburbs which involved actively in the processes of creation and maintenance of community groups as Mulheres da Periferia, Grupo Reviver, Cantinho da Nossa Senhora de Guadalupe and Giravida. We tried to identify a) meanings to the participation in social movement assigned by the residents; b) factors which influenced the initial involvement and maintenance of participants in groups; c) the relation between the current and past social participation and the features of the social network support and, d) the relation between the generativity and the participation in community groups of middle-age, aging and advanced aging. Procedures: The research used the methodology of Oral History. The testimonies were collected through interviews and an inventory of Generativity (Neri, 1998), in a group of 14 women all catholic practicioners, and members and/or organizers of community groups. Data Analysis: The stories and other information were analysed according to phenomenological paradigm, aiming at aiding the understanding of aging through the information about the ways of life of social players who made - and still make ¿ the story of the region studied. Results: The themes related to personal attitudes, as religiosity and involvement in new projects, appear as favorable in the formation of solidarity values and generativity which both are associated to the dedication to the other. Those factors seemed to have strong relation with improvement of life quality and implementation of a more positive view of aging. The rebuilding of memory, from the oral testimonies, will have aided the development of the consciousness of the tellers by having a social function for the fact of having historical information, which once difused, the community can have an important aggregating and educational role and besides this, being markers of the way of female engagement in the region. Conclusions: The involvement in community groups was favorable for the well aging of the tellers due to the enlargement of the social network support through the solidarity networks, positive view about aging and will of perpetuation of the groups. Oral History is an important tool which allows, on one hand, to revive useful information for the understanding of social relations. On the other hand, it aids the elderly who participated as tellers, a rebuilding of their stories, improvement of self-steem and social valorization

Texto completoThe general aim of this dissertation is to describe and analyse how public old-age care in Sweden has developed and changed during the last century. The study applies a provider perspective on how care has been planned and professionally carried out. A broader social policy perspective, studying old-age care at central/national as well as local/municipal level, is also developed. A special focus is directed at the large local variation in care and services for the elderly. The empirical base is comprised of official documents and other public sources, survey data from interviews with elderly recipients of public old-age care, and official statistics on publicly financed and controlled old-age care and services.
Study I addresses the development of old-age care in Sweden during the twentieth century by studying an important occupation in this field – the supervisors and their professional roles, tasks and working conditions. Throughout, the roles of supervisors have followed the prevailing official policy on the proper way to provide care for elderly people in Sweden; from poor relief at the beginning of the 1900s, via a generous level of services in the 1960s and 1970s, to today’s restricted and economy-controlled mode of operation.
Study II describes and compares two main forms of public old-age care in Sweden today, home help services and institutional care. The care-load found in home-based care was comparable to and sometimes even larger than in service-homes and other institutions, indicating that large care needs among elderly people in Sweden today can be met in their homes as well as in institutional settings.
Studies III and IV analyse the local variation in public old-age care in Sweden. During the last decades there has been an overall decline in home help services. The coverage of home help for elderly people shows large differences between municipalities throughout this period, and the relative variation has increased. The local disparity seems to depend more on historical factors, e.g., previous coverage rates, than on the present municipal situation in levels of need or local economy and politics.
In an introductory part the four papers are linked together by an outline of the demographic situation and the social policy model for old-age care in Sweden. Trends that have been apparent over time, e.g. professionalisation and market orientation, are traced and discussed. Conflicts between prevailing ideologies are analysed, in regards to for instance home-based and institution-based care, social and medical culture, and local and central levels of decision-making. ’Welfare municipality’, ‘path dependency’, and ‘decentralisation’ are suggested as a conceptual framework for describing the large and increasing local variations in old-age care. Finally, implications of the four studies with regard to old-age care policy and further research are discussed.

Texto completoAmong the new technologies heralded as part of the emerging bioeconomy are plastics made from plant material, known as bioplastics. This dissertation examines the political and historical underpinnings of the bioplastics that are now being offered as an alternative to petrochemicals in the United States. As a case study of "green" technological development, bioplastics challenge dominant conceptions of innovation for sustainability. The bioplastics being developed and marketed today are the outcome of interventions in commodity crop prices, incubation of research on biomass during periods of fossil fuel dominance, and the commercialization of publicly funded research. Their origins can be traced at least as far back as the 1920s, when advocates of "chemurgy" encouraged the federal government to create research centers to discover new industrial uses of agricultural crops.
Research in science and technology studies (STS) indicates that social structures shape perceptions of problems, condition viable solutions, and limit the diversity of stakeholders and ideas present in the social construction of technology. This study examines these processes in the history and current debates about bioplastics. The dissertation asks who has influenced the social construction of bioplastics and why bioplastics have become part of a larger bioeconomic vision now. Theoretical insights are drawn from the sociological theory of the treadmill of production, which argues that environmental problems cannot be solved in a capitalist system in which the federal government, private industries, and organized labor continuously seek the expansion of production and consumption at the expense of environmental sustainability. Major players in the chemical and biotechnology industries have pursued bioplastics as a means of continuing economic growth and consumption of goods, even as petroleum becomes costly and environmentalists voice objections to petrochemicals. There are many critiques of bioplastics and their impacts at every stage of bioplastics, from sourcing feedstocks from food crops to disrupting existing recycling and composting systems. Nevertheless, the bioplastics currently on the market were not designed to resolve these environmental concerns. Increasingly, however, activists are using non-governmental institutions, particularly the development of voluntary standards, to shape the industry and technology. The study examines the extent to which such reforms might lead to the production of more sustainable alternatives to petrochemicals.
Ultimately, this dissertation presents the history and politics surrounding the field of bioplastics in order to highlight how things "might have been otherwise" and what changes in society could be useful for producing more sustainable technologies.

Texto completoThis dissertation focusses on the culture of visual display during the 1930s and 1940s, more specifically photo contests and photo books based on photographs of children. The overall purpose of this investigation has been to study how photo contests and photo books have been a part of the negotiation of the organization of welfare for children and the family. It examines how the family, the welfare state and the nation were constructed by means of representations of children, the messages that were thus established, and the political visions that were communicated. Analyses of images and texts show that photo contests and photo books presented a unified message which said that more and better children were needed, but also how photo contests and photo books communicated disparate visions of how this goal should be reached. The study highlights how useful representations of children were in the construction of welfare. They had the potential to communicate both different visions and a shared vision of the welfare society.
