Women without a partner can become single mothers by choice through the use of fertility treatments. In Belgium, the decision to accept a candidate single mother by choice rests with the fertility clinic’s multidisciplinary team of fertility practitioners. As a result, the fertility practitioners fulfil a gatekeeping role. However, this can cause an intra-role conflict as the responsibility to select the best fitting candidates is at odds with the responsibility to help patients. In this explorative study, we examine how fertility practitioners cope with the strain resulting from intra-role conflict in the decision-making process regarding single motherhood by choice in Belgium. The findings showed that practitioners appear to mainly resort to problem-focused coping, by constructing a grassroots criteria list and by shifting their role from screening agent to counsellor. These results are based on ten open in-depth interviews with fertility practitioners employed in the multidisciplinary teams of fertility centers, using a reflexive interview lead.

A recent universal public health problem, intrauterine mortality (IUM), is probably the least documented demographic and social phenomenon in Morocco caused by data scarcity. This study aims to measure the IUM’s intensity and effect on women’s fertility. IUM quotients and their components (early fetal and late fetal mortality) were estimated by constructing the mortality table. An empirical examination of IUM’s impact on fertility was conducted using a direct method, the Bongaarts model (1978) and Leridon (2002) method. The data is from the 2009–2010 National Demographic Survey. The results show that in 2009–2010, the IUM quotient reached 272% pregnancies: 290% in urban areas versus 251% in rural areas. This IUM reduced potential fertility by 6% using the direct method. In particular, abortion reduced potential fertility by 5% versus 3.5% in 2009–2010, using the Bongaarts and Leridon methods. Given the importance of abortion, its impact on fertility, and its multidimensional consequences, it is interesting to research the determinants associated with the recourse to abortion as a significant public health issue.

This fertility awareness-based method relies on the identification of cervico-vaginal secretions to identify the fertile window. Female genital cutting and traditional vaginal practices, such as the use of desiccants, may affect the presence or absence of secretions and therefore the woman's perception of her fertility. These issues and their implications for service delivery of the method are discussed.

In many human societies, high male social status associates with higher fertility, but the means by which status increases lifetime fitness have not been systematically investigated. We analyse the pathways by which male status begets reproductive success in a small-scale, Amerindian society. Men who are more likely to win a dyadic physical confrontation, i.e. dominant men, have higher intra-marital fertility for their age, and men with more community-wide influence, i.e. prestigious men, exhibit both higher intra-marital fertility and lower offspring mortality. Both forms of status elicit support from allies and deference from competitors, but high status men are not provisioned more than their peers. Prestigious but not dominant men marry wives who first give birth at earlier ages, which multivariate analysis suggests is the strongest pathway between status and fitness in this population. Furthermore, men are motivated to pursue status because of fitness gains both within and outside of marital unions: dominant and prestigious men have more in-pair surviving offspring as well as more extra-marital affairs.

Abstract The fertility potential of human sperm populations can be assessed by the presence of head-directed mannose ligand receptors (mannose-specific lectin) and the occurrence of spontaneous acrosome reactions after incubation under capacitating conditions in vitro. We have reported previously on the interaction between anti-hypertensive medications and their effects on these parameters of male fertility potential. In this report we document the effects of cessation of calcium ion channel blocker medication on male fertility. Motile spermatozoa from a 30 year old infertile patient on a calcium ion channel blocker as anti-hypertensive treatment had subnormal expression of mannose-specific lectin and did not exhibit spontaneous acrosome reactions. Three months following discontinuation of the medications, complete recovery of both the expression of head-directed mannose ligand receptors and the acrosome reaction was documented, though sperm motility and morphology remained unchanged. The couple had 2 years of infertility and previously failed to conceive through seven cycles of Pergonal/intra-uterine insemination. Conception occurred on the second Pergonal/intra-uterine insemination cycle after the husband discontinued calcium ion channel blocker medication. Calcium ion channel blockers may adversely affect sperm fertilizing potential. Discontinuation of such medications enhances the chances for conception.

