Indolent cutaneous B-cell lymphomas (CBCL) are a rare disease for which the therapeutic recommendations are based on clinical reports. Recommendations for solitary lesions include surgery or irradiation. However, the high relapse rates may require less invasive repeatable therapy. This study seeks to retrospectively assess the efficacy of intralesional rituximab (ILR) for indolent CBCL when compared with intravenous rituximab (IVR). Patients treated for indolent CBCL with ILR or IVR at the Division of DermatoOncology of the University Hospital Heidelberg were eligible for this study. Characteristics of lymphoma, treatment response, and adverse events were assessed. Twenty-one patients, 67% male at a median age of 52 (range 17–80), were included. Nineteen (90%) had only localized lymphoma (stage T1 and T2). Complete response was achieved in 92% (11/12) of ILR after a median of one cycle (three injections) and 78% (7/8) of IVR patients after a median of six cycles. Half of ILR patients and 78% of IVR patients showed relapse after a median of 15 and 23 months, respectively. Adverse reactions were usually mild and were limited to the first injection of ILR. One patient with IVR contracted a pulmonary infection. ILR may be an alternative to the intravenous administration of rituximab for localized indolent CBCL.

OBJECTIVE To determine the dosing regimen of intravenous ranitidine (IVR) most likely to achieve a gastric pH of ≥4 in critically ill pediatric patients. METHODS A retrospective cohort study was designed and included patients younger than 19 years with gastric pH samples taken from a nasogastric tube within 24 hours after a dose of IVR in an intensive care unit. Data collection included patient demographics, clinical variables, IVR dosing, and gastric pH samples. Descriptive statistical analysis and multivariable logistic regression analysis with clustering of patients was performed to determine variables associated with odds of obtaining a pH of ≥4. RESULTS A total of 628 patients (1356 nasogastric samples) met study criteria (median age 1.3 years [IQR, 0.33, 5.7 years]; 53% male). The IVR dose was 0.90 ± 0.30 mg/kg per dose every 8.1 ± 2.9 hours, and 60.9% of patients (n = 383) had a pH ≥4. Patients with a pH value ≥4 had gastric pH samples taken earlier after a dose of IVR (6.7 ± 5.0 vs. 5.9 ± 4.7 hours, p < 0.001) but had no difference in IVR dose per kilogram (0.88 ± 0.31 vs. 0.88 ± 0.26, p = 0.86) or frequency of dosing (7.9 ± 3.2 vs. 7.9 ± 3.2 hours, p = 0.89). A multivariable logistic regression model identified increasing age, decreased kidney function, and decreased time to pH sample after an IVR dose with significantly greater odds of pH ≥4. CONCLUSIONS The IVR dosing to maintain a gastric pH ≥4 in critically ill pediatric patients should occur more frequently than every 8 hours. Gastric pH evaluation may be necessary to assess IVR efficacy.

Resumen
Individual vocal recognition (IVR) has been well studied in mammals and birds. These studies have primarily delved into understanding IVR in specific limited contexts (e.g. parent–offspring and mate recognition) where individuals discriminate one individual from all others. However, little research has examined IVR in more socially demanding circumstances, such as when an individual discriminates all individuals in their social or familial group apart. In this review, we describe what IVR is and suggest splitting studies of IVR into two general types based on what questions they answer (IVR-singular, and IVR-multiple). We explain how we currently test for IVR, and many of the benefits and drawbacks of different methods. We address why IVR is so prevalent in the animal kingdom, and the circumstances in which it is often found. Finally, we explain current weaknesses in IVR research including temporality, specificity, and taxonomic bias, and testing paradigms, and provide some solutions to address these weaknesses. This article is part of the theme issue ‘Signal detection theory in recognition systems: from evolving models to experimental tests’.

Purpose: To compare the efficacy and safety of intravitreal ranibizumab (IVR) in monotherapy or associated with panretinal photocoagulation (PRP) versus conventional PRP, for high-risk proliferative diabetic retinopathy (PDR) without vitreoretinal traction. Procedures: Multicenter randomized trial, with 3 treatment arms: PRP versus IVR alone and PRP + IVR combined treatment. Follow-up was performed at months 3, 6 and 12. Results: Thirty-five subjects were randomized and 32 used for analysis. Complete regression of neovessels elsewhere occurred in 100% (PRP + IVR), 75% (IVR) and 69.2% (PRP) and for neovessels of the disk in 44.4% (PRP + IVR), 37.5% (IVR) and 30.8% (PRP). During the 1-year duration of treatment, there was no need for laser rescue treatment in IVR-treated eyes. Conclusions: This trial suggests that the use of IVR is safe and may have a beneficial effect in the treatment of eyes with high-risk PDR. Message: Ranibizumab appears to have a place in the treatment of PDR.

