Within maintenance activities and industrial operations, human is subjected to different kind of stresses and situation that could result in mistakes and accidents. The human errors in maintenance and manufacturing are an unexplored latter such that a little focusis invested in this area. The report aims to widen up the understanding of the human error in maintenance and manufacturing area. Aviation and marine operations are the most sectors that are subjected to human errors according tothe literature. There aredifferent types of human error that have effect on quality and overall effectivity. Human reliability models are one method to quantify human errors and usually used for the identification of human errors and HEP calculation. The most common reliability measurement methods are HEART, THERP and SLIM which are used depending on application and industry. As a part of efforts to define differences between those reliability models, literature including different industries is used and itis found that expert judgement influences the success and accuracy of such methods. There are many causes for human errors depending on the application but, communication and procedures followed are the most contributing factors. There is always a probability of existence of human errors as the mistake done by workers are inevitable. Industry 4.0 can help in decreasing human errors through the introduction of operator 4.0 as well as other approaches like training and upgrading organizational standards.

The effective scheduling of fighter aircraft maintenance in the Air Force is crucial to overall mission accomplishment. An effective maintenance scheduling policy maximizes the use of maintenance resources and aircraft availability. Currently, maintenance scheduling is a time consuming process that is carried out by airmen whose sole responsibility is to manually generate a maintenance schedule that balances maintenance requirements and flying requirements. In this thesis, we seek to represent the maintenance scheduling process using a mathematical model that ultimately generates an optimal maintenance schedule. First, we address the scheduling of phase maintenance, the most significant preventative maintenance action, for fighter aircraft. We use a mixed integer program (MIP) to model the phase maintenance scheduling process. The MIP generates a daily maintenance and flying schedule that ensures that the maintenance workload is evenly distributed across the planning horizon. We find that the computational performance of the MIP formulation is less than desirable for large instances of real-world data. Motivated by the need for improved computational performance, we develop an alternative formulation that disaggregates the original MIP into two subproblems that are solved sequentially. The two-stage formulation of the phase maintenance scheduling problem has significantly better computational performance while generating a feasible daily maintenance and flying schedule. We then address the maintenance scheduling process that is unique to aircraft with low-observable (LO) capabilities. The LO capabilities of an aircraft degrade over time according to a stochastic process and require continuous maintenance attention. We show that the characteristics of the LO maintenance process allow it to be modeled as a variant of the mulitarmed bandit (MAB) problem. We then present a variant of the heuristic proposed by Whittle that has been shown to provide near optimal solutions for MAB problems. Applying Whittle's heuristic to the LO maintenance scheduling problem, we generate a simple index policy that can be used to schedule aircraft for LO maintenance. We then compare the index policy to alternate policies and show by simulation that the index policy leads to relatively better fully mission capable (FMC) rates, a common measure of overall fleet health.

In this M.Sc. thesis organisational and technical aspects related to the implementation of an Opportunity Based Maintenance (OBM) scheme for Oil and Gas facilities have been discussed. Organisational aspects of importance include agility and resilience. Agility can be understood as the ability of organisation to benefit from unexpected events. Resilience can be understood as damage control. Logistics support elements from the space industry can also be beneficial to implement in the logistics support of Oil and Gas facilities. Elements from the space industry include the implementation of Onshore Support Centre (OSC), prepositioning of spares and tools and standard procedures for critical equipment. A simplified oil and gas production system has been used as a basis for a Failure Mode Effects and Criticality Analysis (FMECA), identifying critical components that should be considered for Condition Monitoring (CM). The results of the FMECA were that centrifugal compressors and shell and tube heat exchangers were ranked as critical equipment. Gate valves were also ranked as critical but have been neglected in the further analysis due to heat exchangers and compressors being more critical. Several different CM methods have been described, emphasis have been put on detection method, application and words of warning when implementing the method. CM methods relevant to the critical equipment have also been discussed. For compressors the most relevant CM methods includes Vibration monitoring, Power and Load monitoring. In addition to these general methods special software applications are also available through different distributors. For monitoring heat exchangers internal inspection is most commonly used. The inspection interval can be determined by a Risk Based Inspection (RBI) scheme. When an inspection is due Eddy Current Testing (ECT) or gas leak tests can be used to determine diminishing pipe thickness in the tube section of the heat exchanger. HXAM supplied by ABB is also an interesting solution offering real time measurement of the performance of the heat exchangers. Lastly how logistics planning is related to maintenance of oil and gas facilities today have been discussed. How a shift to OBM will impact the logistics planning have also been discussed. Means that can reduce any negative impact that a shift to OBM may have has also been discussed. One such measure is the investment in a fully automated warehouse. Also lessons that can be learnt from the logistics support in the space industry have been emphasised again.

Scheduling of winter maintenance operations such as plowing or salting is a difficult and complex problem. Proper selection and timing of such operations is critical to their effectiveness, however scheduling decisions must often be made with strict time and resource limitations imposed upon them. A decision support system which analyses current road conditions and makes scheduling suggestions based on them would be a valuable step toward improving the quality of treatment, while simultaneously reducing the burden of scheduling on maintenance managers. This thesis proposes a real-time scheduling model based on an Operations Research framework that can be used by maintenance managers to develop and evaluate alternative resources allocation plans for winter road maintenance operations. The scheduling model is implemented as an Integer Linear Program and is solved using off-the-shelf software packages. The scheduling model takes into account a wide range of road and weather condition factors such as road network topology, road class, weather forecasts, and contractual service levels, and produces a vehicle dispatch schedule that is optimal with respect to operating costs and quality of service. A number of heuristics are also explored to aid in efficient approximations to this problem.

