Texto completoThe concept of sexual health was institutionalized by the World Health Organization (WHO) in the 1970s, when controversies between psychoanalysis and cognitive-behaviourism were growing. The new working definition proposed by the WHO in the early 2000s endorsed the cognitive-behavioural approach articulated to utilitarianism, by firmly embedding it in the continuation of its concept of health as a state of well-being. This concept thus combines an epistemological dimension with a moral and social dimension. This research work aims to shed light on the contemporary uses of the concept of sexual health by tracing its genesis, which far precedes its institutionalization by the WHO. We thus go back to its first occurrences in the first half of the 19th century, when health refers mainly to the absence of disease. We adopt a method of historical epistemology, with work on the texts of the time in the Anglo-American medical field and in the field of medicine on the European continent. While it is common to question the concepts of the medical and health fields, particularly those related to sexuality, from a social perspective that highlights their normative and biopower dimensions, this work aims to study the concept of sexual health by conducting an internal analysis of the fields in which the concept has been forged and deployed. Without contesting the relevance of the socio-political approach, the methodological choice that acknowledges its internal rationality makes it possible to better emphasize what can be irreducibly normative in the concept of sexual health. The latter is therefore analysed according to the scientific model in which it emerges, namely vitalist hygienist physiology. This genesis will shape its history until today, while a sexology that makes it its object, and a mechanistic sexual medicine that also refers to it, are in opposition. The history of biological epistemologies sheds light not only on the genesis and uses of the concept of sexual health, but also on its relationship to the field of sexual pathology. The opposition of epistemological models results in different, if not adverse, therapeutic approaches to the conception and treatment of sexual problems. According to the WHO, sexual health is not reduced to the absence of sexual diseases, but is holistic and aims to cover the mental and social aspects as well, in a preventive and educational approach in the area of public health. This last point also underlines the ethical burden, from a eudemonic perspective, conferred on it by its hygienic uses since the 19th century. This work thus allows at the same time: 1) to place the concept of sexual health in a longer history than that which usually begins after the Second World War; 2) to enrich the historiography of sexology, for which the concept of sexual health has become major in the 21st century; 3) to question the history of sexuality as developed by Michel Foucault; 4) to shed light on the relationships between the normal and the pathological that are at stake in the concept of sexual health and in the controversies between psychodynamics and cognitive-behaviourism; 5) to examine the idea that contemporary uses of this concept would contribute to the medicalization of sexuality

Texto completoOf interest of everyone, health is not an easy concept to figure out. Although there is no universal definition of health, its various uses and meanings allow us to specify its biological, psychic and social dimensions. In animal health, the biological dimension remains the most studied, there is therefore no consensual definition of international animal health. So how to understand the health of marine shellfish? A living being can be functionally described in physiopathology by monitoring biomarkers under laboratory conditions. In men and many animals, "normal" biomarker values have been proposed; these have been determined by carrying out measurements or observations in samples of healthy populations, the rigorous selection of these populations and samples being a major issue. This approach has long been based on the idea that a living being is in "good health" when it is in the "norm", or otherwise expressed, that its biomarker values are part of the distribution of biomarker values obtained in a population in "good health". Different stages then led to changing the terminology of "normal" or usual values, sometimes called medical constants, to prefer those of reference values and reference intervals. The definitions of these terms and their determination have been the subject of numerous publications and international standards which constitute the scientific body of the theory of reference values. After choosing biomarkers of interest in biological health in the Pacific oyster Crassostrea gigas (first issue), to establish healthy oyster populations (second issue), is it possible in a first study to determine reference intervals of these biomarkers in a reference sample group (third issue), then to verify that they provide elements of hypotheses on the health, by carrying out complementary studies of comparison with the observed values of biomarkers in Pacific oysters experimentally infected by pathogens, OsHV-1 (ostreid herpesvirus type 1) on the one hand, Vibrio aestuarianus on the other (fourth issue)?

