HIV Pre-Exposure Prophylaxis (PrEP) is a public health strategy to reduce the probability of HIV acquisition by people at high risk of exposure. This paper addresses the perception of psychologists in the primary health care public system about the PrEP policy in the state of Mato Grosso, Brazil. Through semi-structured interviews, we sought to understand the therapeutic itinerary in the unit where it takes, as well as the opinions and involvement of the psychologists in the implementation of PrEP. There is a lack of definition regarding the attributions and scope of action of the psychologist in relation to PrEP in health units. In some, reception is mandatory in the first service, in others only if there is a request from the user. The interviews revealed that there are distrust and prejudice in relation to the policy and users' social behaviour, as well as the recognition of the lack of training and dissemination as the primary motivations for the low adherence. Psychologists proved to be important professionals in implementing PrEP. Notwithstanding, more dialogue is needed with management at the local and state level so that doubts, beliefs, and social representations are discussed and do not compromise the effectiveness of the policy.

Contexte : Le Canada a approuvé les objectifs mondiaux du Programme commun des Nations Unies sur le VIH et le Sida visant à mettre fin à l’épidémie du syndrome d’immunodéficience acquise (sida), notamment en réduisant à zéro les nouvelles infections par le virus de l’immunodéficience humaine (VIH) d’ici à 2030. Compte tenu de l’efficacité de la prophylaxie pré-exposition (PPrE) pour prévenir les nouvelles infections, il est important de mesurer et de rendre compte de l’utilisation de la PPrE afin de contribuer à la planification des programmes et des politiques de prévention du VIH. Méthodes : Les estimations annuelles des personnes utilisant de la PPrE au Canada ont été générées pour la période de 2014 à 2018 à partir de l’ensemble de données géographiques de surveillance des ordonnances d’IQVIA. On a utilisé un algorithme pour distinguer les utilisateurs de fumarate de ténofovir disoproxil/emtricitabine (TDF/FTC) pour la PPrE de ceux qui l’utilisent dans le cadre d’un traitement ou de la prophylaxie post-exposition. Nous fournissons le nombre estimé de personnes qui utilisent la PPrE dans huit provinces canadiennes par sexe, groupe d’âge, spécialité du médecin prescripteur et type de paiement. Résultats : Le nombre estimé d’utilisateurs de PPrE a augmenté de façon spectaculaire au cours de la période d’étude de cinq ans : il est multiplié par 21, passant de 460 en 2014 à 9 657 en 2018. La prévalence estimée de la PPrE était de 416 utilisateurs par million de personnes dans les huit provinces en 2018. Presque tous les utilisateurs de PPrE étaient des hommes. L’utilisation a augmenté chez les deux sexes, mais l’augmentation a été plus importante chez les hommes (par 23 fois) que chez les femmes (par 5 fois). L’utilisation a augmenté dans toutes les provinces, bien qu’il y ait des différences entre les provinces relativement à la prévalence de l’utilisation, à la répartition par âge et aux types de médecins prescripteurs. Conclusion : L’utilisation de la PPrE au Canada a augmenté entre 2014 et 2018, ce qui témoigne d’une meilleure connaissance et d’une plus grande adhésion à son utilisation pour prévenir la transmission du VIH. Elle est toutefois inégale en fonction de l’âge, du sexe et de la géographie. Puisque les nouvelles infections au VIH continuent de se produire au Canada, il sera important d’affiner l’utilisation de la PPrE, car les populations à risque plus élevé d’infection au VIH doivent se voir offrir la PPrE dans le cadre de soins de santé sexuelle complets.

