Objectives – To ascertain the annual number of users who were discharged home after visits to the emergency department, grouped by age, gender and number of visits during the calendar year, and to assess whether an increasing number of visits to the department predicted a higher mortality. Methods – This is a retrospective cohort study, at the emergency department of Landspitali University Hospital, Reykjavik capital city area, Iceland. During the years of 1995 to 2001 19259 users visited the emergency department, and were discharged home and they were follow-up for cause specific mortality through a national registry. Standardised mortality ratio, with expected number based on national mortality rates was calculated and hazard ratios according to number of visits per calendar year using time dependent multivariate regression analysis were computed. Results – The annual increase of visits to the emergency department among the patients discharged home was seven to 14 per cent per age group during the period 1995 to 2001, with a highest increase among older men. The most common discharge diagnosis was the category Symptoms, signs and abnormal clinical and laboratory findings not elsewhere classified. When emergency department users were compared with the general population, the standardised mortality ratio was 1.81 for men and 1.93 for women. Among those attending the emergency department two times, and three or more times in a calendar year, the mortality rate was higher than among those coming only once in a year. The causes of death which led to the highest mortality among frequent users of the emergency department were neoplasm, ischemic heart diseases, and the category external causes, particularly drug intoxication, suicides and probable suicides. Conclusions – The mortality of users of the emergency department who had been discharged home turned out to be higher than that of the general population. Frequent users of the emergency department had a higher mortality than those visiting the department no more than once in a year. Since the emergency department serves general medicine and surgery patients, not injuries, the high mortality due to drug intoxication, suicide and probable suicide is notable. Further studies are needed into the diagnosis at discharge of those frequently using emergency departments, in an attempt to understand and possibly prevent this mortality

The South African public sector is important for the sustainable growth and development of the country. One of its major responsibilities is to ensure that all citizens have access to and receive services. Achieving a high degree of productivity is an important objective of public service organisations across the world, given the pressure to deliver quality public goods and services within the limits of ever-increasing resource constraints. The South African public service is no exception to this global phenomenon. The challenge of the public service is therefore to continuously improve performance in order to meet citizens’ needs. The focus of this study is the challenges facing service delivery in the Department of Home Affairs. The aim of this study is to explore and describe the challenges that hinder full transformation and maximum customer/client satisfaction in regards to service delivery offered by the Department of Home Affairs. The objectives are: to identify the challenges experienced by staff offering services to clients at the Department of Home Affairs and to identify the problems experienced by clients who receive services at Home Affairs. The study employs a qualitative research methodology and uses observation and individual interviews as data collections tools. The findings from both sources indicate that the clients were not happy with the quality of service they receive at the ID section. Amongst other things, they complained about lack of information, lack of guidance, unprofessional staff, and technical problems that they had experienced. However, the staff also encountered a number of challenges, including being short staffed, lack of resources, and system problems as the system is new to them and they have not received enough training before it was implemented.

The framework of this study is based on the reforms of human resource leave management in the field of public administration for the South African public sector. The researcher’s interest lies in the increasing complexity of leave management development pertaining to policy-making, administrative processes and efficiency in the South African public sector. This study aimed to assess leave management at the Department of Correctional Services (DCS) and the Department of Home Affairs (DHA). The DCS and DHA form part of the seven departments in the Justice, Crime Prevention and Security Cluster. The study aimed to assess the coping strategies of the two departments especially with the services rendered in a tight schedule and constraints. Furthermore, this study was also influenced by the findings of the investigations led by the Public Service Commission (PSC) which aimed to determine effectiveness of leave management in the South African Public Service. The objectives of this study were realised by means of a literature review and research instruments. The literature review was consulted to understand the context of leave management. This study applied the qualitative technique for reporting data. The interviews were used to ascertain the processes of managing leave whereby two sets of questionnaires were developed. The interviews posed the questions to the management of the DCS and DHA and the employees of the two departments. The management of annual leave entitlements is important to maintain the wellbeing of the employees, an organisation’s productivity and to help minimise associated financial implications incurred. The study found that the accrual of leave by public servants is a practice with significant financial implications for the state as employer. If leave is not managed and controlled effectively it can result in substantial losses in terms of unwarranted expenditure. The financial and other implications of leave prompted an investigation by the P S C aimed at determining how effective leave is managed in the Public Service. The study also found that reasonable measures were put in place for managing leave in the public sector. The office of the Health Risk Manager was developed to manage the process for approving the leave of absence as it relates to temporary incapacity leave, permanent incapacity leave and the occupational diseases. The empirical surveys conducted indicated that the two departments are well equipped for managing leave as they are required by labour law. The practices of planning, communication, control measures were also proven to be utilised in the two departments. The two departments render their services in a constrained manner, however the management has attempted to ensure that service delivery is not compromised when some of the members are on leave. Leave entitlements forms part of the accountability framework for the public sector especially when leave is attached to financial resources which are part of public money. In conclusion this study concludes that the public sector must consider the employee assisted systems for applying for leave. Such practices could also enhance the administrative backlogs encountered when recording leaves. Application of leave through such systems may also improve the productivity of the leave officers and reduce the cost of administrative resources.

