Abstract Background Patients with Turner syndrome may have multiple associated metabolic comorbidities, some of which are rare. We present a case report with Turner syndrome and congenital hypoparathyroidism. Clinical Case A 30-year-old Caucasian female with Turner syndrome and primary ovarian insufficiency was referred for evaluation of osteopenia and elevated TSH. On her initial visit, the patient was asymptomatic, physical exam was significant only for short stature. The DEXA scan showed a T-score of -2 at the lumbar spine and -1.8 at the femoral neck. TSH was elevated at 4.81 (0.5–5.0 mIU/L), with normal serum levels of free T3 and free T4 consistent with subclinical hypothyroidism. Patient previously underwent aortic root imaging with no evidence of dilatation. Her only home medication was a combination oral contraceptive tablet (Yaz; drospirenone and ethinyl estradiol) for primary amenorrhea. The patient was started on calcium 500 mg daily and vitamin D3 400 IU. On her next visit, patient's calcium was 8.7 mg/dL (8.6–10.6 mg/dL), phosphorus 4.4 mg/dL (3–4.3 mg/dL), albumin 3.9 g/dL (3.4–5.4 g/dL), 25-hydroxy vitamin D 63.2 ng/dL (30-100 ng/dL), 24-hour urine calcium 93 mg/day (100-300 mg/day), and PTH 20 pg/mL (15–65 pg/mL) suggestive of hypoparathyroidism. There have only been a few case reports of congenital hypoparathyroidism in patients with Turner syndrome. (1) The patient was started on calcitriol 0.25 mcg daily and her calcium was increased to 500 mg twice daily. Repeat serum calcium was 9.1 and 25-hydroxy vitamin D 75.6 ng/dL showing that the patient responded appropriately to calcium and calcitriol supplementation. In the absence of a history of neck surgery or autoimmune diseases, the etiology behind hypoparathyroidism in this patient with Turner syndrome appears to be secondary to genetic mutation. Conclusion We highlight the importance of measuring parathyroid hormone level in patients with Turner syndrome who exhibit symptoms or signs of osteopenia as part of diagnostic workup. Resumen
The article outlines two directions of comprehension of the existential phenomenon of musical art (cataphatic and apophatic) throughout the history of culture within the framework of the Christian-Eurocentric tradition. The basis of methodological research includes Christian-idealistic and Christian-anthropological positions. These provisions of objective idealism contain a part of the Creator's Plan in the realization of his Image. The presentation of the main trends is presented in a contrasting ratio, which is characteristic of musical compositional thinking. The discourse of the problem is expressed in a cataphatic-preaching form. In the process of considering this phenomenon, the method of synthesis of ideas about music was applied as a reflection of the human being of such domestic thinkers as A. S. Klyuev, A. F. Losev, V. V. Medushevsky, M.O. Orlov, O.V. Leontieva and foreign philosophers R. Ingarden, A. Schopenhauer, F. England, D. Turner. The author makes an attempt to update the musical discourse from the perspective of Eurocentric Christian traditions by including the language of theology in it. Research in a given direction reveals the deep foundations of musical creativity, which arouse interest in the social significance of the master creator, leveled by post-industrial society. The scientific novelty of the research is contained in the choice of sources that reveal the European understanding of the existential essence of musical art as a historically formed part of the musicological discourse. According to the results of the study, it can be concluded that, unlike other types of art, music remains irreconcilably resistant to various forms of evaluation and paraphrasing, explanation and description. The essence of human existence is fully reflected in the musical art.

Abstract Background Ring chromosomes can be formed by terminal breaks of two arms of a chromosome and their rejoining, leading to a loss of genetic material. They may also be formed by telomere-telomere fusions with no deletion, resulting in the formation of a complete ring. Mosaic X-ring chromosomes are extremely rare and have highly variable phenotypes. Here, we report a case with a mosaic X-ring chromosome in a patient with Turner syndrome, and we illustrate the unreported complicated mechanism using chromosome analysis and fluorescence in situ hybridization (FISH). Case presentation A 10-year-old girl of short stature presenting Turner syndrome was admitted to our hospital. The patient’s clinical characteristics were subsequently documented. Genetic analysis showed a karyotype of mostly 45,X[140]/46,X,r(X)[60]. The X ring chromosome was cytogenetically characterized as 45,X/46,X,r(X)(p22.32q21.1), with a length of approximately 74 Mb. Conclusions Taken together, we report a rare case with a mosaic X ring chromosome in Turner syndrome and we believe this case expands our collective knowledge of mosaic structural chromosomal disorders and provides new insight into clinical management and genetic counseling for Turner syndrome.

