Resumen
Fissure in the third metacarpal/tarsal parasagittal groove and proximal phalanx sagittal groove is a potential prodromal pathology of fracture; therefore, early identification and characterisation of fissures using non-invasive imaging could be of clinical value. Thirty-three equine cadaver limbs underwent standing cone-beam (CB) computed tomography (CT), fan-beam (FB) CT, low-field magnetic resonance imaging (MRI), and macro/histo-pathological examination. Imaging diagnoses of fissures were compared to microscopic examination. Imaging features of fissures were described. Histopathological findings were scored and compared between locations with and without fissures on CT. Microscopic examination identified 114/291 locations with fissures. The diagnostic sensitivity and specificity were 88.5% and 61.3% for CBCT, 84.1% and 72.3% for FBCT, and 43.6% and 85.2% for MRI. Four types of imaging features of fissures were characterised on CT: (1) CBCT/FBCT hypoattenuating linear defects, (2) CBCT/FBCT striated hypoattenuated lines, (3) CBCT/FBCT subchondral irregularity, and (4) CBCT striated hypoattenuating lines and FBCT subchondral irregularity. Fissures on MRI appeared as subchondral bone hypo-/hyperintense defects. Microscopic scores of subchondral bone sclerosis, microcracks, and collapse were significantly higher in locations with CT-identified fissures. All imaging modalities were able to identify fissures. Fissures identified on CT were associated with histopathology of fatigue injuries.

Resumen
A study has been made of the striations and fissures developed in the aluminum alloy 2024-T3 during fatigue crack growth. Fissures were found to form on inclined facets. They were uniformly spaced as the result of a shielding process. Striation spacings were in accord with da/dN values at the higher levels of K investigated, but at low K levels striation spacings were larger than the corresponding da/dN values. The percentage of the fracture surface containing striations varied with the K level, ranging from less than 1 % at low K levels to 80 % at higher K levels. The reason for the discrepancy between the spacing of striations and the corresponding da/dN values is discussed.

Resumen
Failure analysis of the petrochemical plant’s bank-wall side boiler tube has been conducted to determine the root cause of tube failure. The tube material is low carbon steel ASTM A178 grade A. Visual examinations of the cracked surface revealed that the fractured surface is flat, without plastic deformation, and several longitudinal and transverse fissures are present. The SEM and optical microscope examinations show that the cracks were intergranular and transgranular. A hydrogen attack caused the intergranular crack, and thermal fatigue produced the transgranular crack. Boiler tube failure was caused by a steam blanket on the sloping tube’s top inner diameter that induced iron oxide deposition and accumulation. Hydrogen was produced after a chemical reaction at the deposit-metal interface between the iron oxide deposit and ingress steam. Local temperature variation on the top part of the sloped tube occurred during the splashing and evaporation of water, promoting thermal fatigue.

Resumen
The fractured rock mass in the western cold region is affected by freezing and thawing disasters and is prone to local damage and fracture along the fissures’ ends. The fatigue damage induced by repeated frost heave and traffic loads seriously endangers the stability of cold region roadbed. This paper selects sandstone as the research object. Firstly, 20 freeze–thaw cycles were performed on fractured sandstone samples with different inclination angles of 30°, 45°, 60°, and 90°. Subsequently, triaxial compression and triaxial fatigue loading tests were conducted to explore the mechanical properties and fracture morphology evolution mechanism during the compression process of freeze–thaw fractured sandstone. Nuclear magnetic resonance technology (NMR) was used to measure the H-containing fluid inside rock pores. The microscopic damage characteristics inside the rock were analyzed from the NMR T2 relaxation spectrum signal and pore size distribution characteristics. Based on the internal variable theory of continuum mechanics, a fatigue model of freeze–thaw fractured sandstone with different inclination angles was established. The results show that sandstone strength was negatively correlated with the fracture dip angle, and the axial deformation and shear failure angle were positively correlated with the fracture dip angle. The mechanical properties of the sandstone were deteriorated by fatigue loading. When the crack angle was 90°, the fatigue failure strength of the rock sample was the lowest. The T2 spectrum distribution of the fractured sandstone mainly had three peaks and the pore size was mainly medium and small pores. There was a small leftward shift after freeze–thaw cycles and fatigue loading. The T2 spectrum area was significantly affected by fatigue loading, with the highest rate of change at a crack angle of 30°. Through the fine correspondence between the axial residual deformation and the deformation modulus, a fatigue model with different crack inclination angles was established using the axial residual deformation as the internal variable, and the rationality of the model was verified by fatigue loading tests.

