Many critically ill patients undergo a period of immobilization with detrimental effects on skeletal muscle, effects which seem most pronounced in the first days of critical illness. Diagnosis of intensive care unit muscle weakness (ICUAW) is often made after discontinuation of sedation when significant nerve and/or muscle damage may already have occurred. Recently, there has been interest in early mobilization during the acute phase of critical illness, with the goal of preventing ICUAW. Neuromuscular electrical stimulation (NEMS) is an alternative form of exercise that has been successfully used in patients with advanced chronic obstructive pulmonary disease (COPD) and chronic heart failure. NEMS is a rehabilitation tool that can be used in critically ill, sedated patients, does not require patient cooperation, and is therefore a promising intervention to prevent muscle dysfunction in the critically ill. When applied early during the course of critical illness, NEMS can preserve muscle morphology and function. Available evidence suggests that NEMS may have a preventive role in the development of ICUAW and could even contribute to a shorter duration of weaning from mechanical ventilation. Studies are needed to evaluate the long-term effect of NEMS and to explore NEMS settings and delivery characteristics most appropriate for different subgroups of critically ill patients.

Labour pain can be severe, and relieving this pain can reduce physiological stress on mother and baby. Epidural and spinal analgesia is by far the most effective modality. Epidurals do not increase the likelihood of Caesarean delivery. Entonox is the next best choice for labour analgesia. Intravenous opioids, especially pethidine, have very little analgesic effect and cause sedation as well as other negative effects on the mother and baby. Transcutaneous electrical nerve stimulation has a limited role confined to early labour.

Deaf and hard-of-hearing (DHH) children are more at risk for mathematical difficulties than hearing children. The few studies that have focused on preschoolers showed that even at this age DHH children already lag behind their hearing peers. Early math skills are important for later mathematics performance and school success in general, so early stimulation of these skills is needed. Research has shown that the home environment plays an important role in the development of early math skills. Because parent–child math activities predict later achievement, it is important to increase parents’ awareness of the importance of math activities for young DHH children. This chapter focuses on early math skills in DHH children and describes how parents can enhance these skills in the home environment.

Acute HIV infection is often missed but should be recognized. Most chronically infected individuals are asymptomatic. However, some patients with chronic HIV infection may present with certain clinical and laboratory abnormalities prior to the diagnosis of an opportunistic infection. HIV wasting syndrome is infrequently diagnosed in the era of antiretroviral therapy (ART). Recognition of HIV wasting is important because it carries adverse prognostic implications. Management includes a multifaceted approach, including ART, lifestyle and nutritional support, appetite stimulation, and possibly hormonal agents. The newer antigen–antibody test can detect new HIV infection as early as 15 days after exposure. Screening is important because most chronic HIV infection is asymptomatic.

This chapter investigates Michel de Montaigne’s treatment of the emergent discourse of reason of state. It shows that Montaigne neither affirms nor rejects reason of state; instead, he argues both for and against it without resolution. His purpose is to highlight the danger of following elite justifications for violence, given either in terms of political necessity or with moral reasons. Montaigne seeks to call into being a public with the capacity to resist elites’ stimulation of popular demand for intolerant policy and their promises to supply it. In doing so, Montaigne develops an original early theory of public opinion and its role in tolerance conflicts. The chapter engages with Bernard Williams’s conception of tolerance in terms of “political realism.”

The testis descends in response to androgen stimulation in the third trimester of pregnancy. Germ cell maturation which normally occurs in early childhood is impaired or absent in undescended testes. The well-documented phenomenon of secondary ascent is now thought to account for a sizeable proportion of boys undergoing orchidopexy in later childhood. Inguinal hernias and communicating hydroceles are caused by persistence of a patent processus vaginalis. Surgery is always indicated for inguinal hernias but the majority of communicating hydroceles resolve spontaneously. Testicular torsion accounts for 90% acute scrotal symptoms in adolescents, in whom urgent surgical exploration is mandatory unless there is compelling evidence of an alternative diagnosis. Treatment is rarely justified for asymptomatic varicoceles in this age group.

