Background and PurposeThe World Health Organization's International Classification of Functioning, Disability and Health (ICF) is gaining recognition in physical therapy. The Extended ICF Core Set for Stroke is a practical tool that represents a selection of categories from the whole classification and can be used along with the ICF qualifier scale to describe patients’ functioning and disability following stroke. The application of the ICF qualifier scale poses the question of interrater reliability. The primary objective of this investigation was to study the agreement between physical therapists’ ratings of subjects’ functioning and disability with the Extended ICF Core Set for Stroke and with the ICF qualifier scale. Further objectives were to explore the relationships between agreement and rater confidence and between agreement and physical therapists’ areas of core competence.Subjects and MethodsA monocentric, cross-sectional reliability study was conducted. A consecutive sample of 30 subjects after stroke participated. Two physical therapists rated the subjects’ functioning in 166 ICF categories.ResultsThe interrater agreement of the 2 physical therapists was moderate across all judgments (observed agreement=51%, kappa=.41). Interrater reliability was not related to rater confidence or to the physical therapists’ areas of core competence.Discussion and ConclusionThe present study suggests potential improvements to enhance the implementation of the ICF and the Extended ICF Core Set for Stroke in practice. The results hint at the importance of the operationalization of the ICF categories and the standardization of the rating process, which might be useful in controlling for rater effects and increasing reliability.

Abstract Introduction Pediatric pilocytic astrocytomas (PA) are the most common pediatric central nervous system tumor. Surgical resection is the primary treatment for PA with five-year survival rates up to 95%. Despite a favorable prognosis, our understanding about the prognostic value of histopathological findings, such as histopathologic qualifier* or BRAF alterations is evolving. Methods Patients treated for a WHO grade 1 PA at Washington University in St. Louis/St. Louis Children’s Hospital were analyzed for clinical details, including pathology diagnosis (*histopathologic qualifier refers to designations in the diagnosis such as “WHO Grade I pilocytic astrocytoma with increased proliferative index”). BRAF alterations include gene fusions and point mutations. Results 224 patients were analyzed (51% female, mean age 9.6 years). Tumors were located in the cerebellum/fourth ventricle (50%), optic pathway/hypothalamus (15%), brainstem (12%), and cerebral cortex (11%). BRAF alterations were identified in 55/77 patients (71.4%) and additional histopathologic qualifiers were present in 27/220 patients (12.3%). 196 patients (87.5%) underwent surgical treatment and 22 (9.8%) had biopsy alone. 45 patients (22%) displayed tumor progression or recurrence after resection. The presence of a histopathologic ‘qualifier’ in the topline or BRAF alteration was not associated with tumor progression or recurrence (p=0.36, p=0.77). Ki-67 proliferative indices were not predictive of progression or recurrence (p=0.94), including when controlling for extent of resection and adjuvant therapy. BRAF alterations, specifically KIAA1549 fusions, were associated with cerebellar/fourth ventricular tumor location (p<0.001) and younger patient age (p=0.03). Extent of resection was the only predictor of outcome identified in this study; patients with gross total resection had significantly lower rates of progression and recurrence (p<0.0001). Conclusion BRAF alterations and histopathologic qualifiers were not associated with tumor progression or recurrence in pediatric PA, although BRAF fusions were more common in tumors located in the cerebellum/fourth ventricle and in younger patients.

(1) Background: Secondary acute myeloid leukemia (AML), i.e., AML arising from prior therapy (therapy-related) and/or an antecedent hematologic disorder (AHD) is generally associated with worse outcomes compared to de novo AML. However, recognizing the prognostic importance of genetic characteristics rather than clinical history, secondary AML is now considered a diagnostic qualifier rather than a separate disease entity. (2) Methods: To assess the association between clinical history and AML outcomes in the context of allogeneic hematopoietic cell transplantation (HCT), we retrospectively analyzed 759 patients with de novo AML, 115 with AHD AML, and 105 with therapy-related AML who received first allografts while in first or second remission. (3) Results: At the time of HCT, these three cohorts differed significantly regarding many patient and disease-specific characteristics, including age (p < 0.001), gender (p < 0.001), disease risk (p = 0.005), HCT-CI score (p < 0.001), blood count recovery (p = 0.003), first vs. second remission (p < 0.001), remission duration (p < 0.001), measurable residual disease (MRD; p < 0.001), and conditioning intensity (p < 0.001). Relative to patients with de novo AML, relapse rates were similar for patients with AHD (hazard ratio [HR] = 1.07, p = 0.7) and therapy-related AML (HR = 0.86, p = 0.4) after multivariable adjustment, as were relapse-free survival (HR = 1.20, p = 0.2, and HR = 0.89, p = 0.5) and overall survival (HR = 1.19, p = 0.2, and HR = 0.93, p = 0.6). Non-relapse mortality was higher for AHD AML (HR = 1.59, p = 0.047). (4) Conclusions: These data suggest that the clinical history by itself contains limited prognostic value for adults with AML undergoing allografting, supporting the most recent approach to use this information as a diagnostic qualifier rather than a disease entity.

