BackgroundPreviously, we demonstrated that radiation therapy (RT) combined with Bempegaldesleukin (BEMPEG;NKTR-214), a first-in-class CD122-preferential IL-2 pathway agonist, led to enhanced anti-tumor efficacy through a T cell-dependent mechanism. However, we observed only modest systemic responses to BEMPEG/RT across several murine tumor models. Therefore, we explored alternative approaches to improve systemic tumor-specific immunity. We evaluated whether intratumoral NKTR-262, a polymer-modified toll-like receptor (TLR) 7/8 agonist, combined with systemic BEMPEG treatment resulted in improved tumor-specific immunity and survival compared to BEMPEG combined with RT. We hypothesized that BEMPEG/NKTR-262 immunotherapy would promote synergistic activation of local immunostimulatory innate immune responses followed by systemic adaptive immunity to significantly improve tumor regression and overall survival.MethodsTumor-bearing mice (CT26; EMT6) received BEMPEG (0.8 mg/kg; iv), RT (12 Gy x 1), and/or intratumoral NKTR-262 (0.5 mg/kg). Flow cytometry was used to evaluate CD4+ and CD8+ T cell activation status in the blood and/or tumor (7 days post-treatment) and NK cell activity in the tumor (1, 3 days post-treatment). The contribution of specific immune subsets was determined by depletion of CD4+, CD8+, or NK cells. CD8+ T cell activity was determined in vitro by tracking apoptosis in an Incucyte assay. Data are representative of 1–2 independent experiments (n=5–14/group) and statistical significance was determined by 1-way ANOVA (p-value cut-off of 0.05).ResultsBEMPEG/NKTR-262 resulted in significantly improved survival compared to BEMPEG/RT. BEMPEG/NKTR-262 efficacy was NK and CD8+ T cell-dependent, while BEMPEG/RT primarily relied on CD8+ T cells. Response to BEMPEG/NKTR-262 was characterized by a significant expansion of activated CD8+ T cells (GzmA+; Ki-67+; ICOS+; PD-1+) in the blood, which correlated with reduced tumor size (p<0.05). In the tumor, NKTR-262/BEMPEG induced higher frequencies of GzmA+ CD8+ T cells exhibiting reduced expression of suppressive molecules (PD-1+, TIM-3+), compared to BEMPEG/RT. Indeed, CD8+ T cells isolated from BEMPEG/NKTR-262-treated tumors had greater cytolytic capacity than those from BEMPEG/RT-treated mice. CD8+ T cell expansion (blood) and activity (tumor) depended upon the initial NK response, as neither occurred in the absence of NK cells. BEMPEG/NKTR-262 uniquely induced the expansion of early and high effector NK cells.ConclusionsCombining BEMPEG with NKTR-262 lead to an early and robust NK cell expansion not observed in the BEMPEG/RT combination. The improved tumor regression and survival was dependent on the NKTR-262 driven expansion of NK cells. A clinical trial of BEMPEG/NKTR-262 for patients with metastatic solid tumors is in progress (NCT03435640).

BackgroundTumor cell death caused by radiation therapy (RT) can trigger anti-tumor immune responses in part because dying cells release adjuvant factors that amplify and sustain DC and T cell responses. We previously demonstrated that bempegaldesleukin (BEMPEG:NKTR-214, a first-in-class CD122-preferential IL-2 pathway agonist), significantly enhanced the anti-tumor efficacy of RT through a T cell-dependent mechanism. Because RT can induce either immunogenic or tolerogenic cell death, depending on a multitude of factors (radiation dose, cell cycle phase, and tumor microenvironment), we hypothesized that providing a specific immunogenic adjuvant, like intratumoral NKTR-262, a novel toll-like receptor (TLR) 7/8 agonist, to the tumor site would further improve systemic tumor-specific immunity by promoting synergistic activation of local immunostimulatory innate immune responses. BackgroundNKTR-262 is a small-molecule agonist of toll-like receptors (TLR) 7/8. Given by intratumoral (IT) injection, NKTR-262 is retained within the tumor microenvironment (TME) and promotes an immunostimulatory milieu and tumor antigen release. Bempegaldesleukin (BEMPEG) is a CD122-preferential IL-2 pathway agonist, which increases proliferation and tumor infiltration of CD8+ T cells and natural killer (NK) cells. Preclinically, NKTR-262 plus BEMPEG combined innate immune signaling and enhanced antigen presentation, with sustained T-cell activation, resulting in tumor growth inhibition of treated and abscopal lesions.MethodsThis phase 1 dose-escalation study enrolled patients with relapsed/refractory, advanced/metastatic solid tumors (REVEAL; NCT03435640). Patients received escalating doses of NKTR-262 (0.03 mg to 3.84 mg IT) followed 3 weeks‘ later by BEMPEG (0.006 mg/kg IV) q3wk utilizing a 3+3 design. The primary endpoint was safety and tolerability, including definition of the recommended phase 2 dose (RP2D). Other endpoints included antitumor activity, pharmacodynamics, and pharmacokinetics.ResultsAs of June 15, 2020, 36 patients were enrolled. One dose-limiting toxicity, transient transaminase elevation, was observed at the highest NKTR-262 dose (3.84 mg). The most frequent treatment-related adverse events were flu-like symptoms, fatigue, nausea, and pruritus, consistent with the known profile of BEMPEG. Early evidence of clinical activity was observed in patients with metastatic melanoma, with a disease control rate (partial response [PR] + stable disease) of 41.2% (7/17 patients), including two patients with PRs after progression on two prior immunotherapy regimens. Preliminary analyses showed dose-dependent induction of CXCL10 and type 1 interferon genes, consistent with TLR7/8 engagement. CD11c+ target cells were significantly more abundant in baseline melanoma biopsies than other tumor types (p<0.001). Induction of TLR7/8-responsive genes correlated with CD11c+ baseline density (p<0.05). Minimal TLR7/8-dependent changes in immune cell subsets or inflammatory cytokines were observed in peripheral blood, reflecting favorable TME modifications driven by retention of NKTR-262. Increased activation of CD4+, CD8+, and NK cells in blood were observed, consistent with BEMPEG mechanism of action.ConclusionsNKTR-262 plus BEMPEG led to engagement of the entire immune activation cascade required for systemic tumor clearance. Robust TLR7/8 engagement supported the NKTR-262 mechanism of action, while the minimal toxicity profile underscored the benefit of local delivery of NKTR-262, and the BEMPEG combination induced systemic activation of T and NK cells. These data support the RP2D of NKTR-262 (3.84 mg IT) plus BEMPEG (0.006 mg IV) q3w, and the initiation of the phase 1b dose-expansion phase, which is exploring concurrent dosing, with or without nivolumab, in relapsed/refractory metastatic melanoma patients.Trial RegistrationNCT03435640Ethics ApprovalThe study was approved by the institutional review board of each participating site.

