Orthodontics is one branch of dentistry which deals with dynamic manipulation and function of the stomatognathic system. Likewise, it has to witness the dynamic changes in its diagnosis method and treatment planning. The field of orthodontics has witnessed many revolutionary techniques and evolved enormously in terms of benefiting the patient. CAD/CAM is one promising methodology that has shown a significant impact in modern dentistry and surpassed some of the challenges an orthodontist has been facing all these years. It has changed the view through which we see the patient’s condition. The application of 3D technology allows the practitioner and patient to utilize virtual treatment planning software to better identify case objectives and visualize treatment outcomes. CAD/CAM has many applications in orthodontics, which include aids for diagnosis and treatment planning, clear aligner treatment, customized lingual appliances, customized brackets with patient-specific torque, machine-milled indirect bonding jigs, robotically bent archwires, indirect bonding systems, customization of orthodontic appliances like distalization appliance etc., Acceleration of orthodontic treatment (OT) has received growing attention in recent years, especially in adult patients.Digital workflow is the current trend in orthodontic practice and has speckled every aspect of orthodontics in terms of documentation, study casts, analysis of dental malocclusion, smile designing, treatment planning and fabrication of orthodontic appliances. The current article aimed to establish a narrative description of the digital workflow and applications of the CAD/CAM process in Orthodontics.

Nanotechnology is the science of manipulating matter, measured in the billionths of a nanometer, roughly the size of two or three atoms. It is widely used in our day-today life including its use in medicine and is considered as a vital current technology of the 21st century based on its economic and scientific potential. Its application is being experimented in various domains in orthodontics, from surface coatings to the development of novel materials. Orthodontic materials must have specific characteristics such as biological safety, functionality, and adequate tissue response. They have to pass specific biocompatibility tests to meet regulatory standards. Any material used in oral cavity might encourage unnecessary disturbance due to its complex and varied environment. The nanomaterials have many advantages in the field of orthodontics, especially with improved mechanical and antimicrobial properties. Nanoparticles can easily penetrate tissues and can affect biological behaviours at different levels. The introduction of nanotechnology gives better opportunities to both patient and orthodontist to new physicochemical, mechanical, and antibacterial properties of nanosized materials and can be used in coating orthodontic wires, elastomeric ligatures, and brackets, producing shape memory polymers and orthodontic bonding materials. The present review article focuses on the application of nanoparticles in orthodontics. This article presents a brief overview of nanotechnology, types of nanoparticles, biological safety of different nanoparticles used in orthodontics and their applications in the field of dentistry and orthodontics. The software enables the analysis of dental mobility and the provision of digital treatment choices. Cephalometric analysis, digital imaging, intraoral and extraoral image capture, morphing capability to show patients what their teeth could look like after treatment, case presentation capability, treatment planning capability, and oral maxillofacial surgery applications are typical features of orthodontic practice software and orthodontic digital treatment planning systems. You can give patients graphics through these technologies that will assist them in understanding the benefits of proceeding with orthodontic or surgical-orthodontic therapy. Since patients can now comprehend and see the treatment plan, communication between clinicians and patients has also become easier due to digitization. Thanks to digital orthodontics, getting a healthier and straighter smile is much easier. Because of this, the majority of this assessment is based on software that is currently accessible for orthodontic applications.

Abstract The formation of white spot lesions, or enamel demineralization, around fixed orthodontic attachments is a common complication during and following fixed orthodontic treatment, which marks the result of a successfully completed case. This article is a contemporary review of the risk factors and preventive methods of these orthodontics scars. Preventive programmes must be emphasized to all orthodontic patients. The responsibility of an orthodontist is to minimize the risk of the patient having decalcification as a consequence of orthodontic treatment by educating and motivating the patients for excellent oral hygiene practice. Prophylaxis with topical fluoride application should be implemented: high-fluoride toothpastes, fluoride mouthwashes, gels and varnishes during and after the orthodontic treatment, especially for patients at high risk of caries.

Nanotechnology has gained importance in recent years due to its ability to enhance material properties, including antimicrobial characteristics. Nanotechnology is applicable in various aspects of orthodontics. This scientific work focuses on the concept of nanotechnology and its applications in the field of orthodontics, including, among others, enhancement of antimicrobial characteristics of orthodontic resins, leading to reduction of enamel demineralization or control of friction force during orthodontic movement. The latter one enables effective orthodontic treatment while using less force. Emphasis is put on antimicrobial and mechanical characteristics of nanomaterials during orthodontic treatment. The manuscript sums up the current knowledge about nanomaterials’ influence on orthodontic appliances.

With the development of digital information technology and big data technology, the medical industry has also undergone tremendous changes. Traditional medical treatment mainly relies on the technical experience of the attending doctor for treatment, and there is no sophisticated instrument or scientific analysis system to assist in treatment. With the improvement of people’s living standards, people’s attention to teeth has increased significantly. Traditional orthodontics is based on the subjective judgment of orthodontists and manual treatment. Due to the differences in the experience of orthodontists, the traditional orthodontic effect is often very poor. Using digital information and big data technology to carry out quantitative diagnosis and treatment analysis of teeth, 3D modeling, and simulation of prosthesis, personalized treatment of the prosthesis model, and finally applied to orthodontics, digital-based orthodontics make the orthodontic diagnosis and treatment process evidence-based, safer, and more effective. This article compares orthodontics and traditional oral orthodontics based on the mathematical model, to analyze the comfort of orthodontics, the aesthetics of orthodontics, the matching degree of aligners, and the stability of the environment in the periodontal ligament. It is concluded that the average orthodontic comfort based on the mathematical model is 85.6%, and the average aesthetic degree is 64.0%, which are more than 20% better than traditional orthodontics. It is also superior to traditional orthodontics in terms of the degree of matching of the appliance and the stability of the environment in the periodontal ligament. Therefore, the combination of mathematical models and orthodontics can lead to better orthodontic results.

