Traditionally, amloki is used as a well known supplement in pregnancy, which is rich in trace elements.To determine the effects of iron and amloki on serum zinc level in IDA with pregnancy. The study was a non blind, non random sampling interventional type of clinical trial. This study was performed in the Department of Physiology, Dhaka Medical College, Dhaka on 43 pregnant women between 13th to 20th weeks of gestation with IDA from July 2016 to June 2017. They were recruited from Outpatient Department of Obstetrics and Gynaecology of Dhaka Medical College Hospital. Anaemic pregnant women supplemented with oral iron and amloki were considered as study group (A) and control group (B) were with only iron supplementation for 45 days. Serum zinc level was estimated in the laboratory of the Department of Soil, Water and Environment, University of Dhaka, Bangladesh. For statistical analysis, Paired Student’s‘t’ test and Unpaired Student’s‘t’ test were considered using SPSS 22.0 version. Significant decrease (p<0.001) of serum zinc level was observed after intervention of iron in both groups. In this study, there was no significant difference in serum zinc level in between study and control group. It can be concluded that oral iron supplementation causes decrease in serum zinc level in IDA with pregnancy. Amloki shows no significant role in preventing decrease of serum zinc level in this study. Emblicaofficinalis (amloki) is traditionally used to treat iron deficiency anaemia. Background: Common clinical practice of prescribing Iron supplementation for Iron deficiency anemia (IDA) in pregnancy is associated with a number of side effects. Emblica officinalis (amloki) is a well known dietary supplement used by traditional practitioners to treat anemia. Objective: To evaluate the effects of oral supplementation of Emblica officinalis on iron status in pregnant women with IDA. Methods: This prospective interventional study was carried out in the Department of Physiology, Dhaka Medical College (DMC), Dhaka from July 2016 to June 2017 on 43 pregnant women aged 18 to 36 years between 13th to 20th weeks of gestation with IDA. They were recruited from the Out-patient department of Obstetrics and Gynaecology, DMC Hospital. Among them 24 were supplemented with amloki and iron (IAS) and 19 women received only iron supplementation (IS). Study variables were estimated at the baseline and after 45 days of supplementation. Serum iron, ferritin and Total iron binding capacity (TIBC) were estimated following standard laboratory methods. Data were expressed as mean ± SD. Paired and Unpaired Student’s t-test were used for statistical analysis. Results: Serum iron levels were significantly higher (p<0.001) and serum TIBC were significantly lower (p<0.001) in both groups after supplementation compared to their baseline value. But post supplementation serum ferritin level was significantly higher (p<0.01) only in IAS group compared to that of the baseline. Again, after intervention, serum iron level was significantly higher (p<0.05) and serum TIBC was significantly lower (p<0.01) in IAS group when compared with those of IS group. Conclusions: Data concluded that oral Emblica officinalis supplement along with iron was more effective than only iron supplementation to improve serum iron status in pregnant women with IDA. A recent pandemic called coronavirus (Covid-19) has been spreading rapidly around the world this year, 2020. While some countries have confirmed cases and deaths, others as Mexico start to see far approaches of what is likely to be a large coronavirus outbreak. A video posted in YouTube by Andres Manuel Lopez Obrador, known by his nickname AMLO, portrayed his opinion regarding the coronavirus and the security measure. This paper, based on Critical Discourse Analysis theory and Systematic Functional Linguistics, explore AMLO’s speech by analyzing focused on transitivity and modality to discover what is behind the Mexican president’s speech and its social consequences. The findings suggest that AMLO’s speech has a different intention than convince the Mexican population that everything is fine.

On his first day in office, on 1 December 2018, freshman President of Mexico, Andrés Manuel López Obrador (AMLO) delivered a speech outlining 100 policy priorities of his administration. The present study analyzed the contributions of this government’s program relating to food security and their environmental implications, and whether they contributed to strengthen the state or improved human security, considering that the poor and marginalized were at the center of AMLO's campaign. In total 45 policy priorities were geared to consolidate the state, while 55 contributed to improving human security. Only six were related to food security, including stipends to food producers and purchasing grains at guaranteed prices, a fertilizer distribution program and subsidies for cattle husbandry and fisheries/aquaculture. These programs contributed to advancing 10 of the 17 Sustainable Development Goals, especially those related to Zero Hunger and Reduced Inequalities. Various policy programs had explicit considerations towards climate change and land degradation, including the exclusion of natural protected areas from agricultural subsidies, and recognized that food production is vulnerable to climate change. The four agricultural programs analyzed may advance AMLO’s goal of avoiding food imports, while curbing rural poverty. However, available evidence is mixed regarding animal acquisition loans, which are likely to have adverse environmental outcomes. Finally, the program for developing agroforestry operations is already contributing to deforestation, and further ecosystem degradation is most likely to occur from the introduction of timber and fruit species to natural forests as this program does not preclude the inclusion of recently cleared plots. If human development goals are to be reached, along with fulfilling the international commitments on sustainable development and environmental conservation, policies need to be implemented that simultaneously tend to a booming transnational industry, while bringing forward the rural poor, who amount to nearly half of the country's population.

