Modern assessment of left ventricular (LV) diastolic function should be based on the estimation of degree of LV filling pressure (LVFP), which is the true determinant of symptoms/signs and prognosis in heart failure.In order to achieve this goal, standard Doppler assessment of mitral inflow pattern (E/A ratio, deceleration time, isovolumic relaxation time) should be combined with additional manoeuvres and/or ultrasound tools such as: ◆ Valsalva manoeuvre applied to mitral inflow pattern. ◆ Pulmonary venous flow pattern. ◆ Velocity flow propagation by colour M-mode. ◆ Pulsed wave tissue Doppler of mitral annuls (average of septal and lateral E′ velocity).In intermediate doubtful situations, the two-dimensional determination of left atrial (LA) volume can be diagnostic, since LA enlargement is associated with a chronic increase of LVFP in the absence of mitral valve disease and atrial fibrillation.Some new echocardiographic technologies, such as the speckle tracking-derived LV longitudinal strain and LV torsion, LA strain, and even the three-dimensional determination of LA volumes can be potentially useful to add further information. In particular, the reduction of LV longitudinal strain in patients with LV diastolic dysfunction and normal ejection fraction demonstrates that a subclinical impairment of LV systolic function already exists under these circumstances.

The complexity of left ventricular (LV) function(s) assessment in heart failure patients is related to the complexity of heart anatomy, but also to the complexity of electromechanical interaction, and to the load dependency of all the parameters that could be applied in clinical practice. Three perpendicular axes orienting the global geometry of the LV define the local cardiac coordinate system: radial, circumferential, and longitudinal. Speckle tracking is the technique of choice for quantifying myocardial deformation (regional and global). Longitudinal LV deformation, which is predominantly governed by the subendocardial region, is the most vulnerable component of LV mechanics and therefore most sensitive to the presence of myocardial disease.

The heart functions by means of a three-dimensional arrangement of myofibres supported by an extracellular matrix which plays an important role in maintaining the size and shape of the heart. In both atria, the structure of the walls and the atrial septum confers a three-dimensional arrangement of muscle bundles and myoarchitecture that allows preferential electrical intra- and interatrial conduction which is important for a better understanding of atrial activation and arrhythmias. The myoarchitecture within the ventricular walls has a three-dimensional arrangement of myofibres, within a supporting matrix of fibrous tissue, which changes orientation from being oblique in the subepicardium to circumferential in the middle and to longitudinal in the subendocardium, allowing the chambers to change in shape and size through the cardiac cycle. Within each ventricle, the circumferential portion is the thickest transmurally, with the longitudinal portion the thinnest. The three-dimensional arrangement of the ventricular mesh serves to realign the myocytes during ventricular contraction, accounting for the extent of systolic mural thickening. Abnormal myoarchitecture in combination with alterations in the connective tissue matrix provide the structural basis for abnormalities in myocardial function.

The Massachusetts General Hospital (MGH) Longitudinal Studies of Attention Deficit Hyperactivity Disorder (ADHD) evaluated and followed a large sample of both boys and girls with ADHD and controls without ADHD, along with their families, ascertained from psychiatric and pediatric sources. These studies documented that ADHD in both sexes is associated with high levels of persistence onto adulthood; high levels of familiality with ADHD and other psychiatric disorders; a wide range of comorbid psychiatric and cognitive disorders including mood, anxiety, and substance use disorders; learning disabilities with reading and math; executive function deficits; emotional dysregulation and autistic traits; as well as educational, social, and occupational dysfunctions. The MGH studies also suggested that stimulant treatment significantly decreased the risk of developing comorbid psychiatric disorders, substance use disorders, and impaired functional outcomes. The studies also documented the neural basis of the persistence of ADHD using resting-state functional magnetic resonance imaging (fMRI).

Cancer therapeutics may induce cardiac damage in the left and the right ventricle. Radiotherapy most frequently induces valvular damage, carotid stenosis, and coronary artery disease. Pericardial disease may be due to both chemo- and radiotherapy. The manifestations of both chemo- and radiotherapy can develop acutely but also become overt years after their performance, in particular after radiotherapy. The main cardiac damage of cancer therapeutics-related cardiac dysfunction (CTRCD) corresponds to the reduction of left ventricular (LV) systolic function. The Expert Consensus document from ASE and EACVI has defined CTRCD as a decrease in LV ejection fraction (LVEF) of greater than 10 percentage points, to a value less than 53%. The accurate calculation of LVEF at baseline and during follow-up is extremely important. The assessment of LV longitudinal function, in particular of speckle tracking-derived global longitudinal strain (GLS), can provide additional information, allowing early, subclinical detection of CTRCD. The ideal strategy could be to compare the measurements of GLS obtained during chemotherapy, with the one obtained at baseline. An integrated approach with the use of echocardiography at standardized, clinical preselected intervals with biomarker (ultrasensitive troponin) assessment prior to each chemotherapy cycle could be suggested in patients at high risk of CTRCD. Follow-up after therapy should depend on the type of chemotherapy/radiotherapy and the presence/absence of on-therapy CTRCD. Long-term follow-up should be planned after radiotherapy.

This chapter summarizes the available epidemiological evidence supporting the current diagnoses grouped in the disruptive behavior disorder (DBD) cluster. It seems DBDs are common disorders, but although researchers have made great strides in capturing the prevalence of these disorders in normal and clinical populations, and although there are some very solid longitudinal findings, there are still many unknowns that need to be corrected. Most of the difficulties encountered in epidemiology are a function of the ongoing changes in the descriptive diagnostic criteria and the lack of a truly state-of-the-art three-level epidemiological design. This chapter discusses these issues including implications for clinical practice.

