♦ Complex regional pain syndrome (CRPS) is a disabling chronic pain condition of unknown aetiology♦ Traditionally it was thought to be rare; however, prospective studies demonstrate it to be common following both trauma and operative procedures involving the upper and lower limbs♦ The condition is usually self-limiting over a maximum period of 2 years, although minor abnormalities may remain♦ In a minority of cases it does not resolve and is responsible for severe chronic disability♦ Treatment is aimed at functional restoration of limb function supported by pharmacological intervention.

Over 45,000 soldiers became amputees during the Civil War. The scale of wartime disability challenged American beliefs that masculinity and patriotism were virtually inseparable from able-bodiedness. By the war’s end, the amputated limb had become a recurring subject in music, photography, and literature. This essay discusses representative Civil War era songs about amputee veterans and analyzes how they musically and lyrically negotiated the cultural scripts of disability, masculinity, and patriotism. These scripts became especially complex when able-bodied women performed the songs in the voices of disabled veterans. Three songs are discussed in detail, including the song “Old Arm, Good Bye,” in which a soldier sings a love ballad to his freshly amputated arm, thanking the arm for its strength and loyalty to the Union. These songs reconstructed the disabled veteran, and indeed the amputated arm itself, as complex symbols of both patriotism and Victorian masculinity.

Diabetes affects a large and growing percentage of the population in most countries of the world. Diabetes causes many different health problems, but among the most severe and disabling is peripheral neuropathy. This progressive, often painful nerve condition causes suffering and disability and also predisposes patients to developing musculoskeletal deformities and foot ulcers that may threaten life and limb. This chapter reviews briefly the significance of this condition, the underlying pathophysiology, and surgical considerations. Surgical decompression is a possible treatment for this neuropathy and may help prevent disastrous complications of diabetic peripheral neuropathy. However, foot surgery in the setting of diabetic peripheral neuropathy also carries significant risks.

Smith-Lemli-Opitz syndrome (SLOS) is characterized by prenatal and postnatal growth retardation, characteristic face, genital and distal limb anomalies, and intellectual disability. There is a weak correlation between the metabolic disturbances with clinical severity and with genotype, but wide ranges exist within each group. SLOS is infrequently described in adults. The associated malformations are usually treated in infancy and childhood and have only limited influences in adulthood. Main physical problems are scoliosis and pectus formation, sun sensitivity, and disturbed hearing and vision. Rarely adrenal insufficiency is present. Marked cognitive impairment and behavioral problems including self-harm and aggressive outbursts may have a significant impact on quality of life. Cholesterol supplementation decreases sun sensitivity, but cognition and behavior seem to respond less well.

Soft tissue injuries of the hand are common and may present variously to therapists, general practice, emergency departments, or surgeons. Simple injuries can be treated without specialist input, but the majority of hand trauma should be referred to dedicated hand surgery centres. Diagnostic error and delayed specialist treatment may lead to poor outcomes and long-term disability. This chapter provides a detailed overview for non-specialists, as well as an aide memoire for hand surgeons and hand therapists. It includes the principles of general wound care, the management of nail injuries, the treatment of extravasation and high-pressure injection injuries, and gunshot wounds. We describe different levels of upper limb amputation, as well as microvascular reconstruction (both replantation and revascularization). We also cover thermal injuries (burns and frostbite), trauma-induced cold sensitivity, and factitious injury.

This chapter is a brief overview of the major sympathetic pain syndromes and their clinical characteristics, treatment, and preventative measures. It offers a concise overview of distinguishing characteristics, pathogenetic mechanisms, available treatment options for major sympathetic pain syndromes, and in-depth discussion pertaining to complex regional pain syndrome (CRPS) and the influence of specific surgical procedures on the development of this syndrome. Risk factors and pathogenetic mechanisms related to the emergence of CRPS after orthopedic and spine surgeries have been analyzed, as well as therapies and practices used preemptively to prevent new CRPS and worsening of chronic pain and disability in the affected limb. Finally, possible therapies for the postsurgical period are discussed to facilitate rehabilitation and speed up recovery after orthopedic and spinal surgeries in this patient population.

Abstract: The implications of functional loss following cancer is an area of psychosocial oncology that is rarely ventured. This is especially true in the context of limb and sensory losses, which have important repercussions on the patient’s well-being, namely as the individual is required to reassess and redefine his or her identity in face of these adversities. This chapter explores the implications of these losses via the intersection of the World Health Organization’s International Classification of Functioning, Disability, and Health with key oncological attributes of body image disturbances that seek to render explicit psychological mechanisms underlying impairments, limitations, and restrictions. Emphasizing the use of a standard framework for the assessment of functioning is essential, especially in understudied areas. Through this perspective, further insight is provided for the methodological and biopsychosocial assessment of functioning and body, and implications for clinical inquiry and practice are proposed for the advancement of cancer survivor care.

Antebellum Americans confronted anxieties about many issues, such as industrialization, immigration, and urbanization, that found expression in blackface minstrelsy and freak shows. In these performances, racial fears, gender worries, and the insecurities of an emergent working class combined with the specter of disability to assuage the concerns of white, working-class audiences partly by reinforcing whiteness, masculinity, and nondisability as markers of citizenship. From the “laughable limp” of an elderly, enslaved groom who inspired Thomas “Daddy” Rice to craft his infamous Jim Crow character to displays of the supposedly 161-year-old disabled body of Joice Heth, minstrelsy and freak shows routinely conflated race, gender, and disability on the antebellum stage. This practice reached its pinnacle with Thomas “Japanese Tommy” Dilward, one of only two black men to perform in blackface before the Civil War.

That Shakespeare adds a limp to the received characterization of Richard III is only the most conspicuous instance of his interest in how actors walked, ran, danced, and wandered. His attention to actors’ footwork, as an originating condition of performance, can be traced from Richard III through A Midsummer Night’s Dream and As You Like It into Macbeth, which is preoccupied with the topic and activity all the way to the protagonist’s melancholy conclusion that ‘Life’s but a walking shadow, a poor player | That struts and frets his hour upon the stage’. Drawing on classical and early modern accounts of how people walk and should walk, on ideas about time and prosody, and the experience of disability, this chapter cites episodes in the history of performance to show how actors, including Alleyn, Garrick, and Olivier, have worked with the opportunities to dramatize footwork that are provided by Shakespeare’s plays.