In order to achieve the objectives of transparency and accountability, Mexican President Andrés Manuel López Obrador (AMLO) has offered a press conference every morning since he took office. This situation seemed to be a transcendental change in the field of democratic dynamics and political communication in Mexico; however, not merely a means of communication, these conferences have instead become a method of government. Using postulates of Mimetic Theory, this essay analyzes AMLO’s conferences, showing how this daily practice has become propaganda for the regime.

Abstract This article inquires the charismatic populism of López Obrador (AMLO) and Mexico’s return to hyper-presidentialism. Drawing on the literature on populism and hyper-presidentialism it explores the tension between charismatic populist leadership and independent institutions, especially the Supreme Court and the National Electoral Institute serving as a counterbalance to the president’s agenda, which aims for institutional ‘transformation’. The main argument, that AMLO is defying the integrity and independence of these institutions, is sustained by analyzing a) the Zaldívar Law named after the former Supreme Court President, a last-minute amendment that would extend his term; b) the public consultation on the prosecution of former presidents; c) the recall vote, and d) the electoral reform. The article concludes that AMLO’s charismatic populism, his transgression of constitutional constraints to the executive power and use of meta-constitutional powers means a return to hyper-presidentialism, which also raises concerns about Mexico’s struggling democracy.

But in September 2023, AMLO, as he is commonly known, made a trip to Colombia and Chile, Latin American countries that are also currently governed by the left. In Colombia, AMLO met with President Gustavo Petro and participated in the Latin American and Caribbean Conference on Drugs, where Petro called for an end to the failed strategy of “viewing drugs as a military problem and not as a health problem for society.” AMLO said that preserving family unity and combating poverty are essential to that fight.

In 2018, recently elected presidential candidate Andres Manuel Lopez Obrador (AMLO) promised the construction of a new refinery in Mexico. Arguing a lack of energy independence and the urgent need to ‘rescue’ petro-state giant PEMEX and the stateowned electricity company CFE from the mismanagement and neoliberal policies of previous administrations, the Dos Bocas Refinery (DBR) became one of the main flagship projects of AMLO’s administration symbolizing a discourse of energy security and national pride. This paper reviews the process of approval and construction of the refinery by assessing, the material and relational character of energy infrastructure, the “politics and poetics” that are built into the promises of infrastructure projects, and the shifting temporalities of infrastructure and their interaction with emerging ‘petropopulist landscapes’ which serve as material evidence of oil-led development. Drawing on Anthropology's and Geography’s ‘infrastructural turn’, this paper reviews a series of government documents, speeches and declarations supported by interviews with energy experts to understand the symbolic meaning of energy infrastructure and how the DBR has become deeply entangled with a nationalist political project which has instituted an inertial path-dependence towards the continued use of fossil fuels, off-staging other concerns associated to climate change and the energy transition at the national level.

In order to achieve a more convenient and intelligent handling of LOTOS specifications, an approach is proposed wherein specifications are translated into an appropriate equational logic language and interpreted. LOTOS has an excellent formal model for describing communication processes and equational logic languages have the capability of inferencing and data abstraction. Naturally, the marriage of LOTOS and equational logic provides a powerful framework with features that could not be realized in existing technologies. To demonstrate this potential, AMLOG is chosen as the appropriate equational logic language. Apart from rapid prototyping of specifications, some of the exciting possibilities of the new framework that are demonstrated include stepwise refinement and testing for correctness of specifications, facilities for property checking and verification of bisimulation equivalences. Also, the derivation of behavior expressions from trace histories is shown. The approach, extensible to other FDTs as well, is a step towards realizing an intelligent communication protocol specification environment.