The hospital emergency department (ED) is a critical source for health care amid a complex healthcare system in the United States. It is the gateway to care for a broad range of people, arriving from a variety of locations. With this wide reaching net and a decreasing trend in hospital beds, EDs throughout the United States are experiencing overcrowding. ED crowding has various tactical and strategic facility management impacts ranging from facility occupancy issues to adverse health outcomes. Among other factors, recent research has cited the sharp increase in ED visits over the years and ED patient boarding as key contributors to crowding. Home hospital care is a model in which health care is delivered at an individual’s home as a substitute for hospital-level inpatient short-term acute care. Clinical research has shown home hospital to be an effective care model for select illnesses presenting frequently to EDs, such as congestive heart failure, community acquired pneumonia, chronic obstructive pulmonary disease, and cellulitis. While there exist distinct clinical and social criteria for which delineate eligible individuals, home hospital care models have been linked with the potential to free inpatient beds. The overarching objective of this study is to investigate the relationship between home hospital care and ED crowding. To achieve this objective, the study examined the relationship between home hospital care and ED crowding, specific to ED boarding performance at a large, urban, teaching hospital facility. A methodology for identification of potential home hospital patients was used through clinical and social criteria, and a scale for the range of clinical eligibility rates was established for the five suitable illnesses. The study modeled patient flow and bed demand, and utilized computer simulation modeling to assess the impact of home hospital care on ED boarding performance. Various models were simulated to represent different home hospital intervention types. The models incorporated home hospital through an ED Referral program, Inpatient-Transfer Referral program, Community Referral program, and a fully integrated home hospital program. Three scenarios were run for each model to assess practical possibilities for the utilization of the freed bed hours from a home hospital program. This research contributes insight and understanding of home hospital’s impacts on ED crowding. The insight from this study quantifies the effects of a home hospital program on ED boarding and inpatient bed demand. The modeling study is contributes an analytical understanding of the impacts that home hospital could potentially have on crowding, which could prove useful in the struggle against ED congestion. This understanding helps to provide a more thorough understanding of home hospital, and could aid in an organization’s decision-making process of whether to implement a program. The presented modeling methodology for analyzing home hospital and ED crowding can also be used as a model format for researchers and practitioners for analytical purposes in future studies.

Background: The overuse of the emergency department (ED) for non- critical patients has been associated with overcrowding and a rise in healthcare cost. Green Valley Fire Department (GVFD) has created a program, Fire-Based Urgent Medicals Service (FBUMS) with a nurse practitioner (NP). Patients can call 9-1-1 or the ""NP hotline"" and request to be seen by the NP instead of being immediately transported to the hospital via ambulance. Purpose: The purpose of this project is to evaluate the impact of the nurse practitioner led FBUMS, on ED visits and ambulance transports. Methods: A survey was mailed to all persons, age 18 and older, who were seen by the NP with FBUMS between February 2017 and March 2017. The survey asked about the reasons for contacting GVFD, the type of treatment received, and whether they went to ED after treatment. Data analysis: Descriptive statistics including frequencies, percentages, means and standard deviations were used to analyze each of the answered survey questions in Microsoft Excel©. Results: Surveys completed (n=42). The majority, 39 (92.9%) stated they did not receive care at the ED within 72 hours following their appointment with the NP, three (7.1%) stated they did. By dramatically decreasing transport to the hospital and associated ED treatment, it is estimated to have saved approximately $53,425 in ambulance costs and $54,210 in ED treatment for a total savings of $99,632.52. Conclusion: A Fire-Based Urgent Medical Service led by a nurse practitioner decreased emergency room visits and ambulance transports.

Aim. The aim of this study was to examine the relationship of advance care planning protocols with hospitalization, rehospitalization, and emergency department use rates in U. S. home health agencies (HHA). Background. Since 2003, CMS has required HHAs to report on quality outcomes such as hospitalization, rehospitalization, and emergency department use rates, made publicly available online. Advance care planning (ACP) is a conversation about beliefs, goals, values, future treatment choices, and designation of a surrogate decision-maker, that someone has in advance of a health crisis. Most existing studies on ACP have taken place outside of HHAs among populations with serious illnesses such as HIV/AIDS, cancer, dementia, and end stage renal disease. Meanwhile, the U.S. home health population is living longer with chronic conditions such as pulmonary and cardiovascular illnesses. Effective January 1, 2016, the Center for Medicare and Medicaid Innovation implemented the Home Health Value-Based Purchasing (HHVBP) Model among home health agencies (HHAs) in nine states representing each geographic region in the United States. Agencies in these states began competing on value in the HHVBP model, and reimbursement rates began to be tied to quality performance (innovation.cms.gov). As part of HHVBP, CMS implemented an additional process-level mandate requiring them to report on ACP, though this data is not publicly available. It is currently unknown how ACP protocols in HHAs may affect agencies’ overall rates of acute care services use. Methods. Electronic surveys about ACP protocols were distributed to HHAs. Existing data about demographics, diagnoses, hospitalization, rehospitalization, and ED use were accessed online via CMS websites. Descriptive and regression analyses were conducted using the electronic survey results and the existing data. Results. Associations between the variables were observed and compared to the hypotheses. Statistical significance was found in the relationship between ACP protocols and hospitalization, where one increased the other increased. Several trends were found: Agencies with increased total percentage of cardiac and pulmonary diagnoses tended to have increased hospitalization rates; agencies with increased average age of patients tended to have increased ACPP scores; and agencies with increased proportion of Black patients tended to have higher hospitalization rates.

