Resumen
Abstract Heterotrimeric Ga subunits are the essential molecular switches that activate intracellular signaling in response to G-protein coupled receptor (GPCR) ligation. Gai2, one of 16 human Ga subunits serving nearly 800 GPCRs, plays critical roles in leukocyte biology; however, how Gai2 modulates human immunity in vivo is unknown. We found a dominant missense mutation in GNAI2, the gene encoding the heterotrimeric G-protein Gai2, in a patient with life-threatening, multisystem autoimmunity and susceptibility to mucocutaneous bacteria and virus infections. The mutant proteins impaired immunity by uncoupling chemokine receptor signaling and thereby interfering with leukocyte chemotaxis in vitro and to sites of inflammation in vivo. They also augmented T cell effector responses and diminished the requirement of co-stimulation for T cell activation by constitutively signaling downstream co-stimulatory pathways. These results identify essential roles of Gai2 in maintaining immune-competence and self-tolerance in humans and highlight Gai2-responsive pathways as potential candidates for new therapeutics for autoimmune diseases.

Resumen
Abstract Clinically non-functioning pituitary adenomas (NFPAs) represent the second most common subtype of pituitary tumours, representing 15-43% of all pituitary adenomas. NFPAs are usually benign tumours with no clinical evidence of hormonal hypersecretion. In most of the cases the tumours are arising from the gonadotroph linage, but silent corticotroph, somatotroph, thyrotroph and rarely lactotroph cell characteristics can be discovered with immunostaining. As a result of lack of hormonal hyper-secretion, the diagnosis of NFPAs is made most often when the patient presents with mass effects due to a macroadenoma. NFPAs are most common tumour types in surgical series, and their primary treatment is indeed surgery. However, complete resection is achieved only in about 66% of the cases, and 20% of gross total resected tumours recur after 10 years. Over the past few decades, remarkable progress has been achieved regarding medical treatment options for pituitary tumours. However, NFPAs remain the only subtype with no widely accepted pharmacological treatment. Dopamine receptor type 2 (DRD2) and somatostatin receptor (SST) expression have been demonstrated in NFPAs, prompting the investigation of dopamine agonists and somatostatin analogues as potential treatment strategies. The DRD2 agonist cabergoline is already used as first-line treatment of prolactinomas to induce tumour shrinkage and reduce prolactin secretion. Here we aim to investigate the efficacy of cabergoline on human NFPA tissue. We assessed DRD2 expression levels via immunohistochemistry and qPCR in a large cohort of NFPAs (n=40) and in few prolactinomas (n=10). D2DR has two different isoforms, long (D2RL) and short (D2RS), which are differently expressed and can induce different effects on cell viability. In cabergoline-sensitive prolactinomas D2RS is the predominant isoform. NFPAs shown 10-fold and 5-fold decrease expression of D2RS and D2RL compared to prolactinomas, respectively. Additionally, in NFPAs we observed a significant decrease of cAMP production after cabergoline treatment (45% ±7; p&lt;0.0001); however, viability after one-week cabergoline treatment showed only 4% (±0.7; p&lt;0.0002) reduction, compared to a 10% decrease in prolactinomas (p&lt;0.05). We also observed no difference in secretion of chromogranin A release in NFPAs upon treatment with cabergoline. We have also seen in NFPAs downregulation of Ga i2 protein expression levels, highlighting the general decrease of expression levels in the dopamine pathway (50% less, p&lt;0.004). Our data suggest that the difference in cabergoline responses between NFPAs and prolactinomas may be due to their distinct expression of D2DR isoforms, which differ by 29 amino acids in the third cytoplasmic loop, essential for G-protein binding. It has been shown that D2RL, but not D2RS, requires Gai2. The difference in D2R isoform expression could translate to distinct G-protein activation, ultimately leading to the contrasting viability results after cabergoline treatment. Resumen
The study objective was to understand how the contralesional labyrinth contributes to gaze and gait stability after unilateral vestibular deafferentation (UVD). Head impulse testing (vHIT) was completed in 37 individuals [22 women (59%); age 52.13 ± 11.59 yr, mean ± SD] with UVD from vestibular schwannoma resection. Compensatory saccades (CS) and vestibulo-ocular reflex (VOR) gain were analyzed for both ipsilesional and contralesional impulses. Gait speed (10-m walk test) and endurance (2-min walk test) were collected for 35 individuals. CS were recruited during contralesional head rotation regardless of VOR gain on either the ipsilesional [ρ = 0.21 (−0.14, 0.57); Spearman rank (95% confidence interval)] or contralesional side [ρ = −0.04 (−0.42, 0.35)]. Additionally, the latency of these CS (151.19 ± 52.41 ms) was similar to that of CS generated during ipsilesional rotation (165.65 ± 21.62 ms; P = 0.159). CS recruited during ipsilesional vHIT were of a higher velocity ( P < 0.001) and greater frequency ( P < 0.001) compared with contralesional CS. VOR gain asymmetry was significantly correlated with gait speed [ρ = −0.37 (−0.73, −0.01)], yet individual VOR gain was not correlated [ipsilesional: ρ = 0.17 (−0.20, 0.55); contralesional: ρ = −0.18 (−0.52, 0.15)]. Our data reveal CS are recruited at similar latencies without correlation to VOR gain or direction of head rotation, and that the central integration of ipsilesional and contralesional vestibular afference correlates with gait. Together, our data suggest the brain considers vestibular afference from both sides when generating related behavioral output after UVD. NEW & NOTEWORTHY After unilateral vestibular deafferentation, compensatory saccades (CS) have similar latencies regardless of the direction of head rotation, and those CS generated during contralesional head rotation are unrelated to extent of vestibular loss. Additionally, the extent of asymmetry in residual vestibular function, not the extent of vestibular loss, correlates with gait speed. Our data suggest a common mechanism is responsible for the generation of CS and restoration of gait speed in vestibular compensation.

