Resumen

This paper compares three case studies of family businesses in the rural tourism and hospitality sectors in Canada, Sweden, and Australia. Goals for start-up, development, and ultimate disposition of the businesses are examined through cross-case analysis within the theoretical framework of the business and family life-cycle. Analysis reveals remarkable similarities reflecting the prominence of lifestyle considerations, location preferences, and uncertainty over disposition of the businesses. This paper assesses goals revealed through these cases and pertinent literature from the tourism and hospitality sectors in the context of three stages in family business evolution. It also draws research and management implications.

Resumen

The New Religious Movement called “The Family” was founded by Anne Hamilton-Byrne (1921–2019) in 1963 in Melbourne, Australia. Hamilton-Byrne taught that she was Jesus Christ and proclaimed her first follower, Dr. Raynor Johnson, to be John the Baptist. The Family combined Christian, Hindu, and New Age ideas with the use of psychedelic drugs, and an eschatological focus on the emergence of a new society after the destruction of the present era. The Family first came to media prominence in 1987 as a result of police investigations, and more recent memoirs and documentaries have also generated media interest around Hamilton-Byrne and her movement. This article discusses The Family and the leadership of Anne Hamilton-Byrne, exploring the role of the media in consolidating her powerful image and in revealing deeply held concerns about “folk devils,” “brainwashing cults,” and maternal and spiritual deviancy in twenty-first century Australia.

Resumen

A home embodies a sensorial space that is layered with personal memories and traces of history. The success of a home in providing a strong sense of place depends on various factors such as geographical location, climatic conditions, and occupants’ world-views and perceptions. This paper explores Muslims’ perceptions of privacy, modesty and hospitality within their homes through their lived experiences. This case study focuses on three Muslim families living in Australian designed homes within the same suburb of Brisbane, Australia. The study provides prefatory insight into the ways in which these families perform their daily activities and entertain their guests without jeopardizing their privacy needs. The study examines the significance of modesty in the design of Muslim homes as a means by which family members are able to achieve optimum privacy while simultaneously extending hospitality to guests inside and outside their homes. The findings of this study provide opportunities too, for expanding research into culturally adaptable housing systems to help meet the changing needs of Australian multicultural society.

Resumen

The purpose of this article is to examine the disconnect happening in relation to family–centre partnerships. Developing partnerships with families is hotly debated and provides challenges for educators teaching in the early childhood sector. Using a comparative case study analysis, several research studies conducted in the states of New South Wales and Victoria, Australia, are examined to illustrate these disconnects. These issues are examined within Belonging, Being and Becoming: The Early Years Learning Framework for Australia, a national framework that is common to all programs across Australia, which identifies practice, principles and learning outcomes for young children. This disconnect is related to the language that is used by the early childhood staff and misunderstood by the parents, the ways communication occurs and its ineffectiveness. The article argues that there is a need to move beyond the current rhetoric of engaging in partnerships with families to a space that allows for transparency, reciprocity and new language.

Resumen

This paper sets the context for a review of family preservation and family reunification research by briefly noting the national and international crisis that currently surrounds foster care. It then presents the recent family preservation and family reunification research from the US and Australia. Some of this material is drawn from the book by Maluccio, Ainsworth and Thoburn (2000), ‘Child welfare outcome research in the United States, the United Kingdom and Australia’. The decision to focus on the US material stems from the fact that these terms originated there in the 1980s and this is where the major research studies are to be found The final comments focus on the re-emphasis on permanency planning and adoption, at least in New South Wales (NSW), and the implications of this for family preservation and reunification services.

Since the 1980s, victims’ voices have been increasingly heard and have been influential in policy debates. Since that time, the nature and presence of those voices has changed shape and form from the influence and presence of victim centred organizations to the rise of the high profile individual victim. The purpose of this article is to explore the presence of one victim’s story, Rosie Batty, and to examine her influence on the rise of the policy agenda on family violence in Australia. This article considers the ways in which this story gained traction and influenced the reform of family violence policy in Australia, and considers the extent to which an understanding of this process contributes to an (emergent) narrative victimology.

(1) Background: Studies have shown that older people prefer to continue living in their own home and community as they age; however this is dependent upon available services and social support. In Australia about two thirds of people will age at home. The Australian Government provides home care packages to support ageing in place yet in rural areas not all services are available. The lack of employment opportunities in rural areas often results in family residing at a distance reducing available social support. This study aims to evaluate informal social support and its influence on ageing in place amongst older people in three Australian rural communities in Australia. (2) Methods: A multiple embedded case study was undertaken in three diverse rural communities. Eleven older rural residents ageing in place aged 65+ were interviewed about their ageing experience and plans for their future in the light of available social support along with 15 members of their social networks. Social networks were then visually depicted with the use of ecomaps and network members were interviewed. (3) Results show that kin and non-kin social networks support ageing in place however ageing is a time of change and reflection. (4) Conclusions: There is a need for more discussion within these networks when it comes to future planning.

The pervasiveness of gender inequality in the media and entertainment industry has become an issue of growing public interest, debate and agitation. Whether it is the gender pay gap, the ongoing presence of the casting couch, the absence of women film directors, the experiences for women and men are strikingly different. Drawing on the findings of a case study of how performers manage care and precarious paid work in film, television and theatre production in Australia, this article provides a context in which work and care regimes can be analysed. Individualised negotiations with agents and producers are buttressed by individualised arrangements with family and extended networks to accommodate complex and changing needs. Despite high unionisation among performers, the key finding is that the overwhelming tendency was to deal with issues individually or as a couple, without reference to the union or through collective avenues.

