Abstract Background: Extending adjuvant endocrine treatment (ET) with aromatase inhibitors (AI) to 7-10 years decreases the risk of relapse in hormone receptor-positive (HR+) breast cancer (BC). However, such benefit comes at the price of higher incidence of skeletal and cardiovascular (CV) events. Biomarkers predicting such toxicities might help clinicians in tailoring adjuvant ET to patient’s needs. We conducted a prospective study to assess whether SNPs in the gene encoding for the aromatase enzyme (CYP19A1) affect the risk of skeletal and CV events in HR+ early BC patients enrolled in the GIM4 trial. Methods: The GIM4 trial randomized HR+ BC postmenopausal patients who had been already treated with 2-3 years of adjuvant tamoxifen to either 3-2 years or 5 years of adjuvant letrozole. Four SNPs of CYP19A1 were evaluated: rs10046, rs4646, rs479292 and rs727479. SNPs were genotyped through PCR on DNA obtained from patients’ peripheral blood samples. Skeletal and CV events were assessed from randomization in the GIM4 trial to last follow-up, disease recurrence or death. Skeletal events were defined as the onset of osteoporosis or bone fractures. CV events were defined as the onset of thrombosis, embolism, stroke, myocardial infarction, hearth failure, or arrythmia. Univariate and multivariate logistic regressions were performed to evaluate the association between SNPs and skeletal and CV events. Bonferroni correction for multiplicity was used for univariate SNPs association tests, with a corrected alpha of 0.012. Only associations with an alpha level <0.012 were considered significant and included in multivariate models.Multivariate models’ covariates included: treatment arm (5 vs 3-2 years letrozole), (neo)adjuvant chemotherapy, previous hormone replacement therapy, age, BMI, previous biphosphonates, hypercholesterolemia and hypertension at baseline. SNPs were tested for Hardy–Weinberg equilibrium (HWE) and linkage disequilibrium (LD). Interaction between SNPs of interest and treatment arm was also investigated. Results: Of 2,056 patients enrolled in the GIM4 trial, 647 entered this translational study. All SNPs were in HWE. The CT and TT variants of rs10046 were less associated to skeletal events compared to variant CC, suggesting a dominant effect of the minor T allele (odds ratio [OR]: 0.47,. 95% confidence interval [CI]: 0.28-0.79, p=0.005). The association was independent from the other covariates (adjusted OR [aOR]: 0.43, p=0.002). Patients with rs10046-TT had no significant differences in the odd of skeletal events according to treatment arm. Conversely, in patients with rs10046-CC, skeletal events were more likely with 5-year letrozole (interaction p=0.043). Compared to rs10046-CC, rs10046-TT/CT was also less associated with CV events at both univariate (OR: 0.46 95% CI: 0.27-077, p=0.003) and multivariate analysis (aOR: 0.41, p<0.001).The rs727479—GG variant was associated with more CV events compared to the GT and TT variants (OR: 2.30, 95%CI: 1.27-4.15, p=0.009), suggesting a recessive effect of the minor G allele. The association remained significant at multivariate analysis (aOR: 2.30, p=0.010). No significant interaction was observed among treatment arms, rs10046 or rs272479 variants (interaction p=0.070 and 0.061). Rs10046-CC and rs727479-GG variants were in high LD (D’=0.92, haplotype-CG frequency: 39%). Hence, the association between genotype rs10046-CC/rs727479-GG and CV events was investigated. The association was significant at the multivariate analysis (aOR: 2.79, p=0.002). With such genotype, the odd of CV events was significantly increased with the 5-year letrozole (interaction p=0.046). Conclusions: SNPs of CYP19A1 could be useful biomarkers of long-term toxicity in HR+ BC patients who are candidates for adjuvant AI. Abstract Background: A previous study on 22 metastatic breast cancer (mBC) patients (Allegretti et al. Mol Cancer 2021) has associated drug resistance to Ado-trastuzumab emtansine (T-DM1) with two sets of genomic events: a) reversal of HER2 amplification, b) ‘oncogenic replacement’ of HER2 by alternative cancer drivers. To expand on this, we designed GIM21 (Gruppo Italiano Mammella) study. Materials and Methods: GIM21 is a multicentre, prospective study; tumor and circulating total nucleic acids (tTNA and ctTNA) were obtained from primary and metastatic lesions (n=36 altogether), as well as plasma