The present study assessed the effect of birthweight on reproductive performance, including a possible mechanism, in male pigs. Ten newborn male piglets, including five normal birthweight (NBW) piglets and five intra-uterine growth restricted (IUGR) piglets, were used in the study. All piglets were weaned on Day 28 and fed the same diet during the experiment (10 months). Average daily weight gain, feed intake and the feed conversion ratio were higher in NBW than IUGR piglets. Similarly, testis volume and the number of Leydig and Sertoli cells in the distal portion of the testes were higher in NBW than IUGR piglets (P < 0.05). Semen volume (P < 0.05) and the total number of spermatozoa per ejaculate (P = 0.08) were lower in IUGR boars. Testosterone concentrations on Day 141 and prostaglandin E2 concentrations on Days 82 and 141 were higher in IUGR than NBW boars (P < 0.05). The malondialdehyde content of seminal plasma was higher in IUGR boars, whereas sperm glutathione peroxidase activity was lower in IUGR versus NBW boars (P < 0.05). Expression of DNA methyltransferase (Dnmt) genes Dnmt1, Dnmt3a, histone-lysine N-methyltransferase (Suv39h2), and lysine (K)-specific demethylase Kdm4a was upregulated in testes from IUGR boars. These findings suggest that growth restriction affects sperm production via reproductive organ development and epigenetic regulation.

Introduction: The incidence of infertility is about 10 to 15 % among reproductive age group. The cause of infertility may be either due to male factor or female factor or both. The main aim of this study was to evaluate the success rate of intrauterine insemination (IUI) in a private centre of Pokhara. Materials and Methods: This was a centre based retrospective study done in private fertility centre. Sub-fertile couples who were treated from January 1st 2015 to December 31st 2016 were enrolled for the study. Couples with unexplained male factor, ovulatory dysfunction, unilateral tubal occlusion and ejaculatory dysfunction were included in this study. Three hundred and eighty IUI cases were retrospectively reviewed. Clinical pregnancy rate was the primary outcome. Result: The success rate of IUI was 15.7%. It was higher among unexplained infertility cases. Clinical pregnancy rate was directly associated with the age of the patients, indications of infertility and number of cycles. Conclusion: Success rate of IUI in infertile couples who had unexplained infertility, tubal factor, ovulatory dysfunction was higher than male factor infertility and ejaculatory dysfunction.

Purpose. To present the results in a midterm followup of laparoscopic testicular preservation in adults with intra-abdominal cryptorchidism.Methods. We analyzed 14 adult patients with cryptorchidism (19 testes) submitted to transabdominal laparoscopic evaluation and treatment of the condition. Data was collected retrospectively from a prospective database from August 2005 to May 2010. It analyzed patient age, affected side, procedure, mean operative time, mean hospital stay, postoperative testis position, intra- and postoperative complications, presence of malignancy in the removed testis, and midterm outcomes: size of the testis, development of tumors, and fertility.Results. Mean patient age was 29 (18–54) years. Thirteen (68.4%) testes were preserved. There were no intra- or postoperative complications. After a mean followup of 48.7 (20–64) months, all patients submitted to orchiopexy had the organs correctly positioned, although 2 testes were significantly smaller than before the procedure. No patient presented any signs of malignancy during the followup. Fertility was not preserved in bilateral cases.Conclusion. Laparoscopic testicular evaluation and eventual preservation are feasible and safe. In a midterm followup, testicular tumor is not a concern, and unfortunately, fertility may not be preserved in bilateral cases.

Greenhouse studies were conducted to determine the effect of phosphorus (P) fertility on intra- and interspecific competition between lettuce and spiny amaranth for 4 wk after emergence. Total lettuce shoot biomass per pot and weight per plant increased 39 and 44% in response to increased P fertility, respectively. P fertility had no impact on growth of spiny amaranth. Total shoot biomass of spiny amaranth increased with increasing density from four to eight plants, however, lettuce did not. Total shoot biomass of both species increased as density increased from 4 to 16 plants. Spiny amaranth, but not lettuce, weight per plant decreased in response to intraspecific competition. Reciprocal yield analysis showed that spiny amaranth produced 2.4 times more biomass than lettuce when competing intraspecifically and four times more biomass under interspecific competition. Lettuce weight per plant was not affected. Relative yield analysis indicated that spiny amaranth was more competitive than lettuce regardless of P fertility. However, increased P fertility increased competitiveness of lettuce. Relative crowding coefficients indicated that spiny amaranth at the low density with low P fertility was 33 times more competitive than lettuce. Addition of P caused lettuce and spiny amaranth to be equally competitive at the lowest density; however, at the highest density, spiny amaranth was 4 tunes more competitive than lettuce regardless of additional P.