Background: Turner syndrome (TS) is a rare genetic developmental disorder characterized by gonadal dysfunction, short stature and heart defects, among others. Women with TS often suffer from severe fatigue, for which they are typically referred to endocrinologists. The diagnostic work-up is generally time-consuming and invasive, but it rarely solves the problem. To prevent the personal and financial burden of unnecessary diagnostic procedures, it is crucial to understand fatigue in TS. Therefore, in this study, we explored the association between fatigue and endocrine and non-endocrine comorbidities in a - for rare disorders - large group of TS women. Design: Cross-sectional study. Methods: 170 genetically confirmed TS women who attended the TS reference center underwent a systematic health screening, including a structured interview, complete physical examination, biochemical measurements, perceived stress and fatigue questionnaires and additional tests when indicated. Results: Median age was 32.6 years (IQR 23.9 - 41.4 years) and median BMI was 25.4 kg/m2 (IQR 22.6 - 28.5 kg/m2). One in three TS women experienced severe fatigue. Liver enzyme disturbances and body mass index were significantly associated with higher fatigue scores, while childhood growth hormone treatment was associated with lower fatigue scores. Perceived stress was highly correlated with fatigue (ρ = 0.71; P < 0.001). Conclusions: There was no association between fatigue and most endocrine and non-endocrine disorders, which implies that fatigue is only partly explained by somatic disorders. The high correlation between perceived stress and fatigue suggests that TS-specific neuropsychological processes may play an important role in the etiology of fatigue in TS women. We provide a practical algorithm for the endocrine, non-endocrine and psychological approach to fatigue in TS women. Presentation Date: Friday, June 16, 2023

Abstract Background/Aims Systemic lupus erythematous (SLE) is a chronic autoimmune (AI) disease of unknown etiology. It predominantly presents in women and only 1 in 10 of sufferers are men. We sought to examine the clinical pattern in male lupus patients in a tertiary multi-ethnic lupus centre in East London, UK. Methods Male lupus patients were identified through review of a pre-existing lupus patients database and clinic lists at Barts Trust. All patients’ electronic notes were reviewed prior to a telephone interview. All the participants fulfilled the ACR 1997 criteria for SLE. Descriptive data analysis was performed for all identified patients. Results Fifty-one patients were identified. Sample demographics showed that male lupus was more prevalent among Afro-Caribbeans (37%,n=19), followed by Asians (33%,n=17) and Caucasians (29%,n=15). The median age was 40.5 (IQR:32.5-52.7) years, with median age at SLE diagnosis 28 years (IQR:23-45).Family history of an AI condition was seen in 25%, whilst 14% had a family history specifically of SLE. Possible significant stressor prior to diagnosis reported in 45% (n = 23) patients. Serological evidence of previous infection with EBV, VZV or parvovirus was found in 76%. Although all patients were below the state pension age of 66, more than half were unemployed. The majority of males had multiorgan involvement including renal (67%,n=34), cutaneous (55%,n=28), hematological (45%,n=23), musculoskeletal (31%,n=16), cardiac (27%,n=14), pulmonary (20%,n=10), and neurologic (14%,n=7). Renal biopsy was done in 33/34 renal cases and the showed proliferative class-III/IV in 65%. Hypertension was a significant comorbidity in 39%, 47% had a concomitant AI condition. Juvenile-onset SLE was diagnosed in 6% of the patients with median age at disease onset 10 years (IQR:14-16,100% Caucasians). These males all had kidney nephropathy class-IV/V. Regarding serology, 94% were positive for antinuclear antibody, 40% speckled, 31% homogenous, and 8% for nucleolar and mixed pattern. Two patients ANA test was not recorded but they had confirmed renal/cutaneous biopsy findings.Three-quarters were positive for anti-double stranded DNA. Ro and La antigens were detected in 35% and 8%, while anti-Sm and anti-RNP antibodies were detected in 37% and 43%, respectively. Antiphospholipid antibodies were identified in less than a third, of which 40% presented with thrombotic events. Triple antiphospholipid antibodies was detected in 53%, 20% had LA, 15% had aCL, and 7% had either anti-B2GP1 or aCL/anti-B2GP. Conclusion Our dataset demonstrates a higher prevalence in Afro-Caribbean and Asians and a young age of onset (median 28 years) with presentation about 10 years earlier than most other reports. The majority were unemployed and reported significant life-stressors. Over three-quarters of patients had evidence of prior infection.Most of the lupus males had multiorgan involvement and over two-thirds had renal involvement, proliferative class-III/IV. AbstractObjective. To describe the effects of wearing individual protection equipment against biological hazard when performing a simulated resuscitation.Methods. Uncontrolled quasi-experimental study involving 47 volunteers chosen by random sampling stratified by sex and professional category. We determined vital signs, anthropometric parameters and baseline lactate levels; subsequently, the volunteers put on level D individual protection equipment against biological hazard and performed a simulated resuscitation for 20 minutes. After undressing and 10 minutes of rest, blood was extracted again to determine lactate levels. Metabolic fatigue was defined as a level of lactic acid above 4 mmol/L at the end of the intervention.Results. 