Resumen
The type 1 diabetes mellitus (T1DM) represents one of the most common diseases in childhood. Diabetes is linked to gastrointestinal complications (GI) such as intestinal enteropathy. The aim of this study was to characterize the duodenal histology in pediatric patients with T1DM in comparison to those without. Patients and methods. We enrolled 108 pediatric patients with T1DM (47 boys, 61 girls, mean age 13.1 ± 1.9 years) who were referred to the GI unit with duodenal biopsies collection (Children’s Republican Hospital, Ufa, Russia). Out of all children with T1DM, 74 (68.5%) showed histological signs of duodenal inflammation. In the comparison non-T1DM group, 21 patients (67.7%) showed inflammatory changes in duodenal mucosa. Histological evidence of chronic duodenitis was revealed significantly higher (p < 0.001) in group 2 of T1DM children (47/51) than in group 1 of T1DM (27/57). Celiac disease (CD) related duodenal lesions were found in 10 of T1DM patients (9.2%) and all confirmed by the serology. None of the comparison group children demonstrated CD signs on histology. Conclusion. The results of the present study demonstrated that chronic duodenitis is frequently present in patients with T1DM, but chronic duodenal inflammation is not always a consequence of CD or infection presence. Resumen
Purpose The purpose of this study was to examine the extent to which parenting styles is associated with diabetes control in children (aged 5—12 years) with type 1 diabetes, and on child and parent quality of life. Methods Data were collected from a total of 216 parent and child dyads, from 4 pediatric diabetes clinics in southern Ontario, using a cross-sectional survey methodology. Each parent and child independently completed the questionnaires. The study instruments included the Parenting Dimensions Inventory, Pediatric Quality of Life (diabetes specific), and chart reviews for glycosylated hemoglobin (A1C) levels. Results The results of the study demonstrated that parenting styles were not correlated with diabetes control and were weakly correlated with quality of life. Most parents reported behaviors of authoritative or democratic parenting. The mean glycosylated hemoglobin (A1C) for children in the study was slightly above optimal target range, at 8.4%. Parental education had a weak negative correlation with diabetes control. Conclusions Parenting styles are not associated with diabetes control and quality of life in children with type 1 diabetes. However, further research should assess the impact of the determinants of parenting on children with type 1 diabetes and quality of life.

Resumen
Chronic diseases, such as childhood diabetes mellitus (DM), are a challenge for sick children and their families. Paediatric DM is characterized by a complex therapeutic management treatment, which has a physical and psychological effect on the whole family and entails multiple losses for the parents. Investigators point out that parents of children with DS react to the diagnosis with anger, denial, frustration, and depression and that mothers differ in their reactions from fathers. The teenager goes through a very intense period of his life, full of emotional tensions and outbursts, anger, sorrow, fear, and shame for his desires, thoughts, and himself. Thus, a disease in the period of adolescence is very likely to harm the adolescent’s self-image and negatively affect body image and perception of strength and ability. For this reason, the investigations carried out in the context of this field are necessary to focus not only on the needs of the children but also on those of their parents, as this is how it can be achieved better control. Despite all the difficulties in the management of the chronic disease of their child, it seems that the families of diabetic children adequately respond to the demands of intensive care and probably manage satisfactorily the psychological, social, and physical levels of their child’s chronic illness.

Abstract. The antidiuretic effect and pharmacokinetics of 10 to 20 μg of intranasal (IN) and 200 to 400 μg of oral (po) 1-deamino-8-D-arginine vasopressin (DDAVP) were studied in 10 paediatric diabetes insipidus patients. A significant increase in urine osmolality was obtained with all doses, maximum within 2 h and still present at 8 h. At 12 h after administration, the ratio urine osmolality/plasma osmolality was above 1 only after 20 μg intranasally and 400 μg perorally. The free water clearance decreased rapidly with all doses and was similar in magnitude and duration for both the intranasal and peroral routes of administration and remained negative for more than 8 h. The maximum plasma concentrations of DDAVP, measured with a specific and sensitive RIA method, was dose-dependent and there was not significant difference in time until maximum concentration was obtained or in plasma half-life between the two routes of administration. The ratio established, 1:20, by calculating the area under the curve showed a bio-equivalence between 10 μg IN and 200 μg po and between 20 μg IN and 400 μg po of DDAVP. This work further emphasized the effectiveness of the oral route and the rapidity of absorption. By continuous monitoring of DDAVP plasma values we have demonstrated that peak values were reached within one hour after administration. This study demonstrates that the doses needed to treat diabetes insipidus patients by the oral route will be approximately 20 times greater than by the nasal route.