Introduction. Polymer nanoparticles are a key tool to deliver drugs to specific sites and to increase drug bioavailability. Aim. This research aims to use poly amide-disulfide nanoparticles as drug delivery systems. Method. Amlodipine (Amlop) was used as a model, forming Amlop-polymer nanocomposites. In this work, we investigated the effect of independent variables (polymer, Fe3+, Al3+, and pH) on the dependent variables (loading efficiency (%LE), zeta potential, and particle size). Nanocomposites were prepared by an inotropic method. Nanocomposites were characterized by powder X-ray diffraction (PXRD), field emission scanning electron microscopy (FE-SEM), Fourier transform infrared spectroscopy (FTIR), and a release study. Results. From the XRD data, the Amlop-polymer nanocomposite shows semi crystallinity. In addition, the disappearance of drug peaks indicates that the drug was incorporated between the polymer molecules and was amorphous in behavior. The FTIR for the nanocomposite shows the functional group of the drug, which indicates the incorporation of Amlop into the nanocomposite. From FE-SEM, the results showed that our nanocomposites have an average particle size of approximately 130 nm. The release of amlodipine from the Amlop-polymer nanocomposite was found to be controlled, with approximately 85% within approximately 24 hours. Conclusion. The amide-disulfide polymer nanoparticles are promising carriers for different types of drugs.

8067 Background: We studied the value of a proposed prognostic index (PI) generated by baseline absolute monocyte (AMC) and lymphocyte (ALC) counts for pts with DLBCL, using values as previously reported (Leukemia 25:1502-9, 2011). Methods: From 03/07 to 01/09, 245 consecutive pts with untreated DLBCL receiving standard R-CHOP from the MDACC database were evaluated. Baseline AMC and ALC were retrospectively recorded. High AMC (≥610/uL) and a low ALC (≤1000/uL) were examined as dichotomized variables for progression-free (PFS) and overall survival (OS). An AMLPI was generated, stratifying pts into 3 risk groups (RGs): low-(AMC <610/uL and ALC >1000/uL), intermediate-(AMC ≥610/uL or ALC ≤1000/uL), and high-risk(AMC ≥610/uL and ALC ≤1000/uL). The prognostic effect of the AMLPI and the IPI were examined by multivariate analysis (MVA). Results: Ninety (37%) had high AMC and 71 (29%) had low ALC. By univariate analysis, a high AMC was associated with inferior PFS (p=0.01) and OS (p=0.03). The frequencies of AMLPI RGs were: low-105 pts (43%), intermediate-119 (48%), and high risk-21 (9%). With a median follow-up of 22 months (range <1-42), 3-year PFS and OS rates for these RGs were 80%, 61%, and 46% (p=0.007) and 92%, 76%, and 60% (p=0.006), respectively. Three-year PFS rates for IPI 0-2 and 3-5 RGs were 73% and 58%, respectively (p=0.0004); comparable OS rates were 88% and 68%(p<0.0001). For pts with IPI 0-2, 1-year PFS rates for AMLPI low, intermediate, and high RGs were 92%, 89% and 80% (p=0.022); comparable 1-year OS rates were 96%, 95% and 80% (p=0.049). By MVA, AMLPI effect (low vs. high RGs) on PFS was significant (p=0.046) as was IPI effect (0-2 vs 3-5, p=0.005); similar results were observed for OS (p=0.052 and p=0.003, respectively). Conclusions: Baseline AMC and AMLPI are significant variables for PFS and OS for pts with DLBCL receiving R-CHOP. AMLPI can identify pts with low, intermediate, and high-risk disease for PFS and OS, particularly for those with IPI 0-2. AMLPI may also add prognostic value beyond that of the IPI.