Resumen
Dental implants are becoming increasingly used in current dental practice. This increased demand has motivated manufacturers to develop varieties of product through design, but also looking for new materials used to improve surface characteristics in order to obtain a better osseointegration. But the increase in the use of implants goes to a consequent increase in the number of failures. These failures are caused either by treatment complications (peri-implantitis), by fatigue breakage under mechanical over-stress, by defective raw material, or due to errors during the insertion procedures. Although they are rare, these complications are serious in dentistry. Before to market a dental implant to clinical practitioners, the product is validated among other determinations in number of biocompatibility research. Raw material issues, details about its structure and properties are less published by the scientific literature, but all this are subject of a carefully analysis of the producers. Breaking of dental implants during surgical procedures, during the prosthetic procedures or during use (chewing, bruxism, accidents, etc.), is the second most common cause of loss of an implant after consecutive peri-implantitis rejection. Although the frequency of this type of failure for a dental implant is much smaller than those caused by the peri-implantitis, a detailed study of broken implants can explain possible causes. The use of scanning electron microscopy (SEM) in the study of the cleave areas explain the production mechanism of cleavages, starting from micro-fissures in the alloy used for the production of dental implants. These micro-fissures in weak areas of the implant (anti-rotational corners of the polygons, etc.) could generate a serious risk of cleavage first time when a higher force is applied.

Resumen
The sources of radioactive contamination of the primary coolant by fission products when the unit is operating at the rated power are as follows: defect fuel elements with gas leakiness and substantial damages, surface contamination of the outer surfaces of fuel claddings, superficial contamination of structural materials of fuel assemblies. Initially in the reactor operation (if there are no manufacturing defects in fuel elements), the contamination of the coolant by fission products is determined by the release into the reactor coolant circuit of fission fragments of Uranium-235 (due to their kinetic energy) that is present on the outer surfaces of fuel elements as contamination in their manufacturing. During normal operation of the reactor, the integrity of cladding may fail due to various processes of corrosion fatigue type. These processes result in, first of all, micro-fissures and then in large defects in the claddings, which is accompanied by an increase in the release of fission products from fuel elements into the primary coolant.

Resumen
This study examined RNEF with scanning electron microscope (SEM), for surface flaws, internal voids and inclusions. Six commercial brands of 4% taper RNEF: PF (#25, 25mm, Dentsply Tulsa Dental Specialties, Tulsa, OK), GTX (#20, 25mm, Dentsply Tulsa Dental Specialties, Tulsa, OK), NRT (#25, 25mm, MANI Inc., Tochigi, Japan), FKG (#25, 25mm, FKG, Dentaire), K3 (#25, 25mm) and TF (Twisted File #25, 27mm) (Sybron Kerr, Orange, CA) were examined. Full length and 4mm segment of each RNEF brand was mounted in Al plate for surface morphology and conductive epoxy resin for cross-sectional imaging with SEM (JEOL JSM-820) respectively. EDX spectrum was collected from specimens with EDX spectrometer coupled to SEM. EDX software was used for quantitative analysis. This study demonstrated the existence of surface flaws as machining marks, notches, crevices, fissures and pits. Internal voids of micro-to sub-micrometer magnitude were observed. Ti-rich inclusions were found dispersed in the equiatomic to near-equiatomic NiTi substrate of RNEF. Surface flaws, internal voids and inclusions were common findings on RNEF studied. Their effects on the corrosion and fatigue resistance of RNEF deserve further investigation.