A popular application that runs on a Voice-over-IP network is the Interactive Voice Response (IVR), which provides a way for interacting with an end user's phone via a telephony interface by presenting a set of audible menu options, and collecting the user's Dual Tone Multi-Frequency responses as the user presses the telephone numeric keypad, and recording the user's voice. This thesis takes a closer look into IVR systems on an IP-based network, specifically in networks that support the Session Initiation Protocol (SIP) for controlling the telephony signalling, and more specifically on IVR systems that have their SIP signalling controlled by a SIP Application Server (SIPAS) implementing the SipServlet 1.0 or 1.1 specifications and where a SIP-based Media Server is used to stream IVR media to the end-user. We describe existing ways for supporting an IVR development in a SIP network, and then we propose an alternative way of accomplishing the same task: the IVRObject. With the help of prototypes, the IVRObject is compared with the existing state of the art against three criteria: (a) how easy it is to develop, (b) how portable the development solution is, (c) and how scalable the solution is in order to sustain a high call volume. As a conclusion, it will be highlighted that the IVRObject provides an easy mechanism for development of IVR-based applications running on a SIPAS, that it is portable to different media server vendors, and that it supports a test strategy that can be leveraged to improve software development quality and faster development. This makes the IVRObject a good alternative especially for enterprise-based IVR applications where scalability is less of an issue than in carrier-space applications.

Mobile phones and Interactive Voice Response (IVR) applications are being progressively used in developing countries to collect voice-based reports about bad governance or poor public service delivery, reported by citizens. Such systems (e.g. Avaaj Otalo, Foroba Blon, etc.) can give an opportunity to rural users in developing countries to easily influence and participate in public affairs. Despite the ongoing efforts on using such solutions, the lack of an efficient system of administration can cause delays in broadcasting the collected reports as quickly as possible, to reach the relevant authorities. This thesis presents the results of a real-world deployment of an administrative functionality for an IVR system called U-Call, used in the Northern districts of Uganda. U-Call Administrative Functionality (UCAF) interacts with the U-Call administrators through mobile phones and gives the moderator access to the registered users. It allows administrators to easily publish and tag audio reports over the Web using their mobile phones. It also uses a semantic tagging module to increase findability and information categorization on the U-Call’s website. After an initial validation and successful evaluation of UCAF in the field, during a trip to Uganda, additional features were incorporated, such as multiple authentication process and dynamic tagging. UCAF and its additional features was succefully delivered to the end user, as part of the  U-Call reporting system.

Les systèmes vocaux interactifs (en anglais Interactive Voice Response : IVR) sont omniprésents de nos jours. IVR est une technologie de téléphonie qui permet aux utilisateurs d’accéder aux services offerts par un système d’information automatisé d’une entreprise via un clavier de téléphone ou des commandes vocales. L’infonuagique (en anglais Cloud Computing) est un nouveau paradigme qui par l’entremise de serveurs permet d’héberger et de fournir des services sur l'Internet avec de nombreux avantages inhérents. Il offre trois modèles de services majeurs: Infrastructure en tant que service (en anglais Infrastructure as a Service : IaaS), Plateforme en tant que service (en anglais Platform as a Service : PaaS) et Software en tant que service (en anglais Software as a Service : SaaS). L’infornuagique est basée sur la technologie de virtualisation qui permet la coexistence de différentes entités sur le même matériel physique. Ces entités peuvent être des systèmes partageant le même équipement matériel, des applications hébergées par le même système d’exploitation ou bien des réseaux complets partageant les mêmes routeurs. Le principal objectif visé par cette technologie est le partage efficace de ressources physiques. Plusieurs applications multimédias sont offertes aujourd’hui dans les environnements infonuagiques. Cependant, au meilleur de notre connaissance, il n’existe pas d’architecture qui crée et gère les applications IVR dans de tels environnements. Par conséquent, ce mémoire propose une nouvelle architecture permettant de créer à la volée et de gérer d’une manière efficace des applications IVR dans un environnement infonuagique. Il décrit une nouvelle architecture d’une infrastructure de virtualisation d’applications IVR et démontre son potentiel à travers un cas d’utilisation. Il propose deux stratégies de gestion de ressources et d’ordonnancement de tâches comme un part essentiel au partage des ressources dans une telle architecture.