The railway infrastructure needs to be safe, reliable and efficient in order to meet the growing demand of sustainable transportation methods. One of the main problems the railway industry faces today is that a higher traffic load increases the need for maintenance, at the same time as it reduces the availability of gaps in the timetables to perform maintenance activities. Unmanned Aerial Vehicles, UAVs, have in recent years been adopted commercially due to their potential of increasing work efficiency and productivity. Different actors in the railway industry have recently started to explore and test the possibilities of implementing UAVs. The objective of this master thesis was to investigate and define use case scenarios where the use of UAVs would create value for railway infrastructure operations and maintenance activities. It is meant for both stakeholders in the railway industry to gain better understanding of capabilities and limitations of UAV technology but also provide recommendations to UAV manufacturers to understand the railway industry and potential UAV applications. Theoretical research and qualitative user studies with UAV professionals and relevant stakeholders within the railway industry were conducted in order to gain insight in the railway industry and to identify potential use case scenarios. The research showed that maintenance activities to a large extent are performed either manually by walking along the tracks which is inefficient, physically demanding and dangerous or by using test/measurement vehicles which require track occupancy. It was concluded that the use of UAVs would mainly create value by; enabling remote inspection and operation, accessing the infrastructure without track occupancy or the need of roads. At the same time, improve the working conditions, efficiency and quality of maintenance activities. The thesis resulted in 15 potential use case scenarios for UAVs in the railway industry and proposals for common UAV solutions based on functional requirements.

Obemannade flygfordon, även kallade drönare, har under de senaste åren tillämpats mer frekvent i kommersiella syften för att bland annat uppnå ökad effektivitet och produktivitet. Aktörer inom järnvägsindustrin har nyligen börjat utforska och testa möjligheterna att använda drönare. Syftet med detta examensarbete var att undersöka och definiera potentiella tillämpningar av drönare med syfte att skapa värde för drift- och underhållsarbete inom järnvägen. Denna rapport är avsedd för intressenter inom järnvägsindustrin att få bättre förståelse för kapaciteten och begränsningar av drönarteknik samt ge rekommendationer till drönartillverkare för att bättre förstå järnvägsindustrin och potentiella användningsområden. Teoretisk undersökning och kvalitativa användarstudier med drönarexperter och relevanta intressenter inom järnvägsindustrin genomfördes för att få insikt i järnvägsindustrin samt för att identifiera problemområden. Studien visade att underhållsverksamheten i stor utsträckning utförs antingen manuellt genom att gå längs spåren vilket är ineffektivt, fysiskt krävande och farligt eller genom att använda test/mätfordon som kräver tillgång till spår. Arbetet resulterade i 15 potentiella tillämpningar av drönare i järnvägsindustrin samt förslag på gemensamma drönarlösningar baserade på funktionella krav. Slutsatsen drogs att tillämpningen av drönare i järnvägsindustrin främst kan skapa värde genom att; på distans utföra underhållsaktiviteter och inspektioner, få tillgång till infrastrukturen utan behov av spår eller vägar. Detta resulterar i förbättrade arbetsförhållanden samt ökad effektivitet och kvalitet på underhållsarbetet.

Detta arbete har utförts tillsammans med SAS, för att undersöka om ett base maintenance koncept med ett intervall på 36 månader skulle vara mer kostnadseffektivt än 24 månader för deras nya Airbus A320 NEO flotta. SAS, precis som alla andra flygbolag utnyttjar sina flygplan på sina egna och specifika vis. Eftersom inga flygbolag opererar på samma sätt kan inte heller underhållet skötas på samma sätt för en specifik flygplanstyp. Underhållsprogrammet behövde därför ses över för att optimeras genom en paketering. Paketeringen utförs för att så många underhållsåtgärder som möjligt skall placeras i större checkar, vilket i sin tur leder till ett effektivare och billigare underhåll. Paketeringen av underhållsåtgärder görs inte enbart ur punktlighetsperspektiv och kostnadsperspektiv, utan det är även ett sätt att garantera flygsäkerhet. Eftersom säkerheten är det absolut viktigaste för ett flygbolag blir detta väldigt intressant, då man inte enbart kan spara pengar på detta, utan kan även garantera säkerhet. Dessa två koncept som paketerats, 24 och 36 månader, jämfördes genom en simulering för att se vilket som var bäst ur ekonomisk synpunkt. De intressanta värdena för simuleringen är de värden som skiljer sig åt mellan de olika koncepten. Dessa är kostnaden per mantimme och antalet gånger en åtgärd kommer att utföras under en given tidsperiod. Genom simuleringen blev det tydligt att ett koncept på 24 månader fortfarande är det mest optimala ur ekonomisk synpunkt för SAS.
This thesis is performed in collaboration with SAS to examine if a base maintenance concept with an interval of 36 months would be more cost-efficient than the old concept of 24 months on their new fleet of Airbus A320 NEO`s. SAS, just like every other airline do have their own specific ways to operate, and therefore the maintenance can’t be done in the same way for a specific aircraft type. The maintenance program needed to be reviewed so it could be rebuilt and packaged. The packaging was done to place as many tasks as possible in the heavy maintenance checks to optimize the maintenance efficiency and cost. Packaging tasks isn’t just performed for punctuality and cost perspectives, but it´s also to ensure flight safety. Safety is the most important part for an airline. The packaging is then very interesting due to not only the cost savings, but also to be able to ensure safety. A simulation between these two concepts were done to compare which one of them that would be the best from an economical perspective. The values that’s interesting for the simulation is the values that differ between the two concepts. These values are the cost per man-hour and the quantity a specific task is being done over a given period. After the simulation, it became clear that the 24 months’ concept is still the optimum out of an economic perspective for SAS operations.