Australian data can reflect on British questions, about the quality of immigrant labour, and the opportunities gained by migrating, in the nineteenth and early twentieth centuries. Three case studies are presented. The first uses quantitative methods and convict transportation records to argue that Ireland suffered a “brain drain” when Britain industrialized, siphoning off the cream of its workers to England and some, eventually, to Australia. Drawing on an entirely different type of data, the second study reaches strikingly similar positive conclusions about the qualities of Australia's early assisted immigrants: three splendidly visible immigrants stand for the tens of thousands of people who sailed out of urban and rural Britain to the distant colonies. A no less optimistic view of Australia's immigrants half a century later is demonstrated in the third case study on female domestic servants. Often referred to as the submerged stratum of the workforce, the most oppressed and the least skilled, the label “domestic servant” obscured a wide range of internal distinctions of rank and experience, and too often simply homogenized them into a sump of “surplus women”. This study helps to rescue the immigrant women from this fate and invests them with individuality and volition, offering the vision of the intercontinentally peripatetic domestic, piloting her way about the globe, taking advantage of colonial labour shortages to maximize her mobility and her family strategies. Best of all, these migrants emerge as individuals out of the mass, faces with names, people with agenda.

Purpose This paper aims to build on opportunity identification and resource-based view theory, this paper examines the internationalization process of family firms. The role of innovativeness in influencing the internationalization rate of family firms is explored in terms of customer responsiveness, proactiveness and risk-taking. Design/methodology/approach Multiple case studies were carried out with several family businesses in the information technology industry in Australia. Findings The findings of this research suggest that innovativeness of family firms depends on responsiveness to customer needs in the international market while involving a degree of risk-taking. In addition, the results suggest family firms are not proactive in their pursuit of international market opportunities. Research limitations/implications If family firms focus more on becoming more innovative, then they will stand a better chance to successfully enter more international markets. Originality/value The paper highlights the need for family firms to embrace their innovativeness as a distinctive capability for their internationalization efforts. Several new research directions that may enhance the understanding of family firm innovation and internationalization are suggested.

During the year 1888 — the centenary of white settlement — Australia celebrated the jubilee of Queen Victoria together with the advent of electricity to light Tamworth, the first town in the Southern Hemisphere to receive that boon. In the north-eastern colony of Queensland, serious debates involving local administrators included membership of the Federal Council, the annexation of British New Guinea and the merits of a separation movement in the north. In this distant colony, events in Ireland — such as Belfast attaining city status or Oscar Wilde publishing The happy prince and other tales — had little immediate global impact. Nevertheless, minds were focused on Irish matters in October, when the scion of a well-established west Ireland family — a select member of the traditional Tribes of Galway, no less — was named as the new governor of Queensland. The administrators of the developing colony roundly challenged the imperial nominators, invoking a storm that incited strong opinions from responsible governments throughout Australia and around the world.

Australian social workers, like their American peers, have questioned the relevance of brief intensive-family-preservation services (IFPS) to child neglect. Three scenarios involving physical and environmental neglect are compared and contrasted, and the experience of an Australian pilot IFPS program is explored, highlighting the use made of the worker's time in the home, the significance of family history, social support, and issues in termination and follow-up. The case studies suggest three subtypes of neglect, each of which has different implications for the delivery and organization of IFPS.

In both Australia and Brazil there are rapid changes occurring in the macroenvironment of the dairy industry. These changes are sometimes not noticed in the microenvironment of the farm, due to the labour-intensive nature of family farms, and the traditionally weak links between production and marketing. Trends in the external environment need to be discussed in a cooperative framework, to plan integrated actions for the dairy community as a whole and to demand actions from research, development and extension (R, D & E). This paper reviews the evolution of R, D & E in terms of paradigms and approaches, the present strategies used to identify dairy industry needs in Australia and Brazil, and presents a participatory strategy to design R, D & E actions for both countries. The strategy incorporates an integration of the opinions of key industry actors (defined as members of the dairy and associated communities), especially farm suppliers (input market), farmers, R, D & E people, milk processors and credit providers. The strategy also uses case studies with farm stays, snowball interviewing techniques, semi-structured interviews, content analysis, focus group meetings, and feedback analysis, to refine the priorities for R, D & E actions in the region.

<div style="left: 100.81px; top: 546.808px; font-size: 10.6639px; font-family: sans-serif; transform: scaleX(0.992902);">Despite a significant increase in enrolments of postgraduate international Muslim students within Australian universities, little is known about their perceptions of life within Australian homes while undertaking their studies. The aim of this study is to investigate the ways in which students’ cultural and religious traditions affect their use of domestic spaces within the homes in which they reside. The research found that participants faced some minor difficulties in achieving privacy, maintaining modesty and extending hospitality while able to perform their daily activities in Australian designed homes. The findings suggest that greater research attention needs to be given to the development of Australian home designs that are adaptable to the needs of a multicultural society. Australian society encompasses diverse cultural customs and requirements with respect to home design, and these are yet to be explored.</div>

This article reports on the findings of a 2009 survey conducted under the auspices of the Childwatch International Research Network about how children’s participation rights, as set out in Articles 12 and 13 of the UNCRC, are respected in private family law proceedings internationally. Court-based and alternative dispute resolution processes and the roles of relevant professionals engaged in child-inclusive practices are considered, as well as religious, indigenous and customary law methods of engaging with children. The findings from the 13 participating countries confirm an increasing international commitment to enhancing children’s participation in family law decision-making, but depict a wide variety of approaches being used to achieve this. Case studies from Australia, India, Israel and New Zealand are included to illustrate differing models of children’s participation currently in use in decision-making processes following parental separation.

