Measurements were made of the ability of 20 subjects to acquire 3, 6 or 9 stationary visual targets and then replace them after they had been removed. The targets were viewed with various sized fields-of-view (FOVs) using a Visually Coupled Airborne Systems Simulator (VCASS), which was mounted on the head and which used head position information to provide space-stabilized computer-generated images. Targets were presented with a blank background or a terrain background. Subjects were instructed to use as much time as they required or to be as quick as possible searching for the targets. Mean times to search for the targets were faster with the larger FOVs and faster with fewer targets. Replacement accuracy was not sensitive to the FOV but decreased with increasing number of targets. Search times were slower, but replacement accuracy was greater with a terrain background than with a blank background. In the fast search conditions, the number of guessed target replacements decreased with decreasing numbers of targets and increasing FOV. It is concluded that target replacement performance was sensitive to manipulation of the independent variables and as such is a potentially useful metric of spatial awareness.

Abstract OBJECTIVES We evaluated and compared midterm recurrence results of our patients with great saphenous vein insufficiency who were treated with a 1470-nm diode laser using 2 different types of fibre catheter kits. METHODS A total of 61 consecutive patients were treated between 2013 and 2014 with a bare fibre (BF) tip (BF group) and 60 consecutive patients were treated with a radial fibre (RF) tip (RF group) from 2014 to 2016. First-year venous clinical severity scores (VCSSs) were compared with VCSS before endovenous laser ablation and at the first-month follow-up. Patients were examined for recurrence and classified according to the system developed by Stonebridge. RESULTS There was no significant difference between the 2 groups in terms of VCSS. Examination with Doppler ultrasonography showed no recurrence in the RF group, whereas recurrences were detected in 6 patients in the BF group, which was statistically significant (P = 0.028). All of the recurrences were type 1b (incompetent tributaries) varicose vein recurrences. The VCSS of the patients with recurrence were the same as the scores of patients without recurrence (0.5 ± 0.55). CONCLUSIONS Varicose vein recurrence was more often seen in the BF group than in the RF group. Recanalization-induced and neovascularization-induced recurrences were not found in either group. Saphenofemoral side branch-induced recurrence was more significant in the group treated with the BF tip.

Introduction: In 2010, the American Venous Forum developed a Venous Clinical Severity Score (VCSS) scoring system to assess the severity of Chronic Venous Insufficiency (CVI), where this system was said to be more comprehensive than the CEAP system. However, VCSS validation was still lacking. The purpose of this study was to examine VCSS for reflux and leg vein diameter based on ultrasonography. Method: This study was a cross-sectional diagnostic test on women workers with standing work positions of 114 people (228 limbs). VCSS assessment and ultrasound examinations were carried out on all subjects. The relationship between VCSS with reflux and leg vein diameter was analyzed using an odds ratio with a 95% confidence interval. The relationship between VCSS with reflux and leg vein diameter was analyzed using an odds ratio with a 95% confidence interval. Results: From 228 examined limbs, the VCSS score of 0-3 was 18.4%, and the score of ≥4 was 81.6%. Reflux was found in 21.9% of the limbs. There was a significant relationship between VCSS and reflux in leg veins. The diameter of the great saphenous vein was between 2.1-12.2 mm, the femoral vein was 7.1-17 mm, the popliteal vein as 3-11.4, and the small saphenous vein was 1.7-7mm. When VCSS was analyzed for association with venous diameter, a significant relationship was found. VCSS sensitivity compared with reflux based on USG was 78%, specificity was 98.31%, positive predictive value was 92.86%, and the negative predictive value was 93.86%. Conclusion: From the results of this study, it was concluded that the VCSS score could be used as a method for assessing chronic venous insufficiency. Although VCSS is was to assess the severity of the chronic venous disease, VCSS can also be used for screening because it shows a good relationship with the reflux and venous diameter of the limbs based on ultrasound. Objective To evaluate the results of radiofrequency ablation (RFA) of the great saphenous vein (GSV) using one versus two 20 s energy cycle treatment in the proximal 7 cm segment of the GSV. Methods All patients who underwent RFA of the GSV from 1 May 2013 to 30 September 2013 in eight of our vein centers were included. Duplex ultrasound scans (DUSs) were performed prior to treatment on all patients and 2–3 days, and 1 month after procedure. Demographic data, GSV diameters, and other relevant data were recorded. Clinical, Etiologic, Anatomic, Pathologic (CEAP) classification and Venous Clinical Severity Scores (VCSSs) were determined prior to ablation and one month later. Patients who developed endovenous heat induced thrombosis (EHIT) were followed till resolution. Results A total of 205 patients had one cycle treatment (group A) and 204 had two cycle treatment (group B). The two groups were comparable in their demography, CEAP classification, and VCSS scores. The rate of failure of ablation and incidence of EHIT were also not significantly different. The incidence of complications was low, <5% in both groups and all were minor. Conclusion Two cycle treatment of the proximal GSV for vein ablation does not improve the success rate of vein closure in the short term, compared to one cycle treatment. It also does not increase the risks of DVT, EHIT, major bleeding, and other complications. However, we do not know at what diameter two cycles may be superior to one cycle.

This paper examines the petroleum industry's willingness to participate in conservation agreements for the lesser prairie chicken, a candidate for listing under the Endangered Species Act (ESA). Voluntary conservation agreements with assurances (VCAAs) can incentivize habitat conservation and sustain economic development. Using data on oil and natural gas wells in Kansas and Oklahoma, I develop a discrete choice model to examine company preferences for locating wells and participating in VCAAs for the lesser prairie chicken. Participation in VCAAs is low, but I find participating wells are concentrated in areas with the most crucial habitat.

Antibacterial viscose cellulose sponges (VCSs) were fabricated by heating cellulose xanthogenate (viscose) containing HAuCl4·nH2O. Viscose was used as the reducing agent and stabilizer for the in situ synthesis of Au nanoparticles (AuNPs) onto the VCSs. The morphology, structures, thermal properties, mechanical performance, and antibacterial activities of the sponges were investigated. Results indicate that AuNPs were uniformly immobilized in the VCSs, and the resulting complexes (AuNPs@VCSs) showed enhanced thermal stability and mechanical properties. Additionally, the AuNPs@VCSs exhibited remarkable antibacterial activities, with zone of inhibition diameter of 35.7 and 37.1 mm for Staphylococcus aureus and Escherichia coli, respectively. The process is simple and applicable at the industrial level and can be applied to the fields of cleaning and sanitation.

