Cost effectiveness analysis is increasingly advocated as a basis for health policy. Analysts often compare expensive interventions with highly cost-effective programs such as hypertension screening, implying that if the former were curtailed resources would be reallocated to the latter and the efficiency of health care would improve. However, in practice, savings are unlikely to be targeted in this way. We present refined policy models that take into account actual patterns of resource allocation in the United States, and provide more realistic estimates of the likely uses of savings. We illustrate the implications of these models in an analysis of the effects of diverting funds from an expensive but effective practice. Eliminating such a practice would actually worsen the overall cost-effectiveness of U.S. health care unless there are radical changes in health policy. Cost effectiveness analysis incorrectly predicts health and cost outcomes of policy initiatives because it ignores the political constraints to health care decision-making.

In an interview, Dr. Thier said he hopes to turn Massachusetts General into another kind of model: one that increases cost-efficiency without compromising basic research or patient care. "A way of life is changing," Dr. Thier asserted. Academic medical centers are being forced to rein in costs quickly, he contended, because of growing price- pressure from insurers, as well as the movement in Washington to enact healthcare legislation. Top teaching hospitals haven't conducted much cost-oriented research involving patients. Instead, the hospitals have concentrated on discovering fundamental medical techniques, often working at the molecular level. Research on treatment costs and effectiveness has been carried out mostly by schools of public health and by such think tanks as Rand Corp... Other big teaching hospitals are starting research on treatment costs and effectiveness, said George Lundberg, editor of the Journal of the American Medical Association. "I expect such research to flourish with the pressures of health-system reform," he said. "Now that it's so much in the best interests of the largest health-delivery groups to discover the best ways to deliver maximum services for minimum cost," Dr. Lundberg added, "it's not at all surprising that such institutions will enter health-services research in a big way."

Abstract Objective.—To illustrate how laboratorians and pathologists must demonstrate accountability for efficiency (how well something is done), effectiveness (what is done), and cost-effectiveness (a proxy for value, in which value equals quality per cost). Data Sources.—A literature search was conducted, including documents in the National Library of Medicine. Study Selection.—The literature on cost-effectiveness of laboratory testing was reviewed. Conclusions.—The demand for proving the value of newer and more expensive medical technologies, including newer medical tests, will increase substantially. Payers, including Medicare, commercial insurers, and employers, will demand accountability and elimination of the abuse and misuse of ineffective testing strategies. Pathologists and laboratorians play a key role in guiding the most cost-effective use of testing strategies, including the judicious use of algorithms.

