SummaryThe research on anxiety disorders of the department of psychiatry of the Vrije Universiteit is briefly reviewed. Studies have included comparisons between pharmacotherapy and cognitive behaviour therapy and combination treatments of panic disorder and of obsessive-compulsive disorder. Also, the hyperventilation model of panic disorder could be rejected as a result of an ambulatory monitoring study. Provocation studies with adrenalin in panic disorder have shown less support for a psychological model of panic than expected. Recommendations for future research include studies on anxiety in the very young and in the elderly, studies on comorbidity in anxiety, especially with alcoholism and depression and the development of treatment manuals for general practice. The issue is raised whether investing in neuroimaging studies will critically advance our knowledge in the near future.

Addictive behavioral disorders are multifactorial diseases with clinical, neurophysiological, and genetic heterogeneity, a high comorbidity with other disorders, and a low curability. The etiopathogenetic mechanisms of non-chemical forms of addictive behavior have not been sufficiently studied, which makes it difficult to search for effective therapeutic procedures.Objective: to study the psychological and genetic components of a non-chemical addictive disorder as the phenomenon of codependency.Patients and methods. The investigation enrolled 256 women who were divided into three comparison groups: 1) those with the phenomenon of codependency, 2) phenotypically healthy women; 3) a population sample. Psychometric testing was carried out using the «Hand Test» by E. Wagner (adapted by A.I. Gerasimov and S.N. Enikolopov) and the clinical and genealogical characteristics of women with the phenomenon of codependency were studied. Results and discussion. There was a statistically significant predominance of the level of aggressiveness as autoaggression in the structure of the personality profile of women with the phenomenon of codependency (t=2.924–3.015; p=0.004–0.005). The clinical and genealogical characteristics of persons with addictive behavioral disorder as the phenomenon of codependency suggest that there is a statistically significantly high frequency of secondary alcoholism among first-degree and second-degree relatives or both and first-degree male relatives (p<0.001).Conclusion. The phenomenon of codependency as a non-chemical addiction includes psychological and genetic components. Women with codependency had autoaggressive destructive behavior patterns and a family history of alcoholism. The identified psychogenic characteristics can be considered as a risk for an addictive disease and somatoform disorders.

There has always been an interest in social-pathological problems in the society. Documents confirming our wish to find out the causes of ‘other’ types of behavior as opposed to the ‘usual’/accepted types date back to the Ancient and Middle Ages. The social significance of the study of social-pathological phenomena is evident for they allow to reveal dysfunctionalities in certain structural elements or failed socialization, which are most probably determined by destabilization of the social system and value priorities. Alcoholism as a form of deviant behavior is usually considered the most common and influential factor of destroying the physical and psychological health and of negative changes in human behavior. The article describes the level of alcoholism and the ways of perceiving it in Bosnia and Herzegovina based on the empirical data collected within the broader scientific study of the social changes in one part of Bosnia and Herzegovina - Republic of Srpska. The survey was conducted on the representative sample of 220 respondents (reflecting the social-demographic and social-economic characteristics of the general population) in 7 municipalities of the Republic of Srpska. The questionnaire consisted of both closed and partly open questions to reveal the opinions of the respondents on specific deviant forms of behavior and to find out whether the respondents are engaged in some of these deviant forms depending on their attitudes and knowledge about them.

Objective: Recent Australian Government initiatives have emphasised problems with service provision to the ethnic mentally ill. This study aims to address the paucity of contemporary data describing the disposition of the ethnic mentally ill in hospital settings. Method: Patterns of admissions for psychiatric disorders to all hospitals in Perth, Western Australia, for the 3 years from 1990 to 1992, of migrants and the Australian born were compared using data from the Western Australian Mental Health Information System. Results: The overall rates for European migrants showed a ‘normalisation’ towards those of the Australian-born. There were high rates for the schizophrenic spectrum disorders in Polish and Yugoslavian (old terminology) migrants. There were low admission rates for South-East Asian migrants, predominantly those from Vietnam and Malaysia. Rates for alcoholism were low in Italian and all Asian migrants. There were high rates of organic psychosis, especially in those older than 75 years, among the Italian and Dutch migrants. The relative risk of a first admission in the 3 years being an involuntary admission to a mental hospital was almost twice that of the Australian-born for migrants from Poland, Yugoslavia, Malaysia and Vietnam. Conclusions: The results imply the possibility of significant untreated and/or undiagnosed psychiatric morbidity in the South-East Asian-born. They also indicate a need for further exploration of the unexpectedly high levels of psychiatric morbidity among some ethnic elderly groups, specifically the Dutch- and Italian-born. The findings demonstrate the persistence of high rates of presentation for psychotic disorders among Eastern European-born populations, many years post migration.