Spheroid cultures are among the most explored cellular biomaterials used in cardiovascular research, due to their improved integration of biochemical and physiological features of the heart in a defined architectural three-dimensional microenvironment when compared to monolayer cultures. To further explore the potential use of spheroid cultures for research, we engineered a novel in vitro model of the heart with vascularized cardiac spheroids (VCSs), by coculturing cardiac myocytes, endothelial cells, and fibroblasts isolated from dissociated rat neonatal hearts (aged 1-3 days) in hanging drop cultures. To evaluate the validity of VCSs in recapitulating pathophysiological processes typical of the in vivo heart, such as cardiac fibrosis, we then treated VCSs with transforming growth factor beta 1 (TGFβ1), a known profibrotic agent. Our mRNA analysis demonstrated that TGFβ1-treated VCSs present elevated levels of expression of connective tissue growth factor, fibronectin, and TGFβ1 when compared to control cultures. We demonstrated a dramatic increase in collagen deposition following TGFβ1 treatment in VCSs in the PicroSirius Red-stained sections. Doxorubicin, a renowned cardiotoxic and profibrotic agent, triggered apoptosis and disrupted vascular networks in VCSs. Taken together, our findings demonstrate that VCSs are a valid model for the study of the mechanisms involved in cardiac fibrosis, with the potential to be used to investigate novel mechanisms and therapeutics for treating and preventing cardiac fibrosis in vitro.

Background Chronic and mental health conditions are increasingly prevalent worldwide. As devices in our everyday lives offer more and more voice-based self-service, voice-based conversational agents (VCAs) have the potential to support the prevention and management of these conditions in a scalable manner. However, evidence on VCAs dedicated to the prevention and management of chronic and mental health conditions is unclear. Objective This study provides a better understanding of the current methods used in the evaluation of health interventions for the prevention and management of chronic and mental health conditions delivered through VCAs. Methods We conducted a systematic literature review using PubMed MEDLINE, Embase, PsycINFO, Scopus, and Web of Science databases. We included primary research involving the prevention or management of chronic or mental health conditions through a VCA and reporting an empirical evaluation of the system either in terms of system accuracy, technology acceptance, or both. A total of 2 independent reviewers conducted the screening and data extraction, and agreement between them was measured using Cohen kappa. A narrative approach was used to synthesize the selected records. Results Of 7170 prescreened papers, 12 met the inclusion criteria. All studies were nonexperimental. The VCAs provided behavioral support (n=5), health monitoring services (n=3), or both (n=4). The interventions were delivered via smartphones (n=5), tablets (n=2), or smart speakers (n=3). In 2 cases, no device was specified. A total of 3 VCAs targeted cancer, whereas 2 VCAs targeted diabetes and heart failure. The other VCAs targeted hearing impairment, asthma, Parkinson disease, dementia, autism, intellectual disability, and depression. The majority of the studies (n=7) assessed technology acceptance, but only few studies (n=3) used validated instruments. Half of the studies (n=6) reported either performance measures on speech recognition or on the ability of VCAs to respond to health-related queries. Only a minority of the studies (n=2) reported behavioral measures or a measure of attitudes toward intervention-targeted health behavior. Moreover, only a minority of studies (n=4) reported controlling for participants’ previous experience with technology. Finally, risk bias varied markedly. Conclusions The heterogeneity in the methods, the limited number of studies identified, and the high risk of bias show that research on VCAs for chronic and mental health conditions is still in its infancy. Although the results of system accuracy and technology acceptance are encouraging, there is still a need to establish more conclusive evidence on the efficacy of VCAs for the prevention and management of chronic and mental health conditions, both in absolute terms and in comparison with standard health care.

Controlling the time point and site of the release of active ingredients within the gastrointestinal tract after administration of oral delivery systems is still a challenge. In this study, the effect of the combination of small capsules (size 3) and large capsules (size 00) on the disintegration site and time was investigated using magnetic resonance imaging (MRI) in combination with a salivary tracer technique. As capsule shells, Vcaps® HPMC capsules, Vcaps® Plus HPMC capsules, gelatin and DRcaps® designed release capsules were used. The three HPMC-based capsules (Vcaps®, Vcaps® Plus and DRcaps® capsules) were tested as single capsules; furthermore, seven DUOCAP® capsule-in-capsule combinations were tested in a 10-way crossover open-label study in six healthy volunteers. The capsules contained iron oxide and hibiscus tea powder as tracers for visualization in MRI, and two different caffeine species (natural caffeine and 13C3) to follow caffeine release and absorption as measured by salivary levels. Results showed that the timing and location of disintegration in the gastrointestinal tract can be measured and differed when using different combinations of capsule shells. Increased variability among the six subjects was observed in most of the capsule combinations. The lowest variability in gastrointestinal localization of disintegration was observed for the DUOCAP® capsule-in-capsule configuration using a DRcaps® designed release capsule within a DRcaps® designed release outer capsule. In this combination, the inner DRcaps® designed release capsule always opened reliably after reaching the ileum. Thus, this combination enables targeted delivery to the distal small intestine. Among the single capsules tested, Vcaps® Plus HPMC capsules showed the fastest and most consistent disintegration.

Summary Background Great saphenous vein (GSV) incompetence is involved in the majority of cases of varicose disease. Stratification of venous disease severity is still difficult. This study aims to correlate GSV diameters with C of CEAP and the venous clinical severity score (VCSS). Methods Legs without GSV reflux (Control legs, Group 1) and legs with untreated isolated GSV reflux and varicose veins limited to the GSV territory (Group 2) were studied clinically and with duplex ultrasound in a prospective study. The GSV diameters were measured both next to the saphenofemoral junction (SFJ) and at proximal thigh (PT) and correlated to the C of CEAP and VCSS. Results The control legs-group 1 were: n=33, 6 male, mean age 53, mean BMI 26.The legs with reflux-group 2 were: n=78, 16 male, mean age 54, mean BMI 27.The mean diameters for the SFJ ( ± SD) for groups 1 and 2 were 6.4 ± 1.8 and 9.9 ± 3.4. For PT they were 3.6 ± 0.9 and 5.9 ± 1.8 respectively. In legs with reflux the SFJ diameter correlates strongly with the PT diameter (r=0.69) and moderately with the C of CEAP and VCSS; 0.42 and 0.45 respectively. The PT diameter correlates slightly better with the C of CEAP and VCSS than the SFJ diameter (0.55 and 0.57). The mean values of VCSS for groups 1 and 2 were 0.70. and 4.69. The C of CEAP and VCSS show a strong correlation among them with r=0.79 in group 2 and 0.80 in the whole sample. Conclusion The GSV diameters next to the SFJ and particularly at the PT in patients having reflux correlate strongly with both the C of CEAP and VCSS. Recording the GSV diameters at the SFJ and PT in a standardized way may improve comparison of published data and contribute to choice of treatment in the future.