Conservation of Australian plants depends on a spectrum of activities from descriptive and experimental biological studies to active management and restoration of wild ecosystems by local communities who value their native biota. On the basis of the premise that available resources for conservation will not allow for all threatened biodiversity to be saved, some systematists and conservation geneticists argue that phylogenetic relationships should be used to set conservation priorities. The principle advocated is that characters, not species number, should become the currency of conservation, that cladistic analysis of phylogenetic pattern provides a predictive means of modelling the underlying distribution of characters among taxa, and that priority should be given to that subset of taxa with the greatest number of character states. This approach has been applied for some time in the conservation of genetic resources within species (e.g. Eucalyptus caesia), and has been an extra impetus for action with taxonomically isolated endangered species such as the recently discovered araucarian Wollemi pine (Wollemia nobilis). However, most plant conservation activity in Australia has paid little heed to phylogenetic relationships. Degree of threat has been a driving motivation, with endangered species receiving legislative and management attention irrespective of their systematic relationships. Moreover, the current revolution in understanding plant phylogeny associated with DNA sequence studies highlights the need for caution in accepting results of morphologically based analyses. A series of studies on the kangaroo paw and bloodroot family Haemodoraceae highlights this cautionary tale. The derivation of phylogenetic principles relevant to ecosystem and landscape processes is a new field of some promise to conservation managers. An understanding of the Gondwanan origins and landscape evolution of the south-west Australian flora provides a useful case study. Scaling up phylogenetic knowledge of genetic resources within species, and of the evolutionary relationships of taxa to an integrated overview of best management practices for all taxa at the local landscape level, is perhaps the most effective contribution phylogeneticists might make to help conserve Australian plants.

This paper presents some reflections on this natural disaster, which affected many members of the ANZAM community and left a permanent mark on those directly affected and those who watched as the waters washed away lives, memories and many of the things that give people a sense of place. Amongst tragedy came stories of generosity of human spirit as well as the occasional case of greed and callousness. When all was over, however, there was no doubt about the human side of disaster management. Through two case studies of a family-owned business and a large organization impacted by the flood in Brisbane, we highlight some lessons for disaster management and propose some strategies for building social capital in times of disaster.

In June 1997, 13-month-old Jaidyn Leskie disappeared from Moe, a rural Victorian town. His body was found in January 1998. Through a discussion of three presentations of ‘loss’, this paper contends those involved in the case were constructed by the media as ‘bogans' — powerless outsiders — because they defied categorisation within narrow conceptions of ‘normal Australian society’. While Jaidyn himself was a ‘lost child’, his family and associates were likened to a ‘lost tribe’, whose alliances, feuds and kinship networks became exotic, exploitative entertainment. Lacking rhetorical tools to ‘explain’ such a distinctive culture, media coverage constructed bogans as victims of failed social policy: their culture ‘caused’ by economic downsizing, unemployment, drug use and single parenthood. Finally, when members of Jaidyn's family accepted money for media interviews, they were painted as ‘losing their innocence’. This reveals insecurities underpinning the concept ‘bogan’: evidently, bogans were not supposed to engage in media manipulation themselves.

Family farms, whilst declining in number, are the purpose of this paper studying the rural entrepreneurship in family farms as they are at the heart of rural communities and the overall agricultural industry and comprise a high percentage of total farms. This paper takes a case study approach using in-depth semi structured interviews to delve into the types of entrepreneurial strategies that are distinctive of family farms in terms of their sustainability approach to farming. Dairy farms in the West Victorian area of Australia are studied and the findings suggest that family farms can increase their regional competitiveness and international standing by focusing on their collaborative, social and sustainable entrepreneurial strategies.

PurposeDrawing on the conservation of resources theory and social exchange theory, this study aims to examine the underlying relationships linking work-to-family enrichment (WFE) and family-to-work enrichment (FWE) to perceived supervisor support and ultimately, job satisfaction among social workers.Design/methodology/approachData were collected from members of a social work professional body (n = 439) through an internet-based questionnaire and analysed using confirmatory factor analysis and structural equation modelling.FindingsPerceived supervisor support mediated the relationships between work–family enrichment (specifically, WFE-Development, WFE-Affect and FWE-Efficiency) and job satisfaction.Research limitations/implicationsSocial workers who worked in a positive work environment that uplifts their moods and attitudes (WFE-Affect), have access to intellectual and personal development (WFE-Development) and felt supported by their supervisors reported higher levels of job satisfaction. Those who possessed enrichment resources were found to be more efficient (FWE-Efficiency) also perceived their supervisors to be supportive and experienced higher job satisfaction. Future studies should consider other professional groups and incorporate a longitudinal design.Practical implicationsPromoting work–family enrichment among social workers can contribute to positive work outcomes such as perceived supervisor support and job satisfaction. HR practitioners, supervisors and organisations can promote work–family enrichment among social workers through introduction of family-friendly policies (e.g. flexitime, compressed workweek schedules) and providing a supportive work–family friendly environment for social workers.Originality/valueAlthough several work–family studies have linked work–family enrichment to job satisfaction, the present study shows how each dimension of WFE and FWE affects social workers' job satisfaction.