samples (n=501), the latter serially collected coincident with medical imaging re-evaluations. tCNAs/ctTNAs were sequenced by ultra-deep, largely overlapping 50-gene panels on Ion Torrent Gene Studio S5. Ortogonal dPCR validation was by dPCR (Quant Studio 3D). ctTNAs were correlated with clinical readouts and patient outcomes. Results: from September 2018 to January 2022, 50 HER2+ mBC patients receiving T-DM1 as second-line treatment. Median time to progression was 6.5 months (range 2.0-27.2). As previously shown, only a minority of patients (9/50 – 18%) retained residual HER2 amplification in blood at baseline, likely due to HER2 counterselection during previous therapy lines; all of them (9/9) underwent further neutralization during T-DM1 treatment, but an HER2-neutral blood status was reached in only 5 patients. In contrast, 2 HER2-neutral patients acquired HER2 amplification, suggesting either insufficient T-DM1 pressure or an unprecedented clonal escape mechanism. Overall, the circulating HER2 status did not correlate with progression free survival (PFS), further highlighting a loss of clinically relevant HER2 oncogenic dependence. At baseline, 24 circulating mutations were detected in 29/50 (58%) patients, 9 of which did not overlap with tissue mutations. Of note, carrying a given aberration in either blood or tissue resulted in outcome trends (p=0.16), but no clear association with therapeutic response. Rather, it was serial monitoring (appearance of any aberration in the blood) that predicted a dismal clinical outcome in 16/41 (39%) patients, with a median time to progression of 2.8 months (2.2-3.9). Unfortunately, liquid biopsy (LB) was confirmed to miss most patients developing brain (3/3 patients) or skin (2/3 patients) metastases. Most circulating alterations present at progression (40/60) were actionable (OncoKB level < 3B) in 34/41 (82.9%) patients, suggesting that LB may guide therapeutic strategies in post-T-DM1 settings. Conclusions: GIM21 trial showed that LB accurately predicts clinical outcome and reveals actionable drivers of T-DM1 escape. The final analysis are ongoing. Abstract Background: Although male breast cancer accounts for <1% of all breast cancer diagnoses, its incidence is increasing. Palbociclib in combination with an aromatase inhibitor or fulvestrant is indicated for treating HR+/HER2- ABC in men. Adding to a previous analysis describing real-world patient characteristics and treatment patterns in men with HR+/HER2- ABC, this analysis describes additional patient characteristics, study site characteristics, and preliminary tumor response data in male patients enrolled in the POLARIS study. Methods: POLARIS is a prospective, noninterventional real-world study of patients with HR+/HER2- ABC who received palbociclib as deemed appropriate by a physician; the targeted enrollment is 1500 patients from approximately 110 US and Canadian sites. Patient and study site data were collected from the treating physicians’ routine clinical assessments (eg, patient medical charts, surveys, site questionnaires). Real-world tumor response was determined based on the treating physicians’ assessments of imaging results and clinical progression. Results: As of December 17, 2020, 15 men were enrolled across 12 US sites; most were community-based sites (75%; Table 1) and most physicians (58.3%) had been treating patients with ABC for 11-20 years. The median age of patients was 66 years, 46.7% of patients had visceral disease, and 41.7% had bone-only disease. Seven patients received palbociclib plus an aromatase inhibitor, 7 patients received palbociclib plus fulvestrant, and 1 patient received palbociclib plus tamoxifen. Two patients initiated palbociclib treatment at 100 mg due to comorbidities. Five patients had ≥1 comorbidity (ie, anemia, neutropenia, cellulitis, myocardial infarction, hypertension, neoplasm, or cerebrovascular disorder). All patients received ≥1 concomitant medications (Table 2). Medications commonly received included antihyperglycemics (53.3%), antidepressants (46.7%), opioids (40.0%), antihyperlipidemics (26.7%), and antithrombotics (26.7%). Among all 15 patients, 1 had a best tumor response of complete response (CR) and 4 patients had a partial response (PR), as reported by the physician. Among patients who received palbociclib in the first-line setting (n=9), 1 had a CR and 3 had a PR, as reported by the