25.5% of the participants finished the simulation with an unfavorable metabolic tolerance pattern. The variables that predict metabolic fatigue were the level of physical activity and bone mass -in a protective formand muscle mass. People with a low level of physical activity had ten times the probability of metabolic fatigue compared to those with higher levels of activity (44% versus 4.5%, respectively).Conclusion. He is, surely, the only heterosexual male in the country who could get away with being photographed half-naked and smothered in baby oil for GQ and still come over as an icon of masculinity. (GQ October 2002. Article on Beckham as GQ’s Sportsman of the Year, 264) Indeed. Let us tear our thoughts away from the image of David basted in oil and consider the extract as one of innumerable examples of the media fascination with Beckham. Given his penetration in Europe, Asia, Latin America and Africa, we can take as self-evident that Beckham is a quantifiably significant figure in contemporary global popular culture. By any measure of celebrity and any taxonomy of fame (Turner 15-23), Beckham qualifies as a striking example. He has inevitably appeared in a number of recent academic publications as an exemplar of celebrity and sports culture (Whannel, Turner, Cashmore and Parker) and, more notably in Cashmore’s book, as the focus of a social biography (Beckham). In his book Understanding Celebrity, Turner provides a comprehensive overview of the vast literature which has developed on issues of celebrity and fame, painting a broad picture of concerns divided between the significance of the apparent explosion in celebrity ‘culture’ and the focus on celebrities themselves. Within the literature on the social significance of celebrity culture, we can discern two key themes. First, celebrity culture is a manifestation of globalised commodity consumerism in advanced capitalism and second, its social function as a system of meanings and values which is supplanting traditional resources for self and social identities in late modern culture, including structures such as class, gender/sexuality, ethnicity and nationality. Whilst the authors mentioned above both draw on and contribute to these arguments, their focus remains broad, citing Beckham as a key manifestation of the complex interdependence between globalised sports and media industries, and transformations in gender and consumption. For example, although Cashmore’s book is solidly researched on the impact of media finance on football and has a sound argument on the significance of consumerism, he is prone to generalisations about the transformations in masculinity and celebrity culture which he suggests are central to understanding Beckham’s significance. Turner suggests that there needs to be more focused empirical work on the specific construction of celebrity since ‘modern celebrity…is a product of media representation: understanding it demands close attention to the representational repertoires and patterns employed in this discursive regime’ (8). This is how this short piece offers a contribution to the literature – drawing on a qualitative analysis of articles on Beckham, my discussion focuses on the meanings of Beckham’s celebrity and whether they can tell us something about the way the culture of fame operates. I have drawn selectively from my data, but a fuller discussion of both the data and grounded theory methodology can be found in a previous article (Rahman). Out of the six categories of meaning established through the grounded theory procedures used in the study, my contention is that masculinity is a core nexus in ‘cultural circuitry’ (Hall) – making the stories relevant, understandable, and often controversial. Moreover, the accompanying photo spreads often create a tension with the text, emphasising dissonant/controversial images which testifies to a dynamic of respect/ridicule in the representations. To be more precise, there is a construction of deference to Beckham’s professional status and to the Beckham family as the premier celebrity unit in the UK. Deference to and respect for their status is evident not only in those magazines which have paid for the privilege of access, but also the more gossip orientated celebrity weeklies such as Heat (18-24 May: 6-8): ‘those lucky enough to be asked to join David and Victoria enjoyed one the most extravagant soirees in recent memory. The sheer scale of the £350000 shindig