This research was undertaken to investigate the challenges faced by employees at the Home Affairs Department, the transport Department and the Department of Education at King Williams Town in the Eastern Cape Province. A quantitative approach was used in this research. The sample consisted of 100 respondents of whom 98 returned completed questionnaires. The answers of the respondents were the data of this study and these were analyzed and interpreted in respect of the hypotheses of the research. The research involved the collection of detailed career, personal and structural perceptions of 98 employees. The data was used to establish the disparity among employees, from lower management to top management. It was found that even though there was a problem of discrimination in the olden days there has been a change in the sense that employment equity, affirmative action and diversity management have been introduced since 1994.

Resumen
The project explores the expression of meaning in architecture against the backdrop of the City of Tshwane Metropolitan Municipality as post-apartheid capital city. The architectural aim of the project is the consolidation of the National Department of Home Affairs and the design of the headquarters of this department. The project starts with a brief exploration of the context of a post-colonial and post-apartheid city, and the aims and identity linked to an African democracy in the context of multiple cultural identities. The search for a national identity is linked to the existential question of ‘being’, which is related to an experiential understanding of physical surroundings. Case studies include recent public buildings that form part of an era of searching for identity and contribute to the discovery of an underdeveloped element of multi-sensory experience in recent architectural projects related to the new democratic government.

The manner in which public sector institutions are structured has an impact on the delivery of effective and efficient services to citizens.Achieving a high degree of productivity is an important objective of public service organisations across the world given the pressure to deliver quality public goods and services within the limits of ever-increasing resource constraints. The South African Public Service is no exception to this global phenomenon. The focus on the public service is therefore continuously to improve performance to meet citizens’ needs. Where as performance is directed at measuring outputs achieved, productivity goes an important step further by measuring the relationship between the resources used to achieve outputs. The focus of this study is the analysis of change and transformation strategies in the Department of Home Affairs, Province of the Eastern Cape. The primary purpose was to assess the current state of service delivery in the Department of Home Affairs and to ascertain how continuous monitoring and evaluation could be factored in to enhance the delivery of services. The target population was the employees and clients of the Department of Home Affairs. Employees were selected randomly and asked to participate in the study. The sample was a convenient-purposive sampling and a quantitative research questionnaire was used.

Avoidable use of the Emergency Departments (EDs) constitutes a significant public health problem, which has health, economic and ethical implications. The factors that affect avoidable use of the EDs are complex and poorly understood. The goal of this dissertation was to examine the role of medical home in avoidable pediatric ED visits, assess the impact of Medicaid policy on ED visits for dental problems and assess the factors affecting follow-up dental care after a dental ED visit. Iowa Household Health Survey data was used for the first study, which included a sample of families with at least one child residing in Iowa. It was found that 68% of parents who took their child to an ED in the previous year thought the ED visit could have been avoided if primary care was available to them. Having a medical home was not found to be associated with pediatric ED visits; however, food insecurity was significantly associated. Parents of children with public insurance, those who were not referred by a healthcare provider and those who reported difficulty in getting routine care appointments were more likely to report an avoidable ED visit by their child. The second study examined a policy change in California where Medicaid eliminated its comprehensive adult dental coverage on July 1, 2009. State Emergency Department Database were obtained from Agency for Healthcare Research and Quality for California for 2006 through 2011. Interrupted time series, a quasi-experimental approach of was used to examine the impact of the policy change on rate of dental ED visits by Medicaid enrolled adults. Segmented linear regression revealed that policy change led to an immediate significant increase in the rate of dental ED visits. The policy had a differential impact on various subgroups based on age, race-ethnicity and residential location. The annual costs associated with dental ED visits made by Medicaid adults also increase 68%. Survival analytic approach was used in the final study to examine the patterns of dental care following a dental ED visit by Medicaid enrolled adults in Iowa. Medicaid claims and enrollment data were used to identify adults with an index dental ED visit in 2011, and then each subject was followed for up to 6 months. About 52% of all adults who satisfied the study inclusion criteria, had a follow up dental visit within 6 months of the index dental ED visit. Cox regression model revealed that adults who had visited a dentist in the year prior to the ED visit had greater hazards of having an early dental follow up after the ED visit. Having repeated dental ED visits was found to have a dose-response relationship to follow-up time to dentist visit, with those having 1 repeat ED visit having 53% hazards and those with 2 or more repeat ED visits having 34% hazards of having a follow-up dentist visit, compared to those with no repeat ED visits. Collectively, the results from this dissertation provide important insights in understanding the complex problem of avoidable ED visits. Factors such as food insecurity and medical home need to be further investigated in their association with avoidable ED visits. State Medicaid policy plays an important role and limiting Medicaid adult dental coverage may lead to an increased reliance of the affected population on EDs for dental care. However, EDs do not provide any definitive dental care, and our results indicate that almost half of the adults with dental ED visit do not have a follow-up dentist visit in the next 6 months.

