Resumen
Endotracheal intubation and tracheostomy are associated with a broad spectrum of complications, which are similar and overlapping, and range from injury to airway structures to nosocomial lower respiratory infections. Anatomical damage to the airway occurs as both early and late complications and is generally the consequence of procedural factors: emergency endotracheal intubation, abnormal anatomy, unsatisfactory patient positioning, inexperienced operator skill, and the use of an inadequate technique. Upper airway endoscopy and bronchoscopy, using the flexible and rigid approaches, are useful for the diagnosis and treatment of the complications of endotracheal intubation and tracheostomy.

The chapter discusses the principles of endourology outlining the principles behind the sub-specialty. Topics disused include endoscopes, cystoscopes, rigid and semi-rigid ureteroscopes, flexible ureterorenoscopes, rigid and flexible nephroscopes, disposable equipment, integrated operating theatres, narrow band imaging (NBI), theatre ergonomics, and antibiotics. Urologic endoscopes are generally of two optical designs: the rigid, rod lens system described by Hopkins, while fibre-optic imaging bundles are used in both rigid and flexible endoscopes. The rod lens system consists of a series of glass rods with polished ends with the key feature of air gaps that act as a lens. Light is carried efficiently along the rod, resulting in a clear and bright image.

This chapter on colorectal surgery covers common anorectal operations, and open and laparoscopic colorectal resections. Operations for fissures, fistulae, haemorrhoids, pilonidal sinus, rectal prolapse, perianal abscess are described. The steps of appendicectomy, ileostomy and colostomy formation and closure, intestinal anastomosis, colonic and rectal resections, abdominoperineal excision, ileal pouch, and Hartmann’s operations are included. Also included is surgery for faecal incontinence, sphincteroplasty, and for obstructed defaecation like STARR (stapled transanal rectal resection) and LVMR (laparoscopic ventral mesh rectopexy). Endoscopic examination like flexible sigmoidoscopy and colonoscopy are described.

Retrograde intrarenal surgery (RIRS), also known as flexible ureterorenoscopy (fURS) is an effective and safe method for the management of intrarenal stones. Its use has increased in recent years with several reports showing stone-free rates as high as shock wave lithotripsy (SWL) for intrarenal stones. This improvement in efficacy of retrograde intrarenal surgery (RIRS) is related to technological advances and improvements in endoscopic techniques that have made retrograde stone removal easier. The main advantage of RIRS compared to open operative or percutaneous intervention is a decrease in operative morbidity for the patient, as well as a faster recovery. RIRS should be considered in patients with kidney stones if they have a contraindication for SWL or percutaneous nephrolithotomy (PCNL), such as the presence of a bleeding diathesis, some cases of morbid obesity, musculoskeletal deformities, or infundibular stenosis.
