Resumen
Background:Health risk behaviors (HRB) are defined as specific forms of behavior associated with increased susceptibility to a specific disease or ill health on the basis of epidemiological or social data. The social implications of HRB include being able to use them as a way to gain respect and acceptance from peers, establishing independence from parents, or providing a subjective feeling of maturity and adequate stress management [1].Objectives:Since chronically ill adolescents such as those suffering from JIA can develop a HRB that is challenging for optimal care the aim of this study was to compare the frequency of HRB in adolescents with JIA and their peers, and to determine whether they change during the course of disease. Methods:Patients ≤ 16 years of age with recently diagnosed JIA (< 12 months) were enrolled in the inception cohort of patients with newly diagnosed JIA (ICON), an ongoing prospective observational, controlled multicenter study started in 2010. Patients from the age of 13 who were questioned about their health behavior and followed up for at least 2 years were selected. HRB were quantified and compared with those of subjects from the general population after matching for age and sex. Data from 2-year follow-up (FU) were used to analyze correlates of multiple risk behavior defined as involvement in two or more risky behaviors. Results:A total of 209 adolescents with JIA (63% female, mean age at baseline 14.4±0.9, mean disease duration 2.6±2.0) and 138 healthy peers (55% female, mean age 14.5±1.0) were included. At baseline, 51% of patients were treated with a DMARD, 21% with a biologic (FU: 59% and 38%). The most common JIA category was rheumatoid factor negative polyarthritis (28%). While at baseline 20% of patients and 4% of controls did not engage in regular physical activity, the proportion at follow-up amounted to 16% and 10%, respectively (OR 3.69; 95%CI: 1.01-13.50). In both groups the proportion of regular smokers, alcohol consumers and drug users increased during the observation period. Significant group differences were found in terms of alcohol consumption and smoking habits, but not in relation to illicit and legal drugs (see table). Patients stated significantly more often that they had not used a condom during their last sexual intercourse (28% vs. 19% controls, p<0.05). Multiple risk behavior was associated with PedsQL™ total score (OR 0.96; 95%CI: 0.92-0.99) and disease duration (OR 0.75; 95%CI: 0.57-0.98).Forms of HRBBaseline2-year follow-upJIAN=209 (%)CGN=138 (%)JIAN=209 (%)CGN=138 (%)Inactivity/smoking/alcohol consumption Physical activity <1/week20.33.716.29.5 Active ≤2/week58.943.454.442.3 Regular smokers2.40.77.82.2 Regular alcohol consumers3.83.615.823.2Drug use 12-month prevalence1.91.48.18.0 Multiple consumers0.50.04.83.6Sexual intercourse Lifetime prevalence3.31.425.223.2 No condom use during last sexual intercourse28.650.028.019.4Conclusion:Although adolescents with JIA became more physically active during the course of the disease, they are as likely, or more likely, to take risky behaviors than their healthy peers, except for alcohol consumption. In order to achieve optimal outcomes, addressing emotional wellbeing and providing mandatory anticipatory guidance appears to be warranted in this population.References:[1]Villarreal-Rodriguez D et al. Resumen
Background. Traditional Chinese medicine (TCM) for the treatment of female infertility remains ambiguous. The aim of the present case-control study was to examine the association between TCM treatment and successful pregnancy among infertile women. Methods. This population-based case-control study included the data from 2,627 infertile women with successful pregnancy and 2,627 infertile women without successful pregnancy using datasets from the Longitudinal Health Insurance Database 2000 of the National Health Insurance Research Database during 2000–2010. The odds ratios (ORs) and 95% confidence intervals (CIs) for the relationship between TCM use and successful pregnancy in infertility women were estimated using logistic regression. Results. Patients who received TCM treatment significantly increased in successful pregnancy (OR = 1.48; 95% CI = 1.31–1.66), compared with patients without TCM. Si-Wu-Tang (OR = 4.25; 95% CI = 2.18, 8.30), Gui-Zhi-Fu-Ling-Wan (OR = 3.27; 95% CI = 2.13, 5.02), and Jia-Wei-Xiao-Yao-San (OR = 3.17; 95% CI = 2.35, 4.28) were the TCM agents that were most strongly associated with successful pregnancy among infertile women. Conclusions. Our study findings indicate that TCM is associated with higher likelihood of successful pregnancy in infertile women, which is worthy of further investigation by randomized control trial.