Purpose: The purpose of this study is to describe the relationship between health literacy, preparedness for discharge, adherence to discharge instructions and difficulty coping after discharge among emergency department patients. Specific Aims: The Aims of this study were to: (1) describe the variability of health literacy of adult patients in an academic tertiary Emergency Department; (2) describe the relationship between health literacy, care transition, and perceived readiness for discharge on the patient’s adherence to discharge instructions and (3) explore whether health literacy, perceived preparation for discharge and care transition, predicts difficulty coping after discharge. Framework: Dr. Meleis’s Transitions Theory was used as a framework. Design: This is a prospective cohort study of adults treated and discharged from the ED. Results: Eighty five percent of the subjects completed the study (n = 132). Subjects satisfied with transition care (P = .025) and who felt more prepared for discharge (P = .035) had less difficulty coping. Subjects more satisfied with care transition were more likely to adhere to medication instructions (P = .029). The higher the satisfaction with discharge preparation, the less likely the subjects were to go to their follow-up appointment (P = 0.051). No associations were found with health literacy. Conclusion: Satisfaction with care transition during the discharge process and feeling well-prepared are related to less difficulty coping after discharge. Nurses have an opportunity to intervene and enhance the discharge experience. This may contribute to more positive outcomes after being seen in an emergency department.

International migration has been a growing phenomenon in the West African community from the late 1960s as the colonial period came to an end and most West African countries gained their independence. During this period this migration trend was essentially from West Africa to Europe facilitated by the relationship that existed between West African states and their former colonial master. In the 1990s Western countries started restricting immigration by applying stricter immigration laws. West Africans who could not make it to the West sought alternative destinations like South Africa where the apartheid regime had just come to an end and the first elected democratic government had been installed in 1994. West Africans in South Africa are mostly economic immigrants and as South Africa's immigration policies changed this group of people faced challenges to acquire the documents required to legalize their stay in the country. This study seeks to analyze why West Africans chose to immigrate to South Africa and specifically Cape Town, their struggle for documentation and the extent to which the possession or non-possession of the correct documents affected their lives in Cape Town. It drew on interviews with Nine West Africans to try and understand this.The study found out that the main reason for West African immigration to South Africa after 1994 was because of the fall of the apartheid regime and the coming to power of the ANC government which re-established diplomatic and economic relations with most West African states. Countries in the West African region were faced with crisis in the 1980s as a result of policies that were implemented in the pursuit to address the ills of colonialism. As the economies of most of these countries declined, most West Africans were faced poverty and became desperate. In their quest for a better live West Africans embarked on immigrating to more developed and affluent countries. Initially they were immigrating to the countries of their colonial masters but with time as more people were immigrating, other destinations in Europe and North America became sought after. Restrictive immigration policies in these countries forced West African to look for new destinations to go to. They found that in South Africa after 1994. Apart from the economic crisis in the West African region, poor governance, corruption, political suppression and tribalism served as push factors in contributing to the immigration of West Africans to South Africa. As pull factors, the reliance on kinship played an important part in most West Africans who immigrated to South Africa. Many of them relied on the friends and family who were resided in South Africa for information, directions and support to make the journey and eventually join them were they are based. The picture and information that the mass media projected about South Africa also had a huge impact on the perception people had about the country. These perceptions contributed to the decision of West Africans to immigrate to South Africa. This study explored the range of visas West Africans sought to enter the country. It found that the visa they chose to apply for was not necessarily the real purpose of their visit but was the most convenient. This allowed them entry into the country and once in they sought other visas and permits to legalise their stay. This study found that it was a long hard journey to convert asylum permits to refugee permits Further the struggle over documentation was mainly because of the logistical short comings of the Department of Home Affairs. This study argues that it is the Department of Home Affairs that renders many of them illegal due to delays in processing the permits. .While they waited for documents, the lives of applicants were full of insecurity and there were difficulties in making a living. The informal sector provided one avenue for some. This thesis argues that applicants were desperate for the correct documents but even though they eventually acquired them these did not necessarily open up opportunities.