Chromosomal polymorphism plays a major role in speciation processes in mammals with high rates of karyotypic evolution, as observed in the family Cervidae. One remarkable example is the genus Mazama that comprises wide inter- and intra-specific chromosomal variability. To evaluate the impact of chromosomal polymorphisms as reproductive barriers within the genus Mazama, inter-specific hybrids between Mazama gouazoubira and Mazama nemorivaga (MGO × MNE) and intra-specific hybrids between cytotypes of Mazama americana (MAM) differing by a tandem (TF) or centric fusion (Robertsonian translocations—RT) were evaluated. MGO × MNE hybrid fertility was evaluated by the seminal quality and testicular histology. MAM hybrids estimation of the meiotic segregation products was performed by sperm-FISH analysis. MGO × MNE hybrids analyses showed different degrees of fertility reduction, from severe subfertility to complete sterility. Regarding MAM, RT, and TF carriers showed a mean value for alternate segregation rate of 97.74%, and 67.23%, and adjacent segregation rate of 1.80%, and 29.07%, respectively. Our results suggested an efficient post-zygotic barrier represented by severe fertility reduction for MGO × MNE and MAM with heterozygous TF. Nevertheless, RT did not show a severe effect on the reproductive fitness in MAM. Our data support the validity of MGO and MNE as different species and reveals cryptic species within MAM.

We use data from the Demographic and Health Surveys to examine the patterns of stall in fertility decline in four Eastern African countries. Contrary to patterns of fertility transition in Africa that cut across various socio-economic and geographical groups within countries, we find strong selectivity of fertility stall across different groups and regions in all four countries. In both Kenya and Tanzania where fertility decline has stalled at the national level, it continued to decline among the most educated women and in some regions. While fertility has remained at pre-transition level in Uganda over the past 20 years, there are signs of decline with specific groups of women (especially the most educated, urban and those in the Eastern region) taking the lead. For Zimbabwe, although fertility has continued to decline at the national level, stall is observed among women with less than secondary education and those in some of the regions. We link these intra-country variations to differential changes in socio-economic variables, family planning programme environment and reproductive behaviour models. The results suggest that declines in contraceptive use, increases in unmet need for family planning, increasing preferences for larger families, and increases in adolescent fertility were consistently associated with stalls in subgroup fertility across all four countries. These results are consistent with models that emphasize the role of declines in national and international commitments to family planning programmes in the premature stall in sub-Saharan fertility transition.

Correlations among computer-assisted spermatozoa motility analyses, Ca2+ fluxes and in vivo fertility of bovine spermatozoa based on a total of 4482 inseminations were investigated in each of four ejaculates from six bulls. The Ca2+ parameters assessed the rate of change in intra- and extracellular Ca + in fresh and cryopreserved spermatozoa from the same ejaculates and were described in another study. Of the seven motility parameters of cryopreserved semen investigated, all differed significantly among bulls but none were related to the in vivo fertility of cryopreserved semen. The amplitude of lateral head displacement, a motility parameter associated with hyperactivation, was positively correlated with the intracellular Ca2+ levels and the rate of Ca2+ accumulation of cyropreserved spermatozoa. The highest fertility was observed when initial extracellular Ca2+ for cryopreserved spermatozoa was high and when Ca2+ efflux rates of cryopreserved cells approached the higher efflux rates of fresh spermatozoa. Fertility was reduced when cryopreserved spermatozoa had initial internal Ca2+ levels greater than those of fresh spermatozoa or when cryopreserved spermatozoa internalized Ca2+ rapidly. Objectives This study aimed to assess the influences of stigma and HIV status on reproductive intention among heterosexual couples living with HIV in China. Methods A cross-sectional study was conducted in Kunming, China among 315 HIV-positive individuals and their spouses (n = 315 couples). An interview questionnaire was used to collect data on intention, desire, HIV Stigma Scale items, and HIV status. Dyadic fertility intention was examined using the actor-partner interdependence mediation model, based on the Traits-Desires-Intentions-Behavior framework. Results The husbands’ or wives’ internalized stigma had significant negative effects on their own fertility desire (β = −0.149, p<0.05 and β = −0.238, p<0.01, respectively). HIV-positive status of the husbands was weakly linked to their own fertility intention (β = −0.181, p<0.05). Husbands’ perceived provider stigma was associated with their own and their wives’ fertility intention via the mediating effect of their fertility desire (β = −0.374, p<0.001 and β = −0.203, p<0.01, respectively). The cumulative influence of their reproductive desire mediated the husband’s perceived provider stigma and the wife’s internalized stigma on their fertility intention. Conclusions Stigma and HIV status were associated with fertility intention among couples living with HIV, mediated by fertility desire. The high intra-couple correlation suggested that counseling should be conducted when both spouses are present together with extensive discussions on concerns regarding HIV-related stigma, potential discrepancies between each partner’s fertility desire and intention, and the influence of one partner on the other.