Geopark is an area that combines the uniqueness of geology, biodiversity, and cultural diversity that is managed for the needs of conservation, education, and sustainable economic development of the community. The existence of this Geopark has opened up opportunities and become a great chance for local communities. Gorontalo province has the biggest potential natural resources for development; in this matter, we need local creativity to build efforts in special characters and also unique for itself. The Gorontalo Geopark is currently undergoing improvements for the development of MSMEs to create innovative products related to the Geopark and actively contribute to the local economic growth of the community. This study aims to understand the existence of geo products as part of MSMEs and find out how the strategy of geo product development for local economic growth can support the development of the Gorontalo geopark. This study uses a qualitative descriptive method with primary data collection techniques through observation, interviews, and secondary data obtained from the relevant agencies. The results showed that geo products are one of the first steps in developing MSMEs to improve the community's economy, although, in practice, there are still problems. In this condition, the local government must formulate a policy strategy for developing MSMEs through geo product, namely fixing the database, increasing community capacity, creating innovation, creating geopark branding, optimizing the use of technology, stakeholder collaboration also promotion strategy. This policy strategy is formulated so that the presence of geo products can support the Gorontalo geopark in improving community welfare.

262 Background: Stereotactic body radiation therapy (SBRT) permits precise delivery of 1-5 high doses of radiation to liver tumors with low risk of toxicity. This study aims to evaluate local control and overall survival (OS) in patients treated with SBRT for primary and metastatic liver lesions. Methods: From 2006 to 2010, 62 patients with 106 liver lesions (71 metastatic, 35 primary) were treated with SBRT. Lesions were most commonly from primary hepatocellular carcinoma (29%), or colorectal (23%), breast (8%), or esophageal metastases (8%). Patients were heavily pretreated: 51% received ≥1 prior form of liver-directed therapy and 54% received ≥1 prior systemic therapy regimen. The number of active hepatic lesions at the time of SBRT was 1 in 49%, 2 in 23%, 3 in 22%, and ≥4 in 7%. Of the metastatic lesions, 42% were accompanied by active extrahepatic disease. Of the primary liver lesions, 69% were Child-Pugh class A, 26% class B, and 6% class C. Median gross tumor volume (GTV) was 8.8 cm3 (range 0.2-222.4). Median biological equivalent prescription dose (BED10) was 100 Gy (42.6-180). The majority of patients received either 3 or 5 fractions (47% and 48%, respectively). Results: Median follow-up for all lesions was 18 months (4.6-46.8). Local tumor control was observed in 92/106 treated lesions with 1- and 2-yr local control rates of 88% and 74%. No correlation was found between local control and lesion type (primary vs metastatic), lesion size, or prior liver directed therapy. Median survival for all patients was 25.2 months, with 1- and 2-year OS of 81% and 52%. Survival was longer among patients with metastatic lesions compared to primary lesions (2-yr OS 63% vs 29%, HR 0.46, p=0.02). The number of active hepatic lesions was an independent prognostic factor for survival (HR 1.77, p=0.002). For patients with metastatic liver lesions, the presence of active extrahepatic disease yielded significantly worse survival (2-yr OS 37% vs 83%). Among HCC patients, a lower CLIP score was associated with improved survival with 2-yr OS of 55% for CLIP score 0, 44% for score 1, 17% for score 2, and 0% for score 3. No patients experienced RILD. Conclusions: Treatment of liver tumors with SBRT provides excellent local control, necessitating further improvements in systemic therapy. No significant financial relationships to disclose.

Transparency is a challenge for public administration reforms for now and beyond towards modern and better governance. The economic crisis that occurred has led to a large erosion of public confidence in the effectiveness of government financial management. This study aims to analyze the key elements of local government transparency in new public governance. The research data was obtained through financial reports on local government websites. The sample selection technique used purposive judgment sampling and yielded a total of 262 local governments in Indonesia. The analytical method used is logistic regression analysis. The results showed that key elements, reflected by the local government size and local financial independence ratio, had a significant influence on the implementation of internet financial reporting, while audit opinions do not have a significant effect. This is due to changes in public perception of the quality of audit opinions provided by the Audit Board. This study emphasizes that local governments that receive a fair or unfair audit opinion cannot prove with certainty that the local government has performed good or poor financial management. Audit opinions obtained by local governments do not always facilitate the publication of financial statements on their official websites.