Language plays an important part in our lives and the effects of language are remarkable and include much of what people believe. This study is about language and seeks to evaluate the extent of the problems of equivalence and non-equivalence in the texts translated from English into IsiXhosa by identifying those problems in the translated text. Translators strive to carry the meaning across from one language to another, as much as they can. Sometimes a translation shows non-equivalence and therefore the meaning gets lost. There are a number of reasons why this occurs. This study will examine some of them and propose solutions to address them.

This thesis evaluated the effectiveness and cost-saving potential of a health service delivery model in Queensland Australia, the Hospital in the Nursing Home program. The research adopted a before-after controlled study design, comparing the outcomes between an intervention hospital and a control hospital during the pre- and post-intervention periods. Findings from the research supported that the intervention was preferred over the current practice, which reduced the attendances to emergency departments (EDs) and inpatient hospitals from patients in residential aged care facilities, shortened their length of ED stay, and saved the overall costs.

Varje år åker Sveriges räddningstjänster på cirka 6000 larm om brand eller brandtillbud i bostäder med följd att även cirka 100 personer omkommer årligen i sina hem. Att det är cirka 100 personer som omkommer varje år är något som ses som ett problem då det oftast är okunskap eller begränsande möjligheter att undsätta sig själv i säkerhet som ligger bakom bränderna och dödsfallen. Att som räddningstjänst utföra hembesök med att åka hem till olika personer och prata om brandskydd och risker i hemmet är något som visats vara väldigt effektivt i Storbritannien. Varje år gör räddningstjänster runt om i Storbritannien cirka 670 000 hembesök i syfte att öka kunskaper och frekvensen av brandvarnare i hemmen. Arbetet med hembesök påbörjades i slutet av 90-talet och har fram tills nu resulterat i att antalet insatser och dödsfall till följd av brand har mer än halverats. I Sverige är det förebyggande arbetet med hembesök inte lika utbrett men flera räddningstjänster har under de senaste åren startat upp och påbörjat genomförandet av hembesök. Myndigheten för samhällsskydd och beredskap, MSB, uppmanar fler räddningstjänster i Sverige att påbörja ett förebyggande arbete själva eller tillsammans med andra räddningstjänster för ökad riskmedvetenhet, kunskap och brandvarnarfrekvens i hemmen. Räddningstjänsten Syd är en av räddningstjänsterna i Sverige som arbetat med hembesök under lång tid och som kunnat visa effekter och statistiska skillnader på att hembesök verkligen gör nytta. Räddningstjänsten Syd har visat en nedåtgående trend på antalet insatser och har en lägre genomsnittlig dödsbrandsstatistik än den samlade dödsbrandsstatistiken för övriga Sverige. Rapporten grundar sig i en förstudie och en intervjustudie där 10 intervjuer genomförts med 8 olika räddningstjänster runt om i Sverige. De intervjuade har olika yrkesroller inom räddningstjänsten som antingen arbetat administrativt eller som utförare av hembesök. Hembesök genomförs främst av operativ personal där den skiftgående styrkan ägnar några timmar i veckan eller månaden på hembesök. Hur hembesök påverkar den operativa förmågan upplever samtliga respondenter som positiv då brandmännen får en bra områdesöversikt och grundläggande kunskaper i byggnadens utformning. Kunskaperna kan sedan visas vara värdefull vid eventuella insatser och insatsens verkan. Inställningen till att arbeta förebyggande med hembesök är något som varierar bland den operativa personalen där det upplevs att det finns motståndare till sådana här typer av arbetsuppgifter. Samtliga respondenter anser att hembesök är något som räddningstjänsten ska fortsätta att arbeta med tillsammans med andra förebyggande åtgärder. Hembesök utgör en viktig del i det förebyggande arbetet men att bara fokusera på hembesök är inte rätt lösning enligt många av respondenterna. Att arbeta med mer riktade och anpassade hembesök mot olika riskgrupper i samhället är något som räddningstjänsterna har som målsättning men att det då krävs en bättre samverkan mellan olika kommunala aktörer för arbetets effekt. Många av respondenterna menar att det administrativa arbetet med hembesök är något som är ganska besvärligt och tidskrävande att hantera då det är mycket information och statistiska uppföljningar som behandlas. Tekniska lösningar kopplade till redan befintliga programvaror är något som räddningstjänsterna vill se framöver för att kunna underlätta planering och administrering. Hur arbetet med hembesök ska kunna förbättras för att komma åt dödsbränder i samhället finns det förslag på förbättringsåtgärder, till exempel att i samband med hembesök även arbeta med individanpassat brandskydd eller tillsyner. Tillsyner är något som aldrig görs mot bostäder men som genomförs på hotell, köpcentrum, industrier etc. Individanpassat brandskydd innebär att brandskyddet anpassas efter individens förutsättningar till ett säkert boende exempelvis personer med olika typer av handikapp eller kognitiva nedsättningar.

