Globally, tuberculosis (TB) is a major cause of morbidity and mortality among people who use drugs (PWUD), particularly those co-infected with HIV. This chapter describes how TB is prevalent in several prison systems by virtue of the concentration of PWUD and people living with HIV. TB is further amplified within this system through overcrowding, poor ventilation, and delayed access to quality prevention and treatment services. In many countries, individuals cycling through prisons are inadequately screened and treated for TB, and affected individuals may have frequent treatment interruptions. For PWUD, relapse to drug use immediately after release from custody can impede continuity of care, which may contribute to the development of drug-resistant TB. Particularly in countries with high incarceration rates, prisons act as amplifiers of TB and drug-resistant TB in the community. The World Health Organization’s recommendations for integration of TB, HIV, and addiction treatment are seldom achieved, especially within prisons. Other factors contributing to poor TB outcomes among PWUD interfacing with prisons include insufficient support to promote medication adherence and co-morbidities, like viral hepatitis that potentiate hepatic toxicity, both of which are prevalent among PWUD.

The storm came on the night of 31 October. It was a full moon, and the tides were at their peak; the great rivers of eastern Bengal were flowing high and fast to the sea. In the early hours the inhabitants of the coast and islands were overtaken by an immense wave from the Bay of Bengal — a wall of water that reached a height of 40 feet in some places. The wave swept away everything in its path, drowning around 215,000 people. At least another 100,000 died in the cholera epidemic and famine that followed. It was the worst calamity of its kind in recorded history. Such events are often described as "natural disasters." This book turns that interpretation on its head, showing that the cyclone of 1876 was not simply a "natural" event, but one shaped by all-too-human patterns of exploitation and inequality — by divisions within Bengali society, and the enormous disparities of political and economic power that characterized British rule on the subcontinent. With Bangladesh facing rising sea levels and stronger, more frequent storms, there is every reason now to revisit this terrible calamity.

The typical story of African American religions narrates the development and power of the Protestant black church, but shifting the focus to the long nineteenth century can reorient the significance of the story. The nineteenth century saw the boom of Christian conversions among African Americans, but it also was a century of religious diversity. All forms of African American religion frequently pushed against the dominance of whiteness. This included the harming and cursing element of Conjure and southern hoodoo, the casting of slaves as Old Israel awaiting their exodus from bondage, the communications between the spirit of Abraham Lincoln and Afro-Creoles in New Orleans, and the push for autonomy and leadership by Richard Allen and the rest of the African Methodist Episcopal Church. While many studies of African American religions in the nineteenth century overwhelmingly focus on Protestantism, this is only part of the story.

Shock remains a major cause of intensive care unit admission. Initially categorized into hypovolaemic, cardiogenic, and distributive shock, understanding of the pathophysiology has recently evolved such that tissue hypoperfusion in all shock states leads to a dysregulated inflammatory response. After 24 hours, septic shock and ischaemiareperfusion related to hypovolaemic and cardiogenic shock share similar haemodynamic and pro-inflammatory profiles. Vascular hyporesponsiveness to catecholamines is a major consequence of this common pathophysiology, which is focused upon activation of NF-κ‎b with subsequent NO overproduction. Myocardial dysfunction is a frequent complication of the cytokine storm that follows septic shock and ischaemiareperfusion. It may worsen haemodynamic status, but nevertheless, remains transient and totally reversible.

Wind storms impact human lives, their built as well as natural habitat. During the last century, society’s vulnerability to wind storms has been reduced by enhanced knowledge of their impact and by controlling exposure through better design. However, only two of the wind systems have so far been considered in the design of buildings and structures, i.e., synoptic winds resulting from macroscale weather systems spanning thousands of kilometers, e.g., extratropical storms, and mesoscale tropical storms spanning hundreds of kilometers and traveling fast, e.g., hurricanes/typhoons/cyclones. During the last two decades, enough evidence has surfaced to support that a third type of very localized wind storms, the non-synoptic winds, are the most damaging in some regions of the world. Thus far there are no design provisions established for the codification of these wind storms. Their characterization in terms of climatology, wind field and intensity, frequency and occurrence, as well as their impact on the built environment, is slowly developing. This handbook presents the state-of-the-art of knowledge related to all these features including their risk, insurance issues, and economics. The research in this area is on the one hand more arduous given the reduced scale, the three-dimensionality, and nonstationary aspects of these non-synoptic winds while, at the same time, its understanding and modeling are being aided by the emergence of novel modeling and simulation techniques which are addressed in this handbook. This will serve as a guiding resource for those interested in learning about and contributing to the advancement of the field.