AbstractThe elevation gain is an important contributor to the total workload in endurance sports. The purpose of this study was to evaluate the influence of the arm swing on elevation gain in three sport watches (Garmin® Forerunner 910XT, Polar® RS800CX and Suunto® Ambit2) on a flat 400 m outdoor track. Altogether, a total of 120 repetitions of 1,200 m were performed at self-selected speeds corresponding to strolling, walking, jogging and running. During the assessment two devices of each sport watch, one secured on the hip and one on the wrist, were worn by the participants. A small but significant (effect size = .39; p < .001) influence of the arm swing on elevation was revealed in all sport watches. Elevation indication errors recorded on the wrist were significantly larger than the ones recorded on the hip (4.0-7.4 vs. 1.2-5.7 m per 1,200 m; p < .05). Furthermore, when wearing the devices on the wrist, errors in elevation indication increased when gait speed increased. Users should be aware that wearing the devices on the hip can significantly decrease measurement errors. This might be especially relevant for activities with high dynamics, such as jogging and running.

Resumen
Several studies have analyzed human gait data obtained from inertial gyroscope and accelerometer sensors mounted on different parts of the body. In this article, we take a step further in gait analysis and provide a methodology for predicting the movements of the legs, which can be applied in prosthesis to imitate the missing part of the leg in walking. In particular, we propose a method, called GaIn, to control non-invasive, robotic, prosthetic legs. GaIn can infer the movements of both missing shanks and feet for humans suffering from double trans-femoral amputation using biologically inspired recurrent neural networks. Predictions are performed for casual walking related activities such as walking, taking stairs, and running based on thigh movement. In our experimental tests, GaIn achieved a 4.55 prediction error for shank movements on average. However, a patient’s intention to stand up and sit down cannot be inferred from thigh movements. In fact, intention causes thigh movements while the shanks and feet remain roughly still. The GaIn system can be triggered by thigh muscle activities measured with electromyography (EMG) sensors to make robotic prosthetic legs perform standing up and sitting down actions. The GaIn system has low prediction latency and is fast and computationally inexpensive to be deployed on mobile platforms and portable devices.

Resumen
Gait period detection, serving as a preprocessor for gait recognition, is commonly studied in the recent past. In this paper, we proposed a novel gait period detection method for depth gait video stream. The method introduces the concept of layered coding for depth images which decreases computational complexity. Furthermore, the extreme value of the sum of layered codes for gait sequence is utilized to judge the period endpoint, which is in accord with the naked-eye observation. In addition, gait recognition experiments on the TUM GAID database are conducted with the description of gait features of one single detected period by the proposed scheme using tensor representation. The high recognition accuracy verifies the effectiveness of the proposed depth gait period detection method.