California community college leaders are looking for strategies to sustain facilities and maintenance operations because the governor only approved the allocation of $87.5 million in the 2014-2015 Budget Act for facilities maintenance operations. Guided by the change and strategy theories, the purpose of this multicase study was to explore the strategies that a select group of college leaders have used to sustain or improve their facilities maintenance operations. The data collection process included a review of college planning documents and semistructured interviews with 10 senior administrators from 3 large California community colleges who have used strategies to address sustaining or improving their facilities maintenance operations. Saldana coding and an inductive analysis process were used to identify themes. Triangulation was employed to increase the trustworthiness of interpretations. The analysis revealed the central role of planning as the strategy leaders should employ to improve institutional success. Funding was an additional theme leaders regarded as the issue that most often undermined planning and effective maintenance operations. All participants acknowledged the need for the integration of planning and funding to create institutional success. These findings suggest that community college leaders who use planning, funding strategies, maintenance strategies, and who empower people to sustain facility and maintenance operations can improve the teaching-learning environment. When community college leaders transform the teaching-learning environment, they enable student success. Student success increases the earning power of students that contributes to social change by expanding the tax-base and creating greater economic development.

Maintenance scheduling is an integral part of many complex systems. For instance, without effective maintenance scheduling, the combined effects of preventative and corrective maintenance can have severe impacts on the availability of those systems. Based on current Air Force trends including maintenance manpower, dispersed aircraft basing, and increased complexity, there has been a renewed focus on preventative maintenance. To address these concerns, this thesis develops two models for preventative maintenance scheduling for complex systems, the first of interest in the system concept development and design phase, and the second of interest during operations. Both models are highly complex and intractable to solve in their original forms. For the first model, we develop approximation algorithms that yield high quality and easily implementable solutions. To address the second model, we propose a decomposition strategy that produces submodels that can be solved via existing algorithms or via specialized algorithms we develop. While much of the literature has examined stochastically failing systems, preventative maintenance of usage limited systems has received less attention. Of particular interest is the design of modular systems whose components must be repaired/replaced to prevent a failure. By making cost tradeoffs early in development, program managers, designers, engineers, and test conductors can better balance the up front costs associated with system design and testing with the long term cost of maintenance. To facilitate such a tradeoff, the Modular Maintenance Scheduling Problem provides a framework for design teams to evaluate different design and operations concepts and then evaluate the long term costs. While the general Modular Maintenance Scheduling Problem does not require maintenance schedules with specific structure, operational considerations push us to consider cyclic schedules in which components are maintained at a fixed frequency. In order to efficiently find cyclic schedules, we propose the Cycle Rounding algorithm, which has an approximation guarantee of 2, and a family of Shifted Power-of-Two algorithms, which have an approximation guarantee of 1/ ln(2) ~ 1.4427. Computational results indicate that both algorithms perform much better than their associated performance guarantees providing solutions within 15%-25% of a lower bound. Once a modular system has moved into operations, manpower and transportation scheduling become important considerations when developing maintenance schedules. To address the operations phase, we develop the Modular Maintenance and System Assembly Model to balance the tradeoffs between inventory, maintenance capacity, and transportation resources. This model explicitly captures the risk-pooling effects of a central repair facility while also modeling the interaction between repair actions at such a facility. The full model is intractable for all but the smallest instances. Accordingly, we decompose the problem into two parts, the system assembly portion and module repair portion. Finally, we tie together the Modular Maintenance and System Assembly Model with key concepts from the Modular Maintenance Scheduling Problem to propose an integrated methodology for design and operation.

High demand of Liquefied Natural Gas (LNG) in recent time requires LNG carriers in more frequent operations in order to meet customers' needs. To ensure that the LNG carriers are always reliable in service, it has become necessary to adopt various advanced modelling techniques such as Genetic Algorithm (GA), fuzzy logic and Evidential Reasoning (ER) for risk/safety assessment and maintenance modelling of LNG carrier operations. These advanced computational techniques can help to overcome challenges posed by uncertainties associated with the LNG carrier operations. Their usefulness is demonstrated using case studies in this research. Firstly, two major hazards of LNG carrier operations such as "failure of LNG containment system" and "LNG spill from transfer arm" are identified and estimated as high risk ones using a risk matrix technique and expert judgement. The causes (failure modeslbasic events) of these high risk hazards are analysed using a Fault Tree Analysis (FTA). The failure logics of their failure modes are established and Boolean algebra is applied to facilitate the evaluation of the failure probabilities and frequencies. Secondly, a GA model is developed to improve the safety levels of the LNG containment system and transfer arm, to minimise their maintenance costs and to realise optimal resource management. The GA is used to optimise a risk model that is developed with exponential distribution and parameters such as failure frequencies, unit costs of maintenance and new maintenance costs of the LNG containment system and transfer arm. Thirdly, the uncertainties of some parameters in the GA model such as unit costs of maintenance are subdued using the strength of Fuzzy Rule Base (FRB) in combination with GA. 125 fuzzy rules of LNG carrier system maintenance cost are developed, which makes it possible to facilitate the evaluation of maintenance cost in any specific LNG risk-based operation. The outcomes of unit costs of maintenance are used in the GA based risk model to update the optimal management of maintenance cost. Finally, the uncertainties of failure modes of the LNG containment system and transfer arm are investigated and treated based on the Formal Safety Assessment (FSA) principle using a Fuzzy ER (FER) approach. The fuzzy logic is used to estimate the safety/risk levels of those failure modes while the ER is used to synthesise them to facilitate the estimation of safety/risk levels of the top events. Risk Control Options (RCOs) are developed to manage high level risks. The costs for each of the RCOs are estimated and synthesised using ER, which facilitated the investigation of the best RCOs in risk-based decision making. There is no doubt that the methodologies proposed possess significant potential for use in improving safety and maintenance of LNG carrier operations based on the verifications of their corresponding test cases. Accordingly, the developed models can be integrated to formulate a platform to facilitate risk assessment and maintenance management of LNG carrier systems in situations where traditional techniques cannot be applied with confidence.