In collaborative software development, the utilization of Version Control Systems (VCSs) is a must. For this, a multitude of pessimistic as well as optimistic VCSs for model artifacts emerged. Pessimistic approaches follow the lock-edit-unlock paradigm whereas optimistic approaches allow parallel editing of one resource, which are therefore the preferred ones. To be flexible for the ever increasing variety of modeling environments and languages such tools should be independent of the modeling environment and applicable to various modeling languages. Those VCS characteristics may implicate a lack of information for the conflict detection method by virtue of firstly receiving solely the state of an artifact without concrete editing operations and secondly due to unavailable knowledge about the semantics of a modeling language. However, in optimistic VCSs concurrent changes can result in conflicts and inconsistencies. In environment and language independent VCSs inconsistencies would even arise more often due to information losses. Hence, accurate conflict detection methods are indispensable for the realization of such VCSs. To tackle this task, the “Semantically enhanced Model Version Control System” SMoVer is presented. With SMoVer it is possible to specify the semantics of a modeling language, needed for conflict detection in order to provide more accurate conflict reports than other current environment and language independent VCSs. In this work, it is exemplified how semantics of a specific modeling language can be specified in SMoVer, how those specifications can improve the accuracy of conflict reports and finally how those can be presented to modelers.

Mục tiêu: Nghiên cứu (NC) được tiến hành với mục tiêu đánh giá kết quả theo dõi 12 tháng sau điều trị suy tĩnh mạchmạn tính chi dưới bằng phương pháp can thiệp nội mạch.Phương pháp: NC được tiến hành trên 60 bệnh nhân (BN) có chẩn đoán suy tĩnh mạch mạn tính chi dưới tại khoa Chẩnđoán hình ảnh, Bệnh viện Bạch Mai, 2018-2019. Phương pháp chọn mẫu thuận tiện được áp dụng.Kết quả: Phân tích trên 60 BN can thiệp nội mạch (40 chân đốt laser và 20 chân đốt sóng cao tần), cho thấy tỷ lệ loại bỏdòng trào ngược trong tĩnh mạch hiển bị suy van là 100%. Đối với nhóm đốt bằng laser, điểm VCSS trung bình trước điều trị là3,67 ± 3,58 (0-11); điểm VCSS trung bình là 1,22 ± 2,03 (0-6) sau điều trị 1 tháng; điểm VCSS bằng 0,41 ± 1,01 (0-4) sau 12tháng. Introduction Quality-of-life and severity scores are both popular measures in medicine. For deep venous obstruction, the VEINES-QOL/Sym and venous clinical severity score (VCSS) are widely used. Combining a patient-reported outcome with a clinical severity score should give a more sensitive outcome for treatment results. To establish and compare their suitability for deep venous disease, we compared the outcomes of both scores in a group of patients who were interventionally treated for deep venous disease. Methods The venous clinical severity scores and VEINES-QOL/Sym scores of a group of patients who had received interventional treatment for deep venous obstruction more than 12 months ago were compared at T0 and T12. Results Both the Spearman’s rho and Pearson’s r show a very weak, negative correlation (statistically significant ( p ≤ 0.05)), between the venous clinical severity score and VEINES-QOL at T12 and between the venous clinical severity score and the VEINES-SYM at both T0 and T12. T0 VCSS-VSYM: rho = −0.219 ( p = 0.052), r = −0.236 ( p = 0.037), T12 VCSS-VQOL: rho = −0.459 ( p = 0.007), r = −0.379 ( p = 0.030), T12 VCSS-VSYM: rho = −0.463 ( p = 0.007), r = −0.432 ( p = 0.012). This negative correlation was not statistically ( p ≥ 0.05) confirmed for T0 VCSS-VSYM. Conclusion The physician-scored venous clinical severity score of patients treated for deep venous obstruction does not correlate well with their self-reported quality-of-life and symptom scores.

Aggregation of the German cockroach, Blattella germanica, is regulated by fecal aggregation agents (pheromones), including volatile carboxylic acids (VCAs). We demonstrate that the gut microbial community contributes to production of these semiochemicals. Chemical analysis of the fecal extract of B. germanica revealed 40 VCAs. Feces from axenic cockroaches (no microorganisms in the alimentary tract) lacked 12 major fecal VCAs, and 24 of the remaining compounds were represented at extremely low amounts. Olfactory and aggregation bioassays demonstrated that nymphs strongly preferred the extract of control feces over the fecal extract of axenic cockroaches. Additionally, nymphs preferred a synthetic blend of 6 fecal VCAs over a solvent control or a previously identified VCA blend. To test whether gut bacteria contribute to the production of fecal aggregation agents, fecal aerobic bacteria were cultured, isolated, and identified. Inoculation of axenic cockroaches with individual bacterial taxa significantly rescued the aggregation response to the fecal extract, and inoculation with a mix of six bacterial isolates was more effective than with single isolates. The results indicate that the commensal gut microbiota contributes to production of VCAs that act as fecal aggregation agents and that cockroaches discriminate among the complex odors that emanate from a diverse microbial community. Our results highlight the pivotal role of gut bacteria in mediating insect–insect communication. Moreover, because the gut microbial community reflects the local environment, local plasticity in fecal aggregation pheromones enables colony-specific odors and fidelity to persistent aggregation sites.

