The incidence of chronic kidney disease (CKD) and end-stage kidney disease (ESKD) is increasing, reflecting an increase in the incidence and prevalence of hypertension and type 2 diabetes. Patients with CKD and ESKD frequently experience episodes of critical illness and require treatment in an intensive care unit (ICU)setting. Management requires specific consideration of their renal disease status together with their acute illness. Mortality in critically-ill patients with ESKD is frequently related to their co-morbid conditions, rather than their ESKD status. Illness severity scoring systems allocate high points for renal variables and tend to overestimate actual mortality. Patients with ESKD and CKD requiring ICU admission have better ICU and in-hospital survival than patients with denovo acute kidney injury requiring renal replacement therapy. Appropriately selected patients benefit from ICU admission and full consideration for ICU care should be given to these patients if required, despite their renal disease status. Cardiovascular disease and sepsis account for the majority of ICU admissions in this population and the aetiology of these conditions differs from that in patients without kidney disease. Optimal critical care management of patients with ESKD and CKD requires that these differences are recognized.

For the treatment of borderline personality disorder, a meta-analysis found statistically significant effects indicating a beneficial effect of dialectical behavior therapy (DBT) over treatment as usual for reduction in anger, parasuicidality, and mental health. All of these effects were moderate to large. However, DBT is not uniquely effective for borderline personality disorder. Two type 1 randomized controlled trials found equal effects for versions of psychodynamic therapy. Single type 1 studies have also supported the efficacy, compared to treatment as usual, of cognitive therapy, schema therapy, and Systems Training for Emotional Predictability and Problem Solving (STEPPS, a therapy that combines cognitive-behavioral techniques and skills training with a systems component) as treatments for borderline personality disorder.

In C-arm systems, the X-ray beam output is determined by both the signal returned from the automatic exposure device (AED) in the receptor and the exposure table set up for the exam type. The settings for the exposure (as well as aspects of the image receptor and display) will affect the qualities or resolutions of the resulting image. This chapter covers the factors and qualities of images produced by an X-ray C-arm system, and the effect that varying exposure factors has on the resulting image quality and resolutions. This includes spatial, temporal, and contrast resolutions, as well as the effects of under- and overexposure, and distortion or artefacts on the produced images.

Mucopolysaccharidoses (MPS) are a group of genetic diseases that affect connective tissues via lack of key lysosomal enzymes. This deficiency leads to storage of partially degraded glycosaminoglycans that build up in multiple organ systems. There are many types of MPS disorders and each has varying features, expected lifespan, enzyme affected, and resulting clinical effect. All MPS are autosomal recessive except Hunter syndrome, which is X-linked. Because each type has a different enzyme deficiency, each has different comorbidities and unique treatment modalities. These patients require multiple anesthetics for diagnostic and surgical interventions. This chapter discusses treatment ad complication issues regarding MPS.

This chapter discusses four grammatical systems in Singapore English that are transferred from Chinese: aspect, pragmatic particles, topicalization, and quantification. Proper analysis of the relevant substrate features reveals extensive clustering: features which form a grammatical system transfer together. Substratum transfer targets the grammatical system, and the transferred system is then exponenced with suitable morphosyntactic materials from the lexifier, filtering out those component features for which the lexifier has no well-formed morphosyntactic exponent. The analysis of the four systems shows that post-transfer stabilization is subject to the normative effect of English. It is argued that data obtained through introspection and corpora are complementary, and substrate-induced grammatical change is best accounted for in a usage-based model that uses the two types of data.