Texto completoBased on a practice in so-called extreme contexts (street and hospital clinics), the present doctoral project aims at exploring the main conceptualizations of health, through several disciplines linked to psychology. Between philosophy (of medicine) and psychoanalysis, our thesis project intends to outline and synergize the meanings of health as a term and an administrative field (part I), a philosophical concept inscribed in a fifty-year old debate (part II) and a notion having a latent expression in the heart of the psychoanalytic clinic (part III). Within this multidisciplinary framework, our doctoral project counters the widespread rejection of the concept of health in psychoanalysis. This project gravitates around an axis of theoretical-clinical and epistemological questioning : to what extent and how does the outline of a Canguilhemian definition of health recapture the hypothesis of the Freudian unconscious ?

Texto completoThe law n° 2002-303 of March 4th 2002, relative to the rights of the patients and to the quality of the health system marks a decisive and debated bend in the maïeutics of sanitary democracy in France. Sanitary democracy is not any more only considered under the angle of the priviledge of the citizen's participative rights in public debate and in decision-making in sanitary matters, but also under the broader angle of the defense of the rights of the "sick person". In that way, the law of March 4th 2002, based on the concept of sanitary democracy, intends to bring to the foreground both a movement of promotion of the rights of the patients (sanitary democracy in the broad sense) and a political project (sanitary democracy in the strict sense). The study bases itself on the dialectics of both conceptions of sanitary democracy such as suggested by the law of March 4th 2002, relative to the rights of the patients and to the quality of the health system. Having raised a semantic rigorous lack on behalf of the legislator of 2002, the study tries to seize the reach and the impact of the highlighted terms of "sanitary democracy", such as they are stipulated. Within the singular colloqium by the law of March 4th 2002, that is under the angle of four main ideas : trust, justice, transparency and responsibility. Then the study focuses again on the essence of the political concept of sanitary democracy, namely the participation of the citizens, that is the sample group of laymen and experts, in debating and decision-making in sanitary matters, especially through public debater, consensus civic conferences. Which allow the confrontation of both experts and laymen' s points of views. All things considered, is the user of the health system becoming a "new citizen" ?

Texto completoThe evaluation of quality of the patients' life is central in health, especially in Public Health research programs and policies. Many instruments are developed and discourses on health related quality of life grow in number and diversity. Several approaches and accounts of the concept coexist. However, this growth and diversity is synonymous with enduring doubts about the validity of the existing concepts of health related quality of life. In order to understand and maybe dissolve the confusion, we have first to agree on a common framework of analysis for the concept of quality of life. But, to our knowledge, this framework of analysis is not available. And yet this framework is necessary in order to explicit, compare and test the theoretical choices that determine the concept of quality of life, and most of all of the good quality of life, underlying the measuring instruments. It is indeed the conceptualization of value, of what is good in life, which is at stake. But this aspect is concealed: the approach of quality of life in health is generally but mistakenly conceived as value free or neutral. This concealment probably explains why a common framework of analysis of the existing concepts of health related quality of life is not available. From an understanding of the logic of value specific to quality of life, we try to develop such a framework of conceptual analysis

Science cannot male health ; the means it implements can only improve it provided they are supported by human responsibility. Apart from this twofold process, the main thing lacking to medecine, taken as a solution to health problems in a given community, will always be "efficiency". The present approch can be dealt with by provinding upgrades for rudimentary health care, synthesizing roles and duties, in order to give back to the communites the paternity of their health system

Texto completoThe payment of hospital fees by patients is a major reason for poverty in China, especially in rural China. To improve the effectiveness and equity of healthcare financing in rural China, we’ll make a feasibility study for the establishment of micro-insurance and reinsurance in rural China. This thesis is composed of four parts. The first part researches the problematic and the justification of this study. The second part is to present the general health situation in China, including health insurance in China, whether rural or urban. In the third part, a study on the application conditions of micro-health insurance in rural areas has been made. Finally, we will consider a system of social reinsurance for micro-insurance that could be created. According to the contribution calculated and the average willingness to pay, we can say that the establishment of micro-insurance in rural areas would be feasible