Objective The aim of the study is to report long-term results after mechano-chemical ablation for the treatment of great saphenous vein incompetence. Methods Mechano-chemical ablation was performed using the ClariVein device with polidocanol as the sclerosant. Clinical-Etiologic-Anatomic-Pathophysiologic (CEAP) classification, Venous-Clinical-Severity-Score (VCSS), anatomical-success, RAND-SF36 and the Aberdeen-Varicose-Vein-Questionnaire (AVVQ) were obtained by clinical examination, questionnaires and duplex ultrasonography through five years’ follow-up (Clinicaltrials.gov, NCT01459263). Results Ninety-four patients (113 great saphenous veins) were included in the study. Five-year follow-up data were available for 75 limbs (66.4%). Freedom from anatomical failure was 78.7% (N = 45) with 64.6% (N = 42) having an improvement in the VCSS ≥1. A total of five reinterventions were performed through five years with a median time to reintervention of 38 months. The VCSS at five years was 2 (IQR 1; 4) and was still significantly improved compared to baseline (p < 0.001). A deterioration of the VCSS was observed in 21.5%. Conclusion Through five-year follow-up, there are a significant number of anatomical and clinical failures in this series, mainly driven by partial recanalizations.

Objectives The venous clinical severity score (VCSS) and the Aberdeen varicose vein questionnaire (AVVQ) improve after treating chronic venous insufficiency (CVI). The aim was to examine how and why they improve by evaluating the change in each individual question. Methods This was an analysis on prospectively collected data from a clinical study on 100 patients (58% female) with CVI (C2 = 34, C3 = 14, C4a = 29, C4b = 9, C5 = 7, C6 = 7) who were randomized to endovenous laser ablation ( n = 50) or foam sclerotherapy ( n = 50). The change scores (performance) of each question of the VCSS (questions 1–10) and the AVVQ (questions 1–13) were calculated by subtracting the score at three weeks, and three months, from the pre-treatment score. Results Both the median, interquartile range (IQR), VCSS and the AVVQ scores improved from 6 (4) and 21.4 (15.1) at baseline to 3 (4) and 18.6 (12.1) at three weeks ( P < 0.0005, P = 0.031) to 2 (3) and 8.8 (13.6) at three months, ( P < 0.0005, P < 0.0005), respectively. The performance of the first three questions of the VCSS (pain, extent of varicosities, oedema) were the most contributory to the overall score. Questions 5, 7, 8, 9 on ulceration improved the most individually but did not contribute significantly to the overall score. Questions 5, 9 of the AVVQ on stocking use and ulceration failed to contribute statistically to the overall improvement at three months. Conclusion The majority of the individual questions of the VCSS and AVVQ responded to change. However, the cause of a poor response was multifactorial with statistical dilution playing a significant role. Stratification of patients according to ulceration may allow better comparisons.

Analyses of case studies are used to evaluate new or existing technologies, measures or strategies with regard to their impact on the overall process. However, data availability is limited and hence, new technologies, measures or strategies can only be tested on a limited number of case studies. Owing to the specific boundary conditions and system properties of each single case study, results can hardly be generalized or transferred to other boundary conditions. virtual infrastructure benchmarking (VIBe) is a software tool which algorithmically generates virtual case studies (VCSs) for urban water systems. System descriptions needed for evaluation are extracted from VIBe whose parameters are based on real world case studies and literature. As a result VIBe writes Input files for water simulation software as EPANET and EPA SWMM. With such input files numerous simulations can be performed and the results can be benchmarked and analysed stochastically at a city scale. In this work the approach of VIBe is applied with parameters according to a section of the Inn valley and therewith 1,000 VCSs are generated and evaluated. A comparison of the VCSs with data of real world case studies shows that the real world case studies fit within the parameter ranges of the VCSs. Consequently, VIBe tackles the problem of limited availability of case study data.

Objectives: The homoeopathic remedy, Hamamelis, has been extensively used to treat varicose veins. However, scientific research conclusively proving its utility is lacking. This study was performed to ascertain the remedy’s therapeutic utility and efficacy in treating varicose veins and its complications using the revised venous clinical severity score (VCSS) tool and Doppler studies. Materials and Methods: A prospective uncontrolled experimental study with purposive sampling was performed; the subjects were selected from the outpatient department. A total of 32 cases, fulfilling the inclusion and exclusion criteria were clinically examined. The VCSS and Doppler test before and after treatment with Hamamelis 30 were performed. Results: The Wilcoxon signed-rank test was applied and the change in the median VCSS was found to be statistically significant. P-value of the pre score was 5.53 ± 4.15 and that of the post score was 3.45 ± 2.36 with a mean change of 2.09 ± 2.87 and 37.69% improvement with P-value of 0.000 < 0.01. The Doppler study showed no significant change or improvement. Conclusion: This study revealed that the homoeopathic remedy Hamamelis 30 is effective in the management of varicose veins and its complications by ameliorating symptoms and improving the VCSS. However, the Doppler study reports showed no changes or improvement perhaps due to the short period of treatment.

Alternative food networks (AFNs) have emerged to improve both environmental and socio-economic aspects of food provisioning, including the living and working conditions of farmers. Their objectives are supposed to be mediated through the shortening of chains and/or the implication of alternative value chain actors (VCAs). Through the application of a social life cycle assessment methodological proposal on two products from three Belgian AFNs, we first verify how the AFNs meet sustainability promises. Second, we investigate how such social sustainability of the assessed products is influenced by the differentiated configurations of chain governance in the AFNs. Such a discussion of root causes of social sustainability performances in product chains have been investigated very little as of yet. Our results show that AFN perform well in some aspects (consumer aspects, work satisfaction, social ties between VCAs), but in some others, AFN chains use similar mechanisms as the ones used by mainstream chains (unbalanced market power, unfair prices, and low commitment between VCAs), with potentially detrimental effects on profitability and employment conditions for VCAs located upstream, i.e., farms. Our framework is useful to highlight social hotspots in product chains, and to discuss these across the differences in the configurations of the chain layout and—in the end—chain governance.

Abstract High variety is a characteristic attribute of any material phenomena and processes involving living matter, i.e., very complex systems (VCSs). We have verified the presence of fundamental constraints on the size/shape diversity and self-organization by the example of mammalian skeleton in four orders (41 species). The properties of more than 4700 multidimensional descriptive models of VCSs were studied. A self-organization parameter R (0 ≤ R ≤ 1) was calculated for each model, and its range of variability was mainly limited to the interval from ~0.10 to ~0.31. The concepts of an abstract Ashby regulator and the Shannon–Hartley theorem were used to explain the variation in the empirical data. It has been concluded that there are significant constraints on the quality of morphological diversity regulation and the possible level of self-organization of VCSs for steady states.