Wave energy has the potential to be a major contributor to the global energy mix. It is estimated that commercial deployment of wave and tidal energy arrays could meet as much as 20% of the UK’s current electricity demand, with an installed capacity of 30-50 GW providing up to 16,000 jobs. However, the wave energy sector has not yet developed into a commercial industry due to several key challenges. One reason private investors have been reluctant to back the sector is that the uncertainty surrounding lifetime costs of wave energy arrays makes it difficult to obtain reliable estimates for overall cost of energy. In order to improve these estimates, a better understanding of the operations and maintenance (O&M) phase of wave energy arrays needs to be gained. This thesis presents an O&M simulation tool designed for wave energy arrays. The work presented uses the model to assess aspects of O&M strategies for two different types of wave energy converter. Uncertainty in the model inputs is also addressed by undertaking a series of sensitivity analyses. The methods and results presented in this thesis highlight the importance of using an O&M simulation tool to plan lifetime logistics for wave energy arrays and obtain realistic cost estimates. The work has also shown how an O&M tool can be used to identify critical components in wave energy converters, thereby helping to design the best device possible for the challenging marine environment. Understanding lifetime costs of wave energy arrays will drive the sector towards commercialisation, bringing wave energy a step closer to fulfilling its potential as a major contributor to global energy production.

Effective and efficient operations management is the cornerstone of any company's success. Presently, because of cost-cutting pressures, all investors look out for companies' operations before making any investment commitment. The South African Government (through the Department of Public Enterprises), as an owner and investor in Eskom, is looking at optimising operational excellence within state-owned enterprises like Eskom. Eskom is presently experiencing problems with increased electricity consumption which it cannot meet due to the limited plant capacity it presently has. These challenges are forcing Eskom to be more efficient and effective in management of the present plant assets (like Matimba Power Station) it presently operates. Matimba Power Station has consistently shown improvement in the areas of plant, financial and operations performance over the last three years. It is presently the standard bearer for the whole Eskom in terms of plant and operational performance. Asset management (through maintenance and management thereof), especially preventative management with in a power utility like Eskom, is a critical factor because supply (generation of electricity) has to meet demand (consumption of electricity) instantaneously as electricity cannot be saved. The planning, scheduling and execution of maintenance (through a work management process) to ensure success of business operations are very critical. An Eskom document titled Routine Work Management Manual emphasised the criticality of preventative management and included a six-step process of work management within the power generation business. The Japanese success in ensuring that operations costs are limited by implementing total productive maintenance (which includes work management) is suggested in the study as a way to go for operational success at Matimba Power Station. Many of the research studies done at Eskom in regard to maintenance were based on and confined to a sampling population of senior staff members like managers, engineers and supervisors. Experience has shown that progress of implementing change (whether in systems or structures) is slow if there was no proactive involvement of all participants and stakeholders, especially employees at lower levels involved in operations. A work management process, which is one of the pillars of total productive maintenance, was recently implemented at Matimba and is currently experiencing teething problems which are being attended to. Employee involvement in making sure of the success of work management is critical. The study investigates the implementation of work management from the employees' perspective in order to address problems for possible full implementation of total productive maintenance.

Worried voices in the Swedish road maintenance and operations industry claim that innovations and technical development has ceased in the last decades. One hypothesis is that it is an effect of the public tendering reform introduced in 1992. Since 2001, the Swedish railroad industry has also introduced public tendering and awarded contracts to private contractors. This study examines the validity of these claims by analyzing the incentives to innovation in the past and at present. The analysis is concluded by proposals how the innovative climate can be improved between the road and rail administrations and their contractors.