The main goal of this project was to enhance the use and usability of Interactive Voice Response Systems (IVR) for older people. The objective of study one was to examine older people’s perceptions of the technology and identify the most common difficulties experienced by older people when interactive with IVR using focus groups. Twenty-six people aged 65 and older took part in the study. Data were analyzed using frequency and chi square analyses. The results revealed negative attitudes towards the technology. Long menus, frustration about not being able to reach an operator and absence of shortcuts were some of the most common difficulties reported by participants. Study two examined the cognitive factors predicting successful IVR interaction in four commercially available IVR systems in a sample of 185 older adults. Linear regressions were performed on the data. Results indicated that working memory and auditory memory were the best predictors of successful IVR interaction. Using the same sample of participants as study two, study three examined older adults’ attitudes towards the four IVR systems in relation to their success in interacting with the technology. The study also evaluated the impact of gender on success and attitudes towards IVR. There was a significant positive correlation between success with IVR and favorable attitudes towards the technology. No gender differences emerged in both performance on IVR tasks and attitudes towards the technology. Study four evaluated the feasibility of using a voice-activated IVR to administer and score three short neuropsychological tests using a sub sample of the original sample of 185 older adults involved in study two and three. One hundred and fifty eight participants took part in the study. Results showed high correlations between the IVR and clinician scoring of the three tests. Nevertheless, a number of discrepancies and technical issues were discovered.

The OpenPhone and BGR systems are used as case studies and their designs have been influenced by field studies with the targeted users. The OpenPhone project aims to design an Interactive Voice Response (IVR) health information system that enables people who are caregivers for HIV/AIDS infected children to access relevant care-giving information by using a telephone in their native language of Setswana in Botswana, Southern Africa. The BGR system allows soccer fans to access results of recently played matches in Premier Soccer League (PSL) of South Africa.