Texto completoThis thesis explores the processes supporting the action of managers and supervisors on occupational health. We used our experiences of professional interventions with team supervisors to question this issue. A first study questions the impasses of the concept of psychosocial risks to consider the action of managers on occupational health of the team members. Then, a second study investigates the operational concept of the psychological dynamics of the job related to discussion about work. For the third study, we test different management devices through discussion using a research-action methodology in an establishment for elderly dependents (EHPAD). Analysis of the results of this third study brings out the resistance to discussion. This requires individual learning, collective and organizational work for discussion with supervisors, teams and stakeholders involved in health at work (management, health professionals at work, social partners). Based on these results, we define the requirements for discussion spaces and the role of supervisors in order to develop psychological health of team members
Vanier, Antoine. "The concept measurement, and integration of response shift phenomenon in Patient-Reported Outcomes data analyses : on certain methodological and statistical considerations". Thesis, Nantes, 2016. http://www.theses.fr/2016NANT1009/document.
Texto completoPatient-Reported Outcomes are increasingly used in health-related research. These instruments allow the assessment of subjective concepts such as Health-Related Quality of Life, anxiety level, pain or fatigue. Initially, the interpretation of a difference in score over time was based on the assumption that the meaning of concepts and measurement scales remains stable in individuals’ minds over time. This assumption has been challenged. Indeed, the self-assessment of a concept is now understood as a contingency of the subjective meaning a subject has of this concept, which can change over time especially as a result of a salient medical event: the “response shift” phenomenon. Since the end of the 1990s, researches on response shift phenomenon has become of prime interest in the field of health-related research. If developments have been made, it is still a young field with various scientific debates on a theoretical, methodological and statistical level. Thus, the broad objective of this thesis is to investigate some methodological and statistical issues regarding response shift concept, detection and integration into PRO data analyses. The manuscript is composed of three main works: a state of the art and synthesis of the works conducted at an international level since response shift phenomenon is investigated, a pilot study investigating the statistical performances of the Oort’s Procedure (a popular method of response shift detection using Structural Equation Modeling) by simulations and a theoretical work about the links between response shift occurrence and semantic complexity of concepts measured and items used

Texto completoThe One Health concept recognizes that human health, animal health and environmental health areinterconnected and promotes multisectoral and multidisciplinary collaboration to efficiently manage health issues atthe human-animal-environment interface. One Health surveillance is strongly encouraged as it suggests theenhanced performance and cost-effectiveness of surveillance. Nevertheless, surveillance systems continue to bedeveloped and operated in a highly sectoral approach. We argue that the challenging operationalization of OneHealth surveillance is partly due to the existence of confusion and uncertainty regarding its practical application aswell as its added value comparing to sectoral surveillance components operating independently. In this context, we developed a conceptual and methodological framework to help the implementation ofOne Health surveillance. First, we characterised One Health surveillance and its application contexts using asystematic literature review of existing One Health surveillance systems followed by an expert opinion elicitation. Then, we mobilized two approaches, stakeholder analysis and companion modelling, to develop and apply twoinnovative methods to facilitate the application of the One Health concept by surveillance actors. Finally, weidentified specific attributes to support the development of a tool aiming at evaluating the quality andappropriateness of implemented collaboration to meet the collaborative objectives. Along with other projects led byresearch teams across the world, our research work contributes to the promotion of One Health surveillance

Texto completoHow to measure the degradation of nature, its "health" condition, to determine the depreciation of its use, not recorded in the balance sheets of the nations? My thesis work on the "ecosystem accounting" of the Rhone river basin is an experimental first on a new methodology integrable with the still incomplete tools of the national accounts of the type GDP. Consuming ecological capital, our renewable resources, without amortizing means the creation of ecological debts. The goal of ecosystem accounting is to produce a decision support tool to avoid the net degradation of ecosystems through science-informed public policies. The tool produces ecological balances based on geographical information and measured in physical terms, making it possible to perform modelizations and simulations in order to estimate the internalisation of externalities (Figure). I have developed various synthetic indicators relating to the functions of ecosystems and their integrity, to calculate the ecological potential of the Rhône watershed. The accounting diagnosis is accompanied by a spatial description of the changes observed in order to better understand their relevance. The tool is likely to help the various territorial entities to position themselves on the management of their strategic renewable resources (water, soil, biomass, infrastructures and ecological services) in the context of a changing world order: major geopolitical issues in terms of security and food and energy sovereignty, in their link with public health

