Pelvic ring injuries can be life and limb threatening. The mechanism of injury can often be a good indicator of the type of injury; the Young & Burgess classification deploys that concept to full effect. Early identification based on mechanism of injury and improved prehospital care can play a major role in the outcome following such injuries. Pelvic ring injuries can lead to significant haemorrhage. Mechanical measures to stabilize the pelvis, in addition to modern concepts of damage control resuscitation (DCR), have been shown to be effective in early management of potentially life-threatening haemorrhage. Emphasis is now entirely on protecting the primary clot following a pelvic ring injury. Mechanical disturbance by log rolling the patient or springing of the pelvis are strongly discouraged. Early radiological clearance of the pelvis is encouraged. The lethal triad of coagulopathy, acidosis, and hypothermia should be corrected simultaneously to improve outcome. A traffic light system for monitoring venous lactate as an indicator of the patients’ physiological state can help the intensive care practitioner and the surgeon identify optimum timing for surgery. Pelvic ring injuries are associated with significant concomitant injuries. Limb trauma can also be life or limb threatening. Early identification, splinting, and resuscitation follow the same guidelines as pelvic ring injuries. Open long bone fractures should be managed by senior orthopaedic and plastic surgeons.

The world’s oceans constitute a single, interconnected planetary system, and their effective protection, therefore, demands an integrated approach. Yet, the decentralized nature of the international legal order means that the challenges for coordination are immense. Chapter 10 explains the difficulties of developing a coherent and comprehensive legal framework for the protection of the marine environment, with a particular focus on the challenges of promoting multipurpose marine protected areas in light of the proliferation of sectoral regulatory regimes. The chapter then assesses the possible mechanisms for promoting coordination between relevant treaties. Both top-down global coordination of environmental protection and bottom-up cooperation are considered. The prospects for this issue to be addressed through the ongoing negotiations on a new legally binding instrument for the conservation of marine biodiversity in areas beyond national jurisdiction are also taken into account.

Membranoproliferative glomerulonephritis (MPGN) is synonymous with mesangiocapillary glomerulonephritis and refers to light microscopic appearances of a kidney biopsy in which there are increased mesangial cells and matrix with thickening of the glomerular capillary walls, often with a double contour appearance. MPGN represents morphological appearances caused by a wide range of diseases, most of which are systemic and involve activation of the immune system. It commonly presents as nephrotic syndrome, alternatively with proteinuria, haematuria, and varying degrees of hypertension and renal dysfunction. MPGN was historically characterized into types 1–3 according to the location of immune deposits, but a more useful classification is by whether the underlying disorder results in prominent glomerular immunoglobulin deposition (with secondary complement deposited) or if there is glomerular complement, but scanty or no immunoglobulin deposited. The immunoglobulin group includes MPGN caused by infections, autoimmunity, cryoglobulinaemia, and paraprotein production. The complement group (C3 glomerulopathy) includes dense deposit disease and other complement disorders. Similar light microscopic appearances without deposition of immunoglobulin or complement are sometimes seen in patients with chronic thrombotic microangiopathies. Management and prognosis depend on aetiology.

The mechanism for maintaining balance in man is complex. Vision, proprioception, and vestibular inputs are integrated in the central nervous system, and modulated by activity from the cerebellum, the extrapyramidal system, the reticular formation, and the cortex. This integrated, modulated information provides one mechanism for control of oculomotor activity, controls posture, gait, and motor skills and allows perception of the head and body in space. Recent evidence also supports an effect upon autonomic function, cognition, and emotion. The complexity of the system is such that pathology in a variety of different bodily systems, including the endocrine system, the cardiovascular system, and the haemopoietic system, can impact upon vestibular activity, in addition to primary otological and neurological pathology.Patients with dysfunction in the vestibular end-organs or vestibular pathways commonly complain of symptoms of dizziness, vertigo, unsteadiness, light-headedness, imbalance, and a plethora of synonyms associated with a sense of instability. Not infrequently, in an attempt to define their ‘unphysiological’ experience, patients use rather vague and imprecise semantics. The clinical distinction between dizziness, a symptom of non-specific pathological significance, and vertigo, a hallucination or illusion of movement, is rarely made, although the latter is a cardinal manifestation of a disorder of the vestibular system (Dix 1973). Ten to 20 per cent of all ‘dizzy’ patients are reportedly seen in neurology clinics (Dieterish 2004), therefore it behoves the neurologist to have a clear diagnostic strategy, including knowledge of detailed neuro-otological examination, to enable appropriate diagnosis and management of the patient with vestibular symptoms.

