Resumen
Risk assessment has become a large and anxiety-provoking part of the work of many psychiatrists. This article unpicks the different meanings of the word ‘risk’ to seek out the source of that anxiety, looking at both statistical and sociological ideas about risk. A risk assessment is often a subjective, plastic and context-dependent statement about a patient that carries strong moral overtones. Risk management is also highly anxiogenic for those charged with carrying it out, because uncertainty about the future is impossible to eliminate and the consequences of an adverse outcome in the patient may also carry hazards to the doctor making the risk assessment. This leads to behaviour change in people carrying out risk assessments as they attempt to minimise their anxiety in a rationally selfish way, often with unintended negative consequences for patients, doctors and health service providers generally. Some possible strategies for minimising this effect are considered.

Transfer pricing is one on the greatest problem of the global system of taxation and therefore the efficient TP tax control is of special importance. As the risk-oriented approach allows to improve the TP tax control, tax administrations as well as businesses should apply and develop it for the timely risks identification. This assumption is based on the review of foreign and domestic scientific literature provided in this article. This study aims to analyze the significance of TP risk management system and its impact on the TP tax control and voluntary tax compliance as well as to develop proposals on the TP risks assessment methods, focusing on Ukrainian tax regulation as well as the OECD and the EU tax framework. The research methods include systematic and comparative analysis of scientific literature, deduction, induction, analysis, synthesis and systems approach. To fulfill the objective of this study the analyses of legislative regulation of the TP control at both the international and the country level is provided, focusing on the stage of the monitoring of the controlled transactions. For the enhancement monitoring stage of the TP control the algorithm for the risk identification and assessment for the monitoring of controlled transaction (CT) is suggested. The study also provides for the methodology on comparison of the profitability of taxpayers with the average in the industry and methodology for building the TP risks matrix. The study results revealed the significance of the TP risks management processes standardization that allows its automatization and could contribute to the TP tax control strengthening as well as an TP compliance improvement. The prospects for future research could be focused on development of an algorithm for comparing the prices of CT with the quoted prices for raw materials.

Risk assessment methods have evolved over time to become better at evaluating risks associated with single chemical exposures, yet are limited in evaluating risks faced by communities with exposures to a range of chemical hazards via multiple routes. Recognizing the need to identify better approaches to assess disparate impacts facing certain populations due to environmental pollution, the authors evaluated activities within the Office of Environmental Health Hazard Assessment (in California’s Environmental Protection Agency) that had begun addressing these concerns by moving beyond standard risk assessment practices. On the basis of these examples, the authors identify 4 overarching principles to guide community assessments and explore how their application can improve these assessments. More work is needed in several areas, including developing information on exposure differences due to cultural practices, lifestyle patterns, and other factors; developing methods to recognize and take into account increased susceptibility; developing tools for assessing cumulative impacts; and improving communication with communities.

SynopsisRisk involves consideration both of the consequences of accidents and the frequency with which the accidents occur. Indeed formally risk is equal to the product of frequency and consequences. The important question of the perception of risk by the public and by the professional is first addressed. Two tenets are proposed as being a suitable summary of the public requirement:1. If it can happen, then it must not matter.2. If it matters, then it must not happen.A mathematical interpretation is placed upon these tenets and is shown to be consistent with various professional safety targets. The tenets do not indicate what numerical values for risk would be acceptable to the public but they do show how the consequences of accidents should diminish as the frequency or likelihood of a particular accident increases. It is argued that the best way of determining what level of risk the public accepts is to be guided by statistics for man-caused accidents. These, it transpires, pose risks which are considerably greater than those implied, for example, by the professional targets for nuclear reactors. The risk posed to the public by two energy installations is summarised. The one installation, situated on Canvey Island, exports energy in the form of gas, some of which (methane) is pumped into a national gas grid. The other installation, the Sizewell “B” Pressurised Water Reactor nuclear power station has not yet been constructed, but a comprehensive risk assessment has been undertaken, the results of which are summarised. The two installations are comparable in the sense that each exports a power of the order of a million kilowatts (in the form of gas in the one case and electricity in the other). Both have been the subject of Public Inquiries. The risks posed by the Canvey installations are accepted, since they only constitute a small fraction of the risks which the public run in any case during the course of their everyday lives. The predicted risks for the PWR are smaller still. The form taken by the risks posed by both installations corresponds broadly with the two tenets. That is to say the greater the consequences the lower should be the frequency of a particular accident.

Reading about the assessment and management of risk began, in earnest, for me in 1993. Previously, I had assumed that good training in the management of suicidal patients, reasonable clinical skills, a high-quality community team and the rarely utilised option of referring worrying patients to the forensic psychiatrists together represented an adequate basis for practice in an inner-city catchment area. This naïve assumption was challenged by the popular and political reaction to the actions of Christopher Clunis and Ben Silcock in December 1992, both people with a schizophrenic illness known to services whose dangerous behaviour put them on the front page of newspapers in the UK. A groundbreaking textbook demonstrates that other medical specialities have, partly because of the threat of litigation, been more effective in analysing sources of risk and developing strategies to manage risk (Vincent, 1995). Improving quality is a central plank of the latest National Health Service reform (Department of Health, 1998). There is no doubt that the political driver for this is “the dark side of quality” (Vincent, 1997), how to deal with perceived or actual performance failures in health care such as the seminal ‘Bristol case’, in which cardiothoracic surgeons with an unusually high mortality rate in carrying out a complex procedure were found guilty of serious professional misconduct (Smith, 1998).

Risk assessment forms an integral part of clinical practice. Traditionally, risk has been portrayed as a binary concept, and its assessment regarded as a test that can be correctly or incorrectly classified. However, this article discusses how risk assessments are less straightforward than is commonly perceived and are often complicated by multiple forms of uncertainty. These uncertainties arise where psychiatrists are unsure about their interpretation of information, where information is missing, or where interpretation of the risk situation is open to challenge. They centre on doubts about the accuracy and the defensibility of assessment of patients' risk status and the need for risk containment. Like other professionals, psychiatrists adopt a range of strategies to resolve uncertainties. These strategies, which often involve some ‘risk-taking’, enable the practising clinician to make a more confident decision. There is an argument for including ‘certainty’ as a theoretical feature of risk assessment in psychiatry and for recognising it as a multifaceted phenomenon. There is also an argument for considering with greater precision the manner in which uncertainty is managed within psychiatric risk assessments.