Methods: With objectives centered on evaluating shellac’s effectiveness, biocompatibility, and impact on patient outcomes, a comprehensive search across multiple databases was conducted, including PubMed, Scopus, and Web of Science. This study’s selection criteria targeted studies assessing shellac’s use in orthodontic applications, measuring treatment effectiveness, biocompatibility, and patient satisfaction while excluding those not directly involving orthodontic applications or lacking empirical data. Results: Through a qualitative synthesis of the extracted data—encompassing study design, sample size, treatment outcomes, and adverse effects—the findings reveal shellac’s potential benefits in orthodontics, such as enhanced patient comfort and comparable treatment outcomes to traditional materials. However, the review also notes variability in study designs and outcomes, indicating the need for further research. Conclusions: This study concluded that shellac presents a promising alternative in orthodontic materials, recommending additional studies to standardize assessment methodologies and confirm its long-term advantages.

Objectives: Orthognathic surgery is the only treatment option for some cases in orthodontics. Two main goals were aimed in this study. The first goal was to determine the top three keywords that Google searchers look for “orthognathic surgery” using the Google Trends™ application. The second was to assess the quality and reliability of the webpages yielded by searching those four keywords on Google. Materials and Methods: Google Trends was searched using the keyword “Orthognathic Surgery.” Then, Google was searched using keywords: “Orthognathic surgery” and the other first three keywords. An oral surgeon and an orthodontist analyzed the quality and reliability of the websites using the DISCERN assessment tool. Results: The correlation between evaluators was low. Forty-six of the 70 websites were the websites of different oral and maxillofacial clinics and orthodontic clinics. The percentage of websites was 11% for personal doctor websites, 10% for academic journals, 7% for institutes, 6% for orthodontic clinics, and 1.4% for dental clinics. Overall rating scores of the websites for researchers were 2.41±1.35 for oral surgeon and 1.99±1.49 for orthodontist. Conclusions: The reliability and quality of the information in all of the websites was very low. The data on the websites did not have any scientific review processes such as review-peer review.

The aim of this review article is to encourage and promote the orthodontic community to integrate digital elements in their practice. This review article discusses in detail about the various aspects of digital orthodontics involving digital office, study models, three-dimensional imaging, rapid prototyping, virtual treatment planning, artificial intelligence and role of robots. This review article provides an insight into the capabilities and clinical application on currently available digital orthodontic technological systems.

Background: Reverse engineering (RE) or back engineering is a process that analyzes a physical object to obtain the primary data of the same project. RE technologies have different applications in industrial settings and productive chains; however, with the advent of digital technologies in dentistry and orthodontic fields, they are involved in the new diagnostic and clinical digital workflow. For example, 3D model scanning, 3D facial scanning, models superimposition, digital orthodontic setup, anatomical volumetric assessment, soft tissue analysis, orthodontic digital guided systems, and prototyped orthodontic appliances represent a few examples of the application of RE in orthodontics. Moreover, clinicians can manipulate the data derived from original digital file to enhance diagnosis and communication with other clinicians and dental technicians; however, RE and digital technologies systems are not exempt from shortcomings, including costs and knowledge curve. In this regard, the aim of the present manuscript was to describe the use of reverse engineering technologies in modern digital orthodontics and provide helpful information for those specialists who are at the beginning of the transition from analogic to digital orthodontic workflow.

There was a need to establish an educational opportunity for general dental practitioners with an interest in orthodontics, which would lead to a diploma. The diploma in primary care orthodontics provides this opportunity and has established clear standards of orthodontic patient care for primary care practitioners with a special interest in orthodontics. Demand for the course leading to the diploma in primary care orthodontics has continued to grow, with the cohort for the October 2009 intake almost completely fulfilled a month ahead of the application deadline.

Background: There is a constant pursuit for substituting orthognathic surgical options by minimally invasive pre-orthodontic surgical procedures. Application of osseous resective surgery for alveolar reshaping is referred to here as "surgical periodontics for accelerated orthodontics". A parallel randomized clinical trial was designed to evaluate the clinical outcome of class 2 division 1 malocclusion with skeletal discrepancy using pre-orthodontic surgical procedures, comparing periodontally accelerated osteogenic orthodontics with surgical periodontics for accelerated orthodontics.Methods: Twenty-four adult orthodontics patients selected for this study were randomly divided into 2 equal groups. One group was treated with periodontally accelerated osteogenic orthodontics with augmentation grafting and the other was with surgical periodontics for accelerated orthodontics. These procedures were followed by fixed orthodontics treatment. Comparative evaluation of alveolar bone thickness was done by cone beam computed tomogram for both the groups.Results: The cephalometric parameters, A point nasion B point (ANB) angle and over jet of these subjects before and after the surgical interventions at three, six and twelve month’s intervals were compared to the base values, showing changes within 3 months when treated with surgical periodontics for accelerated orthodontics and within 6 months when treated with periodontally accelerated osteogenic orthodontics, without significant change in periodontal supporting alveolar bone thickness.Conclusions: Surgical periodontics for accelerated orthodontics and periodontally accelerated osteogenic orthodontics are effective pre-orthodontics surgical procedures for accelerating orthodontic treatment, without bringing any change in periodontal alveolar bone thickness.

