AbstractThe clinical dentistry and endodontic procedures involve very technique-sensitive procedures, therefore exposing the operator to risks of causes not only damage to patients but also leads to malpractice. Among various disciplines of dentistry, endodontics-related cases witness the most frequently filed malpractice claims. This is due to the fact that the endodontic treatment procedures involve operative and surgical procedures, using a variety of medicaments and techniques. The endodontic procedural errors can be preoperative errors (such as incorrect diagnosis and misinterpretation), intraoperative errors including root canal and pulp chamber perforations, ledge formation leading to apical transportation or zipping, hypochlorite accidents, and fracture of instruments. More critically, failure to use rubber dam may result in inhalation or ingestion of endodontic instruments. Under such circumstances, the endodontist may have to face legal consequences. Due to the increased healthcare load and patients’ awareness, it is important to know the legal ramifications of adverse effects, failed restorations, or other complications, to avoid any legal ramifications of endodontic procedures and associated techniques. Therefore, precautions must be taken to prevent any postsurgical complications, patient complaints, and/or failures. For this purpose, the operator must consider ethical principles and adhere strictly to the standards of healthcare while performing the diagnosis and treatment. A referral toward a specialist or consultant endodontist is always an appreciable option and should be considered in the best interest of the patient. The aim of the article is to highlight various aspects of malpractice in clinical endodontics, and associated materials and challenges. In addition, commonly occurring operating errors during endodontic treatment, possible consequences, precautions, and management have been discussed.

Abstract The aim of this review is to discuss the digital planning and the use of guided technology in Endodontics. The complexity of the root canals anatomy and the challenges in the microorganism’s control represent risk factors for failure after the infected root canal’s treatment. Scientific improvements associated with technological advances have enabled better predictability of therapeutic procedures results. The development of efficient and modern devices provided safer root canal treatments, with shorter clinical visits and greater patient comfort. Digital endodontics incorporated different tools and developed its own, advancing even further in resolving complex cases. The faithful copy of the internal anatomy provided by the advancement of CBCT devices and software’s, associated with the digital resources of 3D planning and printing, enabled the advent of guided endodontics. This technique is used at different stages of endodontic treatment, with specific indications and greater result predictability. Therefore, this study critically reviewed the potential clinical application of this guided access technique, and the operative steps for its safe performance in managing complex endodontic cases. The main indications are accessing calcified root canals, performing endodontic surgeries in difficult access areas, removing fiberglass posts, and accessing teeth with developmental anomalies. In summary, guided endodontics has been a precise strategy, effective, safe, and clinically applicable. This procedure represents incorporating technological resources and digital planning in the Endodontist clinical practice, increasing predictability to complex cases.

Specialisation in endodontics allows for endodontic referrals by general dental practitioner (GDPs) and the study of factors influencing referral. These centre on a triad consisting of the referral process, non-clinical and clinical reasons for referral. Many non-clinical factors have been identified which may influence the referral process to the endodontist. A systematic review study was undertaken into the main non-clinical factors influencing endodontic referral by general dental practitioners to endodontists. Such awareness and appreciation of these factors benefits the commercial aspect of the referral practice, increases access by reducing barriers to care, and ultimately improves patient care. A literature search yielded three papers that met the eligibility criteria. All studies included were cross sectional survey studies completed by GDPs. The main non-clinical factors seen from the studies include: • Availability. • Personality, relationships and communication. • Location. Availability presented as a common thread throughout all the studies. In conclusion, endodontic referral is multifactorial and influenced by several factors, that are not related to the nature of the endodontic disease, and this is a dynamic process. Due to the lack of high level studies, and limitations of the available studies, further research is suggested into relevant area of non-clinical endodontic factors for endodontic referral and thus allowing for further analysis.

Pendahuluan: Gigi dengan kerusakan mahkota yang sangat luas sering menimbulkan masalah dalam perawatan endodontik seperti kesulitan dalam penempatan klem rubber dam sehingga isolasi yang kurang memadai serta kemungkinan terjadinya kebocoran koronal yang mengakibatkan risiko kegagalan perawatan endodontik. Gigi dengan keadaan tersebut membutuhkan restorasi sementara pre endodontic agar dapat dilakukan isolasi yang memadai, penempatan klem yang efektif selama perawatan dan pemulihan estetik sementara selama perawatan saluran akar. Tujuan laporan kasus ini untuk menjelaskan pembuatan pre endodontic build-up menggunakan alat tip plastik sekali pakai selama perawatan saluran akar pada gigi insisif rahang atas dengan kerusakan mahkota yang sangat luas. Laporan kasus: Pasien perempuan berusia 53 tahun datang ke Klinik Konservasi Gigi RSGM Unpad dengan keluhan gigi seri rahang atas kiri patah saat makan. Gigi tersebut pernah dirawat saluran akar satu tahun yang lalu namun tidak selesai. Pemeriksaan klinis menunjukkan kehilangan mahkota sampai batas servikal, dan terdapat gambaran radiolusen yang difus di area periapikal. Diagnosis berdasarkan American Association of Endodontics (AAE) adalah previously initiated therapy disertai periodontitis apikalis asimptomatik. Pembuatan pre endodontic build-up dengan teknik canal projection dilakukan untuk mendapatkan isolasi sekaligus sebagai restorasi sementara dan dilanjutkan dengan perawatan saluran akar dan pembuatan mahkota all porcelain dengan fiber post. Simpulan: Pembuatan pre endodontic build-up menggunakan teknik canal projection berhasil dilakukan selama perawatan saluran akar gigi insisif lateral kiri atas dengan kerusakan mahkota yang sangat luas. Pre endodontic build-up dengan teknik canal projection pada kasus ini dapat memberikan isolasi yang memadai, memudahkan penempatan klem rubber dam serta memberikan pemulihan estetik yang baik selama periode antar kunjungan. ABSTRACTIntroduction: Extensive crown damage often causes problems in endodontic treatment, such as difficulty placing rubber dam clamps, resulting in inadequate isolation and the possibility of coronal leakage, which results in treatment failure risk. This condition requires pre-endodontic temporary restorations to provide adequate isolation, effective clamping during treatment, and temporary aesthetic restoration during root canal treatment. The purpose of this case report was to describe the fabrication of a pre-endodontic build-up using a disposable plastic tip device during root canal treatment of a maxillary incisor with extensive crown damage. Case report: A 53-year-old female patient came to the Conservative Dentistry Clinic of Universitas Padjadjaran Dental Hospital to complain of a fractured left maxillary incisor that occurred while eating. The tooth had a root canal treatment one year prior but was not completed. Clinical examination revealed crown loss to the cervical margin and a diffuse radiolucent appearance in the periapical area. According to the American Association of Endodontics (AAE), the diagnosis was previously initiated therapy with asymptomatic apical periodontitis. Therefore, the pre-endodontic build-up using the canal projection technique was performed to obtain isolation and temporary restoration, continued with root canal treatment and the manufacture of an all-porcelain crown with fibre post. Conclusions: The pre-endodontic build-up with canal projection technique has been successfully performed during root canal treatment of the maxillary left lateral incisor with extensive crown damage. Pre-endodontic build-up with canal projection technique can provide adequate isolation, facilitate the placement of rubber dam clamps, and provide good aesthetic recovery during the period between visits.

