Texto completoTraditionally it had been believed that patients diagnosed with bipolar disorder experienced a complete clinical recovery between acute episodes and did not suffer from an impairment in the general functionality, unlike patients diagnosed with schizophrenia. However, in the last decade of the twentieth century and especially the first of the twenty-first century there has been a significant increase in the number of studies that have shown that these assumptions were wrong. We now know that bipolar patients show neurocognitive deficits and psychosocial impairment even in phases of clinical stabilization, but recent studies detect that these patients also have alterations in social cognition (SC). This project aimed to study the neurocognition, social cognition and social functioning in bipolar patients and healthy first-degree relatives, and to carry out this objective two studies were designed. The first study tried to analyze whether the cognitive impairment of euthymic bipolar patients was related to the clinical course of these. The working hypotheses were that patients with higher number of relapses in general, and especially in manic phase would get worse on neurocognitive outcome evaluations. We also hypothesized that patients with a history of major relapses presented with psychotic symptoms worse neurocognitive performance. To carry out this study 26 bipolar patients were recruited over a period of euthymia for at least one month, from the Center for Mental Health Hospital Consortium of Vic. Neurocognitive performance of the 26 patients were evaluated and clinical information was collected for the study of patients' records. The results of this study showed that patients who had a history of more phases of mania and a greater number of previous admissions to psychiatric units had worse neurocognitive functioning, particularly in the area of processing speed. The second study objective was to study social cognition in a sample of euthymic bipolar patients and a sample of first-degree relatives. It also tried to analyze the level of social functioning of both groups and observe the interplay between social cognition and social functioning. Finally, we proposed to compare different assessment tests from the SC in order to see whether the more complex and naturalistic are more useful for the study of SC in bipolar patients (presumably with more subtle deficits). For this study a sample of 31 euthymic bipolar patients attending the Centre for Mental Health Hospital Consortium of Vic, 18 healthy first-degree relatives and 31 healthy controls were recruited. The three study groups with neuropsychological tests, SC tests and Social Functioning test were evaluated. Clinical variables evolution histories of bipolar patients were also collected. The results of this study showed that euthymic patients with BD and first-degree relatives had deficits in ToM that went beyond those associated with poorer neurocognitive performance. In our sample of bipolar patients drug treatment, duration of illness and a greater number of depressive relapses were associated with worse ToM. Furthermore, we found that bipolar patients had worse social functioning than the group of first-degree relatives and the healthy control group, and that the deficit in the social functioning of the patient sample was associated with depressive symptoms at the time of study and with a larger number of periods of euthymia with subdepressive symptoms.

Texto completoEl objetivo del presente estudio es establecer si existe asociación entre los niveles de inteligencia fluida y cognición social en niños y niñas de 11 a 14 años con y sin trastorno de conducta y observar si existen diferencias entre ambos grupos. Para llevar a cabo la investigación se realizó un estudio cuantitativo, usando el método experimental con un enfoque retrospectivo. La muestra estuvo constituida por niños y niñas de 11 a 14 años de edad, pertenecientes a colegios municipalizados con altos índices de vulnerabilidad escolar, residentes de la comuna de Maipú que se atienden en dos Cesfam de atención primaria dependientes del Municipio de Maipú y en la Corporación de salud Mental Familiar de la comuna. (Cosam – Maipú). La muestra fue de tipo no probabilístico y por conveniencia y con un diseño de tipo ex post facto, transeccional o transversal. Tanto a la muestra experimental como control, ambas con 33 participantes equiparados por sexo, se le aplicaron instrumentos para medir las variables en estudio. Para la variable inteligencia fluida se utilizó el Test de Matrices de Raven y para la variable cognición social se utilizaron las técnicas test de los ojos o miradas de Baron – Cohen y el test de empatía IRI de Davis. (Índice de reactividad interpersonal) Al distinguir entre ambos grupos, con y sin trastornos de conducta, se observaron diferencias significativas para las variables inteligencia fluida y cognición social, en cuanto al componente empatía, sólo en el subcomponente cognitivo de fantasía (test IRI). No se observaron diferencias significativas en cognición social, en el componente teoría de la mente ni en los otros subcomponentes de la empatía, tales como: toma de perspectiva, preocupación empática y malestar personal (test IRI). Por otra parte, si bien estadísticamente resulta significativa la diferencia en el subcomponente Fantasía, esta diferencia no se torna significativa de acuerdo a un criterio clínico, ya que implicaría que los niños con trastorno de conducta son menos fantasiosos que los niños sin trastorno de conducta. Tampoco es relevante que el grupo sin trastorno de conducta sea fantasioso, puesto que esto es esperable debido al factor edad

