Resumen

Virtual reality (VR) is a valuable tool for the treatment and prevention of psychiatric disorders and dysfunctional behaviors. Although VR software is mainly developed following a disorder-specific approach, this randomized controlled trial (RCT) will test the efficacy of a new transdiagnostic VR application (H.O.M.E. VR-based psychological intervention) in improving dysfunctional behaviors, three transdiagnostic factors concurrently (emotion regulation, experiential avoidance, and psychological flexibility), and stress. Three groups screened as at-risk for nicotine dependence, alcohol abuse, and eating disorders will be assigned to the H.O.M.E. VR intervention and compared to a waiting-list (WL) condition. Participants will be assessed before and after the H.O.M.E. intervention/WL and at the 3- and 6-month follow-ups in the levels of the displayed dysfunctional behavior, the three transdiagnostic factors, and stress. Changes in dysfunctional behaviors, transdiagnostic factors, and stress in each population VR group and differences in such improvements between each population of the VR and WL groups will be evaluated using mixed-model repeated measure analyses of variance. It is expected that, after the H.O.M.E. intervention and at follow-ups, participants will display improvements in physical and psychological health compared to controls. The H.O.M.E. protocol is expected to result in a cost-effective option to tackle cognitive–behavioral factors shared among several psychopathologies and dysfunctional behaviors.

Resumen

Implemented in 2002 by the Duke-Durham Neighborhood Partnership, Project H.O.P.E. has improved the quantity and quality of afterschool programs for the youth of Durham, NC. Project H.O.P.E. provides tutoring programs, enrichment resources, and evaluation support to non-profit community partner organizations located in the low income Durham neighborhoods surrounding Duke University. Duke University undergraduates who provide tutoring services to the Durham youth in the afterschool programs gain from valuable reciprocal service learning experiences. Project H.O.P.E. is an effective model of the mutual benefits that can be gained from effective university and community engagement in the service of at-risk students.

Resumen

The Farm Management and Technology Program (FMT) is a 3-year post-secondary vocational agriculture program. FMT students may choose to specialize in horticulture. Since January 1995, all horticulture students have been involved in a hands-on, practical educational experience called “H.O.R.T.” (Horticultural Opportunities for Real Training). The students operate a small horticultural “business.” They must plant, maintain, harvest, and sell several horticultural crops, including greenhouse and field-grown vegetables, apples, berries, and potted flowers. H.O.R.T. lasts two semesters: January through April and September through December. Students may choose to do H.O.R.T. for 2 years to broaden and deepen their horticultural learning. Through active participation in H.O.R.T., students will achieve the technical competencies required by the FMT program and specified by Quebec's ministries of education and agriculture. Each year of H.O.R.T. counts for 5-2/3 credits out of a program total of 90-1/3 credits. The goals of H.O.R.T. are not so much the acquisition of “book knowledge” (lower part of the cognitive domain) as the development of technical skills, planning and decisionmaking abilities, business sense, and proper communication (higher-order cognitive skills as well as psycho-motor and affective skills).

Ageing at home is the expressed wish of the fastest growing older adult population. Life care, not just medical/health care, can holistically address care needs of this population. Receiving care at home provides the best opportunity for care providers to address a person’s care needs within their natural environment- the place they call home. With heightened concerns of COVID-19 transmission, older audits are hesitant to have multiple care providers in the home and staff are overwhelmed with work as several team members are off due to personal and/or medical reasons. Coupled with the current state of the Canadian homecare practice setting at the system level, a change is needed. The pandemic became the catalyst to transform/evolve the way SE Health provides care for the betterment of our clients, staff and organization- thus spearheading the H.O.P.E.- Home, Opportunity, People, Empowerment model of care. This leading organizational model of care provides nurses with optimal autonomy and flexibility to deliver best life, home and community care. Empowerment provides nurses with optimal autonomy and flexibility to deliver excellent life care with clients and their circle of care members. Caseload management and self-directed scheduling are unique home and community care competencies that are fostered and supported by optimization of operational processes. Therefore, operationalization of nursing practice in this model requires both optimization of Service Coordination function and digital enablement. One digital innovation, MySE Life App was created to address this need. The MySE Life App is a bespoke digital application designed by nurses, for nurses that provides access to key information to support holistic care delivery in a single application. The tool empowers informed decision making around planning, coordination, and delivering exceptional quality care by giving nurses access to client information on demand. The components of H.O.P.E. Model of Care brings homecare back to basics and care back to the future. To bring this ambition to life, we employed implementation science to bring rigor to the implementation approach and support the change management of over 8,000 leaders across the organization nationally. As Primary Nursing is the core of this organizational model, first year of implementation has focused on optimizing scope of nursing practice and continuity of care. H.O.P.E. nursing competency development through a Novice to Expert journey was also undertaken. Three key success insights will be shared with real life implementation scenarios: 1) empowering the nurse and the client, 2) implementation science informed execution, and 3) operationalizing nurse practice with enabling technology and redesigned structures, processes, and leadership. Learning insights can inform healthcare organizations in how creating a nurse empowered practice culture impacts client safety, quality care and sustainable business outcomes. As the implementation moves towards sustainability in nursing, the learnings will also be applied to accelerate the implementation across other health care disciplines such as personal support workers and allied health disciplines in the organization. The implementation of H.O.P.E. Model of Care illustrates how home and community care can work better for people and the broader system in a resource constrained environment.