Healthy riverine systems provide ecological services that can be translated into human use and consumption. However, human activities cause environmental degradation limiting available pristine water supply and raising public health concerns. In the present study, four tributaries surrounding Mt. Matutum Protected Landscape (MMPL) were selected (Glandang, Linan, Kawit, and Amlok) for evaluation of their physicochemical and microbial characteristics between wet and dry season. The pH and total dissolved solids (TDS) of the river waters ranged from 6.75-8.68 and 55.96-221.89 ppm, respectively, with a pronounced pH fluctuation and TDS increase in Glandang and Linan tributaries from upstream to downstream stations. All tributaries showed varying dissolved oxygen (DO) levels upstream but become relatively stable downstream. All DO values, however, are below the standard limit set for freshwater quality. In terms of microbial load, total coliform ranged from 51-275 cfu/ml while E. coli showed absence (0 cfu/ml) to as high as 77 cfu/ml. Higher total coliform counts were observed on Kawit and Amlok tributaries which are situated in areas that are relatively more forested than the other two rivers. Presence of E. coli was also detected in the water samples from all tributaries. Temporal variation is significant indicating increased pH, TDS and microbial load during the wet season and DO during the dry season. Purpose: Great variety and controversies surround the management strategies of acute multiligament knee injuries (aMKLIs) and no established guidelines exist for resource-limited practices. The aim of this study was to compare the management approach of acute knee dislocations (AKDs) by orthopedic surgeons from nations with different economic status. Methods: This descriptive cross-sectional scenario-based survey compares different management strategies for aMLKIs of surgeons in developed economic nations (DEN) and emerging markets and developing nations (EMDN). The main areas of focus were operative versus non-operative management, timing and staging of surgery, graft choice and vascular assessment strategies. The members of the Societe Internationale de Chirurgie Orthopedique et de Traumatologie (SICOT) were approached to participate and information was collected regarding their demographics, experience, hospital setting and management strategies of aMLKIs. These were analyzed after categorizing participants into DEN and EMDN based on the gross domestic product (GDP) per capita. Results: One-hundred and thirty-eight orthopedic surgeons from 47 countries participated in this study, 67 from DEN and 71 (51.4%) from EMDN. DEN surgeons had more years of experience and were older (p < 0.05). Surgeons from EMDN mostly worked in public sector hospitals, were general orthopedic surgeons and treated patients from a low-income background. They preferred conservative management and delayed reconstruction with autograft (p < 0.05) if surgery was necessary. Surgeons from DEN favored early, single stage arthroscopic ligament reconstruction. Selective Computerized Tomography Angiography (CTA) was the most preferred choice of arterial examination for both groups. Significantly more EMDN surgeons preferred clinical examination (p < 0.05) and duplex doppler scanning (p < 0.05) compared to DEN surgeons. More surgeons from EMDN did not have access to a physiotherapist for their patients. Conclusions: Treatment of aMLKIs vary significantly based on the economic status of the country. Surgeons from DEN prefer early, single stage arthroscopic ligament reconstruction, while conservative management is favored in EMDN. Ligament surgery in EMDN is often delayed and staged. EMDN respondents utilize duplex doppler scanning and clinical examination more readily in their vascular assessment of aMLKIs. These findings highlight very distinct approaches to MLKIs in low-resource settings which are often neglected when guidelines are generated.

Background: Hypertension is a major public health problem associated with large health burden as well as high economic burden at individual and population levels. A prospective observational study carried out in in-patients admitted to general medicine department of tertiary care hospital to evaluate the most cost effective therapy among the monotherapy of Amlodipine and combination therapy of telmisartan + hydrochlorthiazide. Methods: A total of 120 Hypertensive patients case records prescribed with monotherapy of Amlodipine and combination therapy of Telma H were reviewed and evaluated using descriptive statistics and ‘p’ value and assessed for their financial burden based on the cost per tablet for an year, Cost effectiveness ratio for the patients with controlled Blood Pressure by using a particular drug. Results: Out of 120 patients females were more (59.2%) and most of them were under the age group of 41-60 years. Majority of the patients had a known history of hypertension (65%). Most of the patients were prescribed with Telma H 40/12.5mg (40%) followed with Amlong 5mg (35.8%). There was a significant positive correlation (i.e., p value= 0.05) between persistence patterns of patients with drug therapy. Probability of patients with controlled Blood Pressure taking Amlong 10 mg were 82.35% followed by patients with Amlong 5mg. Conclusion: This study clearly showed that monotherapy of amlodipine is the most cost effective therapy than combination therapy of Telma H in order to reduce financial burden to the patient as well as to decrease the complications. The present study was to assess the effect of coal mining on health of workers and surrounding peoples in northern coalfields limited Singrauli, India. For data collection, pre-structured questionnaire was specially prepared and survey was also done in two year (2016-2017 and 2017-2018). Sample was selected randomly from local people of selected areas. Sample size was 150 respondents from each year in which 30 each from Amlori, Nigahi, Jayant, Dudhichua and Khadi a project. Thus the result revealed that local people were suffering from many health related problems in which most of the people were found respiratory and lung diseases. It is suggested that local people should wear mask, drink purified water and should plant more and more tress surrounding areas.