Texto completoSince the 1950s, the pharmaceutical industry is still affected by the scandals of health scandals (thalidomide 1961, 1962 and 1963), Distilbène (1977), Vioxx (2004), Mediator (2009), breast prostheses PIP (2010), ...These are generally health products that have a harmful effect that is not detected earlier in the collective management of these products throughout the health product development chain. These harmful effects have generated disastrous consequences in France and abroad ( some organizations say that these are cases of pain that have caused illness, cancer or death), but also in economic and financial terms (national health costs), image and trust. This is the diagnosis made about the functioning of the pharmaceutical industry. This diagnosis reveals the existence of a dilemma that this sector must undoubtedly resolve. Various situations are often mentioned to refer to this dilemma. Some situations refer to the negligence and imprudence of institutions and organizations for the control and supervision of health products, others are oriented towards the fault and error of the actors involved in the collective management of these products, and others are about fraud perpetrated by actors mandated to develop health products that help the population to eliminate the different diseases. This thesis focuses on situations of fraud committed by big pharma as a research subject. She positions him in the field of white-collar crime, a term invented by American sociologist Edwin. Sutheland in the 1930s). The analysis of this research subject poses a practical question: How big pharma performs to normalize fraud in the health product development chain that includes national, transnational and global levels. It is important to note that this thesis does not aim to stigmatize big pharma, which are essential actors because they contribute to the disappearance of diseases by providing health products to society. Rather, it focuses on a phenomenon that has been clearly identified and that represents a threat to the planet, which has already affected health systems several times, thousands of people in the world and caused considerable human and financial losses. Thus, it relies on the experience of institutional, organizational and individual actors in the health sector, in terms of health scandals marked by pharmaceutical fraud, to answer research questions. The research results show that, big pharma is instrumentalising P.O.I, a structured and structuring organisation, to perform the actors who are involved in the collective management of health products in such a way that their practices and discourse are consistent with what she thinks, says and does. It is through this mechanism that pharmaceutical fraud is normalized at the national, transnational and global levels. More specifically, the P.O.I is a network organisation composed of human and non-human actors.Hidden within the big pharma, it materializes the ability of this enterprise to produce a generalized effect in the process that describes the different phases of realization of a health product, which will encourage the actors involved to legitimize the fraud in order to serve the interests of the entreprise in which it is housed

Texto completoAims: The number of researches on QOL is increasing. However, the QOL concept is characterized by lack of agreement and showed conceptual problems. The objective of our study is to understand how philosophy can help to renew our understanding of the QOL concept. Methods: Analytical process started with a literature review in Philosophy (Antiquity to contemporary) and Public Health (in Medline-1990-2010). Conceptualization and measurement theories of happiness, preference, satisfaction, and autonomy have been also studied in order to reveal the underlying theories. Through these concepts we have isolated patient's moral expectations. These moral expectations are formalized as values corresponding to the four ethical principles developed by Beauchamp and Childress, in The principles of biomedical ethics. These principles constitute a recognized consensual platform, based on the four international principles: autonomy, beneficence, non-maleficence and justice. Results: The Public Health approaches of QOL don't focus enough on the moral expectations of children. To assess the applicability of the principles in the field of health, we are chosen twenty-eight QOL's measurement instruments. Then, we confront these instruments to ethical principles, in order to determine how the moral expectations of children are explored. This confrontation has showed that all the instruments don't tackle the four fundamental moral expectations. Nevertheless, a change in perspective has taken place during these four decades: questionnaires made since the late 1990s put more emphasis on the four ethical principles

Texto completoIntroduction. Clinical reasoning (CR) is a key competence that students in health sciences must acquire. This thesis argues that concept mapping improves midwives students' performance in clinical problem solving. Objectives. Identify midwives students CR difficulties and their explanatory causes; identify the use of concept mapping and show compatibility between concept mapping and recommendations for clinical reasoning; introduce concept maps (CMs) in clinical solving learning sessions and observe their effects.Methods. Mixed exploratory study, literature review and experimentation including 97 students nested in three schools of midwifery: 570 questionnaires and 352 CMs were analysed. Results. Seven causes of CR difficulties were identified. Literature confirms CMs may be a relevant tool for clinical reasoning teaching and learning. By making knowledge explicit CM helps one out of two students to improve clinical problem solving performance when CM include a correct core concept and when concepts are hierarchically organized. The best performing students are those who acquire the construction of CMs.Conclusion. The research validates the starting assumption and leads to proposals of using CMs in CR learning. Concept mapping might be an interesting activity to foster CR and identify the difficulties that students may encounter