Child abuse constitutes a social problem whose dimensions we cannot yet quite grasp and whose causes are as multi-faceted as they are difficult to identify. Over the past two decades or so, ever since the re-awakening of interest in the topic, there has been an abundance of theorising and speculating about causes of child abuse. Theories which attempt to explain child abuse have ranged from the individual - psychological and specific deviant behaviour (like alcoholism and other types of addiction) - to socio-economic factors, faulty child-parent relationships, lack of family support resources and many more. Each of these theories, even if underdeveloped, has something to contribute to our understanding but none, by itself, has enabled us to say that if only we concentrate our resources on it we will be able to tackle child abuse at its very roots. What we have begun to learn is that we are dealing with a complex situation.

The present study examined protective factors associated with the wellbeing of 93 youth from a refugee background resettled in Brisbane, Australia. Wellbeing was defined as an absence of psychological distress and the presence of subjective wellbeing. Students at Milpera State High School, a special English language school, completed a battery of questionnaires. Hierarchical multiple regression analyses examined the relationship between protective factors and wellbeing, while controlling for sociodemographic characteristics. The results indicated that higher levels of school connectedness and acculturation were significantly associated with lower levels of psychological distress. Further, higher levels of school connectedness, acculturation and resilience, in addition to having a permanent visa, were significantly associated with higher levels of subjective wellbeing. Notably, 55% of the variance in subjective wellbeing was explained jointly by these factors. School connectedness, acculturation, resilience, and visa certainty were instrumental in enhancing aspects of wellbeing in the present sample of students from a refugee background. Implications for refugee-related policy and strategies in schools, mental health services, and at broader governmental levels are discussed.

Three groups of neuropsychiatric out-patients, homogeneous according to parameters of violent behaviour, were identified in a previous study using a cluster analysis procedure and compared in this study on psychometric variables. These groups were (a) a group manifesting frequent, impulsive violence (n= 35), (b) a non-violent group (n= 57), and (c) a group whose violent behaviour was much less frequent and severe than in the first group, and more provoked (n= 31). Impulsively violent patients showed language and visual-perceptual deficits, but no other neuropsychological or intellectual deficits. The MMPI F, K and Ma and MacAndrews Alcoholism scales and a perceptual organisation factor derived from the Holtzman Inkblot Test significantly discriminated groups. Results of this and the initial study have implications regarding psychological aspects of aggression and underlying biological mechanisms.

Alcohol use disorders are common and make a significant contribution to the burden of disease throughout the world. This is especially true among the younger age groups. Although these disorders are common, evidence suggests that those affected do not seek help for their disorders. In order to understand this, reviews of the treatment literature and the epidemiological data on prevalence and correlates of alcohol use disorders and treatment seeking are presented. These reviews confirm that effective treatments exist and that screening in primary care can be efficacious. The reviews also highlight deficits such as the need for more epidemiological evidence on the validity of DSM definitions of alcohol use disorders and for more Australian data on the prevalence and correlates of the disorders and related treatment seeking. This thesis sets out to address these deficits applying sophisticated statistical techniques to data from a large nationally representative Australian sample. A confirmatory factor analysis of the eleven criteria that specify alcohol dependence and abuse examined the validity of DSM-IV definitions of alcohol use disorders and the best solution was found to be a single factor, not two as currently defined. These findings question the bi-axial nature of alcohol use disorders that has underpinned their definition since the publication of DSM-III-R in 1987. Data from this national sample also confirm that, in line with research from other western countries, Australians have high levels of alcohol use disorders, especially amongst males and younger people. Also no association was found between alcohol dependence and treatment seeking, and young people were least likely to seek treatment. However, a relatively large proportion of young people who drink had been in contact with their GPs in the past year; demonstrating that there is ample opportunity for screening and referral for treatment for alcohol use disorders in this vulnerable group. This research has found that although alcohol disorders are not necessarily associated with disability, there are those who can benefit from treatment. It suggests that outcomes for such individuals may be improved by better specification of disorders as well as improved access to best treatments.