The metal welding is accompanied by the formation of mechanically non-homogenous sections of welded connection. The pipeline welded connections also have sections, which are different in structure, chemical composition and mechanical properties. The mechanical inhomogeneity affects the load bearing capacity of welded connection and the structure as a whole, which is necessary to take into consideration when performing calculation analysis. So far, the specialists have established the dependencies in assessment of welded connection strength with various types of heterogeneous sections. However, this phenomenon has received little attention in case of pipeline welded connections made of low carbon steels. The existing theoretical models do not reflect actual anisotropy of mechanical properties of the welded connections and weld adjacent zone. The present study considers the model of welded connections of K56 pipe steels with various strength characteristics of sections of welded seam and weld adjacent zone, without defects. The assessment of mechanical inhomogeneity influence on load bearing capacity of welded connections was performed by applying the finite-element modelling of its stress-strain state. The developed numerical model helps to determine and optimize the criteria of testing of full scale samples of pipe steel welded connections with regards to the implementation of local strengthening effect. The research results demonstrated that the degree of contact strengthening in welded connections with X-shape grooving is higher than in welded connections with V-shaped grooving by 8 % at similar relative thickness of soft interlayer. The suggested numerical model can be applied for detailed calculations of pipelines with regards to the mechanical inhomogeneity of its welded connections.

262 Background: Invasive urothelial bladder carcinoma is typically treated with radical cystectomy (RC) plus pelvic lymph node dissection (PLND) +/− chemotherapy. Local-regional failures (LF) following cystectomy are a significant problem. Adjuvant radiation therapy (RT) could potentially reduce LF but currently has no defined role because of previously reported morbidity. Modern RT techniques with improved normal tissue sparing have rekindled interest in adjuvant RT. Stratifying patients by differing LF risk could facilitate selection for adjuvant RT. Methods: From 1990–2008, 442 patients with urothelial bladder carcinoma were prospectively followed at the University of Pennsylvania after RC+PLND +/− chemotherapy with routine pelvic CT or MRI. Univariate and multivariate competing risk analyses identified subgroups with differing LF risk. Results: On univariate analysis, stage pT3-4, total nodes removed (<10 vs. ≥10), positive margins, positive nodes, hydronephrosis, lymphovascular invasion, and mixed histology were significant predictors of LF, while use of chemotherapy, number of positive nodes, surgical diversion type, age, gender, race, smoking history and BMI were not. Node density was a marginal predictor of LF. On multivariate analysis, only stage ≥pT3-4 and nodes removed (<10) were significant independent predictors of LF with hazard ratios of 3.17 and 2.37 respectively (p<0.01). Analysis identified 3 patient subgroups with significantly different LF risk: low-risk (pT0-2), intermediate-risk (pT3-4, ≥10 nodes), and high-risk (pT3-4, <10 nodes) with 5-year LF rates of 8%, 23%, and 42% (p <0.01). Conclusions: This study of local-regional recurrence risk factors after RC is based on the largest reported, prospectively maintained patient database with routine follow-up pelvic CT surveillance. LF after RC varies significantly among different subgroups. This risk stratification model could facilitate selection for future adjuvant radiotherapy trials.

O carcinoma extrapulmonar de pequenas células é um tumor agressivo e raro, com prognóstico sombrio e tratamento semelhante com seu homólogo pulmonar, possuem geralmente disseminação ao diagnóstico, com abordagem de acordo com sítio de localização e extensão, havendo bons resultados com uso de quimiorradioterapia. Paciente do sexo masculino, 39 anos, iniciou com quadro de obstrução nasal, lacrimejamento e turvação visual com exoftalmia, após investigação para o quadro, foi encontrado lesão sólida expansiva em região nasossinusal com efeito de massa, que, após análise imunohistoquímica concluiu tratar-se de EPSCC. Iniciado terapia com boa resposta logo no primeiro ciclo de quimioterapia, com alívio dos sintomas referidos. Apesar de raro e agressivo, o carcinoma de pequenas células apresenta similaridade ao mesmo tipo histológico pulmonar, que permite tratamento semelhante, porém, é fundamental a abordagem sobre o tema tendo em vista a otimização e individualização deste de acordo com o local de acometimento e benefícios ao paciente, em sua morbimortalidade.

Resumen
El presente artículo examina los principales alcances que asume el Presupuesto Participativo en tanto política innovadora de articulación entre Estado y sociedad en el contexto argentino reciente. Sobre la base de un abordaje focalizado en el rol de las instituciones y los actores implicados en el despliegue de nuevas formas de intervención pública y democracia local en términos de gobernanza, se busca indagar fundamentalmente dos cuestiones: i) su papel en el proceso de legitimación del poder político-institucional y mejora de la gestión municipal, y ii) su repercusión en el despliegue de un patrón de ciudadanía activa. A partir de un diseño cualitativo-cuantitativo de investigación donde se consideran distintas dimensiones y casos, se analizan los principales resultados que el instrumento genera en el plano de funcionamiento de las estructuras institucionales del gobierno local, de los estilos de gestión e implementación y por último, de las nuevas modalidades de participación y acción colectiva con impactos a nivel territorial.