Formal theological education underwent a twentieth-century revolution that both enriched and fragmented popular Bible reading. Its legacy includes deeper understanding of the Bible and its contexts, new professions of teaching and scholarship, more professional clergy, and liberating critiques of oppressive ideas and practices. Yet its legacy also includes significant fractures, between academy and church, Bible and theology, and so on. Biblical interpretation became marginal to American intellectual life as it became more informed. Theological education’s hermeneutical story highlights the practical-moral agenda of professionals: they sought to inform, frequently even to reform, how Americans approach their Bibles—with historical awareness and various theological-political agendas ideally displacing literalist private application. The mutual popular and professional tensions in this story, given their impact on its interpretation, call for more generous scholarly attention to popular aspirations for understanding the Bible.

Today coral reefs, perhaps more than other marine systems, are suffering from numerous pressures. As a result, many have collapsed as functioning ecosystems. Nutrient pollution, sewage pollution, industrial pollution, landfill, coral diseases and diseases of other important groups of organisms, as well as over-extraction of fish, invertebrates and even the limestone rock itself, have all contributed to the demise of over one-third of the world’s reefs. More recently, climate change, notably causing a sea temperature rise, which in turn has led to coral bleaching and the death of component corals, has added to the stress imposed on this ecosystem. In the future, ocean acidification, sea level rise and an increase in the frequency and severity of storms will add further stress. Many of these factors interact, making the precise responses of reefs to these changes very complex.

The consultation liaison psychiatrist frequently encounters questions of decision-making capacity for hospitalized geriatric patients. This trend will only continue as the population ages and questions about the ability of aging patients to make medical decisions and broader life decisions arise more and more frequently. Consultation liaison psychiatrists tasked with determining these capacities may be faced with a duality of roles: responsibility to the patient but also protective obligations imposed by laws and regulations. Consultation liaison psychiatrists should engage these evaluations carefully and be forthright with patients. An approach focusing on the nature and cause of incapacity, the potential for reversibility of incapacity, adequately informing the patient, relying on colleagues in occupational and physical therapy as well as speech and language pathology for functional assessment, and understanding the patient’s life history and story can lead to results respectful of both the patient’s well-being and dignity. This chapter presents forensic issues relevant to the geriatric psychiatry consultation-liaison service through an illustrative clinical vignette.

Frequently we hear about the work of volunteers from paid staff, from research, but not always from the volunteers themselves in their own words. In this chapter we have tried to explore what happens in the unique place that volunteers have within hospice and palliative care services; somewhere between staff, patients, and families. The stories in this chapter have all come directly from volunteers from Italy, Australia, Romania, India, United States of America (USA), and the United Kingdom (UK) in their own words. Translation, where required, has tried to remain true to the original story and all volunteers have agreed the translation to ensure that it has not changed the essence of the story. Volunteers have described their roles, what volunteering means to them, and the impact that it has on their lives.

This chapter first sets out the book's purpose, which is to trace the history and continued significance of multiracial representations, in order to challenge a dominant U.S. cultural narrative that imagines multiracial people as symbols of the declining significance of race. It then turns to a discussion of contemporary multiracial Asian American representations. Multiracial Asian American representations form an especially productive ground to explore the contradictions of racial narratives in the United States. Understanding why Asians, particularly multiracial Asians, have so frequently been held up as examples of the eventual triumph of a colorblind United States can help us see what interlocking racial narratives make this such an alluring story. If we contextualize that story within politics, social hierarchies, and a longer historical trajectory, it becomes clear that leaving Asians out of discussions of color blindness and multiracial meaning in the United States serves only to naturalize and render invisible racial inequalities and power hierarchies.