Early childhood has been identified as a critical period for providing nutritional intervention, with nutritional adequacy during the first 1000 days having long term implications for human development. South Africa’s policy environment accordingly aims to support the development of all children through providing services supporting care and nutrition of children so that ‘no one is left behind’. However, the reality is that for the economically marginalised who live in poverty, these services are inaccessible and the whereabouts of many children, especially those under 5, remain unknown to the state. This study looks at township childcare facility as a key intervention point for nutrition provision, documents the obstacles and challenges they face in securing food for the children in their care and describes the strategies they use to combat these challenges. Using a mixed method approach, data were gathered on all ECDs operating in two Cape Town townships – Vrygrond, a semi-formal township, and Sweet Home Farm, a deeply informal settlement – and a typology was developed which represented the differentiation between these informal businesses in terms of a continuum of connectedness and disconnectedness with the regulatory environment.

The Affordable Care Act of 2010 proposed a new integrated care model for individuals with chronic mental illness and multiple medical comorbidities by using Behavioral Health Homes (BHH). The purpose of this doctoral project was to evaluate the effectiveness of the BHH in reducing emergency department (ED) use in the community mental health outpatient setting. Guided by the American Association for Critical Care Nurses synergy model for patient care a cross-sectional, post-test only with comparison group quasi-experimental research design was used. A de-identified data set of 68 patient records in the BHH group and 73 patient records in non-BHH as a control group were analyzed using logistic regression. The analysis revealed that participants in the BHH were statistically less likely to visit the ED. Sensitivity was 16.2%, specificity was 95.2%, positive predictive value was 54.5%, and negative predictive value was 76.2%. The Homer-Lemeshow and omnibus test of model coefficients showed the model was a good fit (p=.726, p=.007). Participants in BHH were .225 less likely to visit the ED. For every year of reduction in age, the odds of visiting the ED increased by a factor of 1.0. Females had a 1.8 higher odds of visiting the ED than males. This study provides evidence for the effectiveness of the BHH in reducing visits to the ED. Standard measures to track ED use in BHH are essential to understanding reasons for ED use and reducing nonurgent use. The BHH has the potential to transform health care delivery toward an all-inclusive model of care. Providers can utilize the findings of this project to promote social change by targeting patients with serious mental illness and reducing health disparities by emphasizing preventive care and eliminating barriers to care.

This thesis focuses on the emergence of the teenaged consumer as a market segment in Canada during the 1930s, 1940s, and 1950s. It challenges the notion that teenagers were of little interest to retailers until economics and demographics shaped the more numerous and prosperous post-war teenagers of the Baby Boom generation. Using evidence from corporate records and analysis of mail order catalogues, the study examines how department store retailer, the T. Eaton Company, Limited, began to cultivate a distinct and lucrative teenaged consumer in the 1930s, and thereby began shaping the teenaged consumer. The thesis contextualizes the case study of Eaton’s by exploring the varied expectations that adults had of young people at the time, using census records and magazines (Chatelaine, Canadian Home Journal and Mayfair) to explore concerns about young people’s transition to adulthood. It then focuses on how Eaton’s made a concerted and sustained effort to attract teenager customers to its catalogue and stores. Analysis of its semi-annual catalogue highlights the emergence of specialized clothing size ranges and styles, revealing that Eaton’s increasingly viewed the teenaged years as an important in-between life stage. Eaton’s also instituted teenage advisory councils to both glean market trends and provide a venue for what it considered education for novice consumers. Eaton’s presented consumption as a way to prepare young people for adult roles, legitimizing teenaged participation in the consumer marketplace and contributing to wider debates about when and how teenaged Canadians should reach maturity.

This study evaluated specific social and demographic factors and the bearing they had on successful rehabilitation outcome.

The purpose of this study was to assess the effects of participation in a family coaching program on perceptions of parental self-efficacy, families' use of encouragement, and reinforcement behaviors. A second purpose of the study was to ascertain the lasting effect on families participating in a family involvement intervention that included coaching. Families with prekindergarten and/or kindergarten children attending school in a low-income neighborhood and neighboring child care centers were recruited for the study. The Family Coaching Institute, the family involvement intervention for this study, consisted of three 5-week, 2-hour biweekly sessions. Attendance ranged from 3 to 15 sessions. Child care, dinner, learning activities, materials, books and supplies were provided. Participants were encouraged to use the activities at home with their children between sessions. Pre-intervention and post-intervention interviews were conducted with the participants using scales designed to measure parental self-efficacy, encouragement, and reinforcement behaviors. Family members also participated in a focus group and completed the Family Involvement Learning Survey 6 months after the intervention. Results of the study indicated there were no statistically significant differences in responses from the beginning to the end of the intervention on the scales designed to measure parental self-efficacy, encouragement, and reinforcement behaviors. These findings are discussed in the context of a response shift bias. In contrast, ratings on the Family Involvement Learning Survey indicated participation in the intervention had a strong impact on family behaviors.