BackgroundSelf-harm is a serious risk factor for suicide, a major public health concern, and a significant burden on the NHS. Rates of self-harm presentation in primary care are rising and GPs interact with patients both before and after they have self-harmed. There is significant public and political interest in reducing rates of self-harm, but there has been no robust synthesis of the existing literature on the role of GPs in the management of patients who self-harm.AimThis study aimed to explore the role of the GP in the management of patients with self-harm behaviour.Design and settingA systematic review and narrative synthesis of primary care literature.MethodThis systematic review was conducted and is reported in line with PRISMA guidance. Electronic databases systematically searched were MEDLINE, PsycINFO, EMBASE, CINAHL, Web of Science, and AMED. Two independent reviewers conducted study screening and selection, data extraction, and quality appraisal of all included studies. Thematic analysis was conducted.ResultsFrom 6976 unique citations, 12 studies met eligibility criteria and were included. These 12 studies, published from 1997–2016, of 789 GPs/family medicine physicians from Europe, the US, and Australia were of good methodological quality. Five themes were identified for facilitating GP management of self-harm: GP training, improved communication, service provision, clinical guidelines, and young people. Four barriers for GP management of self-harm were identified: assessment, service provision, local, and systemic factors.ConclusionGPs recognise self-harm as a serious risk factor for suicide, but some feel unprepared for managing self-harm. The role of the GP is multidimensional and includes frontline assessment and treatment, referral to specialist care, and the provision of ongoing support.

AbstractThis article explores the removal or exclusion in the late 1940s of people in interracial marriages from two corners of the newly formed Commonwealth of Nations, Australia and Britain's southern African colonies. The stories of Ruth and Sereste Khama, exiled from colonial Botswana, and those of Chinese refugees threatened with deportation and separation from their white Australian wives, reveal how legal rearticulations in the immediate postwar era created new, if quixotic, points of opposition for ordinary people to make their voices heard. As the British Empire became the Commonwealth, codifying the freedoms of the imperial subject, and ideas of universal human rights “irrespective of race, color, or creed” slowly emerged, and claims of rights long denied seemed to take on a renewed meaning. The sanctity of marriage and family, which played central metaphorical and practical roles for both the British Empire and the United Nations, was a primary motor of contention in both cases, and was mobilized in both metaphorical and practical ways to press for change. Striking similarities between our chosen case studies reveal how ideals of imperial domesticity and loyalty, and the universalism of the new global “family of man,” were simultaneously invoked to undermine discourses of racial purity. Our analysis makes a significant contribution to studies of gender and empire, as well as the history of human rights, an ideal which in the late 1940s was being vernacularized alongside existing forms of claim-making and political organization in local contexts across the world.

This article considers the disjunctures that exist between the temporalities of legal status and those of migrants’ lived experiences, and explores the relationship between precarity and temporality. Ethnographic research conducted among recent migrants from China living in Perth, Western Australia with a focus on migrants who have been sponsored by employers to work and remain in Australia, demonstrates that while migrants may hold temporary or permanent visas, their migration objectives and settlement processes do not necessarily accord with their formal status. Many individuals who arrived in Australia with the intention of quickly attaining permanent residency find their plans are stymied by shifting circumstances and changes to migration legislation. They instead continue to experience the precarious employment, liminality and family disruption that come with a prolonged and indeterminate temporariness. Meanwhile others have become permanent residents despite arriving as self-imagined sojourners, employment in Australia very often only the next step in a series of temporary labour migrations. Even after many years of permanent status, however, these migrants commonly experience a limited sense of belonging and imagine futures that entail circular patterns of on-migration. The case studies presented disrupt the sense of permanence that is implied in secure legal statuses, and provide evidence of the lasting impact of precarious temporalities.

The barriers to women’s achievement and career progression in the higher education sector have been well researched. It has long been acknowledged that career breaks for child-rearing, and women’s self-beliefs about their abilities can impact negatively on their careers, and many programs and policies have been implemented to redress these around the world. This article is focussed around a regional Australian university, with multiple campuses distributed over 1000 km across two states. Courses, schools, and work teams are often spread across multiple campuses, and travel between campuses is sometimes a necessity; one that is time-consuming and requires time away from family. For some women, travelling is not possible due to family and other commitments or constraints. This paper explores how working in a regional university, with distributed campuses, has an additional impact on women’s career progression. Through auto-ethnographic accounts of four female staff members, we explore the intersection of gender and location through case studies of personal experiences, investigating the effects that distance and travel limitations can have on participation in work team and networking events, access to professional development opportunities, and career progression within the institution.

Parents play an important role in children’s learning of their heritage language (HL) in immigrant countries. Fostering HL learning is a hard task for parents, particularly in immigrant families, and this difficulty is exacerbated in cross-cultural families. The existing studies have not fully addressed the importance of consistent parental perceptions and language management in children’s HL learning. This gap is particularly clear in the research concerning learning Chinese as an HL among cross-cultural families’ children living in English-speaking immigrant countries such as Australia. The present qualitative study used semi-structured interviews to fill this gap by examining the perceptions and language management strategies of three cross-cultural families in Australia whose children are learning Chinese as one parent’s HL. The results suggest that, in English countries, Chinese-background and non-Chinese-background parents in cross-cultural families have quite different opinions about their children learning Chinese, which are reflected in their dissimilar language management strategies. The results highlight the importance and challenges of developing a stable family language policy in cross-cultural families in order to maintain their children’s HL learning.

This is a case study of the Australian company Jonathan M. Shiff Productions and its ‘tween’ program, action series H2O: Just Add Water. The program has sold in 150 countries including the United States, where it was ‘the first non-American live action to be bought by Nickelodeon in America’ and screens every Sunday night as family entertainment. It is also the highest rating children's drama series on Nickelodeon UK. While Australia's content regulations are important to its production, of critical importance is ZDF Enterprises, the commercial arm of one of Germany's two public service broadcasting channels, and worldwide distributor and production partner for all Jonathan M. Shiff productions. Case studies such as the following provide useful insights into the shape and operations of mediascapes elsewhere, and where our own media environment may be heading. They also offer a glimpse into the way the international market place is organising along forms of cooperation designed to facilitate global distribution of cultural content. A central proposition of this case study is that the structural conditions of multi-channel environments require certain adjustments in form, content and business modelling that have essentially coalesced around the operation of brand management.