Chronic low back pain (CLBP) is a common reason for physician office visits among the elderly. Predictive factors for CLBP are female sex, social isolation, hypertension, and joint pain. In the elderly, CLBP may be related to degenerative spinal stenosis with disk degeneration and overall spondylosis. A detailed medical history and a targeted, comprehensive physical examination are the initial approaches to rule out underlying disease that requires urgent attention. Clinical and evidence-based approaches to management suggest avoiding early MRI or CT, as imaging in elderly patients has proven both impractical and uneconomical. Instead, good clinical judgment should be used for making diagnoses. Consensus on the best initial approaches for managing CLBP has not yet been achieved, and conservative therapy is suggested, varying from use of pharmacologic agents, physical therapy, electrical stimulation, and physical manipulations to epidural injections. Surgical alternatives are avoided due to confounding and multiple comorbidities in older patients.

Psychiatric neurosurgery is defined as neurosurgery for treating psychiatric disorders that do not have identified structural brain anomalies. Early psychiatric neurosurgery procedures such as lobotomy became discredited in the 1970s because of severe complications. After a nearly 30-year hiatus until the late 1990s, psychiatric neurosurgery experienced a revival. Today, modern psychiatric neurosurgery is more precise and safer than its historical predecessors. Deep brain stimulation has become an established treatment for treatment-refractory Parkinson’s disease, and has been tested in several hundreds of patients with different psychiatric disorders. Reports about its successful application have also stimulated the development and application of ablative psychiatric neurosurgery such as thermal or radiofrequency ablation, as well as Gamma Knife® radiosurgery and magnetic resonance-guided focused ultrasound. This chapter analyzes the pros and cons of a range of different existing and emerging psychiatric neurosurgical procedures and evaluates them ethically.

An overview of the significance of emotions in mysticism during the medieval period would not be complete without an account of two other paradigms of affective arousal, namely, the suffering of Christ and the sacrifice of martyrdom. A mysticism of the passion of Christ, and of martyrdom as an imitation of the passion of Christ, emerged already in the early times of the church. In many ways, monastic asceticism follows this pattern, emphasizing acts of self-mortification, of spiritual martyrdom, and of mystical death, often invoking a psychomachy that includes the investment of the passions as well. The metamorphosis of the passions is based on practices that include the reading of the scriptures and mystical contemplation, but also liturgy and prayer. The practice of memory through reading, liturgy, and prayer that is at the center of the Christian life is for the most part also a practice of emotional stimulation. This article examines medieval mysticism, memory and prayer, spiritual sensation and emotion, negative theology and affective mysticism, and the link between the passion of Christ and the history of emotions.

This book maintains that container shipping is vital to the actualisation of globalisation, and that without it, globalisation would remain a concept rather than reality. It argues that container shipping has been academically overlooked as a global business sector in favour of more prominent sectors such as oil or arms trade, and aims to provide a complete history of containerisation from the 1950s to the turn of the millennium. This history explores the growth of the container industry due to prominent innovation in vessel design, early adoption of the internet, large international mergers, and significant physical alterations to the global port system. With particular emphasis on the east-west trade, the chapters cover the growth and development of the container industry, to the social changes experienced by seafaring labour forces, the cultural impact of the container - bringing a domineering land-presence to maritime activity, through to the environmental concerns surrounding the industry. The study is not a quantitative economic analysis of the industry, rather, an updated history that strives to demonstrate the importance of transport infrastructures to any consideration of global business sectors, by providing evidence of the container industry’s stimulation of the global economy.