Management system for enhancing transfer of knowledge in wind power industry has not received sufficient research attention in recent times. In some cases, the wind power plant owner does not control the management system for operation and maintenance activities. Most of these wind power plants are under contract and rely upon the turbine vendor to perform most of the maintenance works and subsequently share their experience at the initial stage of operation. This research investigates the management system for the operations and maintenance activities of the offshore wind plant in Lillgrund. The research also explores the type of learning method that was adopted by the wind turbine vendor (Siemens) to transfer the operation and maintenance knowledge to the operator and owner (Vattenfall) within the speculated period. It was realized that in the next one year, the Vattenfall would be in full control of the operations and maintenance activities of the offshore wind power plant in Lillgrund. The co-management arrangement will give Siemens a good reputation and gainful experience in the wind power industry. The arrangement is achievable due to Siemens strategy to strive for constructive and long-standing relationships with their customer, based on trust, respect, and honesty. Vattenfall on the other hand, is aiming to be the partner of choice for their suppliers at the same time as best serving their internal customers. The provision for the training during the co-management period enables Siemens to strengthen their relationship with Vattenfall in this industry. In addition, Siemens also maintain close relationship with their customers and develop a large part of their portfolio, frequently on site. Vattenfall improves profitability and value creation, as a fundamental prerequisite for continued growth. The management systems of Vattenfall can be related to professional bureaucracy, this is due to the fact that it was organized to accommodate Siemens experts. Vattenfall benefits from the co-management activities of the operation and maintenance of the Lillgrund wind power plant for a specific period of time. The outcome of the research work has proven that there is an effective time-dependent proportionality for a gradual transfer of the technical knowledge of operation and maintenance from Siemens Wind AB to the Vattenfall personnel. The research started from the perspective of the maintenance method by Swedish standard for wind power, and the way things are being carried out in a more practical way in Lillgrund plant.

Theorists have shown that continuous improvements of the organization are a requirement in today’s global market. The nature of the required improvements can vary from business to business. This study is conducted within a business where the marked demand is higher than the average production rate of the product. Improvements in this case will be referred to in-crease the production rate by assessing the bottleneck section of the plant. In order to increase the production rate, the unplanned stoppages of the bottleneck section needs to be reduced. By reducing the unplanned stoppages of the bottleneck section the profitability of the business will increase, since the bottleneck section is regulating the output of the plant. The angle from which the availability will be increased is through maintenance. A model is developed and implementation example is presented in order to show the increase in the availability of the bottleneck section. The model will provide a systematic approach to define market demand, bottlenecks, severity of assets, criticality of failures and a maintenance decision making pro-cess regarding the profitability of maintenance investments. The result of the model is in-creased availability of the bottleneck section, leading to a higher profit. The study is conduct-ed on Cementa AB, owned by Heidelberg Cement Group.

Tillgängligheten skall ökas genom underhåll av flaskhalsen. En modell är utvecklad och kommer implementeras som ett exempel för att öka tillgängligheten genom underhåll. Modellen kommer ge ett systematiskt arbetssätt för att identifiera efterfrågan, flaskhalsar, allvarlighetsgraden av tillgångar (maskiner/objekt), kritikalitetet av potentiella fel och en grund för en beslutsprocess gällande lönsamheten av underhållsinvesteringar. Resultatet av modellen är ökad tillgänglighet av flaskhalsen som leder till ökad vinst. Studien är gjort på Cementa AB, ägd av Heidelberg Cement Group.

As some of the most technically complex systems in the world, United States fighter aircraft require a complex logistics system to sustain their reliable operation and ensure that the day-to-day Air Force missions can be satisfied. While there has been a lot of attention among academics and practitioners regarding the study of this complex logistics system, most of the focus has been on availability of spare parts that are indeed essential for the smooth operations of the fighter aircraft. However, in recent years there has been an increasing awareness that maintenance resources are an equally important enabler and should be considered together with inventory issues. The maintenance resources required to repair the fighter aircraft are expensive and therefore limited. Moreover, there are various types of maintenance that compete for the same resources. It .is therefore imperative that the allocation of maintenance resources is done as efficiently as possible. In this thesis, we study two areas of fighter aircraft maintenance that could significantly benefit from improved resource allocation and scheduling strategies. We use quantitative and qualitative data from Air Force data-bases and logistics personnel to develop an innovative modeling framework to capture these challenging maintenance problems. This modeling framework is based on a generalization of the of the well-known multi-armed bandit superprocess problem. Using these models, we develop index policies which provide intuitive, easily implemented, and effective rules for scheduling maintenance activities and allocating maintenance resources. These policies seem to improve on existing best practices within the Air Force, and perform well in extensive data-driven simulated computational experiments. The first area is focused on the challenges of scheduling maintenance for the low observable (stealth) capabilities of the F-22 Raptor, specifically, maintenance of the outer coating of the aircraft that is essential to maintain its radar invisibility. In particular, we generate index policies that efficiently schedule which aircraft should enter low observable maintenance, how long they should be worked on, and which aircraft should fly in order to maximize the stealth capability of the fleet. Secondly, we model the maintenance process of the F100-229 engine, which is the primary propulsion method used in the F-16C/D and F-15E aircraft. In particular, we generate index policies to decide which engines should take priority over others, and whether or not certain components of the engines should be repaired or replaced. The policies address both elective (planned) and unplanned maintenance tasks.

Ericsson develops and builds mobile telecommunication networks. These networks consists of a large number of equipment. Each telecommunication company has a staff of administrators appointed to manage respective networks.

In this thesis, we investigate the requirements for an access control model to manage the large number of permissions and equipment in telecommunication networks. Moreover, we show that the existing models do not satisfy the identified requirements. Therefore, we propose a novel RBAC model which is adapted for these conditions.

We also investigate some of the most common used commercial tools for administrating RBAC, and evaluate their effectiveness in coping with our new proposed model. However, we find the existing tools limited, and thereby design and partly implement a RBAC managing system which is better suited to the requirements posed by our new model.