The incorporation of nanoparticles into endodontic sealers aims at increasing antimicrobial activity of the original material. The aim of this study is to incorporate the nanostructured silver vanadate decorated with silver nanoparticles (AgVO 3, at 2.5%, 5%, and 10%) into three endodontic sealers and evaluate the antibacterial activity of freshly sealers, surface topography and chemical composition, and setting time. The AgVO 3 was incorporated into AH Plus, Sealer 26, and Endomethasone N at concentrations 0%, 2.5%, 5%, and 10% (in mass). All the achievements of modern therapeutic dentistry are based on many years of experience in scientific research, supported by the practical activities of doctors. Numerous data on the formation of dentistry in different eras make it possible to track the development trend of endodontics as an independent discipline from the earliest stages of the development of medicine. The discovery was made in northern Italy by an international team of archaeologists. The carious cavity was exposed to abrasive materials, as evidenced by numerous notches on the walls. That is why two editors, Niraj Kinariwala and Lakshman Samaranayake, united another 18 contributors from totally 6 countries (Austria, Brazil, China, Denmark, Hungary, and India) bringing to the world Guided Endodontics. The textbook consists of 10 Chapters; all of them are dedicated to planning, manufacturing and application of all types of endodontic guides: (1) types depended upon their use in endodontic treatment (non-surgical and surgical guides), (2) types depended upon their support, and (3) types from classification of surgical endodontic templates. Perfect illustrations: computed tomography (CT), micro-CT data, virtual surgical planning, digital workflow, intraoperative images, and control X-rays immerse the reader in the smallest details of the guided treatment. Chapter 8, “Static Guided Approach in Surgical Endodontics” perfectly describes guided microscopic root-end resection using stereolithographic manufacturing what is extremely useful in modern practice. In summary, this textbook is a phenomenal chance for every endodontists to absorb all we need to know about state of the art principles and techniques of guided endodontics. I can honestly say that with profound knowledge and appropriate implementation of these techniques any practice limited to endodontics will reach the highest level of modern endodontic specialty.

Photodynamic Therapy (PDT) has been growing in endodontics due to the excellent potential for in vivo root canal decontamination. This literature review aims to discuss the effectiveness of PDT in assisting as an adjuvant to endodontic treatment and the different clinical applications and their results. Thorough research was performed between March and May 2020, using the “Terapia fotodinâmica” and “Endodontia” descriptors indexed in the Descriptors in Health Sciences (DeCs) and the “Photodynamic therapy” and “Endodontics” descriptors indexed in the Medical Subject Heading Terms (MeshTerms –MeSH). The databases searched were SciELO, Pubmed, and LILACS, for publications between 2013 and 2020. In this bibliographic survey, it was selected 28 articles for a full-text analysis and used as references. The bibliographic survey showed that PDT effectively assists the endodontic treatment, offers a high potential for eliminating bacteria such as Enterococcus faecalis, and is little invasive and risk-free for patients. It was concluded that PDT is clearly effective, but its clinical applicability is still subject to different protocols, which requires developing a protocol for common use.

AbstractNon-surgical endodontic retreatment should always be the first option for reintervention when the initial endodontic treatment fails. The surgical treatment, called periradicular surgery, will be the procedure of choice when there is no success after the conventional endodontic retreatment. The purpose of this article is to describe clinical case of endodontic surgery, associated with guided tissue regeneration (GTR). A male patient, 24 years old, was referred for endodontic surgery on tooth 12 after two unsuccessful endodontic interventions. During the surgery, osteotomy, lesion curettage, apicectomy, retrograde obturation with Mineral Trioxide Aggregate (MTA), and filling of the bone failure with lyophilized bone and reabsorbable collagen membrane were performed. After six months of follow-up, the patient did not present any type of painful symptomatology. The endodontic surgery, associated with a technique of guided tissue regeneration, was efficient to solve this clinical case. Keywords: Endodontics. Apicoectomy. Guided Tissue Regeneration. Resumo O retratamento endodôntico não cirúrgico deve sempre ser a primeira opção de reintervenção quando o tratamento endodôntico inicial falha. Já o tratamento cirúrgico, ou cirurgia parendodôntica, será o procedimento de escolha quando não há sucesso após o retratamento endodôntico convencional. O objetivo deste artigo é descrever um caso clínico de cirurgia parendodôntica, associada à regeneração tecidual guiada (RTG). O paciente, gênero masculino, 24 anos, foi encaminhado para cirurgia parendodôntica no dente 12 após duas intervenções endodônticas sem sucesso. Durante a cirurgia foram realizadas manobras de osteotomia, curetagem da lesão, apicectomia, obturação retrógrada com Mineral Trióxido Agregado (MTA), além de preenchimento da falha óssea com osso liofilizado e membrana de colágeno reabsorvível. Após seis meses de acompanhamento do caso, o paciente não apresentou nenhum tipo de sintomatologia dolorosa. A cirurgia parendodôntica, associada à técnica de regeneração tecidual guiada, foi eficiente para solucionar este caso clínico. Palavras-chave: Endodontia. Apicectomia. Regeneração Tecidual Guiada.

Given the importance of risk management to avoid mishaps, to achieve a quality result and to ensure a favourable outcome, challenging endodontic cases are best treated by clinicians with the appropriate level of training and experience. Digital and technological innovations in endodontics have led to the development of web-based and smartphone-compatible case-difficulty assessment tools that can help less-experienced dentists identify endodontic management complexities. These interactive tools may also be used for other applications, including primary and secondary care triage, research and dental education. Similarly, advances such as artificial intelligence and mixed reality technologies, are predicted to also benefit endodontics and help support dentists in the management of complex endodontic cases. CPD/Clinical Relevance: Digital and technological developments may help improve the management and treatment of endodontic cases.