Abstract: The purpose of this thesis is to show that the concept of natura to Saint Augustine contributes to a philosophy of nature. For this, three perspectives are considered: 1 - The controversy against the Manichaeism, which assigned evil to the material substance; 2 - The dispute between Augustine and Pelagio about the human nature; 3 - The environmental point of view of Augustine on the relations between human nature and natura creata. Following this course, the concept of natura remains attached to the notion of divine grace and acquire four meanings: theological, existential, historical and environmental. Among moral and political problems, the Bishop of Hipona expands their ontic notion of natura according to the trinomial God - human nature - natura creata, and contributes to a philosophy of nature while arational discourse about the being in change.

Texto completoThis research analyzes cattle health through individual and collective competencies, based on Commons institutionalism. The first part defines a new institutional conceptual framework, alternative to the neoclassic microeconomic models, to study animal health. Health management is analyzed at farm level in terms of farmers' competencies and capabilities considering the relationship between individuals and unorganized going concern. It integrates thereafter organized going concerns and defines production focused organizations and health focused organizations. The competencies and capabilities have both territorial components. The model is validated empirically in the second part, using the national cattle identification database and the milk control program database, for 2005 and 2006. Dairy cattle mortality and milk bacteriological quality (somatic counts) are explained with linear models and static and dynamic panel models, respectively. A global effect of farmer's health competencies on the overall health issues of the farm is shown, but competencies must be translated into capabilities (actions) before impacting on health. The spatialization of the models reveals some territorial correlations that clearly show the effect of institutions on animal health. The institutional analysis shows that an important part of the herd health rests on the farmer's individual competencies, as well as on some collective competencies. These competencies can be strengthened and revealed into capabilities with more accurate and regular preventive interventions in farms and with more farmers' education

Texto completoCharacteristic of the philosophical thought in the XIII century, the doctrine of the transcedentals emerges from the eleven questions that figure as an introduction in the Summa de bono by Philip the Chancellor. Such theory is first developed in Alexander of Hales’ Summa theological, in Albertus Magnus’ De bono treatise and in his Super Dionysium De divinis nominibus. Thomas Aquinas elaborates, since the perspective of his antecessors, such a theory, expanding the numbers of maxime communia. Among these, the notion of verum is highlighted in our research. Insofar ratio veri implies in adaequatio intellectus et rei, we aim to demonstrate how adaequatio unifies the concepts of veritas rei and veritas intellectus.

Texto completoUnderstanding how students elaborate the concepts during the school trajectory is an important tool in the teaching learning process, since they can assist the design and planning of learning activities. In this context, taking into account that the cell is a key concept in the biological sciences, this study aimed to understand how students elaborate the concept of cell in their trajectory, using as collateral the comic "Lung and Her Crew", and propose actions that can be used in basic education contributing to more effective development of the concept of cells and Science Teaching. The study was conducted with students from elementary and middle schools in two public schools in Santa Maria, RS. For data collection semi-structured questionnaires to evaluate the use of the comic book "Lung and his gang," as a pedagogical tool for students were used, and semi-structured interviews to understand how students elaborate the concept of cell and if there is increased complexity of cell concept reported by students throughout the school year. In the analysis of the interviews sought to identify the methods of generalization of the concept, establishing categories based on the stages of development of the concepts developed by Vygotsky. From the survey data we proposed educational activities that can contribute to a more effective development of the concept of cells and Science Teaching. The results obtained in this study revealed that for the question of the cells, the high school students are those who have had some notion of the concept of cells. We note that, although students may have an idea about what they say, the real complexity of the issue probably was not perceived in its entirety by the student and should not be incorporated into their cognitive structure. However, we should point out that the concept can be merely discursive without an actual of what the subject conceptualized reflection. These results reinforce the idea that the description, whether written or verbal, that cells are the morphological and microscopic units that make up living things, can not pass a speech with little cognitive or scientific significance. That is, cell, gene, chromosome "are things of biology" that are related to life, but rather precisely within the cognitive structure of the students.

