Consistent with trends in adults, the weight-control practices of children and adolescents appear to reflect a heightened concern with body image.7,16–20 The potential long-term consequences of inappropriate weight-control practices in young people are serious and include disordered eating practices,21–23 growth retardation, delayed menarche, amenorrhoea, osteoporosis, and psychological disturbances.12,24–26 The commonly employed weight-control practices, dietary modification, and exercise are pervasive,8 and not limited to the female population. As medical practitioners are widely consulted and held in high esteem by the general public, they are in a good position to identify potential problems in children and adolescents in their care.11 The aim of this chapter is to provide an overview of the range of factors contributing to inappropriate eating and activity behaviours, particularly in children and adolescents.

Resumen
Many of the same behavioural and brain disturbances observed in addiction are also seen in obesity and binge-eating disorder. This suggests that there are shared neural substrates between substance addiction and compulsive food consumption. Food intake and appetite are regulated by numerous appetite hormones that exert their effects through brain systems involved in reward sensitivity, stress, impulsivity, and compulsivity. There is now emerging evidence that appetite hormones (e.g. ghrelin, glucagon-like peptide-1, orexin) can modulate addictive behaviours (e.g. craving) and the intake of alcohol and drugs. Therefore, there is an emerging shift into a new field of testing drugs that affect appetite hormones and their receptors in the brain, and their use in regulating the brain mechanisms that lead to relapse in addiction disorders.

If a pregnant woman has not had pre-conception counselling, it is important to ensure as soon as possible in early pregnancy that she understands her nutrient requirements, and what comprises a balanced diet and healthy body composition. The advice should include weight gain goals in pregnancy, a list of unsafe foods, healthy exercise behaviours, and the effects of alcohol and drugs. Energy intake needs in early pregnancy do not increase from pre-pregnancy levels; women should focus on eating well for pregnancy, and not eating more. Specific attention should be paid to intakes of folate, vitamin A, vitamin B12, vitamin D, iron, and iodine. Glucose status should be monitored in mid-pregnancy. Energy requirements increase in mid- to late pregnancy, but attention should also be paid to the macronutrient balance. If the diet is low in oily fish as a source of polyunsaturated fatty acids, supplementation should be considered.

Bariatric surgery is the most effective treatment for severe obesity and its weight-related comorbidities. As the use of bariatric surgery has increased, concerns have arisen about problematic eating behaviors (EBs) and eating disorders (EDs) in this population. This chapter describes the current literature detailing EDs and problematic EBs and weight control practices (WCPs) in the post-bariatric-surgery population. It begins with a description of EDs in this population. Next, it describes problematic EBs, followed by a review of postoperative gastrointestinal problems that influence EBs. Third, it reviews the WCPs that can evolve. Finally, it describes models of “food addiction” as they apply to eating-related pathology in the post-bariatric-surgery population.
