Obesity is the most common nutritional disorder affecting children in the developed world. The marked increase in prevalence is linked to environmental and behavioural changes; children’s energy expenditure has undoubtedly decreased. Obese children often become obese adults; children from lower socioeconomic groups are more likely to be obese. Obesity is a risk factor for ischaemic heart disease, hypertension, stroke, type 2 diabetes, depression, and certain cancers. There is no proven preventive strategy but limiting time in front of computer and TV screens appears important. Obese children are relatively tall; the combination of obesity with short stature suggests underlying disease (e.g. hypothyroidism; Cushing’s syndrome should be considered). Treatment should be considered if body mass index is greater than the 98th centile and the family are willing to make the necessary lifestyle changes. Drug and surgical treatments have a role in a small number of children, but should always be used on a background of a behavioural weight management programme.

While 40–70% of the variation in fat mass between individuals may be genetically determined, environmental factors remain crucial. Cross-sectional and longitudinal studies indicate a positive association between obesity and depression. While some evidence suggests that depression is a risk factor for obesity, other findings indicate that obesity is a risk factor for depression. Therefore the directionality of this relationship remains unclear. Alternatively, there may be common mediating biological or environmental contributors accounting for this association. Potential biological mediators include dysregulation of the HPA axis, leptin resistance, and inflammatory immune responses. Environmental and psychological mediators may include a history of abuse and binge eating. It is also possible that the association between obesity and depression is most pronounced among particular subsets of individuals (e.g., women, those with more severe obesity). A better understanding of this depression-obesity association is needed to guide treatment recommendations for obese clients with comorbid depression. Future research is also needed to determine who is most vulnerable to experiencing comorbid depression and obesity.

Childhood obesity has become a major public health concern. Obesity affects perioperative anesthetic management from airway management and drug selection to postoperative monitoring requirements. This chapter explains the perioperative implications of childhood obesity and addresses multiple considerations in the formulation of an anesthetic plan for the obese child. The chapter examines the anesthetic implications of obesity and its effects on various body systems, discusses the principles of perioperative management of obese children, and grants a working knowledge of pharmacokinetics of common anesthetic drugs in obese children and dosage implications. A case study of an obese 13-year-old boy with late-onset Blount disease who presents for proximal tibial osteotomy with internal fixation is presented to illustrate the concepts in the chapter.