Resumen
Aim. To examine the efficacy of a six-month Kung Fu (KF) program on physical fitness in overweight/obese adolescents.Methods. Subjects were randomly assigned to the KF or sham exercise (Tai Chi, TC) control group. Physical measurements in cardiovascular fitness and muscle fitness occurred at baseline and after 6 months of training thrice weekly.Results. Twenty subjects were recruited. One subject was lost to follow-up, although overall compliance to the training sessions was46.7±27.8%. At follow-up, the cohort improved in absolute upper (P=.002) and lower (P=.04) body strength, and upper body muscle endurance (P=.02), without group differences. KF training resulted in significantly greater improvements in submaximal cardiovascular fitness (P=.03), lower body muscle endurance (P=.28; significant 95% CI: 0.37–2.49), and upper body muscle velocity (P=.03) relative to TC training.Conclusions. This short-term KF program improved submaximal cardiovascular fitness, lower body muscle endurance, and muscle velocity, in overweight/obese adolescents with very low baseline fitness.

Resumen
Abstract Background: Anlotinib, a novel multi-target tyrosine kinase inhibitor that effectively inhibits VEGFR, PDGFR, FGFR, c-KIT, c-MET, and RET, monotherapy has been proven effective in HER-2 negative metastatic breast cancer, but its efficacy in early-stage triple-negative breast cancer (TNBC) is unknown. This phase 2 study aims to evaluate the efficacy and safety of adding anlotinib to neoadjuvant chemotherapy in patients (pts) with primary TNBC. Methods: Pts with clinical stage II/III TNBC were to be treated with 5 cycles of anlotinib (12mg, d1-14, q3w) plus 6 cycles of taxanes (docetaxel 75 mg/m2 or nab-paclitaxel 125 mg/m2, d1 and d8, q3w) and lobaplatin (30 mg/m2, d1, q3w), followed by surgery. The primary endpoint was pathological complete response (pCR) in the breast and axilla (tpCR; ypT0/is ypN0) and the secondary endpoints include pCR in the breast (bpCR; ypT0/is), event-free survival (EFS), invasive disease-free survival (iDFS), overall survival (OS), and safety. Exploratory study included biomarker analysis and efficacy comparation based on FUSCC classification (IHC-based). Results: From Jan 2021 to Feb 2022, a total of 24 pts were enrolled. The median age was 50 years (range, 26-64), 54% were postmenopausal, 75% were nodal involved, 29% had stage III, and 79% were Ki-67 high (≥30%). At the data cut off time of 30th Jun 2022, all 24 pts received at least one dose of study treatment and underwent surgery. Overall, 21 pts received five courses of anlotinib. Two pts discontinued anlotinb for safety reason, and one pt discontinued anlotinb due to missed dose in cycle 4. After surgery, 14 out of 24 pts achieved a tpCR (58.3%; 95% CI, 36.6%–77.9%), and the bpCR rate was also 58.3% (14/24). Of the 18 pts with the node-positive disease at diagnosis, 15/18 (83.3%) became ypN0. Based on the FUSCC IHC-based subtypes, the tpCR rates were 66.7% (6/9) for BLIS subtype, 80% (4/5) for IM subtype and 0% (0/4) for LAR subtype, respectively. Next-generation sequencing revealed that the most commonly mutated genes in these pts were TP53 (19/21, 90.5%), MYC (7/21, 33.3%), BRCA1 (5/21, 23.8%), PIK3CA (4/21, 19.0%), BCL2L11 (4/21, 19.0%), and RB1 (3/21, 14.3%). Subgroup analysis showed that the tpCR were 71.4% (5/7) and 42.9% (6/14) in MYC-amplified and wild-type pts, respectively, and 80% (4/5) and 43.8% (7/16) in BRCA1-mutated and wild-type pts, respectively. All of 24 pts in the safety population showed at least one treatment emergent adverse events (TEAEs). Grade 3 or 4 TEAEs occurred in 14 pts (58.3%), and the most common events were leucopenia (29.2%; n=7), neutropenia (29.2%; n=7), thrombocytopenia (20.8%; n=5), anemia (16.7%; n=4), hypertension (12.5%; n=3), and oral mucositis (8.3%; n=2), respectively. No treatment-related deaths occurred. Conclusions: The addition of anlotinib to neoadjuvant chemotherapy showed manageable toxicity and promising antitumor activity for pts with early-stage TNBC. The study is still ongoing, and the enrollment has been completed. Clinical trial information: ChiCTR2100043027. Funding: Chia Tai Tianqing Pharmaceutical Group Co., Ltd. L. Resumen