physician. Conclusions: These findings add to the current knowledge of the male patient population enrolled in the POLARIS study and provide preliminary information on clinical outcomes. This population represents a real-world patient cohort with a heavy disease burden. Although longer follow-up of this patient cohort is warranted, these results continue to support palbociclib as a treatment for men with HR+/HER2- ABC. Pfizer (NCT03280303). Abstract Introduction: Engaging patient advocates in research is a practice that identifies and prioritizes patient perspectives and needs throughout the academic or clinical research process. “Patient advocates” may refer to those receiving care, survivors, caregivers, or other individuals with a stake in a patient’s respective health outcomes. Research shows that engaging advocates in research can have a multifaceted positive impact, including better quality research, expanded applicability of research, and greater understanding of findings within the patient community. However, most researchers report limited engagement of advocates in their research citing significant challenges in its implementation. Purpose:The purpose of this project was to identify the needs of researchers and advocates involved in research at the UNC Lineberger Comprehensive Cancer Center (Lineberger), in order to expand the capability and quality of their engagement. Lineberger recognized the importance of understanding the current engagement structure and identifying ways to increase meaningful opportunities for advocates to participate throughout the research process.Methods:To assess the needs of Lineberger researchers and advocates, we took a mixed-method approach to data collection and analysis. The needs of researchers were assessed quantitatively using a Qualtrics survey aimed at measuring researchers’ involvement and satisfaction with involving advocates in research, current communication methods, and training needs. Four interviews were also conducted with key informant researchers. To assess advocate needs, we conducted a focus group with five Lineberger advocates to explore their connection process with researchers, their training experience, and perceived utilization in cancer research. Quantitative data collected from the survey was analyzed using both Qualtrics’s internal report output and Excel. The interview and focus group recordings were reviewed and critical ideas were transcribed to identify themes.Results: Several common themes emerged when assessing the needs of researchers and advocates. Both groups felt the connection process between researchers and advocates could be improved. The lack of a formal process was a hindrance, with 25% of researchers reporting at least one experience where they were unable to find an advocate to include in their research. Participants also felt training would be helpful. Moreover, advocates and researchers felt that more frequent and streamlined communication would improve project work. Lastly, both groups expressed a desire to expand the advocate pool to include more diverse voices that adequately represent the community.Recommendations:We proposed several recommendations to address the identified needs. To increase connection between advocates and researchers, we recommend a centralized list of trained and vetted advocates available to all Lineberger researchers, and a process, run by a dedicated coordinator, to match available patients with study needs. To improve and increase communication between the two groups, Lineberger should develop a bidirectional, web-based communication platform. For increased efficacy of advocate contributions to research, Lineberger should provide formal, systematic training for researchers and advocates , each group receiving training on distinct, tailored topics. We recommend that the patient pool be intentionally diversified to include categories of gender, race, age, education level, economic status, and cancer-specific knowledge, which would better represent the Lineberger patient. profile. Lineberger should also provide adequate compensation for advocates involved in Lineberger research to ensure the engagement is equitable and ethical. e4912046This scoping exploratory review was aimed at analyzing the challenges that the so-called post-truth world represents for teaching in Ibero-Latin American higher education. With the increased