This thesis aims to provide the thermal condition of melt pool convection by CFD simulation, which is important to the assessment of the invessel melt retention (IVR) strategy widely adopted in Generation III pressurized water reactors (PWRs). As a severe accident mitigation measure, the IVR strategy is realized through external cooling of the lower head of a reactor pressure vessel (RPV). To achieve the coolability and retention of the corium pool in the RPV lower head, the heat flux at the outer surface of the vessel should be less than the critical heat flux (CHF) of boiling around the lower head. Under such condition, the integrity of the RPV is guaranteed by the adequate thickness of the unmelted vessel wall. The thesis work starts from the selection and validation of a turbulence model in the CFD computational tool chosen (Fluent). Afterwards a numerical model is set up for estimation of melt pool heat transfer of a reference PWR with the power capacity of 1000 MWe, including a mesh sensitivity study. Based on the numerical model of a twolayer melt pool, four tasks are carried out to investigate the effects of Zr oxidation ratio, Fe content, and radiation emissivity on heat flux profiles, as well as the focus effect under extreme conditions. Selection and validation of the turbulence model are conducted by comparing the simulation results of different turbulence models with the DNS data on the convection of volumetrically heated fluid layer bounded by rigid isothermal horizontal walls at equal temperature. The internal Rayleigh numbers of the flow reach up to 10e6. The comparison shows a good agreement of the SST k-ω turbulence model results with the DNS data. The simulations with the Zr oxidation ratio of 0, 0.2 and 0.5, correspondingly, the oxide layer of 1.389m, 1.467m and 1.580m, and the metal layer of 0.705m, 0.646m and 0.561m in height, show that, the temperature of the oxide layer will increase with Zr oxidation ratio, while the temperature of the metal layer will decrease resulting in more heat transfer through the oxide layer sidewall and less top radiation. Nevertheless, the effect of the Zr oxidation ratio is not pronounced in the range of 00.5. The simulations with the Fe mass of 22t, 33t and 45t, and respective height of the metal layer of 0.462m, 0.568m and 0.646m, show that, the inner metal layer will significantly increase the temperatures of both the metal layer and the oxide layer. The percentage of heat transfer at the oxide layer sidewall will increase to supplement the reduction of that at the metal layer. The simulations with the radiation emissivity of 0.2, 0.35, 0.45 and 0.7 show that, the emissivity below 0.45 has an impact on heat transfer, and the temperatures and sidewall heat flux of both the oxide layer and the metal layer will increase with decreasing emissivity. The impact is negligible when the emissivity is above 0.45. The simulations under the hypothetically extreme conditions with either an adiabatic top boundary or a very thin metal layer show the focusing effect may occur, i.e., the heat flux through the metal sidewall is larger than that in the oxide layer. But the local high heat flux is flattened by the vessel wall with good heat conductivity. In summary, the simulations demonstrate that, except for the cases under extreme conditions, the heat fluxes of the melt pools in all other cases are significantly lower than the CHF of external cooling of the lower head. Therefore, the safety margin of the IVR strategy of the PWR chosen is seems sufficient. However, due to some limitations (e.g., simplification and assumptions) in the simulation cases and coupling of different influential factors, as indicated by the present study, the precise predictions of heat flux under all scenarios are still difficult. Therefore, the conclusions could not be generalized to the other conditions or other configurations of the molten pools. By discussing the model and simplifications/assumptions adopted in this work, the improvement directions of the numerical model and other perspectives are proposed at the end of the thesis.
Denna avhandling syftar till att tillhandahålla det termiska tillståndet för smältbassängskonvektion genom CFD-simulering, vilket är viktigt för bedömningen av IVR-strategin som allmänt antagits i tryckvattenreaktorer (PWR) i Generation III. Som en åtgärd för att mildra allvarliga olyckor realiseras IVR-strategin genom extern kylning av det nedre huvudet av ett reaktortryckkärl (RPV). För att uppnå kylbarhet och kvarhållning av koriumbassängen i det nedre RPV-huvudet bör värmeflöde vid den yttre ytan av kärlet vara mindre än det kritiska värmeflödet (CHF) som kokar runt det nedre huvudet. Under sådant tillstånd garanteras RPV: s integritet av den osmälta kärlväggens tillräckliga tjocklek. Examensarbetet startar från valet och valideringen av en turbulensmodell i det valda CFD-beräkningsverktyget (Fluent). Därefter sätts en numerisk modell upp för uppskattning av smältbassängens värmeöverföring av en referens PWR med en effektkapacitet på 1000 MWe, inklusive en nätkänslighetsstudie. Baserat på den numeriska modellen för en tvålagers smältbassäng utförs fyra uppgifter för att undersöka effekterna av Zr-oxidationsförhållande, Fe-innehåll och strålningsemissivitet på värmeflödesprofiler, liksom fokuseffekten under extrema förhållanden. Val och validering av turbulensmodellen utförs genom att jämföra simuleringsresultaten för olika turbulensmodeller med DNS-data för konvektionen av volymetriskt uppvärmt fluidskikt avgränsat av styva isoterma horisontella väggar vid lika temperatur. De interna Rayleigh-siffrorna i flödet når upp till 10e6. Jämförelsen visar att SST k-ω turbulensmodellresultaten överensstämmer med DNS-data. Simuleringarna med Zr-oxidationsförhållandet 0, 0,2 och 0,5, motsvarande oxidskiktet på 1,389 m, 1,467 m och 1,580 m, och metallskiktet på 0,705 m, 0,664 m och 0,561 m i höjd, visar att temperaturen av oxidskiktet kommer att öka med Zr-oxidationsförhållandet, medan metallskiktets temperatur kommer att minska vilket resulterar i mer värmeöverföring genom oxidskiktets sidovägg och mindre toppstrålning. Ändå är effekten av Zr-oxidationsförhållandet inte uttalad i intervallet 00,5. Simuleringarna med Fe-massan på 22t, 33t och 45t och respektive höjd av metallskiktet på 0,462m, 0,568m och 0,664m visar att det inre metallskiktet avsevärt kommer att öka temperaturerna för både metallskiktet och oxiden lager. Andelen värmeöverföring vid oxidskiktets sidovägg ökar för att komplettera minskningen av den vid metallskiktet. Simuleringarna med strålningsemissiviteten 0,2, 0,35, 0,45 och 0,7 visar att emissiviteten under 0,45 påverkar värmeöverföringen, och temperaturerna och sidoväggens värmeflöde för både oxidskiktet och metallskiktet kommer att öka med minskande emissivitet. Effekten är försumbar när strålningen är över 0,45. Simuleringarna under de hypotetiskt extrema förhållandena med antingen en adiabatisk övre gräns eller ett mycket tunt metallskikt visar att fokuseringseffekten kan uppstå, dvs. värmeflödet genom metallsidan är större än det i oxidskiktet. Men det lokala höga värmeflödet plattas ut av kärlväggen med god värmeledningsförmåga. Sammanfattningsvis visar simuleringarna att, förutom fall under extrema förhållanden, är värmeflödet från smältpoolerna i alla andra fall betydligt lägre än CHF för extern kylning av nedre huvudet. Därför verkar säkerhetsmarginalen för IVR-strategin för den valda PWR tillräcklig. På grund av vissa begränsningar (t.ex. förenkling och antaganden) i simuleringsfall och koppling av olika inflytelserika faktorer, vilket indikeras av den aktuella studien, är de exakta förutsägelserna av värmeflöde under alla scenarier fortfarande svåra. Därför kunde slutsatserna inte generaliseras till de andra förhållandena eller andra konfigurationer av de smälta poolerna. Genom att diskutera modellen och förenklingar / antaganden som antagits i detta arbete föreslås förbättringsriktningarna för den numeriska modellen och andra perspektiv i slutet av avhandlingen.