Voice Control Systems (VCSs) provide a user-friendly interface for issuing voice commands to smart devices. However, the security of VCSs remains a challenge, as evident from two distinct attack classes: (i) inaudible attacks, feasible when the attacker is in proximity to the victim, and (ii) audible attacks, which can be executed remotely by embedding attack signals into audio. In talk introduces a novel class of attacks known as Near-Ultrasound Inaudible Trojan (NUIT). NUIT attacks combine the advantages of both classes, being both inaudible and capable of remote execution. Additionally, NUIT attacks can achieve end-to-end unnoticeability, a crucial aspect often overlooked in the literature. Notably, NUIT attacks leverage victim speakers to compromise victim microphones and associated VCSs, eliminating the need for specialized speakers. The feasibility of NUIT attacks is demonstrated, and an effective defense against them is proposed.

Abstract Introduction The first vascular composite allotransplantation (VCA) was performed in 1998, with the first post-burn VCA in 2003. Since then, 21 burn patients have been recipients of VCAs. Controversy exists regarding safety of VCA in burn patients due to high levels of immunologic sensitization attributed to use of cadaver allograft. We seek to understand unique considerations of VCA in burn reconstruction and its relative safety compared to VCA in non-burn reconstruction. Methods A systematic review of VCA in burn reconstruction was conducted using the PubMed, Media and EBSCO database to retrieve all articles and news stories studying VCA from Jan. 2000 to May 2019. Only articles containing composite tissue allotransplantation, VCA, and reconstructive surgery were included. Articles that did not report outcomes or patient information were excluded. Reported cases were grouped according to injury and tissue (face vs. upper extremity) with mortality as a dependent variable. Statistical analysis was performed with the Fisher exact test. Results Of 63 articles obtained, 21 met criteria. After secondary review by the senior author, 6 more articles were included. To date, 147 VCAs are reported in 112 patients, including 40 face VCAs and 107 upper extremity VCAs. Of these, 30 (20%) were performed for post-burn deformities, and 117 (80%) for non-burn deformities. A total of 21 post-burn patients received VCAs with 4 subsequent deaths (19%). Post-burn recipients of upper extremity VCAs had significantly higher mortality than non-burn recipients of the same (p = 0.01), but there was no significant difference in face VCA mortality (p = 1.0) or overall mortality (p = 0.06) between groups. Conclusions VCA is a forefront procedure employed worldwide for reconstruction of devastating injuries. Presently, burn patients are at greater risk of mortality after upper extremity VCA, but not face VCA. Burn patients will benefit from focused investigation of unique post-burn risk factors of VCA mortality. Applicability of Research to Practice Disparities between post-burn and non-burn VCA outcomes suggest the presence of unique risk factors in burn patients. Investigation of unique risk factors will improve safety of VCA for burn patients.

Abstract Numerous decision support tools have been developed to assist stormwater managers to understand future scenarios and devise management strategies. This paper presents one such tool, the Vulnerability, Consequences, and Adaptation Planning Scenarios (VCAPS) process, and reports on experiences from its deployment in 10 coastal communities on the Atlantic and Gulf coasts. VCAPS helps to elucidate local complexities, couplings, and contextual nuance through dialogue among technical experts and those with detailed contextual knowledge of a community. Participants in the process develop qualitative scenarios of climate change impacts and how different management strategies may prevent or mitigate undesirable consequences. The scenarios help stormwater managers diagnose potential problems that may emerge from climate change and variability, which can then be subject to further detailed analysis. The authors describe five challenges faced by stormwater managers and how insights that emerge from scenario-based processes like VCAPS can help address them: characterizing the implications of interacting climate stressors that originate stormwater, bringing all available expertise and local knowledge to bear on the problem of stormwater management, integrating local and scientific information about coupled human–environment systems, identifying management actions and their trade-offs, and facilitating planning for sustained coordination among multiple public and private entities.

AbstractThis work developed a model for predicting the volatilization flux from the unsaturated soil contaminated by volatile chemical substances (VCSs) such as mercury and benzene. The model considers a series of phenomena under the unsaturated condition such as multi-phase flow consisting of a non-aqueous phase liquid, water, and gases together with the permeation of rainfall into the surface soil, the volatilization/condensation of VCSs, and the adsorption/desorption of VCSs. On this basis, this work clarified a mechanism for the generation of a volatilization flux at the ground surface. In addition, the effects of various transport phenomena in the surface soil on the magnitude and seasonal changes in this flux due to variations in weather factors such as rainfall level, temperature, and air pressure were quantitatively evaluated. This newly developed prediction model can be utilized to estimate dynamic variations in the flux under real-environmental conditions.

Vascularized composite allografts (VCAs) refer to en bloc heterogenous tissue that is transplanted to restore form and function after amputation or tissue loss. Rat limb VCA has emerged as a robust translational model to study the pathophysiology of these transplants. However, these models have predominately focused on hindlimb VCAs which does not translate anatomically to upper extremity transplantation, whereas the majority of clinical VCAs are upper extremity and hand transplants. This work details our optimization of rat forelimb VCA procurement and sub-normothermic machine perfusion (SNMP) protocols, with results in comparison to hindlimb perfusion with the same perfusion modality. Results indicate that compared to hindlimbs, rat forelimbs on machine perfusion mandate lower flow rates and higher acceptable maximum pressures. Additionally, low-flow forelimbs have less cellular damage than high-flow forelimbs based on oxygen uptake, edema, potassium levels, and histology through 2 hours of machine perfusion. These results are expected to inform future upper extremity VCA preservation studies.

