Texto completoSince the end of the 19th century, the history of Augustinian historiography has been characterized by numerous and varying attempts to find breaking points in Augustine’s philosophy and life, as well as strong interests about his intellectual development. The analyses of Augustine’s writings against the Pelagians, with an emphasis on methodology refutation, is an efficient interpretative key of Augustine’s intellectual and philosophical development. This thesis is divided into two chapters, both dedicated to the analysis of Augustine’s writings against the Pelagians, and also containing a short appendix in which consideration is given to the sermons delivered by Augustine in the early stages of the anti-Pelagian controversy. In the first chapter, De peccatorum meritis et remission et de baptismo parvulorum, written in order to refute the theories sustained by Pelagians and condemned by the Council of Carthage in 411, is analyzed. De spiritu et littera is then analyzed, explaining Augustine’s perspective that the impeccantia is possible only by the implementation of Christ. The analysis of De natura et grazia follows, in which Augustine asserts, in agreement with what Pelagius wrote in his De natura, stating that human nature is good, but emphasizes that it is in need of God’s grace in order to desire and act well and according to God’s will. The analysis continues with De perfectione iustitiae hominis, composed to refute the Definitiones ut dicitur Celestii, a book in which Celestius proposes sixteen arguments in order to show that humans have the ability and must live without sin. De gestis Pelagii is then analyzed, a book in which Augustine examines and assesses the orthodoxy of Pelagius’ defense, given during the Synod of Diospolis. The first chapter is concluded with the analysis of De gratia Christi et de peccato originali, where the important topics involved in the controversy against the Pelagians are summarized. In the second chapter De nuptiis et concupiscentia is analyzed, a book in which Augustine distinguishes the evil of the concupiscence from the goodness of marriage. The analysis of Contra duas epistolas Pelagianorum follows, written to refute two Pelagian letters that circulated in Rome as propaganda. In conclusion, Contra Iulianum and Contra secundam Iuliani responsionem, Opus imperfectum are analyzed; they were composed to respectively disprove Ad Turbantium and Ad Florum of Julian of Eclano, where Augustine’s doctrine of original sin is accused as Manichean. In the appendix, Augustine’s Sermones are analyzed; it is probable that these sermons against the rising Pelagian heresy were given before 411 or, in any case, before its official intervention in the Pelagian controversy. This thesis shows that despite the undeniable evolution of Augustine’s thoughts, which forced him to redefine some of his theological positions, the methodology outlined in Dialogues and adopted since his conversion to Christianity is not disqualified. According to that methodology, philosophical speculation must be triggered by God’s calling and conducted by preceding faith to reason. [edited by Author]

Texto completoItaly has introduced a support for adults affected by psychiatric disorders called IESA (Inserimento Eterofamiliare Supportato di Adulti) as an alternative to hospital facilities institutionalization, and other types of residential care. Patients are accommodated and supported in private homes, sharing the family life. The aim is the rehabilitation and social integration of patients with a better quality of life. Objective. Purpose of this longitudinal study was to evaluate the outcome of IESA considering psychopathology, psychological and social well-being and family functioning. Methods. 40 subjects: 20 patients and 20 hosts. Clinical assessment was performed at the beginning of cohabitation and at follow-ups after 1, 3, 6 and 12 months. Psychometric instruments: BPRS, GAF, PWB, SQ, FAD. Statistical analysis: General Linear Model with repeated measures analysis of variance and effect-size calculation. Results. 15 male and 5 female patients, 17 of them Italians. Eleven patients meet DSM-IV-TR for schizophrenia and psychotic disorders, 5 for mood disorders and 4 for personality disorders. After living with a family 3 were hospitalized and 4 required emergency psychiatric assessment. Eight patients changed or decreased psychopharmacotherapy and 3 patients didn’t need it anymore. The psychological well-being (PWB) increased; psychopathological symptoms (BPRS/SQ) decreased and level of functioning (GAF) improved. Families included 11 men and 9 women, 19 of them Italians. Eleven people work and 8 are retired. Mean age: 55 years. Marital status: 8 married, 6 unmarried, 4 divorced and 2 widowed and 9 of them have children. Psychological well-being (PWB) of families increases, functioning family (FAD) improved and global functioning (GAF) remained constant over time. Discussions and conclusions. IESA seems to improve psychopathology, reducing maladaptive behaviours, increasing patient’s social skills and promoting integration. In addition, IESA appears to decrease the costs of chronic psychiatric disorders, reducing access emergency medical services, urgent psychiatric assessment and days of hospitalization.

Texto completoBACKGROUND. Heart failure is often the result of the clinical progression of several common diseases, such as ischaemic heart disease, arterial hypertension, and cardiomyopathy, with an incidence of 2 million cases per year in the world, 470,000 in Europe, and 66,000 in Italy. Heart failure is the most frequent cause of hospitalization in older people. In the last decade, there has been a rapid increase in the use of Health Technology Assessment, pacemakers, and defibrillators, which have ensure better clinical results. Implanted heart devices play an important role in telemonitoring heart failure. Telemonitoring systems enable to check, at any time, current and previous data produced by the implanted device, resulting in early detection of arrhythmias or incipient anomaly of the stimulation or heart defibrillating system. PURPOSE. To evaluate if nurses dedicated to telemonitoring heart failure (HF) patients with implanted cardiac devices, such as PM, ICD, ICD-CRT and LOOP RECORDER, reduce hospital readmissions and mortality. METHODS. A study with a convergent parallel mixed method design was used with two components: a descriptive qualitative study and a retrospective observational cohort study. A sample of 849 remotely monitored patients with implanted cardiac devices was recruited from January 2011 to December 2016 from electronic records. The primary endpoint of the study is to check how this activity reduces readmissions. Qualitative data were collected through semi-structured interviews with 10 dedicated nurses, to understand what type of education and practical training they had received in relation to their current practice in the field of cardiac nursing and evaluate how nursing impacts on the organization of care in terms of nursing sensitive outcomes. RESULTS. We found that in the centres where there was nurse dedicated to remote monitoring, hospital readmissions decreased by almost 20%, of patients who met admission appropriateness according to their diagnosis on admission. Mortality decreased in the centres with a nurse fully dedicated to remote monitoring of HF patients compared to centres where there was no dedicated nurse. A correlation between death and baseline diagnosis is highlighted. In hospitals where the seniority of cardiac nurses was higher, there was a significant reduction of readmissions and mortality. Conclusion (s). The benefits for the patients were found to be relevant in terms of reducing social costs: reduction of transport costs for patients by 60%, reduction of working days lost by the patient and / or carers, failure to give up personal activities (patient and family members) and greater patient satisfaction and acceptance with better follow-up adherence. From the data received through the telemonitoring systems, there was a reduction of inappropriate shocks, prevention of heart failure events, reduction of the number and duration of readmissions and improved survival.
