Objective: To audit maternal deaths at the regional hospital center of Saint-Louis. Materials and methods: This was a retrospective cross-sectional study of all maternal deaths that occurred between January 1, 2017 and December 31, 2019 at the Saint-Louis regional hospital center. Data were entered on EXCEL 2013 software. Analysis was performed on Epi Info version 3 software. Results: During our study, 73 cases of maternal death were collected. Maternal mortality was 520 per 100 000 live births. The mean age of the patients was 28.1 years±6.8. Multiparous women accounted for 43.8% and nulliparous women for 1.3%. The causes of maternal death were obstetric hemorrhage (28.77%), hypertension and its complications (20.55%), labour dystocia (15.07%) and anemia (6.85%). Direct obstetric causes of death were responsible for 64.38% of cases and indirect obstetric causes were seen in 35.62%. Death occurred postpartum in 68% of cases, during pregnancy in 25% and during labor in 7%. Death was preventable in 77.5% of cases. Conclusion: Maternal mortality is a real health problem in our practice. It affects young women. Indirect causes of death are becoming more significant causes of death in our practice. Maternal mortality is avoidable in the majority of cases.

Viruses with encephalitogenic potential can cause neurological conditions of clinical and epidemiological importance, such as Saint Louis encephalitis virus, Venezuelan equine encephalitis virus, Eastern equine encephalitis virus, Western equine encephalitis virus, Dengue virus, Zika virus, Chikungunya virus, Mayaro virus and West Nile virus. The objective of the present study was to determine the number of arboviruses with neuroinvasive potential isolated in Brazil that corresponds to the collection of viral samples belonging to the Department of Arbovirology and Hemorrhagic Fevers, Evandro Chagas Institute (SAARB/IEC) of the Laboratory Network of National Reference for Arbovirus Diagnosis from 1954 to 2022. In the analyzed period, a total of 1,347 arbovirus samples with encephalitogenic potential were isolated from mice; 5,065 human samples were isolated exclusively by cell culture; and 676 viruses were isolated from mosquitoes. The emergence of new arboviruses may be responsible for diseases still unknown to humans, making the Amazon region a hotspot for infectious diseases due to its fauna and flora species characteristics. The detection of circulating arboviruses with the potential to cause neuroinvasive diseases is constant, which justifies the continuation of active epidemiological surveillance work that offers adequate support to the public health system regarding the virological diagnosis of circulating arboviruses in Brazil.

Abstract Macrophage controls infection and inflammation via releasing pro-inflammatory cytokine IL-1β. NLRP3 inflammasomes assembly triggers a signaling cascade to process pro-IL-1β in the active form via activating caspase-1. In the lungs, dysregulated NLRP3 activation escalates inflammatory diseases such as acute lung injury (ALI), ventilator-induced lung injury (VILI), and idiopathic pulmonary fibrosis (IPF). These inflammatory disorders are associated with changing tissue compliance/stiffness due to accumulated extracellular matrix (ECM). Tissue-resident macrophages interaction with ECM via integrins transduce “mechanotransduction” signaling. However, the molecular mechanism of how mechanotransduction cross-talks with NLRP3 pathways and inflammatory response is not clear yet. We are examining NLRP3 activation in lung resident alveolar macrophages (AMs) during mechanical stress. For that, we culture macrophages on collagen-coated surfaces with altered stiffness (elastic modulus of 0.2 to 64 Kilo Pascals (kPa)). We observed that the NLRP3 inflammasome pathway is responsive to surface stiffness and IL-1β processing suppressed by a stiffer surface (64 kPa) when compared to a softer surface (0.2 kPa). Similarly, culture surface stiffness also regulates monocyte-to-macrophage differentiation. AMs show altered transcriptome signatures with changing the stiffness and favored activated macrophage phenotype at 0.2 kPa. Further, activation of the integrin-induced signaling also reduced the NLRP3-induced IL-1β release. Future examination of adhesion-based integrin-induced mechanical signaling and NLRP3 pathway will reveal the therapeutic potential in associated inflammatory diseases such as ALI/fibrosis. Abstract