Texto completoCognitive reserve refers to the brain’s capacity to cope with pathology in order to minimize symptoms. The concept of cognitive reserve was developed in the context of dementia and to date has gained wide interest in the field of psychiatry. Higher cognitive reserve is a positive moderator and is associated with a later onset of psychosis and better recovery. However, there are few studies about cognitive reserve and mental disorder and to the best of our knowledge, none of the previous longitudinal studies on cognitive reserve focused on an adult sample of first-episode psychosis. A limitation present in all cognitive reserve studies undertaken on a psychiatric population is that cognitive reserve has been estimated using heterogeneous methods due to the as yet unavailability of a valid instrument to measure cognitive reserve in this population. The objective of this thesis is to characterize cognitive reserve in patients with a first-episode of psychosis, analyzing the differences between two diagnostic groups (affective and non-affective psychoses) and the role of cognitive reserve in each of them. Likewise, to develop and investigate the psychometric properties of the Cognitive Reserve Assessment Scale in Health (CRASH), developed specifically to measure cognitive reserve in people with severe mental illness. The role of cognitive reserve was analyzed and higher scores for cognitive reserve were observed in the control group than in the patient group. The cognitive reserve proxy was able to predict functional, clinical and cognitive outcomes at baseline and at 2-year follow-up in a first episode psychosis sample, playing a differential role according to the diagnosis (affective vs non-affective psychosis). The results of the analysis of reliability and validity of the CRASH scale revealed a strong internal consistency and good convergent validity. In conclusion, cognitive reserve is a key concept in order to understand the clinical, functional and neuropsychological outcomes of patients with a first episode of psychosis and may aid in the implementation of personalized interventions. The CRASH scale is expected to allow for the homogeneous assessment of the concept of cognitive reserve in the psychiatric population.

Texto completoLa disfunción cognitiva representa el mayor obstáculo para la recuperación funcional de los pacientes con trastorno bipolar (TB). El objetivo de esta tesis es describir la evolución de la disfunción cognitiva en pacientes con TB y su relación con diferentes marcadores neurobiológicos. Se realiza un estudio longitudinal de pacientes con TB eutímicos apareados con controles sanos (n=54), analizando variables clínicas, cognitivas y funcionales en un periodo de seguimiento de 6 años. Se estudia también un subgrupo de excelentes respondedores a litio (ERL) (n=20). Se estudia el impacto de la obesidad a nivel transversal y longitudinal. Por último, incluyendo pacientes en fase maníaca (n total=133), se estudian los niveles de biomarcadores periféricos. La persistencia de la disfunción cognitiva se relaciona con una peor funcionalidad psicosocial. El subgrupo de ERL, también presentan déficit cognitivo y funcional sostenido. Factores neurobiológicos como la obesidad y las neurotrofinas modulan la cognición. Estos resultados permiten comprender más la naturaleza de la disfunción neurocognitiva en el TB.
Cognitive dysfunction represents the greatest barrier to functional recovery of patients with bipolar disorder (BD). The aim of this thesis is to describe the evolution of cognitive dysfunction in patients with BD and its relation with different neurobiological factors. A longitudinal study of euthymic patients with BD matched with healthy controls (n = 54) was performed analyzing clinical, cognitive and functional variables in a 6-year period of follow-up. A subgroup of excellent lithium responders (ELR) (n = 20) were also studied. The impact of obesity was studied cross-sectionally and at long-term. Finally, including patients in the manic phase (n = 133), the levels of peripheral biomarkers were analysed. The persistence of cognitive dysfunction was associated with poorer psychosocial functioning. The ELR subgroup also displayed cognitive and functional deficits at long-term. Neurobiological factors such as obesity and neurotrophins modulate cognition. These results allow a better understanding of the nature of neurocognitive dysfunction in BD.

