Texto completoBackground: Fetal Alcohol Spectrum Disorder (FASD) is the leading preventable cause of non-genetic mental retardation. General practitioners (GPs) are at the forefront of prevention and detection of FASD. While the prevalence of FASD in the region is said to be the highest in France, the regional health agency has pointed to shortcomings in the detection of alcohol consumption, particularly among pregnant women, and low participation of general practitioners in training courses on FASD.Objective: The aim was to understand the levers and barriers GPs face in preventing and identifying FASD, and to take action to strengthen these practices.Method: An action research project was carried out. For its 1st stage, a narrative review of the literature was carried out, supplemented by two qualitative studies adopting a grounded theorizing approach with GPs. The modeling resulting from these studies enabled us to move on to the 2nd stage, the development of actions using a Delphi round consensus method. The actions are currently being implemented. Results: The review identified 23 scientifically evaluated actions in the fields of prevention, detection, screening and diagnosis of FASD. GPs' discomfort in dealing with alcohol, particularly in women, was an obstacle to preventing and identifying FASD. They felt able to identify neurodevelopmental disorders. These factors led to the development of prevention and detection actions by consensus on 24 actions, resulting in 11 consensual actions. Four actions have been implemented. Feedback to the participants, the final stage of the action research, began with a communication to the experts on the Delphi round panel. Discussion : This action research project serves as a model for field-based research aimed at responding to a priority local public health issue. This method is transferable to the identification and prevention of other public health issues to general practice. Similar action research should be carried out in other local public health issues in primary care. They would be complementary to a cluster randomized trial focusing on training GPs in prevention and detection

Texto completoIn Reunion Island, cervical cancer is the third leading cause of cancer in women. Primary prevention is based on vaccination against papillomavirus (HPV). A study on cervical smears and biopsies in Reunion showed that the most frequent HPV on the island were HPV 16, 31, 51, 52, mostly contained in the nonavalent HPV vaccine (Gardasil 9). Thus 96.8% of lesions could have been prevented by the vaccine. However, in Reunion Island, the coverage rate is low (12.2%). The main obstacles found among mothers of adolescents and general practitioners were lack of information and vaccine mistrust. The objective of the study was to evaluate the impact of a health promotion program on HPV vaccination coverage. In this prospective, controlled superiority interventional study, a promotion program took place in a middle school, which was compared to a control school where no specific intervention was planned, during 2020-2021 school year, in the south of Reunion Island. The program combined: information for students (lectures, personalized information in the school yard, public health campaign created by students), parents, general practitioners, free vaccination at school (in a "health bus" parked in the school yard) with the nonavalent HPV vaccine. At the end of school year, vaccination coverage was significantly increased in the school where the intervention took place. The most effective communication method to raise acceptability of vaccination among students was the participatory method, where students were actors in the creation of a public health campaign. However, the low participation rate in vaccination campaigns could be explained by the fear of serious adverse effects due to lack of information and knowledge, mistrust, and the taboo related to sexuality and pregnancy among adolescents, depriving children of a dialogue with their parents on the subject. Implementation of a health promotion program and free vaccination in schools increased vaccination coverage. These results are promising and are a stepping stone towards the extension of this program to the entire island of Reunion; and hopefully someday the burden of cervical cancer may be reduced

Texto completoThe main purpose of this study was to put to the test a possible correlation between the level of phonological conscience evaluated in great section of nursery school and the performance in a transcription task one year later, at the end of the preparatory course. In this aim, we observed over three years a group of more than sixty pupils of a private school of the city of Saint-André (Réunion island) by proceeding particularly with an evaluation of the level of phonological conscience in great section of nursery school in november 2001 and february 2002, followed by a test evaluating the level of transcription involving the same population at the end of the preparatory course, in june 2003. Among other results, it appeared that the correlation between the two studied variables was statistically significant. Therefore, the score obtained at the test of total phonological conscience in the great section of nursery school appears to be a good predictor of the performance in transcription as measured with a test in preparatory course. The acceptability of the different tasks involved in this study was very good and then, the selected tests could be useful in the early detection and prevention of possible troubles of reading and spelling

Texto completoThis PhD thesis aims at understanding the emergence, the stakes and the acceptability of gerontechnologies in Reunion Island. Since the early 2000s, new technologies in health and autonomy became increasingly important in the public debate on aging. Since the 1960s, public policies of old age are built on a paradigm shift of aging. This paradigm has switched from a conception based on the autonomy loss to a more global conception taking into account all individual situations and existence fields impacted by age. Moreover, they have been built leaving more and more latitudes to the territories for the aging 'management'. Reunion Island is experiencing a rapid aging of its population in a high poverty context. This context encourages the territory to innovate and find global solutions in the most anticipated way possible. The social and technological innovations studied in this thesis allow two analysis levels. Based on the autonomy loss and prevention management, the first level is related to the gerontological analysis and highlights the importance of institutional context as a framework for regulating the relationship between individuals and innovation. The second level is closer to the elderly concerned by the use of these health and autonomy technologies. The complexity of the acceptability process can be analyzed through the prism of social reconfigurations induced by aging

Texto completoOur thesis "Oral language, prereading, comprehension and prevention of illiteracy: a linguistic analysis of oral productions of pupils of preschool older section in Reunion Island" mobilizes the theoretical choices of descriptive linguistics in the light of a dynamic grasp of the language facts (Staudacher-Valliamée, 1999) in Reunion schools. Our doctoral research corresponds to the linguistic and logical axis of the device "Say, read, calculate in preschool older section". Its purpose is to study the observation of students in the acquisition of oral language in French as a second language (Véronique, 2009) when they start pre-reading comprehension. The linguistic survey conducted in 16 classes concerns 544 pupils, known as sensitive, in a potential school drop-out situation. The corpus of base of 2,500 statements, of a first exploratory diagnostic test (TD1) startingfrom illustrative images, expands to 5009 statements in the second experimental test (TD2), on images of an album of youth literature. Two new tools had to be developed, complemented by the AntConc software, for an analysis adapted to the linguisticdynamics of our corpus: an observation grid of the grammatical units (names, pronouns of people, determiners, verbs) and another one of 7 indicators of understanding. On the criterion of oral productions conforming or not conforming to lexical expectations and semantics, understanding or not, the results obtained differentiate 3 types of profiles: the confirmed preselector, the pre-selector in difficulty and the silent one. The spin-offs from our doctoral research have led to Action 3, in partnership with the Reunion Academy, within the framework of an action plan against illiteracy (2010-2015). Our results highlight in a new way the link between the construction of young students' oral language and their visual memory as they start writing. A contribution to the didactics of the oral (Dolz, Schneuwly,1998) advocates the consideration of the language of the pupils impregnated with the characteristics of Creole oral society, perceptible in the referencing and informational organization of discourse (Picoche,1993). The pedagogical applications reflect the contents of training, development of a tool of diagnosis of lexical skills and semantic knowledge of the students