Mining companies are required to show greater environmental care and responsibility. Environmental cost classification and reporting can help achieve the effectiveness and efficiency of environmental cost control. This study aims to determine and compare the effectiveness and efficiency of environmental cost control of mining companies, determine the company's eco-efficiency, and determine its effect on eco-efficiency. This comparative descriptive research and quantitative research uses secondary data, namely data on environmental costs and financial data of PT TIMAH Tbk. (TIMAH) and PT Aneka Tambang Tbk. (ANTAM) for 2016-2020 which collected through documentation. Data were analyzed using ratio analysis, trend, and simple linear regression. The results of the study found that: (1) TIMAH and ANTAM have not adopted environmental cost reporting based on the theory of Hansen et al., (2) Environmental cost control ANTAM more effective than TIMAH, (3) Environmental cost control TIMAH more efficient than ANTAM, (4) The highest eco-efficiency of TIMAH obtained in 2016, while the highest eco-efficiency of ANTAM obtained in 2018, (5) The effectiveness of environmental cost control has a significant effect on eco-efficiency. Background: Reusing single-use medical devices is a very important and complicated process since theimplementation requires both operational and technical skills, even for professional users. The aim of our studywas to determine the cost effectiveness and efficiency of reusing single-use medical devices. Method: The study was a cross-sectional study conducted between July and December 2013. It compared twogroups of patients who underwent endoscopic retrograde cholangiopancreatography (ERCP) examination at the Digestive Endoscopy Center, Cipto Mangunkusumo Hospital, Jakarta. Patients in the first group received new single-use medical devices; while patients in the other group received the re-used single-use medical devices.Reprocessing for reusing single-used medical devices was conducted according to standard procedures ofdecontamination and sterilization. Results: Reusing medical devices were more commonly found (50.9%) in ERCP procedures than usingnew medical devices (49.1%). There was no significant difference on operator satisfaction between using there-used and new medical devices (p = 0.062). There was lower average cost for reusing medical devices thanusing new medical devices (IDR 198,818,250.00 vs. IDR 594,354,000.00; p = 0.000); percentage of successrate for reusing was lower than new medical devices (80% vs. 90,6%; p = 0.203). There was also no significantdifference regarding the negative impacts such as fever or infection of reusing medical devices compared tousing new medical devices (p = 0.676). Conclusion: This study has shown good effectiveness in terms of operator satisfaction, success rate andimpacts on patients. The cost for reusing medical devices is more efficient than using new medical devices.Keywords: effectiveness, efficiency, reusing medical devices, new, endoscopy ABSTRAKLatar belakang: Penggunaan alat medis sekali pakai (single-use) yang dipakai kembali (re-use) menjadi sangat penting dan kompleks dalam menerapkan kemampuan operasional dan teknik pada profesional penggunaalat tersebut. Tujuan penelitian ini adalah untuk menilai efektifitas biaya dan efisiensi penggunaan perangkatmedis single-use yang dijadikan re-use. Metode: Penelitian ini merupakan penelitian potong lintang dilakukan pada Juli hingga Desember2013. Penelitian membandingkan dua kelompok pasien yang menjalani pemeriksaan endoscopic retrogradecholangiopancreatography (ERCP) di Pusat Endoskopi Saluran Cerna (PESC), Rumah Sakit CiptoMangunkusumo, Jakarta. Pasien pada kelompok pertama menerima tindakan dengan alat medis baru; sedangkanpasien di grup lainnya menerima tindakan dengan alat medis re-use. Proses penggunaan alat medis re-use dilakukan sesuai dengan prosedur dekontaminasi dan sterilisasi yang berlaku. Hasil: Penggunaan alat medis dalam tindakan ERCP lebih banyak menggunakan alat re-use (50,9%) dibandingkan alat baru (49,1%); kepuasan operator pada alat re-use maupun alat baru tidak berbeda secarasignifikan (p = 0,062); rata-rata biaya pada penggunaan alat re-use lebih rendah dibandingkan pada alat baru (Rp 198.818.250,00 vs. Rp 594.354.000,00; p = 0.000); persentase keberhasilan penggunaan alat re-use lebihrendah dibandingkan pada alat baru (80% vs. 90,6%; p = 0,203); dampak berupa terjadi demam atau infeksipada penggunaan alat re-use dan alat baru tidak menunjukkan perbedaan yang signifikan (p = 0,676). Abstract This article explores how Agile methodology, DevOps principles, and Cloud Computing fit into the software industry. The synergy study focuses on Axes Software, the oldest logistics software company in Romania. The effects of the association of these technologies during the development cycle of a software product are explored. Both their impact in the operational activities and the transformation of the development mode, as well as the financial impact, are evaluated. In the first part, theoretical aspects of Agile and DevOps ways of working and cloud technologies are explored. The amplification of the potential of the association of the three concepts is examined. With the basis formed, the paper continues with a detailed case study of Axes Software. It looks at the adoption of new technologies and way of working. Challenges and successes encountered are also considered. Key performance indicators such as release frequency, bug incidence (dropped from 72 to 4), and development velocity are examined to validate the effectiveness of this integration. The financial aspects, including initial investment and savings, are also analysed, leading to an assessment of the return on investment. Notably, deployments of new software versions now take two days instead of eleven, while the market witnessed a 275% increase in clients preferring the company’s cloud-hosted solutions over on-premises options. Finally, the effects of the association for software developers are distinguished, emphasizing continuous learning, leadership vision and supporting digital transformation.

В статье представлены результаты исследования сущности экономической эф фективности. Целью работы явилось исследование развития понятийного аппарата данной катего рии применительно к сельскому хозяйству и в частности к производству зерна. В ходе обобщения ре зультатов исследования ряда российских и зарубежных ученых была сформирована авторская позиция по пониманию категории «экономическая эффективность» применительно к зерновому хозяйству. При этом наибольшей значимостью в исследовании можно считать выделенное авторами представление формирования экономической эффективности с точки зрения процессного подхода и взаимосвязь эко номической эффективности с эффективностями использования ресурсов, производства и реализаци ей зерна, а также государственной поддержкой. The article presents the results of the study of the essence of cost-effectiveness. The purpose of the work was to study the development of the conceptual apparatus of this category in relation to agriculture and in particular to grain production. In the course of summarizing the results of the study of a number of Russian and foreign scientists, an author's position was formed on understanding the category "economic efficiency" in relation to grain farming. At the same time, the greatest significance in the study can be considered the presentation of the formation of economic efficiency by the authors in terms of the process approach and the relationship of economic efficiency with the efficiency of the use of resources, production and sale of grain, as well as state support.

This study aims to analyze the effect of the effectiveness of the implementation of supply chain management and operational cost efficiency on the company's performance with competitive advantage as a moderating variable. The research population is the decision maker on the CV. Eight Eight as many as 36 people. The results showed that the effect of the effectiveness of the implementation of supply chain management and operational cost efficiency had a positive effect on company performance. Meanwhile, competitive advantage weakens the effect of the effectiveness of implementing supply chain management and operational cost efficiency on company performance. Purpose – The aim of this paper is to construct a model of cost efficiency and service effectiveness for a university e-library to allocate e-resources cost and to attain quality of service enhancement and cater to the needs of readers with existing e-resources. Design/methodology/approach – The paper establishes an assessment model for the cost efficiency and service effectiveness of a university e-library in Taiwan. It then proceeds with an empirical study and analysis of related data collected from e-libraries of 16 universities. A discussion of the results of the study and suggestions for the adjustment of the university e-libraries follows. Findings – The paper combines two models of cost efficiency and service effectiveness for the first time to analyze and consider the output results created by the input cost of university e-libraries in Taiwan and their utilization by readers. Originality/value – The paper builds a figure for the relationship of e-library cost efficiency and service effectiveness at 16 universities in Taiwan and then divides it into four types.

