Texto completoRelying mainly on aid and advice, it may happen that, confronted with problems of subjectivation limits, the work becomes inoperative in the Educational Action in Open Medium. As a clinical psychologist in such a service, we had to overcome this hurdle, imagine other techniques and develop our role in this context. We have designed and gradually implemented a diagnostic tool, a therapeutic device as well as an autonomous theoretical concept: the story of a pregnancy. It is a work of associative history around perinatality that we can define also, at the suggestion of Rene Roussillon, as a focal association device. In order to give a place to the psychic reality, generally ignored in Educationnal Action, we called around this original thread (although in the parentage of the family story of Freud) to a narrative work of the conception times, of pregnancy, birth and breastfeeding as well as the couple meeting and the desire for a child, of women-mothers yet stuck in incestual problematic (Racamier) and narcissistic identity suffering (Roussillon). We illustrate in this work, through numerous cases and six clinical studies, how this new modality of approach to these women has allowed us to overcome a form of initial psychological deafness (and perhaps bilateral…) and served to relaunch stimulating work that has motivated this writing, and encourages us to think that the story of a pregnancy could be generalised to other settings and considered as a valuable tool for the clinicians

Texto completoDuring the last few decades of the 20th century, state policies aimed at protecting children underwent profound changes across the Western world. These changes can be attributed, first and foremost, to the “discovery” of child abuse in the 1960s in the United States, its construction as a social problem and repercussions across the globe in the following decades. On the other hand, the ratification of the U.N. Convention on the Rights of the (1989) by most countries in the world contributed to this process of change.This thesis explores how these global transformations were translated in Argentina into a national policy for the protection of endangered children. Based on an ethnographic study on child protection services in poor neighborhoods in the city of Buenos Aires, the day-to-day management of child abuse is examined. An analysis of the local discourses and practices of state agents provides insight into the methods and implications of the political and moral treatment of endangered children in the world today

Texto completo« A child is being beaten » ; « a child is being killed » : for most of our contemporaries, this is an epitome of horror — a transgression of what is still considered as sacred in our socalled modern culture.For some others, it is a clinical reality : the one we observe through what is usually called « child abuse » and infanticides.And for everybody (for every subject), it is also the presentification of a phantasy — the most basic of all, as it concerns on one hand the relationship with the father and the primordial masochism, and on the other one the relation with the ideal and the primary narcissism. It is the most fondamental of phantasies so much that it can be considered as an account of its « formula », the text expressing its logic at best.Intertwining these two matrixes — structure of the phantasy and clinical reality of child abuse — and exploring the ins and outs ; this is the issue analyzed in the thesis. First, what is shown here is how feminity can lead to maternity and what is at stake in « desiring a child », even in the desire for the child’s death (i.e. in the « ravage » when it leads to the acting out). Then which extreme can the clinical approach get close to with « The Munchausen syndrom by proxy » on one hand, and « neonaticides » with the keeping of the bodies of the new borns on the other one (i.e. where horror can be shown to hide all the better and to hide to be better exhibited).Hence the possibility, by unfolding the figures of the mother, of the child, and of the evil, to shed light on the logic of the phantasy thanks to the clinical reality — and vice versa.In other words, to throw light on the subjective structure, seized in the « heart of being »

Texto completoThe goal of this research is to evaluate and understand the repercussions of physical and psychological abuse on the development of children who are victims. The focal point is to identify the mental appraisal the mother has of the child and how it inherently drives her to mistreat the child. The aim of the study was to ascertain the child’s place in the mother’s mental thoughts with the assumption of the portrayal of an infanticide fantasy at work. Culturally, it was pertinent to broaden the notion of abuse way beyond the common syndromes especially associated with children that act bizarre. The study aimed to understand the modalities of expression of the child below the age of three through behaviors externally and internally expressed. Fifteen children from 6 months to 3 years of age and their mothers were selected for this study. Different methods were used such as, the revised version of the Brunet-Lézine scale, the "Malette projective Petite enfance " test, observation and clinical research interviews. The results highlighted the act of fantasy in the 12 mothers in different forms. Right from the experience of pregnancy, to childbirth, to nursing the child, etc. and the effects of abuse that hinder the development of these children