The aim: The purpose of the study was to determine the indications for the use of articulator in orthodontics based on the results of the analysis of scientific and foreign literature. Materials and methods: 23 scientific sources were selected, from more than 11 countries over the past 39 years (1981 – 2020) devoted to the use of orthodontics articula¬tors. As a result of processing 23 key international scientific papers on the use of articulators in orthodontics, we can talk about three areas of opinion regarding this problem. Conclusions: To sum up 30.34% of authors consider the articulator to be an obligatory orthodontist’s tool for correct diagnosis. The idea of 34.78% of specialists is that the articulator cannot simulate the physiology of the TMJ – therefore, it is more a source of additional errors than an assistant in the orthodontic diagnostics. The rest of the scientists (34.78%) agree that it is advisable to use the articulator selectively – namely, if the doctor deals with “complex” orthodontic pathologies.

Corticotomy-facilitated orthodontics is a clinical treatment modality comprising the application of conventional orthodontic forces combined with selective decortication of the alveolar process of the bone, which generates a localized process of bone remodeling (turnover) that enables accelerated orthodontic tooth movement. Compared with conventional orthodontic treatment, corticotomy-facilitated orthodontics is associated with reduced treatment time and reduces the frequency of apical external root resorption; however, this modality increases morbidity and financial costs. Although the clinical outcomes of corticotomy-facilitated orthodontics appear favorable, no results of evidence-based investigations of long-term outcomes are available in the literature, and the long-term effects of corticotomy-facilitated orthodontics on the teeth and periodontium are unclear. This narrative review discusses the biological events associated with corticotomy-facilitated orthodontics. Authoritative articles found in relevant databases were critically analyzed and the findings were integrated and incorporated in the text.

Orthodontic anchorage is a necessity for every treatment and must be carefully evaluated by the orthodontist. It is defined as the resistance to unwanted dental movement of a tooth or a number of teeth by using different techniques. The purpose of the present paper is to highlight the subperiosteal anchorage applied to orthodontics; this technique has been debated in the literature and the purpose here is to summarize the fields of application. During the first check of previous literature 548 results were found, which have been reduced to 19 selected papers after application of the inclusion criteria and the elimination of duplicates. Multiple electronic databases were searched from 1 January 1995 to 31 December 2020 in order to identify papers eligible for current review. The data obtained by this review underlined the versatility of onplants used as absolute anchorage during orthodontic treatments, the advantages and disadvantages, the biomechanical properties and survival rates, and the clinical procedure. Further clinical studies and research are required to explore other kinds of application and to state specific guidelines; however, this study represents an update and a starting point for clinicians who want to use these devices and for further improvement of the technique.

Objective. To investigate the efficacy of orthodontics plus implant anchorage in orthodontic treatment. Methods. This randomized controlled study was conducted on 90 patients who had orthodontic treatment in our hospital between October 2019 and October 2020, and they were assigned to either a control group (n = 45) or an observation group (n = 45) via the random number table method. Patients in the control group received orthodontics while those in the observation group underwent orthodontics plus implant anchorage. The two groups were compared in terms of clinical indexes, efficacy, orthodontic state, adverse reaction rate, quality of life, and satisfaction. Results. After treatment, orthodontics plus implant anchorage led to lower gingival attachment level, gingival bleeding index, plaque index, and periodontal probing depth versus orthodontics alone ( P < 0.05 ); orthodontics plus implant anchorage contributed to a higher efficacy versus orthodontics alone (91.11% vs. 73.33%, P < 0.05 ); orthodontics plus implant anchorage resulted in smaller molar displacement, larger protrusion distance of the upper central incisor, and larger inclination angle of the upper central incisor ( P < 0.05 ); orthodontics plus implant anchorage was associated with fewer adverse reactions (4.44% vs. 26.67%, P < 0.05 ) and a higher quality of life scores versus orthodontics alone ( P < 0.05 ); orthodontics plus implant anchorage yielded a higher satisfaction level versus orthodontics alone (95.56% vs. 66.67%, P < 0.05 ). Conclusion. The orthodontics plus implant anchorage offers a promising solution in orthodontic treatment. It is conducive to restoring dental indicators and improving quality of life and satisfaction. It is therefore worthy of application.

Poor oral hygiene has been considered one of the main problems routinely faced in the orthodontic treatment. Orthodontic appliance creates an environment that provides mineral loss from the dental enamel. Such condition is clinically seen as white spot lesions and cavitations in the most severe cases. The aim of this study was to evaluate the effects of a fluoride varnish application as a caries prevention method for clinical orthodontics. The experiment analyzed dental enamel adjacent to orthodontics accessories after treatment. In addition, it was observed the calcium, phosphorus and fluoride contents on enamel treated with a fluoride varnish. The results showed that fluoride varnish application is a simple and fast technique that could be useful in preventing enamel demineralization associated to orthodontic treatment. Scanning electron microscopy revealed significant amount of calcium fluoride-like material deposited on enamel and energy dispersive x-ray analysis demonstrated a large incorporation of calcium and fluoride to the enamel of the treated specimens. It was concluded that fluoride varnish could indeed be considered an efficient preventive method to enhance enamel resistance against the cariogenic challenges during orthodontic therapy.