Abstract AML is a heterogeneous disease and several prognostic factors are well established in younger patients. Less work has been done on parameters affecting outcome in older patients, aged 60 years or over, especially in the majority who receive non-intensive therapy. We used data from the Medical Research Council AML11 trial (n=842) to develop a prognostic index and validated this using the subsequent Leukaemia Research Fund AML14 trial (n=943). Patients in AML11 received intensive standard induction chemotherapy (daunorubicin/Ara-C based); those in AML14 received either similar intensive therapy (AML14I, n=790) or non-intensive therapy with either hydroxyurea or low-dose Ara-C (AML14NI, n=153). Patients with APL were excluded from AML11 analysis since they were not eligible for AML14. Overall survival (OS) was analysed using multivariate Cox regression modelling to identify factors independently related to outcome, and a prognostic index was created for OS using the regression coefficients. From this patients were assigned to 3 equal sized risk groups (good, standard, poor). In AML11, adverse cytogenetics (−5, del(5q), −7, abn(3q), complex), increasing white blood count (WBC), secondary AML, poor performance status (PS) and increasing age were related to poor OS (all p&lt;0.0001). Validation using AML14I confirmed these findings for cytogenetics, WBC and secondary AML, but not for age or PS. In AML14NI, poor OS was predicted by adverse cytogenetics, poor PS and high WBC, but not by age or type of AML (de novo/secondary). OS by risk group for each trial is shown in the table. In all three trials, the differences between risk groups were highly significant (all p&lt;0.0001), though by three years all patients in AML14NI had died apart from one good risk patient. A greater proportion of AML14I patients fell into the good risk group (68%) than in AML11, suggesting that, when a non-intensive option is available, clinicians tend to opt for this for older and/or less fit patients, perhaps explaining the lack of prognostic significance of age and PS in AML14I. Although the outcome of older AML patients is worse than younger patients, this study shows that it is still possible to identify risk groups, using similar parameters to those found in younger patients. These can be used to determine, through prospective stratification of randomised trials by risk group, which patients should be offered intensive therapy with curative intent with the prospect of reasonable survival, and which should not. Similarly, with non-intensive therapy, patients vary considerably in their outcome, though none have good survival, and some patients should probably be offered palliative care only. When using machine learning methods to make predictions, the problem of small sample sizes or highly noisy observation samples is common. Current mainstream sample expansion methods cannot handle the data noise problem well. We propose a multipath sample expansion method (AMLI) based on the idea of linear interpolation, which mainly solves the problem of insufficient prediction sample size or large error between the observed sample and the actual distribution. The rationale of the AMLI method is to divide the original feature space into several subspaces with equal samples, randomly extract a sample from each subspace as a class, and then perform linear interpolation on the samples in the same class (i.e., K-path linear interpolation). After the AMLI processing, valid samples are greatly expanded, the sample structure is adjusted, and the average noise of the samples is reduced so that the prediction effect of the machine learning model is improved. The hyperparameters of this method have an intuitive explanation and usually require little calibration. We compared the proposed method with a variety of machine learning prediction methods and demonstrated that the AMLI method can significantly improve the prediction result. We also propose an AMLI plus method based on the linear interpolation between classes by combining the idea of AMLI with the clustering method and present theoretical proofs of the effectiveness of the AMLI and AMLI plus methods.

Background: This study was a multicenter, randomized, double-blinded, placebo-controlled phase III clinical trial to investigate the efficacy and safety of an olmesartan/amlodipine single pill plus rosuvastatin combination treatment for patients with concomitant hypertension and dyslipidemia. Methods: Patients with both hypertension and dyslipidemia aged 20–80 were enrolled from 36 tertiary hospitals in Korea from January 2017 to April 2018. Patients were randomized to three groups in a 1:1:0.5 ratio, olmesartan/amlodipine single pill plus rosuvastatin (olme/amlo/rosu) or olmesartan plus rosuvastatin (olme/rosu) or olmesartan/amlodipine single pill (olme/amlo) combination. The primary endpoints were change of sitting systolic blood pressure (sitSBP) from baseline in the olme/amlo/rosu vs. olme/rosu groups and the percentage change of low-density lipoprotein cholesterol (LDL-C) from baseline in the olme/amlo/rosu vs. olme/amlo groups after 8 weeks of treatment. Results: A total of 265 patients were randomized, 106 to olme/amlo/rosu, 106 to olme/rosu and 53 to olme/amlo groups. Baseline characteristics among the three groups did not differ. The mean sitSBP change was significantly larger in the olme/amlo/rosu group with −24.30 ± 12.62 mmHg (from 153.58 ± 10.90 to 129.28 ± 13.58) as compared to the olme/rosu group, −9.72 ± 16.27 mmHg (from 153.71 ± 11.10 to 144.00 ± 18.44 mmHg). The difference in change of sitSBP between the two groups was −14.62± 1.98 mmHg with significance (95% CI −18.51 to −10.73, p < 0.0001). The mean LDL-C reduced significantly in the olme/amlo/rosu group, −52.31 ± 16.63% (from 154.52 ± 30.84 to 72.72 ± 26.08 mg/dL) as compared to the olme/amlo group with no change, −2.98 ± 16.16% (from 160.42 ± 32.05 to 153.81 ± 31.57 mg/dL). Significant difference in change was found in LDL-C between the two groups with −50.10 ± 2.73% (95% CI −55.49 to −44.71, p < 0.0001). Total adverse drug reaction rates were 10.48%, 5.66% and 3.7% in the olme/amlo/rosu, olme/rosu and olme/amlo groups, respectively with no statistical significance among the three groups. Serious adverse drug reactions did not occur. Conclusions: Olmesartan/amlodipine single pill plus rosuvastatin combination treatment for patients with both hypertension and dyslipidemia is effective and safe as compared to either olmesartan plus rosuvastatin or olmesartan plus amlodipine treatment.