The field of vascularized composite allografts (VCAs) has undergone significant advancement in recent decades, and VCAs are increasingly common and accepted in the clinical setting, bringing hope of functional recovery to patients with debilitating injuries. A major obstacle facing the widespread application of VCAs is the side effect profile associated with the current immunosuppressive regimen, which can cause a wide array of complications such as infection, malignancy, and even death. Significant concerns remain regarding whether the treatment outweighs the risk. The potential solution to this dilemma would be achieving VCA tolerance, which would allow recipients to receive allografts without significant immunosuppression and its sequelae. Promising tolerance protocols are being studied in kidney transplantation; four major trials have attempted to withdraw immunosuppressive treatment with various successes. The common theme in all four trials is the use of radiation treatment and donor cell transplantation. The knowledge gained from these trials can provide valuable insight into the development of a VCA tolerance protocol. Despite similarities, VCAs present additional barriers compared to kidney allografts regarding tolerance induction. VCA donors are likely to be deceased, which limits the time for significant pre-conditioning. VCA donors are also more likely to be human leukocyte antigen–mismatched, which means that tolerance must be induced across major immunological barriers. This review also explores adjunct therapies studied in large animal models that could be the missing element in establishing a safe and stable tolerance induction method.

Resumo Os Vórtices Ciclônicos de Altos Níveis (VCANs) subtropicais são sistemas de baixa pressão em médios e altos níveis da atmosfera, com centro frio e que se encontram segregados para o lado equatorial das correntes de jato. A divergência de massa no setor a jusante dos VCANs pode organizar os movimentos ascendentes na atmosfera e precipitação. Diante da contribuição dos VCANs para alterações no tempo e os poucos estudos desses sistemas na literatura, o presente trabalho descreve a evolução sinótica e energética de um VCAN que causou evento extremo chuvoso no deserto do Atacama, entre os dias 24 e 26 de março de 2015. Para o desenvolvimento do estudo foram utilizadas a reanálise NCEP/DOE, dados de precipitação do TRMM e informações do satélite GOES-13. Entre os resultados têm-se que a advecção horizontal de vorticidade relativa anticiclônica é importante para amplificar a crista que adentra o setor sudoeste/sul do cavado em 300 hPa fazendo com que o ar frio fique aprisionado em latitudes mais baixas, contribuindo para a formação do VCAN. A energética de Lorenz mostrou que o ambiente ao qual o VCAN estava inserido participou de forma importante no ciclo de vida do VCAN. Por exemplo, na fase de VCAN, os processos baroclínico, barotrópico e de fluxo na fronteira atuaram como fonte de energia cinética do distúrbio (KE).

Pendekatan pada penilaian sistem e-learning, yang selama ini terbelah antara bertumpu pada satu faktor, atau berupaya menggabungkan keduanya tanpa membedakan kekhasan karakteristiknya. Upaya unifikasi tanpa membedakan karakter tersebut akan menyulitkan perumusan tindak lanjut hasil penilain yang dilakukan, sebab tindak lanjut pasca penilaian memiliki perbedaan pada kedua jenis faktor tersebut. Faktor-faktor pedagogis maupun usabilitas sistem mungkin berpengaruh, diperoleh dari hasil studi literatur pada penelitian-penelitian sebelumnya. Pengujian terhadap faktor-faktor tersebut dilakukan dengan melakukan survei kepada para pengguna sistem VCLASS di Universitas Indo Global Mandiri (UIGM). Selain diharapkan mendapatkan faktor-faktor yang diperlukan dalam penyusunan kerangka penilaian usabilitas sistem e-learning, penelitian ini juga dapat memberikan kontribusi terhadap penyempurnaan sistem VCLASS UIGM.

Abstract The interfacial shear strength (τi) between magnesium phosphate ceramics (MPC) and sugar maple (Acer saccharum) has been evaluated by means of a pullout test method. The binder system monopotassium phosphate (MKP) and magnesium oxide (MgO) resulted in the highest τi at a MKP/MgO ratio of 3:1. The influence of the levels of MPC binder and aggregates on the interfacial properties has been observed based on a mixture design model. The aggregates were Portland cement (PC), wollastonite (WO), and vitrified calcium aluminosilicates (VCAS). The MPC binder level was the primary factor influencing τi, while WO and VCAS can mutually promote τi. However, τi is decreased when PC is mixed with the two other aggregates. The environmental durability of VCAS-based MPC (V-MPC), PC-based MPC (C-MPC), and WO-based MPC (W-MPC) was assessed by treatments at conditioning (90% relative humidity) and water immersion. The V-MPC/maple has very poor moisture performance, while the C-MPC/maple and W-MPC/maple had reduced interfacial shear stresses but still maintained bond integrity unlike the V-MPC.

The study focuses on communication strategies, specifically examining how they are perceived and used in English-speaking activities, as well as the impacts of verbal communications strategies (VCSs) on the performances of second-year English students from the Faculty of Foreign Language Teacher Education and the Faculty of English at the University of Danang-University of Foreign Language Studies (hereafter called EFL students for brief) as well as to propose appropriate solutions to assist them to improve their communication skills. Data were collected from 280 questionnaires, and the analysis of the in-depth interviews was conducted with 10 student informants and observations of 40 students in total. The findings show that most students were familiar with the term “communication strategies”, though they did not know exactly what the term referred to. Additionally, EFL second-years employed all twelve studied VCSs at various frequencies. Both positive and negative influences were reported, and judgement about errors from surrounding people was chosen as the biggest obstacle when applying VCSs. Several solutions were discussed in the study to solve these problems.

Although vascularized composite allografts (VCAs) have been performed clinically for a variety of indications, potential complications from long-term immunosuppression and graft-versus-host disease remain important barriers to widespread applications. Recently it has been demonstrated that VCAs incorporating a vascularized long bone in a rat model provide concurrent vascularized bone marrow transplantation that, itself, functions to establish hematopoietic chimerism and donor-specific tolerance following non-myeloablative conditioning of recipients. Advances such as this, which aim to improve the safety profile of tolerance induction, will help usher in an era of wider clinical VCA application for nonlife-saving reconstructions.