The fragmented approaches to delivering health care services in the United States, along with the associated structural inefficiencies and unsustainable increases in health care costs affecting all payers, compel the need for reform. Various federal and state-level delivery system reform models have emerged in response. The Medical Home (MH) is one of such reform models. In 2004 a national initiative entitled "The Future for Family Medicine Project" identified the lack of emphasis on comprehensive primary care, especially for chronic care patients, and proposed the introduction of MHs to improve comprehensive primary care delivery for every patient. Oregon's MH variant, the Patient-Centered Primary Care Home (PCPCH), was introduced in 2009 as part of a state-wide health reform initiative ushered in by the passage of House Bill 2009 to promote the Triple Aim. Since 2011, over 600 primary care clinics have been recognized as PCPCHs. Proponents of the model argued that it will help improve comprehensive primary care services upstream and reduce inappropriate utilization of Emergency Department (ED) and Inpatient (IP) care and expenditures downstream. Evidence on the model's application to reduce ED and IP utilization and expenditures have so far been mixed. Based on growing interests in the effects of the model's application to provide care for different types of patients, this research was designed to evaluate the policy effects of the application of PCPCHs, with a focus on PCPCHs that treat greater proportions of chronic care patients, to answer the following questions: (1) What is the average chronic disease burden of PCPCHs, and how does their average chronic disease burden compare to the communities PCPCHs are in pre-post PCPCH recognition? (2) How do primary care expenditures change based on the chronic disease burden of PCPCHs? (3) Do PCPCHs that engage more high chronic disease burden patients have more reductions in ED and IP expenditures? For this research, a chronic disease burden measure was developed from 10 markers of chronic conditions. This measure was then used to stratify PCPCH clinics and their comparators into high and low chronic disease burden clinics. The research was designed as a natural experiment, utilizing difference-in-difference methods to measure outcome differences pre-post PCPCH policy implementation and comparing outcome differences between PCPCHs and their control groups. The unit of analysis was PCPCH clinics. The theoretical perspectives that informed this research were Risk Selection and Complex Adaptive Systems (CAS). Data from Oregon's All Payer All Claims (APAC) data system, which included 16 quarters of claims and eligibility data from fourth quarter 2010 to third quarter 2014, as well as PCPCH attestation data on 525 clinics were utilized for this research. The results suggest that the chronic disease burden for PCPCHs was significantly lower than their comparator groups before clinics recognition as PCPCHs, but the chronic disease burden did not change after clinics recognition as PCPCHs. Average primary care expenditures did not change after PCPCH recognition. Average ED and average IP expenditures for high chronic disease burden PCPCHs did not change but rather decreased significantly for low chronic disease burden PCPCHs. The results imply that the distribution of chronic disease burden in PCPCHs is important and related to ED and IP expenditures, but in a different direction than expected. The results also suggest that focusing on low chronic disease burden patients in PCPCHs could help reduce ED and IP expenditures in the short and medium terms. Policies to engage a broader mix of chronic disease burden patients in PCPCHs could help increase savings from ED and IP utilization. The results also suggest the need for more research to improve current understanding of how PCPCHs are impacting health care trajectories in the current delivery system environment.

Tracing the influences and references to the Muses in written language from Ancient Greece through the end of the English Renaissance, I discover transformations and revivals in their usage. There are shifts from dependence on deified inspiration to the development of personal insight. Also, there appears to be a conscious substituting of the Muses with the beloved and Cupid or Apollo. But the Muses' religious significance returns in Paradise Lost. The first part of this thesis focuses on the early Greek writers: Homer, Hesiod, Pindar, Plato, and the Latin writers: Ovid, Virgil, Boethius. The second part addresses the English poetic tradition from Chaucer through Milton. The poets cited for this section are Chaucer, Spenser, Sidney, Shakespeare, Donne, Marlowe and Milton. Presentations of the Muses or a personally chosen muse during these literary periods display conceptions of what originally motivates literary creation. I cover both the epic and lyric poetic traditions.

This study investigates the first Woman's Department at a World's Fair in the Deep South. It documents conflicts and reconciliations and the reassessments that post-bellum women made during the World's Industrial and Cotton Centennial Exposition in New Orleans, the region's foremost but atypical city. It traces local women's resistance to the appointment of northern abolitionist and suffragist, Julia Ward Howe, for this “New South” event of 1884-1885. It also notes their increasing receptivity to national causes that Susan B. Anthony, Frances E. Willard, and others brought to the South, sometimes for the first time. This dissertation assesses the historical forces that goaded New Orleans women, from the comfort of their familiar city, to consider radical notions that would later strengthen them in civic roles. It asserts that, although these women were skilled and capable, they had previously lacked cohesive force and public strategies. It concludes that as local women competed and interacted with women from across the country, including those from pioneering western territories, they began to embrace progressive ideas and actions that, without the Woman's Department at the Exposition, might have taken years to drift southward. This is a chronological tale of the journey late-nineteenth-century women made together in New Orleans. It attempts to capture their look, sound, and language from their own writings and from journalists' interpretations of their ideals, values, and emotions. In the potent forum for exchange that the Woman's Department provided, participants and visitors questioned and revised false notions and stereotypes. They influenced each other and formed alliances. Although individuals spoke mainly for themselves, common themes emerged regarding education, jobs, benevolence, and even suffrage. Most women were aware that they were in a defining moment, and this study chronicles how New Orleans women seized the opportunity and created a legacy for themselves and their city. As the Exposition sought to (re)assert agrarian and industrial prowess after turbulent times, a shift occurred in the trajectory of women's public and political lives in New Orleans and, perhaps, the South more broadly. By 1885, southerners were ready to insinuate their voices into the national debate on women's issues.

