Texto completoThe regular supply of nutrients and oxygen to the organs is ensured by the regular contraction of the heart, a major organ of the cardiovascular system. As a result of certain cardiac diseases, cardiac valves may not function properly, which can lead to a retrograde flow of blood. In the case of the mitral valve, located between the left ventricle and the left atrium, it is referred to as mitral regurgitation. It is necessary to quantify the severity of mitral regurgitation in order to propose an appropriate treatment. In the first part of this document, a 3D mathematical model of cardiac hemodynamics is developed and integrates a mitral regurgitation model. We will also take the opportunity to model the isovolumetric phases of the heart. A relatively accurate model of cardiac hemodynamics, but nevertheless reasonable in term of numerical complexity, is thus obtained at the end of this first part. The second part of this document describes the strategy adopted to allow the fusion of medical images with the numerical simulations. An automatic method allowing the personalization of the mathematical model developed in the first part of the manuscript, based on medical images, is presented, allowing a systematic evaluation of the PISA method. Finally, as the methods presented are still too expensive from a numerical point of view, we conclude with the presentation of a blood flow reconstruction method combining Color Doppler images with physical constraints related to blood incompressibility

Texto completoCardiac arrhythmias are heart rhythm disruptions which can lead to sudden cardiac death. They require a deeper understanding for appropriate treatment planning. In this thesis, we integrate personalized structural and functional data into a 3D tetrahedral mesh of the biventricular myocardium. Next, the Mitchell-Schaeffer (MS) simplified biophysical model is used to study the spatial heterogeneity of electrophysiological (EP) tissue properties and their role in arrhythmogenesis. Radiofrequency ablation (RFA) with the elimination of local abnormal ventricular activities (LAVA) has recently arisen as a potentially curative treatment for ventricular tachycardia but the EP studies required to locate LAVA are lengthy and invasive. LAVA are commonly found within the heterogeneous scar, which can be imaged non-invasively with 3D delayed enhanced magnetic resonance imaging (DE-MRI). We evaluate the use of advanced image features in a random forest machine learning framework to identify areas of LAVA-inducing tissue. Furthermore, we detail the dataset’s inherent error sources and their formal integration in the training process. Finally, we construct MRI-based structural patient-specific heart models and couple them with the MS model. We model a recording catheter using a dipole approach and generate distinct normal and LAVA-like electrograms at locations where they have been found in clinics. This enriches our predictions of the locations of LAVA-inducing tissue obtained through image-based learning. Confidence maps can be generated and analyzed prior to RFA to guide the intervention. These contributions have led to promising results and proofs of concepts

Texto completoInhibition of KACh channels by the bee venom peptide tertiapin-Q rescues inherited cardiac conduction defects, sino-atrial bradycardia, and atrioventricular block in models of congenital dysfunctionSinus node dysfunction (SND) is a widespread disease of heart automaticity. SND refers to a multitude of sinus node (SAN) disorders characterized by failure to generate or conduct the cardiac impulse. The only currently available therapy for chronic SND is the implantation of an electronic pacemaker. Epidemiological studies forecast an increasing need for pacemaker implantation during the next 50 years, with the ageing of the population. It is thus an important medical and societal issue, to develop innovative therapies for SND. Pharmacologic inhibition of the G-protein activated K+ current (IKACh) could be a new therapeutic option to treat bradycardia and SND associated with other cardiac pathologies.We tested whether inhibition of IKAch by the peptide Tertiapin-Q could rescue SND and conduction dysfunction in Cav1.3-/- mice carrying concurrent ablation of L-type Cav1.3 and T-type Cav3.1 channels (Cav1.3-/-/Cav3.1-/-), mice carrying loss-of-function of f-channels (HCN4-CNBD) and Nav1.5 haploinsufficient (Scn5a+/-) mice.We employed telemetric ECG recordings of heart rate (HR), SAN pacemaking and AV dysfunction in mice before and after administration of different doses of Tertiapin-Q.Tertiapin-Q significantly improves the HR of Cav1.3-/-, Cav1.3-/-/Cav3.1-/-, and HCN4-CNBD from doses of 0.1 to 5 mg/kg. HRs of Tertiapin-Q-treated mice were similar to those recorded in untreated wild-type mice. Tertiapin-Q also improved cardiac conduction of Scn5a+/- mice by 24%.Pharmacological inhibition of IKAch by Tertiapin-Q prevents SAN dysfunction and improves conduction in three models of congenital bradycardia suggesting the possibility of pharmacologic development of IKACh targeting to manage SND and conduction disease, to delay or replace the implantation of an electronic pacemaker