Texto completoThe main objective of this work is to identify the mechanisms (specially, defense mechanisms) and the psychodynamic processes at work in child protection system institutions. The child protection system is presented in terms of contextual concepts as well as a literature review on the clinical and psychopathological aspects of the different actors involved in the system (abused children, abusive parents, and child protection professionals). A hypothesis was formulated using psychodynamic theories postulating that parental and infantile psychopathology implicates a subjective participation of child protection professionals. This process was studied in terms of the relationships between the different participants using qualitative methodology (case studies of five different types of placements situations), including semi-directive interviews, biographical interviews, and projective tests. The results from this study allowed for the formulation of statistical hypotheses concerning the metamotivational states of child protection professionals with regards to their work (89 subjects). The results indicate the pertinence of applying psychodynamic theory to institutional relationships and provide for the development of theoretical and practical perspectives within the framework of service delivery in child protective sevices

Texto completoAmong child abuse, which is a major public health issue, Pediatric Abusive Head Trauma is one of the major causes of death in abused children. The aim of this thesis was to assess the opportunity of screening for physical child abuse. The first identified obstacle was the lack of knowledge about the beginning of physical child abuse, and the vagueness and lack ofstandardization of its definition. Therefore, we conducted a study to assess the perception of physical violence by parents toward their children, by parents and professionals in the emergency department of the university hospital of Bordeaux. The professionals were more tolerant than parents and the perception of a same situation could vary according to the professional. We have illustrated this variation with a clinical case of Pediatric Abusive Head Trauma, when the child had not already serious consequences of child abuse. Similarly, the difficulty of early identification of abused children was responsible for the fear of mother’sstigmatization. And we discussed the impact of management of the children and their mother.Finally, we evaluated the impact of a primary prevention program and screening program ofPediatric Abusive Head Trauma using a Markov model considering identified uncertainties.The simulation confirmed the potential benefits of primary prevention program documentedthe huge uncertainty regarding benefits associated with screening of Pediatric Abusive HeadTrauma. Future research should in particular focus on describing the effects of wrong lyidentifying parents as abusers

Texto completoOn December 31, 2019, SARS-CoV-2 was first detected in Wuhan province, China. The rapid spread of this virus led to a global pandemic that forced governments to take severe restrictive measures, such as general population lockdowns. These measures led to major changes in the personal and professional lives of French people.Our focus was particularly on the healthcare system. This work presents the results of two national studies carried out during the first peak of the epidemic, which illustrate the impact of the pandemic on the healthcare population. Caregivers are represented by intensive care unit (ICU) staff; users by children hospitalized for physical abuse.Our work shows the impact of the pandemic-era measures on abused children and intensive care unit staff, for whom the health crisis revealed pre-existing inequalities and exacerbated difficulties.The ICU staff experienced intense distress at work. However, they did not take advantage of the help offered by the professional hotlines. Children under the age of 5 were hospitalized more frequently as a result of physical abuse, a phenomenon that may be explained by the exacerbation of risk factors for violence during this period.The events of spring 2020 revealed the weaknesses of the French healthcare system and led to the convergence of a number of factors that encouraged interpersonal and institutional abuse.Four years after the outbreak of the epidemic, we need to consider how to best restructure our healthcare system. Moving towards a holistic approach to care and developing multi-professional networks appear to be crucial steps towards enabling caregivers and users to regain confidence and to find meaning in the hospital system

Even if these reasons are defined by the law, we can think that their interpretation implies a part of subjective perception. Indeed, how to define the conditions of an education "seriously compromised" (Article 375 of the Civil Code)? To have "manifestly" lost interest in one's child (article 350 of the Civil Code)? Can we give an objective definition of "the interest of the child"? The aim of this research was to analyze arguments leading to the placement of minors, by comparing three periods at 5-year intervals, from 2000, just before the publication of two major laws, in 2002, one renewing the social action and the other one redefining parental authority and introducing "the interest of the child". We thus hypothesized that the arguments put forward to request or pronounce the placement have evolved over time, particularly in view of the legal evolutions. For that purpose, we analyzed semantically, with the TROPES software, the closed files of 65 placed children, according to several criteria: period; age of children (0-6, 6-12 and 12-18) and the gender of children. We have thus been able to highlight an evolution of words used, with social reports and judgments have been expanded over time and have increasingly focused on the living conditions of the child and his psychological state, whatever the age and gender. However, some differences have been revealed: the age and gender of children influence the expression of the symptomatology of the abuse suffered, but also the perception of the different professionals, social workers and judges