Resumen
8028 Background: KRAS-mutant NSCLC, usually reflecting tobacco-related carcinogenesis, represents an unmet medical need in lung cancer therapy. Trametinib plus docetaxel (doc) enhances growth inhibition and apoptosis of NSCLC cell lines in vitro with and without RAS/RAF mutations when compared with either agent alone. Methods: This 2-part, multiarm, phase I/Ib, open-label study evaluated the safety and efficacy of trametinib plus chemotherapy (NCT01192165). Part 1 determined the recommended phase II dose (RP2D) for trametinib+doc in patients (pts) with advanced solid tumors. In part 2, NSCLC pts were stratified as KRAS WT or mutation unknown (WT) or KRAS-mutant (KRAS) and were treated with trametinib + doc at the RP2D. Primary study objectives were safety and tolerability; secondary objectives were efficacy and pharmacokinetics (PK). Exploratory mutational profiling was done using circulating-free DNA from plasma and available archival tumor tissue. Results: As of January 2013, 46 NSCLC pts (24 WT [67% had ≥ 2 prior therapies] and 22 KRAS [41% had ≥ 2 prior therapies]) have been treated at the trametinib + doc RP2D (2.0 mg + 75 mg/m2 + growth factors). Diarrhea, fatigue, asthenia, and nausea were the 4 most frequent toxicities. Dose reduction occurred in 10 pts (22%), mostly for diarrhea, fatigue, mucositis, neutropenia, and skin fissures (all 4%). Preliminary PK suggests no drug-drug interaction. In KRAS pts, the best investigator-assessed response (confirmed + unconfirmed) was 3 partial response (PR; RR = 17%) and 8 stable disease (SD; 44%); additionally, 4 pts had > 20% tumor shrinkage. The current disease control rate (DCR) is 61%; 4 pts have not had postbaseline scans. In WT pts, 5 PR (RR=21%) and 11 SD (46%) were observed (67% DCR). Final response and progression-free survival data will be reported upon maturity. Biomarker analyses, including assessment of KRAS mutation subtype vs efficacy, are ongoing. Conclusions: MEK inhibition with trametinib + doc (+ growth factors) demonstrates tolerability and clinical activity in both KRAS-mutant and WT NSCLC, exceeding expectations for each drug alone and warranting further study. Clinical trial information: NCT01192165.

Resumen
A preceding study of the competition between fracture modes in monolithic brittle materials in cyclic loading with curved indenters in liquid environments is here extended to brittle layers on compliant substrates. The fracture modes include outer and inner cone cracks and radial cracks that initiate from the near-contact zone and penetrate downward. Outer cone cracks are driven by stresses from superposed Hertzian and plate flexure fields; inner cone cracks also grow within these fields but are augmented by mechanical driving forces from hydraulic pumping into the crack fissures. Radial cracks are augmented by mechanical driving forces from developing quasiplasticity zones beneath the indenter. Basically, the crack-growth rates are governed by a crack velocity relation. However, the hydraulic and quasiplastic mechanical forces can cumulate in intensity with each cycle, strongly enhancing fatigue. Plate flexure generates compressive stresses at the top surface of the brittle layer, somewhat inhibiting the initiation, and tensile stresses at the lower surface, strongly enhancing the far-field propagation. The tensile stresses promote instability in the crack propagation, resulting in through-thickness penetration (failure). Experiments on a model bilayer system consisting of glass plates bonded to thick polycarbonate bases are presented as an illustrative case study. In situ observations of the crack evolution from initial growth to failure reveal that each fracture mode can dominate under certain test conditions, depending on plate thickness, maximum load, and sphere radius. Implications concerning the failure of practical layer systems, notably dental crowns, are discussed.