26 Background: NKTR-262 is a novel intratumoral prodrug designed to promote tumor antigen release, an immune stimulatory environment, and antigen presentation. NKTR-214 increases CD8+ T cells and NK cells in the tumor microenvironment. The combination of NKTR-262 with NKTR-214 accesses innate and adaptive immunity to provide an abscopal response and systemic tumor immunity. The Phase 1b/2 REVEAL trial is enrolling. Methods: In a 3+3 Phase 1 design, pts who are relapsed/refractory to ≥1 CPI receive escalating doses of intratumoral (IT) NKTR-262 followed by combined q3w treatment of IT NKTR-262 and fixed-dose intravenous (IV) NKTR-214 (0.006 mg/kg). Phase 1 will evaluate PK, safety, efficacy, and determine recommended Phase 2 dose. Scans are conducted every 9 wks (± 1 wk). Peripheral blood (PB) and tumor tissue are collected to measure biomarkers of immune activation. In Phase 2 expansion, NKTR-262 + NKTR-214 ± IV nivolumab (360 mg) will be assessed in specific tumor cohorts of IO-naïve and experienced pts. Results: As of 06 Nov 2018, 11 pts had received at least one cycle of NKTR-262 + NKTR-214 therapy. 7/11 pts were efficacy evaluable with at least one on-treatment scan (4 MEL, 2 SARC, 1 CRC). 2/4 R/R melanoma pts had uPRs with 100% (scan 2) and 40% (scan 1) reductions in target lesions per RECIST 1.1, respectively. Both patients remain on treatment. Two SARC pts had stable disease at scan 1. No pts experienced G3 or higher TRAEs or immune-related adverse events (AEs). Most common G1-2 AEs were transient flu-like symptoms. Gene expression analysis of tumor and PB show dose-dependent induction (4 to 16-fold) of TLR target genes. Consistent with the mechanism of action for NKTR-214, PB flow cytometry shows an average 13-fold increase from baseline in Ki67+ CD8+T cells in 6 pts. Conclusions: NKTR-262 + NKTR-214 engage the immune activation cascade from local tumor antigen production to anti-tumor T cell response. Initial dose levels of NKTR-262 with fixed-dose NKTR-214 were well-tolerated with early evidence of clinical activity. These data support continued evaluation of the combination +/- nivolumab. BackgroundTumor cell death caused by radiation therapy (RT) triggers antitumor immunity in part because dying cells release adjuvant factors that amplify and sustain dendritic cell and T cell responses. We previously demonstrated that bempegaldesleukin (BEMPEG: NKTR-214, an immunostimulatory IL-2 cytokine prodrug) significantly enhanced the antitumor efficacy of RT through a T cell-dependent mechanism. Because RT can induce either immunogenic or tolerogenic cell death, depending on various factors (radiation dose, cell cycle phase), we hypothesized that providing a specific immunogenic adjuvant, like intratumoral therapy with a novel toll-like receptor (TLR) 7/8 agonist, NKTR-262, would improve systemic tumor-specific responses through the activation of local innate immunity. Therefore, we evaluated whether intratumoral NKTR-262 combined with systemic BEMPEG treatment would elicit improved tumor-specific immunity and survival compared with RT combined with BEMPEG.MethodsTumor-bearing mice (CT26; EMT6) received BEMPEG (0.8 mg/kg; intravenously), RT (12 Gy × 1), and/or intratumoral NKTR-262 (0.5 mg/kg). Flow cytometry was used to evaluate CD4+ and CD8+ T cell responses in the blood and tumor 7 days post-treatment. The contribution of specific immune subsets was determined by depletion of CD4+, CD8+, or NK cells. CD8+ T cell cytolytic activity was determined by an in vitro CTL assay. Data are representative of 1–2 independent experiments (n=5–14/group) and statistical significance was determined by 1-way analysis of variance (ANOVA) or repeated measures ANOVA (p value cut-off of 0.05).ResultsBEMPEG+NKTR-262 significantly improved survival compared with BEMPEG+RT in a CD8+ T cell-dependent manner. Response to BEMPEG+NKTR-262 was characterized by a significant expansion of activated CD8+ T cells (GzmA+; Ki-67+; ICOS+; PD-1+) in the blood, which correlated with reduced tumor size (p<0.05). In the tumor, BEMPEG+NKTR-262 induced higher frequencies of GzmA+ CD8+ T cells exhibiting reduced expression of suppressive molecules (PD-1+), compared with BEMPEG+RT (p<0.05). Further, BEMPEG+NKTR-262 treatment induced greater tumor-specific CD8+ T cell cytolytic function than BEMPEG+RT.ConclusionsBEMPEG+NKTR-262 therapy elicited more robust expansion of activated CD8+ T cells compared with BEMPEG+RT, suggesting that intratumoral TLR stimulation provides superior antigen presentation and costimulatory activity compared with RT. A clinical trial of BEMPEG+NKTR-262 for patients with metastatic solid tumors is in progress (NCT03435640).

Partitioning within-species genetic diversity is fundamental to conservation of the bioheritage, current viability, and evolutionary potential of individual taxa. We conducted a hierarchical analysis of genetic diversity in Dolly Varden (Salvelinus malma) and bull trout (Salvelinus confluentus) involving analysis of hybrid zones between Dolly Varden and bull trout, analysis of phylogenetic structure within species across their native ranges using mitochondrial DNA, and a microsatellite DNA survey of population subdivision of bull trout within single watersheds. Our analyses documented hybridization and some introgression between Dolly Varden and bull trout across a geographically widespread zone of secondary contact between the two species. Both species were subdivided into two major mtDNA lineages, and one lineage in Dolly Varden may have arisen through introgression with bull trout. Bull trout have low levels of microsatellite diversity within populations, but there was substantial interpopulation variation in allele frequencies. Allele frequency distributions suggested that recent, severe bottlenecks occur frequently in bull trout populations. Our results illustrate partitioning of genetic variation at distinct levels of biological organization (species, phylogeographic lineages, local populations), and we address how such nested variation is fundamental to conservation of biodiversity.

O presente estudo, que compõe o projeto maior de pesquisa intitulado “ INCLUSÃO SOCIAL e DESENVOLVIMENTO LOCAL NO MS: Estratégias e recursos de inovação e tecnologia nos processos de inclusão social em Campo Grande, MS.”, integrando a linha de pesquisa ‘’Função social do direito e interculturalidade’’ através da subárea ‘’Direito e mudanças sociais’’, tem como foco demonstrar que a exclusão no ambiente escolar não é apenas contra os deficientes, e sim contra um enorme grupo do que sofre estigmas de exclusão social. A pesquisa demonstra que nos dias de hoje a exclusão é um debate sensível, porém necessário de ser feito, e porque não no ambiente escolar, onde a criança e o jovem são livres para dialogar, conhecer, aprender e ensinar, na educação temos o futuro. Este estudo se caracteriza como pesquisa etnográfica, com base nos estudos de descriminação e a importância de se dialogar sobre esses assuntos dentro de sala, na tentativa de alcançar uma sociedade com menos exclusão social. O foco primordial desse artigo é trabalhar essa exclusão, demonstrar como o papel do professor é fundamental, na tentativa de alcançarmos uma sociedade futura menos desigual, apresentar ao leitor como as diferenças nos ensinam e nos ajudam.