Background The rapid increase in mental health disorders has prompted a call for greater focus on mental health promotion and primary prevention. Web- and mobile app–based interventions present a scalable opportunity. Little is known about the influence of human support on the outcomes of these interventions. Objective This study aimed to compare the influence of 3 modes of human support on the outcomes (ie, mental health, vitality, depression, anxiety, stress, life satisfaction, and flourishing) of a 10-week, Web- and mobile app–based, lifestyle-focused mental health promotion intervention among a healthy adult cohort. Methods Participants were recruited voluntarily using a combination of online and offline advertising. They were randomized, unblinded into 3 groups differentiated by human support mode: Group 1 (n=201): standard—fully automated emails (S); Group 2 (n=202): standard plus personalized SMS (S+pSMS); and Group 3 (n=202): standard plus weekly videoconferencing support (S+VCS), hosted by 1 trained facilitator. Participants accessed the intervention, including the questionnaire, on a Web-based learning management system or through a mobile app. The questionnaire, administered at pre- and postintervention, contained self-reported measures of mental well-being, including the “mental health” and “vitality” subscales from the Short Form Health Survey-36, Depression Anxiety and Stress Scale-21, Diener Satisfaction With Life Scale (SWLS), and Diener Flourishing Scale. Results Of 605 potential participants, 458 (S: n=157, S+pSMS: n=163, and S+VCS: n=138) entered the study by completing registration and the preintervention questionnaire. At post intervention, 320 out of 458 participants (69.9%; S: n=103, S+pSMS: n=114, and S+VCS: n=103) completed the questionnaire. Significant within-group improvements were recorded from pre- to postintervention in all groups and in every outcome measure (P≤.001). No significant between-group differences were observed for outcomes in any measure: mental health (P=.77), vitality (P=.65), depression (P=.93), anxiety (P=.25), stress (P.57), SWLS (P=.65), and Flourishing Scale (P=.99). Adherence was not significantly different between groups for mean videos watched (P=.42) and practical activity engagement (P=.71). Participation in videoconference support sessions (VCSSs) was low; 37 out of 103 (35.9%) participants did not attend any VCSSs, and only 19 out of 103 (18.4%) attended 7 or more out of 10 sessions. Stratification within the S+VCS group revealed that those who attended 7 or more VCSSs experienced significantly greater improvements in the domains of mental health (P=.006; d=0.71), vitality (P=.005; d=0.73), depression (P=.04; d=0.54), and life satisfaction (P=.046; d=0.50) compared with participants who attended less than 7. Conclusions A Web- and mobile app–based mental health promotion intervention enhanced domains of mental well-being among a healthy cohort, irrespective of human support. Low attendance at VCSSs hindered the ability to make meaningful between-group comparisons. Supplementing the intervention with VCSSs might improve outcomes when attendance is optimized. Vaginal cytology can facilitate determination of the estrous stage in dogs. Although some studies recommended the vaginal cotton swab smear (VCSS) method for sample collection, some veterinarians prefer the vulvar stamp smear (VSS) method for its convenience and to avoid causing trauma or introducing pathogens from the posterior vagina to the anterior vagina. However, to the best of the authors’ knowledge, no study has compared the results obtained using VCSS and VSS. In this study, the two methods were used to collect samples from 81 dogs. All slides were blindly examined by three veterinarians. Cells were classified into parabasal cells, intermediate cells, superficial cells, and anuclear cells according to cell outline and nuclear appearance. The predominant cell type was identified and recorded. The agreement rate between the methods for the four types was 90.9%, 86.9%, 62.1%, and 23.3%, respectively. Overall, agreement rate was 65.0%. The agreement for the intermediate and anuclear cells was significantly higher and lower than the expected, respectively ([Formula: see text] for both). The VSS method was reported by the owner to be significantly more accepted than the VCSS method ([Formula: see text]). In conclusion, significant differences in anuclear cell identification between the methods were observed. Therefore, VSS results, particularly those for anuclear cells, should be cautiously interpreted.

Abstract The field of vascularized composite allotransplantation (VCA) is the new frontier of solid organ transplantation (SOT). VCA spans life-enhancing/life-changing procedures such as upper extremity, craniofacial (including eye), laryngeal, tracheal, abdominal wall, penis, and lower extremity transplants. VCAs such as uterus transplants are life giving unlike any other SOT. Of all VCAs that have shown successful intermediate- to long-term graft survival with functional and immunologic outcomes, lower extremity VCAs have remained largely underexplored. Lower extremity transplantation (LET) can offer patients with improved function compared to the use of conventional prostheses, reducing concerns of phantom limb pain and stump complications, and offer an option for eligible amputees that either fail prosthetic rehabilitation or do not adapt to prosthetics. Nevertheless, these benefits must be carefully weighed against the risks of VCA, which are not trivial, including the adverse effects of lifelong immunosuppression, extremely challenging perioperative care, and delayed nerve regeneration. There have been 5 lower extremity transplants to date, ranging from unilateral or bilateral to quadrimembral, progressively increasing in risk that resulted in fatalities in 3 of the 5 cases, emphasizing the inherent risks. The advantages of LET over prosthetics must be carefully weighed, demanding rigorous candidate selection for optimal outcomes.

Objectives: This research aimed to determine the impact of reflux patterns in patients with great saphenous vein (GSV) insufficiency on clinical severity and management. Methods: We evaluated 771 lower extremities of 452 patients having GSV insufficiency with Doppler Ultrasound. We characterized the reflux patterns like previously done in the literature: type 1, GSV reflux without the involvement of perimalleolar region or saphenofemoral junction (SFJ); type 2, GSV reflux involving perimalleolar region; type 3, GSV reflux involving SFJ; type 4, GSV reflux involving both perimalleolar region and SFJ. Then we conducted a study to evaluate the relationship between GSV reflux type and age, gender, venous clinical severity score (VCSS), clinical, etiological, anatomical and pathophysiological elements (CEAP), body mass index (BMI) and gave an effort to compare the groups by therapeutic strategies. Results: The mean age was 44±11 years. The male-to-female ratio was 0.49. The most commonly observed reflux pattern was type 3 (48%) in patients with GSV insufficiency and varicose veins. Patients with type 1 reflux were younger, had lower BMI, and had a better clinical situation (p=0.002). VCSS was associated with perimalleolar region involvement, as increased in type 2 reflux than type 1, and higher VCSS was related to SFJ involvement as defined in type 3 reflux. Type 4 reflux patients had the highest VCSS that means the most severe clinical presentation. Sclerotherapy was the most common treatment modality in type 1 reflux (p<0.001). Also, in type 2 and type 4 reflux, sclerotherapy was more preferred than type 3 (p<0.001). Type 4 reflux pattern required mostly radiofrequency ablation, compared with type 3 (28.2% vs 20.5%; p<0.05) after initial diagnosis. Cyanoacrylate glue embolization was the appropriate therapeutic option in only 3.2% of type 4 reflux patients, which was significantly lower than type 3. Conclusion: According to the reflux pattern classification system based on SFJ and malleolar region involvement as practiced in this study, we described a correlation between VCSS, CEAP, BMI, and the extent of venous insufficiency. This correlation with consideration of cosmetic reasons and vein diameter measurements can suggest further treatment modality. Advances in knowledge: We investigated a practical, clinically applicable, and widely accepted standard method for classifying GSV insufficiency. Mapping venous insufficiency with such a system is essential to determine the clinical severity and the most appropriate treatment modality.