Texto completoThe neurodevelopmental disorders are defined by the Diagnostic and Statistical Manual of Mental Disorders (DSM-5; APA, 2013) as a group of conditions with onset in the developmental period. The disorders typically manifest early in development, often before the child enters grade school, and are characterized by developmental deficits that produce impairments of personal, social, academic, or occupational functioning. The early assessment and monitoring of the neuropsychological development is especially important with children presenting alterations, delays or biological and/or social risk factors. Mainly because of the difficulties in the normal progress of the neuropsychological functions imply a limitation in the exploration and comprehension of the environment. The early detection of the development alterations enables the beginning of the attention and support to parents and infants, which has proved to be beneficial on reducing the effects in the long term. The fundamental goal of this investigation was to verify the validity and utility of the Bayley Scales of Infant and Toddler Development, Third Edition (Bayley-III) (Bayley, 2006) as a cognitive and language assessment instrument in the context of early diagnosis of different subtypes of communication disorders (CD) (phonological disorders, expressive language disorders and mixt receptive-expressive language disorders) and different subtypes of autism spectrum disorders (ASD), defined by the non-verbal cognitive level and by the level of oral language acquired at 4 years old. The Bayley-III Scales are an international benchmark instrument on early development assessment and evaluate the cognitive, language and motor development of infants and toddlers between 1 and 42 months old. This research was designed as longitudinal study with the purpose of achieving the goals stablished for each one of the analyzed populations. The first study proved that the Bayley-III scales are capable of detecting discrepancies between the cognitive level and the language level before the first 42 months in the population with CD. The results indicate that children with CD scored significantly lower than their control groups in all subtests and compounds of the Bayley-III. Additionally, low scores on the Language Composite in the Bayley-III predicted lower scores in the Auditory-vocal Channel of ITPA. A significant correlation was obtained between the Cognitive Scale from the Bayley-III and the General Cognitive Scale of MSCA and of the Mental Processing Composite of K-ABC. The second study confirmed the dependency between the nonverbal cognitive level and the oral language level acquired at 4 years old. Lower scores on the Cognitive and Language Bayley-III scales at 3.5 years old predicted lower cognitive and oral language levels at 4 years old. A significant correlation between the Cognitive Bayley-III Scale and the General Cognitive MSCA Scale and with that of Compound K-ABC Mental Processing was found. Regarding the more practical implications of the presented research we can conclude that the Cognitive and Language scales of the Bayley-III are a useful instrument in the early diagnosis of CD, also able to discriminate its more severe forms. Additionally we can conclude that the Bayley-III Scales are a useful instrument in cognitive and language assessment of ASD. Ultimately the Bayley-III scales can help clinical professionals to attain knowledge into the differentiation between different subtypes and severity levels of neurodevelopment disorders, providing more preciseness on the healthcare decision making process.

Texto completoUn número importante de los síntomas del trastorno límite de la personalidad se manifiestan en el contexto interpersonal, concretamente, por dificultades en la cognición social. El propósito de este estudio es analizar la cognición social en mujeres con trastorno límite de la personalidad, comparado con un grupo control, y estudiar la relación con psicopatología asociada y maltrato en la infancia. La cognición social se evalúa con la Película para la Evaluación de la Cognición Social (MASC; Movie for the Assessment of Social Cognition, Dziobek et al., 2006), instrumento reciente con elevada validez ecológica y validada al español (Lahera et al., 2014). Método: Muestra compuesta por 79 mujeres, 47 diagnosticadas de trastorno límite de la personalidad y 32 constituyeron el grupo control. Se realizaron comparaciones entre pacientes y grupo control en relación con la cognición social. La asociación entre trastorno límite de la personalidad, psicopatología y maltrato se estudió a través de correlaciones, comparación de medias y regresión lineal. Conclusiones: Peor desempeño de la cognición social en las mujeres con trastorno límite de la personalidad comparado con grupo control. La severidad de la sintomatología límite se relacionó con peor funcionamiento de la cognición social general y con ausencia de mentalización. El área cognitiva y el malestar psíquico global se vincularon a fallos en cognición social (en general y en áreas cognitivas) y a errores por hipermentalización. Determinada psicopatología en el trastorno límite de la personalidad se relacionaba con dificultades en la cognición social. Maltrato global y abuso sexual se asociaron a mayor afectación de teoría de la mente cognitiva y negligencia física a errores por ausencia de mentalización. Finalmente, la alteración cognitiva en el trastorno límite de la personalidad predijo en un 26% las alteraciones en la cognición social correcta y el psicoticismo en un 20,8% los errores por hipermentalización.