Resumen

No presente estudo é desenvolvido um modelo numérico para a abordagem de escoamentos turbulentos no regime permanente em bocais do tipo H.O.M.E.R (do inglês: High-speed Orienting Momentum with Enhanced Reversibility) que consiste na mistura de dois jatos incidentes sobre superfícies de Coanda. Essa superfície causa uma deflexão no escoamento permitindo que o bocal atue como um dispositivo de manobra em aplicações aeronáuticas. O principal objetivo é avaliar o modelo numérico desenvolvido comparando com resultados numéricos da literatura. As equações de conservação de massa e quantidade de movimento médias no tempo são resolvidas numericamente através do método de volumes finitos. Para resolver o problema do fechamento da turbulência foi empregada modelagem clássica (RANS – do inglês: Reynolds Averaged Navier Stokes) com modelo k – ε. Primeiramente, um teste de independência de malha será realizado, com o intuito de dispender menos recursos computacionais e obter resultados precisos. Em seguida, serão feitas as simulações em regime permanente, com o objetivo de obter os ângulos de deflexão (α) gerados com a diferença das vazões mássicas injetadas em cada um dos canais do bocal H.O.M.E.R (m*). Posteriormente, esses resultados são comparados com os obtidos numericamente na literatura. Os resultados obtidos tiveram o mesmo comportamento obtido na literatura, onde o aumento da diferença entre os jatos de entrada conduziu a um aumento no ângulo de deflexão do jato no bocal. Com exceção de um valor específico (m* = 0.2) os resultados obtidos no presente trabalho apresentaram uma boa concordância com os preditos numericamente na literatura.

Resumen

In the present work it is performed a numerical study for simulation of turbulent air and water flows in a nozzle based on the Coanda effect named H.O.M.E.R. (High-Speed Orienting Momentum with Enhanced Reversibility). The main purposes of this work are the development of a numerical model for simulation of the main operational principle of the H.O.M.E.R. nozzle, verify the occurrence of the physical principle in a device using water as working fluid and generate theoretical recommendations about the influence of the difference of mass flow rate in two inlets and length of septum over the fluid dynamic behavior of water flow. The time-averaged conservation equations of mass and momentum are solved with the Finite Volume Method (FVM) and turbulence closure is tackled with the k-ε model. Results for air flow show a good agreement with previous predictions in the literature. Moreover, it is also noticed that this main operational principle is promising for future applications in maneuverability and propulsion systems in marine applications. Results obtained here also show that water jets present higher deflection angles when compared with air jets, enhancing the capability of impose forces to achieve better maneuverability. Moreover, results indicated that the imposition of different mass flow rates in both inlets of the device, as well as central septum insertion have a strong influence over deflection angle of turbulent jet flow and velocity fields, indicating that these parameters can be important for maneuverability in marine applications.

During the last 20 years there has been a worldwide effort to develop the physics and technology of linear colliders. Present goals at SLAC, KEK and DESY are to bring the R&D efforts to the point where proposals for 500/1000 GeV cms electron–positron colliders can be officially submitted in the years 2002/2003. The CLIC study at CERN aims at a second generation very high energy electron–positron collider, to be considered after completion of the LHC. The main areas of hardware R&D include efficient accelerating waveguides without harmful higher order mode (h.o.m) effects, high peak power klystrons, klystron modulators and rf-power compression. Test facilities have been put in place for the testing of h.o.m behavior of new waveguide designs (ASSET), focusing on low emittance beams to spot sizes in the nanometer range (FFTB) and damping particle oscillations in a special damping ring (ATF) to prepare low emittance bunch trains of electrons for injection into linear colliders. The TESLA collaboration is making a major effort to develop the required technology for a superconducting linear collider. Test accelerator sections, which employ all the necessary new accelerator components, have been built and are currently being tested at SLAC and DESY.