In space-like separated experiments and other scenarios where multiple parties share a classical common cause but no cause-effect relations, quantum theory allows a variety of nonsignaling resources which are useful for distributed quantum information processing. These include quantum states, nonlocal boxes, steering assemblages, teleportages, channel steering assemblages, and so on. Such resources are often studied using nonlocal games, semiquantum games, entanglement-witnesses, teleportation experiments, and similar tasks. We introduce a unifying framework which subsumes the full range of nonsignaling resources, as well as the games and experiments which probe them, into a common resource theory: that of local operations and shared randomness (LOSR). Crucially, we allow these LOSR operations to locally change the type of a resource, so that players can convert resources of any type into resources of any other type, and in particular into strategies for the specific type of game they are playing. We then prove several theorems relating resources and games of different types. These theorems generalize a number of seminal results from the literature, and can be applied to lessen the assumptions needed to characterize the nonclassicality of resources. As just one example, we prove that semiquantum games are able to perfectly characterize the LOSR nonclassicality of every resource of any type (not just quantum states, as was previously shown). As a consequence, we show that any resource can be characterized in a measurement-device-independent manner.

An analysis of published data on the possibilities of the positron emission tomography method application in complex diagnostic and treatment activities in metastatic breast cancer was performed. The possibilities are described and the method characteristics according to series of studies, in particular with regard to the various localization metastases identification are given. It is noted that, compared with conventional imaging techniques, positron emission tomography is more sensitive and specific method of distant metastases diagnosis. The results of clinical studies on assessment of the method application possibilities in the detection of breast cancer metastases in lymph nodes, various organs and bone metastases are given. It has been shown that the introduction of sodium fluoride into practice allowed to use this agent as bone tissue biomarker, which can be used in positron emission tomography performing. The data that the simultaneous use of positron emission tomography with 18F-fluorodeoxyglucose and computed tomography has a high diagnostic value in relation to the breast cancer distant metastases detection are presented. The method possibilities for the various forms of primary invasive breast cancer identification, including invasive ductal and lobular cancer are considered. The prospects analysis for a further increase of opportunities for positron emission tomography application is performed, in particular, reports on the possibility of so-called hybrid scanning using positron emission tomography and magnetic resonance imaging are considered. It is highlighted that the most important aim of the method use in clinical practice in the prospect may be local-regional recurrence and distant metastasis monitoring after primary treatment for breast cancer.

El texto apareció en un legajo de papeles del año 1961 que fue guardado en un fólder dentro de un expediente con 5 hojas normales de copia.

Veinte años de intervención internacional para luchar contra el terrorismo, contra los talibanes y democratizar y reconstruir las instituciones de Afganistán acabaron en drama humanitario en agosto de 2021. Desde septiembre, los talibanes gobiernan el país con una interpretación estricta de la ley islámica. Muchos afganos sufren represalias y se cuentan por miles los refugiados. Dos contradicciones han marcado decisivamente la misión internacional en Afganistán: a) que el despliegue de más recursos militares, económicos y políticos para acelerar los resultados produce a menudo efectos contraproducentes; y b) que sin el compromiso y el respaldo de la población local, o sin la capacidad de las fuerzas internacionales de acercarse y entender el contexto local, las misiones implementadas de arriba-abajo (top-down) provocan la fricción, la alienación, el agotamiento y el rechazo de la población.

Residual stress distribution in multilayer semiconductor structure is complicated and has a significant impact on device characteristics and yield, therefore their study is one of the actual tasks of modern device engineering. Purpose of the present work was to develop methods of estimation of actual residual stress distribution at the whole area of semiconductor structure and its elements as well.The estimation of residual stress distribution at the area of semiconductor structure was carried out on the basis of determining of local deformation of some areas of the structure by Makyoh topography. This method is based on consequent measurements of intensity of Makyoh image elements of the structure along the chosen direction followed by calculation of micro-geometrical profile and curvature radius.The estimation of residual stress of topological elements Si–SiO system was carried out by means of calculation of interference pictures obtained in a film-substrate gap after separating of film edge from substrate along open window perimeter.Analytical expressions relating semiconductor structure image characteristics with their deformation were developed by means of finite elements method. The expressions allow determining of local residual stress of chosen area of the structure. The examples of stress calculations in real structures are given.Proposed residual stress calculation methods allow to take into consideration character and curvature form of substrate, and also to estimate their magnitude in real topological elements of semiconductor circuits.

Este estudo objetiva analisar a possibilidade da implantação do Turismo de Base Comunitária (TBC) em Terra Caída, comunidade localizada em Indiaroba, Sergipe - Brasil, e que possui atratividade natural e cultural. A metodologia utilizou aplicação de questionário semiestruturado aos atores locais e às lideranças comunitárias diretamente envolvidas com a atividade turística. A definição dos sujeitos de pesquisa se deu por meio da técnica bola de neve, utilizada para amostragem não probabilística, que consiste em um indivíduo identificar outro de acordo com o perfil buscado, até chegar nas mesmas pessoas e recolher informações semelhantes. Os dados permitiram caracterizar sociodemograficamente a comunidade, descrever a organização comunitária, analisar as percepções sobre o turismo e sobre o potencial turístico local e identificar as fragilidades para a implantação do TBC. Concluiu-se que a comunidade dispõe de potencial para o desenvolvimento do TBC, contudo há entraves, principalmente quanto à limitação da infraestrutura básica e à organização comunitária.