was stunning, even by the standards of Celebville’s most extravagant couple’. Coupled with this respect is a sense of ridicule, often in discrete publications, but also within the same magazine and even sometimes the same article. Ridicule undercuts the celebrity credentials of extravagance and glamour with an implication of tackiness and vulgarity, and this gentle undercurrent becomes stronger when linked to Beckham’s fashion icon status: We’ve supported David through the highlights and lowlights of his various haircuts: the streaked curtains, the skinhead and his travis bickle style mohican. But this latest look is a ‘do too far’ – more village idiot than international style icon… (Heat 13-19 April: 24-5) This dynamic of respect/ridicule relies heavily on another dynamic; that of queer/normative invocation and recuperation. It is not only his fashion icon status being ridiculed here but also his status as a heterosexual masculine icon: People say you’re vain. Do you think so? You can see why people might think you’re a bit of a big girl’s blouse, because you have manicures, sunbeds and bleach your hair. You’re also one of the few footballers to become a gay icon. (Marie Claire June 2002: cover of Beckham, and 69-76) His gender/sexuality is anchored in hetero-family/masculine status but is somewhat dissonant in terms of vanity/grooming and gay icon status. ‘Queerying’ Beckham is not just a technique of ridicule (how very old fashioned that would be!) but also a deliberate destabilisation of ontological anchors which induces a sense of dissonance: An example from Heat (20-26 July 2002) has the cover byline ‘Phwoar! Another new look for Becks’ with a trail for a story on pages 18-20 which has a photograph of Beckham with his nail varnish highlighted and the text: David sported a new blonde barnet and a fitted black suit, and despite the controversy caused by his pink nail varnish he still managed to look macho and absolutely beautiful. This demonstrates some feminisation of Beckham but is counterbalanced by the very masculine anchor of ‘macho’. There is a recognition that the highlighted ambiguity in gender coding is potentially disruptive or controversial and hence it is recuperated – ‘he still managed to look macho’. GQ from June 2002 repeats the play on gender and sexuality, with a cover photo of Beckham lying down, bare torso but in a suit and hat, with one hand showing a ring and nail varnish, and the other in the waistband of his trousers. Inside, on pages 142-55, there follow seven full pages of photos and an interview conducted by David Furnish, a family friend of the Beckhams but also Elton John’s partner and so one of the most visible gay men in celebrity culture. However, rather than any danger of queering by association, the presence of Furnish seems only to enhance the mega-celebrity and hetero status, since he is careful to sound all the right notes of family, football and fatherhood in his questions in the text. Rather, it is the photospread which induces the queerness in this example, with four of Beckham’s naked torso in baby oil, of which one is him in unbuttoned cut-off denim shorts on a weights bench – very retro 1970s gay. In his history of male sports celebrities, Whannel suggests that Beckham is an exceptional figure, both because he is one of the few footballers in the UK to achieve full celebrity status, but also because he transgresses the discipline and work ethic associated with sporting bodies, indulging himself through conspicuous and narcissistic consumption (212). Whannel notes Beckham’s emergence during the development of a men’s style press in the UK, documented thoroughly in Nixon’s study of men’s magazines, which provides an account of the historical moment from 1984-1990 which saw the emergence of ‘new man’ imagery. Drawing on Mort’s contention that this is the first period which showed men being sexualised – a representational strategy previously applied only to women – Nixon concurs with Mort that this moment marks the beginning of men being addressed as a specific gender. However, these images of Beckham push at the boundaries of ‘new man’ constructions and ‘respectable’ images of sporting bodies, suggesting that the deliberate, indelicate and delicious sexualisation of Beckham’s body derives its power from the ‘danger’ this presents to sporting masculinity as well as simply heterosexual masculinity. Thus we need ‘family, fatherhood’ and ‘football’ to anchor the ‘queer’ Beckham. Given these and more recent images (Vanity Fair cover in July 2004, for example), we might be tempted to agree with Cashmore and Parker and Whannel that Beckham is indeed a ‘postmodern’ or ‘hybrid’ celebrity, appearing singularly able to float free of context and to signify many different meanings to many different groups. But the brief examples of the queer/normative dynamic presented here suggest that this is too glib an answer, precisely because there seems to be an explicit recognition of this dynamic: the editor of GQ says of Beckham that ‘he is in touch with