ENGLISH ABSTRACT: Providing basic access to water and sanitation is critical for poverty alleviation and stimulating economic growth, but at the same time it is putting a huge strain on South Africa’s existing economic infrastructure. Local governments allow their existing wastewater infrastructure to deteriorate to the extent that it no longer functions according to its design, and then apply for capital funding for refurbishment. Capital investment alone will not provide a sustainable solution to meet the increased demand on the ageing wastewater infrastructure, since local governments do not have the financial means and technical capacity to adequately maintain and operate their infrastructure. A paradigm shift is required to develop alternative and innovative business models to ensure a sustainable solution providing continued and consistent capacity as a basis for further expansion. Public-private partnerships (PPP) could provide a feasible solution towards building a sustainable technical capacity at local governments. Engaging the private sector must however not be aimed towards access to private sector finance, but rather the improvement in operational efficiency and increased level in service. Obtaining private sector finances will be the wrong focus for engaging in PPP undertakings in South Africa. Private operation must be combined with public financing. The design-build-operate (DBO) model with public financing would be highly viable. Such an arrangement will hold no financial risk to the private sector entity with revenue collection strictly remaining a local government function. The DBO method of delivery (with public funding) will be an effective way to realize cost savings, achieve efficiencies in construction and operation, utilize expertise, and most importantly, for skills transfers and capacity building at local government level. Ultimate sustainability will only be achieved once local government is able to raise the majority of its own funds for operation and maintenance through tariffs and other instruments, including some towards capital redemption. Grant funding must make provision for maintenance and operation of all capital funded projects. All shareholders must join forces in lobbying this concept at the highest political echelons because in terms of Section 154 of the Bill of Rights, national and provincial governments have a duty towards local governments to support and strengthen their capacity, to effectively perform their functions.

Highly deteriorated U.S. road infrastructure, major budgetary restrictions and the significant growth in traffic have led to an emerging need for improving efficiency and effectiveness of highway maintenance practices that preserve the road infrastructure so as to better support society's needs. Effectiveness and efficiency are relative terms in which the performance of a production unit or decision making unit (DMU) is compared with a benchmark (best practice). Constructing the benchmark requires making a choice between an "estimation approach" based on observed best practices (i.e., using data from input and output variables corresponding to observed production units (DMUs) to estimate the benchmark with no elaboration on the details of the production process inside the black box) or an "engineering approach" to find the superior blueprint (i.e., focusing on the transformation process inside the black box for a better understanding of the sources of inefficiencies). This research discusses: (i) the application of the estimation approach (non-parametric approach) for evaluating and comparing the performance of different highway maintenance contracting strategies (performance-based contracting versus traditional contracting) and proposes a five-stage meta-frontier and bootstrapping analytical approach to account for the heterogeneity in the DMUs, the resulting bias in the estimated efficiency scores, and the effect of uncontrollable variables; (ii) the application of the engineering approach by developing a dynamic micro-level simulation model for the highway deterioration and renewal processes and its coupling with calibration and optimization to find optimum maintenance policies that can be used as a benchmark for evaluating performance of road authorities. This research also recognizes and discusses the fact that utilization of the maintenance budget and treatments that are performed in a road section in a specific year directly affect the road condition and required maintenance operations in consecutive years. Given this dynamic nature of highway maintenance operations, any "static" efficiency measurement framework that ignores the inter-temporal effects of inputs and managerial decisions in future streams of outputs (i.e., future road conditions) is likely to be inaccurate. This research discusses the importance of developing a dynamic performance measurement framework that takes into account the time interdependence between the input utilization and output realization of a road authority in consecutive periods. Finally, this research provides an overview of the most relevant studies in the literature with respect to evaluating dynamic performance and proposes a classification taxonomy for dynamic performance measurement frameworks according to five issues. These issues account for major sources of the inter-temporal dependence between input and output levels over different time periods and include the following: (i) material and information delays; (ii) inventories; (iii) capital or generally quasi-fixed factors and the related topic of embodied technological change; (iv) adjustment costs; and (v) incremental improvement and learning models (disembodied technological change). In the long-term, this line of research could contribute to a more efficient use of societal resources, greater level of maintenance services, and a highway and roadway system that is not only safe and reliable, but also efficient.

This thesis validates SIMLOX as a simulation tool for wind energy operation and maintenance (O&M). SIMLOX have been used for modeling O&M in other sectors since 2002 but have recently begun to be used for simulating wind energy. This study validates SIMLOX for the newly introduced sector by creating a model of an existing offshore wind farm. The model used real historical data as input and the output was compared to real system response quantities (SRQs). Example of SRQs which were compared was time based availability, energy based availability, energy production, weather days, and item replacements. The study showed that SIMLOX is very well suited for this type of studies. It also showed that the major difficulty lies in obtaining proper input data. All models simulated the exact right number of major item replacements. The time based availability was considered to be the most important SRQ. The time based availability turned out to be between 0.11 - 0.66 percentage points too high compared to reference data. This most certainly depended on an overestimated work schedule for the technicians. Validation is the process of determining the degree to which a model is an accurate representation of the real world from the perspective of the intended uses of the model. This report presents all necessary knowledge to validate SIMLOX as a tool for wind energy simulations but it is thus up to the user to decide whether or not the output is accurate enough for their intended use.