Desde la introducción de la válvula Charlotte1 como una adaptación alternativa y compasiva para una oxigenación CPAP2 no aerosolizante en pacientes con COVID-19, varios informes de Italia, España y otros países muestran que la adaptación de la máscara de snorkel no solo funcionó como se esperaba sino evitó que también los pacientes se deterioren cuando se usaba adecuadamente en etapas tempranas e intermedias de la enfermedad recién descubierta.

Resumen

Elderly with Alzheimer's disease may experience various adverse effects on physical and psychological functions, especially depression level. Effective intervention may reduce depressive symptoms. Therefore, the current study aims to investigate the effect of individualized reminiscence therapy on depression levels among the elderly with Alzheimer's disease. The findings showed that individualized-reminiscence therapy significantly affects the elderly with Alzheimer's depression level. In conclusion, reminiscence was beneficial through stressful life changes, reduce depressive symptoms, improving life satisfaction and having a better quality of life among the elderly with Alzheimer's disease.Keywords: Alzheimer’s disease; Depression; Reminiscence therapy eISSN: 2398-4287 © 2019. Cancer screening rates among American Indian men remain low, without programs specifically designed for men. This paper describes the Community-Based Participatory Research processes and assessment of cancer screening behavior and the appropriateness of the mHealth approach for Hopi men’s promotion of cancer screenings. This Community-Based Participatory Research included a partnership with H.O.P.I. (Hopi Office of Prevention and Intervention) Cancer Support Services and the Hopi Community Advisory Committee. Cellular phone usage was assessed among male participants in a wellness program utilizing text messaging. Community surveys were conducted with Hopi men (50 years of age or older). The survey revealed colorectal cancer screening rate increased from 51% in 2012 to 71% in 2018, while prostate cancer screening rate had not changed (35% in 2012 and 37% in 2018). Past cancer screening was associated with having additional cancer screening. A cellular phone was commonly used by Hopi men, but not for healthcare or wellness. Cellular phone ownership increased odds of prostate cancer screening in the unadjusted model (OR 9.00, 95% CI: 1.11–73.07), but not in the adjusted model. Cellular phones may be applied for health promotion among Hopi men, but use of cellular phones to improve cancer screening participation needs further investigation.