Antiretroviral pre-exposure prevention (PrEP) to stop HIV transmission was 1st approved by the USA Food and Drug Administration in 2012. Despite correlations of decreases in new HIV infections being greatest wherever PrE-exposure prophylaxis has been deployed, the uptake of PrE-exposure prophylaxis is insulant, significantly among populations with disproportionate HIV burden. This narrative review seeks to spot individual and general barriers to PrE-exposure prophylaxis usage in African country. A comprehensive search of recent literature uncovered a fancy array of structural, social, clinical, and behavioral barriers, as well as knowledge/awareness of PrE exposure prophylaxis, perception of HIV risk, stigma from care suppliers or family/partners/friends, distrust of care providers/systems, access to school assignment, prices of PrE exposure prophylaxis, and issues around school assignment facet effects/medication interactions. significantly, these barriers might have totally different effects on specific populations in danger. The complete potential of PrE-exposure prophylaxis for HIV interference won't be realized till these problems area unit self-addressed. Ways to realize this goal ought to embody academic interventions, innovative approaches to delivery of HIV care, resource, and DE stigmatization of PrE-exposure prophylaxis and PrE-exposure prophylaxis users. Until then, PrE-exposure prophylaxis `uptake can still be suboptimal, significantly among people who would like it most. Results: Stigma influences uptake of pre-exposure prophylaxis; HIV risk perception does not affect uptake of pre-exposure prophylaxis; and a significant negative relationship between stigma and pre-exposure prophylaxis uptake. Conclusion: This study was conducted to investigate barriers to oral pre-exposure prophylaxis. Significant findings of the study conclude: stigma influences uptake of oral pre-exposure prophylaxis for HIV. Resumo A Profilaxia Pré-Exposição ao HIV (PrEP) tem renovado o otimismo no controle da epidemia de HIV, não obstante seu contínuo crescimento entre os homens que fazem sexo com homens (HSH). A popularidade dos aplicativos de encontro afetivo-sexual e plataformas de mídias digitais na troca de informações e experiências sobre PrEP nas redes sociais de pares é reconhecida, embora sejam escassos estudos sobre contextos, motivações e alcance em termos da prevenção ao HIV. O artigo objetiva compreender a dinâmica das redes de pares virtuais e presenciais entre HSH para a decisão de usar PrEP, sua revelação e publicização. Estudo qualitativo com uso de entrevistas semiestruturadas com 48 usuários de PrEP de cinco cidades brasileiras. A maioria dos entrevistados compartilha informações e experiências sobre PrEP nas redes sociais de pares, contudo sua publicização revela tensões decorrentes da permanência de estigmas associados à homossexualidade e ao HIV. O protagonismo na revelação do uso da PrEP expressa engajamento em conquistar novos usuários. A relevância das redes sociais de pares no compartilhamento de experiências e informações sobre a PrEP tem potencial para a diversificação do público-alvo, ampliação e democratização da cobertura de PrEP no país.

Introduction: Pre-exposure prophylaxis is defined as the use of antiretroviral drugs to prevent HIV acquisition in uninfected individuals. Recognizing the increasing use of informal pre-exposure prophylaxis in Portugal, CheckpointLX, a community clinic targeted to men who have sex with men in Lisbon, Portugal, began offering counselling and follow-up services prior to formal introduction. This study aims to characterize pre-exposure prophylaxis users attending CheckpointLX before formal pre-exposure prophylaxis introduction in Portugal, and those who were referred to pre-exposure prophylaxis in the National Health Service following formal approval of pre-exposure prophylaxis.Material and Methods: Data was collected by peer counsellors between May 2015 and September 2018 and inserted in a database. Medical care followed the European AIDS Clinical Society recommendations for pre-exposure prophylaxis eligibility, initiation and follow-up. For formal pre-exposure prophylaxis, the General-Directorate for Health’s Pre-exposure Prophylaxis guidelines checklist was used.Results: Until the end of May 2018, CheckpointLX had a total of 90 appointments for wild pre-exposure prophylaxis, of which 64 (71%) were first time visits. As for the 380 service users referred to the National Health Service, most were Portuguese (n = 318, 84%), and the mean age was 31 (8.9) years old. Condomless sex in the last six months with partners of unknown HIV status was the most common eligibility criteria (n = 59, 83%).Discussion: Pre-exposure prophylaxis delivery should be complemented with effective information on the importance of immunization and education on safer practices of drug administration, in the scope of broader preventive sexual health care.Conclusion: Much remains to be done in Portugal to ensure that pre-exposure prophylaxis is available to those who need it the most. Offering pre-exposure prophylaxis at community clinics could be a first step.