his feminine side, but he is so obviously heterosexual that he can afford to be’ (Hot Stars 2-8 Nov. 2002: 36-9). The deliberate induction of dissonance suggests a reflexivity about the constructedness of these representations; a knowing indication that queerying Beckham’s masculinity is not the reality of Beckham, but rather that the queerying is perhaps a hyper reality as Baudrillard might have it. Beckham does not float ‘free’: dialectical signs are precisely mapped onto him. Dyer argues that film stars could be read as signs for specific versions of individuality, but crucially, that these signs reflect the dominant ideological constructions of class, ethnicity and gender/sexuality. In one example, he demonstrates how the sexually transgressive and potentially lesbian elements of Jane Fonda’s star persona are recuperated through the emphasis on her nationality and ethnicity, her ‘all-Americanness’ (81). Similarly, Beckham’s queerness is deliberately deployed as a sign, to be neutralised by heterosexual signs, thus recuperating the ideological dominance of a heteronormative culture. Beckham’s masculinity can be read as a ‘sign’, divorced from traditional referents and re-marked into a queer sign, specifically to promote consumption through the heady mix of respected status and apparently exciting transgression as a key aspect of this status. But this is a simulation, not indicating any ‘real’ queering of either the subject, or indeed of the assumed audience who have to make sense of the sign. Rather, the potential to remark Beckham as ‘queer’ seems to indicate that whilst heterosexual masculinity can be a sign, so perhaps too does queer itself become a sign, similarly divorced from its traditional referents. The ‘reality’ is thus simulated through pre-determined codes of representation, and one such code seems to be that gender transgression is culturally significant. Dialectical signs are mapped onto a reality/hyper reality dynamic, with queerness presented knowingly as the hyper real – after all, the reality is that Beckham is ‘so obviously heterosexual…’ It is possible to argue that the dynamics at work in making these representations effective can be understood as dialectical since there are opposing momentums at work in the construction of celebrity and fame. The respect/ridicule dynamic demonstrates that constructions of celebrity cannot be uncritically deferential. The gentle and knowing ridicule is a collusion between the media(tors) and the audience: an indication that this relationship is the true romance of celebrity culture rather than that between fans and icons. And why should this be so? Precisely because the media needs to continue to feed the desires of the audience but there is no guarantee that the desire will continue when an icon’s star wanes – unless of course, watching the decline is as much part of the romance as building the respect. Marshall argues that celebrity legitimises the individuality central to the lock between consumer capitalism and liberal democracy and the respect/ridicule dynamic exemplifies this function. The necessary continuation of consumption produces a dialectical dynamic, wherein both respect and ridicule exist to permit easy shifts in emphasis whilst maintaining the attention on the celebrity, which promotes continued consumption. Beckham’s own demonisation and rehabilitation in the wake of France 98 testifies convincingly to the necessity for continuity of producing items for consumption, no matter what the spin. Furthermore, the recent scandals over alleged infidelities has generated a production spike in the amount of images and words produced, whilst this time, not directly attacking Beckham. The queer constructions of Beckham amplify respect/ridicule along a specific dimension, supplying a dialectic of its own. The modes of meaning surrounding Beckham do indicate a shift in the possible effective constructions of masculinity, with the incorporation of a feminised interest in fashion (hairstyles, nail varnish, presentation in general) and the affirmation of gay icon/object of desire. It is in these constructions of dissonance that the de-essentialising of masculinity occurs, which may be the productive moment of disruption for those receiving the images and texts, and incorporating them into their own meaning systems around Beckham, footballers, masculinities, heteronormativity. The fact that these queer moments are possible may be testament enough to Beckham’s social significance; he is in the right place at the right time (with the right body and profession) to be our cultural lightning conductor for contemporary anxieties around gender/sexuality. However, the dialectic of queering Beckham has a synthesis which suggests that the route into queerness is not as important as the route out. These are only fleeting materialisations of the queer David Beckham – flashes of fleshy dissonance glimpsed briefly before the recuperation into the heterosexual subject, coded by footie, family and fatherhood. The newer dissonant properties of