Nanotechnology is applied in many fields, such as medicine (nanomedicine) and dentistry (nano-dentistry). The purpose of these innovations and research in this field is to improve human life and health. This article aims to summarize and describe what the most recent and known innovations of nanotechnology in dentistry are, focusing on and paying particular attention to the branch that is orthodontics, and on the application of new nanomaterials in the realization, for example, of orthodontic elastomeric ligatures, orthodontic power chains, and orthodontic miniscrews. Background/Aim: The objective of this research was to evaluate the level of oral hygiene, the prevalence of caries, and the condition of the soft tissues in children and adolescents referred for their first orthodontic examination. Material and Methods: The research was conducted at the Department of Orthodontics of Sarajevo University - at the Faculty of Dentistry with the Dental Clinical Center. A total of 300 patients with mixed and permanent dentition who were referred for their first orthodontic examination were included in the study. The patients were offered to sign a Letter of Informed Consent for this study. All patients were assigned a Decayed, Missing, and Filled Teeth (dmft/DMFT) index. Periodontal indices were evaluated on all first permanent molars and all permanent central incisors. The periodontal indices included the determination of PI,CI,GI, PBI. Results: The results were processed with the application of standard statistical methods, by using the SPSS computer program for statistical analyses (SPSS-Statistical Package for Social Sciences) Version 21.0. The results of this study show a high dmft and DMFT indices in patients referred for orthodontic examination. The dmft index value in the respondents with mixed dentition was 3.0 (1.0 - 6.0). The DMFT index value in the respondents with permanent dentition was 4.0 (0.0 - 6.0), while in the respondents with mixed dentition, it was 1.0 (0.0 - 3.0). Our research also shows that out of 300 patients referred to an orthodontist for their first orthodontic examination, 250 of them (83.33%) have demonstrated an increased value of the Plaque index. Conclusions: The results of this research should be peculiarly alarming for pedodontists and general dentists, cautioning them to implement caries prevention measures more intensively and make referrals to an orthodontist only in case of those patients who have been cured previously.

3D modeling is often used in orthodontics. Most commonly used software today is problem- oriented CAD system (OrthoCAD and others), as well as general CAD software for engineering modeling. Both methods require definition and use of various geometric entities to describe and monitor orthodontic status, but still there have been more than one approach to definition of geometric entities that characterize orthodontic parameters. The aim of this study was to define the referent geometric entities (RGE) on 3D models, perform their classification and provide examples of their application. For defining and monitoring orthodontic parameters the following RGE groups are used: (a) basic geometric entities (point, line, straight line, plane, curve, curved surface); (b) derived geometric entities (coordinate origin, coordinate system, coordinate plane, axis, edge, perspective); and (c) anatomical geometric entities (surfaces, points). In this study, using 11 examples is shown how orthodontic parameters can be modeled over three classification groups RGE. Presented analysis and RGE examples indicate that RGE give interdisciplinary and systematic approach to computer modeling in orthodontics, and create a basis for development and implementation of methods of anatomical features in orthodontics which can be used to set up an integrated orthodontic parameter.

Background: Nowadays the use of intraoral scanners has become a routine practice in orthodontics. It allows the introduction of many treatment innovations. One should consider to what extent intraoral scanners have influenced the everyday orthodontic practice and in what direction should the further research in this field be conducted. This study is aimed to systematically review and synthesize available controlled trials investigating the accuracy and efficacy of intraoral scanners for orthodontic purpose to provide clinically useful information and to direct further research in this field. Methods: A literature search of free text and MeSH terms was performed by using MedLine (PubMed), Scopus, Web of Science and Embase. The search engines were used to find studies on application of intraoral scanners in orthodontics (from 1950 to 30 September 2020). The following keywords were used: “intraoral scanners AND efficiency AND accuracy AND orthodontics”. Results: The number of potential identified articles was 71, including 61 from PubMed, two from Scopus, three from Web of Science and five from Embase. After removal of duplicates, 67 full-text articles were analyzed for inclusion criteria, 16 of them were selected and finally included in the qualitative synthesis. Conclusions: There are plenty of data available on accuracy and efficacy of different scanners. Scanners of the same generation from different manufacturers have almost identical accuracy. This is the reason why future similar research will not introduce much to the orthodontics. The challenge for the coming years is to find new applications of digital impressions in the orthodontic practice.

In recent years, the use of orthodontics has increased, despite the benefits it provides, and the damage caused by orthodonticscannot be overlooked. In the study current, our aim is to assess the prevalence of oral parasites (Entamoeba gingivalis andTrichomonas tenax) among individuals who use orthodontic appliances (fixed and mobile) and individuals withoutorthodontics as a control group. The study included 103 participants, 53(42 females and 11 males) orthodontics patients, and50(37 females and 13 males) non-orthodontics participants (control). collected two specimens for each participant, formicroscopic examination. Chi-square and Fisher's tests were performed to detect any association significant betweeninfection with parasites and orthodontic applications. The results show the rates of infection with these parasites were higherin orthodontic disease, where the percentage of infection with only E. gingivalis was 37.7% in orthodontic patients, while32.0% was in control. The rate of infection with only T. tenax was 5.7% in orthodontic patients while the percentage was 4.0%in the control group. The rates of infection with both parasites (E. gingivalis and T. tenax) were the highest in orthodonticpatients with a percentage of 41.5% and 14.0% in the control group The statistical analysis demonstrated a significantcorrelation between infection and the use of orthodontics, with a level of significance at P≤ 0.05. Also, the effect of otherfactors on the incidence of oral parasites such as (age, gender, gingivitis, orthodontic type, and dental calcification) wasstudied and compared between the two groups. In the study, there is no significant between infection age and sex, and type oforthodontic. Orthodontic appliances can lead to the accumulation of oral parasites which leads to the development of variousdiseases such as periodontal and gingivitis. Therefore, proper oral hygiene practices and regular dental check-ups should beemphasized during orthodontic treatment to prevent such conditions from emerging.