Bioceramics play a key role in endodontics, mostly in surgical endodontics and difficult cases of root canal treatment. Carbonate apatite (CO3Ap) is the natural composition of dental and bone. The endodontic sealer is used in the obturation phase of endodontic treatment, which requires sealing ability and bioactivity to promote osseous repair of teeth and alveolar bone. CO3Ap crystal formation is essential for endodontics sealer as a bioceramic sealer. This study aims to investigate the crystal characterization of CO3Ap through X-Ray diffraction (XRD) and scanning electron microscope (SEM). The bioceramics endodontic sealer comprises dicalcium phosphate anhydrous (DCPA), vaterite, and Ca(OH)2. The powder cement ratio divided into 60% DCPA : 30% vaterite: 10% Ca(OH)2. Powder cement was mixed with 0.2 mol/L Na2HPO4 added by 1% sodium carboxymethylcellulose and 32μg thymoquinone as an aqueous solution at the liquid to powder ratio of 0.6 and set at 37°C and 100% of relative humidity for 72h. XRD result showed that all precursor materials transformed into CO3Ap after 72h treatment. SEM image showed coral-like CO3Ap morphology that is characteristic of CO3Ap. The initial results of novel CO3Ap endodontic sealers show that crystal formation occurs and has potential to be used as an endodontic sealer.

A patient with an unusual endo-periodontal lesion, without coronal decay or damage, likely caused by a deep periodontal lesion with subsequent endodontic bacterial migration, required medical care. Next-generation sequencing (NGS) was used to assess the endodontic microbiota in vestibular and palatal canals after tooth extraction, evidencing a predominant population (Fusobacterium nucleatum) in one endodontic canal, and a mixed bacterial population with six major populations almost equally distributed in the other endodontic canal (F. nucleatum, Porphyromonas gingivalis, P. endodontis, Parvimonas, Peptostreptococcus stomatis, Prevotella multiformis). These data could suggest different, separated ecologic niches in the same endodontic system, with potentially different pathogenicity levels, clinical manifestations and prognoses for every single canal of the same tooth.

Introdução: Lesões endoperiodontais são lesões originadas de produtos inflamatórios encontrados tanto em periodonto quanto em polpa. Tais lesões podem se originar devido a uma infecção pulpar ou periodontal. Visando o prognóstico favorável, é imprescindível o conhecimento da etiologia, realização do correto diagnóstico e elaboração do plano de tratamento que envolve o tratamento endodôntico precedido do tratamento periodontal. Objetivo: O propósito do presente trabalho foi de relatar um caso clínico de lesão endoperiodontal e o tratamento realizado. Relato de caso clínico: Paciente gênero feminino, 51 anos, compareceu à clínica com uma fístula na região do dente 46, procedeu-se com exame radiográfico, rastreamento de fístula, testes endodônticos e avaliação periodontal. Foi diagnosticada lesão endoperiodontal. Executou-se, então, o tratamento endodôntico em sessões múltiplas, utilizando hidróxido de cálcio como medicação intracanal e o tratamento periodontal concomitante; finalizou-se endodontia obturando-se os canais radiculares. Conclusão: Observou-se, no controle, que a associação de tratamentos foi eficaz e houve melhora significativa do quadro, constatando-se silêncio clínico e sucesso do tratamento. Realizar o tratamento conservador a despeito da exodontia foi a melhor escolha para a paciente. Descritores: Endodontia; Periodontia; Polpa Dentária; Periodonto. One of the most challenging and difficult task in clinical practice of an endodontist is to manage pain. Advancements in understanding of pain physiology and pharmacology have provided a key upon which quick decisions can be made for treatment of any endodontic case. Despite advancement in technology, patients perception and apprehension is till date considered as a standard to acknowledge the expertise of an endodontist when undergoing a routine endodontic therapy. The current article aims to reflect effective strategies of management of endodontic pain in pre-operative, operative and postoperative situations. In lieu of clinical relevance of this topic in day to day life of an endodontist the topic was also addressed as a scientific paper presentation in a national conference to welcome thoughts of colleagues at a broader platform.

ABSTRACT Endodontic surgery has developed the perception as an unnecessary procedure, with the emergence of implants. This opinion has been based on endodontic surgery being performed using obsolete concepts and techniques that compromised the potential for clinical success, frequently resulting in the persistence of patient symptoms, periapical pathology, and, ultimately, extraction of the treated tooth. With the emerging concepts of magnification, advanced principles of soft and hard tissue management, use of tissue regenerative root-end filling materials, and enhanced principles of wound closure, surgical endodontics has emerged as a highly predictable and comparatively painless procedure. Periapical surgery should be regarded as the method of choice when orthograde endodontic therapy cannot be performed. Objective.: To correlate the impact of pre-operative anxiety and its influence on endodontic treatment. Methods: Data of 500 patients who were scheduled for endodontic treatment were enrolled in the study. These patients were treated in the Dental Section of Islam Dental College, Sialkot. Modified. Dental. Anxiety. Scale. (MDAS) was used to assess the patient’s anxiety levels during endodontic treatment.. Numeric pain scale. was used to document the pain perceived during endodontic therapy. Results: Anxiety and pain showed a statistically significant correlation-ship during endodontic treatment. Our findings were suggestive of increased pain perception during endodontic therapy and its association with anxiety. Minimal anxiety was found in 25 cases, mild anxiety was found in 216 cases, moderate anxiety was found in 127 cases, high anxiety was found in 129 cases and extreme anxiety was found in 3 cases. 250 patients reported with mild pain, 158 patients reported with moderate pain and 92 cases reported severe pain. Conclusion: Assessment of anxiety levels prior to performing endodontic procedures can pro-actively benefit the patient and the operator. Yes, advanced methods of pain control are available in endodontics but they need to be implicated judiciously. The introduction of NiTi rotary files into endodontic practice is one of the biggest steps towards more successful and better canal treatment. New perspectives in endodontics have been opened, thanks primarily to the extreme flexibility and the feature of smart materials to be able to return to their original shape after deformation. New concepts of making these files in recent years include the introduction of new alloys to improve their features, ie a new specific design of the workpiece that provides efficient cutting and elimination of dentin with a significant reduction in stress and possible complications during hemomechanical processing of canals. The introduction of new alloys for the manufacture of files and their exposure to special thermal treatment has affected their superior features, greater flexibility and increased resistance to cyclic fatigue. The concept of single-file treatment includes various systems that use full rotation, reciprocal movements (or a combination thereof), or systems with eccentric and transaxial movements to increase cutting efficiency and reduce cyclic file fatigue. The new movements of the files also offer greater safety and efficiency because they significantly reduce the contact of the files with the walls and thus the stress during the preparation of the canal. The preparation of the canal with only one file is based on the concept of "less is more" and has fundamentally changed a lot in endodontics (conceptually, procedurally and economically), and made endodontic intervention simpler, faster and more certain. This significantly shortens the duration of endodontic intervention, eliminates the possibility of cross-contamination and significantly saves time and financial expenses for both patients and endodontists. Understanding the design of new NiTi files and the correct choice of movement dynamics during chemomechanical processing allows the practitioner to maximize his endodontic skill in each endodontic technique, to increase the efficiency of the intervention and significantly minimize the possibility of error. The aim of this paper is to present a perspective concept of single-file treatment, to present the development and strategies of different systems of single NiTi files and to present the indications and the possibility of their application in different clinical situations.

