Streptococcus pneumoniae, ou pneumocoque, est la principale cause de pneumonie communautaire, entraînant une morbidité et une mortalité importantes, en particulier chez les enfants de moins de cinq ans et les personnes âgées. L'amoxicilline est le traitement de première intention ; cependant, la résistance croissante à l'amoxicilline complique le traitement de ces infections et les positionnent donc comme un problème majeur de santé publique. L'immunité innée mobilise divers mécanismes de défense antimicrobienne et son ciblage représente une stratégie prometteuse pour résoudre ce problème. Une immunothérapie combinant l'amoxicilline à l'inhalation de flagelline, un agoniste du récepteur de l'immunité innée Toll-like receptor 5, a été développée. La preuve de concept de ce traitement combiné a été montrée dans un modèle murin de pneumonie. La combinaison amoxicilline et flagelline s'est montrée plus efficace que l'antibiotique seul car elle cause une réduction plus importante de la charge bactérienne dans les poumons et de la dissémination systémique et elle augmente la survie. L'efficacité de cette thérapie a été observée dans des modèles de pneumonie primaire ou de surinfection grippale, en utilisant des souches sensibles ou résistantes à l'amoxicilline. La thérapie combinée renforce la réponse immunitaire innée en augmentant de manière significative le recrutement de neutrophiles dans les voies respiratoires et en stimulant la production de médiateurs immunitaires. Le premier objectif de ma thèse était d'étudier l'impact de la thérapie combinée sur la sélection de bactéries résistantes aux antibiotiques. Dans ce contexte, un modèle murin de co-infection utilisant des souches isogéniques sensibles et résistantes à l'amoxicilline a été établi. Ce travail a montré que la flagelline permet de diminuer 200 fois la dose d'antibiotique tout en conservant la même efficacité que l'antibiotique seul. En outre, l'adaptation de ce modèle, qui reproduit la sélection de résistance aux antibiotiques à la suite d'une antibiothérapie, a montré que le traitement adjuvant réduit et retarde l'émergence de bactéries résistantes par rapport à l'antibiotique seul. Ces résultats suggèrent que l'association de l'amoxicilline et de la flagelline permet non seulement d'améliorer l'efficacité thérapeutique, mais aussi de diminuer le développement de résistance aux antibiotiques. De plus, un modèle mathématique a été développé pour capturer la dynamique des populations au cours de l'infection pulmonaire et a estimé que l'effet immunomodulateur de la flagelline multiplie 20 fois la clairance bactérienne. Le deuxième objectif de ma thèse était d'explorer les mécanismes immunitaires responsables de l'effet protecteur induit par la thérapie. L'analyse transcriptomique du poumon a révélé que les voies associées à l'infiltration des cellules myéloïdes et aux fonctions antimicrobiennes sont induites par la flagelline. Mes résultats montrent également que l'efficacité thérapeutique repose sur les neutrophiles, qui sont mobilisés de manière précoce et transitoire dans les voies respiratoires. Ces neutrophiles sont spécifiquement localisés à la périphérie des bronches, des alvéoles et des vaisseaux pulmonaires et présentent une activité phagocytaire accrue. Enfin, l'analyse par séquençage de l'ARN en cellule individuelle (scRNA-seq) a mis en évidence six sous-populations distinctes de neutrophiles. La flagelline semble reprogrammer les neutrophiles recrutés vers un profil antibactérien, contribuant ainsi à l'efficacité du traitement. En conclusion, ce travail a permis d'identifier les neutrophiles comme des effecteurs clés de la protection et des cibles potentielles pour une thérapie dirigée vers l'hôte, étant donné leur plasticité en réponse au traitement
Streptococcus pneumoniae, or pneumococcus, is the primary cause of community-acquired pneumonia, leading to significant morbidity and mortality, especially in children under five and elderly. Amoxicillin is the first-line treatment; however, increasing resistance to amoxicillin complicates the treatment of these infections and exacerbates this public health issue. Boosting innate immunity, which mobilizes various antimicrobial defense mechanisms and is rapidly activated, is a promising strategy to overcome this issue. An immunotherapy has been developed that combines the standard of care amoxicillin treatment with inhaled flagellin, an agonist of Toll-like receptor 5, which stimulates innate immunity. This combination treatment has proven superior efficacy in mouse pneumonia model compared to antibiotic alone, by reducing bacterial load in the lungs, limiting systemic dissemination and increasing survival rate. The efficacy of the combined therapy with amoxicillin and flagellin was observed in primary pneumonia or superinfection of flu, using either amoxicillin-sensitive or -resistant strains. The combination therapy enhances the innate immune response by significantly increasing neutrophil recruitment into the airways and boosting the production of immune mediators compared to amoxicillin alone. The first objective of my thesis was to investigate the impact of the combination therapy on the selection of antibiotic resistance. For this purpose, a mouse coinfection model using isogenic amoxicillin-sensitive and -resistant strains was established. The study shows that flagellin allows for a 200-fold reduction in the required antibiotic dose while maintaining the same efficacy as the antibiotic alone. Furthermore, the adapted mouse coinfection model mimicking antibiotic resistance selection following antibiotic therapy showed that adjunct flagellin treatment reduces and delays the emergence of antibiotic-resistant bacteria compared to antibiotic treatment alone. These findings suggest that combining amoxicillin with flagellin not only enhances therapeutic efficacy but also mitigates the development of antibiotic resistance. In addition, a mathematical model was developed that captured the lung infection population dynamics, estimating a 20-fold enhancement in the immunomodulatory effect of flagellin on bacterial clearance. The second objective of my thesis was to explore the immune mechanisms underlying the therapy-induced protective effects. Transcriptomic analysis of infected lung tissue revealed that flagellin treatment enhances pathways related to myeloid cell infiltration and antimicrobial functions in the airways. My findings also show that the therapeutic efficacy relies significantly on neutrophils, which are mobilized early and transiently into the airways. These neutrophils were specifically localized at the periphery of bronchi, alveoli, and lung vessels and displayed an increased phagocytic activity. Finally, single-cell RNA sequencing analysis revealed high heterogeneity in neutrophil population with six distinct subsets. Flagellin appears to reprogram recruited neutrophils towards antibacterial profile, contributing to the treatment's efficacy. In conclusion, this work identified the neutrophils as key effectors of protection and potential targets for host-directed therapy, given their plasticity and reprogramming in response to treatment