Physical and social activities should be included in an efficient e-coaching for older adults and immersive virtual reality (IVR) is efficient if behavioral changes are transferred to the real physical world. In an experiment con-ducted with 42 older adults (63-85 years-old), participants were asked to interact with a specific immersive virtual reality (IVR) daily for 4 months in their homes, at least 15 minutes per day. Results demonstrate several positive impacts related to the use of the IVR (decrease of weight, decrease of waist circumference, number of steps outside) even if the time spent to use this IVR system decreases. So the transfer from the IVR to the real world is very encouraging.

Abstract When the marine vessels exceed the speed of 30 knots, it is preferred to adopt the waterjet propulsion method due to its high propulsive efficiency, good maneuverability, less vibration and good anti-cavitation performance. The efficiency of the waterjet pump is up to 90% with advanced modern design methods while 7∼9% of total power is lost in the intake duct. In this paper, the flow simulation in an intake duct has been conducted using the modified partially averaged Navier-Stokes method for better understanding of flow features inside the intake duct and instructing how to reduce the power loss at various ship speeds and inlet velocity ratio (IVR) with considering the hull boundary layer. The nonuniformity and perpendicularity at the impeller plane is applied to analyze the flow quality at the outlet plane of intake duct. The results indicate that the nonuniformity decreases while the perpendicularity increases with increasing IVR. Thus a large IVR together with a high ship speed would cause better outflows. Further analyses of the pressure along the ramp and cutwater depict that cavitation easily occurs at the upper side of the cutwater with a larger IVR. The hydraulic efficiency is seen to firstly increase and then decrease with an increase in IVR. The hydraulic efficiency of the intake duct is over 80% during IVR = 0.4∼1.2 with the maximum value of 92.19% at IVR = 0.6.

Molten core materials may relocate to the lower head of a reactor vessel in the latter stages of a severe accident. Under such circumstances, in-vessel retention (IVR) of the molten materials is a vital step in mitigating potential severe accident consequences. Whether IVR occurs depends on the interactions of a number of complex processes including heat transfer inside the accumulated molten pool, heat transfer from the molten pool to the reactor vessel (and to overlying fluids), and heat transfer from exterior vessel surfaces. SCDAP/RELAP5-3D© has been developed at the Idaho National Engineering and Environmental Laboratory to facilitate simulation of the processes affecting the potential for IVR, as well as processes involved in a wide variety of other reactor transients. In this paper, current capabilities of SCDAP/RELAP5-3D© relative to IVR modeling are described and results from typical applications are provided. In addition, anticipated developments to enhance IVR simulation with SCDAP/RELAP5-3D© are outlined.