Around the middle of the 20th century, most Western countries experienced a surge in birth rates, called the Baby Boom. This boom was unexpected at the time and the underlying mechanisms are still not entirely clear. It was characterized by high levels of inter- and intra-country variability in fertility, as some regions even experienced fertility decline during the Boom. In this paper, we suggest that social influence processes, propelling a shift towards two-child families, might have played an important role in the observed changes in fertility. Interactions in social networks can lead new types of childbearing behaviour to diffuse widely and thereby induce changes in fertility at the macro level. The emergence and diffusion of a two-child norm resulted in homogenization of fertility behaviour across regions. Overall, this led to a reduction of childlessness and thus an increase of fertility, as more people aspired to have at least two children. Yet, in those regions where larger family sizes were still common, the two-child norm contributed to a fertility decline. To explore the role of social influence with analytical rigor, we make use of agent-based computational modelling. We explicate the underlying behavioural assumptions in a formal model and assess their implications by submitting this model to computational simulation experiments. We use Belgium as a case study, since it exhibited large variability in fertility in a relatively small population during the Baby Boom years. We use census data to generate realistic starting conditions and to empirically validate the outcomes that our model generates. Our results show that the proposed mechanism could explain an important part of the variability of fertility trends during the Baby Boom era.

AbstractEpidemiological studies have demonstrated an increased risk of developing non-transmittable diseases in adults subjected to adverse early developmental conditions. Metabolic and cardiovascular diseases have been the focus of most studies. Nevertheless, data from animal models also suggest early programming of fertility. In humans, it is difficult to assess the impact of the in utero environment retrospectively. Birthweight is commonly used as an indirect indicator of intrauterine development. This research is part of the ALIFERT study. We investigated a potential link between ponderal index at birth and female fertility in adulthood. Data from 51 infertile and 74 fertile women were analysed. BW was on average higher in infertile women, whereas birth length did not differ between the two groups; thus, resulting in a significantly higher ponderal index at birth in infertile women. Ponderal index at birth has been identified as a risk factor for infertility. These results suggest the importance of the intra-uterine environment, not only for long-term metabolic health but also for fertility.

Selfish genetic elements (SGEs) are diverse and near ubiquitous in Eukaryotes and can be potent drivers of evolution. Here, we discuss SGEs that specifically act on sperm to gain a transmission advantage to the next generation. The diverse SGEs that affect sperm often impose costs on carrier males, including damaging ejaculates, skewing offspring sex ratios and in particular reducing sperm-competitive success of SGE-carrying males. How males and females tolerate and mitigate against these costs is a dynamic and expanding area of research. The intense intra-genomic conflict that these selfish elements generate could also have implications for male fertility and spermatogenesis more widely. This article is part of the theme issue ‘Fifty years of sperm competition’.

