Boundary value problems in PDEs usually require determination of the eigenvalues and Fourier coefficients for a series, the latter of which are often intractable. A method was found that simplified both analytic and numeric solutions for Fourier coefficients based on the slope of the eigenvalue function at each eigenvalue (eigenslope). Analytic solutions by the eigenslope method resulted in the same solutions, albeit in different form, as other methods. Numerical solutions obtained by calculating the slope of the eigenvalue function at each root (hand graphing, Euler's, Runge-Kutta, and others) also matched. The method applied to all classes of separable PDEs (parabolic, hyperbolic, and elliptical), orthogonal (Sturm-Liouville) or non orthogonal expansions, and to complex eigenvalues. As an example, the widespread assumption of uniform capacitance was tested. An analytic model of cylindrical brain cell structures with an exponential distribution of membrane capacitance was developed with the eigenslope method. The stimulus-response properties of the models were compared under different configurations and shown to fit to experimental data from dendritic neurons. The long-standing question was addressed of whether the amount of variation of membrane capacitance measured in experimental studies is sufficient to markedly alter the vital neuron characteristic of passive signal propagation. We concluded that the degree of membrane capacitance variation measured in cells does not alter electrical responses at levels that are physiologically significant. The widespread assumption of uniform membrane capacitance is likely to be a valid approximation.

Medication related errors are a significant cause of morbidity and mortality. In Brazil, most errors are related to prescribing, preparing, and administering medications. One way to deal with this barrier to safe care is through assessment and education of medication calculation dosage skills. Considering the Brazilian reality, this dissertation is a context and language adaptation of an evidence-based intervention called safeMedicate, a program that reinforces learning synthesis in crucial elements of medication dosage problem solving and provides the foundation for development in remaining levels of the hierarchy of learning. A guideline for medication calculation skills development or improvement based on the seven research-based principles for smart teaching was developed. Teaching approaches are beneficial for multiple methods of learning by addressing cognitive, motivational, and developmental goals. Web-based software would be a strong ally on adopting those approaches by complementing the class practice and providing opportunities for practice learning. The two-phases of adaptation and preliminary evaluation of safeMedicate for use in Brazil were guided by the Participatory and Iterative Process Framework for Language Adaptation (PIPFLA) cross-cultural equivalence model. A triangulation method of face validity survey, journaling, and multiple focus groups was used. The focus groups were (1) language adaptation team, (2) panel of experts, and (3) student panel. In order to analyze focus group data, a systematic coding procedure was performed through an iterative process, solving any differences between coders in order to guarantee internal consistency. The main themes were language, visual, content, programing, and data while discussing necessary adaptations of safeMedicate for use in Brazil.

Presently the Douglas College General Nursing Program screens its applicants with the Nelson-Denny Reading Test and the Stanford Diagnostic Math Test in the belief that those tests set appropiate standards for admission to the program. No research had been performed to validate this belief. This descriptive, exploratory study examined the available data to determine the influence of the screening tests on academic achievement. Since the literature documents the influence of demographic variables on academic achievement in nursing programs, the students' ages were also included in this study. The students' scores on the screening tests, and the students' ages, were correlated with their scores on six nursing academic scores in the first two semesters in the program. Canonical correlation analysis was performed on the same data to see if insight could be obtained about the interrelationships of the screening tests, the students ages, and academic achievement. The Pearson r correlations indicate that the influence of the screening tests was small to moderate on academic achievement in the individual courses, and that students' ages demonstrated the weakest correlation. Sub-test 3 of the Stanford Diagnostic Math test consistently correlated the highest with the academic course scores with only two exceptions, and in those cases it was the second highest correlate. The canonical correlation analysis demonstrates that there are two underlying linkages or dimensions between the two data sets. The first dimension demonstrates the dominance of the Stanford Diagnostic Math sub-tests in correlating with academic achievement. The second dimension is too weak to make assumptions about the relationship between the two data sets, but it does suggest that the students' ages influence academic achievement.

Two single-pass methods for fitting multiexponentials to experimental data are described. These methods rely on the construction of a matrix whose characteristic polynomial is used to determine the rates of decay. In the first method, which we call the multiple-delay method, the matrix is constructed using time delays of the experimental data. This method is fast and highly accurate even if the experimental signal contains exponential components with similar rates of decay. In the second method, which we call the successive-integral method, the matrix is constructed using integrals of the experimental data. This procedure yields good results for noisy signals and is a generalization of the method of Martin et al. ((1993) J. Neurosci. Methods, 51: 135-146). In addition, a particular instability of the multiexponential curve fitting problem is identified and a method for overcoming this instability is given.