access to online information media and social networks, netizens are increasingly exposed and may be more vulnerable to false or misleading information that seeks to generate action from emotions rather than reason (GOSWAMI, 2017, Chronicle of Higher Education). The reference search was carried out in the databases of SciELO and La Referencia, from which 26 titles out of 196 were selected. Combinations of terms such as social media, post-truth, fake news, fact-checking, education, higher education, university, teaching, critical thinking, and freedom of expression were used, with the Boolean “Y” connector. The analysis of the references resulted in six thematic categories: main concepts; realms of fake news; news verification initiatives and methods; theoretical analysis and its relationship with education; studies on the factors, perception and credibility of fake news; and addressing misinformation in higher education. The discussion presents the draft of a proposed pedagogical model to be used in higher education and to address misinformation. Includes: critical thinking habits, democratic dialogue, intellectual skepticism, research skills, use of reliable sources of information, and analysis from multiple perspectives.ResumoEsta revisão exploratória de escopo teve como objetivo analisar os desafios que o chamado mundo pós-verdade representa para o ensino na educação superior ibero-americana. Com o aumento do acesso às mídias de informação online e redes sociais, os internautas estão cada vez mais expostos e podem ficar mais vulneráveis a informações falsas ou enganosas que buscam gerar ações a partir de emoções ao invés da razão (GOSWAMI, 2017, Chronicle of Higher Education). A busca das referências foi realizada nas bases de dados SciELO e La Referencia, das quais foram selecionados 26 títulos em 196. Combinações de termos como mídia social, pós-verdade, notícias falsas, checagem de fatos, educação, ensino superior, universidade, ensino, pensamento crítico e liberdade de expressão foram usadas, com o conector booleano “Y”. A análise das referências resultou em seis categorias temáticas: conceitos principais; escopos de notícias falsas; iniciativas e métodos de verificação de notícias; análise teórica e sua relação com a educação; estudos sobre os fatores, percepção e credibilidade das notícias falsas; e aproximação a desinformação no ensino superior. A discussão apresenta o esboço de uma proposta de modelo pedagógico para ser usado no ensino superior e para lidar com a desinformação. Inclui: hábitos de pensamento crítico, diálogo democrático, ceticismo intelectual, habilidades de pesquisa, uso de fontes confiáveis de informação e análise de múltiplas perspectivas.ResumenEsta revisión exploratoria de alcance tuvo como fin analizar los desafíos que para la enseñanza en la educación superior iberoamericana representa lo que se denomina el mundo posfactual (post-truth). Con el incrementado acceso a medios de información en línea y las redes sociales, los cibernautas están cada vez más expuestos y pueden ser más vulnerables a información falsa o engañosa que busca generar acción a partir de las emociones antes que la razón (GOSWAMI, 2017, Chronicle of Higher Education). La búsqueda de referencias se efectuó en las bases de datos de SciELO y La Referencia, de la cual se seleccionaron 26 títulos de 196. Se usaron combinaciones de términos como redes sociales, posverdad, noticias falsas, verificación de hechos, educación, educación superior, universidad, enseñanza, pensamiento crítico y libertad de expresión, con el conector booleano “Y”. El análisis de las referencias dio como resultado seis categorías temáticas: conceptos principales; ámbitos de las noticias falsas; iniciativas y métodos de verificación de noticias; análisis teóricos y su relación con la educación; estudios sobre factores, percepción y credibilidad de las noticias falsas; y abordaje de la desinformación en la educación superior. En la discusión se presenta el borrador de un modelo pedagógico propuesto para ser utilizado en la educación superior y abordar la desinformación. Resumen