Abstract STUDY QUESTION Can markers of human endometrial hypoxia be detected at menstruation in vivo? SUMMARY ANSWER Our in vivo data support the presence of hypoxia in menstrual endometrium of women during physiological menstruation. WHAT IS KNOWN ALREADY Current evidence from animal models and human in vitro studies suggests endometrial hypoxia is present at menstruation and drives endometrial repair post menses. However, detection of human endometrial hypoxia in vivo remains elusive. STUDY DESIGN, SIZE, DURATION We performed a prospective case study of 16 women with normal menstrual bleeding. PARTICIPANTS/MATERIALS, SETTING, METHODS Reproductively aged female participants with a regular menstrual cycle underwent objective measurement of their menstrual blood loss using the alkaline haematin method to confirm a loss of <80 ml per cycle. Exclusion criteria were exogenous hormone use, an intrauterine device, endometriosis or fibroids >3 cm. Participants attended for two MRI scans; during days 1–3 of menstruation and the early/mid-secretory phase of their cycle. The MRI protocol included dynamic contrast-enhanced MRI and T2* quantification. At each visit, an endometrial sample was also collected and hypoxia-regulated repair factor mRNA levels (ADM, VEGFA, CXCR4) were quantified by RT-qPCR. MAIN RESULTS AND THE ROLE OF CHANCE Women had reduced T2* during menstrual scans versus non-menstrual scans (P = 0.005), consistent with menstrual hypoxia. Plasma flow (Fp) was increased at menstruation compared to the non-menstrual phase (P = 0.0005). Laboratory findings revealed increased ADM, VEGF-A and CXCR4 at menstruation on examination of paired endometrial biopsies from the menstrual and non-menstrual phase (P = 0.008; P = 0.03; P = 0.009). There was a significant correlation between T2* and these ex vivo hypoxic markers (P < 0.05). LIMITATIONS, REASONS FOR CAUTION This study examined the in vivo detection of endometrial hypoxic markers at specific timepoints in the menstrual cycle in women with a menstrual blood loss <80 ml/cycle and without significant uterine structural abnormalities. Further research is required to determine the presence of endometrial hypoxia in those experiencing abnormal uterine bleeding with and without fibroids/adenomyosis. WIDER IMPLICATIONS OF THE FINDINGS Heavy menstrual bleeding (HMB) is a common, debilitating condition. Understanding menstrual physiology may improve therapeutics. To our knowledge, this is the first in vivo data supporting the presence of menstrual hypoxia in the endometrium of women with normal menstrual bleeding. If aberrant in those with HMB, these non-invasive tests may aid diagnosis and facilitate personalized treatments for HMB. Abstract STUDY QUESTION Does the oestrogen receptor isoform, ER46, contribute to regulation of endometrial function? SUMMARY ANSWER ER46 is expressed in endometrial tissues, is the predominant ER isoform in first trimester decidua and is localised to the cell membrane of uterine natural killer (uNK) cells where activation of ER46 increases cell motility. WHAT IS KNOWN ALREADY Oestrogens acting via their cognate receptors are essential regulators of endometrial function and play key roles in establishment of pregnancy. ER46 is a 46-kDa truncated isoform of full length ERα (ER66, encoded by ESR1) that contains both ligand- and DNA-binding domains. Expression of ER46 in the human endometrium has not been investigated previously. ER46 is located at the cell membrane of peripheral blood leukocytes and mediates rapid responses to oestrogens. uNK cells are a phenotypically distinct (CD56brightCD16−) population of tissue-resident immune cells that regulate vascular remodelling within the endometrium and decidua. We have shown that oestrogens stimulate rapid increases in uNK cell motility. Previous characterisation of uNK cells suggests they are ER66-negative, but expression of ER46 has not been characterised. We hypothesise that uNK cells express ER46 and that rapid responses to oestrogens are mediated via this receptor. STUDY DESIGN, SIZE, DURATION This laboratory-based study used primary human endometrial (n = 24) and decidual tissue biopsies (n = 30) as well as uNK cells which were freshly isolated from first trimester human decidua (n = 18). PARTICIPANTS/MATERIALS, SETTING, METHODS Primary human endometrial and first trimester decidual tissue biopsies were collected using methods approved by the local institutional ethics committee (LREC/05/51104/12 and LREC/10/51402/59). The expression of ERs (ER66, ER46 and ERβ) was assessed by quantitative PCR, western blot and immunohistochemistry. uNK cells were isolated from first-trimester human decidua by magnetic bead sorting. Cell motility of uNK cells was measured by live cell imaging: cells were treated with 17β-oestradiol conjugated to bovine serum albumin (E2-BSA, 10 nM equivalent), the ERβ-selective agonist 2,3-bis(4-hydroxyphenyl)-propionitrile (DPN; 10 nM) or dimethylsulphoxide vehicle control. MAIN RESULTS AND THE ROLE OF CHANCE ER46 was detected in proliferative and secretory phase tissues by western blot and was the predominant ER isoform in first-trimester decidua samples. Immunohistochemistry revealed that ER46 was co-localised with ER66 in cell nuclei during the proliferative phase but detected in both the cytoplasm and cell membrane of stromal cells in the secretory phase and in decidua. Triple immunofluorescence staining of decidua tissues identified expression of ER46 in the cell membrane of CD56-positive uNK cells which were otherwise ER66-negative. Profiling of isolated uNK cells confirmed expression of ER46 by quantitative PCR and western blot and localised ER46 protein to the cell membrane by immunocytochemistry. Functional analysis of isolated uNK cells using live cell imaging demonstrated that activation of ER46 with E2-BSA significantly increased uNK cell motility. LARGE SCALE DATA N/A. LIMITATIONS, REASONS FOR CAUTION Expression pattern in endometrial tissue was only determined using samples from proliferative and secretory phases. Assessment of first trimester decidua samples was from a range of gestational ages, which may have precluded insights into gestation-specific changes in these tissues. Our results are based on in vitro responses of primary human cells and we cannot be certain that similar mechanisms occur in situ. WIDER IMPLICATIONS OF THE FINDINGS E2 is an essential regulator of reproductive competence. This study provides the first evidence for expression of ER46 in the human endometrium and decidua of early pregnancy. We describe a mechanism for regulating the function of human uNK cells via expression of ER46 and demonstrate that selective targeting with E2-BSA regulates uNK cell motility. These novel findings identify a role for ER46 in the human endometrium and provide unique insight into the importance of membrane-initiated signalling in modulating the impact of E2 on uNK cell function in women. Given the importance of uNK cells to regulating vascular remodelling in early pregnancy and the potential for selective targeting of ER46, this may be an attractive future therapeutic target in the treatment of reproductive disorders. AbstractIn this work, we discuss the development of H.O.S.T., a novel hemoglobin microbubble-based electrochemical biosensor for label-free detection of Hydrogen peroxide (H2O2) towards oxidative stress and cancer diagnostic applications. The novelty of the constructed sensor lies in the use of a sonochemically prepared hemoglobin microbubble capture probe, which allowed for an extended dynamic range, lower detection limit, and enhanced resolution compared to the native hemoglobin based H2O2 biosensors. The size of the prepared particles Hemoglobin microbubbles was characterized using Coulter Counter analysis and was found to be 4.4 microns, and the morphology of these spherical microbubbles was shown using Brightfield microscopy. The binding chemistry of the sensor stack elements of HbMbs’ and P.A.N.H.S. crosslinker was characterized using Attenuated Total Reflectance Fourier Transform Infrared Spectroscopy and UV–Vis Spectroscopy. The electrochemical biosensor calibration (R2 > 0.95) was done using Electrochemical Impedance Spectroscopy, Cyclic Voltammetry, and Square Wave Voltammetry. The specificity of the sensor for H2O2 was analyzed using cross-reactivity studies using ascorbic acid and glucose as interferents (p < 0.0001 for the highest non-specific dose versus the lowest specific dose). The developed sensor showed good agreement in performance with a commercially available kit for H2O2 detection using Bland Altman Analysis (mean bias = 0.37 for E.I.S. and − 24.26 for CV). The diagnostic potential of the biosensor was further tested in cancerous (N.G.P.) and non-cancerous (H.E.K.) cell lysate for H2O2 detection (p = 0.0064 for E.I.S. and p = 0.0062 for CV). The Michaelis Menten constant calculated from the linear portion of the sensor was found to be $${K}_{m}^{app}$$ K m app of 19.44 µM indicating that our biosensor has a higher affinity to Hydrogen peroxide than other available enzymatic sensors, it is attributed to the unique design of the hemoglobin polymers in microbubble.