Background: Children with central nervous dysfunction should be given a variety of long-term physical and occupational therapy interventions to facilitate their self-development and to enhance functional independence in movement, self-care, play, school activities and leisure. The present study aimed to investigate the benefit of implementing context-focused intervention approach along with occupational therapy intervention in children with CNS dysfunctions.Methods: This randomized case control study conducted in 30 children with CNS dysfunction aging between 12 months to 48 months were randomly divided into control and experimental groups consisting of 15 children in each group. Control group received conventional occupational therapy for 45 min, and experimental group received context-focused intervention approach for 30 min along with conventional occupational therapy for 15 min (thrice a week). The patients were evaluated at baseline, after 12 and 24 weeks of follow up period on Canadian occupational performance measure (COPM) to evaluate parental perception about intervention, GMFM (gross motor function measure) to study the motor ability of the patient and ICF-CY (International classification of functioning, disability and health for children and youth) for activity participation and performance with evaluation of context.Results: Significant progress in the satisfaction score during 2nd and 3rd follow ups were noted (p <0.001) in experimental group. All the activities in GMFM score except lying and rolling showed significant improvement in experimental group (p <0.001). In experimental group a significant improvement in performance qualifier of ICF was seen during follow ups (p<0.001). The capacity qualifier scoring of ICF was improved in experimental group during 2nd follow up (p = 0.006). Progress in environmental scores of ICF during all the follow ups was noted (p <0.001) in experimental group. Significant correlation in GMFM scores and COPM performance score was noted during last follow-up in both the groups.Conclusions: The findings of the study showed positive results with context focused therapy and thus can be inferred that new intervention approach context focused therapy along with the conventional occupational therapy is very effective in improving the performance of functional tasks in children with CNS dysfunction.

Study Objective: Studies have shown increased prevalence of restless legs syndrome (RLS) in sleep disordered breathing (SDB), however limited data have focused on the impact of SDB therapy on RLS. We hypothesize that positive airway pressure (PAP) will improve the International Restless Legs Syndrome (IRLS) score among SDB patients compared to patients without PAP. Methods: Patients with AHI ≥ 5 who responded positively to a RLS qualifier question from January 2010 to May 2015 were included in this retrospective study. IRLS score was used to measure RLS symptom severity. Two-sample t-tests and one-way analysis of variance were used to compare changes in IRLS score and linear regression models were created to examine IRLS change with PAP use and PAP adherence (PAP usage ≥4 h nightly for ≥70% of nights), adjusting for potential confounders. Results: In 434 patients (51.9 ± 13.4years, 50.5% female, 77.6% Caucasian; 325 PAP, 109 control), IRLS scores improved from baseline to follow-up, with the PAP group achieving significant improvement after adjustment for covariates (difference in IRLS: −1.8 (CI −3.6,0.00), p = 0.050). In self-reported PAP adherent patients, IRLS improvement was greater than controls (−5.3 ± 7.4 vs. −2.7 ± 7.6 respectively, p = 0.045), and comparable to non-adherent patients (−5.3 ± 7.4 vs. −3.0 ± 7.0, p = 0.091). Conclusions: Among SDB patients with a positive RLS qualifier, those who used PAP therapy achieved significantly greater improvement in IRLS scores than patients who did not use PAP, with more significant changes in the PAP adherent group. This is the first large clinical study to examine these relationships, providing a basis for future prospective interventional trials and informing clinicians of expected improvement in IRLS score in PAP treated SDB populations.