Latar belakang. Masalah pulpa dan periodontal menyebabkan lebih dari 50% kehilangan gigi. Kasus pada laporan kasus ini merupakan lesi primer endodontik dan lesi sekunder periodontal dan secara klinis terdapat sinus tract. Kondisi ini merupakan kasus yang secara kolaborasi dikerjakan oleh bidang konservasi gigi dan bidang periodonsia untukmemdapatkan hasil yang maksimal. Kasus. Terdapat tiga kasus lesi endodonti-periodontal, dua merupakan kasus (gigi 21 dan 37) yang setelah beberapa tahun dilakukan perawatan saluran akar (PSA) mengalami pembentukan sinus tract, dan satu kasus (gigi 47) yang setelah PSA namun tidak memeperlihatkan perbaikan sinus tract yang terbentuk di gingiva. Tindakan bedah flap periodontal dilakukan untuk mencari penyebab, menghilangkan jaringan granulasi, dan memperbaiki kerusakan tulang alveolar yang terjadi. Seluruh kasus terlihat terdapat kerusakan tulang anguler di daerah furkasi pada gigi posterior (37 dan 47), dan daerah interdental pada gigi anterior (21); satu kasus (gigi 21) mengalami fenestrasi di fasial. Defek tulang anguler dan daerah fenestrasi setelah dibersihkan dari jaringan granulasi yang terinfeksi, diisi dengan graf tulang dan ditutup oleh membran pericardium sebagai guided tissue regeneration (GTR), kemudian dijahit. Kontrol 14 hari setelah tindakan bedah, sinus tract telah hilang dan warna gingiva normal. Kesimpulan. Lesi endodontik-periodontal yang memperlihatkan sinus tract yang persisten setelah perawatan saluran akar merupakan indikasi adanya kerusakan periodontal yang kompleks. Defek periodontal kompleks bisa diperbaiki dengan tindakan bedah regeneratif.Kata kunci. Lesi endodontik-periodontal, sinus tract, defek tulang anguler, fenestrasi Abstract Background. Dental pulp and periodontal problems account for more than 50% of tooth loss. The cases in this case report were primary endodontic lesions and secondary periodontal lesions and clinically contained a sinus tract. This report is a collaborative carried out by the conservative dentistry and periodontics to obtain maximum results. Case. There were three cases of endodontic-periodontal lesions, two were cases (teeth 21 and 37) where after several years of root canal treatment had sinus tract formation, and one case (tooth 47) after endodontic treatment but did not show any improvement in the sinus tract in the gingiva. Periodontal flap surgery is performed to find the cause, remove the granulation tissue, and repair the alveolar bone damage that has occurred. All cases showed angular bone defects in the furcation areas of the posterior teeth (37 and 47), and the interdental areas of the anterior teeth (21); one case (tooth 21) had facial fenestration. Angular bone defects and areas of fenestration after cleaning of infected granulation tissue, filled with bone graft and covered by pericardial membrane as guided tissue regeneration (GTR), then sutured. Control 14 days after surgery, the sinus tract was gone and the gingival color was normal. Conclusion. Endodontic-periodontal lesions showing persistent sinus tracts after root canal treatment are indicated of complex periodontal damage. Endodontic therapy seeks to prevent and when necessary to eliminate endodontic infections. The persistence of microorganisms to the mechanical chemical preparation or the intracanal medication is considered the main cause of the failures of the endodontic treatment. In these cases Enterococcus faecalis is the most commonly found microorganism. Photodynamic therapy (PDT) emerges as a promising adjuvant therapy to conventional endodontic treatment to eliminate these persistent microorganisms. This therapy consists in the association of a specific light source with a photosensitizer in the presence of oxygen, producing cytotoxic species to the microorganisms. Thus, the present article aims at a review of the literature on the use of photodynamic therapy in endodontics, a test that is shown to assist in the preparation of the root canal.

Introduction. A radiological evaluation is essential in endodontics, for diagnostic purposes, planning and execution of the treatment, and evaluation of the success of therapy. The periapical radiography is nowadays the main radiographic investigations used but presents some limits as 3D anatomic alteration, geometric compression, and possible anatomical structures overlapping that can obscure the area of interest. CBCT (cone beam computed tomography) in endodontics allows a detailed assessment of the teeth and surrounding alveolar anatomy for endodontic diagnosis, treatment planning, and follow-up. Objective. The purpose of this study was to evaluate the accuracy of CBCT in comparison with conventional intraoral radiographs used in endodontic procedures. Materials and Methods. Statistical analysis was performed on 101 patients with previous endodontic treatments with the relative radiographic documentation (preoperative, postoperative, and follow-up intraoral X-ray) that had underwent at CBCT screening for surgical reasons. The CBCT scans were evaluated independently by two operators and compared with the corresponding periapical images. Results. Our analysis shows that the two radiological investigations statistically agree in 100% of cases in the group of patients without any endodontic sign. In the group of patients with an endodontic pathology, detected with CBCT, endodontic under extended treatments (30.6%), MB2 canals in nontreated maxillary molars (20.7%), second canals in nontreated mandibular incisors (9%), root fractures (2.7%), and root resorption (2.7%) were not always visible in intraoral X-ray. Otherwise, positivity in the intraoral X-ray was always confirmed in CBCT. A radiolucent area was detected in CBCT exam in 46%, while the intraoral X-ray exam was positive only in 18%. Conclusions. Our study shows that some important radiological signs acquired using CBCT are not always visible in periapical X-ray. Furthermore, CBCT is considered as a II level exam and could be used to solve diagnostic questions, essential to a proper management of the endodontic problems.