Understanding soil–plant–microbe inter- and intra- interactions are essential for ensuring proper soil health, quality, and soil-mediated ecosystem services (e.g., nutrient cycling) required for human–plant–animal life. Intensive and unsustainable farming practices can decrease soil microbial biodiversity, fertility, and quality leading to soil degradation, impaired nutrient cycling, and the incapability of soil to support plant growth. Under such a context, soil biological fertility can appear as a regenerative component that has the potential to harmonize and improve soil’s physical, chemical, and biological parameters. This study defines and discusses the microbiome in the rhizosphere, microbial nutrient cycling, and biological soil crusts as the major components of soil biological fertility, and explores the answers to the following questions: (i) how does the rhizosphere promote plant growth, development, and nutrient cycling through soil microorganisms (e.g., bacteria, fungi)? (ii) How can soil microorganisms regulate macronutrient cycling and facilitate biocrust formation? This review reveals that soil biological fertility is crucial for increasing crop resilience and productivity as well as sustainability in agriculture. Additionally, the reintroduction of plant growth promoting rhizobacteria, a quantitative estimation of the root exudate’s composition, identifying the spatiotemporal dynamics of potassium solubilizing bacteria and establishing biological soil crusts in agricultural lands remain the major tasks for improving soil biological fertility and the transition towards regenerative agriculture.

An exceedingly rare, highly vascular tumor with atypical and alarming presentation posed diagnostic and management challenges. A 29 years old lady, complained of abdominal and pelvic pain, nausea, vomiting and repeated spells of fainting attacks following the jumping episode (2-3 times). Emergency admission and investigations at United States of America (USA) revealed a large, complex, highly vascular, irregular mass displaying finger like projections, filling the lower abdomen, pelvis and large amount of intra peritoneal bleed. A provisional diagnosis of arteriovenous malformation (AVM), arteriovenous fistula (AVF), and rupture/leak was made. Uterine artery angiography (UAA), preoperative embolization and possibility of hysterectomy as a life saving measure were suggested, to which the couple declined and got discharged. To save uterus and fertility they presented to us for second opinion and further management. Angiography, magnetic resonance imaging (MRI) and preoperative uterine artery embolization were done followed by exploratory laparotomy which revealed hemoperitoneum and a large weird looking vascular tumor (a veil of sea weeds/dark purple nodules of various sizes resembling cotyledons of placenta). Intra-operative tissue analysis (frozen section) suggested benign pathology. Mass was removed completely and fertility sparing surgery was undertaken. Histopathological diagnosis was cotyledonoid dissecting leiomyoma (CDL). No recurrence/metastasis has been observed on follow up of 5 years.

Genetic divergence of 48 IRRI developed F1 rice hybrids were studied through Mahalanobis D2 and principal component analysis for ten characters in Gazipur and Pabna district during dry season 2012-13. Among 48 genotypes, cluster analysis resulted in five clusters following Ward’s method. During dry season for both regions, cluster I incorporated the highest number of genotypes. Significant differences among the genotypes but non-significant differences among replications were noticed for most of the parameters under study. The inter-cluster distance was larger than intra-cluster distance suggesting wider diversity among the genotypes. In Gazipur, the maximum inter-cluster D2 value was recorded between cluster III and V (6.69) and the minimum between cluster II and IV (0.69). The intra-cluster distance was maximum in cluster IV (3.60) and minimum in cluster I (0.33). In Pabna, maximum inter-cluster distance was exhibited between cluster IV and II (16.43) but minimum between cluster II and III (3.98). Maximum intra-cluster distance (4.40) was recorded in cluster IV whereas minimum in cluster I (0.83). Based on positive value of vector 1 and vector 2, panicle number m-2, spikelet fertility (%) and 1000-grain wt. (g) had maximum contribution towards genetic divergence among the characters studied. Cluster means indicated considerable differences in the mean values of different parameters. The highest cluster means for panicle weight (g), spikelet fertility (%), 1000 grain wt. (g) and grain yield (kg ha-1) were derived from cluster IV. Therefore, more prominence should be given to the lines under cluster IV that having genetic potentiality to contribute better for yield maximization of hybrid rice.