A significant number of the symptoms of borderline personality disorder appear in the interpersonal context, specifically, as difficulties in social cognition. The purpose of this study is to analyse social cognition in women with borderline personality disorder, in comparison to a control group, and to study the relationship with associated psychopathology and child abuse. Social cognition was assessed with the Movie for the Assessment of Social Cognition (MASC, Dziobek et al., 2006), a recent instrument with high ecological validity and authenticated in Spanish (Lahera et al., 2014). Method: A sample of 79 women, 47 diagnosed with borderline personality disorder and the other 32 made up the control group. Texto completoLos estudios genéticos en el marco de la psicosis se complementan con los de índole más ambiental. La complejidad de los primeros a la hora de poder tratar a nuestros pacientes y mejorar su pronóstico hacen que trabajos como el presente se lleven a cabo. Hallar las relaciones que se establecen entre el ambiente familiar, la cognición y los rasgos de personalidad en los pacientes psicóticos es un primer paso para que estudios posteriores puedan hallar qué aspectos modificar a nivel de personalidad y de ambiente familiar para optimizar el rendimiento cognitivo. El objetivo principal de esta tesis es esclarecer las relaciones establecidas entre los rasgos de personalidad, el ambiente familiar y la función cognitiva que se dan en los pacientes con un trastorno psicótico. Para ello se ha recogido una muestra total de 72 pacientes con sus familiares y se les ha administrado una batería de pruebas para evaluar los tres aspectos mencionados: el WCST, los subtest de dígitos y letras y números del WAIS-III, TMT A y B, el test STROOP, el Índice SUA, el NEO-PIr, TCIr, y la escala de ambiente familiar FES. El factor apertura del NEO-PI-R aparece como un factor positivo para el mantenimiento del funcionamiento de dicha función cognitiva, y además, se relaciona de forma inversa con la sintomatología negativa. Los resultados obtenidos parecen indicarnos que estamos ante un espectro de la psicosis y no ante categorías diagnósticas. El énfasis moral-religioso en el núcleo familiar se asocia con un peor rendimiento cognitivo en atención sostenida y cognición global. Un ambiente familiar con elevada organización y control implica una peor flexibilidad cognitiva en los pacientes psicóticos. Un ambiente familiar con una elevada cohesión, expresividad y conflicto también implican un peor rendimiento cognitivo global. También existen rasgos de personalidad que se relacionan con características del clima familiar.
Genetic studies in the context of psychosis are complemented by the more environmental nature. The complexity of the first when it comes to treating our patients and improve its forecast made that works such as are carried out. Find relationships between family environment, cognition and the traits of personality in psychotic patients is the first step so that further studies can find which aspects modify level of personality and family environment to optimize cognitive performance. The main objective of this thesis is to clarify the relationship between the traits of personality, the family environment and cognitive function that occur in patients with a psychotic disorder. This is picked up a total sample of 72 patients with their families and has been administered a battery of tests to assess the three aspects mentioned: the WCST, the subtest of digits and letters and numbers of the WAIS-III, TMT A and B, the STROOP test, index SUA, the NEO-PIr, TCIr and FES family environment scale. Opening factor appears as a positive factor for the maintenance of the functioning of the cognitive function, and in addition, relates in reverse order with the negative symptoms. The results seem to indicate that there is a spectrum of psychosis and not diagnostic categories. The moral-religious emphasis on the nuclear family is associated with worse cognitive performance in sustained attention and global cognition. A family atmosphere with high organization and control involves a worse cognitive flexibility in psychotic patients. A family atmosphere with high cohesion, expressiveness and conflict also imply a worse overall cognitive performance. There are personality traits that are related to characteristics of the family environment.

Texto completoPatients with Bipolar Disorder (BD) show broad cognitive impairments that persist during euthymia despite remission of mood symptoms. Cognitive deficits in BD are associated with impairments in functional outcome and quality of life. Thus, improving cognitive functioning is an important treatment goal in people with BD. Bipolar II disorder subtype (BD-II) has been underdiagnosed in clinical practice, therefore, many aspects of BD-II are still understudied such as neurocognitive functioning. In the present thesis, a systematic review of literature regarding neurocognition in BD-II was carried out. Then, we assessed a sample of strictly defined BD-II euthymic patients to compare it with a sample of healthy controls (HC) to elucidate the neuropsychological profile of this bipolar subtype. A second objective was to study the relationship between neurocognitive performance and functional outcome. BD-II patients showed a significantly lower performance on several measures of attention, learning and verbal memory, and executive function compared with HC. The presence of subthreshold depressive symptomatology and one measure related to executive function and working memory were the variables that best predicted psychosocial functioning. After that, we attempted to analyse cognitive variability also in a sample of euthymic BD-II patients, using a cluster analysis to provide groups of different neurocognitive patterns. Three clusters were detected: an intact neurocognitive group, an intermediate or selectively impaired and a globally impaired group. Approximately one-half of the patients were cognitively impaired. The 3 clusters statistically differed in the premorbid intelligence quotient, the global functional outcome and leisure activities. The globally impaired cluster showed the lowest attainments concerning these 3 variables, which are considered proxies of cognitive reserve. No differences in other clinical characteristics were found among the groups. Taking all these data into account, it seems necessary to implement therapies focused on enhancing functional outcome in bipolar II disorder patients. Hence, we carried out a subanalysis aimed to assess the efficacy of an intervention called Functional Remediation specifically in a sample of BD-II patients. Our results suggest that the Functional remediation appears to be effective in improving the overall functional outcome in BPII, as well as in reducing subclinical depressive symptoms.