Texto completoThe problem of the demarcation between metaphysics and science, as this one is really built, received his problematic with bacon and above all with Kant. From Kant, science, in relation to causality, takes the weak meaning of a non-absolute truth that always grows in course of time. Nevertheless, must metaphysics no more take part in the development of science? We show by the history of biology of the XIXth century that in fact scientifics problems are sometimes very close to metaphysics ones, that science also treats of these, and that its solutions are not always free of metaphysics choices. Consequently, the problem of the demarcation does not seem to us well resolved by Kant and the positivists, like by the neo-positivists and popper in the XXth century, if indeed this problem can be resolved

Texto completoThis text is a research on the concept of virtue in the thought of the “young” Augustine. In the truth, it wants to find a theory of the virtue, that if finds of form separate for the analyzes books. Thus, the texts of Agustine analyzed includes the period enters your after-conversion to the Christianity in the year of 386, where it writes the “dialogues of Cassiciacum”, until the year of 388, in which it writes De Libero Arbitrio Book I and De Moribus Ecclesiae Catholicae et De Moribus Ecclesiae Manichaeorum. The text looks for to verify as the virtue concept appears in the thought of Agustine, bringing many conceptions of the previous thought, manly of the stoics and neoplatonics, but that, after the “discovery” of the faculty of the will in the De Libero Arbitrio Book I, and the association of this with the virtue, passes to be considered manly by Augustine. Leading in consideration the discoveries of De Libero Arbitrio Book I, in the analysis of De Moribus Ecclesiae Catholicae et De Moribus Ecclesiae Manichaeorum, we will show the association of the virtue concept to the love concept, and a new change in relation to the virtue concept.

Texto completoThe Santa Maria watershed is located in southwestern of Rio Grande do Sul State and has area of 15,740 km ². Irrigation by means of agriculture is the largest user of water. The main goal of this research was to analyze strategies of water management based on discourses of social agents of the Committee of Management of Santa Maria River Watershed (CMSMW) from 2008 to 2012 - which means two biennium of management. It was proposed to analyze discourse changes from the minutes of meetings of the CMSMW. To accomplish these aims, firstly it was categorized social agents speeches - which are called ideas - to determine a sample unit of analyses, about water management instruments (resulting in N = 433). After that it was applied the concept mapping method, which was used to organize and to articulate the visual representation of networks of ideas, by using multivariate statistics techniques. Lately, it was developed a database and were included in activities that are part of the internal dynamics of the CMSMW. In total, 45 organizations had ideas recorded in minutes in the period of analysis. The structure of the conceptual map showed that there is a preponderance of the discussions and effective actions of the Committee related to the registration of users for the grant of water use Another aspect is that the uncertainty of supply due to lack of monitoring of discharges is a critical factor in the basin, resulting in a wide trading, but without effective referrals. There was a 13% increase in ideas classified as Water Resources Management Instruments from one biennium to another, and it was shown an improvement in the quality of the minutes. Also, there was a preponderance and increasing qualification of such matters as "system information user and granting of grants" and "strategies for managing supply" with an emphasis on self-management. Despite the large amount of ideas, the method assisted in the analysis of preponderant themes, inter-relationships, decision-making and effectiveness of actions. It is expected that the results of the analysis may assist the Committee in self-assessment of its actions and the direction of its management.

The focus of this work is the symbology, using similes, allegories and metaphors, which Saint Teresa of Jesus used in her work, Meditaciones sobre los Cantares with a view to explain how she felt, experienced and sensed the love that God had for her and that she struggled to repay. For her, the main source for her inspiration was The Song of Songs, the "Shir Hashirim" of the Hebrews, "El Cantar de los Cantares" (in Teresa s language). In the text of Santa Teresa, she did not titled but that their editors have titled Meditaciones sobre los Cantares (SANTA TERESA, 1979, p. 333), the author described the love of God experienced as conflict (and it is perceived an influence of St. Augustine). God was felt as "missing" and experienced as desire, desire to be united to Him in body and soul. This duplicity, in turn, became a mystical love. It Teresa of Jesus is in the same line of some philosophers of antiquity, who understood love as "missing", that is, that we desire what we do not have and love for what we want (Plato, Origen and Augustine). In Meditaciones sobre los Cantares, Santa Teresa expressed their will (mystical, on the one hand, and body on the other) to find God and to be with Him in body and soul. Thus, Santa Teresa showed to be in tune with the more traditional and orthodox theology, which stated that in Christ there are two natures: fully human and fully divine (Council of Chalcedon, 451 AD). To report this desire of meeting, Teresa used symbols of The Song of Songs, expressing them and renewing them in his Meditations work on them Song, a central theme of our project.