La situation d’apprenant induit des phénomènes que l’on pourrait qualifier d’émersifs. De très nombreuses études se sont intéressées aux corrélats neuro-végétatifs de l’apprentissage de tâches cognitives, en particulier dans le cadre des apprentissages scolaires. Il y a cependant très peu de résultats qui concernent l’apprentissage moteur des habiletés complexes. Nous avons enregistré le rythme cardiaque (HR) et la réponse électrodermale (EDR) de participants impliqués dans une tâche d’adaptation à une perturbation visuo-manuelle. Vingt et un adultes droitiers ont effectué une tâche de pointage de cibles statiques avec une tablette graphique. Après une période de pré-test, nous avons introduit une discordance inattendue en rotation entre l’espace moteur (tablette) et l’espace visuel (écran), discordance disparaissant après une période d’adaptation. Quatre participants furent exposés à une perturbation faible (5°) alors que tous les autres subirent une perturbation plus importante (60°). Nous avons trouvé des changements significatifs de HR et de l’EDR lors de l’apparition et disparition de la perturbation, mais aussi lors de la phase d’adaptation dans le groupe 60°. Nous avons en particulier mis en évidence une baisse significative de HR et une augmentation significative de l’EDR lors de l’apparition de la rotation. Ces résultats confirment l’existence d’une réponse neurovégétative notable durant l’apprentissage moteur et ouvrent la voie à l’étude des corrélats neurovégétatifs de l’apprentissage moteur dans le cadre d’habiletés plus complexes et à leur interprétation dans le cadre de l’émersion.

The poly ADP ribose polymerase olaparib is currently approved in front line BRCA-associated epithelial ovarian cancer (EOC), platinum-sensitive recurrence agnostic to BRCA status and for gBRCA as treatment in the fourth line and beyond. Women who are diagnosed with advanced stage EOC face a formidable challenge in overcoming their disease and achieving long-term, disease-free survival. The qualifier here is disease free. EOC is largely exquisitely chemosensitive, especially in the treatment naive (first line) setting and the expectation is that the vast majority of women will complete front line platinum-based chemotherapy with a response. When unselected (not selected by BRCA) women are enrolled on clinical trials, the response rate among those who have measurable disease at the time of chemotherapy initiation is 48% for carboplatin/paclitaxel and 67% for carboplatin/paclitaxel plus bevacizumab. When one considers the addition of women who start chemotherapy without measurable disease, they will likely also end chemotherapy without measurable disease and the overall rate of no evidence of disease at conclusion of chemotherapy approaches 80%. Despite this, the majority of women will suffer relapse of their disease, typically within the first 3 years following completion of therapy. Once recurrent, the disease is highly treatable for many years but no longer considered curable. This review will cover indications for olaparib in ovarian cancer as well as ongoing combination trials and rationale for these combinations.

Background: In 2022, two new acute myeloid leukemia (AML) classifications (World Health Organization, WHO and International Consensus Classification, ICC) and a new edition of the European LeukemiaNet (ELN) prognostic system have been released. These advances have been driven by the need to incorporate molecular characterization into the diagnosis and management of AML. Although important similarities in the diagnostic classifications have been recently recognized by diverse validation studies, the different approach to define AML ontogenesis (primary vs secondary) remains a central issue of the classification discrepancy. While for the WHO 2022 a prior diagnosis of myelodysplastic syndrome (MDS) or MDS/myeloproliferative neoplasm (MPN) is sufficient to define AML myelodysplasia-related (MR), the ICC no longer recognizes the AML arising from MDS (sAML) as a disease entity, and instead introduces a list of “diagnostic qualifiers”, also including therapy-related AML (tAML). Nevertheless, whether these “diagnostic qualifiers” represent a mere “descriptive” information or impact on prognosis and outcome is not well defined. Methods: We studied a cohort of 936 AML patients collected through a multicenter international collaboration across 5 academic centers. An integration of clinical and cytogenetic/molecular data was used to apply the WHO 2016, WHO 2022 and ICC diagnostic classifications and the 2017 and 2022 edition of ELN prognostic risk stratification criteria. Results: Overall,220 cases were classified as secondary AML (155 sAML and 65 tAML). As per the2016 and 2022 WHO editions, the majority of sAML cases, accounting for 98.1% and 95.5% respectively, fell into the myelodysplasia-related changes (MRC) AML and AML-MR categories due to the “disease defining” presence of an antecedent MDS or MDS/MPN. According to the ICC, 81.9% of sAML cases were defined by the presence of a MDS-related genetic profile, belonging to AML with mutated TP53 (AML- TP53) and AML with MDS-related gene mutations (AML-MDSgene) and cytogenetic abnormalities (AML-MDSk) subcategories. The tAML group showed a greater diagnostic heterogeneity, with a higher frequency of KMT2A-related categories (10.8% and 7.7% according to WHO 2022 and ICC, respectively) and AML- TP53 for the ICC (21.5%, Fig. 1A). In terms of outcomes, according to the ELN 2022 risk classification the sAML and tAML groups were classified into favorable risk 5.8% and 13.9%, intermediate 12.3% and 36.9%, and adverse 81.9% and 49.2%, respectively. Notably, while the ELN 2022 edition was effective in stratifying the group of tAML patients receiving conventional treatment (p=0.002), no statistically significant differences were noticed when sAML patients were stratified based on the same ELN 2022 risk categories ( Fig. 1B). We then compared our sAML cohort to AML cases with ICC MDS-related profile not fulfilling the anamnestic criterion of a prior MDS diagnosis (233 cases, 32.54% - AML- TP53/MDSgene/MDSk subcategories), in terms of molecular characteristics and outcome. Remarkably, both groups showed a similar prevalence of MDS genes, except for RUNX1, which was more frequent in the AML cases with the ICC MDS-related profile (14%) than in sAML (6.3%, p =0.034). Finally, both groups showed similar outcomes. Conclusions: Here, we demonstrated the overarching importance of the biological ontogenetic profile in AML (&gt;80% of sAML presented an MDS-related profile) and of the risk stratification according to the new classifications (&gt;80% of sAML belonged to adverse ELN 2022 risk). As to outcomes, 2022 ELN edition confirmed its stratification capability for tAML, while sAML were not adequately stratified. This evidence supports the idea that therapy-related AML should be acknowledged and classified as “second” neoplasms, rather than being categorized as secondary AML. Considering the overlap in AML of MDS-related profile regardless of the presence of an antecedent diagnosis of MDS as “diagnostic qualifier”, these cases could potentially benefit from treatments specifically tailored for MDS-related conditions or those typically reserved for secondary AML, such as CPX-351.