LANGUAGE NOTE | Document text in Chinese; abstract also in English. Studying Li Bai’s works and circumstances before and after his service at the Hanlin Academy, this essay observes a deep-rooted cultural echo of the two fu writers of the Han, Sima Xiangru (179 BC-117 BC) and Yang Xiong (53 BC-18 AD), as well as that of Dongfang Shuo (ca. 154 BC-93 BC), behind Li’s reminiscence of his adventure to Chang’an in the disguise of a “Banished Transcendent.” A trilogy on Li Bai’s reminiscence of his quest for transcendence, for honor, and for dreams in his Chang’an adventure may be established. This trilogy is comprised of: 1) his transcendent roaming to Mt. Tai prior to his journey to the capital, with the feng and shan rituals performed by the emperor, which metaphorically represent the poet’s talent being recognized; 2) his three “Qingping diao” ci-poems composed at the Hanlin, which start with the Western Queen Mother of the Jade Terrace and the Goddess of the Witch Mountains, as a metaphor for the poet’s talent being recognized; and 3) his writing about a dreaming journey to Mt. Tianmu as a metaphor for awakening from his Chang’an dream.

The ancient history stories about the heroes fought for power at the end of Sui Dynasty and at the beginning of Tang Dynasty had a great development among Chinese classic novels, dramas, and other forms of literature. For example, “Shuo Tang Quan Zhuan” which was collated by Yun Hu Yu Sou of Cing Dynasty (abbr. “Shuo Tang”) was very popular among people. Many stories of Folk Literature were based on its plots. When it comes to the study of “Shuo Tang”, many researchers were focus on contrasting the different plots of “Shuo Tang” and other series novels of Sui and Tang Dynasty. On this study, the researcher tries to explore the new interpretation in the stories of “Shou Tang” from digging into the stories of Folk Literature the stories. Take “Gu Ci” which was about ancient stories for example. However, due to the great amount of “Shuo Tang Gu Ci Series”, this study will concentrate on the earlier and well-preserved “Story of Qin Qiong’s Visit and Disturbance in Tai Yuan Fu with illustration” from “Periodicals of Precious Treasures of National Palace Museum”(abbr. “Qin Qion’s Disturbance in Tai Yuan Fu”). The structure of the thesis, the whole article contains five chapters. Chapter One is the Introduction which will give the general ideas of the motivation, the comments of the former results, the research methods, and interpret the basic outlines of this thesis. Chapter Two “The Structure of Gu Ci of Shuo Tang”. First, the research will concentrate on “Qin Qion’s Disturbance in Tai Yuan Fu”, which will probe into the structure, contents and relevant issues. Second, the research will explore the contents and versions of “Shuo Tang””, which will organize the materials found in “The Institute of History and Phiology” and other precious information from overseas as well. Third, construct the story structure of “Shuo Tang Gu Ci Series”, which will interpret the methods and the findings, present the structure, and probe into the meaning and value of constructing the structure of the stories. Because “Gu Ci”is the form of art which is composed of the repetition of prose and rhyme, Chapter Three will concentrate on the structure of rhyme in “Qin Qion’s Disturbance in Tai Yuan Fu”. Moreover, the researcher will devide it into three categories, poems, surtitle, and Zan and Fu, and discuss the relevant questions about the position and the funtion. Chapter Four will analysze the revision of “Shuo Tang” in “Qin Qion’s Disturbance in Tai Yuan Fu”. First, comtrast the differences between “Qin Qion’s Disturbance in Tai Yuan Fu” and “Shuo Tang”from the structure of the plots and the addition of the plots. Second, focus on the transformation of the main charactors. Third, sum up the plots, the traits of the charactors, and the revied parts and analysize the cultural phenomenon which showed in “Gu Ci”. Chapter Five, reflect on the previous chapters in order to have a deeper understanding of “Shuo Tang Gu Ci Series”.