The move towards integrating products and services has increased significantly. As a result, some business models, such as Product Service Systems (PSS) have been developed. PSS emphasises the sale of use of the product rather than the sale of the product itself. In this case, product ownership lies with the manufacturers/suppliers. Customers will be provided with a capable and available product for their use. In PSS, manufacturers/suppliers are penalised for any down time of their product according to the PSS contract. This has formed a pressure on the service providers (maintenance teams) to assure the availability of their products in use. This pressure increases as the products are scattered in remote places (customer locations). Authors have urged that different product monitoring levels are applied to enable service providers to monitor their products remotely allowing maintenance to be performed accordingly. They claim that by adopting these monitoring levels, the product performance will increase. Their claim is based on reasoning, not on experimental/empirical methods. Therefore, further experimental research is required to observe the effect of such monitoring levels on complex maintenance operations systems as a whole which includes e.g. product location, different types of failure, labour and their skills and locations, travel times, spare part inventory, etc. In the literature, monitoring levels have been classified as Reactive, Diagnostics, and Prognostics. This research aims to better understand and evaluate the complex maintenance operations of a product in use with different levels of product monitoring strategies using a Discrete Event Simulation (DES) approach. A discussion of the suitability of DES over other techniques has been provided. DES has proven its suitability to give a better understanding of the product monitoring levels on the wider maintenance system. The requirements for simulating a complex maintenance operation have been identified and documented. Two approaches are applied to gather these generic requirements. The first is to identify those requirements of modelling complex maintenance operations in a literature review. This is followed by conducting interviews with academics and industrial practitioners to find out more requirements that were not captured in the literature. As a result, a generic conceptual model is assimilated. A simulation module is built through the Witness software package to represent different product monitoring levels (Reactive, Diagnostics, and Prognostics). These modules are then linked with resources (e.g. labour, tools, and spare parts). To ensure the ease of use and rapid build of such a complex maintenance system through these modules, an Excel interface is developed and named as Product Monitoring Levels Simulation (PMLS). The developed PMLS tool needed to be demonstrated and tested for tool validation purposes. Three industrial case studies are presented and different experimentations are carried out to better understand the effect of different product monitoring levels on the complex maintenance operations. Face to face validation with case companies is conducted followed by an expert validation workshop. This work presents a novel Discrete Event Simulation (DES) approach which is developed to support maintenance operations decision makers in selecting the appropriate product monitoring level for their particular operation. This unique approach provides numerical evidence and proved that the higher product monitoring level does not always guarantee higher product availability.

Quality insurance is an important part of a manufacturing company in order to achieve and maintain its competitiveness. To measure how the company’s processes are performing, it is necessary to have reliable measurement data. Reliability of measurement data is naturally a crucial and important factor for a company to deliver the right quality to its customers. Without confidence in the measurement data, there is a potential risk that the customers end up with defective products if the measurement results are misinterpreted. This thesis project has been conducted at Volvo GTO in Skövde at the crankshaft manufacturing department. Volvo GTO wants to understand how well their manufacturing process of crankshaft performs by assigning a capability value. One of the requirements for obtaining a reliable capability value is that the measurement system is reliable. Volvo suspects that they do not have a reliable measuring system for measuring the roundness of bearing surfaces on a crankshaft. To clearly identify the problems of the measurement system, several possible causes of errors were identified and then analyzed deeper. These possible causes have attempted to be disproved and the errors which couldn’t be disproven may be a possible cause of error. The result of this thesis project shows that the company doesn’t meet the requirements to perform reliable measurements with such small tolerances. The main cause of the uncertainty in the measurement process is the external factors, such as the measuring environment. This report also includes recommendations on how the company can eliminate the measurement uncertainties to get a more reliable measurement process.
För att ett tillverkande företag ska kunna uppnå och bibehålla sin konkurrenskraft är kvalitetssäkring en viktig del av verksamheten. För att kunna uppmäta hur företagets processer presterar är det nödvändigt att företaget har tillförlitlig mätdata. Tillförlitlighet till mätdata är av naturliga skäl en avgörande och viktig faktor för att ett företag ska leverera rätt kvalitet till sina kunder. Utan tillit till mätdatan finns det en potentiell risk att defekta produkter hamnar hos kunden genom att tolka mätresultatet felaktigt.   Detta examensarbete har utförts på Volvo Group Trucks Operations (GTO) i Skövde på vevaxels-avdelningen. Volvo GTO vill förstå hur väl deras tillverkningsprocess av vevaxlar presterar genom ett duglighetstal. Ett av kraven för att få ett pålitligt duglighetstal är att mätsystemet är tillförlitligt. Volvo anser idag att de inte har ett tillförlitligt mätsystem vid rundhetsmätning av vevaxlar. För att tydligare kartlägga de problem som idag finns med mätsystemet så identifierades ett antal felmöjligheter som analyserades djupare. Dessa felmöjligheter har försökts att motbevisas och de som inte har kunnat motbevisas kan vara en möjlig felorsak. Resultatet uppvisar att det finns brister i de grundförutsättningar som krävs för att en tillförlitlig mätning skall kunna utföras. I arbetets gång är det tydligt att det främst är de yttre faktorerna som skapar en osäkerhet i mätprocessen. Rapporten avslutas med en rekommendation kring hur företaget kan eliminera mätosäkerheten för att få en tillförlitligare mätprocess.