Background Patients with multiple myeloma (MM) have a 9-fold increased risk of developing venous thromboembolism (VTE). Current guidelines recommend pharmacologic thromboprophylaxis in patients with MM receiving an immunomodulatory agent in the presence of additional VTE risk factors (NCCN 2015, ASCO 2014, ACCP 2012). However, putative risk factors vary across guidelines and no validated VTE risk tool exists for MM. Khorana et al. developed a VTE risk score in patients with solid organ malignancies and lymphoma (Blood, 2008). We sought to apply the Khorana et al. score in a population with MM. Methods We identified patients diagnosed with MM within the Veterans Health Administration (VHA) between September 1, 1999 and December 31, 2009 using the International Classification of Diseases (ICD)-03 code 9732/3. We followed the cohort through October 2014. To eliminate patients with monoclonal gammopathy of undetermined significance and smoldering myeloma, we excluded patients who did not receive MM-directed therapy within 6 months of diagnosis. We also excluded patients who did not have data for hemoglobin (HGB), platelet (PLT) count, white blood count (WBC), height and weight, as these are all variables included in the Khorana et al. risk model. Height and weight were assessed within one month of diagnosis and used to calculate body mass index (BMI). We measured HGB, PLT count, and WBC count prior to treatment initiation: within two months of MM diagnosis. A previously validated algorithm, using a combination of ICD-9 code for VTE plus pharmacologic treatment for VTE or IVC filter placement, identified patients with incident VTE after MM diagnosis (Thromb Res, 2015). The study was approved by the Saint Louis VHA Medical Center and Washington University School of Medicine institutional review boards. We calculated VTE risk using the Khorana et al. score: We assigned 1 point each for: PLT ≥ 350,000/μl, HGB < 10 g/dl, WBC > 11,000/μl, and BMI ≥ 35 kg/m2. Patients with 0 points were at low-risk, 1-2 points were considered intermediate-risk and ≥3 points were termed high-risk for VTE. We assessed the relationship between risk-group and development of VTE using logistic regression at 3- and 6-months. We tested model discrimination using the area under the receiver operating characteristic curve (concordance statistic, c) with a c-statistic range of 0.5 (no discriminative ability) to 1.0 (perfect discriminative ability). Results We identified 1,520 patients with MM: 16 were high-risk, 802 intermediate-risk, and 702 low-risk for VTE using the scoring system in the Khorana et al. score. At 3-months of follow-up, a total of 76 patients developed VTE: 27 in the low-risk group, 48 in the intermediate-risk group, and 1 in the high-risk group. At 6-months of follow-up there were 103 incident VTEs: 41 in the low-risk group, 61 in the intermediate-risk group, and 1 in the high-risk group. There was no significant difference between risk of VTE in the high- or intermediate-risk groups versus the low-risk group (Table 1). The c-statistic was 0.56 at 3-months and 0.53 at 6-months (Figure 1). Conclusion Previously, the Khorana score was developed and validated to predict VTE in patients with solid tumors. It was not a strong predictor of VTE risk in MM. There is a need for development of a risk prediction model in patients with MM. Figure 1. Figure 1. Valentin Feliksovich was born on April 27, 1877 in the eastern part of the Crimea, in one of the oldest cities in the world, at a unique resort located on two seas at once Kerch. His father, Felix Stanislavovich, was a pharmacist and worked as a manager of a private pharmacy of D.I. Kundin, whose daughter, Maria Dmitrievna, was his mother. Valentin Feliksovich was born into a family belonging to an ancient family of impoverished Polish nobles. Of his 14 sisters and brothers, only five survived to adulthood.In 1880, the family of Valentin Feliksovich left Kerch for Kherson, then to Chisinau, in 1889 to Kiev. In 1896, Valentin Feliksovich graduated from the gymnasium and art school of Nikolai Ivanovich Murashko.Then Valentin Feliksovich studied for a year at the Faculty of Law, at the private art school of Knirr in Munich, but in 1898, deciding to become a doctor, he entered the medical faculty of Kiev University.Valentin Feliksovich, having received a doctor's degree, worked at the Red Cross hospital in Kiev, during the Russian-Japanese war he headed the surgical