The objectives of this research were to identify alternatives to glyphosate for intra-row (under-trellis) vineyard floor management and to evaluate the potential for intra-row and inter-row (alleyway) groundcovers to reduce vegetative vigor of ‘Marquette’ grapevines (Vitisspp.) in a southeast Nebraska vineyard. The experiment was a randomized factorial design with five intra-row treatments (crushed glass mulch [CG], distillers’ grain mulch [DG], creeping red fescue [CRF], non-sprayed control [NSC], and glyphosate [GLY]) and three inter-row treatments (creeping red fescue [CRF], Kentucky bluegrass [KB], and resident vegetation [RV]). Treatments were established in 2010–2011 and measurements were conducted during 2012 and 2013 on 5- and 6-year-old vines. Soil temperatures were mostly higher under mulches and lower under intra-row groundcovers, compared to GLY. Weed cover in CG, DG, and CRF treatments was the same or less than GLY. At most sampling dates, inter-row soil moisture was lowest under KB. Intra-row soil moisture was highest under DG mulch and lowest under CRF and NSC; CG had the same or lower soil moisture than GLY. Surprisingly, we did not detect differences in mid-day photosynthetically active radiation (PAR) reflectance, despite visual differences among the intra-row treatments. Mid-day vine water potential did not differ among treatments. We concluded it is not necessary to maintain a bare soil strip under established vines in this region, where soil fertility and moisture are non-limiting.

Increasing exploitation of roselle (Hibiscus sabdariffa L.) and its products require that commercial varieties with superior attributes are developed to replace landrace varieties and to meet the growing demand for commercial-scale cultivation of the crop. In this study, twenty landrace varieties of roselle collected from three geographical regions of Ghana were artificially crossed in a pairwise mating scheme. The aim was to obtain relevant information pertaining to fertility status and extent of crossability among the accessions to facilitate breeding of improved varieties. Attempts at hybridisation were made both between and among 6 accessions of Hibiscus sabdariffa var Altissima and 14 accessions of Hibiscus sabdariffa var Sabdariffa. Crosses between accessions of Hibiscus sabdariffa var Altissima and Hibiscus sabdariffa var Sabdariffa yielded no hybrids, suggestive of a crossability barrier between the two mating groups. All intra-varietal crosses involving accessions of Hibiscus sabdariffa var Altissima were successful regardless of direction of cross while only few of the crosses among those of Hibiscus sabdariffa var Sabdariffa produced successful hybrids. Also fertility estimates of members of Hibiscus sabdariffa var Altissima were significantly higher (30.00%-80.00%) compared to those of var Sabdariffa (0.00%-43.00%). This implies that complete pairwise design could be used for intra-varietal crosses involving accessions of Hibiscus sabdariffa var Altissima, whereas only a partial pairwise design is feasible for members of Hibiscus sabdariffa var Sabdariffa.

In modern life, several factors such as genetics, exposure to toxins, and aging have resulted in significant levels of male infertility, estimated to be approximately 18% worldwide. In response, substantial progress has been made to improve in vitro fertilization treatments (e.g. microsurgical testicular sperm extraction (m-TESE), intra-cytoplasmic sperm injection (ICSI), and round spermatid injection (ROSI)). Mimicking the structure of testicular natural extracellular matrices (ECM) outside of the body is one clear route toward complete in vitro spermatogenesis and male fertility preservation. Here, a new wave of technological innovations is underway applying regenerative medicine strategies to cell-tissue culture on natural or synthetic scaffolds supplemented with bioactive factors. The emergence of advanced bioengineered systems suggests new hope for male fertility preservation through development of functional male germ cells. To date, few studies aimed at in vitro spermatogenesis have resulted in relevant numbers of mature gametes. However, a substantial body of knowledge on conditions that are required to maintain and mature male germ cells in vitro is now in place. This review focuses on advanced bioengineering methods such as microfluidic systems, bio-fabricated scaffolds, and 3D organ culture applied to the germline for fertility preservation through in vitro spermatogenesis.