Maintenance costs at Omnicane Thermal Energy Operations (St Aubin) Ltd contribute a significant part ofthe unit cost of electrical energy produced and affect the profitability of the power plant. Hence it is necessaryand crucial to minimize maintenance costs by optimizing maintenance processes to make the plant morereliable and to run economically.The total maintenance cost for OTEOSAL from year 2008 to 2011 is seen to be increasing and has evendouble from 2008 to 2011. The cost of external labor during operation has increased by nearly four times dueto a lot of breakdown on different equipments and also the value of the spare parts store is seen to risebecause many spare parts are bought at random in fear of having a shut down due to unavailability of spareparts. These excess expenses contribute to a loss in profitability. With a good maintenance strategy, the totalmaintenance cost can be reduced by about 30%.Fault Tree Analysis (FTA) and Failure Mode Effect Analysis (FMEA) were done and allowed identifyingcritical equipments at the power plant and the Grate Stocker, one of the most important and criticalequipment for the plant was selected to perform a Quantitative Analysis of the FTAs. The probability offailure for the Grate Stocker is seen to be 0.98 and has reliability as low as 0.02. Quantitative Analysis of FTAand Pareto Analysis will allow having the right quantity of spare parts at the right time without overstocking.From this thesis, it can be said that combining different maintenance and management methods andstrategies based on FTA, FMEA and Pareto Analysis and all these well formalized and documented accordingto ISO 9001 will certainly allow the power plant to gain a lot like availability, reliability and even financiallyfrom maintenance and also will make OTEOSAL ready for new challenges appearing in the energy sector inMauritius.

During development of a complex system, feasibility initially overshadows other concerns, in some cases leading to a design which may not be viable long-term. In particular for the case of Reusable Launch Vehicles, Operations&Maintenance comprises the majority of the vehicle's LCC, whose stochastic nature precludes direct analysis. Through the use of simulation, probabilistic methods can however provide estimates on the economic behavior of such a system as it evolves over time. Here the problem of operations optimization is examined through the use of discrete event simulation. The resulting tool built from the lessons learned in the literature review simulates a RLV or fleet of vehicles undergoing maintenance and the maintenance sites it/they visit as the campaign evolves over a period of time. The goal of this work is to develop a method for uncovering an optimal operations scheme by investigating the effect of maintenance technician skillset distributions on important metrics such as the achievable annual flight rate and maintenance man hours spent on each vehicle per flight. Using these metrics, the availability of technicians for each subsystem is optimized to levels which produce the greatest revenue from flights and minimum expenditure from maintenance.

The United States Navy has a number of entities that work together to ensure aircraft in the Navy are supplied with the parts and materials required to maintain mission readiness. An analysis of the operating and support system costs will characterize cost variance across Organizational, Intermediate and Depot level maintenance. We will examine both labor and material cost for both reparable and consumable items, and categorize those costs by type of maintenance action. This analysis is intended to help in the development of a cost model which could aid in both budget planning and execution.

Manufacturing companies seek ways to eliminate waste from their operations to stay competitive. In this project, the waste is in the mold repair process which involves two main groups, Molding and Tooling. By using process flow mapping and analysis, the waste was identified in the information management aspects of the repair operations. The repair process was re-designed to incorporate existing but unused information systems, eliminating outdated methods that were inefficient. The process re-designs also incorporate several changes to have better accountability at various decision making stages in the mold repair work. Some of the changes have been implemented in a pilot project and have proven to be capable of improving the current methods. Other recommendations require a longer term approach to the problem through training and development of personnel.

This thesis explores different paradigms for incorporating uncertainty with optimization frameworks for applications in chemical engineering and site-wide operations. First, we address the simulation optimization problem, which deals with the search for optimal input parameters to black-box stochastic simulations which are potentially expensive to evaluate. We include a comprehensive literature survey of the state-of-the-art in the area, propose a new provably convergent trust region-based algorithm, and discuss implementation details along with extensive computational experience, including examples for chemical engineering applications. Next, we look at the problem of long-term site-wide maintenance turnaround planning. Turnarounds involve the disruption of production for significant periods of time, and may incur enormous costs in terms of maintenance manpower as well as lost sales. The problem involves (1) the simulation of profit deterioration due to wear and tear followed by the determination of how frequently a particular turnaround should take place; and (2) the consideration of site network structure and turnaround frequencies to determine how turnarounds of different plants may be coordinated over a long-term horizon. We investigate two mixed-integer models, the first of which determines optimal frequencies of individual plant turnarounds, while the second considers maximizing long-term profit through coordination of turnarounds across the site. We then turn to more conventional methods of dealing with optimization under uncertainty, and make use of a combined robust optimization and stochastic programming approach to medium-term maintenance planning in integrated chemical sites. The nature of the uncertainty considered affects two aspects of maintenance planning, one of which is most suitably addressed through a robust optimization framework, while the other is better handled with stochastic programming models. In summary, we highlight the importance of considering uncertainty in optimization as well as the choice of approach or paradigm used through chemical engineering applications that span varied domains and time scales.

This thesis addresses the allocation of gas turbine aircraft engines to maintenance facilities. Scheduling a global engine maintenance network can be very complex and challenging. This project pertains particularly to the V2500 IAE engine maintenance network managed by Pratt & Whitney. Using a mathematical program to automate engine allocation was believed to reduce the workload on the organization and the cost of maintaining the 3100 engine fleet. An introduction to the engine maintenance network will be covered along with an explanation of Fleet Hour Agreements (FHA). A literature review of mathematical programming is included to provide background of pertinent information. The current state of the business is analyzed. An integer linear program is developed to closely represent the current state of the business. Historical data was used to feed the model, and the outputs from the model were compared to actuals. A sensitivity analysis is performed to better understand the constraints of the current business and the feasibility of the model. An optimization model should not be used to plan engine maintenance given the current state of business. The business is too dynamic and the network is highly constrained by capacity. The results also show a much smaller savings than were originally expected. This is mostly due to better understanding the cost of maintaining the engines at the different shops. The variation was much lower than originally expected. The current state is operating close to optimal with great flexibility and should continue on as is.