In the UK, engagement in personal therapy (PT) is an integral requirement of professional training in counselling psychology. However, despite the importance of PT during training, there is no current requirement for practicing counselling psychologists to continue this post-qualification. The aims of this study were: to explore the reasons that counselling psychologists engage (or not) in PT post-qualification; to understand counselling psychologists’ views of PT as contributing to their professional and personal development; and to explore counselling psychologists’ views of PT as a potential CPD activity. Using Charmaz’s (2006) constructivist grounded theory approach to analyse the data, five major categories were constructed through the analysis: personal growth versus personal crisis; practice what you preach; the ideal therapist; compliance and confusion of compulsory PT as trainees; and approval, ambivalence and constraints of PT as post-qualification CPD. The core constructed theoretical category - diverging attitudes towards the role of post-qualification personal therapy - was considered to represent qualified counselling psychologists’ uncertainty about the role of PT therapy in the context of their own ongoing personal and professional development. Most participants described seeking therapy post-qualification for self-reflection and for emotional support in a time of crisis. Many were in favour of PT as a voluntary CPD activity to be engaged in sporadically for short periods over one’s professional career. However, participants were less in favour of PT as a compulsory CPD activity. The findings are discussed with regards to the relevant literature and the implications they have for the discipline of counselling psychology, for training and for the continuing professional development of counselling psychologists. The findings draw attention to the counselling psychologists’ voluntary engagement with PT post-qualification, and has helped to elucidate how PT is understood, viewed and engaged in, or not, by qualified counselling psychologists as an activity for CPD.

The overall aim of this research is to explore the experiences of newly qualified occupational therapy staff, with reference to professional identity and the role of the preceptorship year in developing this. Using a phenomenological methodology, based on the work of Van Manen (1990), seven newly qualified staff were interviewed using a grand tour question. The subsequent transcripts were analysed using thematic analysis that included a priori coding, as well as codes that emerged from a detailed reading of the data. The findings and recommendations are presented as answers to the two research questions which are: How do newly qualified staff, who have recently been through preceptorship, experience the preceptorship process? In what ways is the preceptorship process perceived as having any influence on the development of professional identity? The findings indicate that preceptorship is perceived to be important in the development of professional identity as part of a continuum, and that the preceptorship period offers an unrivalled time of reflexivity and growth and puts a large emphasis on role modelling. Recommendations include action points specifically developed for the Trust where the research was conducted. They may be useful for other organisations who are providing preceptorship programmes. These recommendations include choice and training for the preceptors. The recommendations also highlight that professional identity is the responsibility of every individual member of the profession.