Natural resources are often overexploited. Nevertheless, there are counterexamples of sustainably using common-pool resources. This book analyses the most important factors influencing the management of natural resources. Hence, the important question—What makes some systems successful?—is answered in this book. Based on three of the world’s largest data sets on fisheries, forest management, and irrigation systems, success factors are empirically examined. The book presents a synthesis of twenty-four success factors that explain ecological success, such as participation possibilities. The analysis in this book uses a range of statistical and machine learning methods to develop highly predictive, robust, and empirically sound models that shed new light on factors that have already been investigated. From this analysis the author develops a general model which can predict the success of in natural resource management very well, depending on the identified success factors.

Complementary and alternative medicine (CAM) therapies are a group of diverse medical and health systems, practices, and products not currently considered to be part of conventional medicine. These therapies have growing popularity and it is suggested that more than a third of patients with bipolar disorder (BD) use some form of CAM therapy. Although there are several forms of CAM therapies, including physical therapies, nutraceuticals, herbal remedies, and mindfulness-based interventions, reports in BD are few in number and often of poor quality. Sleep deprivation has the strongest evidence for benefit as an augmentation therapy in bipolar depression. There is promising evidence for other CAM therapies, including exercise, bright light therapy, omega-3 fatty acids, N-acetylcysteine, and the traditional Chinese medicine formulation, Free and Easy Wanderer Plus. However, limitations such as small number of studies, small sample sizes, few randomized investigations, and contradictory findings currently preclude definitive recommendations.

Being published as a celebration of the 60th anniversary of John von Neumann’s “Theory of Self-Reproducing Automata,” this handbook attempts to provide a unique reflection on the nature of computational economics and finance (CEF) in light of natural computationalism. We restructure CEF by including both nature-inspired computing and natural computing. This new framework allows us to have a view of CEF much broader than just the conventional algorithmic consideration. The book begins with a historical review of computational economics (CE), tracing its history far back to the era of analog computing. In these early days, advancements were mainly made using the idea of natural computing, and the subjects pursued by CE were the computing system as a whole, not just numerical computing. The handbook then is organized by distinguishing computing from computing systems. Six chapters (Chapters 2 to 7) are devoted to the former. They together present a review on the recent progresses in CE, as illustrated by the computation of rational expectations, general equilibrium, risk, and volatility. The subsequent 16 chapters are devoted to the computing-systemic view of CE, including natural-inspired computing (Chapters 8 to 12) and network, agent-based computing and neural computing (Chapters 13 to 23). In addition to providing alternative approaches to forecasting, investment strategies and risk management, etc., they enable us to have a 'natural' or more realistic description of the economy, starting from its decision makers; hence, market-design or policy-design issues involving different levels of the economy, be microscopic, mesoscopic and macroscopic, can be simultaneously addressed and coherently integrated. The handbook concludes with a chapter on what we may hope from CE by providing an in-depth review on the epistemological aspects of computation.

Ever since a democratic system of government was adopted and a strategy of planned economic development was launched in India, the planners were quite conscious of the need for an administrative system different from the colonial one to implement the planned objective of development. Kuldeep Mathur, in this volume, examines these administrative reforms and provides a magisterial account of the changes in the institutional process of public administration. The introduction of neoliberal policies revived the concerns about reform and change, thereby giving rise to a new vocabulary in the discourse of public administration. The conventional world of public administration was now expected to adopt management practices of the private sector and interact with it to achieve public policy goals. New institutions are now being layered on traditional ones, and India is becoming a recipient of managerial ideas whose efficacy has yet to be tested on Indian soil. In light of the aforementioned changes, this volume argues that hybrid architecture for delivering public goods and services has been the most significant transformation to be institutionalized in the current era and critiques the neoliberal transformation from within a mainstream public administration perspective.

The chapter describes the change-management process. As hospitals are highly complex organizations with multitudes of entities with many interactions among them, the 3–1–1 model guides managers where to focus their efforts. The International Criminal Tribunal for the former Yugoslavia (ICTY) has left a rich legal, institutional, and non-judicial legacy. Many of the classical juridical footprints of the work of the ICTY are covered in legal treatises or reflective volumes. This book sheds a broader light on some of the working practices, challenges, and critiques of the ICTY. It starts from the assumption that the notion of legacy deserves consideration from multiple disciplinary standpoints, and that there is not one, but at best multiple legacies. It explores (i) historic legacy, that is, the contribution of the ICTY to the framing of facts and events, including the value of the records in combating denial of crimes; (ii) investigative and experiential legacy, including approaches towards identifying evidence, experiences related to the delivery of testimony, and access to justice; (iii) procedural legacies, including lessons learned from the management of proceedings, the treatment of parties, and outreach; (iv) retribution and punishment; (v) normative legacies, that is, the impact of the work of the Tribunal on domestic legal systems; and (vi) ongoing debates over impact and societal reception. The book draws on insights from multiple disciplines and combines voices from inside the Tribunal with external perspectives.