This research aims to elaborate the characteristics of the Qur’anic manuscript of Dolah Bakri (MADB) by using philology and historical analysis. In its chronicle history, it shows that MADB appeared in Wonolelo in the 19th century. The use of paper, the technique of writing, the layout and illumination show its character as a mushaf born with the nuances of Javanese locality. In addition, when his contemporaries used mixed rasm, ‘Uthma}ni} and imla}’i}, MADB consistently used the rasm ‘uthma}ni}. From the social context, in Wonolelo, MADB was not merely as a Qur’anic mushaf which is used as a medium of reading for local Muslim community, but it had social and economic function as well. MADB could be a symbol of an economic power of its owner. MADB could also be a symbol of the integration of Islamic movement and social class identity of economic society.

Abstract Over the past decade, a multitude of research has sought to understand the complexity and role of the reproductive microbiome as it pertains to fertility. Previously, the reproductive microbiome was evaluated using culture-dependent methods; however, recent advancements in culture-independent, 16S rRNA gene amplicon community sequencing have vastly expanded our understanding of the reproductive tract microbiome. Early sequencing studies sought to compare the vaginal microbiome of cattle to the vaginal microbiome of healthy women, which predominantly consists of bacteria in the genus Lactobacillus and believed to be an indicator of fertility. In the vagina of beef cattle, however, there are incredibly low abundances of Lactobacillus and a greater diversity of bacterial species present. Beta-diversity, which examine differences in bacterial communities between samples, does not appear to differ in the vagina between unbred, open, or pregnant cattle. In postpartum beef cattle just prior to breeding, there are greater levels of diversity and increased bacterial species richness in the vagina compared to the uterus. Research on bacterial species within the uterus have primarily focused on pathogenic bacteria in postpartum cattle diagnosed with uterine disease. Fewer studies have investigated uterine bacterial species in presumed healthy postpartum beef cattle and the subsequent effects on fertility outcomes (e.g., pregnant vs. open at day 30). When evaluating the uterine microbiome during an industry standard estrus synchronization protocol, bacterial community abundance and diversity reduce over time regardless of resulting fertility outcomes. The greatest difference in uterine bacterial abundance between resulting pregnant and non-pregnant cattle appears to occur just prior to breeding. Numerous mechanisms could be contributing to the fluctuations in the uterine microbiome in beef cattle including circulating hormone concentrations or local immunoregulation. This presentation will focus on recent research investigating potential mechanisms that may alter the reproductive microbiome and ultimately impact fertility.

The origin of the Saint-Joseph University of Beirut, or U.S.J., dates back to the Seminar of Ghazir founded by the Jesuit Fathers in 1843. The College of Ghazir, established with the intention of training the local Maronite clergy, was transferred to Beirut in 1875. This center for higher studies was named Saint-Joseph University. Pope Leo XIII, in his audience of February 25, 1881, confers the title of pontificalUniversityontheU.S.J. The present article deals with the historical context of the U.S.J., the firstgreatFrench-speaking Jesuit institution in the area which, at the time, bore the name of “Syria.” We studied the historical context for the creation of the Saint-Joseph University of Beirut, located in a province of the Ottoman Empire coveted by the future mandatory power, France. This paper intends to show that after decades of conflict,Catholicsandrepublicansin the empire ultimately buried many of their disagreements by embracing a notion of French civilization that melded both Catholic and republican ideals. Focusing on the much-neglected intersection of politics, religion, and imperialism, it offers a new understanding of both the nature of French culture and politics at the findesiècle, as well as the power of the colonial experience to reshape European‘s most profound beliefs.

The issue of the administrative system in the General Government has not yet been sufficiently studied and developed. This article is of a scientific and research nature. The author presents the structure, competences and staff of the Department of Civil Engineering, one of the main departments of administration of the city of Lublin, the capital city of the Lublin district in the years 1939–1944. The aim of the research was also to show that the entire administrative apparatus in Lublin functioned under the full supervision of the German authorities and was obliged to follow the instructions of the occupier. The issue of territorial and local administration in the area of the General Government definitely requires further research. There are works referring to individual county and municipal administration bodies, but there is a noticeable lack of synthesis taking into account all structural and organizational aspects of administration and containing an assessment of its functioning. The main source of information are archival materials available at the State Archives in Lublin (Files of the City of Lublin 1939–1944). In addition to the characteristics of the Department of Civil Engineering, the article also describes the basics of the administrative system in the General Government and the Lublin district.

Acute pancreatitis (AP) is the most common gastrointestinal indication requiring hospitalisation. Severe hypertriglyceridaemia (HTG) is the third most common aetiology of AP (HTGAP), with a complication rate and severity that are higher than those of other aetiologies (non-HTGAP). The aim of this study was to evaluate the supposedly higher complication rate of HTGAP compared to non-HTGAP. The secondary objectives were to find different biomarkers for predicting a severe form. This was a retrospective study that included patients admitted with AP in a tertiary department of gastroenterology and hepatology. The patients were divided into two groups: HTGAP and non-HTGAP. We searched for differences regarding age, gender, the presence of diabetes mellitus (DM), the severity of the disease, the types of complications and predictive biomarkers for severity, hospital stay and mortality. A total of 262 patients were included, and 11% (30/262) of the patients had HTGAP. The mean ages were 44.4 ± 9.2 in the HTGAP group and 58.2 ± 17.1 in the non-HTGAP group, p < 0.0001. Male gender was predominant in both groups, at 76% (23/30) in the HTGAP group vs. 54% (126/232) in non-HTGAP, p = 0.02; 53% (16/30) presented with DM vs. 18% (42/232), p < 0.0001. The patients with HTG presented higher CRP 48 h after admission: 207 mg/dL ± 3 mg/dL vs. non-HTGAP 103 mg/dL ± 107 mg/dL, p < 0.0001. Among the patients with HTGAP, there were 60% (18/30) with moderately severe forms vs. 30% (71/232), p = 0.001, and 16% (5/30) SAP vs. 11% (27/232) in non-HTGAP, p = 0.4 Among the predictive markers, only haematocrit (HT) and blood urea nitrogen (BUN) had AUCs > 0.8. According to a multiple regression analysis, only BUN 48 h was independently associated with the development of SAP (p = 0.05). Diabetes mellitus increased the risk of developing severe acute pancreatitis (OR: 1.3; 95% CI: 0.1963–9.7682; p = 0.7). In our cohort, HTGAP more frequently had local complications compared with non-HTGAP. A more severe inflammatory syndrome seemed to be associated with this aetiology; the best predictive markers for complicated forms of HTGAP were BUN 48 h and HT 48 h.