ABSTRACTObjective: this questionnaire aimed to evaluate the approval of a new pictograph system for endodontic diagnosis by undergraduate and graduate students/endodontist instead of a conventional method. Methods: after a presentation to a total of 224 participants explaining the pictograph system, the participants (166 undergraduate students and 58 graduate students/endodontist) received a standardized questionnaire with three questions. The data were analyzed using the multprop macro basing on Tukey's test for multiple comparison with (P ? 0.05). The first question was to define the education level of the participants; the second question about which diagnosis method is preferred; and the third question about the intention of using one of the methods in the future. The questionnaire was realized in May 2019. The response rate was 97.7%. Results: the first question showed that 74.10% and 25.90% were undergraduate and graduate students/endodontist respectively. After statistical analysis there was no significant difference between the groups in the second and the third questions. In the second question (71.08% and 62.06%) of undergraduate and graduate student/endodontists respectively preferred the pictograph method over the conventional method. In the third question (60.24% and 51.72%) of undergraduate and graduate student/endodontists respectively showed interest in using the pictograph method in their professional career. Conclusion: the pictograph method in endodontic diagnosis is accepted by the majority of undergraduate and graduate students/endodontists. Training and experience affect the diagnosis making. Intoduction. Endoperiodontal lesions are frequent in clinical practice and may require complex therapy. It is very important to determine differential diagnosis and the origin of the lesion, because the treatment plan is based on aetiology and stadium of the development of the lesion. Objective. The aim of this clinical study was to analyze the efficacy of endodontic treatment in different types of endodontic- periodontal lesions. Methods. Thirty patients and 42 teeth with diagnosed endoperiodontal lesions were included in the study and divided in two groups. Vital pulp therapy was applied to 18 vital teeth where the diagnosis was perio-endodontic lesions. The therapy of infected canal system was applied to 24 teeth with endodontic- periodontal or combined lesions. Standard endodontic procedure consisted of cleaning and shaping of the root canal system and between sessions filling with calcium-hydroxide paste. Obturation was made by lateral compact of the guttapercha and Apexit paste. Efficiency was evaluated clinically and radiographically three, six and twelve months later. Results. Radiographic and clinical follow-up showed a significant radiographic improvement and absence of subjective symptoms in a very high percentage of the treated vital and avital teeth. The results of this study showed that in the observation period of 12 months endodontic therapy was successful in 88.89% of vital teeth, and in the group of avital teeth the percentage was 91.67%. Conclusion. Endodontic treatment of the endoperiodontal lesions was sufficient and it was the basic condition to achieve a complete healing of endodontic-periodontal lesions. Such therapy ensures significant improvement or even complete healing of both types of lesions, of endodontic and periodontal origin.

Objective: The objective of study was to explore the relationship of breathing and pain during endodontic therapy. Materials and methods: 500 patients who presented in the department of operative dentistry were included in the study. Numeric pain scale was used to document the pain perceived during endodontic therapy. Results were analysed using SPSS version 23. Results: Among the 500 cases, 250 cases presented with mild pain, 158 presented with moderate pain while 92 presented with severe pain. 55.4% cases were of nasal breathers while 44.6% cases were of oral breathers. Conclusion: Patients presenting with nasal breathing pattern reported with mild pain perception during endodontic therapy when compared to patients who were oral breathers. Considering life style modifications like breathing patterns, body hydration and sleep impact the patient’s life and pain management during endodontic therapy. Educating patients on proper breathing and its benefits will have positive outcome on endodontic therapy. Keywords: Oral breathing, nasal breathing, pain perception, endodontics.

The purpose of this case report is to emphasize the importance of cone-beam computed tomography (CBCT) and dental operating microscope for endodontic treatment on mandibular second premolar with three separate roots. In endodontics, the possible existence of extra canals must be considered before endodontic treatment is instituted. A wide morphological variation of the mandibular second premolar is known to exist. The incidence of a three-rooted mandibular second premolar is approximately 0.4%. A 43-year-old female patient was referred for endodontic treatment on the mandibular second premolar. IOPA revealed three roots, and hence for confirmation, a CBCT image was taken and root canal treatment was performed under an operating microscope for enhanced visualization. This case report describes the successful endodontic treatment of a mandibular second premolar with three separate roots and canals filled using gutta-percha and AH Plus. Clinical significance: Conventional endodontic treatment can be performed on a tooth with complex root canal morphology using an operating microscope.

INTRODUCTION: Regenerative Endodontics is one of the most fascinating development in modern dentistry. Since Endodontists are the first providers of this type of dental treatment, it is essential to learn their knowledge, attitude and practice of these procedures. Aim: This online questionnaire-based study was conducted amongst Endodontists practicing in Nepal with the aim to learn their knowledge, attitude and practice of regenerative endodontic procedures. MATERIALS AND METHODS: A cross-sectional study was conducted among Endodontists in Nepal. In total 65 Endodontists participated in this study. The questionnaire was distributed to the participants using the available electronic measures. Descriptive statistical analyses were performed on the responses given by Endodontists using SPSS Statistical Software package (version 21.0). RESULTS: A total of 20 questions were included in this study. Out of the responses analyzed, 100% (n=65) had come across the term regenerative endodontics. Respondents had good knowledge about the sources of dental stem cells (87.3%, n=55). A high percentage (98.4%, n=62) of respondents strongly recommended regenerative therapy to be incorporated into dentistry. Among the participants of this study, 87.1% (n=54) had a belief that regenerative endodontic treatment could serve as the alternative treatment option to dental implant. CONCLUSIONS: The Endodontists practicing in Nepal had sound knowledge about regenerative endodontic procedures. They were enthusiastic in practicing regenerative endodontics and interested in stem cell therapy. It’s a high time to uplift the regenerative endodontic procedure from simple revascularization via blood clotting to more advanced procedure.

Cone Beam Computed Tomography is a diagnostic imaging modality that provides high quality, accurate three dimensional (3D) representations. CBCT in endodontics not only gives a three dimensional evaluation of the region of interest but also an appropriate resolution of images that help give a detailed analysis of the tooth and the surrounding alveolar anatomy. It aids in deciding the treatment plan in various fields of dentistry. CBCT has been proved as an essential diagnostic aid for endodontic practice. Since the use of ionizing radiation involves the patient’s and clinician’s safety, adequate knowledge about the appropriate usage of CBCT usage should be considered as a necessity. In endodontics, CBCT is a useful tool that helps in the diagnosis of apical periodontitis, resorptions, perforations, root canal morphology, traumatic injuries, voids. This survey was conducted among dentists. Online google forms were distributed of which 307 participants took part in the survey and the questionnaire contained 19 multiple choice questions, based on the demographic data, knowledge, attitude, practice regarding usage of CBCT in endodontics. On analysing the response to the questionnaire, it was found that the participants have overall good knowledge and were well versed regarding usage CBCT for endodontic procedures. This survey provides the reader with a clearer understanding of the appropriate and unwarranted usage of CBCT for endodontic practice. CBCT can be a powerful tool for endodontic diagnosis as well as in treatment planning and follow up.

Background. Citation analysis has emerged to play a significant role in recognition of the most useful areas of research. Endodontic microbiology has been a topic of interest for endodontists as well as periodontists and oral surgeons. This bibliometric analysis is aimed at identifying and reporting the characteristics of the top 50 cited articles on endodontic microbiology. Methods. The articles were identified through a search on Web of Science (WoS), property of Clarivate Analytics database published on endodontic microbiology. The citation information of the selected articles was recorded. The Journal of Endodontics, International Endodontic Journal, Oral Surgery Oral Medicine Oral Pathology Oral Radiology and Endodontology, Dental Traumatology, and Australian Endodontic Journal were searched in the search title. Descriptive and bivariate analyses were performed using a statistical software package SPSS. Statistical analysis was performed using Shapiro-Wilk, Kruskal-Wallis, Post hoc, Mann-Kendall trend, and Spearman-rank tests. Results. The 50 most cited articles were published from 1965 to 2012 with citation count varying from 1065 to 103 times. The total citation counts of articles recorded were 11,525 (WoS), 12,602 (Elseviers’ Scopus), and 28,871 (Google Scholar). The most prolific years in terms of publications were 2001, 2002, and 2003, with five publications each, followed by 2005 with four. The year with most citations was 1998, with 1,330 citations, followed by 1965 and 2001, with 1,065 and 1,015 citations, respectively. A total of 136 authors contributed to the top 50 most cited articles with 27 corresponding institutions from 12 different countries. The most common methodological design was in vitro study, followed by clinic-laboratory study, literature review, systematic review and meta-analysis, and animal study. Conclusions. The present study provided a detailed list of the top 50 most cited and classic articles on microbiology in endodontics. This will help researchers, students, and clinicians in the field of endodontics as an impressive source of information.