Sons of male alcoholics (SOMAs) are at increased risk for the development of alcoholism, and are apparently characterized by other abnormalities. It is possible that one or more of these abnormalities might serve as a marker for the alcoholic predisposition. Research described in this thesis, conducted in the hopes of identifying such a marker, was designed (1) to separate the relative pharmacological and psychological effects of acute alcohol intoxication upon neuropsychological functioning; (2) to investigate the neuropsychological function of SOMAs with a multigenerational family history of male alcoholism; (3) to examine the relationship between SOMAs' neuropsychological function and their cardiovascular hyper-reactivity to threat of and aversive stimuli; and (4) to investigate the relationship between a number of cardiovascular response patterns and voluntary weekly alcohol consumption. These studies are linked conceptually, within the context of a general theory of information-processing and action.

This study investigated demographic and personality variables, that counselors used in the selection of alcoholics for long term treatment and compared the completion rate for this group against completion rates reported in past studies, in order to determine if certain personality variables are associated with completion. The MMPI, Survey of Drinking Patterns and Effects, and a demographic questionnaire was administered to 355 lower class and "skid row" inpatient alcoholics from a North Tucson alcoholism treatment center. Results showed that counselors selection judgements approximated the characteristics of alcoholics who had completed long term treatment programs in past studies. Completion rates for this group studied were found to be moderately high compared to past studies. It was concluded that certain biographical and psychological variables can be used as selection criteria for determining alcoholics long term treatment completion. Implications for these findings are discussed.

A wealth of data supports the hypothesis that the neurotransmitter serotonin regulates the intake of ethanol, and is involved in the development of alcoholism in humans. Reduced functioning of the serotonergic system hypothetically increases alcohol intake in both animals and humans. In this thesis, it was proposed that the effect of lowered serotonergic function on alcohol intake is mediated by an increase in disinhibition. The hypothesis that lowered serotonin increases disinhibition was tested in separate groups of individuals at high risk for the development of psychopathology: nonalcoholic young men with a strong family history of paternal alcoholism, and adolescent men with previous histories of physically aggressive behavior. Lowered serotonergic synthesis (and thus presumably function) was experimentally induced through a transient dietary reduction in the availability of the amino add precursor of serotonin, tryptophan. Disinhibition was quantified using a go/no-go task previously shown to characterize psychopaths and children with attention deficit hyperactivity disorder as disinhibited. In the first study, acute tryptophan depletion had no effect on aggressive responding on a modified competitive reaction time aggression task, but increased disinhibition in young men at risk for alcoholism. This effect was independent of the tryptophan depletion-induced mood alterations. The effect tryptophan depletion on disinhibition was not replicated in the second study with previously aggressive adolescent men. A number of explanations for this were posited, including the presence of a ceiling effect. An association between disinhibition and executive functioning (cognitive abilities associated with proper functioning of the prefrontal cortex, such as working memory, planning abilities) was demonstrated in the second study. Whereas many studies of adult children of alcoholics (COAs) have utilized clinical subjects, subjects in this study (n = 229) were relatively well functioning college students. The Children ofAlcoholics Screening Test (CAST; Jones, 1983) was utilized to define parental alcoholism.Results of a multivariate analysis of variance indicated that COAs were more likely to exhibit symptoms related to poor psychological adjustment than were their non-COA peers. No significant psychological differences in male and female COAs were found. COAs scored significantly higher than non-COAs (p.< .01) on the Alienation scale of the Psychological Screening Inventory (PSI; Lanyon, 1978), indicating the greater similarity of COAs to those with psychiatric problems. Social Nonconformity, as a measure of psychopathic behavior, was higher for COAs than for non-COAs (p. < .001). COAs from lower income families demonstrated higher scores on the Discomfort scale (p < .01), which measures symptoms. Those subjects who reported parental greater symptomatology and anxiety than those who reported no parental depression (p. ( .001). Although parental divorce rates were higher for COAs (32.4%) as compared to non-COAs (10.9%), no significant association with any of the psychological variables was found for parental divorce.In comparison to non-COAs, both male and female COAs evidenced greater problem alcohol use (p < .001) measured by the Michigan Alcoholism Screening Test (MAST; Selzer, 1971). As demonstrated by a MAST score of 7 or more, 88.8% of male COAs and 30.5% of female COAs indicated serious alcohol-related problems. Parental depression was also significantly related to problem alcohol use in subjects. It was found that the MacAndrew scale (MAC; MacAndrew, 1965) was of little utility for identifying subjects with problem alcohol use.