Texto completoThe development of Emotion Regulation Strategies (ERS) is made through parent-child relationships. This study is specifically focused on foster children and ask the following question: How foster children learn to regulate their emotional states considering the multiple attachments existing between their natural and foster families in the context of foster care? To answer this question, 8 preschool children and their families are enrolled in the research, according to the case study methodology. The research design includes parent-child observational method into an ecological model. Results indicate that the foster mothers adopt focused on the adult's type of behaviour where they lead the interactions and children are not able to express themselves freely. Moreover, the dysfunctional interactions occurring in natural families were mostly regulated by one the parents and some situations were not massively dysfunctional regarding ERS. The adult's ability to let the child lead the interaction and adopt symbolic or anticipative ERS enables children to adopt socially accepted ERS where effortful control, positivity and symbolic elaboration are widely developed. All children included in this study showed various severity of anxious symptoms. The social welfare service's role tends to amplify the situation's instability through parent-child visitation, causing more risk. This type of methodology developed here, put into the light the internal states of the foster children and focus on maltreating and substitutive family systems emotionality

Texto completoThis research studies the consequences of different kinds of ill-treatments on the building of the individual's narcissistic identity. In particular, the ill-treatments by extreme negligence and psychological ill-treatments of which the major consequence would be to hinder the process of identity in the first stages of life. The clinical and psychoanalytical analysis from interview material with adults having been victims of ill-treatments, either during childhood or later, enables to identify: defence mechanisms and psychical adaptations, aggravating factors and resilient factors to ill-treatment traumatism. A differential analysis enables to emphasize three determining factors in building up the narcissistic identity in situations of ill-treatment: the typology of traumatism, the age of the subject at time of traumatism and the ill-treating surroundings. The link pathology present in all ill-treatment situations, whether within the family or outside the family, leads to calling into question the dynamics of the family or institutional system from subject's view in interaction with his family institutional and social environment

In Frankreich ebenso wie in Deutschland versuchen Kinderschutzorganisationen die Risiken ihre diagnostische Methoden bereits im Ansatz so gering wie möglich zu halten und damit ihre Prozesse zu rationalisieren. Dabei laufen sie jedoch Gefahr die Partizipation den Klienten zu vergessen. Die vorliegende Arbeit erstellt eine vergleichende Perspektive zwischen Frankreich und Deutschland, in der Formalisierungsverfahren von diagnostichen Methoden im Kinderschutz sowohl auf der rechtlichen Ebene, als auch in der Praxis der Beschäftigten kritisch beleuchtet werden. Texto completoThe question of children who are victims of mistreatment is currently a central point of the socio-politic scene. The discourses of both moral and expert points of views tend to trivialize this question. They put it away like a phenomena which is not really recognized as being legitimate and without paying attention. Terror and scandal set the tone of a sensationalist media and introduce a widespread suspicion that adults, especially those who play the role of parents, become potential criminals, and in which the child supposed to be fragile and innocent, could become a potential victim. This study which is highly theoretical issue, tries to understand the subjective logic that could be hidden behind such questioning. To respond to it, the start of our work is to adopt the opposite hypothesis of the dominant discourse. Our point of view focuses on the fact that mistreatment is anchoring in a dynamic process rather than in a simple ‘given fact’, that is ill-treated child. This mistreatment construction has specially something to do with the contemporary subjectivities and the place that occupied by the child. At first, our work strives to highlight the terms of this construction by evoking the notions of danger and of victim in relation with the psychic trauma and with of child of law. In a second time, it is interested in a psychoanalytic approach from the concepts of trauma, of infantile sexuality and of phantasm, articulated at the image of the ill-treated child to ultimately consider the notion of mistreatment as a form of the culture uneasiness