Pelvic cyst originated from Mullerian or Wolffian duct are reported frequently but ejaculatory duct cysts are very rare. Here we present a rare case of large ejaculatory duct cyst in a diabetic man. In Bangladesh no such case has yet reported. This will be the first case reported in our country. This man was 48 years old presented with post ejaculatory pain, haematospermia, low ejaculatory volume and infertility. On trans rectal ultrasonogram (TRUS) there was 70ml volume retro prostatic midline cyst was found. MRI pelvis confirms the diagnosis. There are different modalities of treatment ranging from observation to open surgery. We chose to open excision with good post operative outcome. Ejaculatory duct cyst is a rare entity. Malignant transformation also reported in ejaculatory duct cyst. Trans rectal ultra sonogram (TRUS) and MRI of pelvic organs are diagnostic. Treatment depends on size of cyst and fertility. Total excision result in complete recovery from symptoms and disease. Transurethral surgery may conserve natural fertility but persistence in complications. The aim of this study was to test the efficiency of progesterone supplementation post insemination (p.i.) in the fertility of dairy cows under commercial farm conditions. At initiation of the study, 405 cows from three farms -irrespective of their open days- were bred after showing heat or after synchronization. Subsequently, the cows were randomly allocated to receive a progesterone-releasing intra-vaginal device (PRID®) between D5 and D17 p.i (P4+, n=213), or to remain untreated (P4-, n=192). Pregnancy per artificial insemination (P/AI) did not differ between P4+ (42.3%) and P4- (41.2%, P = 0.82) groups. However, the treatment improved P/AI in cows enrolled in the study after the second p.p. insemination (46.8 vs. 25.5, P = 0.02). In the farm that showed the lower days to insemination before treatment, progesterone supplementation tended to raise the P/AI (48.2 vs. 23.5, P = 0.08). However, in the farm with the greater fertility the P4+ group had significantly lower P/AI than the P4- group (37.7 vs. 57.4, P = 0.03). Conclusively, the beneficial effect of the post-insemination administration of PRID is mainly apparent only after the second p.p. insemination and probably during early lactation when fertility is suppressed.

ObjectiveThe objective of this systematic review was to assess the oncologic and fertility outcomes of patients with cervix-confined cancer >4 cm who underwent neo-adjuvant chemotherapy followed by fertility-sparing surgery.MethodsThis study was registered in PROSPERO (registration number CRD42021254816). PubMed/MEDLINE, ClinicalTrials, EMBASE, Cochrane Central Register of Controlled Trials, SCOPUS, and OVID databases were searched from inception to July 2021. The included patients were those with cancer confined to the cervix and tumor diameter >4 cm (International Federation of Gynecology and Obstetrics (FIGO) 2018 stage IB3) with squamous cell carcinoma, adenocarcinoma, or adenosquamous carcinoma who underwent intra-venous neo-adjuvant chemotherapy followed by successful fertility-sparing surgery.ResultsThe initial search identified 2990 articles. A total of 40 patients from 11 studies had attempted fertility preservation surgery (conization, simple or radical trachelectomy) and in 26 patients (65%) it was successful. All patients received platinum-based chemotherapy. A complete pathological response occurred in 56% of patients and two patients (7.7%) had a recurrence. The 4.5-year disease-free survival was 92.3% and the 4.5-year overall survival rate was 100%. Of six patients who tried to conceive, four (67%) achieved at least one pregnancy and three of the five pregnancies (60%) were pre-term deliveries (all after radical trachelectomy). All patients with recurrence received cisplatin and ifosfamide instead of cisplatin and paclitaxel, underwent non-radical surgery, and had residual disease in the final specimen.ConclusionsEvidence for fertility-sparing surgery after neo-adjuvant chemotherapy in patients with cervical cancer and tumors >4 cm is limited, and this approach should be considered as an experimental intervention. As the use of non-radical surgery could be a risk factor, if neo-adjuvant chemotherapy is used, patients should undergo fertility-sparing radical surgery.

Thirty aromatic rice genotypes were studied to assess the nature and magnitude of genetic divergence among them for grain yield and yield contributing characters. Based on 23 characters the genotypes were grouped into 5 clusters. Inter-distances were higher than intra-cluster showing a wide range of genetic diversity among the genotypes of different groups. The highest level of inter-cluster distance (73.48) was found between cluster IV and V. The highest intra-cluster distance (3.364) was computed to cluster V and the lowest 1.851 was observed in cluster IV. Panicle length, days to initial flower opening, spikelet sterility, pollen fertility and harvest index had maximum contribution towards genetic divergence of 30 aromatic rice genotypes. Genotypes having these characters in the genetically distant cluster could, therefore, offer a significant scope for the development of high yielding variety through judicious selection.