department of the Chita hospital as part of the Red Cross medical detachment, later worked as a surgeon in Ardatov, Simbirsk province, in the village of Verkhny Lyubazh and Fatezh, Kursk region. During this period, he married the daughter of the estate manager, sister of mercy Anna Vasilyevna Lanskaya. In 1909, Valentin Feliksovich entered the Moscow University as an external student to Professor P.I. Dyakonov.In 1915 he published the monograph "Regional Anesthesia" and in 1916 defended it as a dissertation with the award of the degree of Doctor of Medicine. By this time, Valentin Feliksovich had mastered operations on the heart, brain, organs of vision, gastrointestinal tract, including stomach, intestines, bile ducts, kidneys, spine and joints. Until 1917, Valentin Feliksovich worked as a doctor in hospitals in central Russia, later as the chief physician of a hospital in Tashkent, professor at the Central Asian State University. Moving to Tashkent is associated with the progression of his wife's tuberculosis, which required her to stay in a warmer climate, but in 1919, the wife of V.F. Voino-Yasenetsky died, leaving him to take care of four children. In February 1921, he was ordained a deacon, a week later a priest, on May 31, 1923, he was tonsured a monk and consecrated a bishop with the name Luke, and a week later he was arrested.At the beginning of 1926, Valentin Feliksovich returned to Tashkent. But on May 6, 1930, he was detained and transferred to Arkhangelsk. In the autumn of 1933, from exile, Valentin Feliksovich wrote a letter to the People's Commissar of Health about the need to organize an institute of surgical infection in the country and, having received no response, went to Moscow at the first opportunity for a personal meeting, at which he rejected the offer to renounce holy orders and instead accept the post of director of the institute.In the autumn of 1934, Valentin Feliksovich published a widely known monograph, including abroad: "Essays on purulent surgery". For several years, Professor Voino-Yasenetsky headed the main operating room of the Tashkent Institute of Emergency Care, but on July 24, 1937, he was arrested again, for the third time. Since March 1940, he has been working as a surgeon a hundred kilometers from Krasnoyarsk in the Bolshaya Murta of the Yelovskaya volost.After the outbreak of the Great Patriotic War, Valentin Feliksovich sends a telegram to M.I. Kalinin, in which he asks to be sent as a surgeon to the hospital, expressing his readiness to continue exile after the Victory.In September 1941, the chief surgeon of the region took V.F. Voino-Yasenetsky by plane to Krasnoyarsk to work as the chief surgeon of the evacuation hospital No. 1515, which was later recognized as the best of the 17 hospitals of the region.Since October 1941, Valentin Feliksovich has been a consultant to hospitals in the Krasnoyarsk Territory and chief surgeon of the evacuation hospital. In the autumn of 1942, he was elevated to the rank of archbishop and appointed to the Krasnoyarsk department. In 1944, Valentin Feliksovich published his monographs: "On the course of chronic empyema and chondrates", "Late resections of infected gunshot wounds of joints". Since February 1944, Archbishop Luke has been the head of the Tambov Department, and is a consultant surgeon at Tambov hospitals. In February 1945, Valentin Feliksovich was awarded by Patriarch Alexy I the right to wear a diamond cross. He is writing the book "Spirit, Soul and Body". Since May 1946, he has headed the Crimean Department in Simferopol and in the same year was awarded the Stalin Prize for the monograph "Essays on purulent Surgery", 130 out of 150 thousand of which he gives to the needs of wartime orphans.In 1955, Valentin Feliksovich completely went blind, but continuing to work and in 1957 dictated his memoirs. V.F. Voino-Yasenetsky died on June 11, 1961 in the rank of Archbishop of Crimea and Simferopol on the Day of All Saints who shone in the Russian land.Valentin Feliksovich is the author of 55 scientific works on medicine, ten volumes of sermons. He was elected an honorary member of the Moscow Theological Academy in Zagorsk. He was awarded the Chojnatski Prize from the University of Warsaw (1916), the Diamond Cross (1944), the medal "For Valiant Labor in the Great Patriotic War" (1945).In 1995, Valentin Feliksovich was canonized as a locally venerated saint in the Crimean diocese, in 2000 he was glorified