In this study, we address two research questions: 1) Can we identify bear market episodes in Australia in the past 20 years? 2) How do investors’ moods change as stock market conditions enter into a bear phase. To address the first question, we use a pattern recognition algorithm, called the penalised LSE approach. By defining bear markets as those stock market regimes where the average returns are statistically significantly negative or below the risk free rate, we are able to detect two bear market periods in Australia in the past 20 years. These are the November 1987 to February 1988 and the April 2000 to May 2000 periods. To address the second question, we study the change in investors’ attitudes to varieties of systematic risk and the aggregate number and dollar value of shares traded in portfolios as a result of the regime switch from pre-bear to bear period. Out of the 7 categories of risk considered in this study, the transition from pre-bear to bear regime in both sample periods had a significant impact mainly on investors’ attitude toward the size risk factor. Investors systematically became more sensitive to firm size as stock market conditions entered into the 1987⁄1988 bear market. In the later sample period, investors’ reaction to firm size was more selective as it depended on the characteristics of the stocks that made up their portfolios. We also find that the regime switches resulted in lower portfolio trading volumes. Based on these results we infer that the November 1987-February 1988 bear market evoked a general sad mood, while the April 2000-May 2000 bear market stirred up both angry and sad feelings in market participants depending on the composition of stocks in their portfolios.

Much empirical evidence attests to a strong relationship between the panic-related disorders and alcoholism. Recent data suggest that anxiety sensitivity (fear of anxiety) may be one common underlying vailable contributing to the large degree of overlap between the panic-related disorders and alcoholism. In fact, some data indicate that the relationship between anxiety sensitivity and alcohol misuse may be particularly strong in women, a group which is generally underrepresented in the alcoholism etiology literature. Research described in this thesis was conducted with the aim of further elucidating the nature of the relationship between anxiety sensitivity and alcohol use/abuse in young adult women. The series of seven experiments included in this thesis demonstrated that: (1) high levels of anxiety sensitivity are characteristic of subjects meeting diagnostic criteria for panic disorder, an anxiety disorder frequently associated with alcohol abuse; (2) female university students demonstrate significantly higher average levels of anxiety sensitivity than male university students; (3) anxiety sensitivity is an important predictor of self-reported rates of alcohol consumption in university women; (4) high anxiety sensitive university students are both more likely to report drinking alcohol primarily to "cope" with negative emotional states, and less likely to report drinking alcohol primarily for social-affiliative motives, than are low anxiety sensitive university students; (5) high anxiety sensitive women display greater degrees of sober subjective-emotional arousal when anticipating aversive stimulation, greater degrees of sober electrodermal reactivity to the aversive stimulation, and greater sensitivity to the dampening effects of alcohol on these measures of reactivity, than low anxiety sensitive controls; (6) high anxiety sensitive women show a sober attentional bias favoring the processing of physically threatening information, which is dampened through th

Resumen
ABSTRACT Background: Mothers experienced enormous physical and emotional changes, especially during childbirth. Birth support role of husbands reassured both husband and wife about labor and birth. This study aimed to review the first-time advisory experience of husbands during labor time of primigravida wife. Subjects and Method: A scoping review method was conducted in eight stages including (1) Identification of study problems; (2) Determining priority problem and study question; (3) Determining framework; (4) Literature searching; (5) Article selection; (6) Critical appraisal; (7) Data extraction; and (8) Mapping. The search included ScienceDirect, Wiley Online Library, ProQuest, and grey literature through Google Scholar search engine databases. The inclusion criteria were English/ Indonesian-language and full-text articles in peer-reviewed journals published between 2009 and 2019. A total of 543,111 articles were obtained by the searched database. After the review process, six articles were eligible for this review. The data were reported by the PRISMA flow chart. Results: Six articles from developed countries (Australia, Sweden, Singapore, Israel, and England) met the inclusion criteria with qualitative and quantitative (cross-sectional) studies. Three main aspects discussed were support, challenges, and psychological conditions of experience of husbands during labor time of pregnancy. Support of husbands was identified as physical and moral. Challenges faced by husbands included lack of preparedness, knowledge, and encouragement from health care professionals. Psychological conditions of satisfaction and concern were found in husbands’ transition to fatherhood. Conclusion: Responsibility, emotion, experience, and barrier of husbands are related to maternal health problems. Better involvement of fathers will be able to enhance better quality of relationships and family health through understanding, experience, and assistance, especially in the childbirth process.