The success of an endodontic surgery depends on the removal of a persistent infection from the external apical surface and necrotic tissue or contaminated filling material within the root canal system followed to a complete filling of the root end preparation. The current concepts of surgical endodontics recommend that its execution with the use of magnification and illumination of the operating microscope, which allows an easier identification of the root apices, small ostectomies and smaller apex resection angles. The objective of this article is to report and discuss a clinical case of endodontic microsurgery where the main resources currently recommended were used to increase the success in surgical endodontics. Based on specific equipments, instruments and materials the endodontic microsurgery increases the predictability of success of the surgery. Among the main advances compared to traditional surgery we could observe less trauma to the operated region which resulted in a faster and more comfortable healing for the patient.

Several radiographs are taken during endodontic treatment, thus providing a large number of images with individual characteristics that can be used for forensic identification of an individual. This study aimed to review the literature related to the use of endodontic radiographs in the identification of cadavers. A bibliographic search was carried out in the following databases: PubMed, Scielo, Periódicos Capes, and Web of Science. The keywords used were: forensic dentistry, endodontics, dental radiography, forensic identification. In total, 24 articles related to radiography in the identification of cadavers were found. Among them, 8 studies were selected for including endodontic radiographs for this purpose, comprising different types of studies: case reports of forensic identification through endodontic radiography (4); “in vitro” studies on the influence of high temperatures on endodontic materials and their radiographic images (2); a literature review on how radiographs can be used for identification (1); a comparative study between antemortem and post-mortem endodontic radiographic images (1). Overall, the present study showed that endodontic radiographs have a significant amount of information that can be used to identify a cadaver. Therefore, it is important to perform and store radiographs properly and for long periods. Literature related to the subject is still scarce and further studies are needed.

Introduction: Periodontal and pulp tissues are closely related through communication pathways present in the periodontium. This interrelation supports the emergence of endo-periodontal lesions, caused by infectious and irritating products, triggering an inflammatory response Objective: The present study aim to report the treatment of a case of endo-periodontal lesion where endodontic treatment was performed prior to periodontal treatment. Case description: On clinical and radiographic examination, the patient was diagnosed with an endo-periodontal lesion on element 48. The proposed treatment plan was endodontics followed by subgingival scaling. Conclusion: Endodontic treatment associated with periodontal treatment led to the resolution of the case, being observed the absence of pain, absence of abscess/fistula, decreased probing depth and tooth mobility, in addition to bone gain seen radiographically.

Perkembangan dan progresi lesi endo-perio ini dipengaruhi oleh faktor etiologi seperti bakteri, jamur, dan virus serta faktor pendukung seperti trauma, resorpsi akar, perforasi, dan malformasi gigi. Pada lesi endo-perio diperlukan rencana perawatan yang tepat agar prognosis perawatan dari gigi tersebut dapat baik. Artikel ini bertujuan memaparkan perawatan kasus lesi endo-perio yang berhasil setelah manajemen endodontik tanpa dilakukan bedah endodontik. Seorang pasien laki-laki berusia 21 tahun datang ke RSGM dengan keluhan gigi belakang kiri bawah sakit. Gigi tersebut pernah ditumpat 1 tahun yang lalu.Pada gambaran radiograf terdapat lesi radiolusen luas pada tulang alveolar sekitar akar distal. Perawatan endodontik dilakukan dengan pergantian bahan dressing kalsium hidroksida sebanyak 3 kali. Pada kontrol bulan kedua terlihat terjadi penulangan pada bagian lesi periodontal tersebut dan pasien tidak mengeluhkan rasa sakit. Kesimpulan hasil perawatan lesi endodontik periodontal dapat dirawat dengan perawatan endodontik non bedah.Management of Nonsurgical Endodontic Treatment on A Combined Endo-period Lesion. The pulp and periodontium have anatomic interrelationships. As the tooth matures, and the root is formed, three main avenues are created between pulp and periodontal ligament, i.e. dentinal tubules, lateral and accessory canals, apical foramen. These are the pathways that may provide a means by which pathological agent pass between the pulp and periodontium, thereby creating the endo - period lesion. Etiologic factors such as bacteria, fungi, and viruses as well as contributing factors such as trauma, root resorption, and dental malformations play a significant role in the development and progression of such lesions. In the endodontic - periodontal lesion is necessary to plan appropriate dental treatment that can lead to a better prognosis. This paper presents satisfactory healing of combined endo - period lesion treatment after nonsurgical endodontic management.A 21 years man came to the Conservative Dentistry Clinic, Prof. Soedomo Dental Hospital with pain complaint on left mandible posterior tooth. One year ago, this tooth already restored with resin composite restoration.Radiographs showed an extensive radiolucent lesions on 36 bifurcations extending to the distal root. After thorough cleaning and shaping, calcium hydroxide was applied intracranial and periodically renewed up to three times. Two months after root canal obturation radiograph evaluation showed ossification at the bifurcation 36. Endodontic Periodontal lesion can be treated with nonsurgical endodontic management.

Endodontic treatment of the teeth with severe root canal curvatures has become one of the greatest problems in clinical practice. Recently, new instruments and techniques have been introduced in the endodontic procedure. The aim of this study was, on the basis of clinical practice, to show the possibilities of the endodontic treatment in the teeth with severely curved root canals, after dependable decision about endodontic instruments and techniques. This article presents a complete endodontic procedure in two teeth with different curvature shape of the root canal in patients that were treated at the Department of Restorative Dentistry and Endodontics, School of Dentistry, Belgrade. Coronal to apical 'crown down' technique was used for the root canal preparation with hand NiTi ProTaper instruments with progressive multicone design. The main reasons for choosing manual technique were complexity, shape of presented curvatures and better tactile sensation control of canal instruments in order to avoid possible mistakes and complications. Clinical X-rays of the teeth with permanent obturation clearly showed that extremes of the root canal morphology could be successfully solved by the right choice of all factors during endodontic procedure.

