Disorders of the psyche (particularly depression and anxiety) are relatively common amongst children with life-limiting conditions. Most of the tools available for evaluating or assessing them in palliative medicine were developed for adults, as were strategies for treating them. The ideal practice is to collaborate with local child and adolescent mental health services in the management of all such children. This chapter covers management of psychological conditions, including depression, anxiety, insomnia, and terminal delirium. For each symptom, causes or general points are detailed, with recommendations on pharmacological approaches covered in detail.

This chapter discusses the somatic symptom disorders, which are a heterogeneous group unified by physical symptoms or concerns that are associated with prominent distress or impairment. Somatic symptom disorders are estimated to account for 1 in 10 primary care patient visits. The relative prominence of somatic symptoms is essential to the difference between illness anxiety disorder, which is an example of the obsessional/cognitive subtype (not prominent) and somatic symptom disorder,, in which the somatic symptoms are prominent. Patients with body dysmorphic disorder, also an Obsessional/Cognitive subtype, are preoccupied with a perceived defect in physical appearance. Patients with conversion disorder (functional neurological symptom disorder) (dissociative sub-type) present with neurological symptoms that cannot be fully explained physiologically. Patients with factitious disorder consciously simulate illness for psychological purposes rather than practical gain.

This chapter has three broad aims: it outlines research on the shared and unique features of OCD and PTSD; it reviews the extant literature on how trauma exposure impacts treatment outcome for OCD; and it offers recommendations for treatment and future research on the intersection of trauma and OCD. Obsessive-compulsive disorder is a complex and heterogeneous condition. Considerable research has been conducted related to subtypes and symptom dimensions, but comparably little attention has been paid to commonly cooccurring psychiatric disorders. One diagnosis that has distinct etiological and prognostic implications is cooccurring posttraumatic stress disorder, and other trauma disorders. The chapter focuses on trauma generally and its impact on OCD. However, the literature also refers specifically to PTSD. Accordingly, the research related to PTSD is highlighted, to distinguish it from literature discussing trauma exposure more generally.

The current chapter provides an overview of psychoanalytic and behavioral theories of obsessive compulsive disorder and related conditions (i.e., hoarding, hypochondriasis, body dysmorphic disorder, trichotillomania, and tic disorders), and reviews the empirical support for these psychological theories. While Freud correctly ascribed compulsive rituals to an anxiety-reducing role, the more fundamental tenets of his drive model of obsessional development, and the subsequent focus on the role of defense mechanisms, have remained largely untested. In contrast, behavioral theories of obsessive compulsive and spectrum disorders revolutionized the psychological conceptualization and treatment of these disorders, and there is strong evidence accumulated over the past 40 years demonstrating the seminal role of operant conditioning processes in the maintenance of obsessive compulsive and related spectrum disorders. The evidence supporting the role of classical conditioning in symptom development is less clear; however, learning theory has contributed a partial understanding of the etiology of these conditions.

End-stage kidney disease (ESKD) accounts for 1-2% of all deaths. Ageing populations means that this proportion will grow steadily over the coming years. Symptom burden in ESKD exceeds advanced cancer, with added renal-specific symptoms, such as itch and restless legs. Pain and depression are also more prevalent. Many renal symptoms go under-recognized and under-treated, especially as they arise from co-morbid conditions, rather than the renal disease itself. The most useful intervention to address symptoms is regular assessment of symptoms, using a valid and reliable global symptom score. Pharmacological interventions to alleviate symptoms need to take account of the severe constraints on using renally cleared drugs, and the high risk of toxicity from accumulation of parent compound or metabolites. The population with ESKD has extensive palliative care needs, and need significant medical, nursing, psychological, and social care to address these as their illness advances towards the end of life.