Objective There is a paucity of research on the quality of evidence relating to primary care workforce models. Thus, the aim of the present study was to evaluate the quality of evidence on diabetes primary care workforce models in Australia. Methods The National Health and Medical Research Council of Australia’s (National Health and Medical Reseach Council; 2000, 2001) frameworks for evaluating scientific evidence and economic evaluations were used to assess the quality of studies involving primary care workforce models for diabetes care involving Australian adults. A search of medical databases (MEDLINE, AMED, RURAL, Australian Indigenous HealthInfoNet and The Cochrane Institute), journals for diabetes care (Diabetes Research and Clinical Practice, Diabetes Care, Diabetic Medicine, Population Health Management, Rural and Remote Health, Australian Journal of Primary Health, PLoS Medicine, Medical Journal of Australia, BMC Health Services Research, BMC Public Health, BMC Family Practice) and Commonwealth and state government health websites was undertaken to acquire Australian studies of diabetes workforce models published 2005–13. Various diabetes workforce models were examined, including ‘one-stop shops’, pharmacy care, Aboriginal services and telephone-delivered interventions. The quality of evidence was evaluated against several criteria, including relevance and replication, strength of evidence, effect size, transferability and representativeness, and value for money. Results Of the14 studies found, four were randomised controlled trials and one was a systematic review (i.e. Level II and I (best) evidence). Only three provided a replicable protocol or detailed intervention delivery. Eleven lacked a theoretical framework. Twelve reported significant improvements in clinical (patient) outcomes, commonly HbA1c, cholesterol and blood pressure; only four reported changes in short- and long-term outcomes (e.g. quality of life). Most studies used a small or targeted population. Only two studies assessed both benefits and costs of their intervention compared with usual care and cost effectiveness. Conclusions More rigorous studies of diabetes workforce models are needed to determine whether these interventions improve patient outcomes and, if they do, represent value for money. What is known about the topic? Although health systems with strong primary care orientations have been associated with enhanced access, equity and population health, the primary care workforce is facing several challenges. These include a mal-distribution of resources (supply side) and health outcomes (demand side), inconsistent support for teamwork care models, and a lack of enhanced clinical inter-professional education and/or training opportunities. These challenges are exacerbated by an ageing health workforce and general population, as well as a population that has increased prevalence of chronic conditions and multi-morbidity. Although several policy directions have been advocated to address these challenges, there is a lack of high-quality evidence about which primary care workforce models are best (and which models represent better value for money than current practice) and what the health effects are for patients. What does this paper add? This study demonstrated several strengths and weaknesses of Australian diabetes models of care studies. In particular, only five of the 14 studies assessed were designed in a way that enabled them to achieve a Level II or I rating (and hence the ‘best’ level of evidence), based on the NHMRC’s (2000, 2001) frameworks for assessing scientific evidence. The majority of studies risked the introduction of bias and thus may have incorrect conclusions. Only a few studies described clearly what the intervention and the comparator were and thus could be easily replicated. Only two studies included cost-effectiveness studies of their interventions compared with usual care. What are the implications for practitioners? Although there has been an increase in the number of primary care workforce models implemented in Australia, there is a need for more rigorous research to assess whether these interventions are effective in producing improved health outcomes and represent better value for money than current practice. Researchers and policymakers need to make decisions based on high-quality evidence; it is not obvious what effect the evidence is having on primary care workforce reform.

Introduction:While medicines and medical tests are developed in a controlled clinical trial environment, postmarketing surveillance in the real world can be challenging. MedicineInsight—a database of longitudinal patient-level clinical information from primary care practices in Australia—is a novel program that collects primary care data to improve postmarketing surveillance at a national level.Methods:MedicineInsight collects de-identified clinical information from primary care practice information systems using data extraction tools. MedicineInsight currently includes 3.6 million regular patients of 3,300 family physicians (general practitioners) from 650 primary care practices across Australia. MedicineInsight data include longitudinal clinical information on diagnosis and medicines (dose, strength, route of administration, medication switches over time, adverse events, and allergies), and pathology testing data. A series of observational studies was developed for postmarketing surveillance of management of a range of health priorities including type 2 diabetes mellitus (T2DM), chronic obstructive pulmonary disease (COPD), depression, and antibiotics use.Results:Forty-four percent of patients with T2DM in the MedicineInsight database did not have a recorded hemoglobin A1c result and thirty-one percent did not have a recorded blood pressure reading in the previous 6 months. While guidelines recommend a stepwise approach to the initiation of COPD therapy, forty-nine percent of patients with COPD (with or without asthma) were prescribed dual therapy at initiation and a small number (4.5 percent) were prescribed triple therapy. Between 2011 and 2015, the annual rate of antidepressant prescribing per 1,000 family physician encounters increased by eight percent. High volumes of antibiotics were prescribed for respiratory tract infections in Australian primary care, notwithstanding guideline recommendations that antibiotics are not recommended in most cases.Conclusions:Large scale, real-world clinical data from primary care practices can play an important role in postmarketing surveillance at a national level.

For the child in residential care who, for whatever reason will not be returning to the natural family, long term foster placement is often seen as a means of providing a suitable alternative family environment. The formation of mutually satisfying emotional ties within a family setting is recognised as being an important ingredient is the healthy psychological development of a child. However, the high rate of foster placement breakdown attests to the fact that these ties are not easily formed.In South Australia, at any one time, approximately 2,300 children are known to be living apart from their parents. Some 1,500 of these children are in foster care. A 1982 Department for Community Welfare survey of a 50% sample of children who had been in continuous foster care for more than three years revealed that, while 60%of the sample had experienced only one foster placement, 24% had experienced three or more foster placements. Figures are similar In the United States where analysis of eleven foster care studies revealed that some 23% of children had three or more placements. (Westman, 1979).