Syftet med denna studie är att undersöka i vilken utsträckning man kan se studier på en legitimationsgrundande psykoterapeututbildning som ett karriärdrag, och hur en sådan karriär i så fall kan definieras. I studien har 60 legitimerade psykoterapeuter med inriktning familjeterapi som studerat vid Linköpings universitet under åren 2002-2014 (n=87) svarat på en enkät med fasta svarsalternativ, samt några öppna frågor. Resultaten visar att titeln som legitimerad psykoterapeut medfört löneökning för 63 %. Av respondenterna titulerar sig 58 % som legitimerad psykoterapeut, 27 % har fått utökade befogenheter i samband med legitimationen, och 12 % har befordrats. De stora arbetsgivarna även efter avslutad utbildning är kommun och landsting, där 73 % är anställda. En markant ökning har dock skett av antalet egna företagare, från 20 % till 40 %. Den karriär som görs av legitimerade psykoterapeuter är huvudsakligen en expertkarriär, dvs. en karriär där man är expert inom sitt yrkesområde, en horisontell karriär. En utbildning till legitimerad psykoterapeut leder till en ökad grad av eget företagande.
This study aims to examine to what extent following a course of study leading to a diploma in psychotherapy can be seen as a strategic career-related move, and how, in that case, such a career can be defined. The degree entitling the recipient to the Swedish title "legitimerad psykoterapeut" is difficult to translate literally; it is usually considered equivalent to, but is not specified as, an M.Sc. It is officially a degree within the health care field, with concurrent obligations, and is hereafter referred to as "qualified psychotherapist". 60 qualified family psychotherapists who studied at Linköping University between 2002 and 2014 (n=87), replied to a questionnaire containing fixed answer alternatives and a few open questions. The results show that acquiring the title of qualified psychotherapist led to a rise in income by 63%. Of all the respondents, 53% actually use the title, 27% acquired more authority, 12% were promoted, and 33% reported being granted a higher level of responsibility after acquiring the title. Family therapy was reported as the major occupation of 40% of the respondents. Local councils (kommuner) or district health authorities (landsting) employed 73% of the respondents, although there has been a significant increase, from 20% to 40%, in the proportion of family psychotherapists who are also partly or solely self-employed. The career of a qualified psychotherapist is thus largely the career of an expert within the field, a "horizontal" career. Completing the requirements for qualification as a psychotherapist leads to an increased level of self-employment.

In South Africa the Center for Child Youth and Family Studies is training practitioners yearly to work as play therapists from a gestalt approach. Once these practitioners successfully complete their training and qualify they could begin practicing play therapy from a gestalt approach. Each of these recently qualified play therapists is then responsible for organising and committing to their own supervision. Therapists who do not attend supervision risk stagnation and burn out due to a lack of positive interaction in relation to receiving knowledge and support from those more experienced in gestalt play therapy and from their peers. At present there is a perceived lack of supervision resources based on a gestalt approach particularly for those recently qualified play therapists working from a gestalt approach that are geographically distant from the areas where training takes place. This study sought to answer the question of what supervision resources are currently available to recently qualified play therapists working from a gestalt approach in South Africa. In order to answer this question combined quantitative and qualitative research approaches were used. An internet survey questionnaire was completed by recently qualified play therapists working from a gestalt approach which formed part of the quantitative section of the research. Structured interviews were conducted via Skype with professionals experienced in the field of Gestalt therapy theory and supervision from a gestalt approach and this formed part of the qualitative section of the research. Overall the results from the merged data indicate a lack of supervisors qualified to give supervision based on a gestalt approach. This factor contributes to the overall lack of supervision resources based on a gestalt approach for recently qualified play therapists working from this approach. Geographical distance from supervision resources places financial and time constraints on recently qualified play therapists that prevent them from accessing supervision based on a gestalt approach. Despite this results indicate those recently qualified play therapists working from a gestalt approach are attempting to meet their responsibility and requirement for supervision.

This chapter comprises three subchapters, and each subchapter offers a brief introduction to 1 of 3 healing modalities: (1) aromatherapy, (2) equine therapy, and (3) creative arts therapies. All 3 healing processes are considered to be well regarded in the field of addiction treatment. Yet the credibility of each one could benefit from more well-designed, large-scale, randomized controlled trials. Even so, there is significant research supporting the efficacy of each of the 3 treatment modalities. Safety is especially important with impulsive patient populations, such as those with dual diagnoses, addiction and trauma. All three modalities are able to be powerful, and at times transformative, when practiced by qualified health practitioners. For patients who struggle with substance abuse, co-occurring disorders and adverse childhood experiences, especially those who have felt as if they were failed by traditional treatment programs, a non-traditional treatment modality may provide just the right intervention.