Incentive policies play a crucial role in encouraging the adoption of renewable energy sources and energy efficiency measures. This study analyzes and compares the incentives for energy transition in the South Tyrol region of Italy using a marginal abatement cost (MAC) curve constructed with an expert-based approach. The incentives for residential energy efficiency, mobility, and boiler replacement are characterized based on assumptions for costs, energy savings, and parameters. The resulting expert-based MAC curve analysis reveals boiler replacement incentives to be the most cost-effective, yielding CO2 reductions at the lowest cost but with limited potential as the incentive is limited to apartment blocks that are not in district heating areas. Mobility incentives enabling electric vehicle adoption have the highest CO2 reduction potential, albeit at higher costs per ton abated. Residential energy efficiency incentives fall between the two for cost-effectiveness and potential. The MAC curve approach provides a useful comparison of cost-effectiveness versus potential, guiding policy prioritization. This techno-economic assessment methodology can be applied to other regions pursuing energy transition. Overall, a balanced policy mix encompassing transport, buildings, and heating is required for comprehensive low-carbon transition.

Numerous scholars have thoroughly studied the topic of choosing machines considering the progress and technological growth seen in machinery options. This scholarly investigation explores decision-making methods specifically designed to aid the selection of machines in manufacturing businesses. Additionally, this research emphasizes the need for decision-making frameworks in manufacturing facilities, highlighting the importance of smart machine selection strategies in those contexts. In this research, we show a dual-MCDM approach that includes DEX—decision experts—and the EDAS method that are popularly employed to solve decision-making problems in both academic and practical industries. Throughout the previous decade, business leaders and managers increasingly use MCDM solutions to overcome machine selection challenges. At this time, while various decision-support technologies and procedures have been developed and used, it is essential that we discuss the sequence of our study objectives and drive the proposed method for widening use in practical firms. In short, this research may be helpful as a literature review for MDCM studies and related topics. It will also help executives, engineers, and specialists determine which equipment or machines to create and increase product quality in manufacturing and industry.

The successful implementation of predictive maintenance for offshore wind farms suffers from a poor understanding of the consequential short-term impacts and a lack of research on how to evaluate the cost-efficiency of such efforts. This paper aims to develop a methodology to explore the short-term marginal impacts of predictive maintenance applied to an already existing preventive maintenance strategy. This method will be based on an analysis of the performance of the underlying predictive model and the costs considered under specific maintenance services. To support this analysis, we develop a maintenance efficiency measure able to estimate the efficiency of both the underlying prediction model used for predictive maintenance and the resulting maintenance efficiency. This distinction between the efficiency of the model and the service results will help point out insufficiencies in the predictive maintenance strategy, as well as facilitate calculations on the cost–benefits of the predictive maintenance implementation. This methodology is validated on a realistic case study of an annual service mission for an offshore wind farm and finds that the efficiency metrics described in this paper successfully support cost–benefit estimates.

In 1972, the Congress of the United States of America passed major legislation called the Clean Water Act, which required all wastewater discharges to meet minimal standards. The Clean Water Act promoted efficiency and cost effectiveness. The Gulf Coast Waste Disposal Authority was created in 1969 by the Legislature of the State of Texas to, among other things, own and operate wastewater disposal systems which would be protective of public health, “terrestrial and aquatic life, the operation of existing industries and the economic development of the state“. Since enactment of the Clean Water Act, other pieces of legislation, rulings by courts of law, and rules established by the U.S. Environmental Protection Agency have thwarted many efforts to operate efficient and cost-effective wastewater treatment facilities. This paper will discuss several of the laws and rules that have discouraged efficiency and cost effectiveness.

The purpose of this study is to use the life cycle cost theory to analyze the efficiency of large medical equipment in hospitals, so as to implement life cycle cost (LCC) management and solve the current problems in hospitals. The analysis model of cost benefit of large medical equipment is established, and the cost-effectiveness of 4 large medical equipment between 2019 and 2021 is investigated and analyzed. In terms of the data in each information system of hospitals, the utilization of large medical equipment is quantitatively evaluated and analyzed by life cycle theory. The results show that the Revolution 256 row has the highest revenue of 113.29%. The annual depreciation of Signa 3.0 T HDxt is the highest, amounting to 4,160,000 yuan. However, there is lack of quality control and preventive maintenance of most equipment during use. The cost and benefit of large medical equipment in hospitals are analyzed, which demonstrates that Signa 3.0 T HDxt shows better effectiveness. Too high hospital warranty cost reflects the weak maintenance strength of hospital engineering technicians. The fundamental point of the maintenance and management of large medical equipment is to strengthen the performance evaluation of medical engineering technicians.