Resumen
According to the data of the World Health Organization a prevalence of obesity has increased since 1975. Almost 2 billion adults are overweight, among them over 650 million are obese. It is also known that overweight and obesity kill more people than underweight. Currently, it is estimated that about 3 billion adults will be overweight and over 1 billion will be obese by 2025. Moreover, among 13 biggest health problems facing people in their country an obesity takes the fifth place. The purpose of the study was to analyze the new data of the scientific publications of recent years explaining new concepts of physiological mechanism of hunger and satiety balance, the causes of excessive weight or obesity, and suggestions for mindful eating, avoiding overeating and food temptation. Results and discussion. The obesity epidemic is now one of the most important public health problems in the world. However, overweight and obesity are preventable. Being overweight increases the risk of death. The results of recent researches suggest that weight gain itself, even if a person stays within the “normal” weight range, also increases the risk of disease and premature death. There are studies that showed that a weight gain even less than 5 kg and within a healthy body mass index range, is associated with an increased risk of disease. Results of other research findings showed that biological indicators of health risk are strongly related to body weight from the lowest body mass index to the highest body mass index, with higher body mass associated with greater health risk. Obesity becomes global health problem. Obese people are at high risk of developing a number of comorbidities, including disorders of gastrointestinal tract, type 2 diabetes mellitus, joint and muscle diseases, respiratory problems and psychological problems, which can significantly affect the quality of their daily life and increase the risk of mortality. The conditions associated with obesity are diverse, but even modest weight loss can reduce the risk of development of cardiovascular disease, diabetes mellitus, obstructive sleep apnea, and arterial hypertension, among many other comorbidities. Even relatively small weight loss can improve patient condition and can act as a trigger for further change, with sustained weight loss achieved through a series of gradual weight loss steps. Conclusion. The increase in obesity prevalence has an essential influence on the global morbidity of many diseases, moreover obesity and overweight affect quality and quantity of health thus being the causes of disability and significantly affect quality of life. Prevention of weight gain has been the focus of much discussion and debate, with many theories about the causes and solutions of the problem. However, prevention programs, which take in account the causes and risks factors, are more effective than weight-loss programs

Abstract Background: Psychometric properties of tests that assess the angular vestibulo-ocular reflex (aVOR) and vestibulospinal reflex function are currently unknown. This study investigated the test-retest reliability and response stability of gaze stabilization, postural sway, and dynamic balance measures in persons with multiple sclerosis (MS) and controls. Methods: Nineteen adults with MS and 14 controls performed passive horizontal head impulses, quiet standing, and dynamic balance tests on two separate occasions. Gaze stabilization measures included aVOR gain, number of compensatory saccades (CSs) per head rotation, CS latency, and gaze position error. Postural sway included sway amplitude and total sway path. Dynamic balance measure included the Functional Gait Assessment. Intraclass correlation coefficient, standard error of measurement (SEM, SEM%), and minimal detectable difference at 95% confidence level were calculated. Results: Test-retest reliability for aVOR gain, CSs per head rotation, and gaze position error was moderate and for each postural sway and dynamic balance measure was good. Low error (SEM, SEM%) for aVOR gain, CS latency, postural sway, and dynamic balance variables and low minimal detectable difference values for aVOR gain and Functional Gait Assessment scores were seen, suggestive of acceptable response stability. Conclusions: These results support the utility of some of the gaze and postural measures for examination and treatment efficacy purposes in people with MS.

Resumen
The case of a 27-year-old female, 5 years after a motor vehicle accident that resulted in spinal cord damage, is reviewed. She progressed well after her accident for 4 years, at which time she developed a neuropathic heel ulceration, aggravated by her calcaneal gait and pregnancy weight gain. The rarity of such a case involving neuropathic ulceration and calcaneal gait after partial deficit of cauda equina function prompted this article.

Resumen
Benign paroxysmal positional vertigo (BPPV) is a common peripheral vestibular disorder. Besides vertigo, unsteadiness is a common subjective complaint. This study aims to gain an overview of gait and postural control changes among individuals with BPPV compared to healthy individuals. Relevant case-control studies were searched from their inception until April 2020 using the Scopus, Web of Science and Science Direct databases. Two independent reviewers assessed the methodological quality using the Newcastle-Ottawa Scale (NOS) for case-control studies. A total of 15 studies which satisfied the eligibility criteria were included. The findings suggest evidence of gait and postural control alteration characterized by slower gait speed, reduced gait velocity, increased anterior-posterior centre of pressure, and increased sway velocity compared to controls. Individuals with BPPV showed evidence of gait and postural control alteration. Future exploration on this issue is vital to support gait and postural control alteration among individuals with BPPV.