Multimodality imaging is vital in any percutaneous cardiac procedure, increasing the precision and accuracy of the procedure to optimize results and reduce complications to a minimum. Three-dimensional transoesophageal echocardiography has progressively become essential, as it provides supportive and precise image information, additive to that obtained by conventional two-dimensional imaging. The rapidly evolving technology to improve visualization and image resolution is aiding the development and performance of these techniques with an increasing need of qualified imaging expertise. This chapter describes the use of transthoracic, transoesophageal, and intracardiac echocardiography approaches in a variety of interventional echocardiography procedures, including valvular heart disease, transcatheter aortic valve implantation, MitraClip® therapy for mitral valve repair, percutaneous closure of paravalvular leaks, and intracardiac shunts.

AbstractThe FACT-JACIE accreditation system is based on a standard-driven process covering all the steps of HSC transplant activity, from donor selection to clinical care. Since the first approval of the First Edition of the Standards in 1998, over 360 HSCT programmes or facilities have been accredited at least once, most of them achieving subsequent re-accreditations (Snowden et al. 2017). The positive impact of the accreditation process in the EBMT Registry has been well established (Gratwohl et al. 2014). Starting with version 6.1, the standards include new items specifically developed for other cellular therapy products, with special reference to immune effector cells (IECs). This reflects the rapid evolution of the field of cellular therapy, primarily (but not exclusively) through the use of genetically modified cells, such as CAR-T cells. FACT-JACIE standards cover a wide range of important aspects that can be of use for centres that aim to be accredited in their countries to provide IEC therapy. Notably, FACT-JACIE accreditation itself is a key (or even a prerequisite) condition in some countries for approval by health authorities to provide commercial CAR-T cell therapy and is also valued by pharmaceutical companies (both those developing clinical trials and those manufacturing commercial products), which also inspect the cell therapy programmes and facilities established at each centre (Yakoub-Agha et al. 2020). Interest in applying for FACT-JACIE accreditation that includes IEC therapeutic programmes is clearly increasing, from four applications in 2017 to 36 applications approved in 2019. The standards do not cover the manufacturing of such cells but include the chain of responsibilities when the product is provided by a third party (Maus and Nikiforow 2017). In any case, all the steps in the process in which the centre is involved (e.g., patient or donor evaluations, cell collection, cell reception, and storage) are covered by the standards, including the appropriate agreements with the internal partners, including the pharmacy department. In addition, from a clinical perspective, IECs may require special safety monitoring systems due to the high frequency of acute adverse events related to the massive immunological reaction against the tumour. Although examples and explanations are found in the standard manual, here, the special importance of identifying and managing cytokine release syndrome (CRS) should be emphasized, and the standards focus not on specific therapeutic algorithms but on ensuring that medical and nursing teams are sufficiently trained in the early detection of this and other potential complications (e.g., neurological complications). They also pay attention to the full-time availability within the institution and its pharmacy of the necessary medication to address complications and the capacitation and involvement of Intensive Care and Neurology Department professionals to provide urgent care if needed. Forthcoming cellular therapy products, currently under investigation, will show a wider range of risk profiles, therefore requiring product-specific risk assessment and consequent adaptation of the clinical procedures for different classes of products. The FACT-JACIE standards will continue to adapt to these future needs to assist centres in their achievement of optimal clinical outcomes.

Abstract The use of music therapy can be crucial for many patients living with cancer. When diag nosed with a life-threatening illness many people feel the need to re-evaluate their lives; to find that creative part that has been suppressed by the realities of day-to-day living. Music therapy, as a co-creative process between patient and therapist, can empower people to find their artistic selves and use their musical expression as a means for exploration and health. Music provides the opportunity to communicate that which cannot be spoken. It can facilitate powerful experiences of reminiscence and distract from the real ities of physical pain. It is the portend of this chapter that music should not be used casually or prescriptively. Often in healthcare settings music is abused; the constant use of background sounds from radios, televisions, and hi-fi systems can become aggravating and potentially stressful. Silence is preferable to the use of uninformed music. Music chosen with and for the patient can, however, be intensely therapeutic. Music therapy as an intricate therapeutic phenomena, should only be practised by qualified and experienced individuals. Music should, however, be embraced and used by all healthcare professionals and is an ideal therapeutic tool that can be used in conjunction with more formal treatment procedures. The empathic use of music, and its potential to inspire the spirit, can enrich all our lives.