Texto completoThe aim of this thesis was to examine the contributions of temperamental dispositions and of maltreating experiences on the development of emotion discrimination abilities between static and dynamic emotional expressions and of emotion attribution abilities in 7 to 16 years old children and adolescents. 84 non maltreated and 48 non clinical maltreated children and adolescent's temperament was assessed (Experiment 1). The development of emotional processing was slower in maltreated children and adolescents, especially for negative emotional experssions, compared to control youngsters. « Timid » and « Impulsive » temperamental profiles on the one hand, and transfer to foster care after 5 years old on the other hand accentuated maltreated youngsters' slow processing. These results suggest that variations in children and adolescents' experiences and internal predispositions influence the interpretation and understanding of emotional signals. These results are discussed with the frame of transactional and developmental theories

While we await a cure, the prevention of transmission and the identification of high-risk factors for women are paramount. This study introduces the hypothesis that one will find a high percentage of childhood sexual abuse among these women. Since we know that two long-term effects of childhood sexual abuse are at the same time the two most common modes of transmission: intravenous drug use and unprotected sexual intercourse, childhood sexual abuse is presented as a high-risk factor for contracting the virus. Using this hypothesis as a starting point, an analytical model, called Women Confronting AIDS (WCA), was designed to identify and explain the key factors involved in the overall functioning of these women and in their adaptation to their first trauma, childhood sexual abuse, and to their second trauma, a diagnosis of HIV or AIDS. Of the 60 women evaluated, each having either an HIV positive status or a diagnosis of AIDS, 59 disclosed having been a victim of sexual abuse during their childhood. Other results (simple statistics, correlations, analyses of regression) support other factors in the WCA model and clarify the fundamental processes and certain components of adaptation or “maladapation” to trauma in HIV positive women or women living with AIDS. The results highlight the need for a psychological intervention to ameliorate their adaptation to the virus, but also the urgent need for an intervention with teenage girls and women who are HIV negative but were sexually abused during their childhood, because they constitute a new population at risk for contracting the virus. In this way, it may be possible to slow the spread of this devastating virus

Texto completoPeople with a history of childhood maltreatment are more likely to present with several psychiatric disorders. Comorbidity between disorders tends to be the rule rather than the exception in individuals who have experienced childhood maltreatment. Given the presence of multiple comorbid psychiatric disorders in this population, the transdiagnostic approach offers a conceptual frame with which to explore common dimensions and risk factors of psychiatric disorders. According to the transdiagnostic approach, dimensions and risk factors shared between psychiatric disorders could be genetic, neurobiological, psychological and neuropsychological. However, few studies have identified neuropsychological mechanisms associated with maltreatment among people with several psychiatric disorders. The general objective of the current thesis is to explore the transdiagnostic neuropsychological mechanisms associated with childhood maltreatment among people with psychiatric disorders. The first study, a meta-analysis, highlighted transdiagnostic neuropsychological mechanisms among people with several psychiatric disorders. Specifically, childhood maltreatment had a significant impact on working memory, verbal episodic memory, intelligence and processing speed. The second study, an empirical study, explored the role of neuropsychological impairments in the presence of psychiatric symptoms among teenagers with a history of childhood maltreatment and multiple psychiatric disorders. This study showed that retrieval in visual episodic memory and planning are predictors of externalizing symptoms among teenagers. Altered executive mechanisms could explain the variance of externalizing symptoms among teenagers with several psychiatric disorders. The work within this current thesis represents the first attempt to explore the common neuropsychological dimensions between psychiatric disorders in a transdiagnostic approach. Indeed, this thesis identifies neuropsychological mechanisms associated with childhood maltreatment that are common among people with several psychiatric disorders. Moreover, some of these neuropsychological mechanisms are predictors of the variance of externalizing symptoms among teenagers. In short, this thesis improves the current understanding of the complex relation between childhood maltreatment, neuropsychological mechanisms and the presence of psychiatric symptoms, according to the conceptual framework of the transdiagnostic approach.