BACKGROUND: Chronic venous disease (CVD) is progressive and has a high prevalence in the economically active population. Its impact on the quality of life of affected individuals is poorly understood. OBJECTIVE: To test for correlations between the CEAP classification of CVD severity and CVD symptoms and quality of life of affected individuals. METHODS: We investigated 91 lower limbs in 59 patients with CVD (CEAP C1- C6). Patients were assessed with a Visual Analogue Pain Scale (VAPS), the Venous Clinical Severity Score (VCSS) and the SF-36 quality-of-life questionnaire. Spearman correlation coefficients were calculated. RESULTS: There were positive correlations between the CEAP classification and scores for VAPS (0.815, p <0.000), VCSS (0.937, p <0.000), and SF-36 in the dimensions Physical Functioning: -0.791, p <0.000; Role Physical: -0.839; p <0.000; Bodily Pain: -0.684; General Health: -0.617, p <0.000; Role Emotional: -0.691, p <0.000). There was no correlation with Vitality: -0.003, p=0.979; Role Social: -0.188, p=0.740 or Mental Health: -0.085, p=0.421. CONCLUSIONS: There were positive correlations between CEAP and both VAPS and VCSS. Chronic Venous Insufficiency progressively affects quality of life (SF-36). Physical and emotional aspects are more severe at later stages of CVD. Vitality, Mental Health and Role Social can be negatively impacted from the early stages of the disease.

OBJECTIVE: To evaluate the efficacy of surgical treatment of varicose veins with preservation of the great saphenous vein. METHODS: We conducted a prospective study of 15 female patients between 25 and 55 years of age with clinical, etiologic, anatomic and pathophysiologic (CEAP) classification 2, 3 and 4. The patients underwent surgical treatment of primary varicose veins with great saphenous vein (GSV) preservation. Doppler ultrasonography exams were carried out in the first and third months postoperatively. The form of clinical severity of venous disease, Venous Clinical Severity Score (VCSS) was completed before and after surgery. We excluded patients with history of deep vein thrombosis, smoking or postoperatively use of elastic stockings or phlebotonics. RESULTS: All patients had improved VCSS (p <0.001) and reduction in the diameter of the great saphenous vein (p <0.001). There was a relationship between VCSS and the GSV caliber, as well as with preoperative CEAP. There was improvement in CEAP class in nine patients when compared with the preoperative period (p <0.001). CONCLUSION: The varicose vein surgery with preservation of the great saphenous vein had beneficial effects to the GSV itself, with decreasing caliber, and to the symptoms when the vein had maximum caliber of 7.5 mm, correlating directly with the CEAP. The decrease in GSV caliber, even without complete abolition of reflux, leads to clinical improvement by decreasing the reflux volume.

Introduction. Pregnancy is the leading cause of varicose veins(VVs) in women. Therapeutic measures during pregnancy include the wearing of compression hosiery and the phlebotropic therapy.Purpose. To evaluate the clinical effectiveness of graduated elastic compression and phlebotropic therapy in pregnant women with VVs.Materials and methods. A total of 88 pregnant women were included, 30 of whom used compression stockings (group 1), 28 women received phlebotonics together with compression (group 2) and 30 were controls. VVs severity was determined using the Venous Clinical Severity Score (VCSS). Quality of life (QoL) was calculated using the Chronic Venous Insufficiency Questionnaire (CIVIQ-20). The calf circumference was measured with a measuring tape in an orthostatic position.Results and discussion. At the end of the study there was a decrease in VCSS (p < 0.001 and p < 0.008) in both treatment groups, whereas the control group showed an increase in VCSS severity (p < 0.001). At the end of follow-up the QOL score in the treatment groups showed a tendency to improve while the control group showed a decrease in QOL. A bilateral decrease in tibial circumference (p < 0.001) was seen in groups 1 and 2 and the adjuvant phlebotropic treatment was found to be superior in controlling the oedema.Conclusions. The combined use of elastic compression and phlebotonics is more effective in eliminating venous oedema in pregnant women with VVs.

Neste estudo foi desenvolvido um Método Objetivo (MO) para identificar Vórtices Ciclônicos de Altos Níveis (VCANs) na região Tropical Sul, baseado na vorticidade relativa e no escoamento horizontal do vento ao redor do centro dos sistemas. O MO foi elaborado usando dados de reanálises do National Centers for Environmental Prediction - National Centers for Atmospheric Research (NCEP-NCAR) e validado, para o período de cinco anos (2002 a 2006), através da comparação entre os resultados obtidos pelo MO e os obtidos pela Análise dos Campos (AC) de vento e vorticidade relativa. Por outro lado, uma avaliação da AC em identificar os VCANs foi feita, para o mesmo período, através da comparação entre os resultados desta e os obtidos usando Imagem de Satélite (IS) no canal espectral do vapor d'água. As AC apresentaram resultados satisfatórios e, portanto, o conjunto dos dados de reanálises do NCEP-NCAR foi aplicado ao MO. Foi observado um bom desempenho do MO em identificar os sistemas, bem como algumas características associadas, tais como: número de ocorrência, número de dias com atuação, posicionamento, direção de deslocamento e região de formação.

Outcome assessment is an important criterion for the objective determination of the risks and benefits of a given procedure. The choice of an assessment instrument is critical in order to generate meaningful and relevant data. Assessment instruments are platforms for comparison and stratification of information that provide a common ground and unified language for discussions on disease processes and therapies. Like many complex conditions, venous disease has benefited from the institution of several assessment instruments designed to clarify elements of the disease process. Among these is the venous clinical severity score (VCSS), which has proved to be a valuable tool for evaluating changes in condition over time with or without intervention. The VCSS has recently undergone a revision to increase its sensitivity and value in interpreting the language of venous disease.