1000-MWe scale Pressurized Water Reactor (PWR) is taking service or under construction all over the world, and larger scale plant is studied and developed for its more competitive economics. Not only design basic accidents are analyzed for nuclear safety, the severe accident must also be considered to meet with the increasing requirement of safety. In the “nuclear power plant design safety regulation” (HAF102) issued by Nation Nuclear Safety Administration (NNSA), aim at the preventing and mitigating of severe accident, the regulation bring forward new requirement, which required that during design phase, NPP should consider setting the preventing and mitigation measurement of severe accident as actually as possible. As an approach to prevent the curium from melting down the vessel and entering the containment when a postulated severe accident occurs, In-vessel retention (IVR) of molten core debris via water cooling of the external surface of the reactor vessel has been introduced into AP1000. External reactor vessel cooling (ERVC) is assumed to be achieved keeping exterior surface of vessel at 400K. It is known to all that different scenario and process results in different IVR molten model. As the core melt, different IVR model is formed at different time, such as two-layer model, three-layer model and four layer model. It is necessary to study the IVR model when severe accident process moves on. This paper studies two-layer and three-layer IVR models and find the features of the models. Based on this, sensitivity study of important parameters has also been analyzed. It is useful for us to understand the mechanism of the molten pool. This paper has some directive significance on future IVR strategy research and also provides theoretical support to safety evaluation of PWR plants.

Resumen
In-vessel retention (IVR) of core melt through external reactor vessel cooling (ERVC) is a key severe accident management strategy to ensure that the vessel head remains intact and eliminate consequent major threats to containment integrity. To maintain the margin against the failure of the reactor vessel and make sure of the feasibility of IVR for its role to confine the molten corium with 1400MW power, CAP1400, Chinese version of large passive PWR, systematic investigations including experimental and analytical researches of IVR are very important to the development of CAP1400. This paper briefly reviews the progress and tasks of a four-year project which was planned to analyze, evaluate, improve and validate the effectiveness of IVR employed in the CAP1400.

We have reported that localization of TMV in tobacco cultivars with the N gene, is associated with a 23 K protein (IVR) that inhibited replication of several plant viruses. This protein was also found in induced resistant tissue of Nicotiana glutinosa x Nicotiana debneyi. During the present grant we found that TMV production is enhanced in protoplasts and plants of local lesion responding tobacco cultivars exposed to 35oC, parallel to an almost complete suppression of the production of IVR. We also found that IVR is associated with resistance mechanisms in pepper cultivars. We succeeded to clone the IVR gene. In the first attempt we isolated a clone - "101" which had a specific insert of 372 bp (the full length gene for the IVR protein of 23 kD should be around 700 bp). However, attempts to isolate the full length gene did not give clear cut results, and we decided not to continue with this clone. The amino acid sequence of the N-terminus of IVR was determined and an antiserum was prepared against a synthetic peptide representing amino acids residues 1-20 of IVR. Using this antiserum as well as our polyclonal antiserum to IVR a new clone NC-330 was isolated using lamba-ZAP library. This NC-330 clone has an insert of about 1 kB with an open reading frame of 596 bp. This clone had 86.6% homology with the first 15 amino acids of the N-terminal part of IVR and 61.6% homology with the first 23 amino acids of IVR. In the QIA expression system and western blotting of the expressed protein, a clear band of about 21 kD was obtained with IVR antiserum. This clone was used for transformation of Samsun tobacco plants and we have presently plantlets which were rooted on medium containing kanamycin. Hybridization with this clone was also obtained with RNA from induced resistant tissue of Samsun NN but not with RNA from healthy control tissue of Samsun NN, or infected or healthy tissue of Samsun. This further strengthens the previous data that the NC 330 clone codes for IVR. In the U.S. it was shown that IVR is induced in plants containing the N' gene when infected with mutants of TMV that elicit the HR. This is a defined system in which the elicitor is known to be due to permutations of the coat protein which can vary in elicitor strength. The objective was to understand how IVR synthesis is induced after recognition of elicitor coat protein in the signal transduction pathway that leads to HR. We developed systems to manipulate induction of IVR by modifying the elicitor and are using these elicitor molecules to isolate the corresponding plant receptor molecules. A "far-western" procedure was developed that found a protein from N' plants that specifically bind to elicitor coat proteins. This protein is being purified and sequenced. This objective has not been completed and is still in progress.