Over the past two decades, artificial intelligence (AI) and machine learning (ML) have undergone considerable development. There have been various applications in medicine and dentistry. Their application in orthodontics has progressed slowly, despite promising results. The available literature pertaining to the orthodontic applications of AI and ML has not been adequately synthesized and reviewed. This review article provides orthodontists with an overview of AI and ML, along with their applications. It describes state-of-the-art applications in the areas of orthodontic diagnosis, treatment planning, growth evaluations, and in the prediction of treatment outcomes. AI and ML are powerful tools that can be utilized to overcome some of the clinical problems that orthodontists face daily. With the availability of more data, better AI and ML systems should be expected to be developed that will help orthodontists practice more efficiently and improve the quality of care.

Entamoeba gingivalis (E. gingivalis) and Trichomonas tenax (T. tenax) are two species of parasitic protozoans that inhabit the human buccal cavity. Poor hygiene increases infection with these parasites. There has recently been a need to use orthodontics, despite its benefits, but the damage caused by orthodontics cannot be overlooked. The current study aims to assess the prevalence of oral parasites among individuals who use orthodontic appliances (fixed and mobile). The study included 200 participants, 100(76 females and 24 males) orthodontics patients, and 100(75 females and 25 males) nonorthodontics participants (control). Gathered one pair of swabs from each participant for microscopic analysis, culture, and implementation of the nested PCR method. Chi-square and Fisher’s tests were conducted to identify any statistically significant connection between parasitic infections and orthodontic treatments. The findings indicated that orthodontic patients had elevated infection rates with these parasites. Specifically, the infection rate of E. gingivalis in orthodontic patients was more infected (47%) than in non-orthodontic subjects (25%) which were the control. The prevalence of T. tenax was only 2.0% in the orthodontic patients while it was 1.0% in the control group. The prevalence rates of both parasites (E. gingivalis and T. tenax) in the orthodontic patients exhibited the highest prevalence of 19% and 16% respectively among the control subjects. The study found a significant correlation between infection and orthodontic applications at P= 0.01. The effects of other factors on oral parasite infection such as (sex, age, and orthodontic type) were also studied and compared between the two groups.

Objective: To study the value of microscrew implant anchorage in orthodontic treatment. Methods: A total of 80 cases received orthodontic treatment in recent two years were selected and divided into experimental group and control group based on their received orthodontic measures, each group contains 40 cases. The control group was treated with general orthodontic treatment plan, while the study group received microscrew implant anchorage for the treatment. The clinical conditions of the two groups were counted and observed. Results: After different treatments, the effect of the study group was significantly better than that of the control group. All the indicators (including the improvement of molar displacement, incisor inclination angle and incisor convex distance) were available. In addition, there are obvious differences between the two groups in the occurrence of adverse reactions including inflammatory reaction, soft tissue edema and discomfort. Conclusion: In oral clinic, microscrew implant anchorage can achieve ideal curative effect for those who need orthodontics. Besides, its safety is relatively high, which is worth popularizing widely.

Malocclusion is one of the three major diseases, the incidence of which could reach 56% of the imperiled oral and systemic health in the world today. Orthodontics is still the primary method to solve the problem. However, it is clear that many orthodontic complications are associated with courses of long-term therapy. Photobiomodulation (PBM) therapy could be used as a popular way to shorten the course of orthodontic treatment by nearly 26% to 40%. In this review, the efficacy in cells and animals, mechanisms, relevant cytokines and signaling, clinical trials and applications, and the future developments of PBM therapy in orthodontics were evaluated to demonstrate its validity. Simultaneously, based on orthodontic mechanisms and present findings, the mechanisms of acceleration of orthodontic tooth movement (OTM) caused by PBM therapy were explored in relation to four aspects, including blood vessels, inflammatory response, collagen and fibers, and mineralized tissues. Also, the cooperative effects and clinical translation of PBM therapy in orthodontics have been explored in a growing numbers of studies. Up to now, PBM therapy has been gaining popularity for its non-invasive nature, easy operation, and painless procedures. However, the validity and exact mechanism of PBM therapy as an adjuvant treatment in orthodontics have not been fully elucidated. Therefore, this review summarizes the efficacy of PBM therapy on the acceleration of OTM comprehensively from various aspects and was designed to provide an evidence-based platform for the research and development of light-related orthodontic tooth movement acceleration devices.