Texto completoOur research deals with the problem of child psychology rarely addressed by the cognitive psychology of education : forms of child abuse. Our focus is on creating a prevention method applicable in Iran. Child abuses have an impact on the bodily, mental, emotional, sexual and even social aspects disrupting the normal growth of the child. This represents a socio-psychological problem not adequated studied in the past to learn the consequences that is implies on the psychological development of children, even if, particularly in France, sociologists and psychologists are interested in since few years. The aim of this study was to establish an educative method in two forms (Conference and Theater-workshop) in order to prevent child abuse, in family context and outside. In this thesis, we have worked out a preventive method that we have realized in Iranian school. Five hundred and four students, 9 and 11 years old have taken part in our study. All of them have done one of the two forms of the preventive program. With the aid of a pretest and posttest questionnaires realized by children, the effectiveness of our educative program has been evaluated. Gender, school and socio-economic effects have been also measured. The main results show that students' knowledge about child abuse and attitudes to have in order to avoid is better after the educative program of prevention than before, and that, compared to students who did not take part in this type of preventive program. In a political context such as Iran's one, prevention by education is the best hope for Iranian children

Texto completoAfter having introduced the theoretical/clinical research project by a historical backdrop of resilience and the proposition of a conceptual framework, the author attempts to refine and formalize the processes at play in resilience, based on psychoanalytical, theoretical referents. She centres her work on the four modalities liable to ensure a function of protection against adversity, while considering internal factors : The role of the defences of the Self, the richness of the Imagination, the quality of mentalization, and an external factor : The "resilience tutor". The author proceeds with a compared study of contrasted unique cases (12 children that have all undergone the trauma of child abuse). The methodology used herein relies on the analysis of an interview (targeted at appreciating certain characteristics of the resilience tutor) and on that of projective tests (Royer tales, Rorschach test, drawings). The results obtained emphasize the importance of defence mechanisms in the first phase of the resistance against the trauma : They constitute the preliminary stage towards building up the trauma. Texto completoImportance: Childhood abuse is a global public health issue yet there is a serious lack of reliable epidemiological data and the figures reported are very often underestimated. A large part of these children are not diagnosed. Moreover, diagnosed children are not recorded in a single database.Objective: The aim of this study is to provide two estimations of the frequency of child physical abuse requiring hospitalization (hospital prevalence) and the corresponding in-hospital fatality. Design, setting and participants: This was a national retrospective study using a national hospital database (PMSI).We included all children aged less than 2 years old hospitalized at least once in France from 2007 to 2014. Infants less than 1 month old were excluded in order to exclude all pathologies and neonatal complications.Three groups were defined: group1 - physically abused children (identified by the ICD 10 codes used in the PMSI concerning physical abuse); group2 - possible physically abused children (suspicious physical injuries associated with child maltreatment); and group3 - all other hospitalized children (the control group).Main outcomes and measurements: First, based on the number of children aged 0 to 2 years in France in 2010, we provide two estimations of the frequency of child physical abuse requiring hospitalization (hospital prevalence): a lower bound represented by the children of group 1 and an upper bound represented by the children of group 1+2.Secondly, we studied the follow-up of these children from their inclusion to 2014, thanks to the linkage of the differents hospital stays; with regard to the number of hospitalizations, their length of stay, their reasons and in-hospital fatality.Results: In our study, 2,628,386 children were included: 4,172 in group1, 12,024 in group2 and 2,612,190 in group3. The national annual hospital prevalence of child physical abuse (and hospital fatality) in children aged 0 to 2 years ranged from 1.6‰ (2.52%) to 5.7‰ (2.01%). Most hospital deaths occurred during the first hospital stay (between 99% and 99.6%) and are 10 times higher than in the control group. The length of stay of abused children is up to 5 times longer than for other children. Conclusions and relevance: To our knowledge, this is the first study to provide an estimation of the frequency of child physical abuse requiring hospitalization and the corresponding in-hospital fatality. Three out of four children may not be identified as being maltreated and are at risk of recurrent abuses. More effective prevention should be implemented by health professionals in order to identify those children