The 1994 democratic government of South Africa brought greater responsibility and growth of school-based management in schools which impacted on the role and workload of school leaders. Principals of public foundation phase school teachers delegated greater responsibility to Heads of Departments who had to ensure that teachers are professionally developed. The assumption was that teachers should be supported and developed professionally to do their best work with learners, and, to be retained. Despite the belief that school-based Teacher Professional Development may have a positive impact, little is known about the challenges Heads of Departments’ of Tshivenda Language in a rural context face in managing teachers for them to promote quality teaching and learning. The aim of this study was to investigate the challenges that heads of departments face in managing the teaching of Tshivenda home language in rural primary schools of Dzindi circuit in Vhembe District. This study was positivist in nature following the quantitative research methodology. Self-administered questionnaires were used to collect data from 107 respondents from the 200 who were sampled through simple random sampling procedure. Several challenges that face the HODs were identified. These include the need to supervise teachers in the process of teaching to improve learning in schools. The study points to a strong need for workshops to fully capacitate HODs of Tshivenda in rural primary schools of Dzindi Circuit. However, the various management challenges that have emerged should be considered. Mentoring of Tshivenda Home Language teachers by HODs remains very crucial for the promotion of teaching and learning in rural primary schools of Dzindi Circuit. The study recommends a need for HODs to motivate teachers in their management process.

At the World's Industrial and Cotton Centennial Exposition, the Woman's Department offered women of all regions of the country an opportunity to exhibit what they considered "woman's work." As women came together and attempted sectional reconciliation, controversy persisted, especially over the selection of northern suffragist Julia Ward Howe, author of the "Battle Hymn of the Republic," as the Department's president. However, during the course of the event, which lasted from December 16, 1884 to May 31, 1885, New Orleanians and other southern women learned skills and strategies from participants and famous women visitors, and these southerners insinuated their voices into the national debate on late-nineteenth-century women's issues.

Resumen

This thesis is structured in six chapters. Chapter I contains an introduction and includes purpose, theory, method, and concepts. The main purpose, as depicted by the title, is to examine the roots of Swedish ideology concerning what today is generally named design, as embodied in the concept of more beautiful or better things for everyday life (in Swedish: ”vackrare vardagsvara”).

Chapter II contains a background and includes philosophical ideas and aesthetic movements in Europe which have influenced the Swedish Society of Arts and Crafts (in Swedish ”Svenska Slöjdföreningen”, abbreviated SSF) which was later renamed the Swedish Society of Crafts and Design (in Swedish: ”Föreningen Svensk Form”). It considers these activities: the Arts and Crafts movement in England, the Swedish national romantic movement, Deutscher Werkbund in Germany, and Swedish moulders of public opinion and new ideas, like Ellen Key, Carl Larsson and Gregor Paulsson.

Chapter III is an ideological biography of Gregor Paulsson. The chapter deals with biographical data and ideological development, and the social aesthetical texts which were important in his activity in the National Museum and as director of The Swedish Society of Arts and Crafts. Gregor Paulsson is considered mainly in his role as social aesthetical propagandist and museologist.

Chapter IV concerns the early history and activities of the Swedish Society of Arts and Crafts seen as an introduction to the Baltic Exhibition 1914, and the subsequent schism which eventually led to its reorganization and a new ideological orientation. Its activities were directed towards increased cooperation between artists and industry, and a special department was established as an employment office for companies and designers under the management of the textile artist Elsa Gullberg. This chapter also includes a brief portrait of key persons in the Society.

Chapter V is a study in several sections of the articles for everyday use seen in industrial practice, with Gustavsberg’s china factory and Orrefors’ glassworks as two separate historical studies. The 1917 Home Exhibition is surveyed as an example of the educational ambitions in the development of people’s taste. The focus of the chapter, however, is the international industrial art exhibition in Paris 1925, Exposition International des Arts Décoratifs et Industriels Modernes, and the debate about it in the Swedish and French press.

Chapter VI consists of a concluding discussion with a final epilogue. It contains suggested questions for future research including relations between design and ethics.
