Resumen
Paget's disease of bone is a focal disorder of bone remodeling accompanied initially by an increase in bone resorption, followed by a disorganized and excessive formation of bone, leading to pain, fractures and deformities. It exhibits a marked geographical variation in its prevalence. In Brazil it predominantly affects persons of European descent. The majority of the reported cases of the disease in Brazil are from Recife, owing to its peculiar mixed European colonization over approximately four centuries. The etiology is complex and involves both genetic and environmental factors. The disease is often asymptomatic and diagnosis is usually based on biochemical markers of bone turnover, radionuclide bone scan and radiological examination. Bisphosphonates, in particular zoledronic acid, are regarded as the treatment of choice for Paget's disease of bone.

Abstract OBJECTIVE AND IMPORTANCE Giant-cell tumors (GCTs) are primary bone tumors that involve long bones in 75 to 90% of patients. They seldom develop in the cranium and are very rare in patients older than 60 years of age. A GCT rarely occurs with Paget's disease; when it does, however, it is most commonly associated with the polyostotic form and tends to involve the craniofacial bones. Pagetic GCTs are less aggressive than GCTs that are not associated with Paget's disease. CLINICAL PRESENTATION We report the case of an 81-year-old woman with a painless left parietal mass and asymptomatic monostotic parietal Paget's disease. INTERVENTION Surgical resection was performed, and histological examination of the lesion demonstrated Paget's disease with a malignant GCT. An incidental, low-grade, small-cell lymphocytic lymphoma also was noted. The patient experienced local recurrence of the malignant GCT and eventually died after developing pulmonary metastases of the malignant GCT. CONCLUSION This case is the first reported example of a patient with a malignant GCT of the cranium associated with monostotic Paget's disease. It provides evidence that not all pagetic GCTs in the cranium are benign, as has been reported.

Aim: To investigate the prevalence of tinnitus and deafness in subjects with Paget's disease of bone.Materials and methods: One hundred and thirty-four subjects in total (77 with Paget's disease) were identified and audiologically examined. Subjects' experience of tinnitus was surveyed via a questionnaire and the results compared with those of age-matched controls.Results: Hearing loss in those with Paget's disease of bone was significantly worse than in controls. Hearing handicap was also much worse in the Paget's subjects. Sixty-one per cent of the Paget's subjects suffered from tinnitus, compared with 36 per cent of the controls.Conclusion: Deafness is a recognized, common sequelae of Paget's disease of bone, and tinnitus is a common accompanying symptom.

ABSTRACT Paget's disease is an idiopathic, non-neoplastic affliction characterized by disordered bone formation and resorption. It affects about 3% of the population. Paget's disease affects mainly the skull, femur, tibia and pelvic bones. Facial skeleton is involved in about 17% of the cases. We present a case of Paget's disease involving maxillary and mandibular skeleton which developed osteomyelitis following extraction of maxillary right posterior teeth.

Resumen
Paget's disease of bone (PD) is a rare disease with late onset whose current prevalence reaches, after the age of 55, PD is characterized by bone fragility, hypertrophy and deformities of the affected bones, caused by foci of increased bone remodeling that contain overactive and large osteoclasts. The consequences are: thickening, decrease in its flexibility, hypervascularization, pathological fractures. Often asymptomatic, Paget's disease never affects the whole of the skeleton but each bone can be affected, it affects in order of frequency the pelvis, the sacrum, spine, femur and skull. We report here the peri operative management of a case of Paget's disease who presented with hemorrhagic shock during intermediate hip prosthesis for femoral neck fracture.