La tesi delinea le vicende biografiche e artistiche del lucchese Giovanni Domenico Campiglia, artista noto per aver preso parte alle maggiori imprese editoriali del Settecento a Firenze e Roma. Il lavoro è articolato in otto capitoli, corredati da appendice documentaria e immagini, che partono con la ricostruzione della biografia. Grazie a nuove ricerche sono emersi particolari inediti della vita dell’artista che hanno permesso di correggere innanzitutto la data di nascita, anticipandola di un anno rispetto a quella tradizionalmente ritenuta corretta. Inoltre il rinvenimento di alcune lettere ignote alla bibliografia su Campiglia e lo studio dei registri parrocchiali, con particolare riferimento agli Stati delle Anime, hanno consentito di precisare altre date del nebuloso periodo giovanile del disegnatore. Il capitolo dedicato alla pittura fa il punto sullo stato delle conoscenze sull’attività di Campiglia in questo campo, che probabilmente non fu al centro dei suoi interessi molto a lungo, ma su cui si aspettano ancora rinvenimenti che possano gettare una luce anche in questa direzione. In tal senso di un certo interesse risulta un quadro a lui attribuito passato pochi anni fa sul mercato antiquario di cui si dà conto, pur con le dovute cautele. Ardua da dipanare è la questione degli allievi dell’artista, su cui le fonti quasi tacciono; se è vero che sappiamo chi furono i suoi collaboratori occasionali nel corso delle imprese editoriali da lui dirette, poco o niente conosciamo invece degli artisti che dovettero transitare dalla sua bottega come allievi. L’incrocio delle informazioni desunte da varie fonti consente adesso di aprire uno squarcio sulla bottega dell’artista e al tempo stesso di confutare alcuni nomi riportati dalla critica moderna come allievi di Campiglia. Il capitolo relativo ai granturisti con cui l’artista fu in contatto nel corso della sua lunga carriera ci mostra un Campiglia perfettamente inserito nell’ambiente internazionale che caratterizzava la città papale nel crescendo continuo di viaggiatori provenienti dal resto d’Europa. La ricerca si è poi indirizzata sullo studio dell’attività grafica di Campiglia, attività in cui riusciva ad esprimersi al meglio, come riconosciuto anche dai contemporanei. I nuclei più consistenti di disegni, a ciascuno dei quali è dedicato un capitolo, sono adesso alla Eton College Library, al Gabinetto Disegni e Stampe degli Uffizi ed un terzo all’Istituto Centrale per la Grafica. I primi ad essere realizzati furono i disegni per il facoltoso committente inglese Richard Topham che alla sua morte lasciò in eredità alla biblioteca del College di Eton i volumi con i disegni da lui commissionati a vari artisti, tra cui, con oltre seicento fogli, Campiglia. I disegni degli Uffizi servivano da modello agli incisori incaricati delle tavole del Museum Florentinum (1731-1762), impresa editoriale promossa da Anton Francesco Gori e volta a mostrare i pezzi di maggior pregio della collezione medicea, e non solo, di scultura, numismatica e glittica degli Uffizi. Si tratta di più di mille fogli, quasi tutti attribuiti a Campiglia, sebbene in alcuni casi sia lecito dubitare dell’autografia. Stesso scopo avevano i disegni adesso conservati all’Istituto Centrale per la Grafica, erano cioè i disegni di riferimento per gli incisori che lavorarono alla pubblicazione del Museo Capitolino (1741-1782), impresa editoriale che si pone come un vero e proprio catalogo del neonato Museo sul Campidoglio e il cui fautore fu monsignor Bottari. I disegni sono stilisticamente diversi tra loro e ciò è dovuto in primis all’evoluzione e alla maturazione artistica dell’artista, ma anche alla diversa destinazione di tali fogli. Se i nuclei romano e fiorentino possono essere accostati in virtù del comune utilizzo dei disegni per l’edizione a stampa, quello di Eton aveva tutt’altro scopo ed era per un committente, Richard Topham, che voleva farne un uso “privato”. C’è infine un ultimo capitolo che prende in esame altri piccoli nuclei di fogli attribuiti a Campiglia, confermandone e confutandone l’attribuzione. Il criterio selettivo è stato l’originalità dei disegni rispetto alle numerose riproduzioni delle tavole delle imprese editoriali che si trovano sparse in diverse collezioni, pubbliche e private. In questo capitolo si è scelto in parte di privilegiare fogli che mostrassero un Campiglia inedito e diverso dal solito (per esempio con un piccolo gruppo di caricature), sebbene nella maggior parte dei casi la produzione riguardi sempre l’antico. Nell’Appendice documentaria sono raccolti gran parte dei documenti citati nel testo e utili alla comprensione dello stesso, in parte trascritti in questa sede per la prima volta.

