Texto completoEven while the number of cancer patients continues to grow, the number of nurse navigators (NNs) has plateaued. Given this situation, NNs and their managers must review, identify and delimit their contribution to the care of cancer patients. The purpose of this study is to provide a detailed description of the care activities actually performed by local and supra-regional NNs. The present research project has been guided by the mixed framework consisting of the main nursing functions, as set out by Dallaire et al. (2008), and the definition of the NN’s role, as set out by Fillion et al. (2012). Working from a non-probability judgment sample consisting of five participants, the student researcher employed a data collection method based on the structured non-participatory observation of the practice of these nurses coupled with individual semi-structured interviews. The results show that NNs perform more care activities in relation to the clinical dimension (55%) than to the organizational dimension (45%). NNs primarily perform co-ordinating and caring functions. Co-ordinating ranks first, at 41% of the observed care activities, while collaboration-related activities (4%) are seldom observed. That being said, and without generalizing the results, it would appear that all care activities are positively affected by the sound performance of an interdisciplinary team working according to a well-structured care trajectory.

Texto completoIntroduction: French care system was built around the management of acute pathologies in the hospital. We understand the difficulty of integrating the management of chronic pathologies into the hospital, both in terms of its organization and the attitudes adopted by the care providers. However, for several years now, support for the commitment of patients to better empowering them has been a sought-after objective in the cancer care pathway. The importance of nursing care in this area no longer has to be proven today. At a time when France is reorienting its health policy towards prevention and the engagement of users in care pathways, and when the Universitarisation of nurses’ education process has been initiated, we wanted to ask ourselves about the drivers and levers for change as well as the obstacles and factors facilitating the patients’ empowerment in the cancer care pathway, under the focus of nursing research. Methods: In the context of 3 studies, practices and/or tools based on experimental approaches (Evidence Based Practice (EBP)) participating in supporting patient empowerment are implemented or questioned from the point of view of their acceptability, particularly in the daily practices of oncology nurses. These studies are implementation, real-life, interventional and multicentric research. The articles presented all report qualitative research, which uses an analysis tool, the Theoretical Domains Framework (TDF) to study implementation problems. Results: Our research shows a major influence of caregivers, and particularly nurses, on the patient's ability to actively integrate their care continuum in Comprehensive Cancer Centers. The methodological implementation framework that we have adapted to French culture has enabled us to deploy a tailored intervention according to the contexts of the centers participating in PHRIP Paract. We have also been able to evaluate the multi-center implementation of a patient education program dedicated to cancer patients treated by oral anticancer therapy, as part of the PEPS Co project. The current culture and organization of nursing care, still very tinged with paternalism, attest to an approach focused mainly on disease and not under an individualized focus considering the patient as a whole. In addition, oncology nurses are not naturally committed to adapting their daily practices despite a changing environment. The methodology used allowed us in particular to identify the obstacles and factors facilitating a better transfer of EBP in oncology nursing care. Conclusion: These studies allowed us to draw up methodological recommendations to prepare the implementation of a change in nursing practices on the cancer care pathway promoting patient empowerment, as well as recommendations for the dissemination of a PE program dedicated to oral anticancer therapies

Texto completoFaced with demographic changes, the rise of chronic diseases including cancer, and in response to the fragmentation of current healthcare systems, care coordination has become a priority in global public health. Cancer patients are particularly at risk of receiving poorly organized care due to the complex nature of the disease and its management, which involves multiple medical, nursing, socio-medical, and social teams operating at different levels of the healthcare system over an extended period. This lack of coordination affects the quality and efficiency of care. Progress in care coordination has been hindered by the lack of a common definition and precision on how it should be implemented. In France, new nursing coordination functions in oncology have been introduced without prior conceptualisation, leading to a wide variety of actions and job titles. This heterogeneity hampers the recognition, understanding of their actions, and the evaluation of their performance. In the absence of a detailed analysis of the scope of action of these professionals and a theoretical framework for care coordination, this thesis project aims, in three parts, to model nursing care coordination intervention in oncology and to identify ways to implement these results into the national health policy of care coordination in France. The first part (conceptual) allowed the development of an innovative reference framework focused on coordination activities through a scoping review and a structured consensus method (nominal group technique) involving decision-makers, organizers, and patient partners. The second part, focusing on practices, produced an operational model of hospital-based cancer coordination nurses’ intervention, by comparing theoretical expectations (framework from part 1) with the practices and perceptions that these nurses, their patients, caregivers, and professional partners had of this coordination intervention. The field survey consisted of a national multicenter mixed-method study through a triangulation of qualitative (observations, interviews, focus groups) and quantitative (validated scales) information on practices, contexts, perceptions, and attitudes towards work among oncology care coordination nurses in France. The third part was dedicated to transferring results to decision-makers through the organisation of a national workshop involving all stakeholders in the field (regulators, hospital administrators, professional associations, researchers, nurses, patient partners). Its objective was to present the modelling results, share experiences from other research teams on the subject, and translate the operational model into concrete actions through consensus-building of a job description, and defining the minimum elements of managerial support and prerequisites for these nurses' functions. This thesis project has generated new knowledge on care coordination, both in general and more specifically in oncology, at a conceptual level (reference framework), interventional level (operational model), and translational level (job description). The work presented in this thesis thus paves the way for optimal deployment of these functions for public health decision-making (implementation of the job description, standardisation of training), and research evaluation of the effects and impact of these interventions