Abstract. Background: There is evidence of heightened vulnerability to nonfatal suicidal behaviors among LGBT populations yet a paucity of studies into fatal behaviors. Aim: The specific aim of this article was to identify factors related to suicide in LGBT individuals in Australia. Method: The psychological autopsy (PA) method with a matched case-control study design was used. PA interviews were conducted with 27 next-of-kin of an LGBT person that had died by suicide. Three living LGBT controls per suicide case, matched by age and gender, were also interviewed. Results: The key factors relating to suicide in LGBT people were a lack of acceptance by family and self (reflected in higher internalized homophobia and shame), negative feelings about own sexuality/gender, and dissatisfaction with appearance. LGBT people who died by suicide also tended to go through coming out milestones 2 years earlier than controls. There was a higher prevalence of aggressive behaviors and a more predominant history of physical and sexual abuse. Additionally, there was greater incidence of depression and anxiety and alcohol and substance use disorders. Conclusion: Specific predictive factors for suicide in LGBT populations in Australia were identified, including significantly poorer mental health outcomes and more violence across an array of measures.

ObjectivePatients with chronic kidney disease (CKD) requiring kidney replacement therapy (KRT) in rural communities encounter many barriers in accessing equitable care and have worse outcomes compared with patients in urban areas. This study aims to describe the perspectives of patients and caregivers on access to KRT in rural communities to inform strategies to maximise access to quality care, and thereby reduce disadvantage, inequity and improve health outcomes.Setting18 studies (n=593 participants) conducted across eight countries (Australia, Canada, the UK, New Zealand, Ghana, the USA, Tanzania and India).ResultsWe identified five themes: uncertainty in navigating healthcare services (with subthemes of struggling to absorb information, without familiarity and exposure to options, grieving former roles and yearning for cultural safety); fearing separation from family and home (anguish of homesickness, unable to fulfil family roles and preserving sense of belonging in community); intense burden of travel and cost (poverty of time, exposure to risks and hazards, and taking a financial toll); making life-changing sacrifices; guilt and worry in receiving care (shame in taking resources from others, harbouring concerns for living donor, and coping and managing in isolation).ConclusionPatients with CKD in rural areas face profound and inequitable challenges of displacement, financial burden and separation from family in accessing KRT, which can have severe consequences on their well-being and outcomes. Strategies are needed to improve access and reduce the burden of obtaining appropriate KRT in rural communities.

Ira Daniel Aldridge is a case in point-a promising lad who in his formative years lacked sustained contact with his father, a celebrated touring black actor whose peripatetic career in the British Isles and later on the European continent kept him away from home for long periods. When the boy rebelled as a teenager, his father sent him abroad, forcing him to make his own way in the world. Ira Daniel settled in Australia, married, and had children, but he found it difficult to support a family. Eventually, he turned to crime and wound up spending many years in prison. The son of an absent father, he too became an absent father to his own sons, who also suffered as a consequence.

Background: Despite evidence for early integration of palliative care for people with advanced cancer and their families, patterns of late engagement continue. Prior research has focused on health professionals’ attitudes to palliative care with few studies exploring the views of patients and their carers. Aim: To explore initial perceptions of palliative care when this is first raised with patients with advanced cancer and their families in Australian settings. Design: Cross-sectional, prospective, exploratory qualitative design, involving narrative-style interviews and underpinned by an interpretative phenomenological framework. Setting/participants: Purposively sampled, English-speaking, adult patients with advanced cancer ( n = 30) and their nominated family caregivers ( n = 25) recruited from cancer services at a tertiary metropolitan hospital in Melbourne, Victoria, Australia. Results: Three major themes evolved which represent the common initial perceptions of palliative care held by patients with advanced cancer and their carers when this concept is first raised: (1) diminished care, (2) diminished possibility and (3) diminished choice. Palliative care was negatively associated with a system of diminished care which is seen as a ‘lesser’ treatment alternative, diminished possibilities for hope and achievement of ambitions previously centred upon cure and diminished choices for the circumstances of one’s care given all other options have expired. Conclusion: While there is an increasing move towards early integration of palliative care, this study suggests that patient and caregiver understandings have not equally progressed. A targeted public health campaign is warranted to disentangle understandings of palliative care as the ‘institutional death’ and to reframe community rhetoric surrounding palliative care from that of disempowered dying to messages of choice, accomplishment and possibility.

Abstract Many first language acquisition (FLA) studies have found a strong correlation between lexical and grammatical development in early language acquisition. For bilingual first language acquisition (BFLA), the development of grammar is also found to be correlated with the size of the lexicon in each language. This case study investigates how a Malay-English bilingual child developed the lexicon and grammar in each of her languages and considers possible evidence of interaction between the languages during acquisition. The study also aims to show that the predominant linguistic environment to which the child was alternatively exposed might have played an important role in her lexical and grammatical development. Thus, the study presents two sets of data: (a) a 12-month longitudinal investigation when the child was 2;10 up till 3;10 in Australia and (b) a one-off elicitation session at age 4;8 when the family was in Malaysia. The findings show that not only the emergence of grammar is linked to the lexical size of the developing languages, but that other variables, mainly the linguistic environment and the bilingual language mode, also influenced the child’s language productions.

Mental health problems are a major public health concern due to their prevalenceand impact at a personal, family, social and economic level. Compulsory communitytreatment is being utilised as a health care approach, despite much disagreement andlack of Australian research demonstrating its effectiveness. This pilot study investigatedthe utilisation and outcomes of community orders from the perspective of a client,carer, case manager and Mental Health Review Tribunal member. The findingsindicate that compulsory community treatment had a positive impact upon hospitalreadmission rates and medication usage. All groups of respondents rated communityorders as being of benefit in a number of areas. Further controlled studies are required.