Il volume raccoglie alcuni saggi dedicati alla figura di Anton Francesco Gori. Punto di riferimento degli studi antiquari toscani del Settecento, il Gori non fu soltanto un erudito, ma il perno organizzativo della cultura fiorentina e delle iniziative bibliografiche e pubblicistiche del tempo. L'immensa mole di lettere a lui inviate, conservata alla Biblioteca Marucelliana di Firenze, è stata oggetto di un Progetto strategico dell'Università di Firenze finalizzato alla catalogazione, informatizzazione e immissione in rete. Il volume si colloca nell'ambito del progetto e comprende oltre ai saggi una significativa antologia delle lettere e un indice completo dell'epistolario per mittenti.

This chapter examines Justice Antonin Scalia’s actions in the notorious case of Bush v. Gore. There, five conservative justices voted to stop the recount of the Florida vote in the election of 2000 and make George W. Bush president. The chapter outlines the position of the justices and focuses on the two opinions that Scalia joined, a per curiam for seven relying on the Equal Protection Clause and a concurrence by Chief Justice William Rehnquist for three of the seven based on Article II. The chapter argues that both of those opinions contradicted virtually all of Scalia’s jurisprudential principles, including those involving standing and the political question doctrine. Further, it argues that both of those opinions—together with the solo opinion Scalia wrote supporting a stay of the Florida recount and certain other contextual factors—demonstrate that Scalia’s actions in the case were deeply personal and inspired by his intense desire to see the Republican candidate win the election.

Anton Martin Slomšek je leta 1862 v Drobtinicah izdal članek z naslovom Slava ro-doljubom in utemeljiteljem našega slovstva, v katerem je izpostavil posameznike, ki so pomembno vplivali na razvoj in nastanek enotnega slovenskega jezika; med njimi so tudi Prleki in prebivalci Slovenskih goric. Posebej je omenil Leopolda Volkmerja in Antona Krempla. Slomšek je bil pobudnik za to, da se organizirajo spominske slovesnosti na pomembne zgodovinske dogodke in naše rojake. Tako sta bili leta 1863 v Ljutomeru in leta 1867 pri Mali Nedelji organizirani narodnokulturni prireditvi, imenovani béseda. Glavno vlogo pri ohranjanju spomina in organizaciji prireditev so imele čitalnice. Časovna oddaljenost dogodkov pa je, žal, povzročila, da je veliko naših rojakov pozabljenih, med njimi tudi duhovnik in pesnik Jakob Košar.

Prispevek predstavlja vizualni pregled krajevnih postaj iz življenja in delovanja An-tona Kremplja, ki ga v slikovni podobi na razglednicah hrani Zbirka drobnih tiskov v Univerzitetni knjižnici Maribor. V zbirki je danes za raziskovanje in prezentacijo na voljo preko 55.000 razglednic domačih krajev in tujine. Z njihovim izborom sledimo življenjski poti Antona Krempla, in sicer od Radgonskih goric, kjer se je 1790 na Poličkem vrhu pri Gornji Radgoni rodil, do krajev šolanja (Gornja Radgona, Maribor, avstrijski Gradec) in delovanja v Svetinjah pri Ljutomeru, Ormožu, Ptuju, Sv. Lovrencu v Slovenskih goricah in nazadnje pri Mali Nedelji. Izbor razglednic štajerskih krajev tako predstavlja gradivo iz zbirke, ki ilustrativno oriše Kremplovo biografsko pot.