The early modern age, conceived in this volume as a period spanning from 1450 to 1700, was an epoch of dramatic cultural and social developments. It witnessed major cultural encounters that produced what is currently labeled the first globalization, and intensified the worldwide circulation of a variety of cultural artifacts—as well as of people, knowledge, and ideas. Taking all these developments into account, it seems inevitable that many human groups in a variety of changing historical circumstances should have produced and practiced, over that period, distinctive forms of memory which it would not be fruitful to amalgamate into one unifying category. Rather than attempting that, the essays in this volume aim to explore a stimulating selection of a wide range of experiences of remembering and forgetting in early modern Europe.

Image-based measurements of myocardial blood flow afford the assessment of coronary circulatory function. They reflect functional consequences of coronary stenoses, diffuse epicardial vessel disease and microvascular dysfunction and structural changes and thus provide a measure of the total ischemic burden. Measured flows contain therefore clinically important predictive information. Fundamental to flow measurements are the tissue tracer kinetics, their description through tracer kinetic models, high spatial and temporal resolution imaging devices and accurate extraction of radiotracer tissue concentrations from dynamically acquired images for estimating true flows from the tissue time activity curves. A large body of literature on measurements of myocardial blood flow exists for defining in humans normal values for flow at baseline and during hyperemic stress as well as for the myocardial flow reserve. The role of PET for flow measurements has been well established; initial results with modern SPECT devices are encouraging. Responses of myocardial blood flow to specific challenges like pharmacologic vasodilation and to sympathetic stimulation can uncover functional consequences of focal epicardial coronary stenoses, of conduit vessel disturbances and disease and impairments of microvascular function. Apart from risk stratification, flow measurements may allow detection of early preclinical disease, influence treatment strategies and identify therapy responses.

Published in the 1890s and early 1900s, many of the essays in her innovative book The Handling of Words and Other Studies in Literary Psychology (1923) show her fascination with the idea of style as a form of contact and transaction between writer and reader, with style creating the perceptual patterns that persuade readers to think and imagine in ways not naturally their own.

Orthodox Radicals explores the origins and identity of Baptists during the English Revolution (1640–1660), arguing that mid-seventeenth century Baptists did not, in fact, understand themselves to be part of a larger, all-encompassing “Baptist” movement. Contrary to both the explicit statements of many historians and the tacit suggestion embedded in the very use of “Baptist” as an overarching historical category, the early modern men and women who rejected infant baptism would not have initially understood that single theological move as being in itself constitutive of a new group identity. Rather, the rejection of infant baptism was but one of a number of doctrinal revisions then taking place among English puritans eager to further their ongoing project of godly reformation. Orthodox Radicals thus complicates our understanding of Baptist identity and addresses broader themes including early modern religious toleration, the mechanisms by which early modern groups defined and defended themselves, and the perennial problem of historical anachronism. By combining a provocative reinterpretation Baptist identity with close readings of key theological and political texts, Orthodox Radicals offers the most original and stimulating analysis of mid-seventeenth century Baptists in decades.

The Introduction offers a brief overview of Paul Slack’s contribution to early modern history, distinguishing between an earlier phase concerned with social policy and the ideas which informed it, and a later phase concerned with the history of political economy, and particularly the shifting discourse of happiness which, he argued, informed it. It then explores recent interest in the history of emotions, distinguishing a variety of approaches to that subject. Reviewing three broad approaches taken by the contributors to the volume, it goes on to suggest that the history of emotions is most stimulating when seen as a focal point for different kinds of history rather than as a discrete subject of enquiry. A further implication is that a variety of forms of expertise need to be brought to bear.

This final chapter asks what it was like to resume ordinary life after serious illness: it traces the patient’s journey from the sickbed to the wider world of society and work. Organized around three areas—spatial, social, and working life—the overarching argument is that the return to normality was found to be physically liberating, socially bonding, and mentally stimulating. Ultimately, patients felt they regained not just their bodily faculties, but all the other aspects of life that they cherished, which sickness had rendered impossible, such as the enjoyment of company, the outdoors, and work. There were, however, significant downsides: residual weakness rendered daily tasks difficult for some patients, certain actions were thought to precipitate relapse, and for those who disliked socializing or working, returning to former employments could be troublesome. This argument challenges the entrenched notion that early modern people rarely retired from normal life during serious illness.