The paper presents distributed generation (DG) system in grid connected mode of operation with asymmetric multi-level inverter (AMLI) topology. Cascaded type DC-DC converter is employed to feed proposed AMLI topology. The DG output voltage (generally low voltage) is stepped up to the required level of voltage using high-gain DC-DC converter. Proposed AMLI topology consists of capacitors at the primary side. The output of high-gain DC-DC converter is fed to split voltage balance single-input multi-output (SIMO) circuit to maintain voltage balance across capacitors of AMLI topology. Cascaded DC-DC converters (both high-gain converter and SIMO circuit) are operated in closed-loop mode. The proposed AMLI feeds active power to grid converting DC type of power generated from DG to AC type to feed the grid. PWM pattern to trigger power switches of AMLI is also presented. The inverting circuit of MLI topology is controlled using simplified Id-Iq control strategy in this paper. With the proposed control theory, the active power fed to grid from DG is controlled and power factor is maintained at unity. The proposed system of DG integration to grid through cascaded DC-DC converters and AMLI structure is validated from fixed active power to grid from DG condition. The proposed system is developed and results are obtained using MATLAB/SIMULINK software.

This article analyzes the official Mexican discourse on migration at the morning press conferences called by the president from Monday to Friday. To that end, a Discourse Analysis was combined to identify representation ideologies with a Content Analysis to locate related themes and key actors. Among the results, it stands out that the official discourse presents Mexico as an effective mediator; while an emotional discourse appeals to non-discrimination and encourages empathy towards migrants, the government measures confirm the hardening of the country's migration policy.

Backround: The effect of antihypertensive drugs on glucose homeostasis and insulin resistance remains an issue under investigation. There is evidence that renin-angiotensin system (RAS) blockers may favorably affect glucose metabolism, while treatment with calcium channel blockers (CCBs) is considered to have an overall neutral metabolic effect. However, the effects on glycemic indices may differ among agents within the same class of antihypertensive drugs. Objective: To evaluate the effects of different fixed-dose single pill combinations of RAS blockers with CCBs on homeostatic model assessment for insulin resistance (HOMA-IR). Methods: Drug-naive patients with arterial hypertension (AH) and impaired fasting glucose (IFG) were randomly allocated to open-label fixed, single pill combinations of valsartan 160 mg/day plus amlodipine 5 mg/day (VAL/AMLO group, n = 54), delapril 30 mg/day and manidipine 10 mg/day (DEL/MANI group, n = 53) or telmisartan 80 mg/day and amlodipine 5 mg/day (TEL/AMLO group, n = 51) for 12 weeks. Glycemic indices and HOMA-IR were determined at baseline and post-treatment. Results: A total of 158 patients were included. All treatment combinations effectively reduced blood pressure (systolic and diastolic) to similar levels (all p < 0.001). A decrease in the HOMA-IR index by 22.55% (p < 0.01) was noted following treatment with TEL/AMLO, while an increase by 1.4% (p = 0.57) and 12.65% (p = 0.072) was observed in the VAL/AMLO group and the DEL/MANI group, respectively. These changes were significantly different between TEL/AMLO and DEL/MANI (p < 0.05) as well as between TEL/AMLO and VAL/AMLO (p < 0.001). Conclusions: Despite similar antihypertensive action, the effect of fixed, single pill combinations with TEL/AMLO, VAL/AMLO and DEL/MANI on insulin resistance is in favor of TEL/AMLO. Pre-modern Central Asia was comprised of three principalities: 1. the Emirate of Bukhara, and Khanates of 2. Khiva and 3. Khokand. These principalities were caught in the web of socio-economic and political crises. To cope with these crises and to solve the problem of the expenditure of huge administrative costs, the Khans and Amirs distributed some portions of land from the state lands (amlok) as grants among their heirs, both civil and military officials. This paper concludes that instead of solving the problems, the distribution of land grant adversely affected the normative political order. The grantees, as landlords, began exercising enormous political and economic powers. These actions created immense exploitation of the subaltern tenants. The landlords siphoned off the tenants’ surplus and consequently subjected them to the most appalling conditions with their housing, food, and clothing.