Since the introduction of Nickel-Titanium alloy as the material of choice for the manufacturing of endodontic rotary instruments, the success rate of the root canal therapies has been significantly increased. This success mainly arises from the properties of the Nickel-Titanium alloy: the biocompatibility, the superelasticity and the shape memory effect. Those characteristics have led to a reduction in time of endodontic treatments, a simplification of instrumentation procedures and an increase of predictability and effectiveness of endodontic treatments. Nevertheless, the intracanal separation of Nickel-Titanium rotary instruments is still a major concern of endodontists, with a consequent possible reduction in the outcome rate. As thoroughly demonstrated, the two main causes of intracanal separation of endodontic instruments are the cyclic fatigue and the torsional loads. As results, in order to reduce the percentage of intracanal separation research and manufacturers have been focused on the parameters that directly or indirectly influence mechanical properties of endodontic rotary instruments. This review describes the current state of the art regarding the Nickel-Titanium alloy in endodontics, the mechanical behavior of endodontic rotary instruments and the relative stresses acting on them during intracanal instrumentation, highlighting the limitation of the current literature.

Introduction: Endodontics is specialty field, but the majority of endodontic treatment in Pokhara is provided by the general dental practitioners. The aim of this study was to assess the practice of endodontic treatment protocols among general dental practitioners in Pokhara, Nepal. Materials and Methods: A cross sectional questionnaire based study was conducted among all the general dental practitioners working in various government, private hospital and clinic of Pokhara. Information about the materials and techniques used in endodontic treatment was collected and descriptive statistical analysis was done. Results: Out of 89 respondents, only 82 completely filled the questionnaires which were included in the study. Most general dental practitioners (96.34%) regularly treat single rooted endodontic cases. Only 10.97% occasionally use rubber dam during endodontic treatment. Most general dental practitioners (96.34%) used radiographic technique for working length determination. Ninety two percent used stainless steel hand files and step back was the choice of technique by 90.24%. Sodium hypochlorite irrigation and calcium hydroxide intra canal medicaments were used by 93.9% and 98.78% of general dental practitioners respectively. 75.6% used zinc oxide eugenol as root canal sealer and 95.12% obturated root canals with lateral compaction technique. The common complication encountered was mid treatment pain. They seem to overuse antibiotics in cases requiring endodontic therapy. Only 52.43% used autoclave for sterilization of endodontic files. About 92.68% felt the need of further endodontic training and 74.39% preferred post graduate program. Conclusion: This study indicates that many general dental practitioners of Pokhara are not following well acknowledged endodontic quality guidelines; hence there is a need for further endodontic training.

During the past few decades, the endodontic treatment has benefited from the development of new techniques and devices, which have improved the outcome and predictability. Important attributes such as the operating microscope and ultrasonics have found indispensable applications in a number of dental procedures in endodontics. Objective and case report: This study describes a case of endodontic retreatment of a maxillary first molar in which the second mesiobuccal canal could be located by extending the access to the mesial direction further than usual and by a closer inspection of the pulp chamber floor. Conclusion: The “microsonics concept” has enhanced the quality of endodontic treatment and retreatment, becoming increasingly more useful in applications such as localization of root canal orifices.

Starting from the stage of disease diagnosis, radiography is of much importance in different stages of endodontic therapy, and further on radiographic evaluation is a tool for assessment of endodontic treatment. There is a heavy dependency on dental radiography in some form or other in the speciality of endodontics. As is the case with any ionizing radiation, radiation hazard is a phenomenon that matters a lot to this group of dental professionals. There needs to be a change in our attitude toward radiation safety measures, as many of our professional colleagues are seen much not to be bothered about the cumulative outcomes of radiation hazard, which can create havoc in our professional and personal lives. This article outlines the potential hazards that can happen by routine radiographic utilization in endodontic setup and tries to highlight the measures that need to be taken to mitigate the negative effects. A bibliometria representa importante recurso para diagnóstico e monitoramento da divulgação do conhecimento científico em determinada área. Esta pesquisa teve como objetivo tipificar o perfil das publicações científicas na área da endodontia em duas revistas de alto impacto. Para tanto, artigos publicados entre janeiro de 2011 e dezembro de 2020 no International Endodontic Journal e no Journal of Endodontics foram avaliados e caracterizados de acordo com o tipo metodológico e nacionalidade do autor principal. Um total de 4335 artigos foram publicados em ambas as revistas, na década avaliada. Os artigos de pesquisa básica em tecnologia representaram 36,6% das produções e apenas 2,9% foram publicados com metodologia de revisão sistemática. A maior parte das pesquisas foi procedente do Brasil (20,2%), seguido pelos Estados Unidos da América (15,0%) e China (8,2%). A grande maioria das publicações foi classificada como pesquisas básicas em biologia ou tecnologia. Além disso, o Brasil figura com o maior produtor de publicações de alto impacto em endodontia entre os anos de 2011 a 2020.

Regenerative endodontics has been defined as biologically based procedures designed to replace damaged structures, including dentin and root structures, as well as cells of the pulp-dentin complex. Presently, two concepts exist in regenerative endodontics to treat non-vital infected teeth - one is the active pursuit of pulp-dentine regeneration to implant or regrow pulp (tissue engineering technology), and the other in which new living tissue is expected to form from the tissue present in the teeth itself, allowing continued root development(revascularisation). Regenerative endodontic procedures (REPs) have evolved in the past decade, being incorporated into endodontic practice and becoming a viable treatment alternative for immature teeth. The authors have summarized the status of regenerative endodontics on the basis of the available published studies and provide insight into the different levels of clinical outcomes expected from these procedures.

The aim of this paper is to present a technique to individualize root canal localization in teeth with calcified root canals using a digitally planned, 3D-printed endodontic guide. Root canal calcification is characterized by the apposition of tertiary dentin along the canal wall. The endodontic treatment of teeth with calcified canals is often challenging. However, digital dentistry meets these challenges. Merging CBCT images with an intraoral scan allows a clinician to prepare an endodontic guide. This article describes the clinical and digital workflow of the guided endodontic access approach in teeth with difficulties in terms of root canal localization due to post-traumatic pulp canal obliteration (PCO) and canal calcification in elderly patients. The path of entry into the root canal system was planned using cone-beam computed tomography (CBCT). The template was printed on a 3D printer using transparent resin. During root canal treatment (RCT), the endodontic tool was inserted through the sleeve until the desired location was reached. The use of an endodontic guide allowed for minimally invasive RCT, avoiding the excessive loss of tooth structures. Navigated endodontics enables clinicians to perform RCT in a more predictable manner and allows clinicians to avoid iatrogenic complications, which improves the treatment prognosis.