In response to the COVID-19 pandemic, there has been a need to adapt and develop psychological interventions that address the mental health of those in need. As a result, Arts for the Blues (A4B), an evidence-based creative group psychotherapy model, originally developed for in-person delivery to address the needs of clients with depression, was transformed into a remote therapy option. This chapter presents an overview of plans and steps so far and offers activities used online during a public workshop with 24 participants and training sessions with 70 psychotherapists (qualified and trainee). Concerns around safety, group sizes, time, and guidance/support are discussed, while the value of online work for clients with depression (adults and children) are explored. It is concluded that even when in-person delivery is possible, online versions will be useful since they encourage a wider reach and make interventions more accessible.

The modern development and improvement of mathematical models that describe the radiobiology of processes in the body occurring under the influence of radiation every year lead to more complicated calculations related to the estimation of its impact both on the effectiveness of radiotherapy and on the possibility of making changes to the radiation treatment regimen. This significantly increases the time spent by medical physics and radiation oncologists and also requires special training of qualified specialists capable of performing such calculations. The aim of the study is to optimize calculations related to the estimation of radiation doses when the radiation treatment schedule changes, by modeling such changes with specially designed software on the basis of the theory of a linear-quadratic radiobiological model. The Web application is accessed via the Internet link https://hypo-calc.github.io/. As an example of using the Web application, the possible cases in clinical practice are considered.

Diabetic foot syndrome (DFS) is a common sequel of long-term diabetes mellitus. There is a urgent need of noninvasive, objective and quantitative diagnostic tools to assess tissue viability and perfusion for a successful therapy. NIR spectroscopy seems to be qualified to measure local capillary hemoglobin saturation of the outer extremities in patients with progressive diabetic disorders. We investigate how NIR spectroscopy can be applied to the assessment of diabetic foot problems such as neuropathy and angiopathy. Thereby we use spatially resolved spectroscopy in conjunction with a specially developed continuous-wave laser spectrometer. Comparison of intra- and interindividual measurements is expected to yield quantitative measures of local tissue viability which is a prerequisite for a successful therapy.

The practicians in the mental health field are living fascinating times. The potential clients of psychological services became more sophisticated while searching to obtain emotional or relationship help. They research their symptoms online and learn about psychiatric diagnosis from DSM, and about a multitude of treatments. Today, we wish to get immediate answers to a problem we are confronted with. Therefore, many of the potential clients want a rapid connection with a qualified and objective therapist who adjusts to their schedule. The time needed to go to the therapist’s office for a face-to-face session is increasingly considered a waste of time, being replaced with online therapy. Obviously, essential skills and abilities are necessary for becoming both an efficient online therapist and an efficient online client. In this study, we shall present the case of a 30 years old client, the reasons for asking online therapy being: agoraphobia, current difficulties in the relationship with her partner and professional difficulties. We shall present the experience of the client while looking for a therapist, her acceptance as online client and the assessment, as well as the scheduling of Skype sessions, and termination of the therapy. We shall illustrate the manner in which elements from Schema Therapy (Young, 2003) are incorporated into short-term exchanges. As a result of online therapy, the agoraphobia disappeared, and the client acquired a healthier repertoire of alternative responses to threats of abandonment. Finally, the client shall be able to benefit from a longer online or offline therapy, but the help of the few exchanges offered becomes clear, and the therapist was able to provide sufficient closure even within the short time-frame.

Abstract Introduction: The etiology of the disorder is not yet well defined, what is known is that several factors can predispose the child to have autism, which may be genetic and environmental factors, according to scientific evidence. The TEA has three measurement degrees, classified as mild, moderate and severe. It is important that the autistic child and his family have the support of a multidisciplinary team that instructs them to overcome the diagnosis and the current situation in which they find themselves. Based on the characteristics presented by the autistic individual, there is a need to develop activities that encourage movement patterns, posture, communication, socialization and psychic, for this reason the physical therapy intervention must occur early, promoting improvements in psychomotor development and daily needs. Objective: Highlight the benefits of physical therapy as a treatment for children with autism spectrum disorder, present with autism spectrum disorder and talk about the family attitude in view of the autism diagnosis. Methods: This study qualifies for its basic nature, being a bibliographical research with a qualitative approach, through scientific journals of the last 10 years. Conclusion: When talking about physiotherapy in ASD, it is possible to understand that as the autistic person undergoes these treatments, their difficulties become smaller. Physiotherapy allows the child to have physical, motor and emotional well-being.