as a confessor (saint) in the host of New martyrs and confessors of Russia. His relics are installed for worship in the Cathedral of Simferopol.On July 14, 2008, V.F. Voino-Yasenetsky posthumously became an Honorary Citizen of Pereslavl-Zalessky. On June 19, 2020, by Decree of the President of the Russian Federation, the Medal of Luke Krymsky was established, awarded for merits in healthcare.In memory of V.F. Voino-Yasenetsky, over 69 churches of St. Luke were opened, including in Russia (Moscow, Balashikha, Voronezh, Yevpatoria, Yeysk, Yekaterinburg, Donetsk, Zheleznogorsk, Kerch, Kovrov, Krasnoyarsk, Murmansk, Nizhny Novgorod, Novosibirsk, Novy Svet, Obninsk, Olginka, Orenburg, Perm, Petrozavodsk, Saki, Saratov, Simferopol, Stavropol, Ulyanovsk, Chelyabinsk, Chita, Yuzhno-Sakhalinsk), in Ukraine (Kiev, Sumy, Odessa, Vinnytsia, Dnipro), Greece and other countries. Monuments and busts were installed only in Russia in the years Moscow, Yeysk, Krasnoyarsk, Lipetsk, Nizhny Novgorod, Simferopol, Tambov and other cities. The name of Professor V.F. Voino-Yasenetsky was given to the Krasnoyarsk State Medical University, St. Petersburg Clinical Hospital, and the Society of Orthodox Doctors of St. Petersburg.

Abstract Background: Aplastic anemia (AA) is a rare disease, with an incidence of 1-2 new cases per million per year in Europe and North America. Primary therapies for AA for patients who do not have a sibling or matched unrelated donor or are unfit for allogeneic hematopoietic stem cell transplantation (HSCT) include immunosuppressive therapy (IST) with the combination of antithymocyte globulin (ATG) and calcineurin inhibitors. Therapeutic options beyond IST are limited and include eltrombopag, which was approved by the US FDA for use in patients with severe AA who fail to respond adequately to IST. The health economic burden of this rare and debilitating condition is poorly understood, especially for refractory cases. Objective: To examine health resource utilization associated with severe AA among patients with insufficient response to IST in real-world practice settings in a recent time period. Methods: We conducted a retrospective, longitudinal chart review of patients with severe AA treated at clinical centers in the US (Dana-Farber Cancer Institute [DFCI]) and France (Service d'Hématologie Greffe, Hôpital Saint-Louis [AGRAH]). Severe AA in this study was identified as having bone marrow cellularity &lt;25%, or 25-50% with &lt;30% residual hematopoietic cells; and at least two of the following laboratory findings: neutrophil count &lt;500 cell/µL, platelets &lt;20,000/µL, reticulocyte count &lt;20,000/µL. Eligible patients were ≥18 years old, first diagnosed with severe AA between January 1, 2006 and January 31, 2016, had insufficient response to at least one course of IST following severe AA diagnosis, and had ≥12 months of medical data after first diagnosis with severe AA. Patients with congenital disorders of hematopoiesis were excluded. Kaplan-Meier method was used to analyze time from first IST to time of HSCT. Cumulative incidence and incidence rates were used to summarize frequency of blood transfusions and AA-related health care resource utilization. The study was approved by local IRB. Results: The study included 34 refractory severe AA patients (NDFCI=20; NAGRAH =14). Mean age at severe AA diagnosis was 43.3 (standard deviation [SD]: 16.8) years and 52.9% (18/34) of patients were women. Median follow-up time after severe AA diagnosis was 56.1 (range: 12.0-118.7) months. Thirty-three (97.1%) patients received ATG in combination with calcineurin inhibitor (cyclosporine or tacrolimus) with or without corticosteroid as primary therapy. Among patients treated with ATG, 51.5% (17/33) patients received only one course of ATG and 48.5% (16/33) patients received ≥2 courses of ATG. The most common secondary AA therapy included eltrombopag (17.6%, N=6) and androgens (8.8%, N=3). The median treatment duration for eltrombopag was 6.4 (range: 5.6-53.4) months. The average frequency of transfusions per patient per month was 2.8 (SD: 2.8) red blood cell (RBC) and 3.3 (SD: 3.5) platelet transfusions. The mean AA-related health care utilization rates per patient per year were 0.8 (95% confidence interval [CI]: 0.6, 1.0) for inpatient visits, 0.5 (95% CI: 0.4, 0.8) for emergency room visits, and 19.1 (95% CI: 17.9, 20.5) for office visits prior to undergoing HSCT. Among the subgroup of patients