masculinity are literally contextualised within ideological codes of heterosexuality. The evident theatricalisation and appropriation may appear to signal a productive route into queerness – from heterosexual to queer (the pink nail varnish, the oiled fashion shoots, the gay gym denim cut offs shot), but what if it is actually working in reverse? What if the cultural effectivity is achieved by appropriating and theatricalising from gay/transgender to heterosexual? – de-essentialising ‘queer’ for productive dissonance and amusement, but safe in the knowledge that there is a secure and policed route out of ‘queerness’ – the encoded red carpet of heterosexual masculinity. The possibilities of a queer visibility are thus denied through the recuperative effects of the dialectics at work. The ridiculing of his gender transgressions may be necessarily gentle, in order to walk the tightrope of respect/ridicule, but they nonetheless assume that transgressions are problematic. Furthermore, the reality/hyper reality dynamic deploys queer as a ‘sign’ precisely in order to effect a recuperation of a normative version of ‘reality’. It seems that the weight of a predominantly heteronormative culture reinforces the dialectics in celebrity culture, making the unproblematic visibility of queer subjects improbable. After all, in these examples – focused one on the world’s premier celebrities – ‘queer’ itself is not actually cool – it seems that only the simulation of queer is cool. The elaborate cross-media spectacle, ‘Australian Idol,’ ostensibly lays bare the process of creating a pop star. Yet with so much made visible, much is rendered opaque. Specifically, ‘Idol’ is defined by the use of carefully-tuned strategies of publicity and promotion that create, shape and reshape a series of ‘authentic celebrities’ – pop stars whose emergence is sanctified through a seemingly open process of public ratification. Yet, Idol’s main actor is the music industry itself which uses contestants as vehicles for crafting intimate, long-term relationships with consumers. Through an analysis of the process through which various contestants in ‘Australian Idol’ are promoted and sold, it becomes clear that these populist icons are emblematic of an industry reinventing itself in a media environment that presents remarkable challenges and surprising opportunities. Curiously, the debates, strategies and motivations of the public relations industry have received little sustained attention in popular music studies. While much has been written about the contradictions between the rhetoric of rebellion and the complicated realities of corporate success (Frank; Negus), less has been written about the evolution of specific kinds of publicity and the strategies that shape their use in the music industry. This is surprising given the foundational role of public relations strategies within the culture industries generally and the music industry in particular. Specifically, what Turner et. al. define as ‘the promotional culture’ is central to the production and marketing of mainstream popular music. The ‘Idol’ phenomenon offers a rich opportunity to examine how the mainstream of the popular music industry uses distinct and novel marketing strategies in the face of declining sales of compact discs, an advertising environment that is extraordinarily crowded with all manner of competing messages, a steady rate of trade in digital song files and ever more effective competition from video games and DVDs. The ‘Idol’ phenomenon has proved to be a bundle of highly successful strategies for making money from popular music. Selling CDs seems to be almost ancillary to the phenomenon, acting as only one profit centre among many. Indeed, we can track the progress and deployment of specific strategies for shaping the creation of what has become a series of musical celebrities from the start of the first series of ‘Australian Idol’ through a continuous process of strategic publicity. The Attention Economy It has been somewhat hysterically estimated that the average resident of Sydney might be presented with around 3000 commercial messages a day (Lee). It is this kind of communication environment that makes account planners go weak in the knees in both paralysing anxiety and genuine excitement. Many have taken to paying people to go to bars, cafes and clubs to talk up the relative merits of a product to complete strangers in the guise of casual conversation. Similarly, commercial buskers have recently appeared on City Trains to proclaim the virtues of the wares they’ve been contracted to hawk. One can imagine ‘Cockles and Mussels’ has been updated as ‘MP3 Players and Really Cool Footwear.’ These phenomena are variously referred to as ‘viral,’ ‘tipping point,’ ‘word of mouth’ or ‘whisper’ marketing. (Gladwell; Godin; Henry; Lee; Rosen) Regardless of what you call it, the problem inspiring these promotional chats and arias is the same: advertisers can no longer count on getting and holding our attention. As Davenport and Beck, Brody and even Nobel Prize winning economist Herbert Simon have noted, the more taxed public attention gets, the more valuable it becomes. By most industry accounts, the attention economy is an established reality. It represents a significant shift of emphasis away from traditional methods of reaching consumers, instead inspiring new thinking about how to create lasting, flexible and evolving relationships with target audiences. The attention economy is a complicated and often contradictory response to a media environment that appears less and less reliable and to consumers who behaviour is often poorly understood, even mysterious (Elliott and Jankel-Elliott). This challenging backdrop, however, is only the beginning for a seemingly beleaguered music industry. Wherever one looks, from the rise of the very real threat of global piracy to the expansion of the video game industry to mobile phones and hand held players to increasing amounts of money spent on DVDs and ring tones, selling CDs has become almost a sideline. The main event is the profitable use and reuse of the industry’s vast stores of intellectual property through all manner of media, most which didn’t exist ten years ago. Indeed, the ‘Idol’ phenomenon shows us how the music industry has been incorporating its jealously-guarded intellectual property and familiar modes of industrial self-presentation into existing media environments to build long-term relationships with consumers through television, radio, DVDs, CDs, the internet and mobile phones. Further, ‘Idol’s’ producers have supplemented more traditional models of communication by taking direct and explicit account of how and where audiences use a wide variety of media. The broad range of opportunities to participate in ‘Idol’ is central to its success. It demonstrates a willingness on the part of producers to accept the necessity of bending somewhat to the audience’s existing and evolving uses of the media. In short, they are simply not all that fussy about how participation actually happens so long as it does. Producers allow for many kinds of participation in order to constantly offer more specific and more active levels of involvement. ‘Idol’ has transformed consumer relationships within the music industry by coaxing into being ever more intimate, active and reciprocal relationships over the course of the contest by encouraging increasingly specific acts by consumers to complete a continual series of transactions. The Use and Reuse of Celebrity In many quarters, ‘Australian Idol’ has become a byword for bullshit. The competition seems rigged and the contestants are not seen as ‘real’ musicians in large part because their experience appears to be so transparent and so transparently commercial. As the mythology of the music industry has traditionally had it, deserving pop stars are established as celebrities through what is a more or less a linear progression. Early success is based on a carefully constructed sense of authentic cultural production. Credibility is established through a series of contestable affiliations to ostensibly organic music cultures, earned through artistic development and the hard slog of touring and practice (see Maxwell 118). The fraught possibilities of mainstream success continually beckon to ‘real’ musicians as they either ‘crossover’ or remain independent all the while trying to preserve some elusive measure of public honesty. As this mythology was implicitly unavailable to the producers of ‘Idol,’ a different kind of authenticity had to be constructed. Instead of a ‘battles of the bands’ (read: brands) contest, ‘Idol’ producers chose to present ‘unbranded’ aspirants (“Sydney Audition”). These hopefuls are presented as appealingly ambitious or merely optimistic individuals with varying degrees of talent. Those truly blessed, not only with talent but the drive to work it into saleable shape, would be carefully chosen from the multitude and offered an opportunity to make the most of their inherent yet unformed ability. Thus, their authenticity was assumed to be an implicit, inchoate presence, requiring the guiding hand of insiders to reach full flower. Through the facilitation of competition and direction provided in the form of knowledgeable music industry veterans who never tire of giving stern admonitions to indifferent performers who do not take full advantage of the opportunity presented to them, contestants are asked to prove themselves through an extended period of intense self-presentation and recreation. The lengthy televised, but tightly-edited auditions, complete with extensive commentary and the occasional gnashing of teeth on the part of the panel of experts and rejected contestants, demonstrate to us the earnest intent of those involved. Importantly, the authenticity of those proceeding through the contest is never firmly established, but has to be continually and strategically re-established. Each weighty choice of repertoire, wardrobe and performance style can only break them; each successful performance only raises the stakes. This tense