Resumen
Introduction: A phantom tumor, or vanishing lung pseudotumor, is an atypical type of pleural effusion in which there is an accumulation of fluid in the pleural cavity. It is a transient and well-demarcated accumulation of pleural fluid in the interlobar pulmonary fissures and is generally associated with congestive heart failure, hypoalbuminemia, renal failure, hepatic failure, pneumonia, and tuberculosis or pleuritis. In Coronavirus disease 2019 (COVID-19), pleural effusions are also considered uncommon, and they could reflect a more severe state of the condition. The correct diagnosis is important to prevent unnecessary and potentially harmful investigation and treatment. In this study, we reported a rare case of a pleural effusion appearing as a phantom tumor in COVID-19 without congestive heart failure. Case: A 29-year-old man was presented with complaints of dry cough, sore throat accompanied by fever, and fatigue. He denied shortness of breath and loss of taste and smell. He had no smoking habit and did not have any past medical history. Physical examinations were within normal limits, except for a slight fever. The laboratory examination showed positive SARS-CoV-2 RT-PCR and an increased liver function test. His chest X-ray revealed findings suggestive of a phantom tumor accompanied by bilateral pneumonia. The patient received COVID-19 treatments. A follow-up chest X-ray revealed the complete resolution of the lesion. Conclusion: A phantom tumor is a rare case. It is not only caused by heart disease but also by other causes, one of which is COVID-19. The treatment for this condition is based on the underlying disease.

Resumen
This paper presents test results of AA2519/TI6AL4V composite laminate structure and mechanical properties. The composite laminate was merged by explosive welding using a transition AA1050 alloy layer. The tested composite after formation was annealed, quenched in water and then aged to strengthen it. The heat treatment process was carried out in order to improve the physical and strength properties of the laminate. Weld endurance properties were determined through fatigue testing performed in constant total deformation amplitude (εac) conditions. The paper also describes the influence of test conditions on hysteresis loop shape, process of cyclic material strengthening and weakening, and fatigue life. Low cycle test results were approximated using a straight line with the following formula: σa = K'(εap)n which enables the K'fatigue strength coefficient, and the n' cyclic strengthening index to be determined. Test results also include fatigue fracture surface analysis. Distinctive fatigue fissure characteristics were investigated through studying fatigue fracture surface microstructure using a scanning electron microscope.

Resumen

Maternal health is a concept indicating the health of mothers during pregnancy, childbirth and the post-partum period. Recent literature indicates that the maternal mortality rate in Iran is 23 per 100,000 live birth. For every death, 20 to 30 mothers suffer from chronic and acute illnesses arising from pregnancy and delivery; therefore, using a valid and reliable indicator will help researchers assess maternal health more precisely. In this longitudinal study, attempt has been made to compare mothers’ health status by using a subjective tool for evaluating the Physical-Maternal Health Index (P-MHI). The most common complaints made by mothers were headache, back pain, mastitis, nipple fissure, breast abscess, abdominal pain, pain in genitalia, constipation, hemorrhoids, anal fissure, urinary problems/Incontinence, fatigue, and dizziness/vertigo. The P-MHI is defined on the basis of these complaints scaling 0-100. The higher the index, the better the health status of mothers. It was administered by structured interviews with 743 participants before and during pregnancy, one week, two, four, and six months after childbirth (2010-2011). Post-natal mothers experience various problems, with back pain and fatigue being the most common. The P-MHI was at its lowest level in the first week after delivery (76 out of 100), followed by the second month (82 out of 100). The results showed that not only do mothers significantly lose their reproductive health after pregnancy, they don't attain their pre-pregnancy health status even six months after delivery (P<0.0005). The trend of the P-MHI identified when and what kind of health care is the most urgent need of post-natal mothers.