Ibogaine is an indole alkaloid derived from the African shrub Tabernathe iboga with broad anti-addictive, anti-depressant, and central nervous system stimulating effects. It is categorized as an oneriogen (or atypical hallucinogen) and has been used in West African tribal rituals for centuries. It was identified by French explorers in the early 1900s, came to the United States in the 1960s, and became marginalized in the mid-1990s after adverse outcomes halted federally funded human trials. Since then legal ibogaine treatment clinics have been established in countries without use restrictions. Ibogaine is a σ‎1 and σ‎2 receptor and μ‎ and κ‎ opioid receptor agonist and a α‎3β‎4 nicotinic and NMDA receptor antagonist. Decades of trials have demonstrated ibogaine’s potential. Human trials of ibogaine consistently demonstrate rapid remission of acute withdrawal symptoms but differ in their findings with regard to abstinence and toxicity. In radiation poisoning, a distinction between exposure and contamination should be made. Decontamination by removing clothes, washing of skin, and removal of debris can remove up to 90% of external contaminated radiation. Treatment of acute life-threatening injuries takes priority over treatment of radiation poisoning. Triage of severely exposed patients can give an indication of dose and severity of the radiation dose absorbed. Survival is related to dose absorbed. Identification of the radiation source should be made by the radiation characteristics to determine the shielding necessary for protection of hospital staff and the antidote required. Early gastric lavage and specific antidotes for ingested radiation poisoning should be used with caution. Death is mainly due to infection and haemorrhage. Acute radiation syndrome (ARS) is a manifestation of haematopoietic, gastrointestinal, cardiovascular, central nervous system, and cutaneous syndromes. Those receiving whole body doses of 1–5 Gy may recover easily with appropriate medical management; those with doses of 6–10 Gy may survive with intensive management; and those with doses of >10 Gy seldom survive. Treatment of ARS is supportive with the use of antibiotics, colony-stimulating factors, blood products, and stem cell transplants. Protection of the staff is by reducing time exposed, increasing distance from source and proper shielding. Psychological counselling should be available to patient or staff if required.

Tick-borne rickettsioses are emerging as more important health problems throughout the world. The spotted fever group including Rickettsia rickettsia can cause encephalopathy, meningitis and brain damage by selectively targeting capillary endothelial cells in the brain, and stimulating inflammation, capillary leakage, hemorrhage, and intravascular coagulation. Rickettsia are are arthropod-borne gram-negative coccobacilli bacteria and are obligate intracellular organisms that do not survive in artificial medium. In North and South America, the most common rickettsial disorder is rocky mountain spotted fever (RMSF) transmitted by the dog tick Dermacentor variabilis or the wood tick Dermacentor andersoni. A characteristic “starry sky” pattern can be seen on MRI imaging of the brain in some patients with RMSF encephalopathy and is thought to reflect the organisms targeting of brain endothelial cells in capillaries the white matter. Early treatment with doxycycline is curative and reverses signs of encephalopathy if given within a few day of onset, but delayed treatment can be associated with permanent neurological disability. The typhus group of rickettsia bacteria include R. prowazekii, which causes epidemic typhus and R. typhi, which causes murine typhus (endemic) typhus in tropical and subtropical parts of the world. Flying squirrels and humans carry R prowazekii and rats are carry R. typhi. Q fever caused by the rickettsia organism Coxiella burnetti is transmitted from farm animals including sheep and is seen throughout the world including the United States.

This book is based on over 40 years work in public health at a time of unprecedented change and challenge. The emphasis is on the practical aspects of working at different levels of action, very much ‘how to do it and how it was done’. As such it is a personal account.