Objective: The results detailed that a few systemic disorders may impact endodontic healing results. A systematic review can complete such analyses to investigate endodontic infection and cardiovascular disease thoroughly. This systematic review and meta-analysis aimed to evaluate the effect of Endodontic Infections on Cardiovascular Disease. Method: Our search strategy was to screen the relevant articles on the electronic databases of Google Scholar, MEDLINE, Cochrane Library, PubMed, ISI, and Embase, published from January 2005 to January 2020. The management of electronic titles was performed using Endnote X9 software. The main keywords for the search process included "Cardiovascular disease OR CVD", "LEO"," Endodontics", "Endodontic Infections" "dental evaluations", "missing teeth", "apical lesions", "bone loss". The current systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Results: Electronic and manual searches resulted in 53 relevant titles and abstracts. Eventually, five publications were eligible for the inclusion criteria required for this systematic review. The risk ratio (RR) was 1, and the 95% certainty interval (CI) was 3.56–15.87; the obtained very low certainty indicated that the lesion of endodontic origin (LEO) might impact Cardiovascular disease (CVD) improvement. Conclusion: Endodontic Infection may or may not exert a few effects on the systemic Cardiovascular disease (CVD), and there was a low level of proof.

Introduction: The term “bioceramic” is used in endodontics to describe various products and is often used in general terms for mineral trioxide aggregate (MTA) and other hydroscopic dental cements (HDC), which creates confusion in the terminology. Aim: The aim of the study is to present a definition and an original classification of bioceramic endodontic sealers. Materials and methods: A total of 123 articles were found by the PubMed search engine using the key phrase “bioceramic endodontic sealers”. Of these 123 articles, we analyzed 20 articles that contain information about the composition, properties, definition, and classification of bioceramic endodontic sealers. Results: In accordance with the collected data on the composition and the delivery form of calcium silicate endodontic sealers, we propose a new definition of bioceramic endodontic sealers that clarifies the specificities of these materials. In the new classification they are divided by two criteria: source of calcium silicate and form of delivery. It also contains the particular products’ names in order to aid their usage in the clinical practice.  Conclusions: The new definition of bioceramic endodontic sealers clears out the confusion in the terminology, which promotes the classification of these products and helps understanding their clinical application.

(1) Background: The regenerative procedure has established a new approach to root canal therapy, to preserve the vital pulp of the tooth. This present review aimed to describe and sum up the different approaches to regenerative endodontic treatment conducted in the last 10 years; (2) Methods: A literature search was performed in the PubMed and Cochrane Library electronic databases, supplemented by a manual search. The search strategy included the following terms: “regenerative endodontic protocol”, “regenerative endodontic treatment”, and “regenerative endodontics” combined with “pulp revascularization”. Only studies on humans, published in the last 10 years and written in English were included; (3) Results: Three hundred and eighty-six potentially significant articles were identified. After exclusion of duplicates, and meticulous analysis, 36 case reports were selected; (4) Conclusions: The pulp revascularization procedure may bring a favorable outcome, however, the prognosis of regenerative endodontics (RET) is unpredictable. Permanent immature teeth showed greater potential for positive outcomes after the regenerative procedure. Further controlled clinical studies are required to fully understand the process of the dentin–pulp complex regeneration, and the predictability of the procedure.

AbstractIntroduction: Perforations are defined in endodontics as a pathological or iatrogenic communication between the canal space and the periodontium. It is the second cause of failure of endodontic treatment. The aim of this research is to evaluate the therapeutic success of iatrogenic perforations in endodontics of Malagasy practitioners in the Analamanga, Boeny and Atsimo Andrefana regions through a survey of 125 practitioners from May to June. 2021.Method: A descriptive retrospective cross-sectional multicenter study was developed. A simple random sampling was carried out for the choice of the Odontostomatologists surveyed and a sampling by convenience for the choice of 3 regions.Results: The study revealed that 12.6% of practitioners had a success rate > 75% during the perforation treatment. Calcium hydroxide has been the most widely used filling material by Malagasy Odontostomatologists, yet practitioners using BiodentineTM have achieved a success rate > 50%.Conclusion: The preservation of a tooth with an endodontic perforation is possible with a variable success rate depending on the filling product used Keywords: endodontic perforation, treatment, success rate

Diseases of the dental pulp often have an infectious origin, and treatments are aimed to control infections of the root canal system. Endodontic treatment principles originally evolved on the basis of trial and error, and only in recent decades have scientific methods been adopted to support clinical strategies. Yet, relevant research on the disease processes, their diagnoses, and efficient treatment are rare in the endodontic literature. Hence, the advancement of biologically based knowledge significant to clinical endodontics has been slow. Therefore, many differences of opinion still prevail in this field of dentistry. This review highlights and analyzes the background of some of the more heavily debated issues in recent years. Specifically, it deals with disagreements regarding the clinical management of pulpal exposures by caries in the adult dentition, definitions of success and failure of endodontic therapy, and causes of and measures to control infections of the root canal system. Clearly, a most apparent gap in the published endodontic literature is the lack of randomized clinical trials that address the more significant controversial matters relating to the management of pulpal wounds, medication, and the number of appointments required for the treatment of infected root canals. However, trials in endodontics require extremely long follow-up periods if valid conclusions are to be generated. Therefore, it is not to be expected that there will be rapid solutions to these issues in the foreseeable future.

Introduction: The endodontic treatment in non-vital teeth is directed to the elimination of the infection through the biofilm removal biomechanics and remnants of necrotic tissue, in order to eliminate infection and generate periapical tissue repair. Objective: To determine the success or failure of endodontic treatment in non-vital teeth performed by dental students under supervision. Materials and methods: In this article 3 clinical cases of patients undergoing endodontic procedures with non-vital pulps track 4 and 6 years are presented. Results: Two of the 3 cases show a process of incomplete periapical tissue regeneration at the time of observation. The third case shows a process of subsequent periodontal disease to endodontics leading to tooth loss. Conclusions: Is necessary to conduct a study with a sample size calculated to determine what percentage of success and failure of endodontic treatment in non-vital teeth, performed by undergraduate dental students under supervision, and to determine those factors that directly influence both outcomes.

ABSTRACT Objective: This study examined the endodontic experience, perceptions of endodontic practice, and self-rated confidence of dental students enrolled in Taibah University, Saudi Arabia. Materials and Methods: A questionnaire was distributed to 41 undergraduate dental students registered in endodontic courses in the 2015 academic year. The questionnaire evaluated their confidence performing nonsurgical root canal treatment. The level of confidence was classified using a 5-point scale as “very confident,” “confident,” “neutral,” “not very confident,” or “not at all confident.” The data were analyzed using SPSS version 20.0 (SPSS, Chicago, IL, USA). Results: The participation rate was 93%. The maxillary incisor was the most common first tooth treated. The students were relatively confident, but their confidence levels were lower regarding endodontic radiology, evaluation of root canal obturation, and determining the correct recall period for the patient. Conclusion: The confidence of undergraduates in endodontics must be enhanced to increase their clinical competence when performing root canal treatment.