Resumen
Summary Objectives: To report the application and clinical outcome of the treatment of radial carpal bone fracture in five dogs using a variably-pitched, headless cannulated compression screw (Acutrak®). Methods: Arthroscopy was performed prior to surgical repair in three cases. In two cases, a fissure within the radial carpal bone was more clearly apparent when oblique indirect illumination was employed at arthrotomy. Following Acutrak® screw placement via arthrotomy, external coaptation was applied in all cases for a period of two weeks. Results: Five dogs were treated, of which four were Boxers, and one was a Labrador Retriever crossbreed. All dogs were lame prior to treatment. There was no history of trauma, and all dogs had soft tissue swelling on the dorsal aspect of the radiocarpal joint. In all cases the fissure or fracture line was oblique in orientation, extending disto-medially from the midpoint of the radiocarpal joint proximally to the level of the second carpal bone distally. All fractures treated demonstrated progressive radiographic healing, and all dogs were free of lameness at six weeks postoperatively, but two of the five fractures failed to reach complete radiographic union. All dogs remained lameness-free with mean follow-up of 12.5 months. Discussion: Acutrak® screws are headless devices which achieve compression, even in small bone fragments. Threads engage both the near and far fracture segments, leading to increased screw-bone contact which maximizes pull-out strength and may increase the fatigue strength of the screw. Clinical significance: Fixation of two-piece canine radial carpal-bone fractures with the Acutrak® screw proved a reliable alternative to conventional compression screw fixation, and may have benefits over alternatives attributable to screw design and application technique.

Resumen
SummaryObjective: To report the use of a 4.5 mm shaft screw for the management of humeral condylar fractures (HCF) and incomplete ossification of the humeral condyle (IOHC) in dogs, and to assess risk factors for complications. Methods: Dogs with HCF or IOHC that were managed with a 4.5 mm shaft screw with a minimum follow-up of six months from surgery were included. Data from the case records were used to identify risk factors for complications. Long-term follow-up was provided by an owner questionnaire and veterinary re-examination.Results: Forty-three elbows were treated in 40 dogs (14 IOHC, 29 HCF). Minor complications were seen in four cases (9%) and major complications in 10 cases (23%). There were no statistically significant risk factors for major complication. Infection resulted in shaft screw removal from three IOHC cases. Four of eight IOHC cases that had a lateral approach had a major complication compared with zero out of six dogs that had a medial approach, but this difference was not significant (p = 0.085). Nine of 12 IOHC cases and 20/23 HCF cases had excellent or good limb use at the time of long-term follow- up (median of 106 weeks, range 26–227 weeks). All fractures healed but IOHC fissure healing was inconsistent. Fatigue failure of shaft screws was not seen.Clinical significance: The use of shaft screws for the management of IOHC and HCF is associated with a relatively low rate of complications (compared to previous reports) and a good long-term outcome.

Resumen
Abstract Objective Herpes simplex encephalitis (HSE) is a rare neurological condition (~2–4 per million) marked by brain inflammation caused by herpes simplex virus (HSV). HSE is associated with both cognitive and neuropsychiatric symptoms, particularly memory and executive dysfunction. We present the case of a 63-year-old, right-handed, St. Lucian female who completed serial neuropsychological evaluations (NPE). Method The patient presented with a six-day history (per collateral) of fever, confusion, and nausea. The patient denied cognitive changes. Magnetic Resonance Imaging (MRI) revealed leptomeningeal enhancement in the right sylvian fissure, multiple right temporal sulci, and inferior right frontal lobe (Figure 1). There was no indication of seizures. Cerebrospinal fluid analysis was positive for HSV-1. A two-week course of intravenous acyclovir was initiated on hospital day three. Results (Table 1). Initial NPE results (day five) revealed global cognitive impairment with sparing of auditory attention; exam was limited secondary to significant fatigue. Following completion of antivirals, subsequent NPE (day 17) revealed similar findings, despite improved alertness; left neglect (Figure 2) and significant anosagnosia (i.e., lack of disease awareness) and anosodiaphoria (i.e., indifference reaction to neurological deficits) were noted. Two-month outpatient follow-up NPE revealed marginal improvement in aspects of language and learning, but continued memory impairment, dense anosagnosia, and anosodiaphoria. Conclusion In this case, the patient presented with salient neuropsychiatric sequelae of HSE that have not been commonly associated with this condition in the extant literature. Her denial of cognitive symptoms may have initially confounded the differential diagnosis, emphasizing the importance of multi-specialty collaboration. Limitations of available normative data also complicated examination.