Resumen

Le partage de commentaires formatifs est inhérent au processus de supervision et l'acceptation de la rétroaction par les apprenants est une étape essentielle à l'apprentissage. Cependant, recevoir des commentaires du superviseur suscite des émotions et les accepter n'est pas facile. Plusieurs recommandations guident les superviseurs sur la façon de partager leurs observations aux apprenants et toutes soulignent l’importance d’encourager l'apprenant à interagir de façon active au processus de rétroaction. Bien que des études dénotent l’effet positif d’informer et de former les apprenants sur la rétroaction, peu s’attardent à les outiller à mieux réagir à la rétroaction. Fondés sur les observations de superviseurs expérimentés, nous proposons un modèle cognitivo-comportemental qui vise à guider les apprenants à aborder la rétroaction avec un état d'esprit de croissance personnelle associé à la position d’apprentissage. Sous le registre de l’acquisition d’une compétence personnelle destinée à mieux accepter les commentaires, le modèle présente un ensemble interdépendant d’attitudes et de comportements destiné à faciliter la gestion des émotions, l’auto-réflexion et l’engagement dans le processus de rétroaction nécessaires à l’apprentissage et à l’acquisition de compétences. L’acronyme H.O.T.E. rappelle aux étudiants les quatre éléments essentiels du modèle : l'humilité, l'ouverture d'esprit, la ténacité et l'explicitation. S’inspirant du courant de la psychologie positive, chaque élément est défini et justifié par des concepts théoriques connus. Pour mieux assimiler les composantes du modèle, l’utilisation de dialogue intérieur est retenue pour faciliter l’entrainement et l’adoption des comportements. L’essence du modèle est discutée à la lumière de la littéracie en rétroaction dédiée aux apprenants.
