The landmark paper discussed in this chapter is ‘Clinical importance of changes in chronic pain intensity measured on an 11-point numerical pain rating scale’, published by Farrar et al. in 2001. The numerical rating scale is now the standard instrument used in chronic pain studies to measure pain intensity. Farrar et al. determined the changes in pain intensity that were clinically significant for studies of chronic pain while measuring the patient’s global impression of change. The paper used pooled data from ten recent studies of pregabalin in 2,724 subjects. The authors reported a consistent relationship between pain intensity and patient global impression of change, regardless of study, disease type, age, sex, study result, or treatment group. A reduction of approximately two points on the numerical rating scale, or of 30% in the global impression of change of pain intensity, represented a clinically important difference.

Temperature is that property of a body which determines whether it gains or loses energy in a particular environment. In classical thermodynamics temperature is defined by the relationship between energy and entropy. Temperature can be defined only for a body that is in thermodynamic and thermal equilibrium; whilst organisms do not conform to these criteria, the errors in assuming that they do are generally small. The Celsius and Fahrenheit temperature scales are arbitrary because they require two fixed points, one to define the zero and the other to set the scale. The thermodynamic (absolute) scale of temperature has a natural zero (absolute zero) and is defined by the triple point of water. Its unit of temperature is the Kelvin. The Celsius scale is convenient for much ecological and physiological work, but where temperature is included in statistical or deterministic models, only thermodynamic temperature should be used. Past temperatures can only be reconstructed with the use of proxies, the most important of which are based on isotope fractionation.

In 1967, Benoît Mandelbrot suggested a mathematical conundrum that involved answering the seemingly straightforward question, ‘How long is the coast of Britain?’1 The answer is surprisingly elusive and dependent on the scale at which one is looking. Increasing the scale unearths greater detail, time and time again, and so the answer grows the closer one looks. The problem is that any measure, at however small a scale, is forced to simplify complex ambiguities that might otherwise reveal further intricacies of their own. This was an entry-point to Mandlebrot’s writings on fractal geometry, but it also chimes with the very ecology and geomorphology of that coast itself, characteristically intricate, ambiguous, and changeable. Large-scale, ocean-facing landforms—such as capes and bays, estuaries, dunefields, and reefs—are well known to have, nestled within them, smaller and often dynamically mobile features such as longshore bars and troughs, berms and beach cusps, not to mention difficult-to-measure caves, inlets, tributaries, and salt marshes. Looking closer still are to be found the ripples, rills, and swash marks of a more minute scale; even within these are to be found the bioturbation structures of intertidal organisms: forms within forms, scales within scales, and worlds within worlds. In the way that it draws the attention down into such minute details as these, while at the same time drawing it up towards an expanse that suggests a space almost planetary in scale, the coast is a highly distinctive geographical environment. And yet it has all too often been overlooked, as if its peripheral relationship to the land has reinforced its peripheral treatment culturally....

James Tenney talks about his “crystal growth” algorithm, an idea that suggests a new “harmonic syntax” for harmonic space. As a quantitative model, it is both suggestively rich for future composition and plausible as a description of the history of tonal expansion. In this algorithm, sets of points are chosen, one by one, in some n-dimensional harmonic space, under the condition that each new point must have the smallest possible sum of harmonic distances to all points already in the set. That is, at each successive stage in the growth of the harmonic lattice, the next ratio added to the set is one whose sum of harmonic distances to each ratio already in the set is minimal. In relation to this algorithm, Tenney considers the Pythagorean pentatonic scale, which may be conceived as a pitch set that arises when extension into the 3,5-plane is just slightly delayed beyond the point where the algorithm would have begun that extension.

Chapter 8 draws attention to meta-governance and how the governing of reforms is affected by how reform processes are organized. The chapter asks how reformers can ensure support for large-scale reforms that are likely to attract profound resistance. The focal point of the chapter is a study of geographical decentralization of central government agencies. The chapter argues that successful meta-governance can be provided for by careful organization of the reform process. The empirical case studied is a large-scale relocation of government agencies in Norway during the early 2000s. In carrying out this reform, the government succeeded against the odds. Most importantly, research has revealed huge constraints on the instrumental control of large-scale reforms in general and of geographical relocation of organizations in particular. Yet, this chapter shows that large-scale reforms can be successfully achieved through careful crafting of the reform organization.