Hesiod’s indebtedness to Near Eastern material is more frequently discussed than the Indo-European background of his poetry. This chapter argues for a holistic understanding of how Indo-European prehistory and Near Eastern analogues contribute together to the formation of Hesiodic language and thought. Concentrating on Theogony 35, ἀλλὰ τίη μοι ταῦτα περὶ δρῦν ἢ περὶ πέτρην;, “But what are these things about a tree or a rock to me?,” I demonstrate that this enigmatic question encapsulates Hesiod’s role as mouthpiece at the head of the simultaneously Indo-European- and Near Eastern-based tradition of Greek poetry. By means of artful phonology here and throughout the proem, Hesiod highlights, in ways not previously noticed, the quite different sounds of the melodious Muses and their loud-thundering father, Zeus, who, like the Near Eastern storm god, has a robust association with prophetic oaks and stones.

In contrast to the wider-ranging if loose definitions that are frequently given of humanism, the definition adopted in this chapter owes much to Kristeller’s study of the term umanista. Concentrating on classical scholarship and the new areas of knowledge and understanding it opened up, this essay deals with education, theology, law, and medicine, four key areas in Montaigne’s humanism and in his relationships with the humanists of his time. The argument is put forward that it is the story of knowledge itself that is at stake in his discussion of humanism. Resumen
This chapter concentrates on Fortuna in archaic Rome. The first part of the chapter is focused on historiography, and studies the connection between Fortuna, King Servius Tullius, and the story of Queen Tanaquil. It is argued that the connection between Servius and Fortuna might have been a late development in the literary tradition on early Rome. In modern historiography Queen Tanaquil is frequently identified with Fortuna, but this connection is not supported by ancient sources, and was first theorized by Bachofen in the nineteenth century. The second part of the chapter focuses on sanctuaries such as the temple of Fors Fortuna, that of Fortuna Muliebris, and that of Fortuna at the Forum Boarium, attempting to determine the significance and the meanings attributed to Fortuna in early Rome.

As Jim Crow segregation came to define black Americans’ place in the nation by the end of the nineteenth century, American memory also became largely segregated. African Americans continued to hold Attucks in high regard, but his name was invoked far less frequently in mainstream popular culture and historical scholarship. As white America all but abandoned its concern for the basic welfare and rights of black citizens, a black hero like Crispus Attucks had little chance to enter the heroic pantheon of the nation. School textbooks, mainstream popular culture, and white Americans in general virtually erased Attucks from the story of the American Revolution. African Americans kept his memory alive in history books, public commemorations, and memorial acts like the naming of children and community organizations.

This chapter discusses how writing The Pilgrim’s Progress, Part II (1684) allowed Bunyan to explore areas of Christian experience that Part I did not address, especially the workings of an ideal church and the spiritual lives of women, including the issue of literacy and women’s access to the Word. Anticipating a younger and female readership for this story of Christiana, her children, and her friend Mercie, Bunyan adopts in Part II a more didactic style, replacing Part I’s emphasis on testifying to spiritual experience with frequent scenes of catechizing. Great-heart the minister creates the spiritual community in Part II and guides it through a landscape far more welcoming to pilgrims than that of Part I. Because women’s ‘burdens’ are internal, connected to their sexuality, they cannot be lost, only contained through marriage, child-bearing, and obedience to male authority.

This chapter deals with the life and career of Zainab Herawi. She began her singing career as a child apprentice in the Herat region of Afghanistan in the 1940s, within the context of a class-based society, where singing by women was considered to be morally questionable. Zainab's is a story of unfulfilled fame because, although she was invited to sing for Kabul Radio when twenty-seven, family and background pressures and constraints led to her returning home, where she continued to sing for wedding celebrations. As a series of choices and struggles involving the interfacing domains of family, location, religion, and vocality, her life as a local singer with a large family was one of frequent anguish and frustration; yet her songs live on in the narratives and performances of musician and scholar Veronica Doubleday.