ABSTRACT Periodontal accelerated osteogenic orthodontics (PAOO) is a procedure applying the clinical science of alveolar corticotomy, particulate bone grafting, and the application of orthodontic forces, for accentuated tooth movement. This is theoretically based on the bone healing pattern known as the regional acceleratory phenomenon (RAP). A series of 12 cases, including 8 females and 4 males, were included in the study. The cases were referred from the Department of Orthodontia to the Department of Periodontology, ADC (R&R), having bimaxillary protrusion with an increased overjet. After initial orthodontic alignment, buccal corticotomy procedure was planned. A full-thickness mucoperiosteal flap was reflected from maxillary canine to canine beyond the root apices. Vertical corticotomy cuts were given in the alveolar bone with piezo blades 2 mm apical to the crestal bone in the inter-radicular space midway between the root prominences and were joined by the horizontal cuts apically. Demineralized freeze-dried bone allograft (DFDBA) was placed in the surgical area. The flaps were sutured and pack was placed. Orthodontic retraction was started 2 weeks after the corticotomy procedure. Using this procedure, the treatment objectives were met in just half to one-third of the reported conventional treatment time and the large overjet was reduced to normal. Pre- and posttreatment clinical parameters were recorded, statistically analyzed, and corroborated with similar orthodontic treatment procedures without the use of the corticotomy technique. The present periodontal (PDL) intervention results in quick and stable results for the correction of bimaxillary protrusion cases. It enhances the esthetics and posttreatment orthodontic stability. Aggressive periodontitis is a type of periodontitis with early onset and rapid progression and mostly affecting young adults who occupy a large percentage of orthodontic patients. The role of the orthodontist is important in screening the disease, making a provisional diagnosis, and referring it to a periodontist for immediate treatment. The orthodontist should be aware of the disease not only before starting the appliance therapy, but also during and after the active mechanotherapy. The orthodontic treatment plan, biomechanics, and appliance system may need to be modified to deal with the teeth having reduced periodontal support. With proper force application and oral hygiene maintenance, orthodontic tooth movement is possible without any deleterious effect in the tooth with reduced bone support. With proper motivation and interdisciplinary approach, orthodontic treatment is possible in patients with controlled aggressive periodontitis.

Artificial intelligence (AI) has already arrived in many areas of our lives and, because of the increasing availability of computing power, can now be used for complex tasks in medicine and dentistry. This is reflected by an exponential increase in scientific publications aiming to integrate AI into everyday clinical routines. Applications of AI in orthodontics are already manifold and range from the identification of anatomical/pathological structures or reference points in imaging to the support of complex decision-making in orthodontic treatment planning. The aim of this article is to give the reader an overview of the current state of the art regarding applications of AI in orthodontics and to provide a perspective for the use of such AI solutions in clinical routine. For this purpose, we present various use cases for AI in orthodontics, for which research is already available. Considering the current scientific progress, it is not unreasonable to assume that AI will become an integral part of orthodontic diagnostics and treatment planning in the near future. Although AI will equally likely not be able to replace the knowledge and experience of human experts in the not-too-distant future, it probably will be able to support practitioners, thus serving as a quality-assuring component in orthodontic patient care.

Recent advances in developmental, molecular and cellular biology as well as biomedical technologies show a promising future for crossing the gap between biomedical basic sciences and clinical orthodontics. Orthodontic research shall utilise the advances and technologies in biomedical fields including genomics, molecular biology, bioinformatics and developmental biology. This review provides an update on the novel and promising evolutions in biomedicine and highlights their current and likely future implementation to orthodontic practice. Biotechnological opportunities in orthodontics and dentofacial orthopaedics are presented with regards to CRISPR technology, multi-omics sequencing, gene therapy, stem cells and regenerative medicine. Future orthodontic advances in terms of translational research are also discussed. Given the breadth of applications and the great number of questions that the presently available novel biomedical tools and techniques raise, their use may provide orthodontic research in the future with a great potential in understanding the aetiology of dentofacial deformities and malocclusions as well as in improving the practice of this clinical specialty.

In the age of artificial intelligence (AI), technological progress is changing established workflows and enabling some basic routines to be updated. In dentistry, the patient’s face is a crucial part of treatment planning, although it has always been difficult to grasp in an analytical way. This review highlights the current digital advances that, thanks to AI tools, allow us to implement facial features beyond symmetry and proportionality and incorporate facial analysis into diagnosis and treatment planning in orthodontics. A Scopus literature search was conducted to identify the topics with the greatest research potential within digital orthodontics over the last five years. The most researched and cited topic was artificial intelligence and its applications in orthodontics. Apart from automated 2D or 3D cephalometric analysis, AI finds its application in facial analysis, decision-making algorithms as well as in the evaluation of treatment progress and retention. Together with AI, other digital advances are shaping the face of today’s orthodontics. Without any doubts, the era of “old” orthodontics is at its end, and modern, face-driven orthodontics is on the way to becoming a reality in modern orthodontic practices.

ABSTRACT Intrusion of single anterior tooth done by fixed orthodontic appliance is a tedious procedure for orthodontist as well as for the patient which may be quite expensive and time consuming. The use of a new modified removable orthodontic appliance is illustrated in two case reports in this article which is especially beneficial for such cases. This removable appliance is comprised of a labial bow of 21 gauge stainless steel wire with soldered high labial bow of 19 gauge stainless steel wire which has a soldered hook permitting the use of elastic. Adams clasp and pinhead clasps are used for retention of the appliance. During intrusive force application, one component is for intrusion of tooth and other component is for labial movement. This appliance is fabricated in such a manner that the labial movement is prevented by labial component of wire and is converted into pure intrusive force as much as possible. Force levels used for tooth movement are kept within physiologic limits while using this appliance. A though, this appliance is advantageous over fixed orthodontics due its simpler and quicker chair-side procedure and low cost of treatment associated with patient comfort, patient co-operation is must for the success of this therapy. Summary. Today, the need for orthodontic treatment of anomalies of tooth position and occlusion is constantly growing. In the practice of orthodontics use a variety of orthodontic appliances. The most widely used non-removable appliances that allow treatment of adult patients. However, dentists often intensify orthodontic appliances to speed up the end of treatment result. Unfortunately, this has a very adverse effect on periodontal tissues. This article analyzes the data of the dental literature on the impact of orthodontic treatment on the hard tissues of the teeth and, especially, on the periodontium. The main factors of periodontal damage during orthodontic treatment are determined. An analysis of possible areas of prevention of periodontal disease during orthodontic treatment was presented. Aim: To evaluate prevalence and knowledge about halitosis among the patients visiting orthodontic department after application of Orthodontics appliances. Study design: Cross-sectional study Place and duration of study: Department of Orthodontics, Bacha Khan College of Dentistry, Mardan from 1st October 2020 to 30th September 2021. Methodology: A questionnaire based study was carried out on a total of 700 orthodontic patients. Only those patients were included who had orthodontic appliances. Results: 52.30% of the respondents were having prevalence of bad breath. Among those who had prevalence of bad breath, approximately two third (73%) were self-aware of their bad breath. 530 (75.71%) were conscious that they should appointment a dentist while majority were uninformed of the medical term for bad breath i.e. meaning of Halitosis 470 (67.15%). Introduction: Nanotechnology is the technique of manipulating matter at nano level. It has assured promising results in many fields of medicine including dentistry. There has been a wide range of application for nano particles in orthodontics, periodontics and also in prosthetic dentistry. Nano particle incorporated biomaterials has given promising results in many disciples of orthodontics. Nanoparticle coated brackets, arch wires and ligatures have assured promising results in revolutionising the orthodontic treatment mechanics. Temporary anchorage devices are one of the recent inventions which has revolutionised the orthodontic field. They are known to provide efficient means of anchorage and also support different types of tooth movements making them less complicated, although they may be associated with infections in the site of insertion and loss of stability. This article discusses the application of nano-particles in temporary anchorage devices, application of the same to reduce the incidence of peri-implantitis and loss of stability and the clinical evaluation of the same.