This study was conducted to analysis the effect of levels of white kabesak (Acacia leucophloea Roxb.) Willd. leaves in the diet on feed intake, digestibility and body weight gain of Kacang goats. The completely randomized block design using 5 treatments and 5 replications was used in this experiment. The treatments were 5 concentrate feeds containing different levels of white kabesak leaves i.e. 0, 10, 20, 30 and 40% in the dry matter (DM) basis (representing T0, T1, T2, T3 and T4 treatments, respectively). The feeds were set up to contain 11.5-12.5% of crude protein (CP). Concentrate feed and native grass was fed at ratio of 60 : 40 was fed to 5 local male goats (age 1-1.5 years old and initial weight of 16.7±5.0 kg). The results showed that DM, organic matter (OM), and CP intake of T0 was not significantly different from those of T1 goats, but it was significantly higher (P<0.05) than those of T2, T3 and T4. The DM, OM, CP, and crude fiber (CF) digestibility as well as body weight gain in T2 goats were significantly higher (P<0.05) and had feed conversion ratio that was significantly better (P<0.05) compared to those of the other feed treatments. In conclusion, the most optimum level of white kabesak (Acacia leucophloea Roxb.) Willd. leaves used in concentrate feed for goat was 20 %.

Background: During pregnancy, pregnant women are susceptible to malaria, contributing significantly to maternal and infant mortality. Objective: This research was conducted to study the effect of Plasmodium berghei infection in pregnant mice on fetal growth retardation through placental cell apoptosis and the change of local vascularization. Methods: Eighteen pregnant Balb/c strain mice resulting from simultanously mating were divided into two groups those were nine pregnant mice used as non infected group and nine pregnant mice infected with Plasmodium berghei on day 9th post mating used as infected group respectively. On day 15th of post mating, all of the pregnant mice were killed. Fetal weights were measured using analytic balance. Apoptosis of placental cells and VEGF expression in the placental tissue were measured using immunohistochemistry. Results: Result showed that there was sequestration of parasite-infected red blood cells (PRBCs) in intervillous space. Statistical analysis showed that the fetal weights in infected pregnant mice group was significantly lower than non infected one (p = 0.01), and the placental cell apoptosis in placental tissue of infected pregnant mice was significantly higher than the non infected one (p=0.00).There was also a significant difference on VEGF expression between infected group and non infected group (p= 0,00). Conclusion: Plasmodium berghei infection in pregnant Balb/c mice can cause fetal growth retardation due to high of placental cell apoptosis and low VEGF expression.

Efficiency of resource allocation in agriculture depends on the functioning of commodity markets. Although the larger markets that are better connected with the transport and communication network are expected to be well-integrated, the same cannot be said about the smaller, more remote markets. This paper tests integration of agricultural commodity markets in Southeastern Punjab. The region is located off the main trading axis of Pakistan, the Peshawar-Karachi highway, and is mostly served by relatively small markets known as mandis. This study focuses on markets for cotton, wheat, and rice in five towns in the region. Cotton and wheat are the main crops in the area while rice is mostly grown as part of crop rotation aimed at controlling salinity. The analytical framework developed by Ravallion was used to conduct tests of market integration for the three selected commodities. Within this framework, it is possible to test for short-run integration, long-run integration or complete market segmentation. The results indicate that, generally, markets are integrated only in the long run, with short-run integration limited to some special cases. Moreover, the smaller markets are more likely to be isolated as compared to the larger markets. The small markets also take longer to fully adjust to the price shock originating from a more dominant central market. Finally, in the case of rice, it is more likely that a market would be isolated if it were small. This implies that farmers’ incentives to grow rice as a means of combating salinity may be constrained by local demand conditions.

The issuance of Law No. 1 of 2022 as a complement to Law No. 23 of 2004 concerning financial relations between the central and regional governments. Fiscal transfer policies are becoming more selective in seeing local needs. However, this needs to be supported by good fiscal management in the regions to carry out development activities that have an impact on increasing income that is more evenly distributed between regions and communities. One of the important instruments of fiscal decentralization is the management of government spending. Therefore, the purpose of this study is to examine the relationship between government spending and per capita income in Southeast Sulawesi. The units of observation analyzed are gross regional domestic product per capita in the previous year, personnel expenditures, goods and services expenditures, capital expenditures, HDI, district and city dummy variables. The model approach in the calculation is panel data regression analysis from 2016-2020. Based on the test results, the probability (F-statistics) is obtained, the equation of income per capita the Hausman test > 0.05, then the random effect model (REM) equation is the best model choice. The results of the analysis show all independent variables have a strong relationship with the dependent variable in the model which is shown by an R-square of 0.982. Variables that show significant effect on the increase in percapita income are (𝐺𝑅𝐷𝑃𝑝𝑟𝑘(𝑡−1)), goods and services expenditure, capital expenditure and HDI, while personnel expenditure has a negative effect on per capita income for both districts and cities in Southeast Sulawesi Province.

Os atributos que apresentaram as melhores avaliações, na percepção dos consumidores, foram: mercadoria, layout e atendimento, indicando que o local possui uma grande variedade de mercadorias, diferencial no atendimento e um boa organização e exposição dos produtos.