Young women wishing to maintain fertility may be reluctant to pursue this. An alternative strategy in women with low-grade PMP has been explored in the form of laparoscopic evacuation of pelvic and ovarian mucin with resection of the appendiceal tumour.MethodsBetween January 2012 and January 2015, four young women (aged 28–35 years) with PMP seeking to maintain fertility underwent laparoscopy, appendicectomy and pelvic mucinous evacuation and washout. Data regarding intra-operative and histopathological findings were collected. Endpoints were fertility-related outcomes and oncological follow-up.ResultsInfertility was a presenting symptom in three of the four women. All four had significant pelvic mucinous disease on radiological imaging and were offered CRS and HIPEC as definitive treatment, but chose laparoscopy with appendicectomy and copious irrigation and washout of the pelvis with stripping of mucinous disease off the ovarian surfaces. Postoperative histology demonstrated a low-grade appendiceal mucinous neoplasm (LAMN) in all patients with acellular mucin or low-grade mucinous carcinoma peritonei in the peritoneal cavity. All patients successfully conceived subsequently and gave birth to healthy babies. After 12–29 months follow-up, all women are well with no radiological or laparoscopic evidence of disease recurrence.ConclusionsIn patients with low-grade PMP, initial therapeutic laparoscopy can restore fertility, whilst providing short- to medium-term disease control. This modality in young women wishing to have children appears to be a feasible alternative to immediate CRS and HIPEC.

Ultrasound is a readily available, safe and portable imaging modality that is widely applied in gynecology. However, there is limited guidance for its use intra-operatively especially with complex gynecological procedures. This narrative review examines the existing literature published on the use of intraoperative ultrasound (IOUS) in benign gynecology and in gynecological oncology. We searched for the following terms: ‘intraoperative,’ ‘ultrasonography,’ ‘gynecology’ and ‘oncology’ using Pubmed/Medline. IOUS can minimize complications and facilitate difficult benign gynecological procedures. There is also a role for its use in gynecological oncology surgery and fertility-sparing surgery. The use of IOUS in gynecological surgery is an emerging field which improves visualization in the surgical field and aids completion of minimally invasive techniques.

Cervical ectopic pregnancy (CEP) is an exceptional type of ectopic pregnancy, representing <1% of the ectopic pregnancy with incidence 1:2500 to 1:12000. It can lead to life threatening complication and torrential bleeding and even necessitate the need for hysterectomy. A 30-year-old patient referred to Lokmanya Tilak Municipal Medical College with transvaginal sonography suggestive of live CEP. Because of patient’s stable condition, it was treated with ultrasound-guided intra-amniotic methotrexate administration. Day 1 β-HCG was 4200 IU/L, day 10 489 IU/L. Gradual decrease of β-HCG levels was coinciding with ultrasonographic finding of absence of gestational sac. During hospitalization, patient reported only minimal vaginal spotting on day 3 of treatment and discharged on day 10 with weekly β-HCG followup. CEP was managed conservatively without surgical intervention, preserving patient’s fertility. Timely detection of CEP and prompt action can save patients fertility with minimal or no surgical intervention.

AbstractDetectability of early stages of ectopic pregnancies has increased due to improvements in ultrasonographic and biochemical techniques. Since the patients’ future procreative plans must be taken into consideration when commencing treatment, the goal of this work was to compare the effects of treatment methods and their impact on fertility. The study included 91 patients treated surgically for ectopic pregnancy. The choice of treatment depended on patients’ general condition, ultrasonographic evaluation and serum level of beta-hCG. A combination of laparoscopic and conservative systemic treatment was applied in 70% of cases. More rapid beta-hCG reduction was noted when laparoscopy and intra-oviductal injection of hyperosmolar glucose or methotrexate (MTX) were combined with intramuscular administration of MTX at a dose of 50 mg/m2. Follow-up examination of 66 patients revealed that the greatest number of spontaneous pregnancies (48%) resulted after this combination therapy. We conclude that this combination treatment is safe and provides satisfactory results in terms of future fertility.

The ovarian ectopic pregnancy OPE is one of the most serious and rarest types of extra-uterine pregnancy, where the implantation of the gestational sac occurs in the ovum. Given the rarity of this condition, in addition to the accompanied morbidity, the risk of their complications, and even death, early diagnosis and treatment are crucial. Its diagnosis is not easy and relies on criteria based on preoperative findings, with poor clinical symptomatology and a difficult ultrasound diagnosis. Its management of choice remains surgical therapy despite the progress in medical treatments with fertility preservation. Patients’ symptomatology, radiological and lab findings, addition to her obstetric history and desire for future procreation must also be taken into consideration.