Resumen
Obstetric medicine and reproductive technology have been spread out worldwide and become the symbol of modernization. Its expansion might displace the traditional treatments which mostly are practiced by the people in developing countries. However, the Malaysian women who lived in a Western country and had a well-educated background still practiced the traditional treatments after giving birth. The study was conducted in 2016 at Durham, a county in the United Kingdom, and it utilized qualitative research by interviewing five Malaysian women who had a birth experience in the United Kingdom. The result of the study revealed that heating the body with hot stone has still mostly practiced by Malaysian women even living in the United Kingdom, where there were optional sophisticated technology and qualified medical professional. In addition, some of them still obeyed the recommended and prohibited foods ruled by the origin culture during the postpartum period. The treatment was conducted at home supported by the family and colleagues whose the same ethnicity and nationality. In conclusion, the national boundaries, high education, and the existence of sophisticated health technology and qualified medical professional are irrelated to why people still undertake traditional treatments. The treatment was primarily chosen because of its health effects on the body after treatments. Therefore, health policymakers have to know and consider the migrant‟s cultural values in order to make the health system convenient and appropriate to either the migrants‟ health. In addition, the study needs further research to find the effectiveness and efficacy of traditional treatments to women‟s health. Facing the pandemic of COVID-19 and the measures of social distancing, in order to reduce the spread of the new coronavirus (SARS-CoV-19), health services are using the technological resources to ensure the continuity of care in remote mode. In this context, the therapy group for women diagnosed with breast cancer has been adapted to the online videoconference. Objective: The aim of this study was to reflect on the professional psychological practice during the coronavirus disease 2019 (COVID-19) pandemic in an online group care with women with breast cancer. Methodology: This is a case report that joins the experiences of psychology professionals as group coordinators and internship supervisors, with reflections about a modality of telecare carried out by a health team, in the format of an online videoconference group. The experiences of the psychology team were understood in light of Martin Heidegger’s existential analytics. Results: Coordinating a support group in the online modality proved challenging for psychology professionals. This model took some time to be adapted in order to make the professionals comfortable in making their statements and for the bond with patients to be established again. During the supervisions, the lack of practical material to be discussed among the interns was noticed, something that was slowly solved with group readings regarding the clinical practice and group dynamics, and later, with the return of the online group. Conclusion: Facing the pandemic threat has opened a new possibility of being-in-the-world with technological mediations of communication among people, which finds an alternative way of being-with-others in the group participation. Psychology care is characterized by offering qualified listening, directed to the embrace of the emotional demands presented. In the experiences related to COVID-19, the questioning of the existential condition leads to an increase in anguish that marks the perception of finitude and allows the subject to assume being toward death.

Resumen
The study in question sought to highlight the importance of nutritional monitoring performed by the professional nutritionist in the prevention and treatment of Chronic Non-Communicable Diseases (NCDs). As well as reflecting on the importance of diet in the preventive care of NCDs and in their treatment, it shows the risk factors of the diseases and discusses how they can be avoided with nutritional monitoring, as well as points out some functional foods that have been found to be beneficial in the fight against these diseases. The research method of this article was a literature review, and full articles were searched to foster the study, laws, resolutions and quantitative and qualitative data to support the article. It can be concluded from the research carried out in this article that the nutritionist is the qualified and indicated professional to prevent and treat NCDs, and their performance in society can reduce health expenses, as well as reduce the prevalence of diseases by adopting diet therapy as prevention and treatment of chronic non-communicable diseases.

Abstract 3 to 5 million Americans are affected by lymphedema and 140 to 200 million patients worldwide [1,2]. The chronic, progressive condition is only treatable, but not curable, leaving patients with debilitating symptoms that can significantly impair their quality of life [3]. Treatments include manual lymphatic drainage (MLD) massage and compression garments, often applied sequentially. MLD is limited by consistent access to a qualified lymphedema therapist. Our wearable device utilizes vibration to mimic the patterns of massage in MLD. The long-term goal is to enable at-home maintenance therapy between MLD sessions and improve the overall management of lymphedema. Acceleration, frequency, and sound levels of our motors were tested and compared to current clinical solutions. Acceleration testing was conducted by placing an accelerometer in a tissue model at varying distances to collect acceleration data at different duty cycles. Frequency data was calculated in MATLAB using FFT. Acceleration, sound frequency, and sound level of our motors were all within a close range of the values collected from clinical tools. The acceleration frequency of the motors rose slightly with an increased duty cycle, but did not exceed the smaller clinical tool frequency values. Based on our testing, the motors used in our device are comparable to clinical tools currently used by certified lymphedema therapists.