maintenance of status as a deserving celebrity runs in tandem with the increasingly attentive and reciprocal relationship between the producers and the audience. The relationship begins with what has proved to be a compelling first act. Thousands of ‘ordinary’ Australians line up outside venues throughout the country, many sleeping in car parks and on footpaths, practising, singing and performing for the mobile camera crews. We are presented with their youthful vigour in all its varied guises. We cannot help but be convinced of the worth of those who survive such a process. The chosen few who are told with a flourish ‘You’re going to Sydney’ are then faced with what appears to be a daunting challenge, to establish themselves in short order as a performer with ‘the X factor’ (“Australian Idol” 14 July 2004). A fine voice and interesting look must be supplemented with those intangible qualities that result in wide public appeal. Yet these qualities are only made available to the public and the performer because of the contest itself. When the public is eventually asked to participate directly, it is to both produce and ratify exactly these ambiguous attributes. More than this, contestants need our help just to survive. Their celebrity is almost shockingly unstable, more fleeting than its surrounding rhetoric and context might suggest and under constant, expected threat. From round to round, favourites can easily become also rans–wild cards who limp out of one round, but storm through the next. The drama can only be heightened, securing our interest by requiring our input. As any advertiser can tell you, an effective campaign must end in action on our part. Through text message and phone voting as well as extensive ‘fan management’ through internet chat rooms and bulletin boards (see Stahl 228; http://au.messages.yahoo.com/australianidol/), our channelled ‘viral’ participation both shapes and completes the meanings of the contest. These active and often inventive relationships (http://au.australianidol.yahoo.com/fancentral/) allow the eventual ‘Idol’ to claim the credibility the means of their success otherwise renders suspect and these activities appear to consummate the relationship. However, the relationship continues well beyond the gala final. In a fascinating re-narration of the first series of ‘Australian Idol,’ Australian Idol: The Winner’s Story aired on the Friday following the final night of the contest. The story of the newly crowned Idol, Guy Sebastian, was presented in an hour long program that showed his home life, his life as a voice teacher in the Adelaide suburbs and his subsequent journey to stardom. The clips depicting his life prior to ‘Idol’ were of ambiguous vintage, cleverly silent on the exact date of production; somehow they were not quite in the past or the future, but floated in some eternal in-between. When his ‘Australian Idol’ experience was chronicled, after the second commercial break, we were allowed to see an intimate portrait of an anxious contestant transformed into ‘Your Australian Idol.’ There could be no doubt of the virtue of Sebastian’s struggles, nor of his well-earned victory. ‘New’ footage began with the sudden sensation reluctantly commenting on other contestants at the original Adelaide cattle call at the prompting of the mobile camera crew and ended with his teary-eyed mother exultant at the final decision as she stood in the front row at the Opera House. Further, not only is the entire run of the first series dramatically recounted in documentary format on the Australian Idol: Greatest Moments DVD, framed by Sebastian’s humble triumph, so are the stories of each member of the Final 12 and the paths they took through the contest. These reiterations serve to reinforce not only Sebastian’s status, but the status of the program itself. They confirm the benevolent success of the industry it so dutifully profiles. We are taken behind the curtain, allowed to see the machinery of stardom grind inevitably to a conclusion, knowing we will be allowed back again when the time is right. Whereas ‘Idol’ is routinely pilloried for its crass commercialism, it remains an unavoidable success. Viewers keep tuning in, advertisers still clamour to sponsor all aspects of the production and the CDs keep selling. Most importantly, the music industry has a showcase for its own operations. The structures of feeling it exists to produce take on a kind of subtle explicitness that ensures their perpetuation. Within an industry faced with threats perceived to be foundational, the creators of ‘Idol’ have produced an audacious and arrogant spectacle. They have made a profitable virtue out of an economic necessity. The expensive and unpredictable process of finding and nurturing new talent has not only been made more reliable, but ‘Idol’ has shown that it can actually turn a profit. The brand of celebrity produced by Idol possesses no mere sheen of populist approval, but embodies that more valuable commodity: popular attention, however reluctant or enthusiastic it may be.
