Resumen

Background: Since 2019, the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is causing a rapidly spreading pandemic. The present study aims to compare a modified quick SOFA (MqSOFA) score with the NEWS-2 score to predict in-hospital mortality (IHM), 30-days mortality and recovery setting. Methods: All patients admitted from March to October 2020 to the Emergency Department of St. Anna Hospital, Ferrara, Italy with clinically suspected SARS-CoV-2 infection were retrospectively included in this single-centre study and evaluated with the MqSOFA and NEWS-2 scores. Statistical and logistic regression analyses were applied to our database. Results: A total of 3359 individual records were retrieved. Among them, 2716 patients were excluded because of a negative nasopharyngeal swab and 206 for lacking data; thus, 437 patients were eligible. The data showed that the MqSOFA and NEWS-2 scores equally predicted IHM (p < 0.001) and 30-days mortality (p < 0.001). Higher incidences of coronary artery disease, congestive heart failure, cerebrovascular accidents, dementia, chronic kidney disease and cancer were found in the deceased vs. survived group. Conclusions: In this study we confirmed that the MqSOFA score was non-inferior to the NEWS-2 score in predicting IHM and 30-days mortality. Furthermore, the MqSOFA score was easier to use than NEWS-2 and is more suitable for emergency settings. Neither the NEWS-2 nor the MqSOFA scores were able to predict the recovery setting.

Resumen

The symbiotic organism search (SOS) algorithm is a promising meta-heuristic evolutionary algorithm. Its excellent quality of global optimization solution has aroused the interest of many researchers. In this work, we not only applied the strategy of multi-group communication and quantum behavior to the SOS algorithm, but also formed a novel global optimization algorithm called the MQSOS algorithm. It has speed and convergence ability and plays a good role in solving practical problems with multiple arguments. We also compared MQSOS with other intelligent algorithms under the CEC2013 large-scale optimization test suite, such as particle swarm optimization (PSO), parallel PSO (PPSO), adaptive PSO (APSO), QUasi-Affine TRansformation Evolutionary (QUATRE), and oppositional SOS (OSOS). The experimental results show that MQSOS algorithm had better performance than the other intelligent algorithms. In addition, we combined and optimized the DV-hop algorithm for node localization in wireless sensor networks, and also improved the DV-hop localization algorithm to achieve higher localization accuracy than some existing algorithms.

Resumen

Abstract Assessing tactile function among children with intellectual, motor, and communication impairments remains a clinical challenge. A case control design was used to test whether children with global developmental delays (GDD; n = 20) would be more/less reactive to a modified quantitative sensory test (mQST) compared to controls (n = 20). Reactivity was indexed by blinded behavioral coding across vocal, facial, and gross motor responses during the mQST. On average the children with GDD were significantly more reactive than controls to most tactile sensory modalities including light touch (p = .034), pin prick (p = .008), cool (p = .039), pressure (p = .037), and repeated von Frey (p = .003). The results suggest the mQST approach was feasible and highlights the GDD sample was more reactive than controls to a range of stimuli.

Abstract Objectives Intrathecal baclofen (ITB) pumps used to manage spasticity in children with cerebral palsy (CP) also improve pain outcomes for some but not all patients. The purpose of this clinical feasibility study was to explore whether a quantitative sensory testing approach could a) be modified and used to subgroup individuals into sensory profiles and b) test whether the profiles were related to postimplant pain outcomes (i.e., pain responsive or pain persistent). Subjects A purposeful clinical sample of nine children with CP (mean age = 12.5 years, male = 56%) and complex communication needs participated. Methods A prospective within-subject design was used to measure proxy-reported pain before and after ITB implant. Pain response status was determined by proxy-reported pain intensity change (&gt;50% change in maximum rated intensity). A modified quantitative sensory testing (mQST) procedure was used to assess behavioral responsivity to an array of calibrated sensory (tactile/acute nociceptive) stimuli before surgery. Results Seven individuals with presurgical pain had mQST differentiated sensory profiles in relation to ITB pain outcomes and relative to the two individuals with no pain. Presurgically, the ITB pain responsive subgroup (N = 3, maximum rated pain intensity decreased &gt;50% after ITB implant) showed increased behavioral reactivity to an acute nociceptive stimulus and cold stimulus, whereas the ITB pain persistent subgroup (N = 4) showed reduced behavioral reactivity to cold and repeated von Frey stimulation relative to the no pain individuals. Conclusion Implications for patient selection criteria and stratification to presurgically identify individuals with CP “at risk” for persistent postprocedure pain are discussed.

