Resumen
The effect of microwave (MW) heating on the changes in wet gluten content, Gluten Index, Falling Number and amylographic characteristics was studied in sprouted wheat grain. Different moistures of wheat in two ranges of 10–11% and 15–17% and two end temperatures of MW heated samples (60 and 80°C) were applied to wheat samples. Falling Number and Gluten Index increased with increasing absorbed energy during MW heating, whereas gluten content decreased. Amylographic maximum increased due to a-amylase inactivation progressively with increasing absorbed energy as well. The greatest relative changes occurred when the end temperature of MW heated samples 80°C and moisture 15% were used. An improvement effect on the baking quality of sprouted wheat was found due to an increase in amylographic maximum with higher energy doses and higher end temperatures of MW heated samples. It was a consequence of Falling Number increase and Gluten Index increase with lower energy doses. The negative effect of higher energy doses was proved in a decrease in wet gluten content.

Resumen
The development of a uniform method of epidemiological assessment and grading of malocclusion has been of interest for several decades. Recently, Daniels and Richmond (2000) proposed a new orthodontic index namely the Index of Complexity, Outcome and Need (ICON). Their aim was to develop a single index for assessing treatment inputs and outcomes.

The aim of this study was to critique the ICON and to assess to the extent to which each component of the ICON fulfils the ideal requirements of the ideal index as identified in a World Health Organization Report (WHO, 1966). The study was performed in three parts: 1) a gold standard was established to test reliability and validity of the ICON
2) to assess ease of use and simplicity of the index
3) and to test the applicability of the index on patients and study casts. The results showed that the ICON identified 25% of the cases as &lsquo
no treatment&rsquo
, as apposed to the 100% of the gold standard. Validity of the index was shown to be &lsquo
poor&rsquo
for complexity (? = 0.2) and degree of improvement (? = 0.34) and &lsquo
excellent&rsquo
for outcome. Reliability was high for all the components except for treatment need (? = 0.63). This study concluded that except for complexity and degree of improvement, the index performed well with respects to reliability, validity (of treatment outcome), ease of use and simplicity and applicability to patient and study casts.

Resumen
Different from the standard treatment discovery framework which is used for finding single treatments for a homogenous group of patients, personalized medicine involves finding therapies that are tailored to each individual in a heterogeneous group. In this paper, we propose a new semiparametric additive single-index model for estimating individualized treatment strategy. The model assumes a flexible and nonparametric link function for the interaction between treatment and predictive covariates. We estimate the rule via monotone B-splines and establish the asymptotic properties of the estimators. Both simulations and an real data application demonstrate that the proposed method has a competitive performance.

Resumen
With its widespread use across the country and increasing evidence of its effectiveness, the wraparound process has been accepted widely as a feasible alternative to restrictive residential treatments for children with severe emotional and behavioral disorders. Yet wraparound has been implemented and conceptualized in such a variety of ways that many have begun to question whether it truly is a single definable approach. Recently, a conceptual model for wraparound was offered that included ten essential elements as the key ingredients for this approach. Subsequently, the Wraparound Fidelity Index (WFI) was designed to measure the degree to which an intervention adheres to these ten elements. The purpose of the current study was to use data collected via the WFI to provide the first empirical test of wraparound’s conceptual model. Programs providing wraparound to children with severe emotional and behavioral disorders and their families used the WFI to collect data from caregivers (n = 481), youths (n = 355; 11 to 19 years), and resource facilitators (n = 610). Confirmatory factor analysis (CFA) was used to test the fit of a series of structural models consistent with the proposed element model of wraparound. First, CFA models were examined separately for each of the elements. Second, CFA models that represent the full wraparound model were tested, separately for each of the three informants. And third, a multitrait-multimethod (MTMM) analysis was conducted using a final CFA model including all elements (traits) and the three informants (methods). Findings supported the majority of elements and WFI items when tested separately at the first step. However, at the second step, only the youth model provided adequate fit to the data. Significant modification was necessary to yield admissible solutions for the caregiver and resource facilitator models. Finally, an inadmissible solution resulted when the three informants and revised model were tested in step three. Implications of the findings for the wraparound process, the WFI, and future research are discussed.

Resumen
Water and wastewater treatment through the use of membrane filtration technology is one of the processes utilised currently to meet the growing demand for water. While new technologies can harness water from various non-traditional sources such as oceans, there remains the possibility of making drinking water more expensive through the use of costly treatment equipment. To prevent this and ensuing catastrophes around the world, the water industry needs a strategy that keeps the price of water and price of products aiding in the treatment of water controlled into the future. The overall aim of this study is to develop an analytical index that could be used by the water industry to measure, monitor, and control the price of water. A structured method to evaluate membrane manufacturing costs against the lifetime performance of membranes was developed. The method was then extended and a costing model for the application of membrane in water and wastewater treatment plants was established. Thereafter, a software application was developed to aid in the implementation of the analytical index. The results indicate that, with a measurable index in place, the evaluation of technologies with respect to the cost of water production can be effectively carried out.