Resumen
Abstract Background c-MET receptor tyrosine kinase (RTK) activity has been implicated in establishing the oncogenic phenotype across several human cancers with high levels of the activating c-MET ligand, hepatocyte growth factor (HGF). Malignant plasma cells secrete HGF-activator (HGFA), which converts HGF to its active form, and high HGF levels are correlated with a poor prognosis in multiple myeloma. Syndecan 1 (CD138) on malignant plasma cells binds HGF and potentiates interleukin-6-induced growth and migration. HGF stimulation of myeloma cells also activates autophosphorylation of c-MET and other critical downstream signaling pathways promoting oncogenesis. Finally, pre-clinical studies have shown that suppression of c-MET signaling with a number of small molecules, including ARQ 197, induced myeloma cell apoptosis. Tivantinib-mediated cytotoxic response was observed at concentrations of less than 5 µM, which are achievable in the clinic. These findings supported the hypothesis that suppression of the HGF/c-MET signaling axis could be a rational strategy against RRMM. Methods In this phase II study, the efficacy and safety of ARQ 197, a non-competitive and highly selective inhibitor of the c-MET RTK, was studied in patients with RRMM. Primary objectives were to determine the overall response rate (ORR) to single-agent tivantinib in patients who had received one to four prior lines of therapy, and to define the toxicities in this population. A Simon’s Minimax 2-stage design was used for the study. ARQ 197 was administered at a starting oral dose of 360 mg twice daily with meals for each day of every 4-week treatment cycle. This dose was selected from prior phase I investigations in solid tumors, and at this dose level, steady-state plasma levels of ARQ 197 were 7 µM. Treatment could continue providing that patients did not experience undue toxicities, or disease progression. Tivantinib is provided through the Cancer Therapy Evaluation Program (CTEP), and this study was supported by CTEP, as well as the MD Anderson Cancer Center SPORE in Multiple Myeloma. Results A total of 16 patients were enrolled and treated to date, including 9 men and 7 women, who had received a median of 1 prior line of therapy (range 1-3), including stem cell transplant in ten. The mean patient age was 66 (range 49-76), with ethnicity including 13 Caucasian Americans, 2 African Americans, and 1 Asian American. Patients have received a median of 3 cycles of therapy to date (1-11) with one patient continuing on study, and all were evaluable for toxicity, while 11 were evaluable for response based on having completed two treatment cycles. The most common adverse events (AEs) of any grade seen in at least 25% of patients and felt to be at least possibly drug related included fatigue or decreased neutrophils (94% each), pain (81%), myalgias (56%), diarrhea (38%), memory impairment, respiratory disorders, and rash (31% each), and hypertension (25%), and these were predominantly grade 1 or 2. Grade 3 or 4 AEs included neutropenia (31% and 25%, respectively), syncope, infection, pain (13% of each, all grade 3), and anal fissure, cough, fatigue, hypertension, and pulmonary embolism (6% each, all grade 3). Stable disease (SD) has been seen as the best response in 4/11 (36%) evaluable patients, which was maintained for up to 11 cycles, while the remaining patients showed evidence of disease progression. Conclusion Single-agent tivantinib has been well tolerated in patients with RRMM, and the ability to achieve stable disease in patients with previously progressing myeloma does support the possibility that targeting c-MET has some promise. Future studies with rationally designed combination regimens incorporating a c-MET inhibitor and other novel agents may better define the role of this class of drugs in our armamentarium against myeloma.