ABSTRACT Objectives To investigate content of orthodontic-related videos on YouTube to improve the understanding of orthodontic patients' perceptions and treatment experiences. Materials and Methods A systematic search was conducted on YouTube on March 20, 2018, and updated on August 4, 2019, to identify all relevant videos using search terms “orthodontic,” “orthodontics,” “braces,” and “orthodontic braces.” The data set was captured from YouTube Data API (Application Programming Interface) and stored in an Excel database using a query function written in Python. All videos captured were viewed and categorized by three independent dental investigators using thematic analysis. The top 100 videos (by view count) related to patients' treatment experience were further analyzed using discourse analysis. Results A total of 600 orthodontic videos were screened, and 546 were included in the study. Six main themes were identified: (1) individual review of orthodontic treatment (45.8%, n = 250), (2) entertainment (19.8%, n = 108), (3) education (18.3%, n = 100), (4) advertisements (6.6%, n = 36), (5) time lapse of orthodontic treatment (5.3%, n = 29), and (6) do-it-yourself orthodontics (4.2%, n = 23). Of the top 100 videos related to patient's individual review of treatment, patients' main focuses were on pain (24%), problems with chewing and swallowing (12%), and adhesive removal (10%). Conclusions Orthodontic-related YouTube videos are diverse in nature. The most common video category was video providing an individual review of orthodontic treatment experience. Other popular video categories included entertainment, education, and advertisements. A range of do-it-yourself YouTube videos were also identified. YouTube may provide an opportunity for orthodontic professionals to disseminate health information.

Introduction: Orthodontic anchorage is one of the most challenging aspects of Orthodontics. Preventing undesired movement of teeth could result in safer and less complicated orthodontic treatment. Recently, several reviews have been published about the effects of different molecules on bone physiology and the clinical side effects in Orthodontics. However, the effects of local application of these substances on the rate of orthodontic tooth movement have not been assessed.Objectives: The aim of this research was to analyze the scientific evidence published in the literature about the effects of different molecules on orthodontic anchorage.Methods: The literature was systematically reviewed using PubMed/Medline, Scopus and Cochrane databases from 2000 up to July 31st, 2014. Articles were independently selected by two different researchers based on previously established inclusion and exclusion criteria, with a concordance Kappa index of 0.86. The methodological quality of the reviewed papers was performed.Results: Search strategy identified 270 articles. Twenty-five of them were selected after application of inclusion/exclusion criteria, and only 11 qualified for final analysis. Molecules involved in orthodontic anchorage were divided into three main groups: osteoprotegerin (OPG), bisphosphonates (BPs) and other molecules (OMs).Conclusions: Different drugs are able to alter the bone remodeling cycle, influencing osteoclast function and, therefore, tooth movement. Thus, they could be used in order to provide maximal anchorage while preventing undesired movements. OPG was found the most effective molecule in blocking the action of osteoclasts, thereby reducing undesired movements.

Anchorage control is one of the most essential keys for success in clinical orthodontics. Anchorage loss is a primary concern associated with orthodontic procedures. Numerous devices have been proposed and used for over a century to get the appropriate anchorage. Extraoral anchorages such as headgears or facemasks are the most effective tools but are limited because their effectiveness depends on patient compliance. Using skeletal anchorage, such as Temporary Anchorage Devices (TADs), to retract anterior maxillary teeth is an old technique developed in 1945. Despite their small diameter and short length, TADs can provide stable anchorage for multiple tooth movements, including intrusion, retraction, and protraction. This article examined the various applications of temporary anchorage devices (TADs) in clinical orthodontics. The risk factors and complications of TADs application in clinical orthodontics were identified. As the younger generations of orthodontists enter practice and the academic arenas, TADs use will continue to increase if trends continue as they have in the past several years.