The uptake of latent tuberculosis infection treatment (LTBI) is an important measure to prevent active tuberculosis (TB), but has not been well studied in low and middle-income countries (LMICs). The lifetime risk of developing active TB for people with LTBI is 10-15%. Within the national guidelines of National Tuberculosis control Program (NTP) 2020 in Georgia, 6 months isoniazid preventive treatment (IPT) was replaced with 3 months rifapentine treatment. Here we present the results of the care cascade of LTBI treatment of subjects to whom the Tuberculosis Preventive Treatment (TPT) in Georgia was recommended. We performed a cohort-study during 2020-2021 in which we applied a mixed method recommended for TPT. We assessed active pulmonary TB cases to whom IPT was recommended and in cases not completing a tuberculosis preventive treatment, the respective reasons. Among 678-contactedsubjects, 54% were female and 46% were male. There was no age limit. Overall 164 participants initiated TPT, among them 107 completed the treatment. 72% from all participants were in close contact with TB-patients/ index cases and were living in the same household. 49% of index cases were positive to culture test (culture test involves studying bacteria by growing the bacteria on different substances) and 45% were Drug Sensitive Tuberculosis (DST) positive. 23 cases of index cases were MultiDrug-Resitant (MDR) TB patients. 95 (14%) participants were treated at the National Center for Tuberculosis and Lung Diseases, other participants were treated at other regions of Georgia (Kakheti, Kutaisi, Rustavi, Gori). From 22 chil-dren under 5 years of age who were recommended for the Tuberculosis Skin Test (TST), 2 were positive. Our findings highlight very low rates of LTBI treatment recommendation, initiation and completion in Georgia and hence the need for improved monitoring and treatment programs. Review question / Objective: Population:Patients diagnosed as obesity with insulin resistance. Obesity reference: Consensus of experts on the Prevention and treatment of adult obesity in China in 2011 and Consensus of Chinese experts on medical nutrition therapy for overweight/obesity in 2016 were developed by the Obesity Group of Chinese Society of Endocrinology(CSE); BMI≥28. IR reference: According to the Expert opinions on insulin resistance evaluation published by Chinese Diabetes Society, HOMA-IR≥2.68 is regarded as the standard for the diagnosis of IR. Regardless of age, gender and course of disease. Patients diagnosed as obesity with insulin resistance. Intervention:Any kind of acupuncture, moxibustion, acupuncture+moxibustion, warm acupuncture, electropuncture, auricular point, acupoint application and acupoint catgut embedding. Comparison:Other acupuncture treatments, Drug therapy or blank control. Outcome:Primary outcomes: ①Fasting blood-glucose (FBG); ②Fasting serum insulin (FINS); ③Homeostasis model assessment-IR (HOMA-IR); ④Body Mass Index (BMI). Review question / Objective: P:Liver cirrhosis patients with risk factors associated with HE attack;HE patients caused by chronic liver diseases represented by cirrhosis. I: Rifaximin treatment. C: Other drugs or placebo. O:HE incidence; HE improvement; All-cause mortality; Blood ammonia level; PSE index; mental state; NCT-A; NCT-B; Adverse events. Condition being studied: Hepatic encephalopathy(HE) is a neuropsychiatric disorder syndrome based on metabolic disorders, which is caused by severe acute and chronic liver dysfunction or various abnormalities of portosystemic shunt (hereinafter referred to as portosystemic shunt). The research data shows that the prevalence of OHE in patients with cirrhosis is 10-14%, and the prevalence of HE in patients with decompensated cirrhosis is 16-21%. HE can lead to 60-80% of patients with liver cirrhosis with mild cognitive impairment, affecting their ability of daily life and quality of life. When OHE occurs, the one-year mortality rate of patients with liver cirrhosis is 64%, which brings a heavy economic burden to patients and public health resources. Therefore, the prevention and early management of HE is very important.

Review question / Objective: The aim of this systematic review is to evaluate the effect of TNF inhibiting treatment on arterial stiffness (as measured with pulse wave velocity and augmentation index) and arterial wall thickness (as measured with carotid intima media thickness) in rheumatoid arthritis patients. Condition being studied: Rheumatoid arthritis is a chronic autoimmune disorder, which affects approximately 1% of the population worldwide. Information sources: The following electronic databases will be searched for potentially eligible studies: EMBASE, MEDLINE, ClinicalTrials.gov and WHO International Clinical Trials Registry Platform. For the studies identified as eligible for inclusion, similarity tracking will be used to identify more potentially relevant articles with the ‘related article’ feature in PubMed. In addition, a citation search will be performed for included studies to identify articles that have cited them. Reference lists of the included studies and previous reviews on the subject will be searched for potentially relevant studies. ResearchGate profiles of top authors on the subject will be investigated to identify potentially relevant data points. For ongoing or finished studies that are potentially eligible, but without a publication, study authors will be contacted for information. Review question / Objective: To compare and rank the clinical effects of Non-Pharmacological Therapies for Psychological State of Medical Staff in the Post-epidemic Eradifferent. Eligibility criteria: The published randomized controlled trials (RCT) of non-Pharmacological Therapies for Psychological State of Medical Staff in the Post-epidemic Era, regardless of age and gender. Patients had clear diagnostic criteria to be diagnosed. Interventions in the treatment group included were various types of non-pharmacological therapies, including various types of acupuncture therapies (such as simple acupuncture, electroacupuncture, warm acupuncture, acupuncture catgut embedding, Auricular therapy, or the combination of acupuncture and other Non-Pharmacological Therapies), meditation, Baduanjin, Tai Chi, aerobic exercise, yoga, psychotherapy, music therapy, etc.; the control group was conventional treatment groups or different non-pharmacological therapies compared with each other. The results of the report are required to include at least one of the following outcome indicators: The self-Rating Anxiety Scale (SAS), the Self-Rating Depression Scale (SDS), the Pittsburgh sleep quality index (PSQI), and effectiveness rate. The language of the publication is limited to Chinese or English.