In order to identify the impact of changes that have been introduced as a result of decentralization, a study was conducted on the effectiveness of the formation of the revenue side of local budgets in the period before and after the beginning of local government reform. To conduct a comprehensive analysis of changes in the definition of revenues and expenditures of local budgets, the level of the ratio of expenditures made from the local budget to tax revenues has been established. There was a significant increase in the values of the indicator, which means a decrease in the level of tax capacity of local budgets. In the context of the analysis of changes in the composition and structure of local budget revenues, an increase in the differentiation of revenues to general and special funds almost doubled in the period after the changes to the budget and tax legislation. This is evidence of the growing social orientation of budgetary resources and the lack of their focus on economic development of administrative-territorial units of lower levels. The role of basic taxes in ensuring the capacity of territorial communities is determined. The transition to the use of subventions as transfers to local budgets has been noted, which indicates a decrease in the possibility for local governments to independently resolve the issue of directing funds from the provided transfers to the relevant areas. The expediency of determining the relationship of such factors as the number of inhabitants of the community and the level of its financial capacity in connection with the creation at the first stage of formation of united territorial communities as administrative-territorial units with up to 2 thousand people, and and communities with a population of 20 thousand people. As a result, communities with larger populations were found to have greater financial potential. The most effective is the formation of communities with a number of 10-25 thousand people. The reason for the formation of patterns between the number of inhabitants and financial capacity is determined, first, the presence of greater opportunities for business development, the availability of labor resources and the market for products; second, greater potential for sustainable development and infrastructure maintenance and development opportunities in large communities.

The purpose of the study is to present the methodological fundamentals of the development of cultural and educational environment of the city within primary education. We proved the necessity of revising training techniques for teaching culture under conditions of higher education. We explained top priority of orientation towards humanitarian, anthropological and axiological approaches to organization of mastering local lore at school and university. The cultural and educational environment of the city, i.e. the place, where a person was born, is considered as a tool for the interiorization of universal human values. The study gave reasons for importance of involving children in local lore activities at primary school age. Local lore is considered as a scientific basis for designing the content of school subjects; as a cultural training, activity aimed at learning about the heritage of one’s native city, preserving and enhancing the cultural level of the region, its inner potential; as a social activity aimed at developing civic mindedness. The article determined the possibilities of using local lore to teach moral and spiritual values to primary schoolchildren. The study described experience of creating and implementing a project-based research-based master’s degree programme at Saratov State University, aimed at training primary school teachers to use local lore materials in the educational process at school. The article reveals the structural and content components of the educational programme, the main forms and teaching methods. The article outlines the topics for student projects in the field of literary, linguistic, mathematical, and ecological local lore. We presented various methodological forms of incorporating local material in the class and extracurricular activities of younger students: compilation of interactive educational resources (books, reference books, albums, presentations, didactic problem books), preparation of quest games, virtual excursions, subject and intersubject projects. The article gives examples of cross-curricular competitions, mathematical problems with regional content. The priority area is the immersion of students (both students and schoolchildren) in the cultural environment of the city, region.

AimsGiant cell tumour of bone (GCTB) is a locally aggressive lesion that is difficult to treat as salvaging the joint can be associated with a high rate of local recurrence (LR). We evaluated the risk factors for tumour relapse after treatment of a GCTB of the limbs.MethodsA total of 354 consecutive patients with a GCTB underwent joint salvage by curettage and reconstruction with bone graft and/or cement or en bloc resection. Patient, tumour, and treatment factors were analyzed for their impact on LR. Patients treated with denosumab were excluded.ResultsThere were 53 LRs (15%) at a mean 30.5 months (5 to 116). LR was higher after curettage (18.4%) than after resection (4.6%; p = 0.008). Neither pathological fracture (p = 0.240), Campanacci grade (p = 0.734), soft-tissue extension (p = 0.297), or tumour size (p = 0.872) affected the risk of recurrence. Joint salvage was possible in 74% of patients overall (262/354), and 98% after curettage alone (262/267). Of 49 patients with LR after curettage, 44 (90%) underwent repeated curettage and joint salvage. For patients treated by curettage, only age less than 30 years (p = 0.042) and location in the distal radius (p = 0.043) predicted higher LR. The rate of LR did not differ whether cement or bone graft was used (p = 0.753), but may have been reduced by the use of hydrogen peroxide (p = 0.069). Complications occurred in 15.3% of cases (54/354) and did not differ by treatment.ConclusionMost patients with a GCTB can undergo successful joint salvage by aggressive curettage, even in the presence of a soft-tissue mass, pathological fracture, or a large lesion, with an 18.4% risk of local recurrence. However, 90% of local relapses after curettage can be treated by repeat joint salvage. Maximizing joint salvage is important to optimize long-term function since most patients with a GCTB are young adults. Younger patients and those with distal radius tumours treated with joint-sparing procedures have a higher rate of local relapse and may require more aggressive treatment and closer follow-up.Cite this article: Bone Joint J 2023;105-B(5):559–567.

Resumen
This paper deals with the geopotential approach to investigate the present Brazilian Height System (BHS). Geopotential numbers are derived from Global Positioning System (GPS) satellite surveying and disturbing potential on selected benchmarks. A model for the disturbing potential can be obtained by an existing set of spherical harmonic coefficients such as the Earth Gravity Model 2008 (EGM08). The approach provides absolute evaluation of local normal geopotential numbers (aka spheropotential numbers) related to a so-called World Height System (WHS). To test the validity of the proposed methodology, a numerical experiment was carried out related to a test region in Southern Brazil. The accuracy of the derived geopotential numbers was tested versus local normal geopotential numbers based on 262 GPS/leveling points. The root mean square error (RMSE) value for metric offset of BHS derived from geopotential numbers and the disturbing potential modeling in the test area was estimated to be near 0.224 meters in the absolute view. Therefore, since these spheropotential numbers are referred to a local datum, these results of comparisons may be an indicator of the mean bias of local network due to the effect of local Sea Surface Topography (SSTop) and possible offset between the unknown reference for the BHS and the quasigeoid model in the region.