Book Summary: Computational models of neural networks have proven insufficient to accurately model brain function, mainly as a result of simplifications that ignore the physical reality of neuronal structure in favor of mathematically tractable algorithms and rules. The standard for evaluating a student's mathematic ability (grade level) for admission to many vocational-technical programs is through the administration of the Tests of Adult Basic Education (TABE). There has come forth a concern from vocational educators, that students entering programs may not be prepared for the mathematics required by the curriculum, even though the student has met the criteria for entry as established by the state curriculum frameworks as evidenced by their scores on a TABE which had recently been administered. Furthermore, questions raised among instructional, administrative and guidance personnel about the congruence of math skills required on the TABE vs. those used by practical nurses on the job supported the need for a study to determine the congruence of these sets of mathematics skills. Using the OMRA inventory developed by David Pucel, the mathematic operations required for job related math applications are indicated by samples collected from active nursing practitioners. Three analysis teams consisting of practical nurses and math experts were established and determined the math operations required for solving the job related math samples collected. The math skills tested by the TABE were then compared to the job related math samples. With the math operations of the variables ranked, the Spearman Rank Correlation was used to evaluate the correlation across the TABE and the mathematic job requirements of practical nursing. Based on 19 math operations identified from the Practical Nursing job math requirements, the results showed that there was little correlation among these two variables (r=. 4974).

Boundary conditions for the cable equation - such as voltage-clamped or sealed cable ends, branchpoints, somatic shunts, and current clamps - result in multi-exponential series representations of the voltage or current. Each term in the series expansion is characterized by a decay rate (eigenvalue) and an initial amplitude (Fourier coefficient). The eigenvalues are determined numerically and the Fourier coefficients are subsequently given by the residues at the eigenvalues of the Laplace transform of the solution. In this paper, we introduce an alternative method for estimating the Fourier coefficients which works for all types of boundary conditions and is practical even when analytic expressions for the Fourier coefficients become intractable. It is shown that terms in the analytic expressions for the Fourier coefficients result from derivatives of the equation for the eigenvalues, and that simple numerical estimates for the amplitude coefficients are easily derived by replacing analytical derivatives by numerical eigenslope. The physical quantity represented by the slope is identified as effective neuron capacitance.

The implantable cardioverter defibrillator (lCD) is the most effective treatment available for terminating potentially life-threatening ventricular fibrillation and ventricular tachycardia. The lCD detects and attempts to correct these arrhythmias by pacing, cardioversion, and defibrillation thereby providing lifesaving therapy to patients at risk for sudden cardiac death. Currently, 150,000 Americans receive ICDs each year. Although most lCD recipients are men, more women are now qualifying for insertion (Stutts, Cross, Conti, & Sears, 2007). Despite its established health benefits, lCD implantation is accompanied by psychological factors which merit research attention. This study investigated the experiences of women who live an lCD. The homogenous, purposeful sample consisted of 15 women who had an lCD that was implanted within the last three years and were receiving follow-up treatment at the same north Florida clinic. Data collection was accomplished through a semi-structured interview specific to the areas of pre-implantation, immediate post-implantation, and discharge home. Results were transcribed verbatim and then analyzed. Five core themes emerged from the transcripts along with multiple subcategories. The main themes included: Psychological Reactions, Physical Comfort, Procedural Issues, Body Image, and Feelings Regarding a Shock. Information obtained from this research is beneficial to nurses providing care to women with ICDs and to primary care advanced nurse practitioners in order to improve the overall health outcome and ongoing care of these women.

Hospital Acquired Infections (HAI) are a global patient safety challenge, costly to treat, and affect hundreds of millions of patients annually worldwide. It has been shown that the majority of HAI are transferred to patients by caregivers' hands and therefore, can be prevented by proper hand hygiene (HH). However, many factors including cognitive load, cause caregivers to forget to cleanse their hands. Hand hygiene compliance among caregivers remains low around the world. In this thesis I showed that it is possible to build a wearable accelerometer-based HH reminder system to identify ongoing nursing activities with the patient, indicate the high-risk activities, and prompt the caregivers to clean their hands. Eight subjects participated in this study, each wearing five wireless accelerometer sensors on the wrist, upper arms and the back. A pattern recognition approach was used to classify six nursing activities offline. Time-domain features that included mean, standard deviation, energy, and correlation among accelerometer axes were found to be suitable features. On average, 1-Nearest Neighbour classifier was able to classify the activities with 84% accuracy. A novel algorithm was developed to adaptively segment the accelerometer signals to identify the start and stop time of each nursing activity. The overall accuracy of the algorithm for a total of 96 events performed by 8 subjects was approximately 87%. The accuracy was higher than 91% for 5 out of 8 subjects. The sequence of nursing activities was modelled by an 18-state Markov Chain. The model was evaluated by recently published data. The simulation results showed that the high-risk of cross-contamination decreases exponentially by frequency of HH and this happens more rapidly up to 50%-60% hand hygiene rate. It was also found that if the caregiver enters the room with high-risk of transferring infection to the current patient, given the assumptions in this study, only 55% HH is capable of reducing the risk of infection transfer to the lowest level. This may help to prevent the next patient from acquiring infection, preventing an infection outbreak. The model is also capable of simulating the effects of the imperfect HH on the risk of cross-contamination.