ABSTRACT Upon the election of Andres Manuel Lopez Obrador (AMLO) on 1 July 2018, the vitality of Mexico’s recent Energy Reform came under fire from investors, policy makers and industry professionals. This paper evaluates the risks posed to investors in cross-border pipeline projects as a result of this regime shift. Risk is evaluated by the present factors that affect investment conditions as well as projections into the future state of the Mexican Energy Reform. An evaluation of the current Mexican natural gas policy and Mexico’s natural gas relationship with the United States demonstrate a high demand for cheap natural gas flowing across the U.S.-Mexico border. While factors such as social insecurity, theft, environmental concerns, and political volatility slowed the development of several cross-border pipeline projects, the presence of newly minted projects on the horizon are a sign that the expansion of Mexico’s pipeline infrastructure is likely to endure.

Mitochondrial dysfunction is associated with cardiovascular damage; however, data on a possible association with kidney damage are scarce. Here, we aimed at investigating whether 1) kidney impairment is related to mitochondrial dysfunction; and 2) ANG II blockade, compared with Ca2+ channel blockade, can reverse potential mitochondrial changes in hypertension. Eight-week-old male spontaneously hypertensive rats (SHR) received water containing losartan (40 mg·kg−1·day−1, SHR+Los), amlodipine (3 mg·kg−1·day−1, SHR+Amlo), or no additions (SHR) for 6 mo. Wistar-Kyoto rats (WKY) were normotensive controls. Glomerular and tubulointerstitial damage, systolic blood pressure, and proteinuria were higher, and creatinine clearance was lower in SHR vs. SHR+Los and WKY. In SHR+Amlo, blood pressure was similar to WKY, kidney function was similar to SHR, and renal lesions were lower than in SHR, but higher than in SHR+Los. In kidney mitochondria from SHR and SHR+Amlo, membrane potential, nitric oxide synthase, manganese-superoxide dismutase and cytochrome oxidase activities, and uncoupling protein-2 content were lower than in SHR+Los and WKY. In SHR and SHR+Amlo, mitochondrial H2O2 production was higher than in SHR+Los and WKY. Renal glutathione content was lower in SHR+Amlo relative to SHR, SHR+Los, and WKY. In SHR and SHR+Amlo, glutathione was relatively more oxidized than in SHR+Los and WKY. Tubulointerstitial α-smooth muscle actin labeling was inversely related to manganese-superoxide dismutase activity and uncoupling protein-2 content. These findings suggest that oxidant stress is associated with renal mitochondrial dysfunction in SHR. The mitochondrial-antioxidant actions of losartan may be an additional or alternative way to explain some of the beneficial effects of AT1-receptor antagonists.

Objectives: To assess the blood pressure-lowering efficacy and tolerability of perindopril/amlodipine fixed-dose combinations in Chinese patients with mild-to-moderate essential hypertension not adequately controlled with monotherapy alone. Methods: In 2 separate double-blind studies, patients received a 4-week run-in monotherapy of amlodipine 5 mg or perindopril 4 mg, respectively. Those whose blood pressure was uncontrolled were then randomized to receive the fixed-dose combination of perindopril 5 mg/amlodipine 5 mg (Per/Amlo group) or remain on the monotherapy for 8 weeks. Patients who were uncontrolled at the week 8 (W8) visit were up-titrated for the Per/Amlo combination, or received additional treatment if on monotherapy, for a further 4 weeks. The main efficacy assessment was at 8 weeks. Results: After 8 weeks, systolic blood pressure (SBP; primary criterion) was statistically significantly lower in the Per/Amlo group (vs. Amlo 5 mg, p = 0.0095; vs. Per 4 mg, p < 0.0001). Uncontrolled patients at W8 who received an up-titration of the Per/Amlo combination showed a further SBP reduction. These changes were mirrored by reassuring reductions in diastolic blood pressure. The fixed-dose combinations were well tolerated. Conclusions: Single-pill combinations of perindopril and amlodipine provide hypertensive patients with a convenient and effective method of reducing blood pressure.