The difficulties that resettled refugees experience in accessing primary health-care services have been widely documented. In most developed countries, pharmacists are often the first health-care professional contacted by consumers; however, the ability of refugees to access community pharmacies and medication may be limited. This review systematically reviewed the literature and synthesised findings of research that explored barriers and/or facilitators of access to medication and pharmacy services for resettled refugees. This review adhered to guidelines for systematic reviews by PRISMA (preferred reporting items for systematic reviews and meta-analyses). Databases were searched during March 2014 and included Scopus, ProQuest Sociological Abstracts, PubMed, Embase and APAIS Health. The Australian and International grey literature was also explored. Nine studies met the quality and inclusion criteria. The research reported in seven of the nine studies was conducted in the US, one was conducted in Australia and the other in the UK. The majority of studies focussed on South-east Asian refugees. Themes identified across the studies included language and the use of interpreters; navigating the Western health-care system; culture and illness beliefs; medication non-adherence; use of traditional medicine; and family, peer and community support. There is a significant paucity of published research exploring barriers to medication and pharmacy services among resettled refugees. This systematic review highlights the need for appropriate interpreting and translation services, as well as pharmacy staff demonstrating effective cross-cultural communication skills.

While there has been extensive research on the health and social and emotional well-being (SEWB) of Aboriginal women in prison, there are few qualitative studies where incarcerated Aboriginal women have been directly asked about their health, SEWB, and health care experiences. Using an Indigenous research methodology and SEWB framework, this article presents the findings of 43 interviews with incarcerated Aboriginal women in New South Wales, Australia. Drawing on the interviews, we found that Aboriginal women have holistic conceptualizations of their health and SEWB that intersect with the SEWB of family and community. Women experience clusters of health problems that intersect with intergenerational trauma, perpetuated and compounded by ongoing colonial trauma including removal of children. Women are pro-active about their health but encounter numerous challenges in accessing appropriate health care. These rarely explored perspectives can inform a reframing of health and social support needs of incarcerated Aboriginal women establishing pathways for healing.

Conservation of Australian plants depends on a spectrum of activities from descriptive and experimental biological studies to active management and restoration of wild ecosystems by local communities who value their native biota. On the basis of the premise that available resources for conservation will not allow for all threatened biodiversity to be saved, some systematists and conservation geneticists argue that phylogenetic relationships should be used to set conservation priorities. The principle advocated is that characters, not species number, should become the currency of conservation, that cladistic analysis of phylogenetic pattern provides a predictive means of modelling the underlying distribution of characters among taxa, and that priority should be given to that subset of taxa with the greatest number of character states. This approach has been applied for some time in the conservation of genetic resources within species (e.g. Eucalyptus caesia), and has been an extra impetus for action with taxonomically isolated endangered species such as the recently discovered araucarian Wollemi pine (Wollemia nobilis). However, most plant conservation activity in Australia has paid little heed to phylogenetic relationships. Degree of threat has been a driving motivation, with endangered species receiving legislative and management attention irrespective of their systematic relationships. Moreover, the current revolution in understanding plant phylogeny associated with DNA sequence studies highlights the need for caution in accepting results of morphologically based analyses. A series of studies on the kangaroo paw and bloodroot family Haemodoraceae highlights this cautionary tale. The derivation of phylogenetic principles relevant to ecosystem and landscape processes is a new field of some promise to conservation managers. An understanding of the Gondwanan origins and landscape evolution of the south-west Australian flora provides a useful case study. Scaling up phylogenetic knowledge of genetic resources within species, and of the evolutionary relationships of taxa to an integrated overview of best management practices for all taxa at the local landscape level, is perhaps the most effective contribution phylogeneticists might make to help conserve Australian plants.

The present study examines: a) the relationship between religious orientations and interest in the ancestral past; and b) the discursive practices of genealogists in dealing with time and death. Genealogy refers to the construction of family pedigrees in terms of births, marriages, and deaths, embellished with stories about the historic family and individual ancestors. Analyses are based on quantitative and qualitative data taken from personal interviews and surveys conducted in Canada (in 1994 and 1998) and Australia (in 1999). Multivariate analyses of national data failed to find effects for religiosity on passive or active interest in genealogy, but found significant negative effects for belief in an afterlife on both measures. Qualitative analyses of genealogists' accounts of their experiences in the pastime revealed symbolic and largely secular strategies designed to extend the time frame of their lives beyond their personal biographies, and to address the twin challenges posed by aging and death on the part of close family members and themselves.

Fatal work incidents result in an array of government responses, and in countries such as the United Kingdom and Australia, this may include the holding of coronial inquests. A common theme from the scant literature is that family members have a strong need to know how and why their loved one died. The inquisitorial nature of inquests suggests potential in uncovering this information, although little is known about families’ experiences with these proceedings. Interviews with 40 bereaved relatives explored their views and experiences of inquests. Findings suggest that families, often frustrated with other investigative processes, want inquests to provide a better understanding of how and why the death occurred, uncover any failings/responsibilities, and thereby move closer to a sense of justice being obtained for the deceased. Families identified problems perceived to impair the process and where improvements could be made to secure a more effective and meaningful institutional response to the fatality.