In contrast to early reports, it is now appreciated that psoriatic arthritis (PsA) can present as a destructive, progressive, and disabling arthritis with consequences as severe as those of rheumatoid arthritis. Longitudinal cohort studies of PsA patients contributed important knowledge about long-term outcomes, such as development of structural joint damage, remission achievement, and physical function. These studies identified predictors for improved outcomes including male gender and lower burden of inflammation at presentation while delayed diagnosis, disability, and joint damage are associated with worse long-term outcomes. These findings suggest early diagnosis and aggressive control of inflammation are important as they may prevent the occurrence of subsequent joint damage. The latter is strongly correlated with long-term outcomes, such as reduced physical function and increased mortality. Development of prediction models using clinical measures, laboratory biomarkers, and imaging is warranted to stratify patients with early disease into risk groups for long-term outcomes.

The concurrent and longitudinal association between intimate relationship dysfunction and depression is well established in both adolescents and adults. This association can be best understood as a bidirectional transactional one, such that intimate relationship dysfunction and depression reciprocally influence one another over time. This chapter reviews the existing research in this area, focusing on the main components and processes of intimate relationships (including how they start, function, and end) in relation to depression. Adolescent research has also focused on how romantic and sexual experiences relate to depression, which is also reviewed. Finally, couple therapy, particularly cognitive behavioral couple therapy, is discussed as an efficacious treatment for comorbidity and intimate relationship dysfunction-depression. Future directions for research are also suggested.

Adolescence is a period of intense transition and change, from social and emotional changes with increased independence from family to physical changes associated with the onset of puberty. The onset of involvement with alcohol and drugs emerges simultaneously with these interrelated biological, cognitive, affective, and social changes. This chapter considers how developmental processes and emerging domains of functioning impact alcohol consumption in adolescence and presents examples of two lines of research that use integrative models to explore these relationships: the National Consortium on Alcohol and NeuroDevelopment in Adolescence, a longitudinal study designed to examine the developmental impact of alcohol and other drug use on neuroanatomy, neurocognition, and behavior; and Project Options, a voluntary, high school-based intervention aimed at reducing dangerous alcohol use. The chapter concludes with a discussion of research questions for future study, highlighting the central function of technological, behavioral, biomedical, and data analytic advances in these efforts.

This chapter summarizes the accumulating evidence that incomplete or even apparent complete recovery of renal function after acute kidney injury (AKI) may be an important contributor to a growing number of incident chronic kidney disease (CKD) and end-stage renal disease (ESRD) cases, largely in excess of the global growth in CKD prevalence. Evidence based on epidemiologic studies supports the notion that even after adjustment for several important covariates AKI is independently associated with an increased risk for both CKD and ESRD. Several risk factors for the subsequent development of CKD among survivors of AKI have been identified. Besides well-known risk factors for CKD in general, such as hypertension, older age, congestive heart failure, diabetes, and proteinuria, AKIN staging and duration also predict longitudinal CKD development. These characteristics may identify a category of at-risk AKI patients at the time of hospital discharge that will need long follow-up times for appropriate screening and surveillance measures for CKD.

The appropriate normalization of exercise performance data for differences in body size underpins the clarification of growth and maturational influences on physiological function. Therefore, scaling is an issue of fundamental importance for all paediatric exercise scientists. The selection and application of a scaling method appropriate for the data and research question being addressed is at least as important as ensuring that the methodology used to collect the data is valid, reliable, and appropriate for use with young people. Several scaling methods are available and some methods can be applied in different ways. Unfortunately, taken as a whole, the extant literature presents a confusing picture as to which of these techniques is preferable, how they should be applied, and the meaning of the results obtained. The aim of this chapter is to clarify these issues through a description of the techniques available for analysing both cross-sectional and longitudinal data sets, highlighting their statistical and theoretical derivations.

The selection and application of a scaling method appropriate for the data and research question being addressed is at least as important as ensuring that the methodology used to collect the data is valid, reliable, and appropriate for use with young people. Several scaling methods are available and some methods can be applied in different ways. Unfortunately, taken as a whole, the extant literature presents a confusing picture as to which of these techniques is preferable, how they should be applied, and the meaning of the results obtained. The aim of this chapter is to clarify these issues through a description of the techniques available for analysing both cross-sectional and longitudinal data sets, highlighting their statistical and theoretical derivations.

This book presents a new neurocognitive theory of dreams that documents the similarities of dreaming to waking thought, demonstrates that personal psychological meaning can be found in a majority of dream reports, has a strong developmental dimension based on excellent longitudinal and cross-sectional studies carried out in sleep labs with children ages 3–15, locates the neural substrate for dreaming in the same brain network active during mind-wandering and daydreaming, and marshals the evidence that shows it is very unlikely that dreaming has any adaptive function. These claims are based on five different sets of descriptive empirical findings that were developed between the late 1950s and the first sixteen years of the twenty-first century. All of these findings were unanticipated by scientific dream researchers and then resisted to varying degrees by dream theorists for a variety of reasons. The first five chapters spell out the theory and the evidence for it without any discussion or criticism of past theories. The next two chapters present detailed criticisms of two major alternative theories. The penultimate chapter presents evidence that it is very unlikely that dreaming has any adaptive function in the evolutionary sense of the term, although humans have invented uses for dreams in religious and healing rituals. In that regard, dreaming has an emergent function in culture that was invented in the course of history due to human cognitive capacities. The final chapter presents a general agenda for future research using new methodologies to test all of the neurocognitive hypotheses.