Background: Malocclusion is a disease with a high incidence rate that is harmful to humans’ health. Fixed orthodontics is an effective method for the treatment of malocclusion. However, the orthodontic process takes a long time, requires frequent visits, causes pain, and increases the risk of complications. Since orthodontic treatment is lengthy, painful and unbearable, and even leads patients to abandon orthodontic treatment, therefore, how to shorten orthodontic treatment duration, and reduce pain is a research hotspot in the orthodontic field. Objective: The study aimed to provide an overview of the existing orthodontic accelerating device and introduce their classification, characteristics and development. Methods: This paper reviewed various productions and patents related to the orthodontic accelerating device. The structural characteristics, differentiations, and applications of the existing orthodontic accelerating device are also introduced. Results: The existing orthodontic accelerating devices were analyzed and compared, and the typical characteristics were concluded. The main problems in its development were analyzed, the development trend was foreseen, and the current and future research on the productions and patents related to the orthodontic accelerating device is discussed. Conclusion: The orthodontic accelerating device is composed of a vibration device having electrical stimulation, magnetic field, a low-level laser, and an ultrasonic device according to the application of different physical loads. Orthodontic accelerating device can effectively reduce orthodontic treatment time by 30%-50%, and can reduce the risk of complications and pain. The dose of the physical load determines the effect of the device. So, an optimal loading dose should be selected . Compared with vibrating devices, other types of devices are less used in clinical practice, therefore, such products and patents should be invented in the future.

Malformations of teeth and dental arches can produce functional modifications intermingled with esthetic alterations. Children’s rehabilitation may be long, requiring multiple interventions. One of the main challenges of contemporary orthodontics is to reduce treatment time by accelerating orthodontic tooth movements. Among the currently used methods, micro-osteoperforations (MOPs) are flapless, minimally invasive perforations that induce a local trauma to the bone, increase healing capacity, and accelerate dental movements. The use of MOPs in orthodontics is spreading but there are no definite and recognized protocols for their application. This scoping review collected the available evidence in the effect of MOPs during orthodontic therapy as compared to current treatments, to summarize the evidence. The guidelines proposed by PRISMA-ScR were followed: original clinical studies carried out from 2010 to 2021 were retrieved by medical databases combining the terms “micro-osteoperforations” and “accelerated orthodontic tooth movement”. From a total of 965 articles, nine were finally selected. The studies’ aims, designs, methods, measurements, outcomes, and main findings were very heterogenous, with a duration ranging from 4 weeks to 7 months. This included only Class I malocclusion to any malocclusion. It assessed the effects of MOPs coupled with a variety of orthodontic mechanics on either the retraction of maxillary canines, the distalization of maxillary molars, or the modifications on premolar roots. Mostly, variations in the number, location, and timing of MOPs impeded a global assessment. Overall, most of the studies (six out of nine) reported moderately useful effects of MOPs, one was negative, and only two found significant advantages of MOPs over conventional treatment. The review synthesized the available evidence about MOP applications in orthodontics and identified some important gaps in knowledge that could be starting points for a systematic review of the literature. In conclusion, even if MOPs can accelerate tooth movements, the variety of aims and methods of the published research prevents suggestion of their widespread use.

The advent of artificial intelligence (AI) in medicine has transformed various medical specialties, including orthodontics. AI has shown promising results in enhancing the accuracy of diagnoses, treatment planning, and predicting treatment outcomes. Its usage in orthodontic practices worldwide has increased with the availability of various AI applications and tools. This review explores the principles of AI, its applications in orthodontics, and its implementation in clinical practice. A comprehensive literature review was conducted, focusing on AI applications in dental diagnostics, cephalometric evaluation, skeletal age determination, temporomandibular joint (TMJ) evaluation, decision making, and patient telemonitoring. Due to study heterogeneity, no meta-analysis was possible. AI has demonstrated high efficacy in all these areas, but variations in performance and the need for manual supervision suggest caution in clinical settings. The complexity and unpredictability of AI algorithms call for cautious implementation and regular manual validation. Continuous AI learning, proper governance, and addressing privacy and ethical concerns are crucial for successful integration into orthodontic practice.

(1) Background: The aim of this study was to assess the preventive effect of tooth surface disinfection treatment, in addition to fluoride application, during fixed orthodontic treatment. (2) Methods: An open label randomized control trial for the evaluation of the dental caries preventive procedure was performed for the patients with high caries risk who had been visited at Department of Orthodontics, Tsurumi University Dental Hospital for orthodontics treatment. The follow-up period was six months. White spot lesions (WSLs) were evaluated by quantitative light-induced fluorescence (QLF). Cariogenic bacteria were monitored and evaluated by bacterial culture. In addition, the oral microbiome was evaluated by a next-generation sequence (NGS). (3) Results: By the mixed effect modeling, tooth surface disinfection treatment significantly reduced cariogenic bacteria and all parameters obtained by QLF. (4) Conclusions: Tooth surface disinfection treatment, in addition to PMTC and fluoride application, were effective for dental caries prevention and keeping a healthy microbiome during orthodontic treatment.

The University of Sydney’s Discipline of Orthodontics has been actively pursuing research in the areas of root resorption, sleep apnea, magnets in orthodontics, implants, acceleration of orthodontic tooth movement (OTM), and remote monitoring (RM). Our research has shed light on many specific factors that influence orthodontically induced inflammatory root resorption (OIIRR). We also explored the effects of some of the most discussed acceleration interventions on OTM and OIIRR, such as vibration, micro-osteoperforations, piezocision, low-level laser therapy, light emitting diode, light amplification by stimulated emission of radiation, and pharmacological substances. Further, we have researched into maximizing orthopedic treatment outcomes of maxillary deficient children with use of intraoral force application with utilization of miniscrew-assisted rapid palatal expander. We are currently trialing use of RM to facilitate orthodontic services in the public dental clinics.