Texto completoUnderstanding psychological factors behind the reluctance to use preventive testing is a significant step towards a more efficient health care policy. Some people visit doctors very rarely because of a fear to receive negative results of medical inspection, others prefer to resort to medical services in order to prevent any diseases. Recent research in the field of Behavioral Economics suggests that human's preferences may be significantly influenced by the choice of a reference point. In the first chapter I study the link between loss aversion and the frequently observed tendency to avoid useful but negative information (the ostrich effect) in the context of preventive health care choices. I consider a model with reference-dependent utility that allows to characterize how people choose their health care strategy, namely, the frequency of preventive checkups. In this model an individual lives for two periods and faces a trade-off. She makes a choice between delaying testing until the second period with the risk of a more costly treatment in the future, or learning a possibly unpleasant diagnosis today, that implies an emotional loss but prevents an illness from further development. The model shows that high loss aversion decreases the frequency of preventive testing due to the fear of a bad diagnosis. Moreover, I show that under certain conditions increasing risk of illness discourages testing.
In the second chapter I provide empirical support for the model predictions. I use a questionnaire study of a representative sample of the Dutch population to measure variables such as loss aversion, testing frequency and subjective risk. I consider the undiagnosed non-symptomatic population and concentrate on medical tests for four illnesses that include hypertension, diabetes, chronic lung disease and cancer. To measure loss aversion I employ a sequence of lottery questions formulated in terms of gains and losses of life years with respect to the current subjective life expectancy. To relate this measure of loss aversion to the testing frequency I use a two-part modeling approach. This approach distinguishes between the likelihood of participation in testing and the frequency of tests for those who decided to participate. The main findings confirm that loss aversion, as measured by lottery choices in terms of life expectancy, is significantly and negatively associated with the decision to participate in preventive testing for hypertension, diabetes and lung disease. Higher loss aversion also leads to lower frequency of self-tests for cancer among women. The effect is more pronounced in magnitude for people with higher subjective risk of illness.
In the third chapter I explore the phenomena of crowding-out and crowding-in of motivation to exercise self-control. Various health care choices, such as keeping a diet, reducing sugar consumption (e.g. in case of diabetes) or abstaining from smoking, require costly self-control efforts. I study the long-run and short-run influence of external and self-rewards offered to stimulate self-control. In particular, I develop a theoretical model based on the combination of the dual-self approach to the analysis of the time-inconsistency problem with the principal-agent framework. I show that the psychological property of disappointment aversion (represented as loss aversion with respect to the expected outcome) helps to explain the differences in the effects of rewards when a person does not perfectly know her self-control costs. The model is based on two main assumptions. First, a person learns her abstention costs only if she exerts effort. Second, observing high abstention costs brings disutility due to disappointment (loss) aversion. The model shows that in the absence of external reward an individual will exercise self-control only when her confidence in successful abstention is high enough. However, observing high abstention costs will discourage the individual from exerting effort in the second period, i.e. will lead to the crowding-out of motivation. On the contrary, choosing zero effort in period 1 does not reveal the self-control costs. Hence, this preserves the person's self-confidence helping her to abstain in the second period. Such crowding-in of motivation is observed for the intermediate level of self-confidence. I compare this situation to the case when an external reward is offered in the first period. The model shows that given a sufficiently low self-confidence external reward may lead to abstention in both periods. At the same time, without it a person would not abstain in any period. However, for an intermediate self-confidence, external reward may lead to the crowding-out of motivation. For the same level of self-confidence, the absence of such reward may cause crowding-in. Overall, the model generates testable predictions and helps to explain contradictory empirical findings on the motivational effects of different types of rewards.