Abstract Motivation ASTRAL is the current leading method for species tree estimation from phylogenomic datasets (i.e. hundreds to thousands of genes) that addresses gene tree discord resulting from incomplete lineage sorting (ILS). ASTRAL is statistically consistent under the multi-locus coalescent model (MSC), runs in polynomial time, and is able to run on large datasets. Key to ASTRAL’s algorithm is the use of dynamic programming to find an optimal solution to the MQSST (maximum quartet support supertree) within a constraint space that it computes from the input. Yet, ASTRAL can fail to complete within reasonable timeframes on large datasets with many genes and species, because in these cases the constraint space it computes is too large. Results Here, we introduce FASTRAL, a phylogenomic estimation method. FASTRAL is based on ASTRAL, but uses a different technique for constructing the constraint space. The technique we use to define the constraint space maintains statistical consistency and is polynomial time; thus we prove that FASTRAL is a polynomial time algorithm that is statistically consistent under the MSC. Our performance study on both biological and simulated datasets demonstrates that FASTRAL matches or improves on ASTRAL with respect to species tree topology accuracy (and under high ILS conditions it is statistically significantly more accurate), while being dramatically faster—especially on datasets with large numbers of genes and high ILS—due to using a significantly smaller constraint space. In 2017, NIST shook the cryptographic world by starting a process for standardizing post-quantum cryptography. Sixty-four submissions have been considered for the first round of the on-going NIST Post-Quantum Cryptography (PQC) process. Multivariate cryptography is a classical post-quantum candidate that turns to be the most represented in the signature category. At this stage of the process, it is of primary importance to investigate efficient implementations of the candidates. This article presents MQsoft, an efficient library which permits to implement HFE-based multivariate schemes submitted to the NIST PQC process such as GeMSS, Gui and DualModeMS. The library is implemented in C targeting Intel 64-bit processors and using avx2 set instructions. We present performance results for our library and its application to GeMSS, Gui and DualModeMS. In particular, we optimize several crucial parts for these schemes. These include root finding for HFE polynomials and evaluation of multivariate quadratic systems in F2. We propose a new method which accelerates root finding for specific HFE polynomials by a factor of two. For GeMSS and Gui, we obtain a speed-up of a factor between 2 and 19 for the keypair generation, between 1.2 and 2.5 for the signature generation, and between 1.6 and 2 for the verifying process. We have also improved the arithmetic in F2n by a factor of 4 compared to the NTL library. Moreover, a large part of our implementation is protected against timing attacks.

Abstract Purpose Sepsis is a life-threating organ dysfunction caused by a dysregulated host response to infection. Being a time-dependent condition, the present study aims to compare a recently established score, i.e., modified quick SOFA (MqSOFA), with other existing tools commonly applied to predict in-hospital mortality. Methods All cases of sepsis and septic shock consecutively observed at St. Anna University Hospital of Ferrara, Italy, from January 2017 to December 2018 were included in this study. Each patient was evaluated with MqSOFA, lactate assay, NEWS and qSOFA. Accurate statistical and logistic regression analyses were applied to our database. Results A total of 1001 consecutive patients with sepsis/septic shock were retrieved. Among them, 444 were excluded for incomplete details about vital parameters; thus, 556 patients were eligible for the study. Data analysis showed that MqSOFA, NEWS and lactate assay provided a better predictive ability than qSOFA in terms of in-hospital mortality (p < 0.001). Aetiology-based stratification in 5 subgroups demonstrated the superiority of NEWS vs. other tools in predicting fatal outcomes (p = 0.030 respiratory, p = 0.036 urinary, p = 0.044 abdominal, p = 0.047 miscellaneous and p = 0.041 for indeterminate causes). After Bonferroni’s correction, MqSOFA was superior to qSOFA over respiratory (p < 0.001) and urinary (p < 0.001) aetiologies. Age was an independent factor for negative outcomes (p < 0.001). Conclusions MqSOFA, NEWS and lactate assay better predicted in-hospital mortality compared to qSOFA. Since sepsis needs a time-dependent assessment, an easier and non-invasive score, i.e., MqSOFA, could be used to establish patients’ outcome in the emergency setting.