treated with eltrombopag, the mean number of RBC transfusions per patient per month was reduced from 2.4 (SD: 2.0) before to 0.9 (SD: 0.8) after eltrombopag treatment, and from 3.0 (SD: 2.3) to 1.3 (SD: 1.6) for platelet transfusions. Similarly, AA-related health care utilization rates were lower after eltrombopag initiation (∆inpatient visits: -0.3 (95% CI: -1.1, 0.5); ∆emergency room visits: -0.6 (95% CI: -1.5, 0.4); ∆office visits: -11.7 (95% CI: -16.2, -7.1) per patient per year). Thirty (88.2%) patients received HSCT with a median time of 12.9 (95% CI: 7.9, 17.3) months after first IST initiation. During the follow-up period, 29.4% (10/34) patients died; nine patients died after HSCT. Two (5.9%) patients transformed to myelodysplastic syndrome and/or acute myeloid leukemia. Conclusion: This is one of the first studies to quantify the transfusion and health resource burden of refractory severe AA. In a small subgroup of patients receiving eltrombopag, there was a trend toward reduction in blood transfusion frequency, AA-related hospitalization rate, and outpatient office and emergency room visits after eltrombopag initiation. RESUMENEl objetivo de este artículo es reflexionar sobre los mecanismos de financiación y de control de las instituciones religiosas por los laicos en las primeras décadas de la conquista y colonización de Hispanoamérica. Investigar sobre la inversión laica en lo sagrado supone en un primer lugar aclarar la historiografía sobre laicos, religión y dinero en las sociedades de Antiguo Régimen y su trasposición en América, planteando una mirada desde el punto de vista de las motivaciones múltiples de los actores seglares. A través del ejemplo de restituciones, donaciones y legados en losAndes, se explora el papel de los laicos españoles, y también de las poblaciones indígenas, en el establecimiento de la densa red de instituciones católicas que se construye entonces. La propuesta postula el protagonismo de actores laicos en la construcción de un espacio cristiano en los Andes peruanos en el siglo XVI y principios del XVII, donde la inversión económica permite contribuir a la transición de una sociedad de guerra y conquista a una sociedad corporativa pacificada.PALABRAS CLAVE: Hispanoamérica-Andes, religión, economía, encomienda, siglos XVI y XVII.ABSTRACTThis article aims to reflect on the mechanisms of financing and control of religious institutions by the laity in the first decades of the conquest and colonization of Spanish America. Investigating lay investment in the sacred sphere means first of all to clarifying historiography on laity, religion and money within Ancien Régime societies and their transposition to America, taking into account the multiple motivations of secular actors. The example of restitutions, donations and legacies inthe Andes enables us to explore the role of the Spanish laity and indigenous populations in the establishment of the dense network of Catholic institutions that was established during this period. Abstract Sanofi Espoir Foundation evaluated impact of its decade long investments, in the amount of over 5 million Euros, on addressing health inequalities globally. Focusing on Saint-Louis department in Senegal, they have mapped specific profiles' experience like pregnant teenager suffering from community stigma, and finally linked each key moment with the health determinants, actors and initiatives. This study puts into perspective the impact the health system, including healthcare practitioners, can have on the pregnancy experience in regards of the influence of other determinants such as religion, culture, education, transportation, labor conditions, physical environment, health habits as well as local actors and initiatives. Each social determinant is highly and quite equally influencing the ante- and post-natal experience of the women, whereas the health system is the key factor during childbirth experience. If social determinants in health are well known, they are not reflected as such into health programs and funding processes. Maternal and newborn health in vulnerable situations is not simply a technical concern but a complex social issue that needs to be managed with an integrated, specifically “local” approach, transforming the objective 17 of the Sustainable Development Goals about partnering and breaking down silos, from an abstract paradigm into a daily process, thanks to community engagement and global accompaniment.