Abstract New, sensitive, rapid, cost-effective, and validated stability-indicating thin layer chromatographic (TLC) method coupled with fluorescence (FL) detection was developed for the quantitative analysis of celecoxib (CEL) and amlodipine besylate (AMLO) in their laboratory prepared binary mixture using the non-fluorescent TLC silica gel 60 plates. Ethyl acetate: diethylamine: 1-propanol (9:1:0.2, V/V) was used as a developing system. The retention factor (Rf) for each drug was 0.80 ± 0.03 and 0.44 ± 0.01 for CEL and AMLO, respectively. The plates were excited at 264 nm for the simultaneous FL measurement of CEL and AMLO, the calibration curves were linear over a concentration ranges of 30.0–300.0 ng/band and 15.0–150.0 ng/band with mean percentage recoveries of 99.80 ± 0.85 and 99.80 ± 0.77 For CEL and AMLO, respectively. The developed method was applied for the stability studies of the cited drugs in their laboratory prepared binary mixture and the forced degradation products were determined when present in presence of the pure drugs so the method can be considered as a stability-indicating one and it was validated as per ICH guidelines and proved to be accurate and precise.

A novel, simple, rapid, and highly selective stability indicating RP-HPLC method was developed and validated for simultaneous estimation of azilsartan medoxomil (AZL) and amlodipine besylate HCl (AMLO) in tablet dosage form having strength of 20 mg and 2.5 mg, respectively. The effective chromatographic separation was achieved on a Phenomenex luna ODS C18 (15 cm X 4.6 mm internal diameter, 3.5 μm Particle size) with a mobile phase composed of phosphate buffer (pH-2.5) adjusted with ortho phosphoric acid : acetonitrile in the ratio of 60:40 v/v. The mobile phase was pumped using an isocratic HPLC system at a flow rate of 0.7 mL/min with injection volume 20μl and quantification of the analytes was done at detection wavelength 254 nm. The retention times were found to be 5.918 min and 14.901 min for AMLO and AZL, respectively. The proposed HPLC method was validated with respect to linearity, ranges, precision, accuracy, specificity, robustness, LOD, and LOQ as per ICH Q2 (R1) guideline. Calibration plots were linear over the concentration range of 75-125 µg/mL and 600-1000 µg/mL with correlation coefficients 0.9966 and 0.9948 for AMLO and AZL, respectively. Forced degradation studies were performed using hydrolysis, oxidation, photolytic, and thermal degradation conditions with good resolution between the degradants and analytes. Degradation products resulting from the stress studies did not interfere with the detection of AMLO and AZL, thus the proposed method is sensitive and stability-indicating. The validated HPLC method was successfully applied to the analysis of AMLO and AZL in tablet dosage form.

Resumen
In standardization process, quality and purity of crude drugs are evaluated by various parameters. For herbal formulations, along with the chemical, physical, phytochemical, in- vivo and in- vitro parameters, it is significant to determine its quality standards. In traditional system of medicine, the developments of analytical parameters are an important step in establishing quality of products, presence of phytoconstituents and its therapeutic efficacy. In this review, Avipathy and Avipattikar churna, both are polyherbal formulations having nine similar herbal ingredients (Sunthi, Marica, Pippali, Amlaki, Musta, Vidanga, Elaichi, Patra, Trivit) but they are different in terms of indication and method of administration. Avipathy churna is targeted mainly to achieve pitta balance whereas Avipattikar churna for all the three dosha specified in Ayurvedic system of medicine. This review provides data on quality control and analytical studies such as Gas Chromatography coupled with Mass Spectrometry (GC-MS), High Performance Thin Layer Chromatography (HPTLC), Atomic Absorption Spectroscopy (AAS), Ultraviolet spectroscopy (UV) and pharmacological evaluation of Avipathy and Avipattikar churna. The analytical study shows the presence of various marker compounds. This helps to draw new conclusions from existing data to identify research areas. They assure the quality, purity and safety of herbal formulations.