Dans cette thèse, nous étudions la conception de cryptosystèmes multivariés qui sont résistants contre les ordinateurs classiques et quantiques. En particulier, nous proposons deux schémas de signature digitale que j'ai soumis au processus de standardisation de cryptographie post-quantique du NIST : GeMSS et DualModeMS. Ces schémas sont basés sur la famille HFE. Nous proposons des paramètres de sécurité basés sur un état de l'art de vingt ans de cryptanalyse intensive. Puis, nous sélectionnons des paramètres qui maximisent l'efficacité. Nous la mesurons avec une nouvelle bibliothèque : MQsoft. MQsoft est une bibliothèque efficace en C qui supporte un large ensemble de paramètres pour les schémas basés sur HFE. Sa performance surpasse toutes les bibliothèques existantes. Nous expliquons dans cette thèse comment nous obtenons une telle performance. Tandis que GeMSS est un schéma qui a une grande clé publique, mais une signature très courte, DualModeMS est basé sur une transformation qui inverse ce comportement. Cependant, sa sécurité est basée sur l'hypothèse que le problème AMQ est difficile. Nous démontrons que cette hypothèse est vérifiée, et nous confirmons nos résultats avec des expériences utilisant les bases de Gröbner. Finalement, nous proposons la première implémentation de DualModeMS. Nous étudions comment l'implémenter efficacement, et comment optimiser le choix des paramètres de sécurité. Nous étendons aussi DualModeMS à l'utilisation du cryptosystème Rainbow à la place de HFE. Ceci permet d'obtenir des tailles de clés et signature intéressantes<br>In this thesis, we study the design of multivariate cryptosystems, which are resistant against classical and quantum computers. In particular, we study two digital signature schemes that I submitted to the NIST Post-Quantum Cryptography standardization process: GeMSS and DualModeMS. These schemes are based on the HFE family. We propose security parameters based on a state-of-the-art of twenty years of intensive cryptanalysis. Then, we select secure parameters which maximize the practical efficiency. We measure this with a new library: MQsoft. MQsoft is a fast library in C which supports a large set of parameters for HFE-based schemes. Its performance outperforms all existing libraries. We explain in this thesis how we obtain this result. Whereas GeMSS is a scheme which has a large public-key but a very short signature, DualModeMS is based on a transformation inverting this trade-off. However, its security is based on the assumption that the AMQ problem is hard. We demonstrate that this assumption is verified, and we confirm our results with experiences using Gröbner basis. Finally, we propose the first implementation of DualModeMS. We study how to implement it efficiently, as well as how to optimize the choice of security parameters. We also extend DualModeMS to the Rainbow cryptosystem instead of HFE. This allows to have interesting key sizes and signature sizes

Resumen

Abstract This chapter is concerned with freedom of access to information on the environment. The United Kingdom still does not have a freedom of information statute for central government although one now exists for local government. The recent EC Directive on Freedom of Access to Information on the Environment as well as the mqst recent United Kingdom legislation referred to below has started an irresistible move to confirm the right of the citizen, pressure group, company, or state agency to know much more about the environment. The concept of a ‘right’ to environmental information is crucial because it improves environmental monitoring systems, increases the quality of environmental information which is gathered, and makes much of that information available to the public. These effects will, in turn, increase public participation in the procedures to control pollution, including the enforcement of environmental laws. What follows is a modest attempt to set out some of the developments in the last few years in this crucial area of environmental law.