Abstract Background Studies examine longitudinal continuity of GP contact though few consider ‘regularity of GP contact’, i.e., the dispersion of contacts over time. Increased regularity may indicate planned ongoing care. Current measures of regularity may be correlated with the number of contacts and may not isolate the phenomenon of interest. Objectives To compare two published and one newly developed regularity index in terms of their ability to measure regularity of GP contacts independently of the number of contacts and the impact on their association with hospitalization. Methods A cohort at risk of diabetes-related hospitalization in Western Australia from 1990 to 2004 was identified using linked administrative data. For each regularity index, relationships with number of GP contacts were assessed. Hospitalization was then regressed on each index with and without number of contacts as a covariate. Results Among 153,414 patients the new regularity index showed a reduced association with number of contacts compared with existing indices. Associations with hospitalization differed between measures; for previously published indices, there were no significant associations between regularity and hospitalization, whereas on the new index, most regular GP contact was associated with reduced hospitalization (IRR = 0.90, 95% CI = 0.88–0.93). When number of contacts was added as a covariate, point estimates for this index showed little change, whereas for existing measures this addition changed point estimates. Conclusion A new measure of regularity of GP contact was less correlated with the number of contacts than previously published measures and better suited to estimating unconfounded relationships of regularity with hospitalization.

ObjectiveMutations in the aryl hydrocarbon receptor-interacting protein (AIP) were recently shown to confer a pituitary adenoma predisposition in patients with familial isolated pituitary adenomas (FIPA). We report a large Samoan FIPA kindred from Australia/New Zealand with an R271W mutation that was associated with aggressive pituitary tumors.Design and methodsCase series with germline screening of AIP and haplotype analyses among R271W families.ResultsThis previously unreported kindred consisted of three affected individuals that either presented with or had first symptoms of a pituitary macroadenoma in late childhood or adolescence. The index case, a 15-year-old male with incipient gigantism and his maternal aunt, had somatotropinomas, and the maternal uncle of the index case had a prolactinoma. All tumors were large (15, 40, and 60 mm maximum diameter) and two required transcranial surgery and radiotherapy. All three affected subjects and ten other unaffected relatives were found to be positive for a germline R271W AIP mutation. Comparison of the single nucleotide polymorphism patterns among this family and two previously reported European FIPA families with the same R271W mutation demonstrated no common ancestry.ConclusionsThis kindred exemplifies the aggressive features of pituitary adenomas associated with AIP mutations, while genetic analyses among three R271W FIPA families indicate that R271W represents a mutational hotspot that should be studied further in functional studies.

e16263 Background: Despite being one of the deadliest tumors worldwide, hereditary factors involved in PC are not fully understood. Thus, our objective is to assess the prevalence of germline mutations in PC cohorts not selected for FH. Methods: A systematic review was performed, using the descriptors related to PC and germline mutations, employing Pubmed, Lilacs, Web of Science and Embase, until August/2020. Three investigators were involved in reviewing titles and abstracts by peers. Exclusions comprehended articles of non-English language, studies investigating endocrine PC or case-reports. For the present analysis, the inclusion criteria were: original full paper available for data extraction, studies with patients unselected for FH and evaluating a panel of at least 10 genes, WES or WGS. Results from our group evaluating a cohort of PC Brazilian patients were also considered. We present prevalence results for germline mutations in 8 genes, comprehending ATM, BRCA1, BRCA2, CDKN2A, CHEK2, MSH6, PALB2 (the most frequently mutated genes in PC) and FANCM (previously detected in our Brazilian cohort). Results: After exclusion of duplicate titles, 2, 035 titles were available. Using the criteria described above, 23 articles were selected for analysis and involved cohorts from Australia, Canada, USA, Japan, Czech Republic and our Brazilian cohort, totaling 11, 165 PCP evaluated. The predominant histology was adenocarcinoma (15/24 studies), 22/24 studies evaluated a panel of genes and for studies providing demographic data, the mean age of PCP was 64 (13-93) years (n = 8, 000). Six out of eight genes were evaluated in at least 10, 600 patients, among which BRCA2 mutations had the highest prevalence, followed by ATM. Mutations in CHEK2 occupied the third place in prevalence, despite being evaluated in only 74.5% of the entire cohort. BRCA1, PALB2, CDKN2A, MSH6 and FANCM showed a prevalence of less than 1%, according to the table below. Although being infrequent among PCP, it is interesting to observe that an identical FANCM mutation was detected in two unrelated Brazilian patients. Conclusions: Analysis of cohorts unselected for FH indicates that BRCA2, ATM and CHEK2 are the most frequently mutated genes in these PCP. Further studies are needed to better characterize the spectrum and prevalence of mutations in PCP from populations other than Caucasians, especially in miscegenated people.[Table: see text]

Intercountry adoption, often of children post-infancy, is one way of forming a family in Australia. However, few studies have invited Australian parents who have incorporated older children into their family to tell their story. Fathers are under-represented in studies of parenting generally and adoptive parenting specifically.As part requirement for a clinical psychology dissertation, with ethics approval from the relevant university, 28 parents (13 fathers and 15 mothers) were interviewed about their experiences of adopting children over the age of 24 months from orphanages in China, Ethiopia, India and Thailand. Although parents’ experiences and recollections were diverse, almost all parents had been confronted by difficult child behaviours, at least initially. Contrary to previous research, the child's gender, age or duration of institutional care did not appear related to parental experience.Six major themes emerged from parent interviews: (1) the long wait and intense emotions of adoption; (2) disparity between expectations and reality; (3) recognition of children's difficult past experiences; (4) parenting as a path to self-discovery; (5) the perception of needing to present as coping; and (6) unmet needs. Mothers blamed themselves for their children's behavioural problems, rather than attributing difficulties to children's previous adverse life events. Both mothers and fathers were reluctant to use support services because they felt scrutinised and feared repercussions, and those who sought assistance generally found professionals ill-informed and unhelpful. Parents made recommendations about how the adoptive parenting process could be improved and expressed a strong desire for more information, both pre- and post-placement.
